    در حیطه طب و مراقبت های سلامت، برنامه های مربوط به تسهیم درآمد باید بدقت مورد بررسی قرار گیرد تا اطمینان لازم حاصل شود که این گونه طرح ها با اصول و قواعد اخلاقی همخوانی دارند. در موضوع تسهیم درآمد دو بحث اخلاقی عمده مطرح می باشد که عبارتند از: رضایت آگاهانه و تعارض منافع. پزشک باید مسوولیت و تعهد حرفه ای خود را با صداقت، امانت و وفاداری نسبت به بیمار، بدون توجه به تعارض با منافع شخصی ایفا نماید. تعارض منافع معمولا بر درآمد و منافع مالی دلالت دارد. این امر ممکن است مباحث مهمی همچون پرداخت های مالی ویژه به پزشک، تسهیم حق ویزیت و خودارجاعی را در پی داشته باشد. مسائلی از این قبیل می تواند تصمیم گیری حرفه ای را مخدوش سازد. برخی کشورها در قوانین حرفه ای پزشکی این موضوعات را مورد توجه قرار داده اند. در این مقاله برآنیم تا ضمن شرح اهمیت موضوعات ذکر شده در صحنه طبابت عملی، برخی مسائل اخلاقی را که باید توسط پزشکان مورد توجه قرار گیرد بیان نماییم.

    مقدمه
    Peroxisome Proliferators Activated Receptor-g2 (PPAR-2g) عضوی از خانواده بزرگ گیرنده های هسته ای می باشد که تمایز آدیپوسیت ها، متابولیسم لیپید و حساسیت به انسولین را تنظیم می کند. هدف مطالعه بررسی ارتباط پلی مورفیسم Pro12Ala ژن PPAR-g2 در افراد ایرانی مبتلا به چاقی می باشد.
    روش ها
    تعداد 78 نفر داوطلب مبتلا به چاقی با BMI>30 kg/m2 و FBS<126 MG/DL به همراه 78 فرد سالم با BMI>30 kg/m2 و FBS<126 MG/DL که مبتلا به چاقی و دیابت نوع 2 نبودند، طی یک مطالعه مورد شاهدی انتخاب شدند و از نمونه های خون کامل آن ها، استخراج DNA صورت گرفت. ژن PPAR-2g با روش PCR تکثیر و سپس چند شکلی مربوطه با روش RFLP (Restriction fragment lengths polymorphism) بررسی گردید. نسبت شانس با آزمون های یک و چند متغیره جهت بررسی ارتباط چند شکلی ژن PPAR-2g با چاقی محاسبه شد.
    یافته ها
    فراوانی آلل G در گروه کنترل 8.9 و فراوانی آلل G در گروه چاق 16.7 و فراوانی آللC درگروه کنترل 91.1 و در گروه چاق 83.3 بود. در گروه چاق بین میزان قند خون ناشتا و BMI با ژنوتیپ ها ارتباط معنی داری وجود داشت. افراد ناقل آلل G، قند خون ناشتای کمتر و BMI بالاتری داشتند. مقایسه فراوانی ژنوتیپ های PPAR-2g در گروه چاق با گروه کنترل با استفاده از آزمونX2 انجام گردید و مشاهده گردید که بین چاقی با پلی مورفیسم Pro12Ala ژن PPAR-g2 ارتباط معنی داری وجود داشت (P=025.0و X2=5.01).
    نتیجه گیری
    نتایج حاصل از این مطالعه نشان می دهد که پلی مورفیسم Pro12Ala ژن PPAR-2g با چاقی رابطه دارد و داشتن آلل G (آلانین) با افزایش BMI و کاهش قند خون ناشتا مرتبط است.

    مقدمه
    داروهای سولفونیل اوره نظیر گلی بن کلامید (گلیبوراید) از چندین دهه قبل در درمان دیابت کاربرد داشته اند اما هنوز ساز و کار دقیق عملشان مورد بحث است. از سوی دیگر گلوکوکیناز به عنوان حسگر گلوکز در سلول های بتای پانکراس مطرح بوده و در هوموستاز گلوکز و ترشح انسولین نقشی کلیدی را ایفا می کند. هدف از تحقیق حاضر، بررسی تاثیر گلی بن کلامید بر ترشح انسولین و آنزیم گلوکوکیناز در جزایر لانگرهانس جدا شده از پانکراس موش صحرایی بوده است.
    روش ها
    جزایر لانگرهانس با تکنیک هضم کلاژناز از موش های صحرایی سالم و یک مدل تجربی از دیابت نوع 2 جداسازی شد. فعالیت آنزیمی با اندازه گیری سرعت تشکیل گلوکز 6 فسفات به کمک تغییرات نشر فلوئوروسانس تعیین گردید. ترشح انسولین از جزایر دستچین شده با تکنیک انکوباسیون استاتیک مورد ارزیابی قرار گرفت. میزان انسولین با روش الایزا اندازه گیری شد.
    یافته ها
    نتایج حاصل از انکوباسیون گلی بن کلامید با جزایر لانگرهانس نشان داد که این ماده ترشح انسولین پایه (در غلظت 2.8 میلی مولار گلوکز) را هم در موش های دیابتی و هم در موش های سالم نسبت به کنترل (بدون دارو) افزایش داده است. در حالی که ترشح انسولین در پاسخ به گلوکز 16.7 میلی مولار از افزایش معنی داری برخوردار نبوده است. از سوی دیگر گلی بن کلامید هیچ گونه اثر تحریکی و یا مهاری بر گلوکوکیناز پانکراس چه در موش های سالم و چه دیابتی نداشته است. اما کاهش فعالیت این آنزیم در موش های دیابتی، دارای تفاوت معنی دار آماری با موش های سالم بوده است.
    نتیجه گیری
    بر اساس یافته های این پژوهش، می توان چنین استنتاج کرد که اثر افزایش دهندگی گلی بن کلامید بر ترشح انسولین از طریق سازوکاری بجز تاثیر بر ترشح انسولین تحریک شده با گلوکز می باشد و نیز تنظیم فعالیت گلوکوکیناز پانکراس، مستقل از گلی بن کلامید است.

    مقدمه
    مطالعات متعددی جهت بررسی پیوند پانکراس به عنوان روشی نسبتا قطعی در درمان دیابت نوع 1 انجام می شوند. از آنجا که میزان حیات جزایر برای پروگنوز پیوند مهم می باشد، در مطالعه حاضر مراحل لازم برای تهیه جزایر سلولی جهت پیوند در رت، انجام شده است. هدف از انجام این مطالعه ارزیابی میزان حیات مجموعه جزایر بدست آمده در روزهای مختلف و در نهایت پیشنهاد زمان مناسب جهت پیوند می باشد.
    روش ها
    پانکراس رت جدا شده، بعد از Chopping، توسط کلاژنازV برای مدت 20 دقیقه در دمای 37oC 37oC هضم شده و 3 بار سانتریفوژ شد.
    سپس جزایر زیر میکروسکوپ جدا شده و در محیط کشت RPMI1640 در دمای 37oC، برای 6 روز انکوبه شدند. هر Well حاوی 45-35 جزیره بود. میزان حیات جزایر پانکراس با ارزیابی میزان رنگ پذیری آنها با Acridine Orange / Propiodine Iodide و بررسی زیر میکروسکوپ فلورسنت تعیین گردید که در ساعت های 0، 24، 48 و روزهای 3 و 5 و 6 انجام پذیرفت.
    یافته ها
    میزان حیات پس از انکوباسیون به تدریج افزایش می یابد تا به حداکثر میزان خود در روز دوم می رسد. پس از آن میزان حیات سلول ها هستیم سیر نزولی را طی می کند به طوری که در روزهای 5 و 6 به شدت کاهش یافته و به حدود صفر می رسد.
    نتیجه گیری
    وضعیت حیاتی سلول های جزیره ای پانکراس پس از جداسازی و تخلیص بدنبال کشت در محیط مناسب بهبود می یابند، به طوری که این سلول ها بهترین آمادگی جهت پیوند را پس از 48 ساعت انکوباسیون بعد از جداسازی از محیط بدن دارند.

    مقدمه
    کنگرفرنگی با نام علمیCynara scolymus L.، گیاهی از خانواده کاسنی (Compositae) است. در این تحقیق تاثیر عصاره هیدروالکلی کنگرفرنگی بر میزان گلوکز، لیپیدها و لیپوپروتیین های سرمی و پیشگیری از بروز دیابت نوع یک مورد بررسی قرار گرفت.
    روش ها
    در این تحقیق از 20 رت نر بالغ به وزن متوسط 250-200 گرم در چهار گروه پنج تایی استفاده شد. رت های گروه شاهد سرم فیزیولوژی دریافت نمودند. گروه دوم (دیابتی شده)، آلوکسان منوهیدرات به میزان 120 mg/kg/bw دریافت نمودند. گروه سوم (دیابتی شده+گلی بن کلامید)، علاوه بر تیمار مشابه گروه دوم، گلی بن کلامید با دوز mg/kgbw 0.5 دریافت نمودند. گروه چهارم (آلوکسان منوهیدرات+کنگرفرنگی)، آلوکسان منوهیدرات را با دوز120 mg/kgbw همراه با عصاره هیدروالکلی کنگر فرنگی با دوز300 mg/kgbw به صورت هم زمان دریافت نمودند. تجویز مواد در همه گروه ها به صورت تزریق داخل صفاقی انجام گرفت. 48 ساعت بعد از آخرین تزریق، از قلب رت ها خون گیری به عمل آمد و فاکتورهای سرمی مورد بررسی قرار گرفتند.
    یافته ها
    نتایج به دست آمده، کاهش معنی دار میزان گلوکز، کلسترول تام، تری گلیسرید، VLDL و LDL را در گروه تحت تیمار هم زمان عصاره و آلوکسان منو هیدرات نسبت به گروه دیابتی شده نشان داد (P<0.05). همچنین در مورد میزان HDL نیز افزایش معنی داری مشاهده گردید (P<0.05). عصاره هیدروالکلی کنگرفرنگی نتوانست میزان قند خون را در حد داروی شیمیایی گلی بن کلامید کاهش دهد (P<0.05)، اما در اثر بر سایر عوامل در حد گلی بن کلامید عمل نموده است.
    نتیجه گیری
    کنگرفرنگی حاوی ترکیبات آنتی اکسیدان است که با اثر حفاظتی بر سلول های بتا، در مقابل آلوکسان عمل می کند. این تحقیق نشان داد که عصاره هیدروالکلی کنگرفرنگی می تواند در پیشگیری از بروز دیابت نوع یک موثر باشد.

    مقدمه
    با توجه به ارتباط هیپرکلسترولمی و بیماری های قلبی عروقی، مطالعه در جهت معرفی روغن هایی که نقش کمتری در افزایش کلسترول خون دارند ضروری می باشد. یکی از این روغن ها، روغن کنجد می باشد. در بررسی حاضر اثر ارده (حاوی روغن کنجد) بر لیپوپروتیین ها و تری گلیسرید خون موش صحرایی مورد بررسی قرار گرفت.
    روش ها
    شصت موش صحرایی نر از نوع آلبینو ویستار به وزن و سن تقریبی یکسان در 6 گروه 10 تایی تقسیم شدند. ابتدا هر 6 گروه به مدت 20 روز تحت رژیم عادی یکسان قرار گرفته و سپس گروه های 6 گانه به مدت 30 روز تحت رژیم های غذایی متفاوت قرار داده شدند. گروه 1 رژیم عادی، گروه 2 رژیم عادی به علاوه ارده، گروه 3 رژیم عادی به علاوه روغن آفتاب گردان، گروه 4 رژیم پرکلسترول (رژیم عادی به علاوه 0.5% اسیدکولیک و 1% کلسترول)، گروه 5 رژیم پرکلسترول به علاوه ارده و گروه 6 رژیم پرکلسترول به علاوه روغن آفتاب گردان به مدت یک ماه مصرف کردند. در انتها، از طریق سینوس چشمی از موش ها خونگیری به عمل آمد و غلظت کلسترول، تری گلیسرید و HDL سرم اندازه گیری شد. VLDL با استفاده از فرمول محاسبه گردید.
    یافته ها
    نتایج نشان داد که غلظت کلسترول خون موش های تغذیه شده با ارده (mg/dl 2.3±38.2) به طور معنی داری (P<0.01) نسبت به گروه 1 (5.9±49.5) کاهش داشت ولی این کاهش در گروه تغذیه شده با روغن آفتابگردان (گروه 3) معنی دار نبود. همچنین ارده به طور معنی داری باعث کاهش کلسترول موش های تغذیه شده با رژیم پرکلسترول (گروه 5) شد. کاهش کلسترول در موش های تغذیه شده با رژیم پرچرب به علاوه روغن آفتاب گردان نیز مشاهده گردید. نتایج حاصل از بررسی تری گلیسرید حاکی از کاهش معنی دار (P<0.05) تری گلیسرید در موش های گروه 2 در مقایسه با گروه کنترل (گروه 1) بوده است در صورتی که این کاهش در موش های تغذیه شده با روغن آفتابگردان (گروه 3) معنی دار نبود. در گروه های تغذیه شده با رژیم پرکلسترول (گروه های 5 و 6) روغن آفتابگردان قادر به کاهش معنی دار تری گلیسرید نسبت به گروه کنترل (گروه 4) نبود. در بررسی HDL، اختلاف معنی داری بین گروه های تغذیه شده با ارده و روغن آفتاب گردان با گروه کنترل مشاهده نشد. VLDL گروه 2 به طور معنی داری نسبت به گروه 1 کاهش یافت (P<0.05) ولی این کاهش در گروه 3 نسبت به گروه 1 معنی دار نیست. تغییرات VLDL در گروهای 5 و 6 نیز نسبت به گروه 4 معنی دار نمی باشد.
    نتیجه گیری
    نتایج بررسی، نشان دهنده اثر کاهش دهنده ارده بر کلسترول و تری گلیسرید سرم در رژیم عادی و همچنین کاهش کلسترول در سرم های هیپرکلسترولمی می باشد. با توجه به نتایج این مطالعه، پیشنهاد می گردد که اثر درمانی روغن کنجد در کاهش کلسترول افرادی که کلسترول خون بالا دارند مورد بررسی قرار گیرد.

    مقدمه
    اضافه وزن یکی از عوامل تهدید کننده سلامتی در جوامع امروز به ویژه جوامع صنعتی و پیشرفته است. افراد چاق نسبت به بیماری های متعددی از جمله دیابت و بیماری های قلبی-عروقی در معرض خطر بیشتری هستند. مشخص گردیده که تغییر در شیوه زندگی به ویژه کاهش وزن و محدودیت کالری به عنوان عوامل غیر دارویی، تاثیر زیادی در کاهش این مشکلات دارند. در بررسی حاضر اثرات احتمالی محدودیت کالری بر تولید نیتریک اکسید (NO) و میزان فشار خون در موش های نرمال و دیابتی شده مورد مطالعه قرار گرفت.
    روش ها
    تعداد 40 موش صحرایی در چهار گروه کنترل (C)، محدودیت کالری (CR)، دیابتیک (D) و دیابتیک تحت تاثیر محدودیت کالری (CRD) مورد بررسی قرار گرفتند. دو گروه کنترل (C) و دیابتیک (D) آزادانه به آب و غذا دسترسی داشتند در حالی که گروه های CR وCRD به مدت چهار هفته و به صورت یک روز در میان به غذا دسترسی داشتند. دو گروه اخیر نیز آزادانه به آب دسترسی داشتند.
    یافته ها
    پس از طی 4 هفته میزان فشار خون و نیتریک اکسید سرم مورد سنجش قرار گرفتند که نتایج حاصل نشانگر این است که میزان فشار خون در گروه های CR و CRD به ترتیب نسبت به گروه های C وD کاهش معنی داری داشته است، در حالی که مقدار نیتریک اکسید سرم در گروه های CR وCRD به ترتیب نسبت به گروه های C و D افزایش معنی داری را نشان می دهد.
    نتیجه گیری
    از بررسی انجام شده می توان چنین نتیجه گرفت که محدودیت کالری می تواند موجب افزایش تولید نیتریک اکسید و همچنین کاهش فشار خون در موش های نرمال و دیابتیک گردد که احتمالا این کاهش فشار خون ناشی از افزایش میزان نیتریک اکسید می باشد.

    مقدمه
    مطالعات اندک انجام شده طی بارداری، نتایج متناقضی از نقش غلظت لپتین در اختلالات مرتبط با بارداری و هموستاز گلوکز را گزارش کرده اند. هدف از این مطالعه بررسی ارتباط سطح سرمی لپتین با ابتلا به دیابت بارداری است.
    روش ها
    این مطالعه به صورت مقطعی در 741 زن باردار مراجعه کننده به بیمارستان های تابعه دانشگاه علوم پزشکی تهران انجام شد. آزمون غربالگری مورد استفاده، GCT (Glucose challenge test) با 50 گرم گلوکز یک ساعته با معیار گلوکز بالاتر از 130 mg/dl بود. در موارد اختلال این آزمون، پیگیری با آزمون GTT (Glucose tolerance test) با 100 گرم گلوکز سه ساعته بر اساس معیار های کارپنتر و کوستان جهت تشخیص دیابت بارداری استفاده شد. غلظت مادری لپتین نیز مورد ارزیابی قرار گرفت.
    یافته ها
    بیماران مبتلا به دیابت بارداری نسبت به زنان سالم دارای سن، تعداد فرزند، نمایه توده بدنی و سطح سرمی لپتین بالاتری نسبت به افراد سالم بودند. در تحلیل رگرسیون لجستیک، بعد از تعدیل شاخص توده بدنی و سن، ابتلا به دیابت بارداری، ارتباط مستقلی با غلظت سرمی لپتین داشت.
    نتیجه گیری
    اندازه گیری لپتین ممکن است در شناخت زنان در معرض خطر ابتلا به دیابت بارداری موثر باشد.

    مقدمه
    تشخیص دیابت بارداری (GDM) نه تنها از نظر پیشگیری از بیماری ها و اختلالات پری ناتال مهم است، بلکه بر پیامدهای طولانی مدت سلامت مادر و کودک نیز تاثیر بسزایی دارد. آزمون های غربالگری دیابت بارداری که هم اکنون مورد استفاده قرار می گیرند، وقت گیر و پرهزینه اند. لذا یافتن آزمون های ساده تر و ارزانتر با حساسیت و ویژگی قابل قبول، ضروری به نظر می رسند. هدف از انجام این مطالعه، یافتن یک نقطه مناسب از قند پلاسمای ناشتا به عنوان تست غربالگری دیابت دوران بارداری می باشد.
    روش ها
    در این کارآزمایی بالینی، 200 زن باردار مراجعه کننده به درمانگاه پری ناتال بیمارستان امام خمینی ساری که حداقل یک عامل خطر ابتلا به GDM، شامل سن بیشتر یا مساوی 25 سال، سابقه حاملگی پر خطر (سقط تکراری، پره اکلامپسی، GDM، ماکروزومی، آنومالی جنینی و مرده زایی)، نمایه توده بدنی (BMI) قبل از بارداری بیشتر یا مساوی 25 kg/m2 و سابقه دیابت ملیتوس در فامیل درجه یک داشتند، وارد مطالعه شدند. مبتلایان به دیابت پیش از بارداری، از مطالعه خارج شدند. جهت همه افراد مذکور در هفته 24 تا 28 بارداری، آزمون گلوکز 50 گرمی درخواست شد و در صورتی که گلوکز پلاسمای یک ساعت بعد، بیشتر از 130 mg/dl بود، آزمون تحمل گلوکز خوراکی 100 گرمی بر اساس پروتکل ADA، 2006 و معیارهای تشخیصی کارپنتر و کوستان انجام شد و افراد مبتلا به دیابت بارداری مشخص شدند. سپس با استفاده از منحنی Receiver Operating Characteristic (ROC)، سطحی از قند پلاسمای ناشتا (FPG) که در تشخیص دیابت بارداری بالاترین حساسیت و ویژگی را دارا بود، تعیین گردید.
    یافته ها
    آزمون 50 گرمی گلوکز در 65 نفر (32.5%) مثبت شد و از این مقدار 58 نفر (میزان بازگشت 89%) جهت آزمون 100 گرمی مراجعه کردند که 20 نفر (10%) مبتلا به GDM تشخیص داده شدند. با استفاده از منحنی ROC با سطح زیر منحنی 0.853، سطح قند خون ناشتای mg/dl 91.5، بهترین حساسیت و ویژگی (به ترتیب80% و 92%) را در تشخیص دیابت بارداری، دارا بود.
    نتیجه گیری
    سطح قند خون ناشتای 91.5 mg/dl? حساسیت و ویژگی مناسبی برای تشخیص دیابت بارداری دارد لذا با توجه به ساده تر و ارزانتر بودن آن نسبت به آزمون 50 گرمی، به عنوان روش غربالگری دیابت بارداری توصیه می گردد.

    مقدمه
    بارداری یک حالت فیزیولوژیک است که به علت وجود میتوکندری های غنی جفت، شرایطی را ایجاد می کند که مورد توجه استرس اکسیداتیو می باشد. فلزات انتقالی به ویژه آهن که در جفت بسیار فراوان هستند، در ایجاد رادیکال های آزاد دخیلند. همچنین مطالعات نشان داده اند که رادیکال های آزاد در ایجاد دیابت بارداری نقش دارند. این مطالعه جهت مقایسه پارامترهای آهن در دو گروه زنان دارای دیابت بارداری و زنان فاقد دیابت بارداری انجام گرفته است.
    روش ها
    در این مطالعه مورد - شاهدی، 34 نفر از زنان باردار دارای دیابت بارداری به عنوان گروه مورد و 34 نفر از زنان باردار فاقد دیابت بارداری به عنوان گروه شاهد انتخاب شده و دو گروه از لحاظ سن، تعداد پاریتی، نمایه توده بدنی و محل مراجعه با هم همسان شدند. پارامترهای آهن بدن شامل فریتین، آهن سرم و ظرفیت کلی اتصال آهن به ترانسفرین، درصد اشباع ترانسفرین، هموگلوبین، متوسط حجم گلبولی و متوسط هموگلوبین گلبولی در هفته های 24 تا 28 بارداری با هم مقایسه شدند.
    یافته ها
    در این مطالعه غلظت فریتین، آهن سرم، درصد اشباع ترانسفرین، هموگلوبین، متوسط حجم گلبولی و متوسط هموگلوبین گلبولی در زنان دارای دیابت بارداری به طور معنی داری بالاتر از گروه کنترل بود. همچنین ظرفیت اتصالی آهن به ترانسفرین نیز به طور معنی داری پایین تر از گروه کنترل بود (P<0.05). در مورد سایر متغیرها شامل نمایه توده بدنی و تاریخچه فامیلی دیابت نوع 2 و دیابت بارداری رابطه معنی داری مشاهده نشد.
    نتیجه گیری
    یافته های این مطالعه، ارتباط معنی داری را بین پارامترهای آهن بدن مادر و دیابت بارداری نشان می دهند که نتایج مطالعات قبلی را تایید می کند. این مطالعه پیشنهاد می کند نقش آهن اضافی ناشی از تجویز مکمل های آهن در طی دوران بارداری در ایجاد دیابت بارداری مورد بررسی بیشتری قرار گیرد.

    مقدمه
    هدف از این مطالعه توصیف توافق بین قند ناشتای مختل (IFG=Impaired fasting glucose) و تحمل گلوکز غیرطبیعی با در نظر گرفتن معیارهای قدیم و جدید تعریف IFG است. همچنین اثر اضافه شدن داده های بالینی نیز بر این توافق مورد ارزیابی قرار گرفت.
    روش ها
    در یک مطالعه از نوع توصیفی و مقطعی، 8766 مرد و زن بالای 20 سال از جمعیت شهری تهران مورد مطالعه قرار گرفتند. بعد از خروج افراد با سابقه دیابت قبلی و یا قند خون ناشتا و بالای 126mg/dl، در باقی مانده افراد قند خون ناشتا و 2 ساعته اندازه گیری شد. پارامترهای تشخیصی و ضریب توافق کاپا برای معیارهای قدیم و جدید IFG در تشخیص تحمل گلوکز مختل(IGT=Impaired glucose tolerance) و دیس گلیسمی (IGT یا دیابت) محاسبه شد. آنالیز رگرسیون لجستیک و منحنی های ROC برای تعیین عوامل خطر بالینی و تعیین cut- point های آنها برای IGT و دیس گلیسمی مورد استفاده قرار گرفت.
    یافته ها
    بعد از اعمال معیارهای جدید، حساسیت IFG برای تشخیص IGT با دیس گلیسمی افزایش یافت اما ویژگی و نسبت درست نمایی مثبت (LR+) کاهش یافت و کاپا افزایش مختصری پیدا کرد (0.158 به 0.286 برای IGT و 0.283 به 0.354 برای دیس گلیسمی). هنگامی که داده های بالینی شامل سن بالای 28 سال، نمایه توده بدنی بالای28 kg/m2 و فشار خون سیستولیک بالایmmHg 125 به تعریف جدید IFG اضافه شدند، توافق IFG با IGT و دیس گلیسمی افزایش محسوسی پیدا کرد، به طوری که کاپا از 0.286 به 0.470 برای IGT و از 0.354 به 0.574 برای دیس گلیسمی افزایش یافت. همچنین +LR از 3.86 به 14.5 برای تشخیص IGT و از 4.46 به 17.4 برای تشخیص دیس گلیسمی افزایش یافت.
    نتیجه گیری
    تعریف جدید IFG همراه با عوامل خطر ساز بالینی در دسترس گروهی از افراد را تشکیل می دهند که احتمال پیش بینی IGT با دیس گلیسمی بر اساس نتایج IFG در آنها زیاد است و ممکن است یک گروه هدف برای برنامه های پیشگیری کننده از دیابت باشند.

    مقدمه
    افزودن فعالیت جسمانی به دارو درمانی در بیماران دیابتی ممکن است تاثیر دارو را افزایش دهد و استفاده از دوز کمتر دارو را ممکن سازد. هدف کلی از این مطالعه اینست که آیا مصرف قرص گلی بن کلامید به همراه تمرین هوازی می تواند به طور موثرتری تنظیم قند خون بیماران دیابتی نوع 2 را بهبود بخشد؟
    روش ها
    در این مطالعه 28 بیمار دیابتی مرد (60-40سال) به طور داوطلبانه در تحقیق شرکت کردند و در سه گروه تجربی: دارو درمانی به تنهایی (مصرف روزانه 5 میلی گرم گلی بن کلامید)، تعامل تمرین هوازی با مصرف روزانه 5 میلی گرم گلی بن کلامید و تعامل تمرین هوازی با مصرف روزانه 2.5 میلی گرم گلی بن کلامید تقسیم شدند. برنامه تمرین هوازی سه جلسه در هفته و هر جلسه به مدت 45 دقیقه رکاب زدن روی چرخ کار سنج با شدت 70-60 درصد (Heart Rate HRRmax Reserve) بود. غلظت ناشتایی هموگلوبین گلیکوزیله (HbA1c)، انسولین، پپتید c و مقاومت به انسولین به عنوان متغیر های وابسته در سه مرحله پیش، میان و پس آزمون اندازه گیری شدند. جهت تجزیه و تحلیل یافته های تحقیق از روش آماری آنالیز واریانس (ANOVA) استفاده گردید.
    یافته ها
    پس از 12 هفته، غلظت (HbA1c) بیماران شرکت کننده در هر سه گروه به طور معنی داری کاهش و غلظت پپتید C پلاسما افزایش یافت (P<0.05). مقاومت به انسولین در هر سه گروه کاهش، ولی غلظت ناشتای انسولین بدون تغییر باقی ماند. در بین گروه ها از نظر تغییرات ایجاد شده در متغیرهای تحقیق تفاوت معنی داری مشاهده نشد (P>0.05).
    نتیجه گیری
    در بیماران دیابتی نوع 2، مصرف گلی بن کلامید به تنهایی و به صورت تعاملی با تمرین هوازی سبب بهبود کنترل گلوکز خون و مقاومت به انسولین می شود و بیماران دیابتی نوع 2 که از قرص گلی بن کلامید جهت درمان استفاده می کنند، ممکن است با انجام تمرین هوازی بتوانند میزان مصرف گلی بن کلامید را کاهش دهند بدون آن که در روند بهبود متابولیکی ایجاد شده اختلالی بوجود آید.

    مقدمه
    حدود 90% بیماران دیابتی را مبتلایان به دیابت نوع 2 تشکیل می دهند. گرچه اثر دیابت در اختلالات پریودنشیوم ثابت شده است، ولی در مورد اثر درمان مرحله اول پریودنتال بر کنترل دیابت نتایج متناقض است. هدف از انجام این تحقیق تعیین اثر مرحله اول درمان های پریودنتال و یا عدم انجام آن بر مقدار هموگلوبین گلیکوزیله (HbA1c) به عنوان معیار کنترل دیابت در افراد دیابتی نوع 2 با بیماری پریودنتال متوسط و شدید مراجعه کننده به درمانگاه غدد بیمارستان امام خمینی در سال 84-1383 بود.
    روش ها
    این مطالعه به روش کار آزمایی بالینی تصادفی انجام شد. 30 بیمار مبتلا به دیابت نوع 2 با بیماری پریودنتال متوسط و شدید، پس از توجیه طرح و کسب رضایت وارد تحقیق شده و آزمایش HbA1c برای آنان با روش HPLC انجام شد. در 15 بیمار به طور تصادفی اعمال جرم گیری و صاف کردن سطوح ریشه و بروساژ انجام گردید و در افراد گروه شاهد درمان های فوق انجام نشد. پس از دو ماه، مجددا آزمایش HbA1c تکرار شد و با آماره مناسب مورد قضاوت قرار گرفت.
    یافته ها
    افراد دو گروه از نظر میزان از دست دادن چسبندگی پریودونتال مشابه نبودند. میزان HbA1c اولیه در دو گروه شاهد و بیمار از نظر آماری تفاوتی نداشت. پس از انجام درمان های پریودونتال گرچه میزان HbA1c در گروه تحت درمان پریودونتال کاهش داشت، ولی بین دو گروه اختلاف معنی دار آماری به دست نیامد.
    نتیجه گیری
    به دنبال مطالعه فوق، درمان فاز اول پریودنتال بر کنترل دیابت نوع 2 موثر نبود.

    مقدمه
    با گسترش الگوی زندگی شهری، شاهد افزایش روزافزون شیوع عوامل خطر بیماری های قلبی عروقی در کشور هستیم. لذا غربالگری این عوامل خطرساز از اولویت های ساختار بهداشتی کشور است. هدف از این مطالعه بررسی ارزش روش غربالگری به کمک سابقه خانوادگی بوده است.
    روش ها
    10 مدرسه راهنمایی واقع در منطقه 6 شهرداری تهران به طور تصادفی انتخاب شدند. سپس پرسشنامه ای جهت بررسی سابقه خانوادگی بیماری های قلبی عروقی در میان دانش آموزان توزیع گردید که به کمک والدینشان آن را تکمیل کردند. پس از جدا کردن خانواده های با یا بدون سابقه خانوادگی، آنها به صورت جداگانه جهت بررسی بیشتر به بیمارستان شریعتی دعوت شدند. برای آنالیز اطلاعات از نرم افزار SPSS ویرایش 10 و تست های آماری T-test، Chi-Square و تست های توصیفی استفاده شد.
    یافته ها
    بر اساس پرسشنامه اولیه، حدود 40% خانواده ها سابقه خانوادگی مثبت داشتند. از میان خانواده های با سابقه فامیلی مثبت (پر خطر)، 72% و از گروه شاهد (گروه کم خطر)، 34% به درمانگاه مراجعه نمودند. میانگین کلسترول تام (TC) و LDL-C در خانواده های پر خطر بطور معنی داری بالاتر بود. میانگین قند خون ناشتا در پدران و فرزندان خانواده های پر خطر نیز بالاتر بود (P<0.05). فراوانی افراد با کلسترول تام بالای 200 mg/dl و قند خون ناشتای بالای 100 mg/dl در خانواده های پر خطر بیشتر بود. فراوانی افراد با LDL-C بالای 130 mg/dl نیز در پدران و فرزندان خانواده های پر خطر بالاتر بود. میانگین سن، فشار خون، BMI، TG و HDL-C در دو گروه تفاوت معنی داری نداشت.
    نتیجه گیری
    میزان مشارکت و فراوانی عوامل خطر قلبی عروقی در خانواده های با سابقه خانوادگی مثبت بطور معنی داری بیشتر است. لذا یافته های این مطالعه، استفاده از سابقه خانوادگی را جهت غربالگری تایید می نماید.

    In the field of health care and medical practice, gain sharing plans should be carefully examined to be sure they reflect the codes of ethics. Ethical principles involved in gain sharing include informed consent and conflict of interest. Physicians must discharge their responsibilities to patients with loyalty, honesty and good faith, all the while avoiding conflict of interest. Conflict of interest rules usually focus on financial gain. This may raise some important issues, including kickbacks, fee-splitting and self-referral. These practices can potentially distort professional judgment. Some countries have rules of professional conduct governing these issues. In this article, we intend to describe importance of the issues of kickbacks, fee-splitting and self-referral in the field of clinical medicine and discuss some ethical issues that should be considered by physicians.

    Background
    Peroxisome Proliferators- Activated Receptor-Gamma2 (PPAR- γ2) is a nuclear receptor that regulates adipocyte differentiation, lipid metabolism and insulin sensitivity. The aim of this study was to evaluate the association of the Pro12Ala polymorphism at the PPAR- γ2 gene in Iranian population with obesity.
    Methods
    The genomic DNAs of the 156 subjects including obese and healthy isolated from EDTA whole blood. Pro12Ala polymorphism detected by Polymerase Chain Reaction – Restriction Fragment Length polymorphism (PCR-RFLP).
    Results
    In the obese group, one sample (1.3%) was as homozygote Ala/Ala genotype, 24 samples (30.8%) were Pro/Ala heterozygote and 53 samples (67.9%)as Pro/Pro genotype were identified. in the control group, one sample (1.3%) was as Ala/Ala genotype, 12 samples (15.4%) were Pro/Ala genotype and 65 samples (83.3%) were Pro/Pro genotype. allele frequencies of Ala in obese subjects (qAla=%16.7)were significantly different from those in control subjects (qAla=%8.9).
    Conclusion
    Our results revealed that Pro12Ala polymorphism in PPAR- γ2 gene associated with obesity in the Iranian population and presence Ala allele cause to significantly higher BMI and lower fasting blood sugar.

    Background
    Sulfonylurea agents such as Glibenclamide (Glyburide) have been widely prescribe in treatment of type 2 diabetic patients for decades, but controversy remains about their precise mechanism of action. On the other hand, glucokinase serves as a glucose sensor in pancreatic β-cells and plays a key role in the regulation of insulin secretion and glucose homeostasis. The aim of the present study was to evaluate the effect of Glibenclamide on insulin secretion and glucokinase activity in the rat isolated pancreatic islets of Langerhans.
    Methods
    The islets from normal and type 2 diabetic rats were isolated by collagenase digestion method. Glucokinase activity was measured via determination the rate of glucose-6-phosphate formation in the fluorometric assay. Insulin secretion from hand-picked islets was evaluated by static incubation technique. Insulin concentration was measured by rat insulin ELISA kit.
    Results
    Our findings obtained from incubation of Glybenclamide with pancreatic islets revealed that this agent increases basal insulin secretion (at 2.8 mM glucose) in both normal and diabetic rats as compared it with control islet (without drug). However, the increase of insulin secretion in response to 16.7 mM glucose was not significant. On the other hand, Glybenclamide had no activating and/or inhibiting effect on pancreatic glucokinase activity in both diabetic and normal Rats. But reduced activity of this enzyme in diabetic rats was significant in comparison with normal.
    Conclusion
    These data show that increasing effect of Glybenclamide on insulin secretion is through a mechanism other than affecting Glucose Mediated Insulin Release. Moreover, the regulation of pancreatic glucokinase does not depend on glybenclamid.

    Background
    Many researches have been conducted on islet cell's transplantation for a definitive treatment of diabetes mellitus type1. As the viability of the islets is the most important factor in predicting the transplantation prognosis, we have designed a study to isolate rat's islets. The aim of the study was to assess the viability of the islets at different stages and suggest the best transplantation time.
    Methods
    Pancreatic islets were isolated from male rats (250-300gr) by standard surgical procurement followed by intraductal HBSS distension, chopping and digestion with collagenase (type V). After being centrifuged for 3 times, the islets were then hand-picked and incubated in 37oC with RPMI 1640 media for 6 days. Each well contained 35-45 islets. Viability of islets was assessed by 2 independent investigators, giving score 0-2 to the color of islets under florescent microscope after Propidium iodide/Acridine orange staining at 6 times: just after the incubation, 24h, 48h, 3rd day, 5th and 6th day.
    Results
    The viability of the islet cells was gradually increased after the incubation as we had the most viability rate after the second day, while it decreased after this period and reached the least rate on the 5th and 6th day.
    Conclusion
    The islet's viability increased following the cell culture after the isolation procedure, as they have the best condition for transplantation after 48 hours. As the islets’ viability is the most critical point in transplantation, further studies evaluating the effects of different interventions on viability is needed.

    Background
    Artichoke with the scientific name of Cynara scolymuse is a plant from compositae family. In this research, the effect of hydro alcoholic artichoke extract on serum glucose, lipids and lipoproteins and prevention of type 1 diabetes mellitus was investigated.
    Methods
    Twenty mature male Rats with mean weight of 200-250 gr in four groups were arranged. Rats in the control group, received physiological serum. The Second group (diabetic) received 120 mg/kgbw Alloxan monohydrate. The Third group (diabetic + Glibenclamide) received 0.5 mg/kgbw Glibenclamide in addition of the similar treatment with second group. The Fourth group (Alloxan monohydrate + Cynara scolymus), received 120 mg/kgbw Alloxan monohydrate with 300 mg/kgbw of Cynara scolymus simultaneously. Prescribing materials in all groups was done as interaperitoneal injection(IP). Fourty eight hours after last IP, blood sample was taken from each animal via cardiac puncture to measure blood factors.
    Results
    The results indicated significant reduction in glucose, cholesterol, triglyceride, VLDL and LDL levels in the treated group with extract and Alloxan monohydrate simultaneously as to compared diabetic group. Also, the result indicated significant increase in HDL level. Hydroalcoholic Artichoke extract could not reduce blood glucose level as compared with Glibenclamide, but had similar effects on other factors in comparison with Glibenclomide.
    Conclusion
    Artichoke contains antioxidants compounds, that plays a protective role on beta cells against Alloxan. The results of this research indicate that hydro alcoholic extract of Cynara scolymus could effectively prevent type 1 diabetes mellitus.

    Background
    A high dietary lipid intake is an important mediator of cholesterolemia and cardiovascular disease. Oils such as Sesame oil are not thought to contribute to such complications although information on the subject seems to be spare and contradictory. Since the ground seed of Sesame or “Ardeh” is commonly used in certain parts of this country (Iran), so its effects on the serum lipid profile of the rats have been investigated.
    Methods
    The study was performed on six groups of male Wistar albino rats (10 in each group) weighing 300-350 gr. Ardeh was prepared by grinding the seeds after they had been roasted at 60ºC. To prepare a high cholesterol diet, 1% cholesterol and 0.5% cholic acid was added. Group 1 consumed the ordinary diet; 2 the ordinary diet plus Ardeh (24%), 3 the ordinary diet plus sunflower oil (10%), group 4 the high cholesterol diet, 5 the high cholesterol diet plus Ardeh, and group 6 the high cholestrol diet plus sunflower oil, all for 30 days. Blood samples were obtained and sera were used to determine the lipid profile
    Results
    The results of the study revealed that addition of Ardeh to the ordinary diet decreased serum cholesterol level significantly (P<0.01) in group 2 (38.2±2.3 mg/dl) as compared to the group 1 (49.9±5.9). Ardeh and sunflower oil had a similar effect on the groups, which consumed the high cholesterol diet. Ardeh also reduced the triglyceride level significantly (P<0.01) in group 2 (65.1±7.6 mg/dl) as compared to group 1 (106.3±15.1). Presence of both Ardeh and sunflower in the diets failed to affect HDL, but VLDL levels in group 2 (13.0±1.5 mg/dl) were significantly (P<0.01) lowered as compared to group 1 (21.2±3).
    Conclusion
    From the results of this study it appears that Ardeh can have a beneficial effect on lipid profiles and its addition to the diet is recommended.

    Background
    Overweight is one of the most worldwide health problems particularly in industrial and developed communities. Obese subjects are at high risk for developing various disorders such as diabetes and especially cardiovascular diseases. It has been well established that life style modification plays an important role in reducing these complications, particularly weight reduction and caloric restriction (CR) as a non- pharmacological approach. In the present study the possible effects of caloric restriction on Nitric Oxide production and blood pressures in rat were investigated.
    Methods
    Four groups of rats were selected as control(C), caloric restriction (CR), diabetic (D), and caloric restriction diabetic (CRD). Control animals fed freely with standard pellet but caloric restricted animals fed an every other day diet for 4 weeks. The systolic blood pressure (SBP) was measured in all groups using the tail-cuff method under the light general anesthesia induced by ether. Nitric oxide (NO x) concentrations were determined in serum using a colorimetric non- enzymatic NO assay kit.
    Results
    In CR experimental groups (CR&CRD) SBP was significantly decreased as compared to control: (P<0.001) and diabetic (P<0.01) rats, while the serum NO x was significantly increased (P<0.01) in both groups as compared to control and diabetic rats.
    Conclusion
    On the basis of obtained results, it could be concluded that caloric restriction may reduce blood pressure and increase nitric oxide in diabetic and non-diabetic rats. Increase nitric oxide is the likely mechanism for decreasing blood pressure.

    Background
    Few studies have investigated maternal leptin concentrations in women with pregnancies complicated by gestational diabetes mellitus (GDM), and these published results are conflicting. We examined the association between plasma leptin concentration and GDM risk.
    Methods
    As a cross-sectional study 741 pregnant women that referred to five university hospital clinics were recruited. The universal screening was performed with a GCT-50g and those with plasma glucose level ≥130mg/dl, were diagnosed as GDM if they had an impaired GTT-100g based on Carpenter and Coustan criteria. The level of insulin was measured during OGTT-100g. Also maternal plasma leptin concentrations were measured.
    Results
    GDM patients had higher age, parity, BMI, and serum leptin concentration as compare with normal pregnancies. In logistic regression model serum leptin levels were independent factor for GDM.
    Conclusion
    Serum leptin concentrations can predict GDM.

    Background
    Detecting mothers with gestational diabetes mellitus (GDM) is not only important in prevention of prenatal morbidities but also has significant effect on neonatal and maternal long term outcomes. Today, there are screening tests for GDM but they are time-consuming and expensive, therefore it seems necessary to perform testes that are uses expensive but with higher sensitivity and specificity. The aim of this study was to determine a cut - off point of fasting plasma glucose (FPG) for screening of GDM.
    Methods
    200 pregnant women referring to the perinatal clinic of Imam Khomeini hospital, (Sari – Iran) were studied. All cases with age ≥25 years old, history of recurrent abortion, GDM, preeclampsia, macrosomia, still birth, diabetes mellitus(DM) in first degree family or pre gestational body mass index ≥25kg/m2 were selected. Those with pre gestational diabetes mellitus were excluded. All of participants underwent a 50 g glucose challenge test (GCT) between the 24th and 28th gestational week. If 1- hour plasma glucose was more than 130 mg/dl, a 3- hour 100g oral glucose tolerance test (OGTT) was performed. The diagnosis of GDM was made by ADA 2006 recommendation (Carpenter and Coustan diagnostic criteria). Referring to the Receiver Operative Characteristic Curve, level of FPG having highest sensitivity and specificity in diagnosis of GDM was determined.
    Results
    From 200 participants, 65 women had positive GCT, of them 58 (response rate 89%) referred for 100g OGTT and 20(10%) were diagnosed GDM. Using ROC curve and under curve area of 0.853; the FBG level of 91.5 mg/dl, showed the highest sensitivity and specificity, 80% and 92% respectively in diagnosis of GDM.
    Conclusion
    FBG ≥ 91.5 mg/dl has good sensitivity and specificity in screening of GDM. Since this is simpler and cheaper than 50g GCT, is recommended as a screening method in diagnosis of GDM.

    Background
    Pregnancy is a condition that favors oxidative stress mostly because of the mitochondria-rich placenta. Transitional metals, especially Iron, which is particularly abundant in the placenta, are important in the production of free radicals. Also studies showed that free radicals has a role in GDM. This study was performed to compare Iron status between gestational diabetes mellitus (GDM) patients and control groups.
    Methods
    As a case- control study 34 women with GDM were compared with 34 Healthy women matched for referred center, age, parity and BMI. Iron status measurements including ferritin, serum iron, total iron binding capacity (TIBC), hemoglobin, MCV and MCH at 24-28 weeks of pregnancy were assessed and compared between two groups.
    Results
    In this study, concentration of serum ferritin, Iron, transferin saturation and hemoglobin, MCV and MCH were significantly higher in GDM group and TIBC was significantly lower in this group as compared with controls (P<0.05). No significant association was observed in other variables including familial history of diabetes and GDM
    Conclusion
    Our findings indicate an association between increased Iron status and GDM. The role of excess Iron from Iron supplementation in the pathogenesis of GDM needs to be examined.

    Background
    The aim of this study was to determine the level of agreement between the impaired fasting glucose (IFG) and abnormal glucose tolerance before and after application of the new IFG definition and to evaluate the impact of adding common clinical data on this agreement.
    Methods
    A cross sectional population based study was carried out in an Iranian urban population which enrolled 8766 men and women over 20 years. Fasting and 2-hour plasma glucose were measured in all subjects excluding those with previously diagnosed diabetes and fasting plasma glucose ≥126 mg/dl. The diagnostic parameters and kappa coefficient between the previous and revised definitions of IFG for detecting impaired glucose tolerance (IGT) and dysglycemia (IGT and diabetes) were calculated. Logistic regression and ROC curve analysis were used to determine the independent clinical risk factors and their optimal cut-points associated with IGT and dysglycemia.
    Results
    After using the new criteria, sensitivity of IFG for detecting IGT or dysglycemia increased but specificity and positive likelihood ratio (LR+) decreased and the κ slightly improved (0.16 to 0.29 for IGT and 0.24 to 0.35 for dysglycemia). Adding the clinical data to the revised criteria considerably improved the agreement between IFG with IGT and dysglycemia (κ increased from 0.286 to 0.470 for IGT and from 0.354 to 0.574 for dysglycemia). This also increased the LR+ from 3.86 to 14.5 and from 4.46 to 17.4 respectively for detecting IGT or dysglycemia.
    Conclusion
    The new IFG definition in combination with common clinical risk factors most likely predicts IGT and dysglycemia.

    Background
    Combination of physical activity and pharmacotherapy in diabetes may augment the effects of the drug and may allow lower doses of medication that can minimize the side effects. The goal of the study was to determine the effectiveness of aerobic training and Glibenclamide combination in type 2 diabetes.
    Methods
    A total of 28 men with type 2 diabetes were divided to 3 groups randomly: Glibenclamide (5 mg daily) only, Glibenclamide (5 mg daily) plus aerobic training, Glibenclamide (2.5 mg daily) plus aerobic training. Aerobic training protocol was performed for 12 week, 3 days (session) a week, 45 minutes in a session (ergo cycle program at 60-70 % heart rate reserve). Fasting glucose, HbA1c, fasting insulin, c-peptide, and insulin resistance were measured at pre, mid and post treatment periods. Analysis of Variance test (ANOVA) were used to evaluate data.
    Results
    HbA1c significantly decreased and c-peptide significantly increased in three groups (P<0.05).There were also no between-group differences for c-peptide and HbA1c (P>0.05). Fasting insulin concentration did not alter in three groups, however, insulin resistance decreased (no significant) after 12 weeks.
    Conclusion
    In type 2 diabetic patients, Glibenclamide treatment alone or combination of aerobic training and Glibenclamide treatment, was effective in improving glycemic control in patients with type 2 diabetes. As a result, in patients with type 2 diabetes, the addition of aerobic training to Glibencelamide treatment allow lower doses of Glibenclamide to be used without impairment in glycemic control.

    Background
    The relationship between diabetes and periodontal diseases has already been proved, but the effect of non-surgical periodontal therapy on the control of diabetes is controversial. The aim of this study was to determine the effect of this type of treatment on the control of diabetes.
    Methods
    In this randomized clinical trial 30 type 2 diabetic patients with moderate to severe periodontitis who were referred to the diabetes clinic of Imam Khomeini hospital during 2004-2005 were studied. The treatment procedure was explained for control group and an informed consent was taken. Scaling and root planning was randomly done for 15 patients, while control group were not treated for periodontal disease. The glycated hemoglobin (HbA1C) and clinical attachment loss was measured for all of the patients before and 2 months after treatment.
    Results
    At the baseline the clinical attachment loss (CAL) was significantly different between two groups. There was no significant difference between baseline HbA1C in the control and the experimental group. After 2 months the HbA1C was reduced in the treated group which was not statistically significant.
    Conclusion
    In this study non-surgical periodontal therapy had not effect on the control of type 2 diabetes.

    Background
    Cardiovascular Diseases are becoming increasingly prevalent due to urbanization and industrialization of our country. Therefore Screening of the Cardiovascular Risk Factors is one of the most important health system priorities. The aim of this study was to evaluate a screening method implementing family history for cardiovascular risk factors.
    Methods
    We chose 10 Junior-high schools in the 6th district of Tehran using simple random sampling. Then a questionnaire asking about family history of cardiovascular disease in the first degree relatives (including grand parents) was distributed between them and was completed – with contribution of parents. Then high-risk and low-risk families were invited separately to Dr Shariati Hospital for further evaluation. SPSS software version 10 using student T-test, Chi-Square and descriptive tests were applied for data analysis
    Results
    Approximately 40% of families had positive family history (high risk). From this group, 72% participated for follow-up. From the low-risk group, only 34% participated. Total cholesterol and LDL-C levels were significantly higher in high risk (HR) group. FBS was also significantly higher in fathers and children of the HR group (P<0.05). High TC, LDL-C and FBS levels were more prevalent among the HR families (P<0.05). No significant differences were observed in age, BMI, BP, TG and HDL-C between the 2 groups.
    Conclusion
    Families with a history of cardiovascular disease participated in greater numbers in the screening process and the prevalence of risk factors in this group was significantly higher. The findings confirm the usability of family history in screening programs.
