محیط شرطی شده سلول های بنیادی مزانشیمی مشتق از بند ناف (UC-MSCs-CM) ترشحاتی با خواص ضد آپوپتوتیک تولید می کند و این پتانسیل را دارد که از آپوپتوز سلول های گرانولوزا (GC) در طی تحریک کنترل شده تخمدان جلوگیری کند.
هدفمشاهده اثر UC-MSCs-CM بر تعامل بین پروتئین های پرو و ضد آپوپتوز و تاثیر تولید فاکتور تمایز رشد 9 (GDF9) در GC بود.
مواد و روش هاUC-MSCs-CM از کشت سلول های بنیادی بند ناف در پاساژ 4 جمع آوری شد. GCs از 23 زن که تحت لقاح آزمایشگاهی قرار گرفته بودند، کشت داده شدند و به مدت 24 ساعت در معرض UC-MSCs-CM قرار گرفتند. سپس RNA GC استخراج شد و بیان mRNA مربوط به BCL-2 X (BAX)، survivin و GDF9 با استفاده از Real-time PCR کمی آنالیز شد. محیط کشت مصرف شده GC برای اندازه گیری فاکتور رشد انسولین 1 با استفاده از روش الایزا، جمع آوری شد.
نتایجبیان BAX پس از قرار گرفتن در معرض UC-MSCs-CM به طور قابل توجهی متفاوت بود (4.09E-7 در مقابل 3.74E-7، 02/0 = p). پس از قرار گرفتن در معرض UC-MSCs-CM هیچ تغییر قابل توجهی در survivin، نسبت BAX/survivin و بیان GDF9 رخ نداد (05/0 < p). سطح IGF-1 CM پس از استفاده از CM به عنوان محیط کشت برای GC به طور قابل توجهی بالاتر بود (28/2 در مقابل 72/1 ± 07/3، 001/0 ≥p). همبستگی مثبت و معناداری بین Survivin و GDF9 یافت شد (966/0 = r، 001/0 ≥ p).
نتیجه گیریIGF-1 تولید شده توسط UC-MSCs-CM می تواند به روش پاراکرین از طریق گیرنده IGF کار کند که می تواند BAX را مهار کرده و تولید GDF9 را حفظ کند. علاوه بر این، تحت تاثیر UC-MSCs-CM، GCها همچنین قادر به تولید IGF-1 هستند که می تواند GC را از طریق فرآیندهای اتوکرین تحت تاثیر قرار دهد.

نانوذرات دی اکسید تیتانیوم در طیف گسترده ای مورد استفاده قرار می گیرند، نتایج اخیر نشان داده شده است که این نانوذرات بر دستگاه تولید مثلی مردان اثرات سیتوتوکسیکی می گذارند.
هدفاثرات مصرف نانو ذرات دی اکسید تیتانیوم بر کروماتین و پارامترهای اسپرم و هیستوپاتولوژی لوله ای سمینی فروس موش های نر بررسی شده است.
مواد و روش هادر این مطالعه تجربی، 32 موش نرNMRI (35 ± 3، سن 12-8 هفته) به چهار گروه تقسیم شدند (در هر گروه 8 موش): گروه های تحت درمان دوزهای 5/2 (گروه اول)، 5 (گروه دوم) و 10 (گروه سوم) میلی گرم در کیلوگرم نانوذره تیتانیوم به مدت 40 روز خوراکی خورانده شدند و گروه کنترل بافر فسفات سالین دریافت کردند. پارامترهای اسپرم، یکپارچگی و کیفیت کروماتین DNA اسپرم با استفاده از رنگ آمیزی کرومومایسین A3، آنیلین بلو، تولوییدین بلو و تانل مورد بررسی قرار گرفتند. رنگ آمیزی هماتوکسلین-ایوزین برای اندازه گیری سلول های اسپرماتوژنیک و قطر کل در لوله های سمینی فروس انجام شد. همچنین سطح هورمون های جنسی و مالون دی الدهید بررسی شد.
نتایجدم غیرطبیعی اسپرم در گروه سوم (91/4 ± 78/28) در مقایسه با گروه کنترل (95/3 ± 75/12) افزایش یافت. در حالی که رنگ آمیزی کرومومایسین A3 در گروه سوم نسبت به گروه کنترل میزان بالاتری نشان داد، با این حال رنگ آمیزی آنیلین بلو و تولوییدین بلو تفاوتی نداشت. کاهش معنی داری در شاخص اسپرماتوژنز و پارامترهای لومن در گروه سوم مشاهده شد. تعداد سلول های لیدیگ، قطر و مساحت سلولی لوله سمینی فروس در گروه های درمان کمتر بود. همچنین سطح هورمون تستوسترون در این گروه ها پایین تر بود و درصد مالون دی الدهید در مایع سمنی بیشتر بود.
نتیجه گیریمکانیسم دقیق نانوذرات تیتانیوم مشخص نیست اگرچه اثرات سیتوتوکسیکی و ژنوتوکسیکی نانوذرات تیتانیوم مربوط به استرس اکسیداتیو است. با توجه به کاربرد گسترده آن ها، نانواکسیدتیتانیوم باید کانون توجه سلامت عمومی باشند.

فعال سازی ناموفق تخمک به دنبال تزرق داخل سیتوپلاسمی اسپرم (ICSI) در نتیجه کمبود کلسیم یک چالش بزرگ است.
هدفما تاثیر محیط کشت فعال کننده تخمک (CAM) را بر نتایج ICSI در موارد آزواسپرمی انسدادی مقایسه کردیم.
مواد و روش هامطالعه حاضر با 152 مورد ICSI در دو گروه فعال سازی تخمک مصنوعی (CAM) و کنترل انجام شد. تخمک های تزریق شده در گروه کنترل در محیط کلیواژ کشت شدند، در حالی که در گروه CAM، تخمک ها از طریق قرار گرفتن در معرض 200 میکرولیتر CAM به مدت 15 دقیقه به صورت شیمیایی فعال شدند. نرخ لقاح و کلیواژ، کیفیت جنین و میزان حاملگی بیوشیمیایی و تولد زنده در هر دو گروه مورد بررسی قرار گرفت.
نتایجمیزان لقاح و کلیواژ پس از استفاده از CAM در زیرگروه آسپیراسیون اسپرم از اپیدیدیم (001/0 ≥ p، 05/0 = p) (PESA) و در زیرگروه استخراج اسپرم از بیضه (TESE) در مقایسه با گروه های کنترل خود تفاوت معنی داری وجود داشت (04/0 = p، 02/0 = p). همچنین میزان بارداری در زیرگروه PESA-CAM به طور معنی داری بیشتر بود (03/0 = p). زیرگروه PESA-CAM تفاوت معنی داری را در کیفیت جنین پس از ICSI نشان داد (04/0 = p). انتقال ناموفق جنین و سقط جنین در هر دو زیرگروه نسبت به گروه کنترل خود کمتر بود، اما این تفاوت معنی دار نبود. به طور معنی دار، نرخ تولد زنده در زیر گروه PESA-CAM بالاتر بود (03/0 = p).
نتیجه گیریتیمار با CAM می تواند میزان لقاح و کلیواژ را در موارد آزواسپرمی انسدادی بهبود بخشد. تاثیر معنی داری بر کیفیت جنین و میزان حاملگی و تولد زنده در موارد PESA وجود داشت.

واکنش های کلیدی در متابولیسم تک کربن با واسطه فولات توسط آنزیم های چرخه فولات تنظیم می شود. نقض چرخه فولات ممکن است با وقوع محدودیت رشد جنین (FGR) در زنان باردار همراه باشد.
هدفاین مطالعه ارتباط بین پلی مورفیسم ژن های چرخه فولات در مادر با ایجاد FGR را نشان می دهد.
مواد و روش هادر این مطالعه مورد-شاهدی، 122 زن باردار با FGR و 243 زن باردار با وزن طبیعی نوزاد وارد مطالعه شدند. مکان های پلی مورفیسم ژن های چرخه فولات شامل rs1805087 5-methylenetetrahydrofolate (MTR) و rs1979277 سرین هیدروکسی متیل ترانسفراز (SHMT1) مورد بررسی قرار گرفتند. بررسی پلی مورفیسم ها از طریق روش تشخیص پروب TaqMan با استفاده از واکنش زنجیره ای پلیمراز انجام شد. برای تجزیه و تحلیل ارتباط پلی مورفیسم از رگرسیون لجستیک استفاده شد.
نتایجمشخص شد که آلل T rs1979277 ژن SHMT1 با ایجاد FGR در چارچوب آللی همبستگی دارد (01/0 >pperm ، 33/2-20/1 CI %95، 67/1 =OR)، مدل های افزودنی (01/0 >pperm ، 37/2-20/1 CI %95، 69/1 = OR)، غالب (01/0 = pperm، 87/2-15/1 CI %95، 81/1 =OR) و مغلوب (01/0 = pperm، 73/4-15/1 CI %95، 34/2 =OR). ارتباط آلل G rs1805087 ژن MTR با وقوع FGR نیز به دنبال مدل مغلوب مشخص شد (04/0 = pperm، 68/8-05/1 CI %95، 01/3 =OR).
نتیجه گیرینتایج ما نشان داد که جایگاه های پلی مورفیسم های rs1979277 SHMT1 و rs1805087 MTR مادری ممکن است با ایجاد FGR مرتبط باشند.

بارداری از طریق روش های کمک باروری (ART) یک تجربه استرس زا است که ممکن است دلبستگی پره ناتال را تحت تاثیر قرار دهد. با این وجود، دلبستگی مادری-جنینی (MFA) و اضطراب در بارداری بعد از ART کمتر مورد بررسی قرار گرفته است.
هدفهدف این مطالعه، مقایسه تغییرات MFA و اضطراب بارداری در سه ماهه اول و سوم در زنان باردار شده از طریق روش های کمک باروری و طبیعی بود.
مواد و روش هایک مطالعه طولی در سال 1398 بر روی 187 زن باردار (بارداری طبیعی= 144، کمک باروری= 43) انجام شد. مشارکت کنندگان با استفاده از نمونه گیری مستمر از یک کلینیک پره ناتال در تهران انتخاب شدند. جهت گردآوری داده ها از مقیاس دلبستگی مادر-جنینی Cranley و پرسشنامه اضطراب مرتبط با بارداری Van Den Bergh استفاده شد.
نتایجدر زنان ART، نمره MFA در هفته 12 بارداری پایین تر از زنان باردار طبیعی بود اما در هفته 36 بارداری، تفاوت معنی داری وجود نداشت. در هر دو گروه MFA در 36 هفته به طور معنی داری بالاتر از 12 هفته بود (001/0 ≥ p). افزایش در نمره MFA به طور معنی داری در زنان ART بالاتر از زنان باردار طبیعی بود (001/0 ≥ p). نمره اضطراب بارداری در دو گروه افت پیدا کرد و تفاوت معنی داری در تغییرات نمره اضطراب بین دو گروه مشاهده نشد (84/0 = p).
نتیجه گیریدلبستگی به جنین در زنان باردار شده از طریق روش های کمک باروری در سه ماهه سوم بارداری مشابه سایر زنان بود و آن ها اضطراب بیشتری را تجربه نکردند.

هایپرپرولاکتینمی یکی از شایع ترین علل ناباروری در زنان است. شیوع تومور هیپوفیز 30-25 درصد در بین این زنان است.
هدفهدف این مطالعه توصیف علل هایپرپرولاکتینمی در زنان نابارور مراجعه کننده به مرکز ناباروری یزد است.
مواد و روش هادر این مطالعه مقطعی، 182 زن نابارور با هایپرپرولاکتینمی از فوریه 2018 تا اکتبر 2019 به مرکز ناباروری یزد مراجعه کرده اند وارد مطالعه شدند. سطح سرمی پرولاکتین با روش Elisa و طبق پروتکل Padtan Gostar Isar اندازه گیری شد. سن، مدت نازایی، علل هایپرپرولاکتیمنی و نوع درمان ناباروری ثبت شد، یافته های MRI نیز اضافه شد.
نتایجمیانگین سنی افراد 36/0± 9/28 سال بود و سطح پرولاکتین بیماران 97/8 ± 76 بود. علت هایپرپرولاکتینمی در 35 نفر (2/19%)، آدنوم هیپوفیز، در 47 نفر (8/25%)، سندرم تخمدان پلی کیستیک و در 14 نفر (7/7%) آدنوم هیپوفیز و سندروم تخمدان پلی کیستیک و 86 نفر (3/47%) هایپرپرولاکتینمی ایدیوپاتیک بود. در این مطالعه بین میانگین سطح پرولاکتین و علل مختلف هایپرپرولاکتینمی ارتباط معنی داری وجود نداشت (317/0 = p).
نتیجه گیریدر این مطالعه هایپرپرولاکتینمی ایدیوپاتیک و سندرم تخمدان پلی کیستیک شایعترین علت هایپرپرولاکتینمی در خانم های نابارور هستند.

دیابت ملیتوس سبب ایجاد استرس اکسیداتیو شده و به آسیب بیضه و ناباروری مردان منجر می شود.
هدفهدف از این مطالعه مقایسه اثرات متفورمین بعنوان یک داروی شیمیایی با سیلی مارین بعنوان یک داروی گیاهی بر روی پارامترهای اسپرم و تغییرات هیستوپاتولوژیکی بیضه در موش های صحرایی نر دیابتی می باشد.
مواد و روش ها32 سر موش صحرایی نر نژاد ویستار (وزن 250-270 گرم) بطور تصادفی به 4 گروه تقسیم شدند. (8 سر در هر گروه). 1) کنترل، 2) دیابتی، 3) دیابتی + متفورمین 200 میلیگرم بر کیلوگرم، 4) دیابتی + سیلی مارین 100 میلیگرم بر کیلوگرم. تزریق داخل صفاقی روزانه به مدت 56 روز انجام شد. در پایان آزمایش نمونه گیری خون برای ارزیابی بیوشیمیایی انجام گرفت. سپس موش ها کشته شدند و بیضه و اپیدیدیم چپ آنها به منظور آنالیز اسپرم و نیز ارزیابی های هیستوپاتولوژیکی و مورفومتری درآورده شد.
نتایجدیابت بطور معنی داری تعداد، تحرک، زنده مانی، بلوغ، و کیفیت کروماتین اسپرم را کاهش داد (001/0 < p). دیابت بطور قابل ملاحظه ای میزان MDA سرم را افزایش و میزان TAC سرم را در مقایسه با گروه کنترل کاهش داد (001/0 < p). اختلاف معنی داری در قطر لوله های منی ساز، ضخامت اپیتلیوم ژرمینال و تغییرات هیستوپاتولوژیکی بافت بیضه در موش های دیابتی در مقایسه با موش های گروه کنترل مشاهده شد (001/0 < p). درمان با متفورمین و سیلی مارین پارامترهای مذکور را بهبود بخشید و این بهبود در حیوانات تیمار شده با سیلی مارین بیشتر بود (001/0 < p).
نتیجه گیرینتایج این مطالعه نشان داد که در موش های دیابتی، متفورمین و سیلی مارین موجب بهبود پارامترهای اسپرم، قطر لوله های منی ساز، ضخامت اپیتلیوم ژرمینال و عوارض هیستوپاتولوژیکی بافت بیضه گردید و این بهبود در گروه درمان شده با سیلی مارین بیشتر بود. لذا به نظر می رسد که سیلی مارین یک نقش موثرتر از متفورمین در درمان ناباروری ایجاد شده توسط دیابت ایفاد می کند.

مداخلات روانشناختی ممکن است مشکلات باروری را کاهش دهد. خلق مثبت، سیالیت، معنا، رابطه مثبت و موفقیت (پرما) یک مداخله شناختی محسوب شده و زوج درمانی یکپارچه-رفتاری (IBCT) نیز یک مداخله رفتاری به شمار می آید. مداخلات روانشناختی مناسب در درمان ناباروری مهم هستند.
هدفهدف از این مطالعه، مقایسه اثر بخشی زوج درمانی مدل پرما و رفتاری تلفیقی بر مشکلات باروری زوجین تحت درمان ناباروری بود.
مواد و روش هامحتوای بسته مداخلات براساس مروری بر متون و نظرات متخصصین رشته های مرتبط تهیه شد. در این مطالعه کارآزمایی بالینی موازی، 42 زوج تحت درمان ناباروری به طور تصادفی در سه گروه (14 زن و 14 شوهر) تقسیم می شوند. گروه 1 مدل PERMA، گروه 2 مداخله IBCT و گروه سوم به عنوان گروه کنترل برنامه های آموزشی درمان ناباروری را دریافت خواهند کرد. متغیر پیامد اصلی مشکلات باروری می باشد که با پرسشنامه مشکلات باروری Fertility Problems Inventory در ابتدای مطالعه، اتمام مداخله هفته 5 و هفته 9 مطالعه بررسی خواهند شد. متغیر پیامد اصلی دیگر رضایت از روش مداخله است که در هفته های 5 و 9 ارزیابی می شود. متغیر پیامد ثانویه تست مثبت بارداری در هفته 12 سنجش خواهد شد. هر سه گروه مداخلات را به صورت ترکیبی از حضوری و مجازی (از طریق واتساپ) دریافت می کنند.
نتیجه گیریمطالعه حاضر نگرانی های اجتماعی، ارتباطی، جنسی و والدی را در زوج مبتلا به ناباروری بررسی می کند. مداخله به روش ترکیبی حضوری و مجازی می تواند روش مناسبی در ارایه خدمات مشاوره ای در شیوع covid-19 باشد. مشاوره زوج در مقایسه با مشاوره گروهی می تواند نتایج بهتری در رفع مشکلات باروری ارایه دهد. این مطالعه سعی دارد تا از طریق یادگیری روابط بهتر و مهارت های حل مسیله، مشکلات باروری را در زوجین تحت درمان ناباروری کاهش دهد و ممکن است به دلیل تاثیر بر این مشکلات بر میزان بارداری و هزینه های درمان تاثیر غیر مستقیم داشته باشد.

The umbilical cord-derived mesenchymal stem cells conditioned medium (UC-MSCs-CM) produces secretomes with anti-apoptotic properties, and has the potential to prevent apoptosis of granulosa cells (GC) during controlled ovarian hyperstimulation.
ObjectiveTo observe the effect of UC-MSCs-CM on the interaction between pro- and anti-apoptotic proteins and the influence of growth differentiation factor 9 (GDF9) production in GC.
Materials and MethodsUC-MSCs-CM was collected from umbilical cord stem cell culture on passage 4. GCs from 23 women who underwent in vitro fertilization were cultured and exposed to UC-MSCs-CM for 24 hr. Then RNA of the GC was extracted and the mRNA expression of BCL-2 associated X (BAX), survivin and GDF9 were analysed using quantitative real-time PCR. The spent culture media of the GC were collected for measurement of insulin growth factor 1 using ELISA.
ResultsThe expression of BAX was significantly different after UC-MSCs-CM exposure (4.09E-7 vs. 3.74E-7, p = 0.02). No significant changes occurred in survivin, BAX/survivin ratio, and GDF9 expression after UC-MSCs-CM exposure (p > 0.05). The IGF-1 level of the CM was significantly higher after the CM was used as a culture medium for GC (2.28 vs. 3.07 ± 1.72, p ≤ 0.001). A significant positive correlation was found between survivin and GDF9 (r = 0.966, p ≤ 0.001).
ConclusionIGF-1 produced by UC-MSCs-CM can work in paracrine fashion through the IGF receptor, which can inhibit BAX and maintain GDF9 production. Moreover, under the influence of UC-MSCs-CM, GC are also capable of producing IGF-1, which can impact GC through autocrine processes.

Titanium dioxide nanoparticles (TiO2NPs) are widely used in many compounds. Recent evidence has displayed some cytotoxic effects of TiO2NPs on male reproduction.
ObjectiveThe effects of TiO2NP administration on sperm parameters and chromatin and seminiferous histopathology of male mice were investigated.
Materials and MethodsIn this experimental study, 32 NMRI male mice (35 ± 3 gr, 8-12-week-old) were divided into four groups (n = 8/each): treated groups were fed orally with 2.5 (group I), 5 (group II) and 10 (group III) mg/kg/day TiO2NPs for 40 days and the control group received phosphate buffered saline. Sperm parameters, DNA integrity and chromatin quality were assessed using chromomycin A3, aniline blue, toluidine blue staining and TUNEL. Hematoxylin eosin staining was performed to measure spermatogenic cells and the total diameter of seminiferous tubules. Also, sex hormone and malondyaldehyde levels were measured.
ResultsAbnormal sperm tails rose in group III (28.87 ± 4.91) in comparison with the control group (12.75 ± 3.95). However, chromomycin A3 staining and TUNEL showed higher levels in group III in comparison with the control group, whereas aniline blue and toluidine blue staining showed no differences. A significantly lower spermatogenesis index and lumen parameters were observed in group III. Leydig cell numbers, cellular diameters and the area of the seminiferous tubules were lower in the treated groups. The testosterone level was also lower in these groups and the percentage of malondyaldehyde in the seminal fluid was higher.
ConclusionExact mechanisms of TiO2NPs are not clear; however, cytotoxic and genotoxic effects of TiO2NPs may relate to oxidative stress. Given their widespread use, TiO2NPs should be a public health focus of attention.

Failed oocyte activation following intracytoplasmic sperm injection (ICSI) as a result of calcium deficiency is a major challenge.
ObjectiveWe compared the effect of cult-active medium (CAM) on ICSI outcomes in obstructive azoospermia cases.
Materials and MethodsThe present study was conducted with 152 ICSI cases, classified into CAM and control groups. The injected oocytes in the control group were cultured in the cleavage medium, while in the artificial oocyte activation group, oocytes were chemically activated through exposure to 200 µL of CAM for 15 min. Fertilization and cleavage rates, quality of embryos, and biochemical pregnancy and live birth rates were assessed in both groups.
ResultsThere were significant differences between the groups in terms of fertilization and cleavage rates after using the CAM in the percutaneous epididymal sperm aspiration (PESA) subgroup (p = 0.05, p ≤ 0.001) and in the testicular sperm extraction subgroup (p = 0.02, p = 0.04), compared to their control groups. Also, the pregnancy rate was significantly higher in the PESA-CAM subgroup (p = 0.03). The PESA-CAM subgroup demonstrated a significant difference in embryo quality after ICSI (p = 0.04). Unsuccessful embryo transfer and abortion were lower in both subgroups compared to the control groups, but this difference was not significant. Surprisingly, live birth rate was higher in the PESA-CAM subgroup (p = 0.03).
ConclusionCAM treatment could improve fertilization and cleavage rates in obstructive azoospermia participants. It had a significant effect on embryo quality, and pregnancy and live birth rates in PESA cases.

Key reactions in folate-mediated single-carbon metabolism are regulated by folate cycle enzymes. Violations of the folate cycle may be associated with the occurrence of fetal growth restriction (FGR) in pregnant women.
ObjectiveTo study the relationship between polymorphisms of folate cycle genes in the mother with the development of FGR.
Materials and MethodsIn this case-control study, 122 pregnant women with FGR and 243 pregnant women with normal newborn weight were enrolled. The polymorphic loci of folate cycle genes including rs1805087 5-methylenetetrahydrofolate (MTR) and rs1979277 serine hydroxymethyl transferase (SHMT1) were examined. The study of polymorphisms was carried out through the TaqMan probe detection method using polymerase chain reaction. Logistic regression was used to analyze the associations of the polymorphisms.
ResultsIt was established that the T allele rs1979277 of the SHMT1 gene was correlated with the development of FGR within the framework of the allelic (OR = 1.67, 95% CI 1.20-2.33, pperm < 0.01), additive (OR = 1.69, 95% CI 1.20-2.37, pperm < 0.01), dominant (OR = 1.81, 95% CI 1.15-2.87, pperm = 0.01) and recessive (OR = 2.34, 95% CI 1.15-4.73, pperm = 0.01) models. The association of the G rs1805087 allele of the MTR gene with the occurrence of FGR was also identified following the recessive model (OR = 3.01, 95% CI 1.05-8.68, pperm = 0.04).
ConclusionOur results indicated that maternal polymorphic loci rs1979277 SHMT1 and rs1805087 MTR may be associated with the development of FGR.

Pregnancy through assisted reproductive technology (ART) is a stressful experience that may affect prenatal attachment. However, maternal-fetal attachment (MFA) and anxiety in pregnancy after ART are understudied in Iran.
ObjectiveTo compare changes in MFA and pregnancy-related anxiety (PRA) in the first and third trimester of pregnancy in women who conceived through ART compared to those who conceived naturally.
Materials and MethodsThis longitudinal study was conducted in 2019 with 187 pregnant women (ART conception = 43, natural conception = 144). Participants were recruited using the consecutive sampling method from a prenatal clinic in Tehran. The Cranley MFA Scale and the Van Den Bergh PRA Questionnaire were used to collect the data.
ResultsThe MFA score in the 12th wk of gestation was lower in the women who conceived with ART compared to in the women who conceived naturally, but there were no statistically significant difference between the groups in wk 36. MFA in both groups was significantly higher at gestational wk 36 than wk 12 (p ≤ 0.001). The increase in MFA score was significantly higher in the women who conceived with ART than in those who conceived naturally (p ≤ 0.001). The anxiety score declined in the two groups and no statistically significant difference was observed in the changes of anxiety scores between the two groups (p = 0.84).
ConclusionPregnant women who conceived through ART were as attached to their fetus in the third trimester of pregnancy as other women and did not experience more PRA.

Hyperprolactinemia is one of the most common causes of infertility in women. The prevalence of pituitary tumors is 25-30% among infertile participants with hyperprolactinemia.
ObjectiveThe aim of this study was to describe the causes of hyperprolactinemia in infertile women referred to Yazd Infertility Center.
Materials and MethodsThis cross-sectional study was conducted with 182 infertile women with hyperprolactinemia who were referred to Yazd Infertility Center from February 2018 to October 2019. Serum prolactin was assessed by the human prolactin ELISA kit according to the Padtan Gostar Isar protocol. The age, duration of infertility, causes of hyperprolactinemia, and type of infertility treatment were noted. The MRI findings were added.
ResultsThe mean age of participants was 28.9 ± 0.36 yr and the prolactin level was 76 ± 8.97 ng/ml. The etiology of hyperprolactinemia among the study participants was 35 participants (19.2%) with pituitary adenoma, 47 participants (25.8%) with polycystic ovary syndrome, 14 participants (7.7%) with pituitary adenoma and polycystic ovary syndrome, and 86 participants (47.3%) with idiopathic hyperprolactinemia. The results of this study showed that there was no statistically significant difference between the mean prolactin levels in participants with different causes of hyperprolactinemia (p = 0.31).
ConclusionIdiopathic hyperprolactinemia and polycystic ovary syndrome are the most common reasons for hyperprolactinemia.

Oxidative stress is a major contributor to diabetes, which can lead to testicular damage and infertility.
ObjectiveThis study aimed to compare the effects of metformin as a chemical drug with silymarin as an herbal agent on the sperm parameters and histopathological changes of testes in diabetic rats.
Materials and MethodsThirty-two male Wistar rats (250-270 gr) were randomly divided into four groups: 1) control; 2) diabetic; 3) diabetic+metformin 200 mg/kg; and 4) diabetic+silymarin 100 mg/kg. Daily injections were administered intraperitoneally for 56 days. At the end of the treatment, blood sampling was performed for biochemical assessment. Then, the rats were sacrificed and their left testis and epididymis were cut for sperm analysis as well as histopathology and morphometric evaluation.
ResultsDiabetes was associated with a reduced sperm count, motility, viability, maturity, and chromatin quality of sperm (p ≤ 0.001). It was also associated with a higher malondialdehide level and lower total antioxidant capacity level of serum in comparison with the control group (p ≤ 0.001). There was a significant difference in the seminiferous tubule diameter, germinal epithelium height, and testicular histopathological alterations in the diabetic rats compared with the control rats (p ≤ 0.001). Treatment with metformin and silymarin improved the above-mentioned parameters and this improvement was more substantial in silymarin-treated animals (p ≤ 0.001).
ConclusionIn diabetic rats, metformin and silymarin improved sperm parameters, sperm DNA integrity, seminiferous tubule diameter, germinal epithelium thickness, and testicular histopathological complications; this improvement was more substantial in the silymarin-treated group. So, the findings of this study suggest that silymarin is more effective than metformin in treating diabetic-induced infertility.

Psychological interventions may reduce fertility problems. Positive emotion, engagement, meaning, positive relationship and accomplishment (PERMA( is a cognitive intervention and integrative-behavioral couple therapy (IBCT) is a behavioral intervention. Appropriate mental interventions are important in infertility treatment.
ObjectiveTo investigate the effect of the PERMA model vs. IBCT in reducing the fertility problems of couples.
Materials and MethodsThe content of the interventions was developed and adjusted based on a literature review and the opinions of experts. In this three-arm parallel randomized clinical trial, 42 couples undergoing infertility treatment will be allocated randomly into three groups (n = 14 couples/each). Group 1 will receive the PERMA intervention, group 2 will receive the IBCT intervention, and group 3 as the control group will receive an infertility treatment training program intervention. The primary outcome will be the fertility problems, which will be measured by completing a fertility problem inventory at baseline, as well as in the 5th and 9th wk as a follow-up. Another primary outcome, satisfaction with the intervention, will also be assessed in the 5th and 9th wk. The secondry outcome will be a positive pregnancy test at wk 12. The interventions will be conducted through a combination of face-to-face and online via WhatsApp.
ConclusionThis study will assess social, sexual, and parenthood concerns. A combination of online and face-to-face interventions will be appropriate given the COVID-19 pandemic. Couple’s counseling may provide better counseling outcomes for fertility problems in comparison with group counseling. This study will try to optimize resilience during infertility treatment through learning better relationship and problem-solving skills, and may have an indirect impact on pregnancy rate, burden of infertility, and costs of treatment due to increased effectiveness.
