    مقدمه و هدف

    فعالیت های خودمراقبتی به عنوان چارچوبی برای آموزش بیماران دیابتی مورد استفاده قرار گرفته است. شواهد نشان داده است بیمارانی که در فرآیند مراقبت خود نقش فعال تری دارند، به نتایج سلامتی بهتری دست پیدا می کنند. مطالعه حاضر با هدف طراحی یک مرور نظام مند از منابع علمی در طول یک دهه گذشته برای شناسایی متغیرهایی که رفتارهای خودمراقبتی را در بیماران مبتلا به دیابت نوع 2 تعیین می کند، انجام شده است.

    مواد و روش ها

    با استفاده از کلید واژه های مرتبط، جستجو در بانک های اطلاعاتی Pub Med، EMBASE، CINAHL، Scopus، Med line، DOAJ، Oxford و سایر منابع علمی، جهت یافتن مقالات فارسی و انگلیسی در فاصله مرداد سالهای 1389 تا 1398 انجام شد. مراحل استخراج و گزارش این مرور نظام مند بر طبق پروتکل PRISMA ارایه شده است.

    یافته ها

    در جستجوی اولیه 1813 مطالعه به دست آمد که پس از حذف مقاله های غیرمرتبط و همپوشان، 62 مورد وارد بررسی نهایی شدند. در کل، 18 متغیر تعیین کننده رفتارهای خودمراقبتی در بیماران مبتلا به دیابت نوع 2 به دست آمد که بیشترین و تاثیرگذارترین متغیرها عبارت بودند از: خودکارآمدی، آگاهی، سطح تحصیلات، سن، مدت ابتلا و سواد سلامت.

    نتیجه گیری

    در مجموع، شش عامل اصلی به عنوان عوامل تعیین کننده رفتارهای خودمراقبتی در بیماران مبتلا به دیابت نوع 2 شناخته شد که خودکارآمدی قوی ترین پیش بینی کننده خودمراقبتی بود و انتظار می رود در طراحی و اجرای برنامه های آموزشی و تغییر رفتار در بیماران دیابتی مورد توجه قرار گیرد. مطالعات بیشتر در آینده ممکن است از یافته های این مطالعه مروری پشتیبانی کند.

    مقدمه و هدف

    دیابت یارداری درجه ای از عدم تحمل گلوکز، ناشی از ناتوانی پانکراس در ترشح انسولین بیش تر برای پاسخ به افزایش مقاومت به انسولین و یا کاهش حساسیت به انسولین  طی بارداری است که اولین بار در بارداری دیده می شود. مطالعات نشان داده که دیابت بارداری  و اختلال عملکردی در تیرویید می تواند اثرات بسیار بدی بر بارداری و کودک داشته باشد. در این پژوهش، ارتباط دیابت بارداری با میزان هورمون های تیروییدی در زنان باردار را بررسی کردیم.

    مواد و روش ها

    این پژوهش مورد شاهدی بر روی 252 مادر باردار در دو گروه دیابت بارداری (87) و سالم(165) مراجعه کننده به مراکز بهداشتی زابل با روش تصادفی انجام شد و سطح سرمی FBS ، تیروکسین آزاد (FT4) و هورمون تحریک کننده تیرویید (TSH) بین دو گروه مقایسه شد. جهت تحلیل داده ها از نرم افزار SPSS نسخه 22 استفاده شد.

    یافته ها

    میانگین سطح TSH در گروه مورد و شاهد به ترتیب 56/1± 75/2 و 07/1± 78/1 میکرولیتر در میلی لیتر بود و در گروه مورد به طور معنی داری بالاتر بود. میانگین سطح FT4 در گروه مورد و شاهد به ترتیب 61/0 ± 5/1 و 68/0 ± 4/1 نانوگرم بود که از نظر آماری در دو گروه تفاوت نداشت. در میان بیماران مبتلا به دیابت ، 16 نفر (48/12درصد) دچار هیپوتیروییدی تحت بالینی بودند و 6 نفر (69/7 درصد) هیپوتیروییدی بالینی داشتند در حالی که در گروه شاهد ، 7 نفر (24/4 درصد) دارای هیپوتیروییدی تحت بالینی و 4 نفر (42/2 درصد) دارای هیپوتیروییدی بالینی بودند که از نظر آماری معنی دار بود.

    نتیجه گیری

    این یافته ها ارتباط معنی داری بین هیپوتیروییدی و دیابت بارداری را نشان می دهد. بنابراین سنجش و ارزیابی دقیق هورمون های تیروییدی در دوران بارداری به ویژه در مادران دیابتی، برای مدیریت و پیش آگهی بهتر دیابت بارداری پیشنهاد می شود.

    مقدمه و هدف

    درمان مبتنی بر پذیرش و تعهد به عنوان روش درمانی موثر برای درمان و کاهش علایم روانشناختی بیماری های مزمن پزشکی از جمله دیابت به کار رفته است. هدف از انجام پژوهش حاضر، بررسی تاثیر درمان مبتنی بر پذیرش و تعهد بر خود مراقبتی و سرمایه روان شناختی بیماران مبتلا به دیابت بوده است.

    مواد و روش ها

    در این پژوهش نیمه آزمایشی از طرح پیش آزمون- پس آزمون با گروه کنترل و دوره پیگیری (سه ماهه) استفاده شد. بدین منظور 30 نفر از افراد مبتلا به دیابت نوع دو که در انجمن دیابت شهرکرد پرونده داشتند با استفاده از نمونه گیری هدفمند انتخاب شدند و به صورت تصادفی در دو گروه آزمایشی و کنترل (هر گروه 15 نفر) قرار گرفتند. ابزار جمع آوری داده ها شامل پرسشنامه خود مراقبتی  و سرمایه روان شناختی بود که توسط شرکت کنندگان در پیش آزمون و پس آزمون تکمیل شدند. سپس گروه آزمایشی، 8 جلسه هفتگی90 دقیقه ای درمان مبتنی بر پذیرش و تعهد را دریافت کرد. در نهایت داده ها با استفاده از نرم افزار SPSS20 مورد تجزیه و تحلیل قرار گرفت.

    یافته ها

    نتایج نشان داد که در سرمایه روان شناختی بین دو گروه مطالعه در مرحله پس آزمون و پیگیری تفاوت معناداری وجود دارد.. همچنین تفاوت بین خودمراقبتی در دو گروه در مرحله پس آزمون و پیگیری معنادار بود. علاوه بر این، تاثیر درمان در گروه آزمایشی پایدار بود و بین گروه ها تفاوت معناداری وجود داشت.

    نتیجه گیری

    با استناد به یافته های به دست آمده از این پژوهش می توان نتیجه گرفت که استفاده از درمان مبتنی بر پذیرش و تعهد در کنار درمان های پزشکی می تواند باعث افزایش توان خود مراقبتی و سرمایه روان شناختی بیماران مبتلا به دیابت شود.

    مقدمه و هدف

    عوارض و مشکلات دیابت با تبعیت از درمان قابل کنترل است. هدف از انجام پژوهش حاضر بررسی اثر بخشی درمان تلقیح استرس بر تبعیت از درمان بیماران دیابت نوع 2 با شاخص  HbA1C  بوده است.

    مواد و روش ها

    روش پژوهش، نیمه آزمایشی با طرح پیش آزمون و پس آزمون و گروه کنترل بود. جامعه آماری پژوهش حاضر شامل بیماران دیابت نوع2 دارای پرونده بیمارستان امام خمینی شهرستان محلات در سال99-1398بود. 50 بیمار دیابتی نوع 2 به صورت نمونه گیری دردسترس انتخاب و در دو گروه آزمایش و کنترل همتاسازی شدند. ابزار پژوهش شامل پرسشنامه تبعیت از درمان و آزمایش همگلوبین A1C برای هردو گروه بود. طی 10 جلسه 90 دقیقه ای برای گروه آزمایش فنون آموزش تلقیح استرس اجرا گردید. پس از اجرای درمان روی هر دو گروه، مجددا پرسشنامه و آزمایش خون برای بررسی هموگلبین A1C انجام گردید و داده های گردآوری شده با استفاده از تحلیل کواریانس یک راهه مورد تجزیه وتحلیل قرار گرفتند.

    یافته ها

    نتایج حاصل از پژوهش، نشان داد که میانگین نمرات تبعیت از درمان دارویی تبعیت از رژیم غذایی در گروه مداخله نسبت به گروه کنترل افزایش یافته بود که این افزایش از نظر آماری معنی دار بود. نتایج بیانگر کاهش معنی دار میانگین هموگلوبین  A1C در گروه مداخله نسبت به گروه کنترل بود.اما کاهش میانگین نمرات تبعیت درمانی در فعالیت فیزیکی معنادار نبود.

    نتیجه گیری

    یافته های مطالعه حاضر می تواند به عنوان روشی جهت کنترل هموگلوبینA1C به کار رود.

    مقدمه و هدف

    دیابت به عنوان شایع ترین بیماری متابولیکی جهان، مشکلات جسمی و روانی فراوانی را برای مبتلایان ایجاد می کند. یکی از گروه های پر خطردر دوران شیوع کرونا، افراد مبتلا به دیابت هستند که نیازمند دریافت خدمات بهداشتی ویژه می باشند. مطالعه حاضر با هدف بررسی رابطه بین اضطراب کرونا و کیفیت زندگی با نقش میانجی کیفیت خواب در افراد مبتلا به دیابت انجام شد. 

    مواد و روش ها

    پژوهش مقطعی همبستگی حاضر در افراد مبتلا به دیابت سنین 25 تا 70 سال با حجم نمونه 200 نفر مراجعه کننده به انجمن دیابت شهرستان سیرجان وابسته به دانشگاه علوم پزشکی و خدمات بهداشتی درمانی شهرستان سیرجان انجام شد. پژوهش به طور هدفمند بر روی داده های جمع آوری شده از مقیاس اضطراب کرونای علیپور و همکاران با پایایی 919/0، مقیاس کیفیت زندگی افراد مبتلا به دیابت (DQOL) با پایایی 0/77 و مقیاس کیفیت خواب پترزبورگ با پایایی  0/86  همراه با تایید روایی آزمون ها انجام و در محیط نرم افزار SPSS23 در سطح معنا داری 0/05.مورد تحلیل قرار گرفت.

    یافته ها

    یافته ها حاکی از آن است که اضطراب کرونا با کیفیت خواب و کیفیت زندگی افراد مبتلا به دیابت همبستگی معکوس و معنادار؛ و کیفیت خواب با کیفیت زندگی آنها رابطه مستقیم و معناداری دارد. طبق یافته های به دست آمده نقش میانجی کیفیت خواب در رابطه بین اضطراب کرونا و کیفیت زندگی افراد مبتلا به دیابت تایید می گردد.

    نتیجه گیری

    نتایج پژوهش حاضر نشان داد که اضطراب کرونا می تواند کیفیت زندگی و کیفیت خواب افراد مبتلا به دیابت را کاهش دهد و کاهش کیفیت خواب می تواند کاهش کیفیت زندگی را به همراه داشته باشد لذا با توجه به اهمیت ویژه این موضوع، درنظر گرفتن مداخلات در برنامه ریزی ها به منظور بهبود کیفیت زندگی و کیفیت خواب بیماران مبتلا  به دیابت در دوران همه گیری کرونا ضروری به نظر میرسد.

    مقدمه و هدف

    افتادن و ترس از افتادن در سالمندان مبتلا به دیابت یک مشکل شایع است. تشخیص افراد در معرض خطر از اهمیت زیادی برخوردار است. تحقیق حاضر با هدف تعیین ارتباط بین ترس از افتادن با تعادل ایستا و پویا در سالمندان مبتلا به دیابت نوع 2 طراحی واجرا گردید.

    مواد و روش ها

    در این مطالعه تحلیلی- مقطعی، 134 سالمند مبتلا به دیابت نوع 2 انتخاب شدند. با استفاده از شاخص بین المللی کارآمدی افتادن FES-I ترس از افتادن مورد بررسی قرار گرفت و بر اساس آن بیماران به دو گروه با و بدون ترس از افتادن تقسیم شدند. آزمونهای تعادلی ایستادن روی یک پا و راه رفتن در مسیر باریک برای هر یک از نمونه ها در هر دو گروه انجام شد. سپس داده ها در نرم افزار SPSS تجزیه تحلیل شدند.

    یافته ها

    از مجموع 134 نفر سالمند، 26/1 درصد سابقه  افتادن داشتند، 70/9 درصد مبتلا به نوروپاتی بودند و بیش از نیمی از آنها ترس از افتادن را تجربه کرده بودند (51/5 درصد). عملکرد ایستا و پویای سالمندان در گروه با ترس از افتادن و بدون ترس از افتادن تفاوت معنی داری را  نشان داد. بدین معنا که نتایج آزمون های تعادلی در گروهی که ترس از افتادن داشتند نسبت به گروه بدون ترس از افتادن نتایج ضعیفتری را نشان داد.

    نتیجه گیری

    نتایج نشان داد تعادل ایستا و پویا در سالمندانی که ترس از افتادن داشتند به طور معنی داری ضعیف تر است. پیشنهاد می شود ضمن مشاوره روانشناسی سالمندان مبتلا به دیابت که ترس از افتادن دارند، تمرینات ورزشی و تعادلی برای ایشان برنامه ریزی و اجرا شود.

    Introduction

    Self-care activities have been used as a framework for patient-centered diabetes education. Accumulating evidence suggests that patients who take a more active role in their care process achieve better health outcomes. The present study aimed to design a systematic review of scientific resources over the past decade to identify variables that determine self-care behaviors in patients with type II diabetes (T2D).

    Materials and Methods

    MeSH keywords were applied to search several databases including PubMed, EMBASE, CINAHL, Scopus, Med line, DOAJ, Oxford, and the Scientific Information Database for English and Persian language studies between August 2010 and 2019. The process of conducting and reporting this systematic review was guided according to the statements of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA).

    Results

    The literature search yielded 1,813 publications, of which 62 were included in this study after eliminating non-relevant and overlapping papers. A total of 18 factors and variables determining self-care behaviors were obtained in patients with T2D. The most influential variables were self-efficacy, knowledge, level of education, age, duration of diabetes, and health literacy.

    Conclusion

    In sum, six main factors were identified as determinants of self-care behaviors in patients with T2D which self-efficacy was the strongest predictor of self-care and is expected to be considered in designing and implementing educational programs and behavior change in diabetic patients. Further studies in the future may support the findings of this review study.

    Introduction

    Gestational diabetes is a degree of glucose intolerance that is first diagnosed or developed during pregnancy due to the inability of the pancreas to secrete more insulin to respond to increased insulin resistance or decreased insulin sensitivity. Many studies have shown that gestational diabetes mellitus (GDM) and thyroid dysfunction can have a variety of adverse effects on the outcomes of pregnancy and offspring. In the present study, the relationship between gestational diabetes and thyroid hormone levels was investigated in pregnant women in Zabol.

    Materials and Methods

    In this case-control study, 252 pregnant women (87 with and 165 without GDM) were randomly admitted during 2020. The serum levels of FBS, free thyroxine (FT4), and thyroid-stimulating hormone (TSH) were compared between the two groups. Data were analyzed using SPSS statistical software (version 22).

    Results

    The mean TSH levels in the case and control groups were 2.75 ± 1.56 and 1.78 ± 1.07 μIU/mL, respectively, which was significantly higher in the case group (P < 0.00). Mean FT4 levels were 1.5 ± 0.61 and 1.4 ± 0.68 ng/ in case and control groups, respectively which were not statistically different in both groups (P = 0.25). Among patients with GDM, 16 (12.48%) had subclinical hypothyroidism and 6 (7.69%) had clinical hypothyroidism, while among non-GDM patients, 7 (4.24%) had subclinical hypothyroidism and 4 (2.42%) had clinical hypothyroidism.

    Conclusion

    These findings indicate a significant association between hypothyroidism and gestational diabetes. Therefore, accurate measurement and evaluation of thyroid hormones during pregnancy, especially in diabetic mothers, is recommended for better management and its prognosis.

    Introduction

    Acceptance and commitment therapy has been used as one of the effective methods for the treatment and reduction of psychological symptoms of chronic diseases, such as diabetes. The present study aimed to determine the effect of acceptance and commitment therapy on the self-care and psychological capital of patients with diabetes.

    Materials and Methods

    This quasi-experimental study was conducted based on a pre-test-post-test control group design and follow-up period (3months). For this purpose, 30 people with type 2 diabetes who had a medical record in Shahrekord Diabetes Association were selected using purposive sampling and randomly assigned to two groups of experimental and control (n=15 in each group). The data collection tools included a self-care questionnaire and psychological capital which were completed by the participants. Finally, the data were analyzed in SPSS software (version 20).

    Results

    The results showed a significant difference in psychological capital between the two study groups in the post-test and follow-up stages (P<0.05). Moreover, the difference between the self-care of the two groups in the post-test and follow-up stages was significant (P<0.05). Furthermore, the effect of the intervention was stable in the experimental group, and there was a significant difference between the groups (P<0.05).

    Conclusion

    As evidenced by the results of this study, it can be concluded that the use of acceptance and commitment therapy, along with medical therapies, can enhance self-care capacity and psychological capital in diabetes patients.

    Introduction

    Complications and problems of diabetes can be controlled by following the treatment. This study aimed to evaluate the effectiveness of stress inoculation treatment on adherence to the treatment among type II diabetic patients with glycated hemoglobin index.

    Materials and Methods

    This quasi-experimental study was conducted based on a pretest-posttest control group design. The statistical population of the present study included patients with type 2 diabetes having a record in the Imam Khomeini Hospital, Mahallat, Iran, in 2009-2010. The samples (n=50) were selected by convenience sampling method and matched in experimental and control groups. The research instruments included a questionnaire of adherence to treatment and a glycated hemoglobin test for both groups. Stress inoculation training techniques were performed in ten 90-minute sessions for the experimental group. After the administration of treatment in both groups, the questionnaire and blood test were performed again to evaluate glycated hemoglobin. The collected data were analyzed using a one-way analysis of covariance.

    Results

    The results of the study showed that the mean scores of adherence to medication treatment (P=0.0001) and adherence to diet (P=0.0001) in the intervention group were increased, compared to the control group, which was statistically significant. Based on the results, a significant decrease was observed in the mean of glycosylated hemoglobin in the intervention group, compared to the control group (P=0.0001). However, the decrease in the mean of adherence to treatment in physical activity was not significant (P=0.062).

    Conclusion

    According to the findings of the present study, stress inoculation training can be used to control glycosylated hemoglobin.

    Introduction

    Diabetes, as the most common metabolic disease in the world, poses numerous physical and mental problems to patients. Diabetic patients constitute one of the high-risk groups in the Covid-19 pandemic and need special health care. The present study aimed to investigate the relationship between Covid-19-related anxiety and quality of life with the mediating role of sleep quality in people with diabetes.

    Materials and Methods

    This cross-sectional correlational study was performed on 200 diabetic people aged 25-70 years who were referred to the Sirjan Diabetes Association affiliated with Sirjan University of Medical Sciences and Health Services. The data were collected using the COVID-19 Anxiety Scale developed by Alipour et al. with a reliability coefficient of 0.919, the Diabetes Quality of Life Scale with a reliability of 0.77, and the Petersburg Sleep Quality Scale with a reliability of 0.86. The data were analyzed in SPSS software (version 23) at a significant level of 0.05.

    Results

    Based on the findings, Covid-19-related anxiety had a significant inverse correlation with sleep quality and quality of life in people with diabetes; moreover, the quality of sleep had a direct and significant relationship with their quality of life. Furthermore,  the findings confirmed the mediating role of sleep quality in the relationship between Covid-19 anxiety and quality of life in people with diabetes (t≥ 1.96; P≤0.01).

    Conclusion

    As evidenced by the results of the present study, Covid-19-related anxiety can reduce the quality of life and sleep quality of people with diabetes, and poor sleep quality can reduce the quality of life. Therefore, the critical importance of this issue highlights the necessity of making interventions to improve the quality of life and sleep quality of patients with diabetes during the Covid-19 pandemic.

    Introduction

    Falling and fear of falling, especially in the elderly with diabetes, is a common problem. It is highly important to identify people at risk. This study aimed to determine the relationship between the fear of falling and static and dynamic balance in the elderly with type II diabetes.

    Materials and Methods

    In this analytical cross-sectional study, 134 elderly people with type II diabetes were selected. The fear of falling was examined using the Falls Efficiency Scale International, and based on that, patients were divided into two groups, namely with and without fear of falling. Single leg standing and Narrow Path Walking tests were performed for each of the samples in both groups. Data were analyzed using SPSS software (version 22).

    Results

    Out of 134 elderly people with diabetes, 26.1% had a history of falling, 70.9% suffered from neuropathy, and 51.5% had experienced fear of falling. Static and dynamic balance tests showed a significant difference in the groups with and without fear of falling (P<0.001). It means that the results of balance tests were poorer in the group with the fear of falling than in the group without the fear of falling.

    Conclusion

    The results showed that static and dynamic balance was significantly weaker in the elderly who had the fear of falling. It is suggested that in addition to psychological counseling for the elderly with diabetes who have the fear of falling, physical and balance exercises be planned and implemented for them.
