بیماری لیشمانیوز (سالک) که نوع جلدی آن شایع است، یکی از 10 بیماری مهم انگلی در دنیاست. ایران نیز در این زمینه یکی از 7 کانون درگیر در جهان بهشمار میرود. این مطالعه برای رسیدن به درمانهای موثر بر سالک جلدی در منابع طب سنتی ایرانی منتخب و فراهمآوردن پیشنهادهای درمانی مناسب هدفگذاری شد.
مواد و روشها:
در این مطالعه کتابخانهای، جهت اولویتبندی مفردات بهکاررفته در درمان لیشمانیوز جلدی در طب سنتی ایرانی، با کلیدواژههای «ریش بلخی، جوش بغدادی، حب بغدادی، بدریش، پشهگزیدگی، پشهخورد، سناخر، دانهسال و سالک» در کتب طب سنتی موجود در کتابخانه جامع طب سنتی و اسلامی (نرمافزار نور) و نیز برخی کتب خطی برجسته از جمله قانون، جستوجوهایی انجام گرفت.
یافته ها:
نتایج این مطالعه نشان داد مرهم اسفیداج مرکب از مرداسنگ، سفیداب قلعی، روغن گل سرخ، کافور و سرکه دارای بیشترین تواتر و امتیاز در میان درمانهای بهکاررفته در سالک در منابع طب سنتی ایران است.
نتیجه گیری:در این مطالعه مشخص شد در هیچیک از منابع طب سنتی مورد بررسی، درمان تک مفرده برای درمان سالک بهکار نرفته و در تمام موارد بیش از دو مفرده استفاده شده است. شاید سه ترکیب درمانی که در روش اولویتبندی، بیشترین امتیازها را بهدست آوردهاند، پیشنهاد مناسبی برای پروژههای تحقیقات آزمایشگاهی و بالینی در زمینه یافتن درمان طبیعی مناسب برای بلخیه یا لیشمانیوزیس جلدی (سالک) باشد.

مسمومیت غذایی ناشی از ادویههای توصیهشده در طب سنتی پیوسته در سراسر جهان اعلام میشود و از این طریق سلامت عمومی جامعه را بهخطر میاندازد. پرسش مهم در این حوزه این است که این محصولات تا چه اندازه دارای آلودگی میکروبی هستند و آیا از نظر مواد خوراکی در محدوده سلامت میباشند.
مواد و روشها:
در این تحقیق، راستیآزمایی نمونه، آلودگی میکروبی و میزان خلوص 12 نوع ادویه مختلف توصیهشده در طب سنتی ایرانی شامل 79 نمونه جمعآوریشده بهصورت کاملا تصادفی از نقاط مختلف شهر تهران در سال 1395، به روشهای خردهنگاری و حساسیت آنتیبیوتیکی انجام شد که با بهکارگیری 8 نوع آنتیبیوتیک به شکل دیسک و با استفاده از جدول استاندارد NCCLS (National Committee for Clinical Laboratory Standards) صورت گرفت.
یافتهها:
نتایج شمارش تعداد کل باکتریها نشان داد که 26 درصد ادویههای بستهبندیشده و 35 درصد نمونه های فله ای، آلودگی بالاتر از حد مجاز داشتند و نمونه های آلوده اغلب شامل باکتریهای مزوفیل هوازی کوکسی گرم مثبت و باسیل گرم مثبت و مخمر و کپک بودند. همچنین تستهای تشخیصی و بیوشیمیایی تایید کرد که نمونهها به باسیلوس آنتراسیس و باسیلوس سریوس آلوده نبودند. دو مورد آلودگی مربوط به آسپرژیلوس نایجر در ادویه های آویشن و زیره سیاه و 10 مورد آلودگی مربوط به ریزوپوس در ادویه های زیره سبز، پودر سیر و زردچوبه مشاهده شد. نتایج آنتیبیوگرام از نمونه هایی که آلودگی بالاتر از حد مجاز داشتند نشان داد که 12 درصد باکتریها به تتراسایکلین، 94 درصد به پنیسیلین، 24 درصد به ونکومایسین، 12 درصد به جنتامایسین، 82 درصد به سفکسیم، 6 درصد به ایمیپنم، 35 درصد به سیپروفلوکساسین و 30 درصد به سفازولین مقاومت نشان دادند.
نتیجهگیری:بررسی های انجامشده نشان دادند با وجودی که نمونه ادویههای بررسیشده کاملا خالص بودند، درصد قابلتوجهی آلودگی میکروبی داشتند که این نتایج جهت انجام مطالعات بیشتر قابلپیگیری هستند.

سلامت و بیماری از مهمترین مفاهیم پایهای پزشکی هستند که تعریف ماهیت و حدود آنها در فلسفه پزشکی با چالشهایی جدی روبهرو است. هدف این مقاله تبیین دیدگاه ابنسینا در خصوص سلامت و بیماری و تحلیل تعاریف امروزی این مفاهیم بر مبنای دیدگاه ابنسینا است.
مواد و روشها:
این مطالعه با رویکرد قیاسی در روش نظریهپردازی دادهبنیاد انجام شد. ابتدا مفاهیم سلامت و بیماری با کلیدواژههای متناسب در متون ابنسینا و طب رایج جستوجو و بررسی شد سپس این مفاهیم بهعنوان قضایای اولیه با مفاهیم عینیتر موجود مرتبط و در چارچوب استدلال قیاسی گزارههای عملیاتی استنتاج شد.
یافتهها:
ابنسینا سلامت را عرضی از جنس کیف نفسانی دانسته و آن را ملکه یا حالی تعریف میکند که مبدا افعال سلیم از موضوع خود است. او همچنین بیماری را عدم سلامت تعریف کرده و فرض حالت سوم بین آن دو را فاقد ضرورت منطقی میداند. سازمان جهانی بهداشت سلامت را آسایش کامل جسمانی، روانی و اجتماعی میداند نه صرفا فقدان بیماری. چنین تعریفی مستلزم فرض حالت سومی بین سلامت و بیماری است. براساس نظریات ابنسینا، فرض حالت سوم به معنای وجودی دانستن هر دو مفهوم سلامت و بیماری است و این امر، دسته قابلتوجهی از اختلالات افعال را که در قالب شناختهشده بیماری یا علامت بیماری تعریف نمیشوند از محدوده توجه پزشک کنار میگذارد؛ درحالیکه بر مبنای تعریف ابنسینا، بیماری امری عدمی (به معنای عدم سلامت) است و هر سطح از اختلال در هر عملکردی، در حقیقت سطحی از بیماری تلقی میشود.
نتیجهگیری:بر مبنای چارچوب نظری ابنسینا، نقطه قوت تعریف سازمان جهانی بهداشت از سلامت، محدودنمودن مرزهای مفهوم سلامت به وضعیت بهینه در ابعاد مختلف و نقطه ضعف آن عدم ظرافت کافی برای پوششدادن همه وضعیتهای غیرطبیعی در کلیه افعال، تحت مفهوم وسیعتری از بیماری محسوب میشود.

خوانش پارادایمیک از تاریخ پزشکی این امکان را فراهم میآورد که بهدنبال بحرانهای رخداده در تاریخ پزشکی و انقلابهای علمی اتفاقافتاده، سیر تحول مفاهیم پایهای را ذیل پارادایمهای گوناگون فهم کنیم. مفاهیم بنیادین در انقلابهای علمی دچار تحول اساسی میشوند وگاهی ممکن است، تصویر رایج از پدیدهها به کلی به نحوی گشتالتی تغییر کنند. در تاریخ پزشکی و با برآمدن طب مدرن در قرون اخیر، مفاهیمی پایهای نظیر علیت و تبیین، بدن و بدنمندی، بیماری و سلامت دستخوش تغییرات اساسی شدند. هدف از این مطالعه، ارایه تصویری فلسفی از ناهممقیاسی مفهومی در مفهوم علیت، با تاکید بر مثالهایی از تعریف سلامت و بیماری در پارادایمهای مختلف پزشکی است.
مواد و روش ها:
در این نوشتار ضمن معرفی و بحث مختصر در باب ناهممقیاسی مفهومی موجود در مفهوم علیت، درباره برخی مدلهای تبیین در انقلاب علمی طب نیز بحث میکنیم. برای این کار از روش تحلیل فلسفی بهره بردهایم تا مبانی فلسفی پارادایمهای طب را مورد تدقیق قرار دهیم. به مدد جستار در تاریخ پزشکی، نمونههایی از فهم علیت و کارکرد آن را ترسیم خواهیم کرد. همچنین با رویکردی تطبیقی و ضمن ارایه مثالهایی از متون هر دو پارادایم، دامنه این بحث را گستردهتر خواهیم کرد.
یافته ها:
مدعای محوری این متن که مبتنی بر پایبندی به اصل ناهممقیاسی مفاهیم در پارادایمهای یک علم شکل گرفته است، عدم امکان وضع مقیاسی واحد برای درک همزمان مفهوم علیت در طبهای سنتی-ایرانی و پزشکی امروز یا همان پزشکی مبتنی بر شواهد است.
نتیجه گیری:این نوشتار ضمن تاکید بر استقلال جنبههای نظری و ابعاد کاربستی پارادایمهای پزشکی، گفتوگو و حتی مقایسه روشمند میان پارادایمها را ممکن و بلکه ضروری میداند؛ اما تاکید دارد که این مقایسه به معنای مفاهمه همزمان هر دو پارادایم نخواهد بود و اجتناب از خلط روششناختی در این تلاش ضروری است.

اضطراب و اختلال خواب از جمله عوارض شایع دوران بارداری است که میتواند بستر مناسبی را برای بسیاری از اختلالات سلامتی در این دوران ایجاد کند. رایحهدرمانی با گل سرخ یکی از درمانهای طب سنتی است که در تسکین درد، کاهش اضطراب، بیخوابی و بهداشت دوره بارداری موثر اعلام شده است و فاقد هر گونه عوارض ناخوشایند گزارششده میباشد که میتواند در کنار درمانهای استاندارد جهت مدیریت مشکلات دوران بارداری بهکار رود.
مواد و روشها:
این مطالعه کارآزمایی بالینی با طرح پیشآزمون - پسآزمون بود که با استفاده از روش نمونهگیری خوشهای تصادفی، تعداد 124 زن باردار مراجعهکننده به 9 مرکز سلامت جامعه شهری ابهر زنجان انتخاب شدند و در گروه آزمون، زنان یک پارچه نخی آغشته با 3 قطره عصاره گل سرخ را هر شب هنگام خواب به مدت 2 هفته در سهماهه سوم بارداری در کنار بالش قرار دادند. برای سنجش اضطراب از پرسشنامه اشپیل برگر (Spiel Berger)و برای سنجش کیفیت خواب از پرسشنامه کیفیت خواب پیتزبورگ (Pittsburgh Sleep Quality Questionnaire) استفاده شد و دادهها با استفاده از نرمافزار و آزمونهای مناسب تحلیل شد.
یافتهها:
میانگین نمرات اضطراب آشکار و پنهان در گروه آزمون با مداخله نسبت به گروه کنترل، کاهش معنیداری یافت و میانگین تغییرات نمرات اضطراب آشکار و پنهان پس از مداخله رایحهدرمانی در دو گروه کنترل و آزمون اختلاف معنیداری با هم داشتند(0001/0>p). همچنین تغییرات نمره کیفیت خواب در دو گروه آزمون و کنترل یکسان نبود، بلکه کیفیت خواب گروه آزمون بهتر بود (0001/0>p).
نتیجهگیری:این مطالعه نشان داد که رایحهدرمانی با عصاره گل سرخ بر کاهش اضطراب آشکار و پنهان و بهبود کیفیت خواب زنان در سهماهه سوم بارداری تاثیر مثبت داشت و این رایحه در دسترس، ایمن و مورد پذیرش زنان باردار و خانوادههایشان بود.

علم اتنوبوتانی به تحقیق و بررسی چگونگی بهکارگیری مردم یک قوم، فرهنگ یا منطقه خاص از گیاهان بومی موجود در آن منطقه میپردازد. با توجه به اینکه روشها و موارد مصرف گیاهان دارویی در مناطق مختلف متفاوت است، پژوهش حاضر با هدف آشنایی با فرهنگ استفاده سنتی از گیاهان دارویی منطقه بلوچستان جهت درمان بیماریها انجام شد.
مواد و روشها:
به این منظور پس از شناسایی گیاهان منطقه بهطور همزمان نام محلی، خواص دارویی، نحوه استفاده و سایر اطلاعات مربوط به گونهها گردآوری شد.
یافتهها:
در این مطالعه 20 گونه گیاهی متعلق به 18 خانواده بررسی شد. بیشترین کاربرد دارویی گونههای مورد بررسی در این منطقه در درمان بیماریهای گوارشی (8 گونه، 6/29 درصد)، مسکن و درمان کوفتگی (6 گونه، 2/22 درصد) و کاهش قند خون و چربی خون (5 گونه، 5/18 درصد) و کمترین آن بیماریهای چشم (1 گونه، 7/3 درصد) میباشد. بررسی نوع مصرف گیاهان بیانگر این نکته بود که بیشترین نوع مصرف بهصورت جوشانده و ضماد است.
نتیجهگیری:
با توجه به کثرت گیاهان دارویی در این منطقه و استفاده وسیع مردم بومی از آنها جهت درمان بسیاری از بیماریها میتوان با مطالعات بیشتر بهویژه در دستهبندی بیماریها به نتایج راهگشایی در زمینه کاربرد گیاهان دارویی در درمان بیماریها دست یافت.

Background and Purpose:
The most common form of leishmaniosis, Cutaneous Leishmaniosis (CL) is one of the ten most important parasitic diseases globally, with Iran being one of the seven endemic areas in the world. This study aimed to investigate effective treatments for Balkhieh in selected Traditional Persian Medicine (TPM) texts and to propose appropriate treatment suggestions for research.
Materials and MethodsA library-based study was performed for prioritization of appropriate herbal treatments for cutaneous leishmaniosis in TPM. Queried terms in traditional medicine books available in the comprehensive library of traditional and Islamic medicine (Noor software) included “Rish-e-balkhi, Josh-e- Baghdadi, Habbe-e Baghdadi, mosquito bite, Sanakhar, Saldane (leishmaniosis) and Leishmaniosis”. Moreover, some prominent manuscripts including the Al-Qanun fi al-Tibb were also searched.
ResultsThe current study revealed that “Marham-e Esfidaj” had the highest frequency and score consisting of Mordasang, Tinny Whitewash, Oil of Rosa damascena, Cinnamomum camphor and vinegar.
ConclusionBased on the data presented herein, none of the reviewed traditional medicine sources recommended single-herb therapy for Balkhieh, and all treatments had at least two components. Maybe, three component combinations that scored the highest based on the prioritization method could be a good suggestion for laboratory and clinical research projects in the field of finding appropriate herbal treatments for Balkhieh or cutaneous leishmaniosis.

Background and Purpose:
Food poisoning with spices recommended by traditional medicine is reported worldwide, and may endanger public health. Hence, the extent of microbial contamination of spices requires to be addressed.
Materials and MethodsIn this study, microbial contamination and purity of twelve Persian Medicine (PM) spices was evaluated in 79 random samples in Tehran in 2016 using powder microscopy and antibiotic sensitivity assessments. Eight disc-shaped types of antibiotics were used for antibiotic sensitivity evaluation using the National Committee for Clinical Laboratory Standards (NCCLS) table.
ResultsThe results showed that total bacteria count was higher than permitted levels in 26% of packaged and 35% of bulk spice samples. Contaminated samples mostly included aerobic mesophilic bacteria, Gram-positive cocci, and Gram-positive bacilli, yeast, and mold. Moreover, diagnostic and biochemical tests confirmed that the samples were not infected with Bacillus anthracis and Bacillus cereus. Two cases of Aspergillus niger contamination were observed in thyme and caraway spices, while ten cases of rhizopus contamination were seen in cumin, garlic powder and turmeric. In antibiogram results of samples with higher-than-permitted contamination, 12% of the bacteria showed resistance to tetracycline, 94% to penicillin, 24% to vancomycin, 12% to gentamicin, 82% to cefixime, 6% to ampicillin, 35% to ciprofloxacin, and 30% to cefazolin.
ConclusionThe data showed that the spice samples were completely pure but they had a significant percentage of microbial contamination. The results of this study could be followed for further studies.

Background and Purpose:
Health and disease are among the most important basic concepts of medicine. However, defining their nature and limits, has faced serious challenges in medical philosophy. The purpose of this article is to explain Avicenna's point of view about health and disease and analyze the current definitions of these concepts based on Avicenna's view.
Materials and MethodsThis study was performed by a deductive approach in grounded theory method. Initially, the concepts of health and disease were studied in conventional medicine and books written by Avicenna. These concepts were then used as initial propositions in a deductive argument to create operational propositions.
ResultsAvicenna considers health as an accident and one of the qualities of the soul. He defines it as a habit or disposition that is the cause of normal functions in its subject. He also describes disease as a non-healthy condition and considers the assumption of a third state between the two to be unnecessary. The World Health Organization (WHO) defines health to be complete physical, mental, and social well-being, not merely the absence of disease. Such a definition requires the assumption of a third state between health and disease. According to Avicenna, this means to consider both concepts as existential issues; and eventually excludes a large number of dysfunctions -which are not categorized as known diseases or disease symptoms- from the physician's attention. Avicenna accounts disease as a matter of non-existence and considers any level of dysfunction in any function as a level of disease.
ConclusionBased on the Avicennian theoretical framework, limiting the health concept boundaries to an optimal state in various dimensions is the strength point of the WHO definition of health; while its weakness is the lack of elegance to cover all subnormal conditions in all functions, under a broader concept of disease.

Background and Purpose:
A paradigmatic reading of medical history results in comprehending the evolution of main concepts based on different paradigms after provoked crises and scientific revolutions in the history of medicine. Basic concepts may fundamentally change in scientific revolutions, i.e., the routine image of phenomena changes in a Gestalt manner. According to modern medicine in the recent century, basic concepts, including causation and explanation, body and physicality, and disease and health have fundamentally changed in the history of medicine. The current study aimed to present and discuss a philosophical image of conceptual incommensurability in the causality concept for emphasizing examples of defining health and disease in different medical paradigms and some models in the scientific revolution of medicine.
Materials and MethodsA philosophical analysis was applied to examine the philosophical foundations of medical paradigms. Searching in medical history helps perceive examples of understanding causation and its function. Moreover, both paradigms are discussed in more detail based on a comparative approach with examples.
ResultsAccording to the incommensurability concepts in the science paradigms, simultaneous understanding of the causality concept is considered impossible in traditional-Iranian and modern medicine, i.e., evidence-based medicine.
ConclusionThis study emphasizes the independence of theoretical aspects and practical dimensions of medical paradigms and also deems dialogue and even methodical comparison between paradigms possible, and even necessary; however, this comparison is not a simultaneous communication between both paradigms and demands avoiding methodological confusion.

Background and Purpose:
Anxiety and sleep disorders are among the most common complications of pregnancy that can provide a breeding ground for many health disorders during this period. Rose aromatherapy is a traditional medicine treatment modality that has been declared effective in relieving pain, reducing anxiety, eliminating insomnia and promoting health during pregnancy. It does not have any reported unpleasant side effects and can therefore be used along with standard treatments to manage pregnancy problems.
Materials and MethodsThis study was a clinical trial with pre-test-post-test design. Using random cluster sampling method, 124 pregnant women referring to 9 health centers of Abhar urban community in Zanjan were selected. A cotton cloth with 3 drops of rose extract was placed next to the pillow every night while sleeping for 2 weeks in the third trimester of pregnancy. The Spiel Berger Questionnaire was used to measure anxiety and the Pittsburgh Sleep Quality Questionnaire was used to measure sleep quality. Data were analyzed using appropriate software and tests.
ResultsThe mean scores of overt and covert anxiety in the experimental group decreased significantly with intervention compared with the control group and the mean changes in the scores of overt and covert anxiety after aromatherapy intervention in the control and experimental groups were significantly different (p<0.0001). Moreover, changes in sleep quality score were not the same in the experimental and control groups, with the experimental group having higher quality sleep (p<0.0001).
ConclusionThis study showed that aromatherapy with rose extract has a positive effect on reducing overt and covert anxiety and improving the quality of sleep in women in the third trimester of pregnancy. This fragrance is available, safe and well-accepted by pregnant women and their families.

Background and Purpose:
Ethnobotany is the study of use of native plants in a particular ethnic group, culture, or region. Due to the fact that the methods and uses of medicinal plants vary in different regions, the aim of present study was to introduce the culture of traditional use of medicinal plants to treat diseases in Baluchestan region.
Materials and MethodsFollowing identification of the plants of the region, the local name, medicinal properties, method of use and other information related to the species were collected simultaneously.
ResultsIn this study, 20 plant species belonging to 18 families were investigated. The most common use of medicinal species in this region were for gastrointestinal diseases (8 species, 29.6%), analgesia and treatment of contusions (6 species, 22.2%), and reduction of blood sugar and blood lipids (5 species, 18.5%), while the least belonged to eye diseases (1 species, 3.7%). Study of the type of consumption indicated that herbs were most commonly used as decoctions and poultices.
ConclusionDue to the richness of medicinal plants in Baluchestan and the widespread use of indigenous plants for treatment of many diseases, it is possible to reach new insights in application of medicinal plants by further studies.
