با وجود تلاش های گسترده جهت کاهش خطای انسانی در حوزه های متفاوت شغلی، همچنان اعمال و رفتارهای ناایمن علت اصلی بسیاری از حوادث می باشند. رخداد خطاهای پزشکی در سیستم های مراقبت های بهداشتی و درمانی به عنوان یکی از مهم ترین عوامل کاهش کیفیت خدمات در این حوزه در سال های اخیر مورد توجه بسیاری قرار گرفته است. ازاین رو این مطالعه با هدف شناسایی و ارزیابی انواع خطاهای انسانی در بین پرستاران بیمارستان مادر یزد صورت گرفت.
روش بررسیاین مطالعه به صورت مقطعی و توصیفی-تحلیلی در سال 1397 در بیمارستان مادر یزد صورت گرفت. تمامی وظایف پرستاری در بخش های زنان، نازایی، اورژانس، مراقبت های ویژه نوزادان و لقاح خارج از بدن بیمارستان، شناسایی و با استفاده از روش آنالیز سلسله مراتبی وظایف (HTA) به زیر وظایف تقسیم و سپس با استفاده از روش SHERPA مورد تجزیه و تحلیل قرارگرفته اند.
نتایجبیشترین تعداد خطاها به ترتیب در بخش زنان (83)، نازایی (75)، اورژانس (53)، مراقبت های ویژه نوزادان (32) و لقاح خارج از بدن (6) بوده است. بیشترین درصد خطا مربوطه به عملکردی در تمامی بخش ها می باشد. کمترین خطا نیز مربوط به خطای انتخاب بود.
نتیجه گیرینتایج این مطالعه نشان می دهند که خطاهای عملکردی سهم عمده ای از خطاها را به خود اختصاص داده اند که می بایست اولویت راهکارهای کنترلی در هر بخش برای این نوع خطاها در نظر گرفته تا از بروز این نوع خطاها جلوگیری شود. میزان خطاهای شناسایی شده بعد از ارایه راهکارهای کنترلی پیشنهادی در تمامی بخش ها نشان دهنده این حقیقت بود که می توان با به کارگیری این راهکارها به سطوح قابل قبولی از ریسک دست یافت.

آتش نشانان در معرض ریسک بالای ابتلا به اختلال استرس پس از سانحه (PTSD) قرار دارند و کیفیت زندگی (QoL) در افراد مبتلا به PTSD به طور جدی در خطر است. پژوهش حاضر به منظور بررسی رابطه اختلال استرس پس از سانحه و ابعاد گوناگون کیفیت زندگی در بین آتش نشانان شهر بیرجند انجام شد.
روش بررسیدر این پژوهش تحلیلی-مقطعی، کلیه کارکنان آتش نشانی شهر بیرجند در سال 1397 به روش سرشماری انتخاب شدند (96=N)؛ وضعیت PTSD و QoL به ترتیب به وسیله نسخه فارسی پرسشنامه های Mississippi (Eshel) و WHOQOL-BREF سنجش گردید. پایایی درونی گونه های فارسی این دو مقیاس با استفاده از آلفای کرونباخ به ترتیب 92/0 و بیش از 7/0 گزارش شده است. تجزیه و تحلیل اطلاعات با استفاده از نرم افزار SPSS ویرایش 21 و آزمون های مقایسه ایT-test مستقل، آزمون تحلیل واریانس یک طرفه، آزمون تعقیبی توکی و آزمون همبستگی پیرسون صورت گرفت.
نتایجاغلب آتش نشانان (5/70 درصد) از علایم اختلال استرس پس از سانحه متوسط (7/11±2/79) رنج می برند. بر اساس مقیاس خودگزارشی، 7/53 درصد آتش نشانان کیفیت زندگی خود را خوب ارزیابی می کنند و 2/43 درصد آتش نشانان از وضعیت سلامت خود راضی هستند. تحلیل های آماری نشان داد که بین نمره PTSD و حیطه های سلامت روانی (03/0=P)، روابط اجتماعی (002/0=P) و سلامت محیط (004/0=P) ارتباط معنادار آماری معکوس وجود دارد و بین نمره PTSD و حیطه سلامت فیزیکی هیچ رابطه معنادار آماری یافت نشد (08/0=P).
نتیجه گیرییافته ها حاکی از وجود نرخ بالای PTSD در بین آتش نشانان و رابطه منفی آن با 3 حیطه کیفیت زندگی آنان می باشد؛ با این وجود اغلب آتش نشان سلامت و کیفیت زندگی خود را خوب ارزیابی کردند. پیاده سازی استراتژی های مقابله با اختلالات روانی در بین آتش نشانان باعث کاهش علایم PTSD و افزایش هرچه بیشتر کیفیت زندگی آنان خواهد شد.

یکی از مهم ترین عوامل زیان آور در صنعت تولید آجر مواجهه با گرد و غباراست. در صورت مواجهه با گرد و غبار سیلیس کریستالی تعیین میزان مرگ ومیر ناشی از سیلیکوزیس و سرطان ریه از اهمیت بالایی برخوردار است. لذا این مطالعه باهدف ارزیابی ریسک کارگران کارخانه ماشینی و کارگاه های سنتی تولید آجر انجام گرفته است.
روش بررسیاین مطالعه به صورت مقطعی- تحلیلی در سال 1395 دریک کارخانه ماشینی و 5 کارگاه سنتی تولید آجر فعال در جنوب شرق استان تهران انجام گرفت. کلیه کارکنان کارخانه ماشینی آجرپزی (40 نفر) و 5 کارگاه سنتی آجرپزی (30 نفر) به صورت سرشماری انتخاب و مواجهه آن ها با گرد و غبار کلی قابل استنشاق و سیلیس کریستالی به روش NIOSH0600 و روش بهینه NIOSH7602 ارزیابی گردید. ارزیابی ریسک مرگ ومیر ناشی از سیلیکوزیس و سرطان ریه بر اساس مدل mannetej و Rice انجام گرفت. داده ها پس از ورود به نرم افزار spss 19 توسط آزمون تی تست تجزیه وتحلیل شد.
نتایجمیانگین غلظت سیلیس کریستالی تنفسی و گرد و غبار کلی تنفسی به ترتیب در کارگاه های سنتی 0/69± 0/651و 23/05± 28/27 و در کارخانه ماشینی 0/27± 0/297 و 17/6± 8/6 میلی گرم بر مترمکعب می باشد. آزمون تی تست نشان داد بین کارخانه آجرپزی سنتی و ماشینی در میزان مواجهه ی شغلی با گرد و غبار کلی تنفسی اختلاف معنی داری وجود دارد (0/001P=) ولیکن در میزان مواجهه با سیلیس کریستالی تنفسی اختلاف معنی داری مشاهده نشد (0/107P=). در هردو کارخانه ماشینی و سنتی ریسک مرگ ومیر ناشی از سیلیکوزیس و سرطان ریه در محدود غیر قابل قبول قرار دارد.
نتیجه گیرینتایج گویای این مطلب است که مکانیزه شدن صنعت منجر به کاهش مواجهه با گرد و غبار کلی قابل استنشاق و سیلیس کریستالی شده است و ریسک مرگ ومیر ناشی از سرطان ریه را کاهش داده است ولیکن هنوز ریسک مرگ و میر ناشی از سرطان ریه و بیماری سیلیکوزیس در هردو کارخانه سنتی و ماشینی بالا است.

صدا یکی از مهم ترین عوامل زیان آور در محیط کار محسوب می شود. آثار فیزیولوژیک صدا بر روی انسان به صورت تدریجی در دراز مدت بروز می کند. علی رغم پیشرفت های صنعتی مواجهه با صدای بیش ازحد مجاز کماکان یکی از معضلات بهداشتی کارگران در جهان می باشد. هدف از انجام این مطالعه بررسی تاثیر صدا بر افت شنوایی کارگران صنعت فولاد بود.
روش بررسیمطالعه حاضر از نوع توصیفی - مقطعی طراحی و اجرا شد. جامعه مورد بررسی 121 نفر از کارگران یک صنعت نورد فولاد بودند که ادیومتری 5 ساله آنها از سال 91 تا 95 مورد مطالعه قرار گرفت. آزمون ادیومتری این کارگران در 5 سال مورد مطالعه توسط یک دستگاه ادیومتر انجام شده بود. نتایج ادیومتری آن ها با رعایت اصول اخلاقی از پرونده پزشکی آن ها استخراج و مورد بررسی قرار گرفت. تجزیه و تحلیل داده ها با استفاده از نرم افزار SPSS ویرایش 21 صورت گرفت و جهت آنالیزهای آماری از آزمون های T-Test و Wilcoxon Signed Ranks Test استفاده شد.
نتایجمیانگین سنی شرکتکنندگان 92/4 ± 88/38 سال و میانگین سابقه کاری 57/3 ± 94/10 سال بود. در این مطالعه، افزایش تدریجی میانگین آستانه شنوایی در فرکانس های مختلف در هر دو گوش از سال 1391 تا 1395 دیده شد.
نتیجه گیریصدا به عنوان یک عامل زیان آور در صنعت فولاد باعث افزایش تدریجی آستانه شنوایی در کارگران در معرض مواجهه می شود.

اسپیرومتری به عنوان یکی از روش های غربالگری بیماریهای ریوی شغلی کاربرد فراوان دارد. کارکنان لابراتوارهای دندانی در معرض بخارات حلال ها ، گازهای سمی، غبارات و آلیاژهای فلزی و آکریل ها و همچنین گچ ها میباشند. این مطالعه با هدف بررسی اسپیرومتریک عملکرد ریه در کارکنان لابراتوار های دندانی شهر یزد انجام شده است.
روش بررسیمطالعه به صورت مقطعی روی 37 نفر از کارکنان لابراتوار های دندانی شهر یزد در سه ماه فصل بهار 1398 انجام شد. به منظور ارزیابی مشکلات تنفسی و بررسی عملکرد ریه کارکنان، به ترتیب از پرسشنامه استاندارد تنفسی و اسپیرومتری مطابق با دستورالعمل ATS/ESR استفاده شد. داده ها توسط نرم افزار SPSS نسخه 18 با استفاده از آزمونهای آماری Mann-Whitney U، Chi square، و Student’s T test مورد تجزیه و تحلیل قرار گرفت.
یافته هامیانگین سن شرکت کنندگان 13/1±59/35سال بود و بیشترین فراوانی سابقه فعالیت شغلی در بازه ی تجربه کاری بالای 15سال بود. 54% افراد شرکت کننده بدون علایم تنفسی بودند و بیشترین فراوانی شکایات تنفسی شامل سرفه خشک و خلط دار بود. در مجموع، شاخص های اسپیرومتری در محدوده الگو نرمال قرارداشت. زمینه کاری تنها عامل موثر بر روی FVC% بود . سن، سابقه کار و فعالیت ورزشی تاثیر معنی داری بر شاخص های اسپیرومتری نداشت .در مجموع شاخص های اسپیرومتری در محدوده الگوی نرمال قرار داشت و تنها 5% از افراد شرکت کننده الگوی انسدادی را نشان دادند.
نتیجه گیریبا توجه به نتایج بدست آمده، عملکرد اسپیرومتریک ریه در کارکنان لابراتوار های دندانی شهر یزد در بازه نرمال قرار داشت و احتمالا کار در لابراتوارهای دندانی تاثیر معناداری در عملکرد تنفسی کارکنان بر اساس شاخصهای اسپرومتری ندارد.

صفحات 53-63مقدمهبا توجه به اهمیت و ضرورت تحلیل حوادث، استفاده از تکنیک های مناسب، جهت آنالیز دقیق حوادث و ارایه اقدامات اصلاحی و پیشگیرانه ی مناسب، جهت جلوگیری از بروز مجدد حادثه، امری ضروری می باشد.روش بررسیدر این مقاله توصیفی- تحلیلی، مهم ترین معیارهای بررسی و انتخاب تکنیک های تحقیق و تحلیل حادثه و انتخاب روش برتر آنالیز حادثه، در حوادث بحرانی صنایع در فاز ساخت، شناسایی و بررسی شد. در این پژوهش با استفاده از تحقیقات پیشین و جمع آوری نظرات خبرگان مهم ترین معیارهای انتخاب یک روش تحلیل حادثه شناسایی گردید. پس از آن با استفاده از روش های تحلیل حادثه TRIPOD BETA و FTA، دو حادثه بحرانی نیروگاهی بررسی شد. سپس ماتریس مقایسات زوجی بر اساس نقاط قوت و ضعف مدل ها تشکیل گردید. در انتها اولویت بندی این دو روش با استفاده از روش تصمیم گیری تحلیل سلسله مراتبی انجام شد.نتایجدر این مقاله 7 عامل کلیدی واقع بینی مدل، توصیفی بودن مدل، سیستماتیک بودن مدل، زمان اجرا، دوره های آموزشی مورد نیاز، توانایی کمی سازی و قابل رویت بودن وقایع به عنوان مهم ترین معیارهای انتخاب یک روش تحلیل حادثه مشخص شد.

حوادث عروقی مغزی یک بیماری نسبتا شایع در جوامع در حال توسعه محسوب می گردد. این بیماری یکی از مهمترین دلایل ناتوانی ناگهانی غیر قلبی محسوب می گردد. از طرف دیگر طبعا این بیماری می تواند در سنین کاری و در رانندگان رخ دهد؛ بنابراین ارزیابی تناسب شغلی در یک راننده با سابقه حوادث عروقی مغزی می تواند یک چالش مهم برای پزشک سلامت شغلی و ایمنی راه باشد.
روش بررسیدر این مطالعه جهت شناخت شاخص های اصلی و نحوه ارزیابی تناسب شغلی در یک راننده با سابقه حوادث عروقی مغزی به جستجوی پژوهش ها در پایگاه های اطلاعاتی معتبر Embase,UpToDate, PubMed, Medline (Ovid), Scopus و web of science پرداخته شده است. شاخص هایی که در این ارزیابی باید مد نظر قرار گیرد شامل نوع و شدت حوادث عروقی مغزی، میزان احتمال خطر تکرار حمله حوادث عروقی مغزی، نوع و میزان نقص عضو در راننده متعاقب حوادث عروقی مغزی و... می باشد. همچنین در این مطالعه به چند راهنما در مورد کمک به تصمیم سازی در مورد نحوه ادامه رانندگی پرداخته شده است.
یافته هاسکته مغزی رایج ترین بیماری عصبی حاد در بین بزرگسالان در سن کار است. تقریبا یک چهارم سکته های مغزی در سنین کاری رخ می دهد. بیماری مغزی عروقی می تواند منجر به ناتوانی ناگهانی غیرمنتظره شود. تقریبا 50 درصد از افراد مبتلا به سکته مغزی در کشورهای توسعه یافته مایل به ادامه رانندگی هستند.
نتیجه گیریمواردی همچون احتمال رخداد حوادث عروقی مغزی و ناتوانی ناگهانی در آینده و مقایسه این میزان با خطر قابل پذیرش و همچنین میزان نقص عضو به وجود آمده به علت حوادث عروقی مغزی و تداخل آن با رانندگی باید مدنظر قرار گیرد. همچنین داروهای مصرفی و عوارض جانبی آن ها و بیماری های همراه دیگر باید در این ارزیابی تناسب شغلی مدنظر قرار گیرد.

Despite widespread efforts to reduce human error in different occupations, unsafe practices and behaviors are still the main cause of many accidents. The occurrence of medical errors in health care systems as one of the most important factors in decreasing the quality of services in this field has received much attention in recent years. Therefore, this study aimed to identify and evaluate all kinds of human error among nurses of Yazd Mother hospital.
Materials and MethodsThis cross-sectional and descriptive-analytical study was conducted at Yazd Mother hospital in 2019. All nursing duties in gynecology, infertility, emergency, neonatal intensive care, and in vitro fertilization were identified and subdivided using hierarchical task analysis (HTA) and then analyzed using SHERPA method.
ResultsThe highest number of errors was in a gynecology (83), infertility (73), emergency (54), neonatal intensive care (31) and in vitro fertilization, respectively. The highest percentage of errors was related to performance in all sectors. The least frequent error was selection error.
ConclusionThe results of this study showed that functional errors account for a large proportion of errors, which should be the priority of control strategies in each segment for these types of errors in order to prevent such errors. The magnitude of the errors identified after the proposed control solutions in all sections indicated the fact that acceptable solutions can be achieved by applying these strategies.

Firefighters are at high risk of developing post-traumatic stress disorder (PTSD), and that Quality of Life (QoL) is seriously compromised in individuals who have PTSD. This study was designed to assess the relationship between post-traumatic stress disorders and quality of life among firefighters.
Materials and MethodsIn this analytical and cross-sectional study, all firefighters in Birjand city were selected by census method (N = 96) in 2018. PTSD and QoL status was assessed by the Persian version of the Mississippi (Eshel) and WHOQOL-BREF questionnaires, respectively. Using Cronbachchr('39')s α, the Iranian version of these two scaleschr('39') internal reliability has been reported to be 0.92 and more than 0.7, respectively. Data analysis using SPSS software version 21 and independent T-test, ANOVA, Tukeychr('39')s test, and Pearson correlation coefficient were used for statistical analysis.
ResultsThe results showed that most firefighters (70.5%) were suffered from moderate PTSD symptoms (79.2± 11.7). Based on the self-report scale, 53.7% of the firefighters rated their quality of life as Good, and 43.2% of the firefighters were satisfied with their health. Statistical analysis showed that there is a statistically significant inverse relationship between PTSD score and areas of mental health (P = 0.03), social relations (P = 0.002), and environmental health (P = 0.004). Between PTSD score and physical health, no statistically significant relationship was found (P = 0.08).
ConclusionThis study has proven a high rate of PTSD among firefighters and its negative correlation with the quality of their life. However, most firefighters rated their health and quality of life as good. Implementing strategies to combat mental disorders among firefighters improves the quality of their life and increases their job productivity.

Exposure to dust is an essential factor in the brick production industry. Determining the mortality rate from silicosis and lung cancer is very important in exposure to crystalline silica dust. Therefore, this study was conducted to risk assessment of workers in machine factories and traditional brick production workshops.
Materials and MethodsThis cross-sectional study was conducted in 2016 in a machine factory and five traditional active brick production workshops located in the southeast of Tehran. All employees of a brick-making machine factory (40 people) and five traditional brick-making workshops (30 people) were selected by census sampling. Their exposure to total respiratory dust and respirable crystalline silica was evaluated by the NIOSH0600 method and NIOSH7602 optimal method. Mortality risk assessment from silicosis and lung cancer was performed based on Mannetej and Rice models. Data were analyzed by t-test after entering SPSS19 software.
ResultsThe average concentrations of crystalline silica and total respiratory dust in traditional workshops are 0.651 ±0.69 and 28.27 ± 23.05, and in a machine factory are 0.297± 0.27 and 17.6 ±8.6 mg / m3. The T-test showed a significant difference between the traditional and mechanical brick factory in occupational exposure to total respiratory dust (P=0.001). However, no significant difference was observed in exposure to crystalline silica (P=0.107). In both traditional and machine brick factory, the risk of death from silicosis and lung cancer is unacceptable.
ConclusionThe results showed that the industrychr('39')s mechanization has reduced exposure to total respiratory dust and crystalline silica and reduces the risk of death from lung cancer. But the risk of death from lung cancer and silicosis is still high in both traditional and mechanical factories

noise is one of the most important harmful factors in the workplace. The physiological effects of noise on humans are gradual in the long run. Despite industrial advances, exposure to excessive noise is one of the health problems of workers in the worldchr('39')s steel industry. This study aimed to investigate the effect of noise on hearing loss in steel industry workers.
Materials and MethodsThis descriptive cross-sectional study was designed and conducted. The study population was 121 workers in a steel rolling industry whose 5-year audiometry was studied from 1391 to 1395. An audiometer performed the audiometric test of these workers in 5 years. Their audiometric results were extracted from their medical records and examined under ethical principles. Data were analyzed using SPSS software version 21, and T-Test and Wilcoxon Signed Ranks Test was used for statistical analysis.
ResultsThe mean age of participants was 38.88 ± 4.92 years and the mean work experience was 10.94 ± 3.57 years. In this study, a gradual increase in the average hearing threshold at different frequencies in both ears was seen from 2012 to 2016.
Conclusionnoise as a detrimental factor in the steel industry causes a gradual increase in exposed workerschr('39') hearing threshold.

Spirometry is used as a screening method for occupational lung diseases. Dental laboratory workers are exposed to solvent vapors, toxic gases, and dust from metal alloys, acrylics, and gypsum. This study aimed to investigate the spirometric function of lung function in dental staff in Yazd.
Materials and MethodsThis descriptive cross-sectional study was performed on 37 technicians of dental laboratories in Yazd in the three months of spring 2017. To evaluate respiratory problems and their pulmonary function, standard respiratory and spirometry questionnaires according to ATS / ESR guidelines were used, respectively. Data were analyzed by SPSS software version 18 using Mann-Whitney U, Chi-square, and Studentchr('39')s T-test.
ResultsMean age of participants was 35.59±1.13. 54% of participants had no respiratory symptoms. The most common respiratory complain were dry and exudative cough. The work field was the only influential factor in FVC%. Age and work experience, and exercise no significant effect on respiratory parameters. Overall spirometry parameters were in the normal pattern range, and only 5% of participants showed a mild restrictive pattern.
ConclusionSpirometric pulmonary function of Yazdchr('39')dental laboratory technicians was in the normal range, and possibly working in the dental laboratory did not have a significant effect on the pulmonary Function of technicians based on spirometric indices.

Pages 53-63IntroductionDue to the importance and necessity of accident analysis, it is necessary to use the proper technique for precise accident analysis and provide corrective and preventive measures to prevent an accidentchr('39')s recurrence.Materials and MethodsIn this descriptive-analytical paper, the most important criteria for investigating and selecting accident investigation and analysis techniques and selecting the best accident analysis method were identified in critical industrial accidents in the construction phase, were identified and analyzed. In this study, the most important criteria for selecting an accident analysis method were identified using previous research and gathering expert opinions. Then, two critical power plant accidents were analyzed using TRIPOD BETA and FTA accident analysis methods. Then the pairwise comparisons matrix was formed based on the strengths and weaknesses of the models. Finally, the prioritization of these two methods was done using the hierarchical analysis decision-making method.ResultsIn this paper, seven key factors, model realism, model descriptive, systematic modeling, run time, required training courses, ability to quantify, and visibility of events, were identified as the most important criteria for selecting an incident analysis method.ConclusionThe TRIPOD BETA method has been introduced as an optimal method for investigating specific events due to its capabilities.Keywords: Accident Analysis, Safety, Error Tree Analysis, TRIPOD BETA

Stroke is considered a relatively common disease in developing societies. This disease is one of the most important causes of sudden non-cardiac incapacity. On the other hand, the disease can naturally occur in working-age and drivers. Therefore evaluation of the fitness for work in a driver with a history of stroke can be a significant challenge for the occupational health physician and road safety.
Materials and MethodsIn this study, to identify the main indicators and evaluate job fitness in a driver with stroke history, research has been conducted in valid databases such as Embase, UpToDate, PubMed, Medline (Ovid), Scopus, and web of science. Indicators that should be considered in this evaluation include the type and severity of the stroke, the probability of recurrence of the stroke attack, the degree and type of limb defect in the driver following the stroke, etc. Also in this study has been paid to some guidelines for help in deciding how to continue driving.
ResultsStroke is the most common acute neurological disease among working-age adults. About a quarter of strokes occur at working age. Cerebrovascular disease can lead to sudden disability. Approximately 50% of people with stroke in developed countries want to continue driving.
ConclusionThings such as the probability of occurrence of stroke and sudden incapacity in the future and comparing this amount with acceptable risk and the amount of limb defect caused by stroke and its interference with driving should be considered. Also, consumption medications and their side effects and other comorbidities should be considered in this job fitness evaluation.
