بیماری سلیاک یک بیماری سوءجذبی مزمن روده باریک ناشی از حساسیت به گلوتن محسوب می شود که می تواند سایر ارگان ها ازجمله کبد، مغز و دستگاه تنفسی را در کنار سیستم گوارشی درگیر کند. هدف از این مطالعه بررسی حجم های ریوی در بیماران مبتلا به بیماری سلیاک در شهر کرمان بود.
روش بررسیاین مطالعه مقطعی از نوع توصیفی تحلیلی بود که جمعیت مورد مطالعه تمامی بیماران بزرگسال مبتا به سلیاک در مرکز ثبت سلیاک بیمارستان افضلی پورشهر کرمان وارد مطالعه شدند. در این مرحله قد و وزن افراد اندازه گیری شد و اطلاعات دموگرافیک آنان در یک برگ چک لیس ت ثب ت گرد ید. سپس بر اساس شرایط اس تاندارد از اف راد اسپیرومتری به عمل آم د و ثبت گردید. داده های به دست آمده وارد نرم افزار آماری SPSS نسخه 22 شد.
یافته هادر این مطالعه 58 بیمار مبتا به سلیاک با میانگین سن 02 / 18 ± 59 / 37 سال مورد بررسی قرار گرفتند. میانگین قد و وزن بیماران به ترتیب، 30 / 14 ± 22 / 157 سانتیمتر و 06 / 17 ± 09 / 57 کیلوگرم بود. نتایج مطالعه نشان داد که میانگین FVC در بیماران موردمطالعه 93 / 0± 15 / 3 لیتر، میانگین 2/68±0/76 FEV1 لیتر، میانگین نسبت FEV1 به 83/06±5/56 FVC درصد و میانگین 6/64±1/61 PEF لیتر بود.
نتیجه گیرینتایج مطالعه حاضر نشان داد که حجم های ریوی بیماران مبتلا به سلیاک در مقایسه با حجم های ریوی مورد انتظار برای سن، جنس و قد تفاوت دارد و نسبت به مقادیر پیش بینی شده کمتر است با این حال، مطالعات بیشتر برای پیگیری این بیماران و بررسی عملکرد تنفسی بیشتر توصیه می شود. .

واکسیناسیون هپاتیت ب یکی از ضرورت ها جهت پیشگیری از بروز عفونت با این ویروس و جلوگیری از بروز بیماری مزمن هپاتوسلوالر کارسینوما است. سنجش وضعیت ایمن سازی پس از دریافت واکسن و شناسایی افراد responder-non بویژه در میان کارکنان مراقبت های بهداشتی و پزشکان بدلیل ریسک باالتر تماس با بیماران مبتال و ترشحات بالینی آنها حایز اهمیت است.
روش بررسیدر این مطالعه طی یک بررسی جامع در میان کارکنان خدماتی، بهداشتی و درمانی بیمارستان کودکان مفید به بررسی وضعیت واکسیناسیون این افراد، سنجش تیتر آنتی بادی HBs-anti ،و ارتباط آن با شاخص های توده بدنی، مصرف سیگار، و داروهای مورد استفاده در افراد دارای بیماری های زمینه ای پرداخته شد. جهت انجام این مطالعه از کیت Italy; PRO.DIA, HBsAb بمنظور سنجش تیتر آنتی بادی استفاده گردید. آنالیز آماری توسط نرم افزار spss انجام گرفت.
یافته هانتایج این تحقیق نشان داد که تیتر مناسب آنتی بادی در 6.66 %این افراد وجود دارد. نتایج این مطالعه موید کاهش تیتر آنتی بادی تا سطح غیرایمن در میان کارکنان دارای سن بیشتر از 50 سال، افراد سیگاری، و کارکنان دارای شاخص توده بدنی کم یا بسیار زیاد، بود.
نتیجه گیریدر این بررسی کاهش میزان تیتر آنتی بادی ارتباطی را با بیماری های زمینه ای و داروهای مرتبط با آنها نشان نداد. وجود تیتر غیرایمن آنتی بادی در میان کارکنان جوان واکسینه شده، انجام مطالعات تکمیلی جهت شناسایی افراد responder-non را پس از دریافت یک دوز واکسن یادآور پیشنهاد کرد.

والدین کودکان مبتلا به اختلال طیف اوتیسم با چالش ها و مشکلات ویژه ای در ارتباط با اختلال فرزند خود رو به رو هستند که باعث می شود این والدین نسبت به والدین دیگر استرس فرزندپروری بالاتری را تجربه کنند، همچنین اختلالات روانشناختی نیز از مولفه های رایج در والدین دارای کودکان مبتلا به اختلال اوتیسم است. این پژوهش با هدف ارزیابی مشکلات روانشناختی و استرس والدین بر مشکلات گوارشی کودکان با اختلال طیف اوتیسم انجام شد.
روش بررسیپژوهش حاضر از نوع توصیفی- تحلیلی است که به روش همبستگی انجام شده است. در این پژوهش با توجه به موضوع مورد مطالعه، جامعه آماری پژوهش را کلیه والدین دارای کودک مبتلا به اختلال طیف اوتیسم مراجعه کننده به مراکز آموزشگاهی استثنایی شهر تشکیل دادند. در این پژوهش نمونه گیری به صورت نمونه در دسترس انجام شد و تعداد 30 نفر آزمودنی به صورت نمونه گیری در دسترس، انتخاب شدند. برای تجزیه و تحلیل داده های این پژوهش از SPSSنرم افزار نسخه 22 استفاده شد.
یافته هانتایج پژوهش نشان داد که استرس والدینی بر اختالالت گوارشی کودکان مبتلا به اوتیسم اثر معنی داری ندارد و نتایج بیانگر تاثیر مثبت و معنی دار اختلالات روانشناختی والدین بر شدت اختلالات گوارشی کودکان مبتلا به اوتیسم بود. نتایج پژوهش بیانگر تاثیر اختلالات روانشناختی والدین بود که نیازمند توجه به به آن به عنوان یک عامل موثر در فرآیند درمان این بیماری است.
نتیجه گیریعواملی ممکن است باعث شیوع مشکلات گوارشی در میان کودکان اوتیستیک از جمله تعداد مختلف باکتری های دستگاه گوارشی و الگوی نمود ژن و همچنین فیبر نامناسب غذایی که به عنوان یک عامل ضد یبوست طبیعی عمل می کند.

بیست و یکمین کنگره گوارش و کبد ایران از روز سه شنبه 25 لغایت جمعه 28 آبان ماه 1400 (16-19 نوامبر 2021)بصورت مجازی برگزارگردید. با توجه به باقی ماندن پاندمی کووید 19 و عدم صدور مجوز از سوی وزارت بهداشت جهت برگزاری کنگره و لزوم رعایت پروتوکل های بهداشتی، هییت مدیره انجمن متخصصین گوارش و کبد ایران برای دومین سال متوالی تصمیم گرفت، کنگره به صورت مجازی برگزار گردد. برخلاف سال گذشته این بار سعی شد از ظرفیت حداکثری دانشگاه های علوم پزشکی کشور استفاده شود، به همین منظور و جهت استفاده از حضور اساتید سراسر کشور برنامه کنگره، طی چهار روز متوالی و در دو سالن موازی با بیش از 175 سخنرانی و پنل و همچنین 170 سخنران برگزار شد. با توجه به پلتفرم های پخش موجود در ایران و پهنای باند اینترنت کشوری، برای برگزاری کنگره از سه پلتفرم پخش آنلاین ،Skyroom و Live Stream IAGH و برای اولین بار از پلتفرم zoom نیز استفاده شد .

Celiac disease is a common malabsorptive bowel disease caused by gluten sensitivity that affects other organs of the body in addition to the gastrointestinal tract, such as the liver, brain, and respiratory system. In this study, we aimed to evaluate the pulmonary volumes in patients with celiac disease in Kerman city.
Materials and MethodsThis cross-sectional descriptive study was performed on all adult patients with celiac disease in the Celiac Registry in Afzalipour Hospital. All patients whose records were complete were included in the study. At this stage, height and weight were measured, and their demographic information was recorded in a checklist. Then, according to the standard conditions, spirometry was performed. Data were analyzed using SPSS software version 22.
ResultsThe study cases comprised 58 patients with celiac disease. The average age of the patients was 37.59±18.02 years. The mean height was 157.22±14.30 cm and the mean weight was 57.09±17.06 kg. The results showed that the mean forced vital capacity (FVC) was 3.15±0.93 L, the mean forced expiratory volume in 1 second (FEV1) was 2.68±0.76 L, the mean ratio of FEV1 to FVC 83.06 ± 5.56%, and the mean of peak expiratory flow (PEF) was 6.64±1.61 L.
ConclusionThe results of this study indicated pulmonary volumes in celiac patients were lower than the normal age- and sex-matched population. Further studies to follow up and assess respiratory problems in these patients are needed.

Hepatitis B vaccination is necessary to prevent infection with this virus and to prevent the development of chronic hepatocellular carcinoma.
Materials and MethodsAssessing the immunization status after receiving the vaccine and identifying non-respondents, especially among health care workers and physicians, is important because of the higher risk of contact with infected patients and their body fluids. In this study, the vaccination status of health and medical staff of a children's hospital was investigated to measure the anti-HBs antibody titer and its relationship with body mass index, smoking, and drugs used in individuals with underlying diseases. In this study HBsAb kit, DIA.PRO; Italy was used to measure the antibody titers. Statistical analysis was done using SPSS software version 26.
ResultsThe results of this study showed that a proper antibody titer is present in 66.6% of the participants. Reduced antibody titers to unsafe levels were measured among the employees older than 50 years, smokers, and employees with low or very high body mass index.
ConclusionIn this study, the decrease in antibody titer did not show any association with underlying diseases and related medications. The presence of an unsafe antibody titer among young vaccinated staff suggested additional studies to identify non-responders after receiving a booster dose of the vaccine.

Helicobacter pylori (H. pylori) is a bacterium that causes gastritis, gastric ulcers, and cancer. A major problem in the treatment of H. pylori-infected patients is re-infection with other H. pylori strains, which suggests the probability of colonization of this bacteria in other tissues of the body. Adenotonsillar tissues as the entrance of the gastrointestinal system might be considered as a place for H. pylori colonization and a reservoir for re-infection. The aim of this study was to survey the co-incidence of colonization of H. pylori in hyperplastic adenotonsillar tissues and its antigen in children.
Materials and MethodsIn this cross-sectional study, 92 children candidates for adenotonsillar surgery in Ayatollah Rouhani Hospital of Babol in 2019 were enrolled. Before the surgery, stool samples were taken and tested with the rapid stool antigen kit for H. pylori. In the operating room, a sample with a size of two mm was separated from the adenotonsillar tissues and was placed in the rapid urease kit. Finally, all adenotonsillar tissues were sent for pathologic evaluation.
ResultsAccording to obtained results from the study, eight (8.5%) patients had a positive stool antigen for H. pylori, and a positive rapid urease test was reported in the samples of adenotonsillar tissues of six patients. Co-incidence of colonization of H. pylori in hyperplastic adenotonsillar tissues and its antigen were reported in two patients. All samples were confirmed by pathology as lymphoid hyperplasia.
ConclusionH. pylori can be colonized in adenotonsillar tissues, and there is a probability of its co-incidence with H. pylori stool antigen in children.

The parents of children with autism spectrum disorder have special challenges and problems with their children’s disorder, so they have more stress in parenting than other parents, also having psychological disorders is very common in these parents. The aim of this study is to evaluate the effect of psychological disorders and parental stress on children with autism spectrum disorder and digestive problems.
Materials and MethodsThis study was a descriptive-analytical one with a correlation method. Based on the purpose of the study, the statistical population was all parents with children with autism spectrum disorder who were referred to special need schools in Isfahan. 30 participants were chosen using the available sampling method. In this study, SPSS version 22 was used to analyze the data.
ResultsThe results showed that parental stress had no significant effect on gastrointestinal disorders in children with autism and the results showed a positive and significant effect of parental psychological disorders on the severity of gastrointestinal disorders in children with autism. The results showed the effect of psychological disorders of parents that need to be considered as an effective factor in the treatment process of this disease.
ConclusionFactors may contribute to gastrointestinal problems in autistic children, including a variety of gastrointestinal bacteria and gene expression patterns, as well as inadequate dietary fiber, which acts as a natural anti-constipation agent.

Although prescribing biological drugs have been common in Iran for nearly a decade and is now more widely available than ever, some physicians may still be unfamiliar with them. This guideline is the first joint statement of the Iranian Association of Gastroenterologists and Hepatologists (IAGH) regarding the proper use of biological drugs in IBD treatment. Many of the current guidelines are based on internationally published data due to a lack of national and regional information (individual and racial differences affecting patients' responses to biologics as well as characteristics). In this guide, we have tried to address various issues, including the indication for starting biological drugs, contraindications, prevention of complications, screening before starting biological treatment, and other follow-ups during treatment in Crohn's disease and ulcerative colitis. The authors of the article hope that based on these strategies, they will provide helpful information based on scientific evidence to physicians around the country to treat their patients with biological drugs successfully. As far as we know, this consensus on the starting and continuing of biologics is the first case to be compiled and published on the use of biological drugs in Iran, and it shows the importance of its application. However, the physicians’ decision about treating their patients is a priority. Naturally, it will be necessary to review the provisions of this agreement after the next 2-3 years.

Common bile duct (CBD) stone is the most common disorder in the biliary tract, which may lead to hospital admission. Opium addiction is one of the most challenging social and health issues in many societies. An important side effect of opium addiction is the Oddi’s sphincter spasm; i.e., it is likely that the prolonged stasis of bile acid increases the risk of gallstone formation in the CBD.
Materials and methodsThis cross-sectional study was done from April 2019 to November 2019 in Ghaem Hospital in Mashhad, Iran. 400 patients aged 20-80 years were enrolled. All patients had a definite diagnosis of CBD stones and were candidates for endoscopic retrograde cholangiopancreatography (ERCP). Demographic information of patients, history of underlying diseases, liver laboratory disorders, the existence of concomitant gallbladder stones, and opium addiction information were collected.
ResultsThe average age of the patients with opium addiction was 65.12 years, and it was significantly higher than the mean age of the patients (61.41 years) without opium addiction (P=0.01). 74.3% were hospitalized, and 25.3% were outpatients. 35.5% of the patients were men, and 64.5% were women. Our study showed that 41.5% of the patients with CBD stones were opiumaddicted. The average CBD stone size in patients with opium addiction (10.15±4.85 mm) was larger than in patients without opium addiction, but a t test showed that this difference was not significant (P = 0.9).
ConclusionIn this study of patients with CBD stones referred for ERCP, we observed a high prevalence of opium addiction in patients with CBD stones.

Irritable bowel syndrome (IBS) is the most common functional gastrointestinal disorder characterized by abdominal pain and changes in intestinal movements in the absence of structural or biochemical abnormalities. In spite of the high prevalence, its etiology is unknown, and there are no specific diagnostic laboratory tests.
Material and MethodsIn a case/control study, 36 biopsy samples taken from patients with IBS and 30 biopsy samples as control were obtained. Expression of Toll-like receptor 3 (TLR 3), Toll-like receptor 9 (TLR 9), and Retinoic acid-inducible gene I (RIG 1) in macrophages, lymphocytes, and plasma cells in terms of percentage and color intensity were evaluated by immunohistochemistry. The obtained data were statistically analyzed using Mann Whitney U and Chi-square tests.
ResultsThe differences between percentage and intensity in the sample and control groups were significant for all three receptors in the epithelial cells. In plasma cells, the difference in TLR 3 and TLR 9 percentage was significant but not significant for RIG. However, in terms of intensity, it was not significant for any of them. In lymphocytes and macrophages, there was no significant difference for any of the receptors.
ConclusionSince the expression of some virus sensing receptors on the different cell types of the intestine increases following infection, we concluded that IBS might be related to viral infections of the intestine. Future studies are needed to reveal the exact nature of the suspected viral agents. Some viruses producing gastroenteritis in humans, such as coronavirus, can be suspected.

Adrenal insufficiency (AI) can be presented with gastrointestinal symptoms such as nausea, vomiting, and abdominal pain. Wereport a case of acute AI presented as intestinalpseudo-obstruction. A 27-year-old man without any remarkable medical history and underlying diseases presented withnausea, vomiting, abdominal pain, anorexia, weight loss, and constipation since 2 months earlier. In studies performed on the patient, a diagnosis of small bowel obstruction with electrolyte disturbance was made.Finally, acute AI wasdiagnosed by small bowel obstruction, which was resolved by corticosteroid treatment.

Hepatitis A viral infection is a common illness across the globe, particularly among the pediatric population. In 30% of infected children below 6 years, it presents as symptomatic hepatitis. Epstein-Barr virus (EBV) infections are common in early childhood and cause infectious mononucleosis in young adolescents. Primary EBV infection presentation as isolated hepatitis without infectious mononucleosis (IM) syndrome is very rare. In tropical countries, co-infections are common. We report acute hepatitis due to dual infection of hepatitis A and EBV in an 8-year-old child.
