    مقدمه و هدف

    عوارض و مشکلات مربوط به دیابت بر کیفیت زندگی بیماران دیابتی اثرگذار است. هدف پژوهش حاضر مقایسه اثربخشی مدیریت استرس و  درمان مبتنی بر پذیرش و تعهد بر کنترل قند خون و کیفیت زندگی بیماران مبتلا به دیابت نوع دو بود.

    مواد و روش ها

    این پژوهش نیمه آزمایشی با طرح پیش آزمون -پس آزمون-پیگیری با 2 گروه آزمایش و 1 گروه کنترل بود. جامعه آماری پژوهش، کلیه بیماران مبتلا به دیابت نوع دو مراجعه کننده به مرکز تخصصی و فوق-تخصصی میلاد تهران در سال 1398 بودند. پس از بررسی معیارهای ورود به مطالعه تعداد 45 نفر با روش نمونه گیری هدفمند انتخاب و با روش تصادفی در سه گروه (هر گروه 15 نفر) جایگزین شدند. گروه مدیریت استرس 12 جلسه و گروه پذیرش و تعهد 8 جلسه 90 دقیقه ای آموزش دیدند. داده های پژوهش با استفاده از مقیاس کیفیت زندگی سازمان بهداشت جهانی (1996) و با آزمایش قند خون ناشتا گردآوری شدند و با استفاده از آزمون تحلیل واریانس مختلط در نرم افزار SPSS نسخه 20 تجزیه و تحلیل شدند.

    یافته ها

    نتایج نشان داد مداخله مدیریت استرس و درمان مبتنی بر پذیرش و تعهد تاثیر معناداری بر کنترل قند خون و مولفه های کیفیت زندگی در بیماران مبتلا به دیابت نوع دو داشته است (05/0>P). همچنین بین اثرگذاری مدیریت استرس و درمان مبتنی بر پذیرش و تعهد بر کیفیت زندگی و کنترل قند خون تفاوت معناداری وجود ندارد (05/0<P) و تنها مولفه ی سلامت روان بود که میانگین نمرات پس آزمون و پیگیری آن در گروه پذیرش و تعهد بیشتر از گروه مدیریت استرس افزایش داشت (05/0>P).

    نتیجه گیری

    با توجه به یافته های مطالعه حاضر، برای بهبود کیفیت زندگی و کنترل قند خون می توان از روش های مدیریت استرس و درمان مبتنی بر پذیرش و تعهد استفاده کرد.

    مقدمه و هدف

    بیماری دیابت به عنوان شایع ترین بیماری ناشی از اختلالات متابولیسم، یک چالش پر اهمیت جهانی و نیز یکی از علتهای مهم مرگ و ناتوانی در سراسر دنیا محسوب می شود. این بیماری می تواند بر سلامت عمومی و احساس خوب بودن و به عبارتی بر کیفیت زندگی بیماران تاثیرات منفی داشته باشد، داشتن آگاهی و رفتارهای مناسب نیز می تواند از عوارض این بیماری بکاهد. لذا مطالعه حاضر با هدف بررسی میزان تاثیر آموزش بر آگاهی،رفتارهای تغذیه ای و کیفیت زندگی بیماران دیابتی طراحی و اجرا گردید.

    مواد و روش ها

    این مطالعه نیمه تجربی برروی 144 نفر از بیماران مبتلا به دیابت نوع 2(دو گروه 72 نفره) شهر زابل از بین مراجعین به مرکز دیابت و مراکز بهداشتی درمانی انجام و داده ها به وسیله پرسشنامه محقق ساخته در مرحله پیش آزمون و پس آزمون جمع آوری و توسط نرم افزار SPSS20 و آزمون های مربوطه مورد تجزیه و تحلیل قرار گرفتند.

    یافته

    در گروه مورد؛ در قبل از مداخله بین سن با آگاهی، کیفیت زندگی و رفتارها، وضعیت تاهل با کیفیت زندگی، سابقه بیماری با آگاهی، و روش آموزش با آگاهی و کیفیت زندگی و در بعد از مداخله آموزشی؛ سن، جنس، تحصیلات و شغل با کیفیت زندگی ارتباط معنی داری داشتند(0/05>P). در گروه شاهد نیز در قبل از آموزش؛ آگاهی با سن، تحصیلات، شغل، وضعیت تاهل، سابقه بیماری، درآمد، BMI و روش آموزش ارتباط معنی داری داشت(0/05>P). همچنین بین سابقه بیماری با کیفیت زندگی و روش آموزش با رفتارهای خودمراقبتی ارتباط وجود داشت(0/05>P) و در بعد از مداخله آموزشی فقط بین روش آموزش با آگاهی و رفتارهای خودمراقبتی ارتباط وجود داشت(0/05>P). از بین متغیرهای اصلی تحقیق؛ در گروه مورد، در بعد از مداخله آموزشی کیفیت زندگی با رفتارهای خودمراقبتی ارتباط معنی داری داشت(0/05>P). دو گروه از نظر آگاهی (بعد از مداخله آموزشی)، رفتارها و کیفیت زندگی در قبل و بعد از مداخله آموزشی با هم اختلاف معنی داری داشتند(0/05>P).

    نتیجه گیری

    آموزش بهداشت، دانش، کیفیت زندگی و رفتارهای خودمراقبتی بیماران را ارتقا می دهد و از عوارض بیماری که می تواند باعث ناتوانی ، مرگ و میر و مشکلات اقتصادی-اجتماعی در جامعه شود جلوگیری می کند.

    مقدمه و هدف

    یکی از عواقب مهم زندگی با دیابت انگ اجتماعی است. در این مطالعه انگ اجتماعی و دانش دیابت دانشجویان ترم 1 و 7 مقطع کارشناسی سنجیده و مقایسه شد.

    مواد و روش ها

    در این مطالعه ی توصیفی-تحلیلی که به صورت مقطعی انجام شد، دانشجویان ترم یک و هفت مقطع کارشناسی دانشگاه علوم پزشکی به صورت نمونه گیری تصادفی انتخاب گردیدند. کلیه دانشجویان داوطلب که رضایت نامه شرکت در مطالعه و همچنین سه پرسشنامه مشخصات دموگرافیک، انگ اجتماعی وابسته به دیابت و دانش دیابت را تکمیل نمودند. پس از ترجمه و بارترجمه، روایی پرسشنامه ی انگ امیک و دانش دیابت با روایی صوری و روایی محتوا توسط 12 نفر از متخصصین حوزه سنجیده شد و نسبت روایی محتوا  برای هر دو پرسشنامه عدد قابل قبول 0/64 و  CVI برای پرسشنامه های دانش دیابت و امیک به ترتیب برابر با 0/87 و 0/88 محاسبه شد. نتایج توسط نرم افزار آماری SPSS نسخه 24 تحلیل گردید.

    یافته ها

    106 نفر از دانشجویان ترم 1 در مطالعه شرکت کردند که 68 نفر (64/2درصد) زن و 38 نفر مرد (35/8 درصد) بودند. 106 نفر از دانشجویان ترم 7 نیز در مطالعه شرکت کردند که 72 نفر (67/9درصد) زن و 34 نفر (32/1 درصد مرد بودند). میانگین نمرات دانش و انگ در دانشجویان ترم اول به ترتیب (0/36) 11/23 و (0/53) 8/19 و در دانشجویان ترم هفتم (0/42) 13/97و (0/56) 8/83 بود

    نتیجه گیری

    دانش دانشجویان 7 نسبت به ترم 1 در مورد دیابت به طور معناداری بیشتر است. اما نمرات انگ اجتماعی این دو گروه اختلاف معناداری با یکدیگر نداشت. این یافته ها نشان می دهد که تنها تغییرات دانش نمی تواند به تغییرات انگ کمک نماید و برای انگ زدایی از دیابت احتمالا نیاز به کاوش مداخلات چندبعدی است.

    مقدمه و هدف

    بیماری های عروق کرونر از علل اصلی مرگ و میر در دنیا می باشند، عوامل مختلفی در بروز و مرگ و میر ناشی از آنها موثرند. دیابت یکی از فاکتورهای خطر بروز سندرم کرونری حاد می باشد. این مطالعه با هدف بررسی میزان قند خون بالا در بدو بستری با مرگ و میر داخل بیمارستانی بیماران با سندروم حاد کرونری انجام گرفت.

    مواد و روش ها

    این مطالعه مقطعی_ توصیفی در246 بیمار بستری با سندرم کرونری حاد در بخش مراقبت ویژه قلبی بیمارستان امیر المومنین زابل  انجام شد. بیماران بر اساس قند خون بدو مراجعه به دو گروه (اول) با قند خون ≥ 200 میلیگرم بر دسی لیتر  و (دوم) کمتر از200 میلی گرم بر دسی لیتر تقسیم شدند و عوارض قلبی عروقی و مرگ و میر طی مدت بستری در دو گروه بررسی شد. داده ها با استفاده از فرم های اطلاعاتی جمع آوری و  بررسی شد و در نهایت توسط نسخه 22 نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.

    یافته ها

    از 246 بیمار مورد مطالعه با میانگین سنی6/5±62/8 ، 166(67/5 درصد) مرد و 106(43 درصد) بیمار  قند خون بدو بستری بالای 200 میلی گرم بر دسی لیتر داشتند. فراوانی سابقه فشارخون بالا ، بیماری عروق کرونر، ، شاخص توده بدنی، کلسترول توتال،  LDL، سطح سرمی تروپونین ، افت کسر جهشی بطن چپ  در گروه اول بیشتر  بود. میزان حوادث قلبی عروقی از جمله نارسایی حاد قلبی ، شوک کاردیوژنیک، آریتمی ، سکته مغزی و مرگ و میر داخل بیمارستانی ناشی از آن نیز در گروه اول بالاتر بود. علی رغم فراوانی بالاتر مرگ در بیماران با قندخون بالا ، سابقه قبلی دیابت تاثیری در افزایش مرگ و میر نداشت.

    نتیجه گیری

    با  اقدامات پیشگیرانه از جمله غربالگری منظم جمعیت در معرض خطر و تشخیص زودهنگام دیابت و کنترل آن می توان از بروز عوارض قلبی عروقی از جمله سندروم های کرونری حاد و مرگ و میر ناشی از آنها پیشگیری و از تحمیل بار اقتصادی، اجتماعی و درمانی بیشتر بر جامعه اجتناب کرد.

    مقدمه و هدف

    با در نظر گرفتن روند شتابان رشد دیابت نوع دو و نقش اثرگذار عوامل شناختی مانند آمیختگی شناختی و عواطف، مطالعه حاضر باهدف تعیین اثربخشی شناخت درمانی مبتنی بر ذهن آگاهی بر آمیختگی شناختی و عاطفه مثبت و منفی زنان مبتلا به دیابت نوع دو انجام شد.

    مواد و روش ها

    پژوهش حاضر نیمه تجربی با طرح پیش آزمون- پس آزمون با گروه کنترل بود. جامعه آماری شامل تمامی زنان مبتلا به دیابت مراجعه کننده به مرکز دیابت شهر بیرجند در شش ماه دوم سال 1396 بودند که 30 نفر به صورت نمونه گیری در دسترس انتخاب و به طور تصادفی در دو گروه آزمایش و کنترل قرار گرفتند. مداخله پذیرش و تعهد طی 8 جلسه 90 دقیقه ای انجام شد. داده های پژوهش با استفاده از پرسش نامه 7 سوالی آمیختگی شناختی گیلاندر و همکاران و مقیاس 20 سوالی ارزیابی عاطفه مثبت و منفی واتسون و همکاران جمع آوری شدند. جهت تحلیل داده ها از آزمون تحلیل کوواریانس چند متغیره به وسیله نرم‏افزار SPSS نسخه 24 در سطح معنی داری 05/0=α استفاده شد.

    یافته ها

    نتایج به دست آمده نشان داد که تفاوت معناداری بین میانگین نمرات آمیختگی شناختی و عاطفه مثبت و منفی در بیماران دیابتی نوع دو وجود داشت (0/05>P).

    نتیجه گیری

    با توجه به نتایج این پژوهش شناخت درمانی مبتنی بر ذهن آگاهی در کاهش آمیختگی شناختی و عاطفه منفی و افزایش عاطفه مثبت بیماران مبتلا به دیابت نوع دوموثر بود.

    مقدمه و هدف

    سالمندان علی الخصوص سالمندان مبتلا به دیابت به دلیل نقص نسبی سیستم ایمنی و بیماری زمینه ای در برابر بیماری های عفونی آسیب پذیرتر اند و به دلایل مختلفی مستعد ابتلا به کووید-19 هستند. بالطبع این شرایط موجب آثار سوء زیادی در سلامت جسمی سالمند می گردد که یکی از این آثار زخم های فشاری هستند. لذا هدف این مطالعه مروری بر مطالعات انجام شده در زمینه روش های پیشگیری و مدیریت زخم های فشاری سالمندان دیابتی مبتلا به کووید-19 بوده است.

    مواد و روش ها

    این مطالعه بصورت مرور روایتی انجام گردید. در ابتدا پایگاه های اطلاعاتی گوگل اسکالر(Google scholar) ، کوکران(Cochrane) ، کلینیکال کی (Clinical Key)، مد لاین و SID جهت جستجو  طبق کلید واژه های تعیین شده انتخاب شدند که تعداد 390 مطالعه مرتبط با موضوع یافت گردید. این جستجو به زبان انگلیسی و فارسی انجام گرفت و از میان این تعداد، 14 مطالعه که واجد شرایط ورود به مطالعه بودند انتخاب  و مورد تجزیه و تحلیل قرار گرفتند.

    یافته ها

    سالمندان دیابتی مبتلا به کووید-19 بعلت التهاب شدید، عوارض عروقی، عدم تحرک، موقعیت خوابیده به روی شکم و  اتصالات تنفسی بیشتر در معرض زخم فشاری قرار دارند. تغییرات سطح انسولین و نیز تغییرات سطح سیتوکاین ها در افزایش التهاب و ایجاد زخم فشاری نقش دارند.

    نتیجه گیری

    مدیریت دیابت، استفاده از پانسمان های پیشرفته ، جلوگیری از دهیدراسیون، تغییر وضعیت قرار گیری سالمند، پایش محل اتصالات تنفسی بیمار و تغذیه مناسب در پیشگیری از ایجاد زخم فشاری نقش دارند.

    Introduction

    Complications and problems related to diabetes affect the quality of life of diabetic patients. The aim of this study was to comparison of the effectiveness of stress management and acceptance and commitment-therapy on blood sugar control and quality of life in patients with type 2 diabetes

    Materials and Methods

    This was a quasi-experimental study with pre-test-post-test-follow-up design with 2 experimental groups and 1 control group. The statistical population of the study was all patients with type 2 diabetes referred to Milad Specialized and Sub-Specialized Hospital in Tehran in 2020. After reviewing the inclusion criteria, 45 people were selected by purposive sampling method and replaced by random method in three groups (15 people in each group). The stress management group received 12 sessions and the acceptance and commitment group received 8 sessions of 90 minutes. Research data were collected using the World Health Organization Quality of Life Scale (1996) and by fasting blood sugar test. And were analyzed using mixed analysis of variance test in SPSS software version 20.

    Results

    The results showed that stress management intervention and acceptance and commitment therapy had a significant effect on blood sugar control and quality of life components in patients with type 2 diabetes (P <0.05). Also, there is no significant difference between the effect of stress management and t acceptance and commitment therapy on quality of life and blood sugar control (P>0.05). And it was the only component of mental health that the mean of post-test and follow-up scores in the acceptance and commitment group increased more than the stress management group (P <0.05).

    Conclusion

    According to the findings of the present study, to improve the quality of life and control blood sugar, stress management methods and commitment therapy can be used.

    Introduction

    Diabetes, as the most common disease caused by metabolic disorders, is a major global challenge and one of the leading causes of death and disability worldwide. This disease can have negative effects on general health, well-being, and the quality of life of patients. Accordingly, the attainment of proper knowledge and behaviors can reduce the complications of this disease. Therefore, the present study aimed to investigate the effect of education on knowledge, nutritional behaviors, and quality of life of diabetic patients.

     Materials and Methods

    This quasi-experimental study was performed on 144 patients with type 2 diabetes mellitus (72 cases per group) in Zabol, Iran. The patients were selected out of those who were referred to diabetes and health centers. The data were collected using a researcher-made questionnaire that was administered at pretest and posttest. Subsequently, the obtained data were analyzed in SPSS software (version 20) through the relevant statistical tests.

    Results

    In the case group, there was a significant relationship between age and knowledge, quality of life and behaviors, marital status and quality of life, history of disease and knowledge, as well as the method of education with knowledge and quality of life before intervention. After the intervention, the quality of life showed a significant relationship with age, gender, education level, and occupational status (P<0.05). In the control group, the knowledge revealed a significant association with age, education level, occupational status, marital status, history of diseases, income, body mass index, and method of education before the intervention (P<0.05). Moreover, there was a relationship between the history of disease and quality of life, as well as the method of education and self-care behaviors (P<0.05). After the educational intervention, the results revealed the relationship of the method of education with knowledge and self-care behaviors (P<0.05). Among the main variables of the study, in the case group, the quality of life was significantly associated with self-care behaviors after the educational intervention (P<0.05). The two groups were significantly different in terms of knowledge (after educational intervention), behaviors, and quality of life before and after the intervention (P<0.05).

    Conclusion

    Health education promotes the patientschr('39') quality of life and self-care behaviors; moreover, it prevents disease complications that can cause disability, mortality, and socioeconomic problems in society.

    Introduction

    Public stigma is one of the important consequences of living with diabetes. The present study aimed to compare and assess diabetes-related public stigma and diabetes knowledge of first and seventh-semester undergraduate students.

    Methods

    In this cross-sectional descriptive-analytical study, first and seventh- semester undergraduate students of Shahrekord University of Medical Sciences in the first semester of 209 were selected by random sampling. All volunteered students who returned the study consent form completed three questionnaires, including demographic characteristics, diabetes-related public stigma, and diabetes knowledge. A number of 16 experts assessed the face and content validity of questionnaires after translation and back translation. Content validity ratio was estimated at 0.64 for both questionnaires, while content validity indices of 0.87, and 0.88 were calculated for diabetes knowledge and diabetes-related public stigma, respectively. The results were analyzed in SPSS statistical software (version 24).

    Results

    A total of 106 first-semester students participated in the study, the majority of whom (64.2%) were female. Moreover, 106 seventh-semester students participated in the study, the majority of whom (67.9%) were female. The mean scores of knowledge and stigma in first semester students were obtained at 11.23 (0.36) and 8.19 (0.53), respectively, and in seventh-semester students, these scores were reported as 13.97 (0.42) and 8.83 (0.56), respectively.

    Conclusion

    As evidenced by the obtained results, seventh-semester students scored higher in the diabetes knowledge questionnaire, as compared to first-semester students. Nevertheless, the public stigma scores of the two groups were not significantly different. These findings suggested that knowledge improvements alone cannot contribute to stigma changes, and multidimensional interventions may be needed to overcome diabetes-related stigma.

    Introduction

    Coronary artery disease (CAD) is one of the leading causes of death worldwide, and various factors are involved in the incidence of CAD and the mortality caused by this disease. Diabetes is one of the risk factors of acute coronary syndromes (ACS). This study aimed to evaluate the impact of hypertension on admission on the in-hospital mortality rate of patients with ACS.

    Materials and Methods

    This cross-sectional descriptive study was performed on 246 patients with acute coronary syndrome hospitalized at the cardiac intensive care unit of Amir Al-Momenin Hospital in Zabol, Sistan and Balouchestan, Iran. Patients were divided into two groups based on their blood glucose at admission, namely the group with a blood sugar of ≥ 200 mg/dl and the group with a blood glucose of < 200 mg/dl. The rate of cardiovascular complications and morbidity were assessed in the two groups during hospitalization. The required data were collected using information forms and finally analyzed in SPSS software (version 22).

    Results

    Out of 246 patients with the mean age of 62.8±6.5 years, 166 (67.5%) subjects were male and 106 (43%) patients were in the first group with a blood glucose of ≥ 200 mg/dl at admission. The frequency of history of hypertension, coronary heart disease, body mass index, total cholesterol, low density lipoprotein, serum troponin, left ventricular ejection fraction was higher in the first group. Furthermore, the rates of cardiovascular events including acute heart failure, cardiogenic shock, arrhythmia, stroke, and in-hospital mortality were significantly higher in the first group. Despite the higher incidence of death in patients with high blood sugar, previous history of diabetes had no effect on increasing the mortality rate.

    Conclusion

    preventive measures, such as regular screening of at-risk population and early diagnosis and control of diabetes, can prevent such cardiovascular complications as acute coronary syndromes and their related mortalities and avoid imposing more economic, social, and medical burden on societies.

    Introduction

    Considering the accelerated development of type II diabetes and the role of cognitive factors, such as cognitive fusion and emotions, the present study aimed to determine the effectiveness of mindfulness-based cognitive therapy on cognitive fusion, as well as positive and negative affect in women with type II diabetes.

    Materials and Methods

    This quasi-experimental study was conducted based on a pretest-posttest design with a control group. The statistical population consisted of all women with diabetes referred to the Diabetes Center in Birjand, Iran, in the second half of 2018. In total, 30 cases were selected by convenience sampling and randomly divided into experimental and control groups. Acceptance and commitment intervention was performed in eight 90-min sessions. The required data were collected using a 7-item Cognitive Fusion Questionnaire by Gillander et al. and a 20-item Positive and Negative Affect Schedule by Watson et al. Subsequently, the collected data were analyzed in SPSS software (version 24) through multivariate analysis of covariance. A p-value less than 0.05 was considered statistically significant

    Results

    The results showed a significant difference between the mean scores of cognitive fusion and positive and negative affect in type 2 diabetic patients (P≤0.05).

    Conclusion

    According to the results of this study, mindfulness-based cognitive therapy is effective in reducing cognitive fusion and negative affect and increasing positive affect in patients with type II diabetes.

    Introduction

    Older adults, especially those with diabetes, are more vulnerable to infectious diseases due to relative immunodeficiency and underlying disease, and are prone to Covid-19 for a variety of reasons. Of course, these conditions cause many adverse effects on the physical health of the elderly, one of which is pressure sores. Therefore, the purpose of this study was to review the studies performed on the prevention and management of pressure ulcers in diabetic elderly with Covid-19.

    Materials and Methods

        This study was performed as a narrative review initially, Google Scholar, Cochrane, Clinical Key, Medline and SID databases were selected for search according to the specified keywords, and 390 related studies were found. This search was conducted in English and Persian and from this number, 14 studies that were eligible for inclusion in the study were selected and analyzed.

    Results

    Diabetic older adults with Covid-19 are more prone to pressure ulcers due to severe inflammation, vascular complications, immobility, supine position, and respiratory connections. Changes in insulin levels as well as changes in cytokine levels play a role in increasing inflammation and pressure ulcers.

    Conclusion

    Diabetes management, the use of advanced dressings, prevention of dehydration, changing the position of the elderly, monitoring the patientchr('39')s respiratory connections and proper nutrition are all involved in preventing pressure ulcers.
