    From 2001 to 2004, we carry on a survey on the utilization of ampicillin among some diarrheagenic patients attending some Teaching Hospitals in Nigeria. From the result 723(58.2%) volunteers responded to the questionnaires, where only 83(11.4%) consult a doctor for prescription, 397(54.9%) from drug retailers while 142(20.1%) self prescribe and 56(7.7%) prescription was base on the advice of friends and relatives. The results also indicate that 229 (31.7%) take full regimen if prescribed by a doctor and 494(68.3%) stop the consumption of the drug when the signs and symptoms subside. Of the 249 diarrheagenic clinical isolates of E coli strains obtained from 2001 to 2004, 78 (31.3%) were obtained from UBTH, 41(16.5%) from ABUTH, 38(15.3%) from NAUTH and 92(36.9%) from UCH with a mean average of 63(25.3%). Among the antibiotics tested, ampicillin was the least susceptible antibiotics while the quinolones were the most susceptible. Most of the E coli strains screened had one or more resistant R plasmids. 52% of the E coli harbour ampicillin resistant plasmids of different molecular weights that ranged from ≤0.451kbp to ≥1.254kbp. Mostly Chromosomal DNA mediated most of the E coli isolates resistant to tetracycline, erythromycin, and ciprofloxacin. None of the Teaching Hospital had a surveillance method for evaluating the emergence of resistance pathogens.

    This a retrospective study performed in Dr. Shariati hospital, Fasa, Southern Iran on pregnant women referred due to burn. Most burns had happened in the second trimester, maternal and fetal mortality were 40.7% and 48.1% respectively. 59.9% of victims were from low socioeconomic class and 23% of them were burned due to suicidal attempts. This study concludes that maternal and fetal mortality is determined by total burn surface area, degree of burn, presence of pregnancy, availability of care facilities and time elapsed between burn and hospital admission. It is also concluded that pregnancy increases the maternal mortality rate in burn.

    At the beginning of the 3rd millennium, one of the most important wishes of the human being is to prevent the causes that disturb the intelligence, especially in the childhood. One of the most famous disorders that seems to have such an effect, is Obstructive Sleep Apnea Syndrome (OSAS), however, only a few studies have described the level of intelligence or cognition in the children with OSAS. Because of finding no preliminary study to search for any relation between the intelligence and chronic nasal or nasopharyngeal obstruction leading to chronic mouth breathing in the literature, the authors arranged a study to evaluate this relation.In a nonrandomized cross- sectional study, 60 otherwise healthy children with 6-12 years of age whom had been referred to the clinic of otolaryngology of Al- Zahra hospital in Isfahan were selected as the case group with the chief complaint of mouth breathing for at least 2 months. Besides, we selected 60 healthy children from 2 schools in Isfahan as the control group that had no history of chronic mouth breathing during last 6 months. All cases were referred to the clinic of psychiatry to test their intelligence. The data gathered and analyzed using T-Test. The study resulted that mean IQ of 60 subjects in the case group, with a range of 89-114, was measured about 100.07± 6.15. Mean IQ of the subjects in the control group was measured about 102.73± 6.14 (range 94-124). There was a statistically significant difference between the mean IQ of the case and control groups; so, there was found a relation between IQ and chronic mouth breathing (P=0.02). It is concluded that the chronic mouth breathing would lead to a lowered IQ comparing with the control group. Some of the most common causes of mouth breathing due to nasal or nasopharyngeal obstruction are adenoid hypertrophy and allergic rhinitis. The treatments of both of etiologies are easily accessible. This means that the lowered IQ of children with chronic nasal or nasopharyngeal obstruction seems to be, at least in some degree, preventable. These initial findings about the relation of IQ and chronic mouth breathing suggest that these are identifiable and preventable sequelae of childhood and that it is very important to evaluate the etiology of this problem and improve the patients'' IQ with the appropriate intervention.
  • Interventional Cardiology in Shiraz University of Medical Science Affiliated Hospitals During Year 2005.

    Cardiovascular disease, especially coronary artery disease, is the major cause of death in Iran after age 44.Revascularization procedures have pivotal role in improving quality of life in these patients. Data from cardiac intervention in Shiraz University of Medical Science can be compared with other centers to improve our faculties and equipments, better management of our future programs and ranking of Shiraz University scientific degree among other universities in Iran. To perform this study, an annual survey (year 2005) on cardiac intervention procedures was performed in Faghihi and Namazi Hospitals of Shiraz University of Medical Science. Randomly 1517 angiograms were selected and analyzed. It was resulted that: 1)During year 2005, 4380 diagnostic angiography and 808 angioplasty were performed of which coronary stenting was performed in 98.4% and drug eluting stents were used in 40.7% of procedures 2)Left circumflex coronary artery is the most frequent coronary involvement by significant lesion, followed by left anterior descending coronary artery.3) Right coronary artery system is dominant system in 88.0 % of angiograms.4) CABG (Coronary Artery Bypass Graft) is the most recommendation by faculties of university.5) Patients accept their doctor''s recommendation well.It was concluded that despite coronary stenting remains the fastest growing procedure in interventional cardiology, in our society CABG is the leading choice of treatment. Although, angioplasty is the typical procedure for single vessel disease patients, multivessel disease patients undergo CABG.

    Henoch_Schoenlein Vasculitis following drug administration is not so common. Here, a case of Henoch_Schoenlein Vasculitis caused by Ranitidine and Naproxen is described which presented with fever, vomiting bloody diarrhea, palpable purpura and azotemia. Histology of the skin showed leukocytoclastic vasculitis of superficial vessels and direct immunofluorescence revealed immunoglobulin A (IgA) in superficial dermal vessels and so confirmed the diagnosis. The patient was traeted with Ranitidine discontinuation and Prednisone administration. A short review of literature is also made.

    Simple renal cysts are very common and most are identified incidentally. Management is usually by observation. Some cysts may rupture spontaneously, mostly into the calyceal system, usually after some minor trauma. Silent rupture into the retroperitoneum monitored by MRI is presented here.
