    در طی دو دهه ی گذشته اثر بخشی چندین درمان تنظیم کننده و سرکوب کننده ی سیستم ایمنی در راستای کنترل بیماری ام اس به اثبات رسیده است. بیشتر این داروها بر فاز التهابی بیماری تاثیر می گذارد و در مقایسه با دارونما تعداد عود حملات و تاحدودی ناتوانی را کاهش می دهد. بتا اینترفرون ها از نخستین داروهایی بودند که به طور نسبی بر سیر بیماری تاثیرگذار بودند. فاکتورهای ویژه ای همچون سیر بیماری، نوع ام اس، آنتی بادی های خنثی کننده و عوارض جانبی باید در درمان با این داروها مورد توجه قرار گیرد. این مقاله مروری به بررسی ویژگی های بیولوژیکی، شواهد دال بر اثربخشی و عوارض جانبی بتا اینترفرون ها می پردازد.

    سابقه و هدف
    کاهش درک گفتار از پیامدهای اصلی نقص شنوایی است. در بسیاری از افراد مبتلا به «نوروپاتی شنوایی» توانایی درک گفتار کاهش شدیدی یافته است و با متوسط آستانه های شنوایی، همخوانی ندارد. در ارزیابی اولیه افراد مبتلا، غالبا عدم ثبت پاسخ برانگیخته ساقه مغز (ABR) و غیبت رفلکس های صوتی علیرغم عملکرد حلزون در محدوده طبیعی مشاهده می شود. این بیماران در آزمایش های سایکوآکوستیک نیز ضعف نشان می دهند، و نقص قابل توجه آنها در تمایز ویژگی های زمانی صوت، به ناهمزمانی فعالیت رشته های عصب شنوایی ربط داده می شود.
    روش بررسی
    مطالعه مقطعی- تحلیلی حاضر روی 16 بزرگسال مبتلا به نوروپاتی شنوایی و 28 فرد با شنوایی طبیعی صورت گرفت. برای کلیه افراد بیمار و شاهد، پاسخ های برانگیخته شنوایی میان رس (ALMR) و دیررس (ALR) ثبت، و اندازه گیری های سایکوآکوستیک «تمایز شدت»، «تمایز فرکانس” و»کشف وقفه«و همچنین آزمایش بازشناسی واژه در سکوت و در نسبت های مختلف سیگنال به نویز انجام شد، و ارتباط یافته های الکتروفیزیولوژیک و رفتاری مورد بررسی قرار گرفت.
    یافته ها
    در افراد مبتلا به نوروپاتی شنوایی، متوسط آستانه های تون خالص (PTA) با امتیاز بازشناسی واژه (WDS) همخوانی نداشت. علیرغم عدم ثبت AMLR در 6 نفر از بیماران، ALR در همه افراد ثبت گردید و بین برخی مشخصات این دو پاسخ و WDS، ارتباط معناداری وجود داشت. افراد مبتلا به نوروپاتی شنوایی در تمایز هر سه ویژگی شدت، فرکانس و زمان صوت، ضعف قابل ملاحظه ای را نشان دادند، و بین نتایج به دست آمده در هر یک از این سه ویژگی و امتیاز بازشناسی واژه، ارتباط معناداری وجود داشت. همچنین در این بیماران، امتیاز بازشناسی واژه در سکوت، تغییرپذیری قابل توجهی را نشان داد و با انجام این آزمایش در حضور نویز، افت چشمگیری مشاهده شد.
    نتیجه گیری
    بر اساس یافته های مطالعه حاضر، به نظر می رسد همزمانی عصبی در پاسخ های برانگیخته شنوایی دیررس نسبت به پاسخ های زودرس (و میان رس)، بازنمایی عصبی متفاوتی دارد. ضعف افراد مبتلا به نوروپاتی شنوایی در بازشناسی هر سه ویژگی شدت، فرکانس و زمان صوت و ارتباط آنها با امتیاز بازشناسی واژه، نقش رشته های عصب شنوایی در پردازش این ادراکات سایکوآکوستیک و تاثیر آنها بر درک گفتار را نشان می دهد.

    سابقه و هدف
    سکته های مغزی یکی از علل شایع افسردگی در سنین بالا می باشند. در حدود 30% موارد، بیماران مبتلا به سکته های مغزی دچار افسردگی می شوند. این مطالعه با هدف بررسی موارد افسردگی بدنبال سکته های مغزی در شهرستان بابل انجام شده است.
    روش بررسی
    در یک مطالعه همگروهی تاریخی و بر اساس یافته های بیمارستانی، میزان بروز افسردگی در 205 بیمار دچار سکته مغزی در طی مدت دو سال مورد بررسی و تجزیه وتحلیل آماری قرار گرفت.
    یافته ها
    از 205 بیمار مورد مطالعه، 108 نفر زن (7/52%)و 97 نفر (3/47%)مرد بودند. در طی این بررسی 37بیمار (18%) دچار افسردگی شده بودند که 16 نفر (2/43%) آنها زن و 21 نفر آنها (8/56%) آنها مرد بودند (2/0= P، 64/0-8/0= CI، 46/1 = RP). از 108 بیمار زن مبتلا به سکته مغزی، 32 نفر بعد از ترخیص از بیمارستان، در آسایشگاه سالمندان نگهداری می شدند که در طول این مطالعه 7 نفر (9/21%) از انها دچار افسردگی شدند در حالیکه از 76 زنی که در خانه نگهداری می شدند، 9 نفر (8/11%)دچار افسردگی شدند (18/0= P، 5/4-75/0= CI، 85/1 RR=).
    نتیجه گیری
    میزان بروز افسردگی به دنبال سکته مغزی در این مطالعه کمتر از سایر گزارشات می باشد و در بیمارانی که پس از ترخیص در آسایشگاه نگهداری می شدند بیشتر از مواردی بوده است که در منزل نگهداری می شدند.

    سابقه و هدف
    بیماری پارکینسون یکی از شایعترین بیماری های نورودژنراتیو با گسترش جهانی می باشد. مرگ نورونی دوپامینزژیک در هسته خاکستری به عنوان اصلی ترین فرآیند پاتولوژیک که منجر به ناتوانی حرکتی در بیماران می شود یافت شده است. برای درمان این بیماری داروهای زیادی بکار می رود اما لوودوپا اولین بوده و هنوز هم موثرترین دارو برای درمان علامتی بیماری پارکینسون می باشد، به کار بردن این دارو همراه با کاهش مورتالیته در مقایسه با زمان قبل از مصرف لوودوپا می باشد. با وجود این مصرف مزمن لوودوپا با محدودیتهای خاصی همراه است که شامل کسترش عوارض حرکتی در گروه عمده ای از بیماران می باشد و دیسکینزی القایی توسط دارو شایعترین عوارض حرکتی دارو است. این مشکلات می تواند به طور جدی عملکرد بیمار را به مخاطره بیاندازد و توانایی بیماران را در بهره مندی کامل از دارو محدود سازد.
    روش بررسی
    این مطالعه به صورت مقطعی – تحلیلی انجام گرفت، که 60 بیمار پارکینسونی مراجعه کننده به کلینیک اعصاب مورد بررسی قرار گرفتند. مطالعه ما شیوع دیسکینزی و دوز لوودوپا و زمان شروع دیسکینزی و محل دیسکینزی در بدن را شامل می شد. نرم افزار مورد استفاده جهت آنالیز آماری SPSS و سطح معنی داری به صورت 05/0 > P-value تعریف شد.
    یافته ها
    شصت بیمار پارکینسونی در این مطالعه شرکت داشتند که 21 نفر زن و 49 نفر مرد بودند، میانگین سنی بیماران 45/66 سال بود. اصلی ترین تابلوی این بیماران برادی کینزی، رژیدیته و ترمور بود که در اکثر بیماران وجود داشت. 11 مورد از بیماران دیسکینزی با شیوع 36% را داشتند که 6 نفر مرد و 5 نفر زن بودند. دوز لوودوپا در بیماران با دیسکینزی، روزانه 620 میلی گرم و در بیماران بدون دیسکینزی روزانه 550 میلی گرم بود. متوسط مدت زمان بروز دیسکینزی بعد مصرف لوودوپا 5 سال بعد بود. سن متوسط شروع بیماری در بیماران دارای دیسکینزی 59 سال بود که نسبت به بیمارانی که دیسکینزی نداشتند کمتر بود و محل شایع دیسکینزی ارومندیبولر بود (9 مورد از 11 بیمار).
    نتیجه گیری
    سه فاکتور مهم در این بررسی مشخص شد که در گسترش عوارض لوودوپا دخالت داشتند. این فاکتورها عبارتند از: دوز روزانه لوودوپای مصرفی، سن شروع بیماری و طول مدت بیماری. بنابراین برای کاهش ریسک دیسکینزی ناشی از لوودوپا ما می توانیم دوز لوودوپا را کاهش بدهیم، شروع لوودوپا را به تعویق بیندازیم و داروهای دیگری چون آگونیستهای دوپامین همراه آن بکار ببریم.

  • بررسی اثر انسداد اتصالات شکافی بر ادم و ضایعات مغزی در مدل تجربی ایسکمی مغزی-موضعی موقتی در موش صحرایی

    سابقه و هدف
    اتصالات شکافی کانال های ارتباطی بین سلولی هستند که نتایج ضد و نقیضی در ارتباط با نقش آنها در ایسکمی مغزی گزارش شده است. اخیرا نشان داده شده، کربنوکسولون (مسدود کننده قوی اتصالات شکافی)، میزان مرگ نورونی را در ایسکمی مغزی گلوبال کاهش می دهد. با این وجود اثر کربنوکسولون در مدل ایسکمی مغزی موضعی-موقتی بررسی نشده است.
    مواد و روش ها
    ایسکمی مغزی موضعی- موقتی با مسدود کردن شریان میانی مغز به روش فیلامنت و به مدت یکساعت و سپس برقراری مجدد جریان خون به مدت 23 ساعت ایجاد می شد. سپس اثر تزریق داخل صفاقی کربنوکسولون (ip mg/kg، 200) در شروع ایسکمی بر حجم ضایعات، ادم مغزی و اختلالات نرولوژیکی حرکتی حاصل از ایسکمی مغزی بررسی گردید.
    یافته ها
    تجویز داخل صفاقی کربنوکسولون (ip mg/kg، 200) در شروع ایسکمی ضایعات کورتکس را 58% و استرایتوم را 63% در مقایسه با گروه کنترل کاهش داد.(001/0 P<) کربنوکسولون اثر معنی داری بر اختلالات نورولوژیکی حرکتی و ادم مغزی نداشت (05/0 P >).
    نتیجه گیری
    نتایج این تحقیق نشان داد، تجویز سیستمیک کربنوکسولون در مراحل اولیه ایسکمی مغزی موضعی- موقتی ضایعات حاصل از ایسکمی را درموش صحرایی کاهش می دهد. این یافته ها همچنین نشان می دهند، احتمالا اتصالات شکافی در گسترش ضایعات مغزی در ایسکمی مغزی موضعی- موقتی دخالت دارند

    سابقه و هدف
    مالتیپل اسکلروزیس شایع ترین اختلال تحلیل برنده غلاف میلین در سلسله اعصاب مرکزی است.این بیماری در سنین اولیه جوانی با تخریب موضعی یا تکه تکه میلین در سلسله اعصاب مرکزی شروع می شود و با دوره های نامنظم بهبود و عود علائم همراه است. همانند دیگر بیماری های مزمن در مبتلایان به مالتیپل اسکلروزیس نیز یکسری نقص های شناختی از جمله نقص در حافظه (Memory loss) شایع است. حافظه به مفهوم کلی، آن دسته از جریان های روانی است که فرد را به ذخیره سازی تجربه ها، ادراک هاو یادآوری مجدد آنها قادر می سازد. با توجه به درگیر کردن جمعیت جوان جامعه و ابعاد از کار افتادگی این بیماری و انواع مشابه و کاربرد بسیار حافظه در زندگی آدمی و با توجه به این نکته که مطالعات صورت گرفته در داخل حاکی از فقر مطالعات بنیادین و کاربردی می باشد، و شیوع قابل توجه این بیماران در ایران ما را بر این داشت تا در این تحقیق، اثربخشی برخی از راهبردهای بهبود عملکرد حافظه را در بیماران مبتلا به مالتیپل اسکلروزیس، که از نقص حافظه رنج می برند، مورد بررسی قرار دهیم.
    روش بررسی
    این تحقیق از نوع نیمه تجربی است. جامعه آماری شامل کلیه بیماران زن مبتلا به M.S در شهر اصفهان در سال 1386 می باشد. تعداد 30 نفر از جامعه آماری که به صورت نمونه گیری در دسترس توسط بیمارستان خورشید، انتخاب و برای اجرای تحقیق در نظر گرفته شدند. نمونه های تحقیق بصورت تصادفی ساده در دو گروه 15 نفری قرار داده شدند. یک گروه شامل گروه آزمایش و گروه دوم کنترل بودند. حافظه بیماران به وسیله 2 آزمون، یکی آزمون حافظه وکسلر و دیگری آزمون محقق ساخته مورد سنجش قرار گرفت که قبل و بعد از آموزش راهبردهای حافظه (طی 8 جلسه، هر هفته 2 جلسه)، این سنجش انجام و پس از یکماه پیگیری صورت پذیرفت.
    یافته ها
    یافته ها نشان می دهد که آموزش راهبردهای حافظه در بهبود حافظه بیماران مبتلا به مالتیپل اسکلروزیس، تاثیر داشته است، چگونگی این تاثیر به وسیله 4 فرضیه مورد بررسی قرار گرفت. در فرضیه اول، نتایج نشان داد که آموزش تقویت حافظه بر نمرات حافظه (حافظه وکسلر) شرکت کنندگان در پس آزمون گروه آزمایشی تاثیر داشته است، (01/0 P<) میزان این تاثیر 65% بود. در فرضیه دوم، همین نتیجه در مورد نمرات آزمون محقق ساخته در پس آزمون گروه آزمایشی بدست آمد، (01/0 P<) که میزان تاثیر آن 69% بود. در فرضیه های سوم و چهارم که 2 آزمون وکسلر و محقق ساخته، پس از پیگیری مورد بررسی قرار گرفتند، نتایج تاثیر آموزش تقویت حافظه در بهبود حافظه بیماران مبتلا به مالتیپل اسکلروزیس در مرحله پیگیری را نشان می دهد. میزان این تاثیر در مورد نمرات هر دو آزمون 67% بود. (01/0 P<)
    نتیجه گیری
    پژوهش حاضر نشان می دهد که آموزش راهبردهای بهبود حافظه و بکارگیری صحیح این راهبردها در زندگی روزمره، حافظه بیماران مبتلا به مالتیپل اسکلروزیس را بهبود می بخشد و نتایج مثبتی بدست می دهد.

    سابقه و هدف
    سندرم میلرفیشر یا (Miller Fisher syndrome) MFS یک واریان از سندرم لیگن باره است که با تریاد افتالموپلژی، آتاکسی و آرفلکسی مشخص می شود. یافته های متنوعی در الکتروفیزیولوژی این بیماران دیده می شودکه عمدتا بر کاهش یا فقدان SNAP (Sensory nerve action potential) در اعصاب محیطی دلالت دارند. در این بررسی ضمن گزارش یک مورد سندرم میلرفیشر، یافته های سریال الکتروفیزیولوژی در آن مورد بررسی قرار گرفته می شود.
    معرفی بیمار: بیمار خانمی است 45 ساله که به علت دوبینی و اختلال راه رفتن در بیمارستان بستری شد. به عبارتی با افتالموپلژی، آتاکسی، آرفلکسی، ضعف صورت و حلق و اختلال حسی دیستال تظاهر کرد. یافته های الکتروفیزیولوژیک در بررسی های هدایت اعصاب حرکتی (NCS) نرمال و کاهش دامنه SNAP در اکثر اعصاب محیطی را نشان داد که در طی سیر بیماری کاهش قابل توجهی نیز پیدا کرد. بیمار به پلاسمافرز پاسخ کمی نشان داد.
    نتیجه گیری
    یافته های الکتروفیزیولوژیک در بیمار حاضر تا حد زیادی با یافته های سایر مطالعات همخوانی دارد؛ اما نکته قابل توجه در مطالعه حاضر، کاهش تدریجی دامنه SNAP در اعصاب اولنار است که مطرح کننده این مسئله می باشد که در سندرم میلرفیشر نیز مشابه با سندرم گیلن باره ممکن است یافته های الکتروفیزیولوژیک در اوایل سیر بیماری نرمال یا نزدیک نرمال باشد و با گذشت زمان یافته های غیر طبیعی خود را نشان می دهند.

    پروتئین C جزء اساسی در سیستم تنظیم کننده انعقاد خون می باشد و افراد مبتلا به نقص ارثی پروتئین C مستعد ایجاد ترومبوآمبولی می باشند. در این مقاله، ابتلا به نقص پروتئین C در سه عضو یک خانواده که در هر سه تظاهر اولیه بیماری با CVA بوده مورد بررسی قرار گرفته است.

    Several immunomodulatory and immunosuppressive therapies have been demonstrated to modify the course of multiple sclerosis during the last two decades. Theses drugs mostly impact the inflammatory phase of the disease and reduce the number of relapses or try to decrease disability comparing to placebo. Beta interferons were among the first agents that partially influence the course of the disease. Special factors such as disease course, type of MS, neutralizing antibodies and adverse events should be considered during administering theses drugs. This review outlines biological properties, current evidences supporting efficacy, and adverse events of beta interferons.

    Background
    Loss of speech perception is a major effect of hearing impairment. In many cases with "auditory neuropathy", speech perception sustains severe impairment and is disproportionate with behavioral pure-tone average. In a primary audiologic evaluation, there is usually no auditory brainstem response (ABR) or acoustic reflex, while the cochlear function is within normal range. Patients with auditory neuropathy exhibit impairments in psychoacoustic evaluations, and their difficulties in sound temporal processing are due to the dys-synchrony of their auditory nerve fibers.
    Methods
    This cross-sectional analytical study was performed on 16 young adults with auditory neuropathy and 28 normal hearing subjects. In both auditory neuropathy and control groups, auditory middle latency response (AMLR) and auditory late response (ALR) were recorded and psychoacoustic measurements including "intensity discrimination", "frequency discrimination", and "gap detection" as well as a word discrimination test in silent and in three different signal-to-noise ratios were carried out.
    Results
    There was no agreement between the pure-tone average (PTA) and word discrimination score (WDS). Despite AMLR absence in 6 patients, auditory late response ALR was recorded in all the individuals and there was a significant correlation between some characteristics of these responses and the word discrimination score. The patients revealed remarkable impairment in the discrimination of intensity, frequency, and temporal characteristics of sound. In addition, there was a significant correlation between each of these characteristics and WDS. The WDS showed considerable variability in individuals with auditory neuropathy and decreased in the presence of noise considerably.
    Conclusion
    It seems that neural synchrony had different representations in short (and middle) auditory evoked responses by comparison with late ones. Auditory neuropathy patients’ difficulties in discriminating the intensity, frequency, and temporal characteristics of sound and their relation with WDS showed eight nerve functions in processing these psychoacoustic perceptions and their effects on speech understanding.

    Background And Objective
    Stroke is a major cause of depression in elderly. About 30% of all stroke patients experience a kind of depression after stroke. This study was performed to assess the incidence of post stroke depression in our region.
    Method
    In a historical cohort and hospital-based study on 205 ischemic stroke patients, depression was determined during two year period after onset of stroke.
    Findings
    Thirty seven patients (18%) had depression during this study period sixteen (43.2%) female and 21male (56.8 %) (RR=1.46, CI= 0.8-2.64, P=0.2). Depression was 21.9% in institutionalized patients.
    Conclusion
    Incidence of post stroke depression in this study was less than other studies.Rate of post stroke depression in home care patients was less than institutionalized cases.

    Background And Objective
    Parkinson's disease (PD) is one of the most common neurodegenerative disease in world wide. Dopaminergic neural death in substantia nigra is subsequently found to be the main pathologic process leading cause to motor disability. For the treatment of this disease many drugs applied, but levodopa was the first and is still a highly effective drug for symptomatic treatment of Parkinson's disease. Administration of levodopa is associated with a decrease in mortality compared to the pre levodopa treatment. However, chronic use of levodopa is associated with certain limitation including of motor complication in the majority of patients. The most common motor complications are drug-induced dyskinesia and these problems can seriously compromise function and limit the patient's ability to benefit fully from the drug.
    Materials And Methods
    This study was a descriptive and cohort study that comprised 60 patients referred to neurological clinic with Parkinsonism. Our study aimes included to determine the frequency of dyskinesia and dosage of levodopa and time of beginning of dyskinesia and the location of dyskinesia in the body. Statistical analysis were used SPSS and statistical significance defined as a p- value<0.05.
    Results
    A total 60 Parkinson’s disease patients were participated in the present study, 49 patients were male and 21 patients female with average age 66.45 years old. The key features of Parkinson's disease which were bradykinesia, rigidity and tremor were present in almost patients. There were 11 cases of dyskinesia with frequency of 36 %, 6 men and 5 women, the dosage of levodopa in patients with dyskinesia was 620 mg / day and 550 mg/day in patients without dyskinesia. The average time of incidence of dyskinesia after used the levodopa in patients was 5 years later. The average age of onset in patients with dyskinesia was 59 Years (less than patient without dyskinesia). And the common location of dyskinesia was oromandibular (9 cases of 11 cases).
  • Evaluation the effect of blockage gap junctions on ischemic damages and edema in experimental model of transient focal cerebral ischemia in rat

    Introduction
    Gap junctions are inter-cellular communicative channels that have been reported controversial results about role of them in cerebral ischemia. Recently have shown that carbenoxolone (CBX، a potent gap junction blocker) significantly reduces neuronal cell death in global cerebral ischemia. However، role of gap junction with using of CBX has not been investigated in transient model of focal cerebral ischemia.
    Material And Methods
    Transient focal cerebral ischemia was induced via filament method in rats by 1 hour middle cerebral artery occlusion (MCAO)، followed by 23 h reperfusion. CBX was administered (200 mg/kg ip) at the beginning of ischemia. Infarct volume، brain edema and neurological motor dysfunction were assessed 24 h after MCAO.
    Results
    Administration of CBX (200 mg/kg ip) at the beginning of ischemia significantly reduces cortical and striatal infarct volumes by 58% and 63%، respectively in comparison with control group (P<0. 001). CBX did not significantly change brain edema or neurological dysfunction (P>0. 05).
    Conclusion
    Results of this study indicate that administration systemic of CBX reduces ischemic injuries in early phase of transient focal cerebral ischemia in rat. These findings also indicate that gap junction may have involved in development of brain damage in transient focal cerebral ischemia.

    Multiple sclerosis is one kind of nervous system diseases that destroys the myelin sheath of the nerve. The destruction of myelin happens in special parts like optical nerves. brain stem، cerebellum، these impaired parts are called plaque. This disease is accompanied with a wide variety of signs and symptoms. (Etemadi and Ashtari،1381). Memory disorder is another sign of cerebral diseases. it is of the most common malfunctions of such diseases. (Bahramnejad،1376). In this study، there has been a great attempt to assess an educational program me to improve the memory affectivity of patients.

    �Determining the effect of training memory strategies to improve the memory of the patients affected with multiple sclerosis.
    Methods
    This is an experimental study in which all female patients suffering from Ms who live in Isfahan were participated in 1386. 30 of the samples who were taken from khorshid hospital went under experimentation. this group was randomly chosen and was divided in to 2 groups of 15،one experimental and the other control group. The experimental group went under 4 weeks of training methods and strategies to improve memory، two sessions each week. The memory strategies are as follow: 1. Enhance meaning fullness 2. minimizing the interference 3. note-taking on white board 4. avoiding too much memorization 5. using calendar 6. organizing 7. Be consistent 8. leveling 9. one step at a time 10. mind relaxation 11. mind exercise 12. Repetition 13. time management rules. After 4 weeks، both groups had the post test using Wechsler questionnaire and MT questionnaire. One month later the follow up test was taken.
    Results
    The results show that training memory-improvement strategies affect the marks of post test. The covariance analysis of the post test marks of Wechsler test in both groups showed that eliminating the effect of control group between the adjusted marks of test token from both groups were significantly different. (p<0. 01). This effect was 65% so training memory strategies have direct effect on memory marks of participants in post test taken from experimental group. The covariance analysis of the post test marks of MT test in both groups showed that eliminating the effect of control group between the adjusted marks of test token from both groups were significantly different (p<0. 01). This effect was 69% so training memory strategies have direct effect on memory marks of participants in post test taken from experimental group. Analysis of covariance in marks of Wechsler memory test in both groups showed that eliminating control variable among adjusted average marks of Wechsler memory test results in significant difference. (p<0. 01) Therefore، training affects the marks. The results of covariance analysis in the post MT test in both groups showed that among the adjusted average marks a significant difference came out (p<0. 01). Therefore، training
    Conclusion
    So the forth proposition is confirmed. This effect was 67% so training memory strategies have direct effect on memory marks of participants in follow up experimental group

    Introduction
    Miller Fisher syndrome (MFS) is a variant of Guillain-Barre syndrome characterized by the triad of ophthalmoplegia, ataxia and areflexia. Different findings are observed in electrophysiologic studies of these patients which mostly demonstrate reduction of sensory nerve action potential (SNAP). Other than presenting a case of Miller Fisher syndrome, her serial electrophysiologic studies are evaluated in this report. Case report: We report a case of 45-years old woman that was hospitalized due to diplopia and gait disorders. She presented with ophthalmoplegia, ataxia, areflexia, facial and oropharyngeal weakness and distal sensory disturbance. Electrophysiologic assessment showed normal motor nerve conduction studies (NCS) and reduction of SNAP's amplitude in most peripheral nerves, which was significantly more reduced during the course of disease. Patient responded slightly to plasmapheresis.
    Conclusion
    Electrophysiologic findings of our patient was mostly similar to other studies however, gradual reduction of SNAP's amplitude in Ulnar nerves was considerable which assists that electrophysiologic findings of Miller Fisher syndrome may be normal or near normal at the time of the beginning of disease course - like what happen in patients with Guillain-Barre syndrome- but may show abnormal findings later.

    Protein C is a fundamental part in the regulatory system of coagnlation. In this article, three cases of the same family with protein C deficiency which all were presented with CVA are reported.
