صفحات 1-7مقدمهارتباط افسردگی و بیماری های عروق کرونر قلب به خوبی اثبات شده است، اما بررسی این ارتباط تا به حال با استفاده از یک روش نیمه کمی و functional انجام نشده است.
روش بررسی، بیمارانی که جهت انجام اسکن قلب مراجعه کرده بودند با استفاده از آزمون افسردگی بک Beck Depression) (Inventory=BDI مورد ارزیابی قرار گرفتند. اسکن قلب بیماران توسط دو متخصص پزشکی هسته ای که از نظرات یکدیگر و همچنین نتیجه آزمون افسردگی بک بیمار بی اطلاع بودند، تفسیر گردید. همچنین با استفاده از مدل استاندارد 20 سگمانی و نمره دهی 5 امتیازی برای هر سگمان، اطلاعات مربوط به هر بیمار به صورت نیمه کمی تفسیر گردید.یافته هادر مجموع 167 بیمار (71 مرد و 96 زن) با میانگین 56.6±11.2 سال مورد بررسی قرار گرفتند. هنگامی که تمامی بیماران مورد ارزیابی قرار گرفتند، رابطه معنی داری بین نمره خونرسانی قلب (Myocardial Perfusion Score=MPS) و نمره BDI دیده نشد ولی هنگامی که تنها بیماران دارای اختلال قابل توجه خونرسانی عضله قلب (MPS<90) مورد ارزیابی قرار گرفتند، ارتباط معنی دار گردید (P=0.017 و r=-0.641). همچنین تفاوت بین نمره BDI در کسانی که دارای خونرسانی عضله قلب در حد طبیعی بودند با افرادی که دارای انفارکت در محدوده دیواره تحتانی قلب بوده اند، قابل توجه بود (به ترتیب 15.06 و 19.92 و P=0.097). حال آنکه این یافته در اختلال خون رسانی سایر دیواره ها یافت نشد.نتیجه گیریدر بیماران دارای اختلال قابل توجه خونرسانی عضله قلب، با افزایش شدت اختلال خونرسانی بر شدت افسردگی بیمار افزوده می شود. همچنین به نظر می رسد نمره متوسط BDI در کسانی که دارای انفارکت در محدوده دیواره تحتانی قلب می باشند بیشتر از افرادی است که دارای خونرسانی عضله قلب در حد طبیعی هستند.

صفحات 8-12مقدمههمانژیوم نئوپلاسم خوش خیم کبدی است و افتراق آن از متاستازها و سایر توده های بد خیم کبدی از اهمیت فراوانی برخوردار است. اسکن 99mTc-RBC با توجه به ویژگی بسیار بالا در این زمینه ارجح به سایر روش ها تصویر برداری است. این اسکن به دو روش پلانار (planar) و اسپکت (SPECT) انجام می شود. نتایج این دو روش در بسیاری از موارد با هم مطابقت دارد و با توجه به اینکه روش دوم بسیار گرانتر و وقت گیرتر می باشد، بر آن شدیم تا با بررسی نتایج اسکن 99mTc-RBC با دو روش مذکور میزان همخوانی نتایج این دو روش را مشخص کنیم.روش بررسی62 بیمار (??-??سال، متوسط سن 42 سال) شامل 41 زن (%67) و 20 مرد (%33)، که جهت توده های کبدی یافت شده در سونوگرافی یا سی تی اسکن، بررسی می شدند، تحت اسکن 99mTc-RBC با هر دو روش پلانار و اسپکت قرار گرفتند. اطلاعات با استفاده از نرم افزار آماری SPSS مورد تجزیه و تحلیل قرار گرفت.یافته هااسکن پلانار در %41 موارد مثبت و در %59 موارد منفی بود، در حالیکه این مقادیر برای اسپکت به ترتیب 68.9 و %31.1 بود. ضریب توافق کلی بین اسکن پلانار و اسپکت در تشخیص همانژیوم کبدی %48 بود و 17 ضایعه (%27) فقط توسط اسپکت تشخیص داده شدند و در اسکن پلانار تشخیص داده نشد. اسپکت %35 از همانژیوم های خلفی و %58.3 از همانژیوم های کوچکتر از 3 سانتیمتر را توسط اسکن پلانار تشخیص داده نشده بودند، تشخیص داد. علاوه بر این، در %75 از موارد همانژیوماهای متعدد، روش اسپکت در مقایسه با اسکن پلانار تعداد بیشتری از ضایعات را نشان داد.نتیجه گیریاگر چه در بسیاری موارد بین نتایج اسپکت و اسکن پلانار توافق وجود دارد، روش اسپکت روش بهتر و قابل اعتمادتری برای تشخیص همانژیوما است و این مساله خصوصا در همانژیوماهای با اندازه کوچک، دارای موقعیت خلفی، و در موارد همانژیوم متعدد توصیه می شود.

صفحات 13-22مقدمهامروزه تصویربرداری از خونرسانی عضله قلب به روش SPET) Single Photon Emission Tomography) به علت ماهیت غیر تهاجمی و هزینه کم و در اختیار قرار دادن اطلاعات با ارزش فیزیولوژیک نسبت به روش آنژیوگرافی جهت تشخیص بیماری های عروق قلبی (Coronary Artery Disease) به خصوص در تشخیص کاهش خونرسانی در عضله قلب (Ischemia) دارای ارزش تشخیصی بسیار بالایی است. اما این تصاویر علل مختلف دچار آرتیفکت می شود که نتیجه این آرتیفکت ها کاهش صحت تشخیصی تصاویر و موارد مثبت کاذب است. یکی از مهمترین آرتیفکت های موثر بر این تصاویر تضعیف می باشد. تضعیف فوتون ها به علل مختلفی مانند پستان چپ در زنان و دیافراگم در مردان و دیواره قفسه سینه در هر دو جنس می تواند بوجود آید. تصحیح تضعیف به علت ماهیت غیر یکنواخت ناحیه قفسه سینه بسیار پیچیده است و از یک بیمار به بیمار دیگر متفاوت است. به نظر می رسد که تنها را حل برای اصلاح تضعیف به دست آوردن نقشه تضعیف بدن با استفاده از تصویربرداری عبوری باشد.روش بررسیبه منظور بررسی امکان استفاده از اسکن های عبوری با ایزوتوپ 99mTC به روش ترتیبی از یک فانتوم مسطح استفاده شد و الگوی تضعیف بدست آمده در تصاویر گسیلی اعمال و در نهایت جهت ارزیابی عملکرد اصلاح تضعیف، تصاویر اصلاح نشده و تصاویر اصلاح شده با نتایج آنژیوگرافی بیماران مورد نظر از نظر وجود یا عدم وجود نقص مقایسه گردید.یافته هااین تحقیق نشان داد که استفاده از تصاویر اسکن عبوری حساسیت آزمون را از %86 به %94 افزایش می یابد اما در مقابل اختصاصی بودن آزمون از %85 به %54 کاهش می یابد. در مجموع صحت تشخیصی تصاویر از %86 به %78 کاهش می یابد.نتیجه گیریاصلاح تضعیف در تصاویر خود باعث بوجود آمدن آرتیفکت های جدیدی در تصاویر می شود که خود عامل تعیین کننده مهمی در تفسیر تصاویر است. در مجموع به نظر می رسد که استفاده از اصلاح تضعیف در بررسی تصاویر به تجزیه خاص نیازمند است. در صورت عدم وجود تجزیه کافی اصلاح تضعیف می تواند ارزش تشخیصی تصاویر را به طور محسوس کاهش دهد.

صفحات 23-27مقدمهاتصال آنتی بادی های مونوکلونال به آنتی ژن های وابسته به تومورها روشی موثر در درمان سرطان می باشد، زیرا این عوامل قادرند بطور اختصاصی این سلول ها را هدف گیری کنند و در واقع به عنوان عاملی موثر برای تشخیص، درجه بندی و مرحله بندی و درمان انواع سرطانها محسوب می گردند.روش بررسیدر این تحقیق بر علیه سلول های سرطان کولون نوعی آنتی بادی مونوکلونال جدید تهیه شد و غلظت آنتی ژن در سلول های مختلف (گلبول های سفید، رده سلولی HT29، LS180 و MCF7) از طریق روش رایوایمونواسی با استفاده از پروتئین جی نشاندار شده با ید- 125 ارزیابی گردید.یافته هانسبت اتصال پروتئین جی نشاندار شده با ید- 125 به گلبول های سفید (WBC)، رده سلولی HT29، LS180 و MCF7 به ترتیب برابر 7.1، 91.2، 75.8 و 40.2 درصد بدست آمد.نتیجه گیریبا توجه به اهمیت آنتی بادی های مونوکلونال، لازم است تا روشی موثر برای ارزیابی قابلیت استفاده آنها در تشخیص و درمان انواع سرطان ارائه شود. در روش ارائه شده برای این منظور از مواد رادیواکتیو استفاده شده است که هیچگونه محدودیتی برای شمارش آن وجود ندارد و علاوه بر استفاده کیفی، بصورت کمی نیز می توان از آن استفاده کرد.

صفحات 28-34مقدمهاستفاده از پپتیدهای نشاندار در پزشکی هسته ای از اهمیت ویژه ای برخوردار است. آنالوگ های سوماتوستاتین نشاندار شده با رادیونوکلیدهای مختلف برای تشخیص و یا درمان تومورهای با رسپتور مثبت سوماتوستاتینی به طور گسترده ای در حال تحقیق و استفاده می باشند. نشان دارسازی آنالوگ های سوماتوستاتین با 99mTc به عنوان رادیونوکلید جایگزین 111In نیازمند اجرای روش های کنترل کیفی مختلف جهت تعیین میزان نشان دارسازی و پایداری کمپلکس ایجاد شده جهت استفاده در کلینیک می باشد.روش بررسیجهت ارزیابی و مقایسه روش های مختلف تعیین خلوص رادیو شیمیایی، سه روش مختلف بررسی با یکدیگر مقایسه شده اند که شامل کروماتوگرافی مایع با کارآیی بالا (HPLC)، کروماتوگرافی لایه نازک (TLC) و استخراج فاز جامد با ستون کوچک (Sep-Pak) می باشند.یافته هاهر سه روش کارآیی لازم برای تعیین خلوص رادیو شیمیایی کیت های پپتیدی ذکر شده را داشتند، اگر چه استفاده از HPLC دقیق تر و موثرتر از بقیه روش ها بود.نتیجه گیریاستفاده از HPLC به عنوان دقیق ترین و کاراترین روش جهت خلوص رادیوشیمیایی کیت های پپتیدی می باشند. استفاده از TLC یا Sep-Pak در صورت در دسترس نبودن HPLC در کلینیک های پزشکی هسته ای و پس از آموزش و به دست آوردن تجربه کافی برای ارزیابی کیت های پپتیدی پیشنهاد می شود.

صفحات 35-41مقدمهدر این مطالعه رادیوداروی [18F] شش-تیا-14 فلوئورو- هپتادکانوییک اسید و یا به اختصار [18F] FTHA که یکی از مهمترین رادیوداروهای حاوی رادیوایزوتوپ پوزیترون دهنده فلوئور-18 میباشد تهیه شده است.روش بررسیبررسی جهت تعیین بهترین دما برای فلوئورینه کردن و آبکافت، زمان و غلظت واکنشگرها به عمل آمد. در محیط بی آب استونیتریل و کریپتوفیکس 222 و در دمای? ?-??درجه واکنش فلوئوریناسیون انجام شد. سپس در محیط قلیایی پتاس 0.2 مولار و در دمای 95 درجه آبکافت انجام گردید و پس از کروماتوگرافی و اسیدی کردن، رادیودارو روی ستون فاز معکوس تغلیظ شد. ماده نهایی توسط روش های RTLC/HPLC تحت کنترل کیفی قرار گرفت و سرانجام توسط آلبومین %5 فرموله و به موشهای صحرایی سالم نر تزریق گردید و آزمونهای پراکنش زیستی و تصویربرداری بر روی آنها انجام گرفت.یافته هامراحل کنترل کیفی محصول نشان داد که رادیوداروی تولید شده، دارای خصوصیات مناسب برای انجام اسکن رادیودارویی است. پس از نشاندار سازی مولکول دارویی و تایید خلوص نسبی آن به روش های کروماتوگرافی خالص سازی بیشتر آن بوسیله فاز معکوس جامد از محلول نهایی انجام شد و رادیودارو با درصد خلوص قابل قبول (بالای 90 درصد) به دست آمد. جذب مناسب رادیودارو در 140-160 دقیقه پس از تجویز در میوکارد مشاهده شد.نتیجه گیریرادیوداروی 18FTHA در زمان 30 دقیقه توسط یک روش نیمه خودکار با خلوص بالای 90 درصد و اکتیویته ویژه مناسب تصویربرداری قابل تهیه میباشد. تهیه مقادیر بالای این رادیودارو برای مصارف انسانی امکانپذیر است.

Pages 1-7IntroductionThe relationship between depression and coronary artery disease is well established; however assessment of this relationship using functional and semi-quantitative imaging (myocardial perfusion scintigraphy) was lacking. The aim of this study was to assess the relationship between the severity of depression and the severity of myocardial perfusion abnormality.MethodsPatients were assessed with rest/stress 99mTc-MIBI myocardial perfusion scintigraphy using Single Photon Emission Computed Tomography (SPECT) and Beck Depression Inventory (BDI-II). Visual and semi-quantitative assessment of myocardial perfusion score (MPS) in a five-scale measurement for standard 20-segment-model were used.ResultsWhen all patients (n=167) were evaluated, no correlation was found between MPS and BDI-score. Considering only those with significant perfusion abnormality (MPS95) (p=0.097), but other myocardial walls failed to show such association.ConclusionIn patients with significant perfusion abnormality, the greater is this abnormality the more severe is the depression. Also patients with inferior wall infarction, have a higher BDI-score as compared with normal subjects.Keywords: depression, Myocardial perfusion, Scintigraphy

Pages 8-12IntroductionHemangiomas are the most frequent benign tumors of the liver and detecting them from metastasis or other malignant hepatic tumors is of outmost importance. 99mTc- RBC scan is highly specific and is the method of choice for this differentiation. This scintigraphy is performed in Planar and SPECT modes. These two procedures are in agreement most of the times, however since SPECT is more expensive and time consuming we planned a study to evaluate the agreement of these two methods.MethodsSixty two patients (age range 20-80 yr, mean age 42 yr), 41 female (67%) and 20 male (33%), who were under evaluation for liver mass detected on abdominal ultrasound or CT scan, underwent 99mTc- RBC scan with both Planar and SPECT methods. The data was processed by computer and SPSS software (11.5).ResultsPlanar scan was positive in 42% and negative in 59% of cases, while for SPECT study these values were 68.9% and 31.1%, respectively. The Kappa value for planar and SPECT scan results was 48%, and there were 17 lesions (27%) which were detected only by SPECT. SPECT scan defined 35% of the posteriorly located hemangiomas, and 58.3% of hemangiomas smaller than 3 cm, which were not detected by planar scans. Also 75% of patients with multiple hepatic lesions showed more lesions by SPECT as compared to planar scan.ConclusionSince there is a weak agreement between SPECT and Planar scan, SPECT is considered the more reliable method in diagnosis of liver hemangioma and this is especially true when the hemangioma is of small size, has a posterior location or in cases of multiple lesions.Keywords: SPECT, Liver hemangioma, Planar scan, Space occupying lesion, 99mTc, RBC

Pages 13-22IntroductionNowadays, Imaging of the myocardial perfusion (MPI) using the single photon emission tomography (SPET) in the diagnosis of coronary artery disease, especially myocardial ischemia, is of great importance. In contrast to the coronary artery angiography, MPI is non-invasive, less expensive and more physiological. Unfortunately, this image is affected by the some artifacts. These artifacts lead to decrease image diagnostic accuracy and increase false positive cases. One of the most important effective artifacts is due to attenuation. These attenuation artifacts are caused by the left breast tissue in women, diaphragm in men and the chest wall in both sexes. Because of the inherent non- uniform attenuation map in the thorax region, this problem is very complex.MethodsThe aim of this study is to correct attenuation artifact with a simple method that is available and very easy to use. In this study we used transmission scanning by 99mTc in the sequential views and an attenuation map was created. After attenuation correction of the original images with attenuation map, non corrected and corrected images were compared with angiography results to apply attenuation correction based on functional findings.ResultsResults show that new artifacts are created and experiences of physicians in the impression of the images is critical. Finally, the sensitivity of images increased from 86.20% to 96.42%, the specificity decreased from 85.71% to 54.54% and the diagnostic accuracy decreased from 86% to 78%.ConclusionAttenuation correction can produce new artifacts, which can influence the way scans are interpreted. It seems that attenuation correction of the images, need special experience. Lack of enough experience in attenuation correction techniques, can comprise the diagnostic accuracy of images.Keywords: Attenuation correction, Myocardial perfusion, SPET, Emission scanning, Transmission scanning

Pages 15-24IntroductionMedullary thyroid carcinoma (MTC) is a rare thyroid cancer secreting calcitonin (CT) which is the most sensitive and specific tumor marker for MTC. This type of thyroid cancer is able to metastasize to different body areas including regional lymph nodes, lungs, liver and bone. The aim of this study was to assess the sensitivity and specificity of [99mTc-DMSA (V)] whole body scan (WBS) in detection of metastases in MTC.MethodsThis descriptive and prospective study was performed in 15 patients with MTC referred to our nuclear medicine centre since 2004 to 2005. These patients were compared regarding age, sex, and duration of the disease. Sensitivity and specificity of each diagnostic modality in detection of metastases were calculated and compared statistically.Results[99mTc-DMSA (V)] showed 91% sensitivity and 75% specificity as compared with serum calcitonin as gold standard. The figures for CT scan were 82% and 45%, respectively. CEA showed 64% sensitivity and 50% specificity.ConclusionIt is concluded that despite the slightly lower sensitivity and specificity of [99mTc-DMSA (V)] as compared to calcitonin (Gold-standard method), this radiotracer can be used for identification of recurrence or metastasis of medullary thyroid carcinoma.Keywords: Metastases, Medullary thyroid carcinoma, 99mTc, DMSA (V)

Pages 23-27IntroductionBinding a monoclonal antibody to tumor associated antigens is an effective method for cancer therapy because these agents can specifically target malignant cells, in fact monoclonal antibodies are effective agents for diagnosis, grading and treatment of different kinds of cancers.MethodsIn this research, a new monoclonal antibody against colon cancer cells was prepared and antigen concentration in different cells determined by a radioimmunoassay method using iodine (I-125) labeled protein G.Results125I-labeled protein G percent binding to white blood cell, HT29, LS180 and MCF7 cell lines were 7.1%, 91.2%, 75.8% and 40.2%, respectively.ConclusionRegarding importance of monoclonal antibody applications, it is necessary to find an efficient method for their evaluation in cancer therapy. Pages 28-34IntroductionUsing labeled peptides enjoys high importance in nuclear medicine. Somatostatin analogs labeled with different radionuclide are vastly investigated for diagnosis or treatment of somatostatin receptor positive tumors. Labeling somatostatin analogs with 99mTc as alternative of 111In needs doing different quality control method for determination of labeling yield and complex stability for clinic applications.MethodsFor determining radiochemical purity, three different methods were compared and evaluated including HPLC, TLC and Sep-Pak.ResultsAll the three methods were suitable for determining of radiochemical purity in peptide kits, although using HPLC was more effective rather than the others.ConclusionHPLC was the best method for determination of radiochemical purity of the peptide kits. Pages 35-41Introduction[18F]-6-thia-14-fluoro-heptadecanoic acid 3b, a free fatty acid, has been used in myocardial PET imaging. In order to establish an automated synthesis module for routine production in the country, a study performed for optimization of the production conditions as well as making modifications.Methods[18F] Benzyl-14-Fluoro-6-thia-heptadecanoate 2b was prepared in no-carrier-added (n.c.a) form from Benzyl-14-tosyloxy-6-thia-heptadecanoate 1 in one step at 90C in Kryptofix2.2.2/[18F] and acetonitrile as the solvent followed by Silica column chromatography. The radiolabeled ester 2 was then hydrolyzed to yield [18F]-6-thia-14-fluoro-heptadecanoic 3b. The final solution was concentrated using C18 SPE system and administered to normal rats for biodistribution as well as co-incidence imaging studies.ResultsThe synthesis took 15 min with overall radiochemical yield of 15-25% (EOS) and chemical-radiochemical purity more than 94%. Automation was performed using a two-pot synthesis. The best imaging time was shown to be 140-180 minutes post injection.ConclusionsUsing this procedure a fast, reliable, automated synthesis for the cordial PET tracer, i.e. [18F]-FTHA can be obtained without HPLC purification step.Keywords: PET imaging, Radiochemical, Fluorine, 18, Myocardium
