با توجه به مشارکت پایینتر از حد انتظار زنان در اهدای خون ترغیب آنان به این امر ضروری است. در همین راستا تدارک آموزشهای مناسب و اثرگذار نیازمند شناسایی عوامل موثر در اهدای خون زنان است. این مطالعه ارتباط سازههای نظریه رفتار برنامهریزی شده و اضطراب تزریق با قصد رفتار اهدای خون در دختران دانشجو دانشگاه علوم پزشکی اصفهان را مورد بررسی قرار داده است.
مواد و روشهااین مطالعه توصیفی- تحلیلی با مشارکت دانشجویان دختر دانشگاه علوم پزشکی اصفهان در نیمسال اول 1401 انجام شد. با روش نمونهگیری طبقهای چند مرحلهای سهمیهای، تعداد 623 نفر از دانشجویان انتخاب شدند و دادههای مرتبط از طریق پرسشنامه الکترونیکی جمعآوری و با نرمافزار 20 SPSS وآزمون های ضریب همبستگی پیرسون و رگرسیون خطی تحلیل شد.
یافتههانتایج نشان داد که تنها 114 نفر(3/18%) از دختران دانشجو، سابقه اهدای خون داشتند. بین سازههای کنترل رفتاری درک شده، هنجارهای ذهنی و نگرش با قصد رفتار اهدای خون رابطه مستقیم و بین اضطراب تزریق با قصد رفتار اهدای خون رابطه معکوس وجود داشت(001/0 ≥p).
نتیجه گیرینظریه رفتار برنامهریزی شده میتواند الگویی مناسب برای انجام مداخلات در جهت ترغیب بانوان به امر اهدای خون باشد، از طرفی استفاده از برنامههای آموزشی مختلف جهت کاهش اضطراب تزریق و همچنین درمان کم خونی زنان به جهت افزایش اهدای خون امری ضروری به نظر میرسد.

آلوایمیونیزاسیون به علت مواجهه لنفوسیتهای B با آنتیژن بیگانه به دنبال تزریق خون ناسازگار ایجاد میشود. یکی از راههای شناسایی آنتیژنهای گروههای خونی، استفاده از آنتیبادیهای مونوکلونال میباشد. هدف از مطالعه حاضر، ترانسفورماسیون سلولهای آلوایمن شده ضد آنتیژن Kell در افراد تالاسمی به کمک ویروس اپشتاین بار و تولید سلولهای لنفوبلاستوئید تولیدکننده آنتی بادی ضد Kell میباشد.
مواد و روشهادر یک مطالعه تجربی، سلولهای تک هستهای خون محیطی(PBMC) از 10بیمار تالاسمی ایمن شده علیه آنتیژن KELL به روش گرادیان غلظتی فایکول جدا و با سوپ ویروسی اپشتاین بار به دست آمده از محیط کشت خط سلولی B95-8 به مدت 4 ساعت مواجه شد. پس از گذشت یک تا سه هفته، تولید کلونهای لنفوبلاستوئیدی تولیدکننده آنتیبادی و اختصاصیت آنتیبادی تولیدی به ترتیب با مشاهده میکروسکوپی، الایزا و روش میکرو هماگلوتیناسیون بررسی شد.
یافتههانتایج به دست آمده نشان داد که مواجهه سلولهای PBMC بیماران تالاسمی ایمن شده علیه آنتیژن KELL با سوپ ویروسی EBV سبب ایجاد کلونهای لنفوبلاستوئیدی میگردد. بررسی سوپ سلولی کلونهای تولیدی نشان داد که آنتی بادی تولیدی توسط این سلولها قادر به شناسایی گلبولهای قرمز حاوی آنتیژن KELL و آگلوتیناسیون آنها با آزمایش میکروهماگلوتیناسیون میباشد.
نتیجه گیریبراساس یافتههای به دست آمده ترانسفورماسیون PBMC بیماران تالاسمی ایمن شده علیه آنتیژن KELL باعث ایجاد سلولهای لنفوبلاستوئیدی تولیدکننده آنتیبادی ضد این آنتیژن میگردد.

مدیریت خون بیمار، اجرای مفاهیم جراحی وپزشکی مبتنی بر شواهد به منظور دستیابی به پیامد بهتر بالینی بیمار است. اولین مرحله استقرار مدیریت خون بیمار، آموزش آن است. هدف از این تحقیق، بررسی دو روش آموزشی حضوری و مجازی جهت استقرار مدیریت خون بیمار بود.
مواد و روشهادر یک مطالعه آیندهنگر در سال 94 در بیمارستان شهید لواسانی، کارکنان بیمارستان به صورت تصادفی از سه گروه پزشکان، پرستاران وآزمایشگاه انتخاب شدند و وارد یکی از گروههای آموزش مجازی و حضوری شدند. تمرکز آموزشدهنده در هر دو روش آموزشی بر اصول و اساس مدیریت خون بیمار، جایگزینهای فرآوردههای خون و تزریق خون سختگیرانه بود. پس از اتمام دوره هر دوگروه در یک امتحان یکنواخت شرکت کردند. برای مقایسه از آزمون آنووا یک طرفه، کایاسکویر و تی زوجی و برای تجزیه و تحلیل از نرمافزار 23 SPSS استفاده شد.
یافتههامیانگین نمره افراد در دوره حضوری 1/10 ± 7/90 (100-64) و در دوره مجازی 6/10 ± 8/90 (100-58) به دست آمد. میانگین نمره افراد در دوره حضوری و مجازی تفاوت معناداری نداشت. میانگینهای مربوط به سابقه کار و سال های تحصیل افراد نیز اختلاف معناداری در دو گروه نشان نداد.
نتیجه گیریدانش کسب شده در شرکتکنندگان این مطالعه تفاوت معناداری بین دوگروه مجازی وحضوری نشان نداد. برای آموزش جهت مستقر نمودن "مدیریت خون بیمار" در بیمارستانهای سطح کشور میتوان از آموزشهای حضوری و مجازی بهره برد.

یکی از آنتیژنهای فرعی گروه خونی،Duffy میباشد که شامل زیرگروههای مختلف است. سیستم گروه خونی Duffy ، از نظر انتقال خون، از اهمیت بالینی برخوردار است، لذا شناسایی دقیق این آنتیژن اهمیت به سزایی دارد. دراین مطالعه، فرآیند محلولسازی غشای گلبول قرمز و خالصسازی آنتیژن بررسی شد.
مواد و روشهادر این مطالعه تجربی، کیسه گلبول قرمز متراکم با ویژگی O- و Fya+ از پایگاه انتقال خون استان تهران، تهیه شده و با به کارگیری دترژانت Nonidet P-40، اقدام به محلولسازی غشای آنها شد. در مرحله بعد، آنتیژن Fya با استفاده از روش کروماتوگرافی تمایلی و به کارگیری آنتیبادی اختصاصی این آنتیژن خالصسازی شد. ویژگی پروتیین خالص شده با روشELISA بررسی شد.
یافتههاویژگی Fya+ در گلبولهای قرمز کیسه RBC با روش آگلوتیناسیون تایید شد. به علاوه، ویژگی آنتیژن تخلیص شده از کیسه یاد شده، با روش ELISA تایید شد. جذب نوریOD 450 nm آنتیژن تخلیص شده در روش ELISA با آنتیبادیهای Fya و Fyb، به ترتیب 137/0 ± 056/1 و 016/0 ± 197/0 (2 n=) بود.
نتیجه گیریجهت دستیابی به آنتیژن Fya، محلولسازی غشای گلبول قرمز با استفاده از دترژانت غیر یونی Nonidet P-40، قابل انجام بوده و میتوان در مطالعههای بعدی از آنتیژن به دست آمده، در زمینههای مختلف از جمله ایمنیزایی و تولید آنتیبادی بهره برد.

سیستم مدیریت نگهداری پیشگیرانه، نقش موثری در پیشگیری نواقص به همراه افزایش ایمنی تجهیزات، کاربران و کاهش هزینهها در روند فعالیت سیستمها دارد. در این مطالعه، میزان بهرهدهی این سیستمها در 6 مرکز انتقال خون تهران، البرز، آمل، همدان[MF1] ، سمنان و قم طی ماههای اردیبهشت تا مهر سال 1400 بررسی شد.
مواد و روشهادر این مطالعه مقطعی، وضعیت مربوط به عوامل مرتبط با اجزا، اتصالات و قطعات 35 سیستم موتورخانه، تهویه و دیزل ژنراتور فعال در6 مرکز انتقال خون از طریق چک لیست، مصاحبه و مشاهده چشمی بررسی شد [MF2] و اطلاعات به دست آمده، با استفاده از نرم افزار 25 SPSS مورد تجزیه و تحلیل آماری قرار گرفت.
یافتههانتایج این مطالعه حاکی از عدم رعایت الزامات در سیستم مدیریت نگهداری ابزار و وسایل شبکه برقرسانی و تاسیسات 6 مرکز با سابقه بیش از10 سال به دلیل عدم وجود نیروهای با تجربه، محدودیتهای مالی و عدم نظارت در بازدیدهای دوره ای موثر، آزمایشهای ایمنی، اصلاحات مورد نیاز در بخش موتورخانه و شبکه برقرسانی بود. میانگین وضعیت عدم نظافت اجزاء در این مراکز 61%، رسوب و خوردگی اتصالات 33%، زنگ زدگی 25% و بهمریختگی و انباشتگی سیم و کابل 67% بود[MF3] .
نتیجه گیرینتایج این بررسی نشان داد، عدم رعایت الزامات سیستم مدیریت نگهداری منجر به بروز آسیبها و صدمات بر عملکرد دستگاهها و همچنین عدم افزایش ایمنی تجهیزات، طول عمر مفید فعالیت و زمان دسترسی به همراه کاهش خرابیها و هزینههای تعمیرات میگردد.

بتاتالاسمی شایعترین بیماری ارثی تک ژنی در سراسر دنیاست که با الگوی اتوزومی مغلوب به ارث میرسد و در اثر جهش در ژن بتاگلوبین ایجاد میشود. از علایم ناقلین بتا تالاسمی، کاهش شاخصهای خونی گلبولهای قرمز و تغییراتی در میزان هموگلوبین از جمله پایین بودن هموگلوبین A1 و بالا بودن هموگلوبین A2 را میتوان نام برد. هدف این تحقیق، یافتن رابطهای بین مقادیر این شاخصها و نوع جهشهای ژنی منجر به بیماری میباشد که میتواند به عنوان روش تشخیص سریع نوع جهش در غربالگری اولیه ناقلین و تشخیصهای پیش از تولد به کار رود.
مواد و روشهادر این مطالعه توصیفی تحلیلی پس از تکثیر و تعیین توالی ژن بتاگلوبین به روش سانجر از جمعیت 200 نفری ناقلین بتا تالاسمی استان خوزستان، ارتباط جهشهای ژن با اندازه شاخصهای گلبولهای قرمز افراد به روش آماری تجزیه و تحلیل واریانس(Anova) و به کمک نرمافزار 26SPSS مورد بررسی آماری قرار گرفت.
یافتههانتایج نشان داد که جهش IVSII-1(G>A)با 5/22% فراوانی شایعترین جهش در این جمعیت بوده و همچنین از نظر آماری ارتباط معناداری بین تغییرات میانگین شاخص MCV افراد نسبت به نوع جهشآنها وجود دارد. به طوری که بیشترین میانگین این شاخص با مقدار 7/3 ± 4/65 در جهش IVSI-110(G>A) مشاهده شد.
نتیجه گیرییافتههای تحقیق حاضر در راستای بسیاری از مطالعههای انجام گرفته در این زمینه بوده و میتواند به عنوان یک راهنما جهت امکان تشخیص سریع نوع جهش در مراکز تشخیصی و درمانی به کار رود.

پرتوتابی فرآورده های پلاکتی یک اقدام درمانی مناسب جهت پیشگیری از بروز بیماری پیوند علیه میزبان ناشی از تزریق خون در گیرندگان مستعد می باشد. با این وجود، برخی از مطالعهها حاکی از کاهش بازیابی پلاکت پس از تزریق و تشدید آسیب های دوران نگهداری متعاقب تابش اشعه گاما هستند. علاوه بر این، القای شرایط اکسیدان به ویژه افزایش تولید گونه های فعال اکسیژن متعاقب تیمار فرآورده های پلاکتی با اشعه گاما در برخی مطالعهها مشاهده شده است. نظر به تاثیر گونههای فعال اکسیژن بر القای فرآیند آسیب دوران نگهداری پلاکت، مقاله مروری حاضر، ضمن معرفی منابع تولید گونههای فعال اکسیژن و نقش آنها در پلاکت ها، به بحث در رابطه با نحوه بروز و اهمیت استرس اکسیداتیو وابسته به دوران ذخیرهسازی در فرآورده های پلاکتی اشعه دیده با گاما پرداخته است. بدین منظور واژه های کلیدی در پایگاههای اطلاعاتی MEDLINE، PubMed و Google scholar جستجو شد و درنهایت 66 مقاله مورد استناد قرار گرفت.

بیماری کرونا ویروس که توسط SARS-COV-2 ایجاد میشود، در مدت کوتاهی اغلب نقاط دنیا را درگیر کرد. در زمان ظهور پاندمی به این گستردگی، بانک خونها نیز تحت تاثیر قرار میگیرند. به دلیل شیوع گسترده SARS-COV-2 دستورالعملهای جدیدی در مراکز انتقال خون اتخاذ گردیده است. بررسی اثرات پاندمی SARS-COV-2 بر مراکز انتقال خون، اهدا و سلامت خون و استفاده از فرآورده های خونی برای درمان آن از اهداف این مقاله بود.
مواد و روشهادر این مطالعه مروری، واژگان کلیدی در پایگاههای Science Direct ، PubMed ، Scopus و ژورنالهای Science Nature ، Elsevier ، Cell ، BMJ ، Blood ، Transfusion و Lancet جستجو شده و نهایتا 99 مقاله استفاده گردید.
یافتههابیماری COVID-19 اثرات منفی بر اهدا و سرویسهای انتقال خون داشته است. از مسایل مهم این است که انگیزه اهداکنندگان با وجود پاندمی حفظ شود و مراکز درمانی با کمبود فرآوردههای خونی روبه رو نشوند. SARS-COV-2 چالشهایی در جذب اهداکنندگان و سلامت خون ایجاد کرده زیرا عفونت میتواند بدون علامت باشد. با این وجود، تا به امروز موردی از انتقال ویروس به واسطه تزریق فرآوردههای خونی گزارش نشده است.
نتیجه گیریCOVID-19 اثرات منفی بر اهدا و ذخایر خون داشته است. مراکز خون سیاستهای جدیدی را برای محافظت از اهداکنندگان و تامین خون ایجاد کردهاند؛ از جمله اضافه کردن پرسشهای جدید به پرسشنامه غربالگری اهداکنندگان و معافیتهای جدید برای ابتلا، واکسیناسیون و مدیریت خون بیماران. پلاسمای هیپرایمیون، اگر چه به عنوان یک درمان احتمالی مطرح میشود، با این حال در این زمینه تناقضاتی وجود دارد.

Considering the lower-than-expected participation of women in donating blood, it is necessary to encourage them to do so. In this regard, the preparation of appropriate and effective trainings requires the identification of effective factors in women blood donation. This study has investigated the relationship between the constructs of the theory of planned behavior and injection anxiety in female students of Isfahan University of Medical Sciences.
Materials and MethodsThis descriptive-analytical study was conducted with the participation of female students of Isfahan University of Medical Sciences in 2022. Six hundred and twenty-three students were selected by stratified multi-stage quota sampling method and the data related to the constructs of the theory of planned behavior and trypanophobia were collected through an electronic questionnaire with SPSS/20 software and coefficient tests. Pearson correlation and linear regression were used for data analysis.
ResultsThe results showed that 114 female students (18.3%) had a history of donating blood. There was a direct relationship between the constructs of awareness, mental norms, perceived behavioral control and attitude with the intention to donate blood; injection anxiety had an inverse relationship with the intention to donate blood (p ≤ 0.001).
ConclusionsThe theory of planned behavior can be a suitable model for conducting interventions to encourage women to donate blood; on the other hand, using different educational programs to reduce injection anxiety and treat anemia in women in order to increase blood donation seem necessary.

Alloimmunization is caused by exposure of B lymphocytes to foreign antigens following incompatible blood transfusions and is associated with many hemolytic complications. One of the ways to detect blood group antigens is monoclonal antibodies. The aim of the present study is transforming of alloimmunized cells against KELL antigen in thalassemia patients by EBV-Transformation method and to produce lymphoblastoid cell line producing anti-KELL antibody.
Materials and MethodsIn an experimental study, peripheral blood mononuclear cells (PBMCs) of 10 thalassemia patients immunized against KELL antigen were isolated by ficoll hypaque method and exposed to EBV-containing supernatant of B95-8 cell line for 4 hours. After one to three weeks, the production of antibody-producing lymphoblastoid clones and the specificity of the antibody were examined by microscopic observation and microhemagglutination method, respectively.
ResultsThe results showed that exposure of PBMCs of thalassemia patients immunized against KELL antigen with EBV viral soup caused the formation of lymphoblastoid clones. Examination of the supernatant of culture of produced clones showed that the antibody produced by these cells were able to identify red blood cells containing KELL antigen and agglutinate them by micro hemagglutination test.
ConclusionsBased on the findings, there is a possibility of PBMC transformation in thalassemia patients immunized against KELL antigen and antibody production.

Patient blood management is an evidence-based medical and surgical concept in order to achieve a better clinical outcome for patients. The first step in the establishment of patient blood management is its training. The purpose of this research is to investigate face-to-face and virtual educational methods for the establishment of patient blood management.
Materials and MethodsIn 2016, a prospective study was designed to implement "patient blood management" in Shahid Lavasani Hospital. The hospital staff were randomly selected from three groups of doctors, nurses and laboratory staff to enter one of the virtual and face-to-face training groups. The training focused on the principles of "patient blood management", substitutes for blood products and restrictive blood transfusion. After completing the course, both groups participated in a uniform exam. One-way Annova, chi-square, and paired t-test were used for comparison. We used SPSS version 23 software for analysis.
ResultsThe average score of people in the face-to-face course was 90.7 ± 10.1 (64-100) and in the virtual course 90.8 ± 10.6 (58-100). There was no significant difference in the average scores of participants in the face-to-face and distance training (p = 0.9). Neither was there any relation between work experience (p= 0.9) and years of education (p = 0.1).
ConclusionsThe knowledge gained in the participants of this study did not show a significant difference between the two virtual and face-to-face groups. In order to implement "patient blood management" in hospitals across the country, face-to-face and virtual trainings can be used.

One of the antigens of the blood group is Duffy, which includes different subtypes. This system is of clinical importance in terms of blood transfusion, so accurate identification of this antigen is very important. In this study, the process of solubilization of red blood RBC membrane and purification of the Duffy antigen, were investigated.
Materials and MethodsIn this experimental study, packed RBCs (O- and Fya+) were obtained from the blood donation center of Tehran province. The membranes of RBCs were solubilized using Nonidet P-40 detergent. In the next step, Fya antigen was purified by affinity chromatography method using a specific antibody against this antigen. The specificity of the purified protein was tested by ELISA technique.
ResultsThe Fya+ characteristic of red blood cells of the RBC bag was confirmed. In addition, the specificity of the antigen was confirmed by ELISA method. The absorbance levels of the purified antigen (OD450nm) in ELISA method were 1.056 ± 0.137 and 0.197 ± 0.016 by employing the Fya and Fyb antibodies, respectively.
ConclusionsThe present research showed that red blood cell membrane solubilization can be done to purify Fy antigen and this antigen may be used in various fields including immunization and antibody production.

The preventive maintenance management system plays an effective role in preventing defects, increasing the safety of equipment and users, and reducing cost of operating systems. In this study, the efficiency of these systems was investigated in 6 blood transfusion centers in Tehran, Alborz, Amol, Hamedan, Semnan and Qom during May to October 2021
Materials and MethodsIn this cross-sectional study, the condition of the components, connections and parts of 35 active engine rooms, ventilation and diesel generator systems in 6 blood transfusion centers was investigated through checklists, interviews and visual observation, and the obtained data was analyzed using the software SPSS 25.
ResultsThe results of this study indicated the non-compliance with the requirements in the maintenance management system of electrical network equipment and facilities in 6 centers with more than 10 years of history due to the lack of experienced personnel, financial limitations and lack of supervision in effective periodic visits, safety tests, and corrections. The need for reforms was imperative in the engine room and electrical network. The average non-cleansing of components in these centers was 61%, deposits and corrosion of connections 33%, rust 25%, clutter and accumulation of wires and cables 67%.
ConclusionsThe results of this study showed that non-compliance with the requirements of the maintenance management system leads to the failure to prevent the occurrence of damages and injuries on the performance of the devices, as well as the failure to increase the safety of the equipment, the useful service life of the operation and the access time, along with the reduction of breakdowns and repair costs.

Thalassemia is the most common monogenic disease worldwide inherited in autosomal recessive manner. Beta-thalassemia is caused by mutations in beta-globin gene. It is characterized by reduction in red blood cell indices including low MCV, low MCH, high RBC count, low HbA1 and high HbA2. Considering these symptoms, finding a relation between erythrocyte indices and kind of mutations can be used as a rapid method in mutation diagnosis and initial screening of carriers.
Materials and MethodsIn this descriptive-analytical study after DNA extraction from 200 beta- thalassemia minors in Khuzestan province by salting out method, we amplified and sequenced beta-globin gene by Sanger sequencing. Finally, we analyzed statistically the beta-globin gene mutations and erythrocyte indices association by SPSS.
ResultsOur results showed that IVSII-1(G > A) is the most common mutation with 22.5% frequency in this population of beta-thalassemia carriers. We found that MCV index variations have a meaningful association with beta-globin gene mutations and the highest mean of MCV (65.4 ± 3.7) was observed in IVSI-110(G > A) mutation.
ConclusionsThe results comply with many other studies performed in this field and can be used for rapid detection of mutation in carrier parents and prenatal diagnosis.

Gamma irradiation (GI) to platelet products is a usual therapeutic procedure for preventing the transfusion associated graft versus host disease (TA-GVHD) in susceptible recipients. Nevertheless, some studies indicate a decrease in platelet recovery after transfusion, associated with higher levels of platelet storage lesion (PSL) in gamma irradiated platelets. In addition, the induction of oxidant stress, especially the increase in the production of reactive oxygen species (ROS) following the treatment of platelet products with gamma rays, has been reported in some studies. Therefore, given the known effects of ROS on PSL induction, the review presented here, while introducing different sources of ROS production during storage, also discusses the importance of storage-dependent oxidative stress in platelet products exposed to gamma radiation. For this purpose, studies published in MEDLINE, PubMed and Google scholar were searched for relevant keywords, among which 66 studies were cited in this review.

Coronavirus disease 2019 (COVID-19) which is caused by SARS-COV-2 has affected most parts of the world in a short period. Transfusion services are also affected during the emergence of a pandemic of this magnitude. Due to the widespread outbreak of SARS-COV-2, new guidelines have been adopted in blood transfusion centers. Considering the impact of the SARS-COV-2 pandemic on blood transfusion services, blood donation, blood safety, and the use of blood products as a treatment for COVID-19 are addressed in this article.
Materials and MethodsIn this review, keywords were searched in Science Direct, PubMed, and Scopus databases, and the sites of Nature Science, Elsevier, Cell, BMJ, Blood, Transfusion, and Lancet journals, and finally 99 articles were used.
ResultsIn this article, in addition to a brief review of patient blood management, blood safety, and donor safety during the COVID-19 pandemic, the use of blood products as a treatment is considered.
ConclusionsHospitals and blood centers have created new policies and guidelines to protect donors and blood supplies including adding new questions to the donor questionnaire and adopting new deferrals for COVID-19 infection and vaccination. Although hyperimmune plasma (convalescent plasma) is proposed as a possible treatment, there are some contradictions in this field.
