    در این بررسی 208 فرد بدون علائم بالینی بیماری های قلبی-عروقی و سابقه ترومبوز وریدی از نقاط مختلف ایران با نژادهای متفاوت مطالعه شدند. در این مطالعه نحوه توزیع آلل جهش یافته MTHFR C677T در نژادهای مختلف جمعیت ایران بررسی شده و نتایج آن با مطالعات صورت گرفته مقایسه شده است. جهت ارزیابی توزیع پلی مرفیسمMTHFR C677T، یکی از عوامل ژنتیکی ایجاد بیماری های قلبی-عروقی (CVD)، در جمعیت ایران از روش هیبریداسیون معکوس برای تشخیص سریع و دقیق استفاده شد. اساس آزمون به روش Multiplex PCR و هیبریدیزاسیون بر روی نوار آزمون است که این نوار شامل خطوط موازی حاوی پروب های الیگونوکلئوتیدی خاص آللی است. فراوانی آللی جهش MTHFR C677T در این بررسی 24/0 بود که با نتایج گزارش شده توسط گلبهار و همکاران از ایران (24/0) کاملا مطابقت دارد. فراوانی جهش MTHFR C677T در ایران کمتر از اروپاست، اما بیشتر از فراوانی های گزارش شده از هندوستان و بیشتر کشورهای منطقه آسیاست. بر اساس اطلاعات نویسندگان، پژوهش اخیر جامع ترین مطالعه ای است که تاکنون بر روی پلی مرفیسم ژنی ترومبوفیلی در ایران انجام شده است.

    آتروفی های عضلانی نخاعی (SMAs) از بیماری های شایع ژنتیکی با توارث اتوزومی مغلوب هستند. این بیماری ها از نظر ژنتیکی و فنوتیپی بسیار ناهمگون (هتروژن) هستند. شیوع این بیماری ها را از 1 مورد در 6000 تا 1 مورد در 25000 نوزاد زنده ذکر شده است. بنابراین، فراوانی ناقلان جهش ها 1 در 40 تا 1 در 80 نفر در جمعیت است. بیش از 90 درصد افراد مبتلا بهSMA، فاقد هر دو نسخه اگزون هفتم ژن SMN1 هستند. فقدان SMN1 به علت حذف یا واژگونی ژنی و تبدیل آن به SMN2 رخ می دهد. بر اساس اطلاعات ما، هیچ مطالعه بزرگ جمعیتی و همه گیرشناختی در مورد این بیماری ها در ایران انجام نشده است و آگاهی ما در مورد فراوانی، شیوع بیماری و شیوع ناقلان بسیار محدود و نارسا است. به دلیل فراوانی ازدواج های خویشاوندی در ایران، به نظر می رسد که فراوانی SMA در جمعیت ایرانی نسبتا بالا باشد. با این پیش فرض، تصمیم گرفته شد به عنوان قدم اول، یک پژوهش همه گیرشناختی به منظور بررسی شیوع بیماریSMA I، شیوع ناقلان، گستره نشانه های بالینی و شدت آنها، و فراوانی ازدواج خویشاوندی در پدر و مادر بیماران انجام شود. طی دو سال، 30734 تولد زنده در زایشگاه های منتخب پیگیری و 4 نوزاد مبتلا به بیماری SMA نوع I شناسایی شدند. درمطالعه ما فراوانی بیماری SMA I، 1 در 7683 و شیوع ناقلان 1 در 43 بود. در پرونده یکی از بیماران نوع خویشاوندی پدر و مادر ذکر نشده بود و با تماس مکرر هم به آنها دسترسی پیدا نکردیم. از سه خانواده دیگر، دو خانواده ازدواج خویشاوندی درجه 3 و درجه 4 داشتند.

    در سال 1892، دکتر بالانتین خیز عمومی (ژنرانیره) زیرجلدی، همراه با تجمع آب در حفره سروزی، را با عنوان هیدروپس جنینی وارد ادبیات پزشکی کرد. حدود پنجاه سال بعد، آسیب شناس مشهور، پوتر، هیدروپس جنینی را به دو گروه ایمونولوژیک و غیرایمونولوژیک تقسیم کرد. در آن زمان بیشتر موارد هیدروپس جنینی ناشی از ناسازگاری خونی (جنین Rh+ با مادر Rh) بود. پس از کشف ایمونوگلوبولین ضد Rh (رگام)، فراوانی گروه ایمونولوژیک به شدت کاهش یافت، تا حدی که در زمان کنونی بیش از 90% موارد هیدروپس جنینی به علت عوامل غیرایمونولوژیک رخ می دهد. تعیین علت و پیشگیری از هیدروپس های جنینی غیرایمونولوژیک بسیار وقت گیر و مشکل است و در غالب موارد امکان پیشگیری وجود ندارد. در مرکز پاتولوژی و ژنتیک ما، از تیرماه 1359 تا اول مهرماه 1388، در میان 1402 مورد کالبدشکافی جنین و نوزاد، 105 مورد هیدروپس جنینی دیده شد که 7 مورد آن (6/6%) را موارد ایمونولوژیک و 67 مورد (8/63%) را موارد غیرایمونولوژیک تشکیل می دادند. در 31 مورد (5/29%) از نمونه های این مرکز موفق به تعیین علت نشدیم. در 12 مورد از این موارد جنین کاملا ماسره بود و امکان بررسی ماکروسکوپی و میکروسکوپی وجود نداشت. در میان موارد غیرایمونولوژیک علل مشخص شده شامل موارد زیر بود: ناهنجاری های کروموزومی (14 مورد)، شامل 7 مورد نشانگان ترنر، 4 مورد نشانگان داون و 3 مورد از سایر اختلالات کروموزومی؛ نارسایی قلبی (13 مورد)، شامل 9 مورد آن ناهنجاری ساختاری قلب و 4 مورد اشکال عملی قلب؛ ناهنجاری های متعدد مادرزادی (12 مورد)؛ ناهنجاری های اسکلتی (8 مورد)؛ عفونت های مادرزادی (7 مورد)؛ ناهنجاری های دستگاه ادراری (7 مورد)؛ اختلالات متابولیک (5 مورد)، شامل یک مورد بیماری پمپه و 4 مورد دیابت مادر؛ کم خونی (4 مورد)؛ ناهنجاری های دستگاه گوارش (2 مورد)؛ و نقص دیافراگم سمت چپ (یک مورد) (سه مورد در دو گروه بصورت مشترک تقسیم بندی شده اند). نقص پرونده های پزشکی مهم ترین عامل عدم دسترسی به علت اصلی هیدروپس جنینی به نظر می رسد. هم چنین، در مواردی که ضایعه عضوی مشخص نباشد، تعیین علت هیدروپس جنینی به تلاش و کنجکاوی پزشکان بالینی و انجام و تکمیل آزمایش های لازم بستگی دارد

    تشخیص پیش از تولد اختلالات کروموزومی با استفاده از روش هیبریدیزاسیون فلوئورسنت درجا (FISH) کاربردهای فراوانی دارد. از روش FISH می توان برای تشخیص انیوپلوئیدی های شایع کروموزوم های 13، 18، 21، X وY استفاده کرد. هم چنین، از این روش برای تشخیص کروموزوم های نامتعادل حاصل از حذف یا جابه جایی هایی که در فرزند پیشین یا پدر و مادر شناسایی شده باشند، استفاده می شود. روش FISH در تایید وضعیت های موزاییک نیز جایگاه خاصی دارد، زیرا امکان شمارش تعداد زیادی سلول را فراهم می کند. در این گزارش، نتایج موارد انجام شده روش FISH در تشخیص پیش از تولد ارائه می شود. در مدت 2 سال، جمعا 60 نمونه جنینی با روش FISH بررسی شدند. از این تعداد، 10 مورد به علت سن بالای بارداری، 29 مورد به علت داشتن شاخص های غیرطبیعی در آزمون های غربالگری و 6 مورد برای داشتن فرزند پیشین دچار اختلال کروموزومی شناخته شده خواهان انجام آزمایش FISH، جهت بررسی انیوپلوئیدی های کروموزوم های 13، 18، 21، X و Y، بودند. در سایر موارد، نمونه ها در 4 نفر ریزحذف اثبات شده در فرزند پیشین، در 4 نفر شک به موزاییسم و در 7 نفر شک به جابه جایی کروموزومی دلیل این بررسی بود. در کلیه موارد پاسخ آزمایش با یک روش دیگر مقایسه شد که در مورد انیوپلوئیدی های کروموزومی شمارش معمول کروموزومی بوده است. در مورد اختلالات ساختاری کروموزومی نیزFISH مجدد متافازی انجام شد. در تمام موارد نتایج دو بررسی مطابقت داشتند. تجربه ما نشان می دهد که این روش برای تشخیص موارد خاص دقت و سرعت قابل قبولی دارد. از مزایای ویژه روش FISH سرعت بالای دست یابی به پاسخ است. به همین دلیل، این روش برای تمام بارداری هایی که در معرض خطر افزون برای اختلالات کروموزومی قابل بررسی با این روش هستند، توصیه می شود.

    فلج مغزی به مجموعه ای از علائم عصبی اطلاق می شود که به اختلال در حرکت و وضعیت کودک منجر می شود. هنوز در بسیاری از محافل پزشکی فلج مغزی را مترادف «آسیب مغزی» می دانند. امروزه شواهد متعدد علمی نشان داده که حوادث پری ناتال در کشورهای پیشرفته به ندرت به عنوان علت فلج مغزی مطرح هستند. مطالعات اخیر نقش پایه ای ژنتیک را در بروز فلج مغزی ثابت کرده است و مانند بیشتر بیماری های نوروژنتیک، نحوه وراثت چندعاملی (مولتی فاکتوریال) برای آن مطرح شده است. این گونه توارث ناشی از تعامل بین عوامل متعدد محیطی و زمینه های متنوع ژنتیکی است. در مراکز پیشرفته، با یک شرح حال و یک الگوریتم تشخیصی دقیق می توان به حدود 75% علل فلج مغزی پی برد. این مساله برای بیماران دچار فلج مغزی و خانواده هایشان اهمیت بسیار دارد و وظیفه پزشک معالج ایشان است که آگاهی های لازم را در مورد علل و احتمال تکرار این عارضه در بارداری های بعدی، به خانواده مبتلایان ارائه کند. در این نوشته کوشش بر آن بوده است که آگاهی ابتدایی در مورد عوامل ژنتیکی مؤثر بر بروز فلج مغزی به خوانندگان ارائه شود.

    ملکول های RNA به واسطه ویژگی های خاص خود قابلیت های قابل توجهی در تنظیم بیان ژن ها دارند. از مهم ترین RNAهای تنظیمی یا ریبورگولاتورها می توان به siRNA، miRNA، ریبوزیم ها، RNA آنتی سنس و ریبوسوئیچ ها اشاره کرد. ملکول های RNA تنظیم کننده قادرند بیان ژن را در سطوح مختلفی، از جمله رونویسی، پس از رونویسی و یا ترجمه، تنظیم کنند. یکی از انواع تنظیم کننده ها که نخستین بار در پروکاریوت ها به آن توجه شد، ریبوسوئیچ ها هستند. ریبوسوئیچ ها دو حوزه (دومین) اپتامر و افکتور دارند. اپتامر می تواند به شکلی اختصاصی به لیگاند متصل شود و عمدتا با تاثیر بر روی ساختار افکتور بر بیان ژن اثر می گذارد. پس از بررسی های اولیه، ریبوسوئیچ هایی نیز در مخمرها و گیاهان شناسایی شد. با توجه به ماهیت تنظیم پذیر ریبوسوئیچ ها و قابلیت آنها در پاسخ گویی به شرایط محیطی، به استفاده از آنها به عنوان حسگر (سنسور) و تنظیم کننده های مؤثر بر بیان ژن ها، توجه شد. به این ترتیب و با تلفیق ویژگی های RNAهای مختلف، اشکال متفاوتی از RNAهای مهندسی شده ساخته می شوند که وجه اشتراک عمده آنها استفاده از خاصیت حسگری RNA است. در این مقاله به معرفی کلی ریبوسوئیچ ها و برخی از حسگرهای مبتنی بر RNA می پردازیم.

    بیشتر از 50 سال وارفارین یکی از کارآمدترین ضد انعقادهای کومارینی خوراکی بوده است. متاسفانه پاسخ بالینی به این دارو چندان قابل پیش بینی نیست و رسیدن به تعادلی برای دست یابی به اهداف بالینی و پیشگیری از عوارض مرتبط با دارو مشکل است. حساسیت به وارفارین پدیده ای چندعاملی و چندژنی است، اما احتمالا ترکیب پلی مورفیسم های ژنتیکی با هم، بیشترین تاثیر را بر حساسیت به وارفارین دارند و در این میان، نقش واریانت های ژنتیکی CYP2C9 و VKORC1 قابل توجه است. سن، وزن، جنسیت، قومیت، مصرف سایر داروها و مکمل های خوراکی و شرایط بالینی فرد، مانند بیماری های مختلف کبدی، نیز بر حساسیت به وارفارین مؤثرند. در این مقاله در مورد عوامل ژنتیکی مؤثر بر حساسیت به وارفارین مطالبی ارائه می شود.

    نشانگان پیرسون صفت نهفته ای است که با سه جزء نشانگان نفروتیک مادرزادی، مشکلات چشمی و یافته های عصبی مشخص می شود. اخیرا جهش در ژن LAMB2، به عنوان عامل نشانگان پیرسون شناسایی شده است. در این مقاله خانواده ای با سه فرزند مبتلا به این نشانگان معرفی می شوند. بررسی ملکولی جهش در ژن LAMB2 را در دو فرزند مبتلا تایید کرده است. نشانگان پیرسون باید در تشخیص افتراقی نشانگان های نفروتیک مادرزادی در نظر گرفته شود. توصیه می شود بیماران دچار علائم نشانگان نفروتیک مادرزادی که فاقد جهش های نفروز مادرزادی نوع فنلاندی هستند، از نظر جهش در ژن LAMB2 بررسی شوند.

    نشانگان فایفر یکی از انواع کرانیوسینوستوزیس با توارث اتوزومی غالب است. علائم این بیماری شامل بسته شدن زودرس شیارهای جمجمه، هیپرتلوریسم، کم عمق بودن کاسه چشم، بیرون زدگی کره چشم، بینی طوطی شکل، انگشت شست پهن و بزرگ در دست و پا و کاهش شنوایی هدایتی است. جهش در ژن های FGFR2 و FGFR1 مسؤول بروز علائم این بیماری است. در این گزارش پسر 2 ساله ای با شکل غیرطبیعی جمجمه، هیپرتلوریسم، بیرون زدگی کره چشم و انگشت شست پهن معرفی می شود. او تنها فرزند یک پدر و مادر غیرخویشاوند است. بر اساس علائم بالینی کودک، بیماری فایفر برای وی مطرح شد. در بررسی ژنتیک ملکولی این بیماری تایید شد.

    Two hundred and eight asymptomatic individuals from different origins of Iran were included in this study in order to assess the distribution of the MTHFR C677T polymorphism responding for cardiovascular disease (CVD) in the general Iranian population using a novel technique reverse hybridization StripAssay for the rapid and simultaneous detection. The test is based on multiplex PCR and hybridization to a teststrip presenting a parallel array of allele-specific oligonucleotide probes for each mutation. The allele frequencies of mutant MTHFR C677T (0.24) in our cohort were quite comparable with those reported by Golbahar et al. (2005) in Iran. Mutant MTHFR C677T in Iran occurred less frequently than among Europeans, but exceeded the much lower frequencies known from India and most of Asia. Here we describe the distribution of mutant allele MTHFR C677T in different ethnicities of Iranian population and compare the results to previously reported data. Our data represent the most comprehensive study to date with respect to thrombophilic gene polymorphism in Iran.

    Spinal muscular atropies are common (genetically determined) disorders. They are heterogeneous both in genetic and phenotypic characteristics. Prevalence of disease was reported between 1 in 6000 to 1 in 25000 in different populations. So, carrier frequency should be 1 in 40 to 1 in 80 in those people. Deletion of both copies of SMA, gene was detected in more than %90 of SMA patients. According to present data there were not any epidemiological study and data in Iran regarding SMAs. So, we do not have accurate information about prevalence, incidence of disease and its carrier frequency. Because of high rate of consanguinity it should be high. With this background, we decided to carry this pilot study out, to determine prevalence, incidence, carrier rate, along with consanguinity rate, clinical spectrum, and molecular abnormality in the affected cases. In a 2 years period, 30734 live-born babies followed, 4 patient affected to SMA type I were detected. Prevalence of SMA I in this population was 1 in 7683, and carrier frequency was 1 in 43. At least 2 third of the newborn babies were product of consanguineous marriages.

    The terminology of hydrops fetalis was introduced by English physician, Ballantyne, in 1892, as generalized subcutaneous edema accompanied by at least one interserosal fluid. Fifty years later, the well known pathologist, Edith L. Potter divided this complex into immune and nonimmune categories. The immune hydrops fetalis comprised quite a number of prenatal mortalities until 1970, when the anti-Rh antigen immune globulin (RhoGAM) was used as a preventive factor, it caused significant decrease of the Immune hydrops fetalis. Nowadays nonimmune hydrops fetalis comprises more than 90 percent of hydrops fetalis cases. Identifying the etiology and prevention of this group is very difficult and time consuming. Among the 1402 prenatal postmortem examinations performed from July 1980 – Oct 2009 in our center, one hundred and ten cases were associated with hydrops fetalis. The largest group are 14 cases of chromosomal aberration including 7 Turner, 4 Down and 3 cases of different aberration followed by 13 cases of cardiac failure, nine cases of abnormality and 4 cases of functional disturbance, the next category consists 8 cases of multiple congenital malformation. Intrauterine infection and urinary tract malformation with 7 cases of each occupied the next group. 5 cases belong to metabolic disease diaphragmatic Hernia and the anemia in 4 cases is the next group, 2 cases belong to GI malformation with one case is in the last position. There are 12 cases of multiple congenital abnormalities. In 31 (29/5%) cases due to the lack of clinical information and paucity of medical documents, which are the most important clue in identifying the etiology of HF, we were unable to clear up the cause of HF. In 12 cases of all those of unknown etiology the body was completely macerated, and unsuitable for gross and histological examination.

    FLUORESCENT in situ hybridization has multiple applications in prenatal diagnosis. It can be used for the detection of common chromosome aneuploidies of chromosomes 13,18,21, X and Y it can be used for the early detection of segmental imbalances detected in former affected offspring or resulting from balanced translocations in the carrier parents. FISH is also a useful tool for ruling in/out of mosaicism detected in routine chromosomal study as it will provide a means of screening and scoring a larger number of cells than theoretically possible with routine chromosomal study. In this report we are presenting results of prenatal diagnosis cases where we have employed FISH techniques for direct analysis or for confirmation of results otherwise obtained. In a two year period 60 FISH tests have been performed on fetal samples. Forty five cases were tested for 5 common chromosomes ten of these cases were referred for advanced maternal age, 29 for abnormal marker screening test results, and 6 for history of former offspring with chromosome aneuploidy. The remaining cases were tested for other reasons including 4 for microdeletion syndromes previously detected in other offspring, 4 for mosaicism suspected in routine chromosomal study, and 7 for possible imbalance resulting from known chromosome translocation in either parent. In all cases, the results were confirmed with another technique, most often the results of routine chromosomal study and in microdeletion cases, reanalysis on metaphases prepared from cultured cells. There was 100% correlation between the results obtained from the two techniques. Our experience shows that this technique has the advantage of a speedy result with considerable reliability for use in the detection of specific cases. We recommend the application of FISH for all high risk pregnancies.

    Cerebral palsy (CP) has been defined as a group of disorders of the development of movement and posture, causing activity limitation, that are attributed to nonprogressive disturbances that occurred in the developing fetus or infant. It is the most prevalent chronic childhood motor disability and is one of the most disabling and costly chronic disorder of children and adults. Prenatal risk factors have been identified in 70-80% of term infants who develop CP, with intrapartum complications such as asphyxia, infections or trauma playing a role only in 10-20% of these infants. There are multiple lines of evidence that strongly point toward genetic factors in CP. This would include familial data, specific identified genetic factors, twin studies, parental age studies, and the high rate of occurrence of multiple minor dysmorphic features in the CP population. Like most medical conditions, CP exhibit complex inheritance. The best model of the inheritance of cerebral palsy is "multi factorial inheritance". This model implies etiologic and genetic heterogeneity with complex interactions with environmental influences. A summary of major categories of prenatal/genetic etiologies in CP would include (1) prematurity (2) primary genetic disorders and syndromes (3) inborn error of metabolism (4) heritable thrombophilias and (5) cerebral dysgenesis and teratogenic influences. In this article we will have a brief discussion of the known genetic influences on the origin of cerebral palsy.

    RNA molecules are employed by diverse mechanisms in the cell. In recent years, researches have demonstrated that RNAs are important as regulators of gene expression. These include antisense RNAs, micro RNAs, siRNAs, ribozymes and riboswitches. Riboswitches are segments of the 5’-untranslated region of some RNAs. These elements act as sensors of signals through mechanisms of molecular recognition. The plasticity of RNA in molecular recognition show high potential of this molecule to act as suitable regulator. According to this ability, researchers attempt to engine riboregulator systems and in last years several riboregulator systems have been engineered. Engineered RNAs could be employed as sensors of intracellular and environmental stimuli to explore the behavior of biological networks and engineered cellular control systems, as antibacterial and as biosensor to detect pathogens. In this review we will focus on some engineered riboregulators and riboswitches as RNA sensors.

    Warfarin has been the mainstay of anticoagulation therapy for more than 50 years. Despite its proven efficacy and low cost, the clinical response to this agent is not predictable and the management of warfarin therapy is challenging because of the large variability in dose requirements and its side effects. Sensitivity to warfarin appears to be multifactorial and polygenic phenomenon. Genetic polymorphisms especially in CYP2C9 and VKORC1 in combination appear to have a major influence on sensitivity to warfarin. Age, sex, weight, disease state, diet, concomitant medications and ethnic affect sensitivity to warfarin. Despite to all limitations because of being multifactor sensitivity to warfarin, prospective studies of genotyping for CYP2C9 and VKORC1 of patients taking warfarin has the clear potential to determine the clinical safe dose of warfarin according to these genetic factors is a practicable goal and set a promising example of personalized medicine. Genetic and non genetic factors affecting sensitivity to warfarin are noticed in this article.

    Pierson syndrome is an autosomal recessive disease characterized by congenital nephrotic syndrome, eye and variable neurological abnormalities. Recently mutations in LAMB2 gene were identified as the molecular cause of Pierson syndrome. In this article, we report on an Iranian family with three offspring affected with Pierson syndrome, in which two were molecularly investigated and a novel homozygous mutation in LAMB2 was identified. We believe that Pierson syndrome should be considered in all infants with congenital nephrotic syndrome, particularly –but not exclusively– if it is associated with ocular anomalies. We recommend molecular investigation of the LAMB2 gene for all infants with congenital nephrotic syndrome who are negative for mutations in the NPHS1 gene defining the congenital nephrotic syndrome, Finnish type.

    Pfeiffer syndrome is one of craniosynostoses syndromes with autosomal dominant inheritance. Clinical findings are craniosynostoses of coronal sutures with ocular hypertelorism, shallow orbits, proptosis, parrot noese, Broad thumbs and big toes and conductive hearing loss. FGFR2 and rarely FGFR1 are the causing genes.In this article, we are reporting a 2 years old boy with abnormal skull, hypertelorism, proptosis, broad thumbs and toes. He is the only child of normal and unrelated parents. The diagnosis based on clinical findings and confirmed by molecular genetic study.
