To study the safety and clinical outcomes of ReLEx SMILE with accelerated cross-linking in eyes with thin cornea.

    Materials and Methods

Fifty-four thin cornea with corrected distance visual acuity 20/25 or better, stable refraction of at least 1 year, age 21 years or older, and residual corneal thickness of greater than 400 mm before performing collagen crosslinking were studied. Following the removal of lenticule, 0.25% riboflavin in saline was injected into the interface and allowed to diffusefor 60 seconds. Finally, eye was exposed to UV-A radiation of 45mW/cm2 for 75 seconds through the cap. Total energy delivered was 3.4 J/cm2.

    Results

54 eyes of 27 patients with mean age of 25.22 ± 2.67 years were treated. Mean follow-up was 6 months. Mean spherical equivalent (SE) was −5.58 ± 1.22D preoperatively and -0.111 ± 0.636D postoperatively. The mean central corneal thickness (CCT) and keratometry changed from 498.39 ± 11.79 μm to 417.85 ± 12.82 μm and 45.47 ± 0.68 D to 41.13 ± 1.13D, respectively. Mean uncorrected visual acuity (UCVA) was 20/25 or better in all eyes. No eyes lost lines of corrected distant visual acuity (CDVA). There were no complications like haze, keratitis, ectasia, or regression.

    Conclusion

Based on the initial clinical outcome it appears that SMILE Xtra may be a safe. Although further follow-up and larger samples are needed to fully confirm these findings, the results suggest that combined small-incision lenticule extraction and intrastromal corneal collagen crosslinking (CXL) are a promising treatment option for patients for whom conventional laser refractive surgery is contraindicated.

Covid-19, which is now pandemic in countries around the world, including Iran. It has shown to cause hypoxia, which can lead to impaired wound healing. In positive covid-19 patients, the patient and his companion’s do not usually do appropriate washing of wounds. In addition, economic issues such as procurement and mask of personnel exposed to the patients with covid-19 disease are also an issue. In this article, Corona suspicious patient presented with a diabetic foot and required hospital admission. As such a patient with specific condition suspicion was required to admit, we needed to create a guideline for him and for upcoming similar cases. The patient was a 74-year-old diabetic man who did not respond to debridement. So the patient underwent below-kneeamputation, which ended up to above the knee amputation due to a Klebsiella infection and failure to respond to non-invasive treatment of wound infection of stump of below-knee amputation and the patient's condition. The question is whether Corona-related hypoxia has exposed the patient to nosocomial infection, or the patient's lack of self-control at home as well as the medical staff lack of care due to fear of corona was the reason? In the period of the patient was under treatment, there was no significant complaint of shortness of breath. Conclusion in patent with diabetic foot and covid-19 suspicion _ especially in critically ill patient may benefit from aggressive approach such as early major amputation because of cost-benefit and minimizing covid-19risk must consider

As coronavirus disease 2019 (COVID-19) death toll continues to surge around the globe, researchers are trying to reposition already-approved drugs for battling against sever acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Diet and nutritional status have long been acknowledged to be associated with certain diseases. Coenzyme Q10 (CoQ10), also known as ubiquinone, is a fat-soluble, vitamin-like substance which plays a pivotal role in mitochondrial bioenergy transfer. Furthermore, it is an anti-oxidant with superb free radical-scavenging activities (1). Emerging evidence also hints that CoQ10 possesses immunomodulatory and anti-inflammatory properties. Regarding the latter, CoQ10 has been observed to inhibit expression of nuclear factor-κB, interleukin-6 (IL-6), and tumour necrosis factor-α (2). In light of the foregoing, CoQ10 has been assessed in numerous studies for its potential in treating various health conditions and maladies such as neurodegenerative disorders, cardiovascular diseases, cancers, periodontitis, diabetes, renal failure, and acquired immunodeficiency syndrome, to cite just a few (1). There have been some attempts to assess potential associations between CoQ10 levels and symptom severity in patients who suffered from pulmonary infections (2-4). In a clinical trial, elderly hospitalized patients with community-acquired pneumonia who received oral CoQ10 (200 mg/d) as an adjunct to ceftriaxone plus azithromycin for 14 days exhibited improvement with defervescence and shorter length of hospital stay as compared to the placebo group (2). Likewise, another study revealed a significant correlation between serum levels of CoQ10 and chest radiographic findings of children with pneumonia caused by H1N1 influenza (3). The same authors also demonstrated that CoQ10 levels were remarkably lower in a pediatric population infected with H1N1 influenza in comparison to both controls and seasonal influenza patients (3). This finding was further substantiated by another investigation in which patients with acute influenza showed significantly lower levels of serum CoQ10 in comparison to healthy controls (P = 0.004), suggesting that diminished levels of CoQ10 may predispose individuals for acquiring viral respiratory diseases (4). Serum levels of CoQ10 in influenza patients were also inversely correlated with certain inflammatory markers (4). Interestingly, one study has reported the beneficial effects of CoQ10 supplementation (100 mg/d for 4 weeks) in asthma patients, which was evident by a significant enhancement in forced expiratory volume in 1 s (FEV1)/forced vital capacity (FVC) ratio (5). Furthermore, different nanosuspensions of CoQ10 for nebulization have been recently developed for pulmonary disorders (6), providing more effective drug medication delivery. A recent study on rats revealed that CoQ10 protects sepsis-induced acute lung injury (7). Of note, the levels of high-mobility group box 1, IL-6, macrophage inflammatory protein 2, and keratinocyte chemoattractant were significantly diminished in CoQ10 group compared with the untreated controls (P < 0.05). Similarly, administration of CoQ10 was shown to ameliorate lung and liver fibrosis in rats through modulation of autophagy in methotrexate treated rats (8). Patients suffered from COVID-19 have augmented levels of pro-inflammatory cytokines, increased risk of pneumonia, and acute respiratory distress syndrome. Owing to obvious anti-inflammatory and immunomodulatory properties of CoQ10, we envisage that the nutrient has the potential for adjuvant therapy against SARS-CoV-2 infection. On the other hand, there will be remarkable fibrotic consequences following the infection in some patients (9). Anti-fibrotic properties of CoQ10 may have a preventive role against pulmonary fibrosis secondary to COVID-19. Future clinical trials should scrutinize the therapeutic benefits of CoQ10 (in an inhaled form or oral administration) in critically ill patients.

    Background

Functional dyspepsia is characterized by a set of symptoms in the upper gastrointestinal tract due to an underlying organic cause and is one of the common causes of patients ’visits to gastroenterologists. A few researches have been conducted on the effectiveness of psychotherapy with respect to the psychological components of these patients. Therefore, the purpose of this study is to investigate the effect of metacognitive therapy on emotional processing components of these patients.

    Materials and Methods

In this study, 40 patients with dyspeptic symptoms were recruited after medical examination by gastroenterologists, performing H.Pylori test and undergoing endoscopy. Moreover, 20 of the patients were randomly assigned to a group for metacognitive therapy and another 20 to the control group. The Baker's emotional processing questionnaire was used at pre-treatment, post-treatment and three months after treatment for evaluating changes in patients' emotional processing styles. Finally, repeated measures analysis of variance was employed for data analysis by using SPSS 19.

    Results

The research data that was obtained by repeated measures analysis of variance indicated statistically significant differences in three emotional processing scales in metacognitive therapy group compared to the control group at pre-treatment, post-treatment and three months after treatment.

    Conclusion

Compared to the control group, the metacognitive therapy of functional dyspepsia patients was more effective in the short-and long-term for coping with difficulty in emotional recognition and experience, difficulty in emotional control and expression as well as inadequate emotional processing.

Diabetes and intermittent periods of fluctuations in blood sugar levels are associated with stress and negative emotions. Due to the fact that these patients are prone to emotional problems, the aim of this study was to evaluate the effectiveness of autogenic training and affect regulation training on anxiety sensitivity in women with type 2 diabetes.

    Materials and Methods

This was a quasi-experimental study with pre-test and post-test design and control group. The statistical population included all women with type 2 diabetes in Tehran who referred to diabetes control centers in 2020. Among them, 45 people were selected by available sampling and replaced in two experimental and one control groups. All three groups completed anxiety sensitivity questionnaire. Autogenic training and affectregulation training were performed on the experimental groups. Data were analyzed using SPSS software version 24 and multivariate analysis of variance.

    Results

The analysis of the results showed that both autogenic training and affect regulation trainingreduced anxiety sensitivity in women with type 1 diabetes (P<0.00), but the effectiveness of autogenic training was not significantly different from affect regulation training (P >0.001).

    Conclusion

Therefore, it can be said that affect regulation training by accepting and expressing the negative emotions, reduced anxiety sensitivity in women with type 2 diabetes. Also, people with high anxiety sensitivity are incapable of moment-to-moment awareness without judgment. Thus, through autogenic training, the skill of processing thoughts and observing emotion as objective events in the mind is strengthened.
