    مقاله ششم از سری مطالعات مروری در مورد بیماری کووید 19 درحالی چاپ می گردد که یکسال و نیم از پیدایش و تهاجم این ویروس می گذرد و همچنان مردم دنیا با آن درگیرند. در این شماره، آخرین یافته ها در مورد تظاهرات پوستی بیماری، تشخیص و درمان بیماری، سیر بیماری های پوستی و همچنین عوارض پوستی واکسن ها ارایه می گردد.

    زمینه و هدف

    درماتیت آتوپیک یک بیماری مزمن التهابی است. این بیماری درنتیجه یک فرآیند التهابی تحریک شده توسط سلول های Th 2 فعال شده در مواجهه با آلرژن ها ایجاد می شود. آکنه وولگاریس یک بیماری التهابی و مزمن پوستی است. مطالعات اخیر نشان می دهند که پروپیونی باکتریوم آکنه باعث القای تولید IL-17 توسط سلول های مونونوکلیار محیطی و سلول های T (Th1/Th17) می شود. مطالعه حاضر جهت بررسی شیوع درماتیت آتوپیک در بیماران مبتلا به آکنه طراحی شده است.

    روش اجرا

    در این مطالعه موردی شاهدی، گروه مورد شامل 75 فرد مبتلا به آکنه و گروه کنترل شامل 75 فرد با عدم ابتلا به آکنه که ازنظر سن و جنس همانندسازی شده اند مورد مطالعه قرار گرفتند. در هر دو گروه به روش مصاحبه علایم درماتیت آتوپیک، آسم و رینیت آلرژیک را در زمان حال و گذشته بررسی کردیم.

    یافته ها

    احتمال وجود درماتیت آتوپیک در زمان حال در افرادی که آکنه داشتند کمتر از افرادی بود که آکنه نداشتند. تفاوت معنی داری بین گروه مورد و شاهد در فراوانی سابقه درماتیت آتوپیک در گذشته، فراوانی آسم و رینیت آلرژیک در زمان حال و گذشته وجود نداشت.

    نتیجه گیری

    وجود رابطه بین آکنه با متغیر درماتیت آتوپیک در زمان حال می تواند موید این مطلب باشد که فعالیت Th2 می تواند باعث کاهش شیوع بیماری هایی با فعالیت Th1 شود. با این حال باید مطالعات بیشتری برای تایید آن صورت گیرد.

    زمینه و هدف 

    تاکنون ارتباط بین تجربیات بیماران و کیفیت خدمات ارایه شده به آنان به طور گسترده بررسی نشده است، اما شواهد نشان می دهد که بهبود تجربیات بیمار می تواند نتایج درمانی را بهبود بخشد. هدف این پژوهش تعیین رضایت مندی والدین کودکان مراجعه کننده به درمانگاه کودکان و عوامل مرتبط با آن بوده است.  

    روش اجرا

    این مطالعه توصیفی با استفاده از پرسشنامه ازقبل طراحی شده Press Ganey به زبان فارسی و ازطریق مصاحبه با 207 نفر از والدین کودکان مراجعه کننده به درمانگاه کودکان بیمارستان پوست رازی در سال97 انجام شد.  

    یافته ها

    از 207 نفر شرکت کننده 87 نفر پدر (42%) و 105 نفر مادر (7/50%) بودند. 15 نفر باقی مانده نسبت های دیگری با کودک داشتند. رضایتمندی کلی (در پاسخ به سوال «درمانگاه ما را به دیگران توصیه می کنید») 8/62% بود. همچنین 5/72% شرکت کنندگان معتقد بودند پزشک خود را به دیگران توصیه خواهند کرد. کمترین میزان رضایت مربوط به معرفی نکردن کارکنان درمانگاه و مشکلات در تنظیم وقت ویزیت بود. عواملی که بیشترین ارتباط را با رضایت کلی بیماران داشتند، به ترتیب شامل شاداب بودن ویزیت توسط پزشک، اطمینان داشتن به پزشک و توصیه پزشک به دیگران بودند.  

    نتیجه گیری

    ارتباط بین پزشک و بیمار به طور معناداری با کیفیت تجربه بیمار در درمانگاه پوست کودکان ارتباط دارد. خانواده ها و بیماران در هنگام ارزیابی تجارب خود، ساده صحبت کردن و حساسیت پزشک را مشخصا مورد توجه قرار می دهند.

    زمینه و هدف

    اونیکومایکوزیس یک عفونت ناشی از قارچ های مخمری، رشته ای و غیررشته ای می باشد که به دلیل شرایط مختلف ایجاد می شود. این مطالعه با هدف بررسی الگوی حساسیت به آزول ها در گونه های کاندیدای جداشده از بیماران ایرانی در شهر تهران انجام شد.

    روش اجرا

    پس از نمونه گیری از بیماران، برای شناسایی سویه های جداشده از محیط کروم آگار کاندیدا استفاده و تست PCR-Sequencing انجام شد. سپس الگوی حساسیت دارویی گونه ها به فلوکونازول، ایتراکونازول و کتوکونازول با استفاده از روش استاندارد  CLSI-M27-A3 / S4تعیین شد.

    یافته ها

    در این مطالعه 34 جدایه کاندیدا شناسایی شد که 26 جدایه (5/76%) کاندیدا آلبیکنس و 8 جدایه (5/23%) کاندیدا گلابراتا بودند. نسبت به فلوکونازول، 4 تا (11%) از جدایه ها حساس وابسته به دوز بودند و بقیه حساس بودند. دربرابر ایتراکونازول، 21 ایزوله (61%) حساس وابسته به دوز و 2 (5%) ایزوله مقاوم شناسایی شد و در برابر کتوکونازول، همه جدایه ها حساس بودند. میانگین هندسی (GM) حداقل غلظت مهاری فلوکونازول، ایتراکونازول و کتوکونازول به ترتیب 32/0، 21/0 و 15/0 میکروگرم در میلی لیتر بود. در این مطالعه، بیشترین گونه جداشده، گونه C. albicans بود. با توجه به مقادیر GM، موثرترین آزول، کتوکونازول بود.

    نتیجه گیری

    شیوع و الگوی حساسیت گونه های کاندیدا به آزول ممکن است در جمعیت های مختلف بیماران متفاوت باشد، بنابراین توصیه می شود پزشکان به نتایج آزمایشات حساسیت دارویی توجه کنند و سپس بیماران را با درنظرگرفتن تداخلات دارویی و عوارض جانبی درمان کنند.

    ورزش و پیشرفت ملت ها آمیختگی زیادی به یکدیگر پیدا کرده اند و علاقه به فعالیت های ورزشی در جهان افزایش یافته است. درماتولوژی ورزشی، رشته ای رو به رشد است. بیماری های پوستی ورزشکاران یک گروه متمایز از بیماری هایی است که فعالیت بدنی تکراری همراه با عوامل استرس زای محیطی، ورزشکار را در بالاترین سطح احتمال ابتلا قرار می دهد. هدف از این مقاله مسلح کردن پزشکان با دانش بنیادی از بیماری های پوستی در ورزشکاران است، عدم شناخت نقش ورزش در پاتوژنز بیماری پوستی، می تواند منجر به تاخیر انداختن تشخیص و درمان نادرست شود. بیماری پوستی می تواند به طور مستقیم یا غیرمستقیم توسط ورزش ایجاد شود، دسته ای از بیماری های پوستی نیز در فرد وجود داشته لیکن با شروع ورزش بروز کرده یا تشدید شده اند. به طور کلی قبل از اینکه یک ورزشکار به ورزش بپردازد، باید از حیث پوستی مورد معاینه قرارگیرد تا از گسترش بیماری های پوستی جلوگیری شود. معاینه به ویژه نواحی کمتر مورد توجه مانند پشت، داخل گوش ها و... از حیث وجود ضایعات، اهمیت دارد. بهتر است از ضایعات بیمار نقشه ای تهیه شود که در طول زمان قابل ردگیری و تغییرات آن قابل پیگیری باشد(Body Mapping) . ورزشکار باید تا 72 ساعت قبل از فعالیت ورزشی علایم عمومی نظیر تب نداشته باشد. هیچ ضایعه پوستی مترشح نباید وجود داشته باشد وگرنه لازم است درمان هایی را قبل از حضور در سالن های ورزشی و استفاده از تجهیزات دریافت کند تا از عدم سرایت بیماری به سایرین اطمینان حاصل شود.

    آلوپسی آره آتا (AA) نوعی ریزش موی ناگهانی غیراسکاردهنده می باشد که به صورت پچ های گرد یا بیضی در هر منطقه مودار می تواند مشاهده شود. این بیماری یک اختلال اتوایمون با پاسخ ایمونولوژیکی علیه فولیکول های مو می باشد که عوامل ژنتیکی و محیطی در ایجاد آن نقش دارند. شیوع AA در حدود 2/0-1/0 درصد در سراسر جهان است. براساس شدت بیماری و محل های درگیری می تواند به انواع آلوپسی با الگویPatchy ، Alopecia reticularis، آلوپسی توتالیس، آلوپسی یونیورسالیس، Alopecia ophiasis، آلوپسی Sisiapho، آلوپسی منتشر و Perinevoid alopecia areata تقسیم شود. گزینه های مختلف درمانی برای AA وجود دارد ولی پس از خاتمه، میزان عود زیادی بروز می دهند. AA با انفیلتراسیون لنفوسیت های T اطراف بولب فولیکول مو شناخته می شود و به عنوان درمان اصلی، کورتیکواسترویید ها مطرح می باشند. یکی دیگر از راه های درمان، ایمونوتراپی موضعی است که از مواد بسیار مهم آن دیفنیل سیکلوپروپنون است (DPCP). این ماده در مواردی که بیش از 50% ریزش مو داشته اند یا مقاوم به درمان شده اند استفاده می شود. مکانیسم دقیق عملکرد DPCP به درستی مشخص نیست ولی احتمالا نوعی رقابت آنتی ژنیک داشته و باعث کاهش تولید آنتی بادی های ضد فولیکول های مو می شود. این دارو می تواند پاسخ ایمنی را در پوست تغییر دهد و نسبت سلول های CD4 به CD8 در اطراف فولیکول مو را به نحوی عوض کند که منجر به بهبودی بیماری شود. بیان شده که میزان رشد مجدد مو ها در درمان با DPCP به طور میانگین 50 درصد است. در این مقاله به بررسی آخرین یافته ها راجع به AA پرداخته و جوانب درمانی آن، بالاخص با DPCP را بیان کرده ایم.

    پچ های میکرونیدل حاوی سوزن هایی با ابعاد میکرون بوده که با عبور از لایه شاخی پوست منجر به افزایش نفوذ دارو شده و در زمینه های مختلف ازجمله دارورسانی، صنعت زیبایی، تجویز واکسن و تشخیص بیماری ها مورد استفاده قرار می گیرد. این مقاله به بررسی انواع میکرونیدل ها می پردازد و عملکرد، طبقه بندی و تفاوت آن ها را شرح می دهد.

    Background and Aim

    Atopic dermatitis is a chronic inflammatory disease that is caused by an inflammatory process stimulated by Th2 cells. Acne vulgaris is a chronic inflammatory skin disease. It is associated with an increase in sebum secretion, unusual pilosebaceous keratinization and an increased inflammatory immune response. Propionibacterium acne can induce IL-17 production and Th1/Th17 response. The present study was designed to determine the prevalence of atopic dermatitis in patients with acne.

    Methods

    In this case-control study, the case group consisted of 75 individuals with acne and the control group consisted of 75 individuals without acne matched in age and gender. Both groups were interviewed for atopic dermatitis, asthma and allergic rhinitis symptoms in the past and present. 

    Results

    Prevalence of atopic dermatitis at present in patients who had acne was significantly lower than those who did not have acne. There was no significant difference between case and control groups in frequency of atopic dermatitis in the past and frequency of asthma and allergic rhinitis in the present and past time.

    Conclusion

    The inverse relationship between prevalence of acne and atopic dermatitis can be related to activation of different immune responses (Th1 versus Th2), but more studies should be done to confirm this relationship.

    Background and Aim

    So far, the relationship between patient experiences and the quality of services provided to them has not been widely studied. However, evidence suggests that improving patient experiences can improve clinical outcomes. The aim of this study was to determine the satisfaction of parents of children referring to the pediatric clinic as well as the related factors.

    Methods

    This descriptive study was conducted using a pre-designed Press Ganey questionnaire in Persian language and through interviews with 207 parents of children who referred to the pediatric clinic of Razi Dermatology Hospital in 2018. 

    Results

    Of the 207 participants, 87 were fathers (42%) and 105 were mothers (50.7%). The remaining 15 had other relationships with the child. Overall satisfaction (recommending our clinic to others) was 62.8%. Also, 72.5% of the participants believed they would recommand their doctors. The lowest level of satisfaction was due to the lack of introduction of clinic staff and the difficulties in setting up an appointment. The factors that most closely correlate with the patientchr('39')s overall satisfaction included respectively: the cheerfulness of the visit by the doctor, the confidence in the doctor, and the likelihood of recommending the doctor.

    Conclusion

    The relationship between a physician and a patient is significantly related to the quality of patientschr('39') experiences in the pediatric dermatology clinic. Families and patients, when evaluating their experiences, focused on clear language and doctorchr('39')s sensitivity.

    Background and Aim

    Onychomycosis is an infection caused by yeast, filamentous and non-filamentous fungi, due to diverse conditions. This study aimed to investigate the pattern of azole susceptibility of candidate species isolated from Iranian patients in Tehran.

    Methods

    After sampling from patients, identification of the isolated strains were performed with Candida chrom agar medium and PCR-sequencing test. Drug Susceptibility pattern of the species to fluconazole, itraconazole and ketoconazole were determined by CLSI-M27-A3/S4 standard method.

    Results

    In this study, 34 candidate isolates were identified, of which 26 isolates (76.5%) were Candida albicans and 8 isolates (23.5%) were Candida glabrata. In fluconazole, 4 (11%) of the isolates were dose-dependent sensitive and the others were sensitive. In itraconazole, 21 (61%) dose-dependent isolates and 2 (5%) resistant isolates were identified, and in ketoconazole, all isolates were sensitive. The geometric mean (GM) of the minimum inhibitory concentration for fluconazole, itraconazole and ketoconazole was 0.32, 0.21 and 0.15 μg/mL, respectively. In this study, the most frequently isolated species was C. albicans. According to the GM values, the most effective azoles was ketoconazole.

    Conclusion

    The prevalence and pattern of susceptibility of Candida species to azole may vary in different populations of patients. Therefore, it is recommended that the clinicians pay attention to the results of drug susceptibility tests and then treat patients by considering drug interactions and side effects.

    Sports and the development of nations are very much intertwined. Sports dermatology is a growing discipline. Athletes’ skin diseases are a distinct group of diseases, and that repetitive physical activity combined with environmental stressors puts the athlete at the highest level of risk.The purpose of this study is to equip physicians with basic knowledge of skin diseases in athletes. Lack of knowledge about the role of exercise in the pathogenesis of skin diseases can lead to delayed diagnosis and consequently, incorrect treatment. Skin diseases can be caused directly or indirectly by exercise; there are some skin diseases which exist in a person but appear or intensify with the onset of exercise. In general, before an athlete should exercise, he should be examined dermatologically to prevent the spread of skin diseases. They should be considered especially for lesions on the less noticeable areas such as the back and inner side of ears. It is better to prepare a map of the patient’s lesions that can be tracked over time, which the possibility of following their changes (Body Mapping). Athletes should have no general symptoms such as fever for 72 hours before exercising. There should be no secretory skin lesions, otherwise, it is necessary to receive treatments before going to the gym and using the equipment to ensure that the disease does not spread to others.

    Alopecia areata (AA) is a sudden non-scarring hair loss that can be seen in any hairy area in the form of round or oval patches. It is an autoimmune disorder with an immunological response against hair follicles that is caused by genetic and environmental factors. The prevalence of AA is about 0.1% to 0.2% worldwide. Depending on the severity of the disease and sites of involvement, it can be divided into: Alopecia with Patchy pattern, alopecia reticularis, alopecia totalis, alopecia universalis, alopecia ophiasis, alopecia sisiapho, diffuse type and perinevoid alopecia areata. There are various treatment options for AA, but once terminated, they have a high recurrence rate. AA is known for the infiltration of T lymphocytes around the hair follicle bulb, and corticosteroids are its main treatment. Another treatment is topical immunotherapy, the most important of which is Diphenylcyclopropenone (DPCP). It is used in cases that have more than 50% hair loss or are resistant to treatment. The exact DPCP mechanism of action is not well understood, but it may have some kind of antigenic competition and reduce the production of anti-hair follicle antibodies. It can alter the immune response in the skin and alter the ratio of CD4 to CD8 cells around the hair follicle in a way that enhances healing of the disease. It is stated that the rate of hair regrowth in DPCP treatment is about 50% on average. In this article, we review the latest findings of about AA and discuss its therapeutic aspects, especially with DPCP.

    Microneedles consist of micron-sized projections similar to needles. They are capable of piercing through the stratum corneum and increase the permeation of active ingredients. Besides active pharmaceutical ingredients, microneedles can also deliver cosmetic agents, biotherapeutics like insulin and vaccines and also are used in diagnostic purposes. This article examines the types of microneedles. It also describes the function, classification, and differences between them.
