صفحه 109اهدافبیماران مبتلا به مطالعه ی حاضر با هدف «تعیین تاثیر استراتژی های ارتقای سلامت بر خودکارآمدی بیماران تحت عمل پیوند مغز استخوان در مراقبت از خود» انجام شد.روش هااین مطالعه ی نیمه تجربی قبل و بعد در بیماران کاندید پیوند مغز استخوان در بیمارستان شریعتی شهر تهران در سال 1388 انجام شد. شصت بیمار به صورت تصادفی به دو گروه سی نفره آزمون و کنترل تقسیم شدند. گروه آزمون، آموزش در مورد استراتژی های ارتقای سلامت را دریافت کردند و با گروه کنترل، مطابق با روتین بخش رفتار شد. سپس خودکارآمدی دو گروه قبل و سی روز بعد از مداخله مقایسه شد. تجزیه و تحلیل داده ها با استفاده از آمار توصیفی و آزمون های تی زوج، تی مستقل، تحلیل کواریانس و کای اسکوار در نرم افزار SPSS11.5 انجام شد.یافته هااختلاف معنی داری بین متغیرهای دموگرافیک دو گروه مشاهده نشد. میزان خودکارآمدی گروه آزمون در مقیاس کلی و ابعاد سازگاری، تصمیم گیری و کاهش استرس نسبت به گروه شاهد و همچنین نسبت به خودشان قبل از آموزش، افزایش یافت (001/0p<). تفاوت نمرات خودکارآمدی در همه ابعاد نسبت به قبل از مداخله از لحاظ آماری معنی دار بود (05/0>p).نتیجه گیریآموزش استراتژی های ارتقای سلامت به بیماران پیوند مغز استخوان، می تواند سبب افزایش اطمینان این بیماران در انجام مراقبت از خود در فرایند درمان شود که در نهایت بقای بیشتر به همراه کیفیت زندگی بالاتری را در این بیماران در پی خواهد داشت سرطان های خون، پس از پیوند مغز استخوان، با مشکلات عدیده ای مواجه می شوند که بر خودکارآمدی آن ها در مراقبت از خود تاثیر منفی می گذارد.

ریختن نرمال سالین داخل تراشه، قبل از ساکشن کردن، یک مداخله ی سنتی پرستاری است. این مطالعه با هدف «بررسی تاثیر ساکشن لوله تراشه، با محلول نرمال سالین و بدون محلول نرمال سالین بر ضربان قلب و میزان اشباع هموگلوبین» انجام شد.
روش هااین مطالعه ی نیمه تجربی با طرح متقاطع بر روی پنجاه بیمار ترومایی بخش مراقبت های ویژه بیمارستان شهید باهنر شهر کرمان در سال 1388 انجام شد. برای هر بیمار، ساکشن یک بار با پنج میلی لیتر محلول نرمال سالین و یک بار بدون محلول انجام شد. ضربان قلب و میزان اشباع هموگلوبین به ترتیب یک، دو و پنج دقیقه قبل و بعد و حین ساکشن توسط دستگاه های مونیتورینگ استاندارد اندازه گیری و ثبت شد. برای تجزیه و تحلیل داده ها از آزمون های تی زوج و تی مستقل و SPSS17 استفاده شد.
یافته هادر هر دو گروه ضربان قلب بعد از ساکشن افزایش داشت و در گروهی که نرمال سالین دریافت کرده بود اندکی بیشتر بود ولی تفاوت آماری معنی دار نبود. تغییرات منفی اشباع هموگلوبین در گروهی که از نرمال سالین استفاده شد در مقایسه با گروهی که استفاده نشد بیشتر بود (05/0> p).
نتیجه گیریمحلول نرمال سالین قبل از ساکشن می تواند کاهش اشباع هموگلوبین با اکسیژن را به همراه داشته باشد و بهتر است برای رقیق کردن ترشحات از راه کارهای مناسب تری مثل مرطوب کردن هوای دمی استفاده کرد تا عوارض آن به حداقل برسد.

صفحه 121اهدافآموزش و تسلط کامل رزمندگان در امدادرسانی به خود و دیگر هم رزمان یکی از مؤثرترین روش ها برای بالا بردن ایمنی رزمندگان در عملیات های نظامی است. این پژوهش با هدف «بررسی تاثیر آموزش خود امدادی و دگر امدادی به دو روش سخنرانی و بسته ی نرم افزار چند رسانه ای بر میزان عملکرد پرسنل نظامی» انجام شد.روش هادر این مطالعه ی نیمه تجربی، شصت نفر از پرسنل نظامی از گردان های رزم منتخب شهر تهران و مشهد، در سال 1389 مورد بررسی قرار گرفتند. نمونه ها به دو گرو سی نفره به صورت تصادفی تقسیم شدند و تحت دو شیوه ی آموزش سخنرانی و بسته ی نرم افزاری چند رسانه ای قرار گرفتند و در انتها عملکرد پایوران دو گروه با آزمون عملی ساختار دار بالینی بررسی شد. داده ها با استفاده از آزمون آماری ناپارامتریک و نرم افزار SPSS15 مورد تجزیه و تحلیل قرار گرفت.یافته هادر ایستگاه های «کنترل علایم حیاتی»، «روش حمل مجروح»، «بانداژ سر» و «رفع آلودگی و مراقبت در حملات بیولوژیک»، در دو گروه مورد مطالعه تفاوت معنی داری مشاهده نشد (05/0 (p>. ولی در ایستگاه های «احیای قلبی و ریوی»، «اقدامات اولیه در سوختگی چشم» و «آتل بندی» نتایج نشان دهنده ی تفاوت معنی دار آماری با استفاده از آمار یومن ویتنی بود (001/0 > p). در ایستگاه «کنترل خونریزی» نتایج گروه سخنرانی بهتر از گروه نرم افزار بود (05/0 (p<.نتیجه گیریبا توجه به مؤثرتر بودن نرم افزار چندرسانه ای در بعضی از ایستگاه ها به خصوص احیای قلبی ریوی و با در نظر داشتن محدودیت های استفاده از روش سخنرانی، گسترش استفاده از نرم افزار چندرسانه ای در آموزش خودامدادی و دگرامدادی توصیه می شود.

صفحه 127اهداف: خواب یکی از نیازهای اساسی انسان است و محرومیت از خواب اثرات سوء متعددی بر جسم و روان انسان می گذارد. با توجه به کاهش کیفیت خواب در بیماران سندرم حاد کرونری، این مطالعه با هدف” بررسی تاثیر گوش بند بر کیفیت خواب بیماران” انجام گرفت.روش هادر این مطالعه کارآزمایی بالینی دو گروهی شاهددار، شصت بیمار سندرم کرونری حاد در بخش های مراقبت ویژه قلب بیمارستان بقیه الله (عج) شهر تهران در سال 1389 به روش مبتنی بر هدف انتخاب و با روش تخصیص تصادفی در دو گروه آزمون و شاهد توزیع شدند. در گروه آزمون، از شب دوم بستری، هر شب مداخله گوش بند انجام گرفت و با استفاده از پرسش نامه ی کیفیت خواب پیترزبرگ، کیفیت خواب بیماران در طول بستری و در پایان مدت بستری مورد ارزش یابی قرار گرفت. سپس داده ها با آزمون تی زوج شده، تی مستقل و ضریب همبستگی اسپیرمن و پیرسون و نرم افزار SPSS19 تجزیه و تحلیل شد.یافته هانمره ی کل کیفیت خواب در گروه آزمون، پس از مداخله (1/2 ± 3/6) نسبت به قبل از مداخله (3/6 ± 3/10) کاهش معنی داری یافت (001/0 p<). همچنین نمره ی کل کیفیت خواب پس از مداخله در گروه آزمون (1/2 ± 3/6) در مقایسه با نمره ی کل کیفیت خواب کسب شده در گروه شاهد (9/1 ± 4/8) تفاوت معنی داری داشت (001/0 p<).نتیجه گیریاستفاده از گوش بند به عنوان یک روش اقتصادی و بدون عارضه می تواند موجب بهبود کیفیت خواب بیماران سندرم کرونری حاد در بخش مراقبت ویژه قلبی شود و به عنوان روشی جای گزین به جای استفاده از درمان دارویی به کار رود.

صفحه 135اهدافپرستاران به عنوان بزرگترین گروه ارایه دهنده خدمات بهداشتی باید از کیفیت زندگی کاری مطلوبی برخوردار باشند تا بتوانند مراقبت ها را به شکل مطلوب به مددجویان ارایه دهند. مطالعه ی حاضر با هدف «بررسی ارتباط مشکلات روان شناختی برکیفیت زندگی کاری پرستاران بخش های مراقبت ویژه» انجام شد.روش هااین پژوهش توصیفی همبستگی از نوع مقطعی بر روی 143 نفر از پرسنل پرستاری شاغل در بخش های ویژه بیمارستان های در شهر تهران در سال 1389 انجام شد. اطلاعات با استفاده از سه پرسشنامه شامل ویژگی های جمعیت شناختی، پرسش نامه ی کیفیت زندگی کاری بر اساس مدل والتون و پرسش نامه ی استاندارد DASS21 جمع آوری شد. اطلاعات توسط نرم افزار SPSS16 و با استفاده از آزمون ضریب همبستگی پیرسون و آزمون کای اسکوار مورد تجزیه و تحلیل قرار گرفت.یافته هابین کیفیت زندگی کاری با استرس (07/0= p) و افسردگی (05/0= p) همبستگی مشاهده نشد ولی با اضطراب همبستگی معکوس ضعیفی وجود داشت (27/0- = r و 002/0= p)نتیجه گیریبا توجه به تاثیر اضطراب بر کاهش کیفیت زندگی کاری پرستاران، توجه به اضطراب پرستاران بخصوص توسط مدیران ضروری است.

صفحه 141اهدافبستری شدن در بیمارستان، به خصوص در بخش های ویژه، با توجه به شرایط و ویژگی های خاص خود، می تواند تنش زا باشد. هدف این مطالعه «تعیین عوامل تنش زای ناشی از بستری شدن در بیماران بخش مراقبت های قلبی» بود.روش هااین مطالعه ی توصیفی مقطعی به روش نمونه گیری در دسترس بر روی 150 بستری در بخش مراقبت ویژه قلب بیمارستان امام سجاد (ع) رامسر درسال 1388 انجام شد. ابزار گردآوری اطلاعات، شامل مشخصات فردی و پرسش نامه ی والیسر بود. برای تحلیل آماری از آزمون پارامتری با نرم افزار SPSS18 استفاده شد.یافته هابالاترین عامل تنش زای جسمی، اتصال به مانیتور و بی خوابی و درزمینه ی عوامل تنش زای روانی، مشکل در انجام تکالیف شرعی و درخواست لوله ادرار و لگن و در زمینه ی تنش زای محیطی، صدای گفتگوی دیگران بود. بین میانگین نمرات عوامل تنش زای روانی با جنس، وضعیت اشتغال و میزان تحصیلات، ارتباط معنی دار مشاهده شد (05/0 p<) و همچنین میان میانگین نمرات عوامل تنش زای جسمی با وضعیت اشتغال و تعداد بستری در سال گذشته ارتباط معنی دار مشاهده شد (05/0 p<).نتیجه گیرییافته ها می تواند به پرستاران در جهت شناسایی عوامل تنش زا و ارتقای کیفیت مراقبت پرستاری کمک کند.

صفحه 149اهدافدر درمان های جای گزین کلیه، هدف تنها طولانی کردن زندگی و حفظ سلامتی نیست، بلکه حفظ و ارتقای کیفیت زندگی را نیز در بر می گیرد. این مطالعه، با هدف «بررسی وضعیت کیفیت زندگی بیماران پیوند کلیه» انجام شد.روش هااین مطالعه ی توصیفی مقطعی در سال 1388 بر روی 220 نفر از بیماران پیوند کلیه در شهر تهران، با روش نمونه های در دسترس انجام شد. ابزار گردآوری داده شامل فرم جمع آوری اطلاعات فردی و پرسش نامه اندازه گیری کیفیت زندگی (SF-36) است. از آزمون های آماری توصیفی و استنباطی و نرم افزار آماری SPSS14 برای تحلیل داده ها استفاده شد.یافته هامیانگین کیفیت زندگی در این بیماران 7/18 ± 6/60 و بیشترین و کمترین نمره به ترتیب در ابعاد درد 84/28 ± 27/66 و محدودیت در ایفای نقش به دلایل جسمی 91/36 ± 67/46 به دست آمد. بین نمره ی کل کیفیت زندگی بر حسب جنسیت، تفاوت معنی دار (001/0=p) مشاهده شد اما در بررسی ارتباط سایر متغیرهای جمعیت شناختی و نمره ی کل کیفیت زندگی ارتباط معنی دار مشاهده نشد (05/0نتیجه گیریدر این بررسی، میانگین کیفیت زندگی بیماران اندکی بیشتر از حد متوسط بود که با توجه به هزینه های پیوند و تحمیل فشارهای روانی و مالی زیاد بر بیماران و خانواده ی ایشان، لزوم شناسایی مسایل و مشکلات پیش روی این بیماران و نیز مسایل مختل کننده ی کیفیت زندگی ایشان، بیش از پیش روشن می شود.

صفحه 153اهدافسندرم پاهای بی قرار، یک اختلال حرکتی است که با میل شدید به حرکت دادن پاها هنگام استراحت مشخص می شود. بیماران مبتلا، اغلب تکان های ماهیچه ای را تجربه می کنند که سبب مختل شدن خواب می شود. میزان شیوع سندرم پاهای بیقرار در بیماران بخش های مراقبت ویژه بیشتر از مردم عادی است. از این رو این مطالعه با هدف «بررسی ارتباط بین سندرم پاهای بی قرار با اختلال خواب» انجام شد.روش هادر پژوهش توصیفی تحلیلی حاضر، سیصد بیمار بستری در بخش های ویژه بیمارستان های وابسته به دانشگاه علوم پزشکی شهرستان ارومیه در سال 1389به صورت نمونه گیری در دسترس انتخاب شدند. برای گردآوری داده ها از پرسش نامه های اطلاعات دموگرافیکی، غربال گری سندرم پاهای بی قرار، شدت سندرم پاهای بی قرار و کیفیت خواب استفاده شد. برای تحلیل اطلاعات از نرم افزار SPSS16 و آزمون های آماری توصیفی و استنباطی استفاده شد.یافته ها3/36 درصد بیماران بستری در بخش های ویژه، دارای نشانه های سندرم پاهای بی قرار بودند. در گروه دارای اختلال خواب 7/52 درصد و در گروه فاقد اختلال خواب بیست درصد مبتلا به سندرم پاهای بی قرار بودند. شانس ابتلا به سندرم پاهای بی قرار در افراد دارای اختلال خواب 45/4 برابر افرادی بود که فاقد اختلال خواب بودند و شانس ابتلا به سندرم پاهای بی قرار در افراد دارای اختلال خواب به مراتب بیشتر از افرادی است که فاقد اختلال خواب بودند.نتیجه گیریبا توجه به شیوع بالای سندرم پاهای بی قرار و اختلال خواب در بیماران بستری در بخش های مراقبت ویژه، استفاده از روش های غربالگری برای شناسایی آن ها و تغییر در سبک زندگی برای کاهش عوامل خطر در این بیماران ضروری به نظر می رسد.

Page 109AimsPatients with blood cancer are encountering numerous problems following bone marrow transplantation which exert negative impact on self-care self efficacy. The present study has been conducted to determine the effect of health-promotion strategies on self-care self-efficacy among patients undergoing bone marrow transplantation.MethodsThis before- after semi experimental study was performed among patients candidate for bone marrow transplantation in Shariati Hospital of Tehran in 2009. Sixty patients were randomly allocated into two groups of 30 participants each as experimental and control groups; the former received education on health promotion strategies and the latter was treated according to the ward's routine. Self efficacy was compared between the two groups before and thirty days after the intervention. Data were analyzed by SPSS11.5 statistical software using descriptive statistics, independent and paired t-test, covariance analysis and chi-square.ResultsNo significant difference was observed between the two groups in terms of demographic variables. The self-efficacy scale was increased in general comparison as well as adaptability, decision-making and stress reduction parameters in control with respect to the experimental group and also at post compared to pre-intervention in both groups (p<0.001). The mean self-efficacy scores was statistically different in all dimensions after in comparison with before the intervention (p<0.05).ConclusionEducation on health promotion strategies to bone marrow transplant patients can increase patient's confidence in performing self-care processes and consequently brings more survival along with higher quality of life among this group.

Pre-suctioning normal saline instillation into the trachea is a traditional nursing intervention. In this line, the present study has been carried out to “evaluate the effect of endotracheal suctioning with and without normal saline solution on heart rate and oxygen saturation”.
MethodsThis semi-experimental study with crossover design was conducted on 50 trauma patients admitted to critical care unit of Shahid Bahonar Hospital of Kerman in 2010. For each patient, suctioning was performed once without and once with 5ml normal saline solution. Heart rate and oxygen saturation were measured and recorded at one, two and five minutes before and after suctioning by standard monitoring apparatus. Data were analyzed by SPSS17 statistical software using independent and paired t-tests.
ResultsHeart rate was increased in both groups after the suctioning; however, it was slightly more in group receiving normal saline but not statistically significant. Negative changes in oxygen saturation were higher in saline group compared to the other (p<0.05).
ConclusionNormal saline instillation prior to suctioning will be accompanied by reduced oxygen saturation, and it is recommended that more suitable approaches, such as moisturizing the inhaled gas, be used for diluting secretions to minimize the related side effects.

Page 121AimsWarrior's education and complete domination over self-aid and buddy-aid is one the most effective approaches for enhancing their safety in military operations. The present study has been performed to “investigate the effect of self-aid and buddy-aid education using lecture and multimedia software package on performance level of military personnel”.MethodsIn this semi-experimental study in 2010, sixty subjects from the selected military battalion of Tehran and Mashhad were investigated. Participants were randomly allocated into two groups of thirty each and were educated using lecture and multimedia software package. In the end, they were evaluated in terms of performance level through objective structured clinical examination. Data were analyzed by SPSS15 statistical software and nonparametric statistical tests.ResultsAt "vital signs monitoring," "carrying method of injured," "head bandage," "self-care and decontamination of biological attacks," stations, no significant difference was observed between the two groups (p>0.05); while, in "cardiac and pulmonary rehabilitation," "first-aid measures for eye burns," and "splinting" stations, the results indicated a statistical difference by Man-Whitney (P<0.001). In "bleeding monitoring" station, lecture-trained group showed more acceptable results compared to software-educated group (p<0.05).ConclusionRegarding more effectiveness of multimedia software in some stations, especially CPR, and considering lecture-related limitations, expanding multimedia software application is recommended in self-aid and buddy-aid education.

Page 127AimsSleep is one of the basic human needs and sleep deprivation causes numerous adverse effects on the human body and mind. Due to reduced sleep quality in patients with acute coronary syndrome, this study was carried out to " determine the effect of earplugs on sleep quality in patients with acute coronary syndrome. "MethodsIn this two-group controlled clinical trial, sixty patients with acute coronary syndrome in the coronary care units of Baqiyatallah hospital in Tehran in 2010, were selected by purposeful sampling method and randomly allocated in two groups of case and control. In the case group, in the second night stay, the intervention of earplugs was done per night and by using the Petersburg's sleep quality questionnaire; sleep quality was evaluated during and at the end of hospitalization. Then data were analyzed by paired T test, independ T test, Spearman and Pearson's correlation coefficient and SPSS 19 software.ResultsTotal sleep quality, score in case group was significantly decreased after intervention (6.3±2.1) than before intervention (10.3±6.3) (p<0.001). Also, total score of sleep quality after intervention in case group (6.3±2.1) has significant different compared to control group (9.1±4.8) (p<0.001).ConclusionUsing earplugs, as an economical and uncomplicated method, can improve sleep quality in patients with acute coronary syndrome in the coronary care units and can be used as an alternative method of treatment instead of drug therapy.

Page 135AimsNurses as the major group of health service providers need to have the satisfactory quality of work life in order to give desirably care for the patients. The purpose of this research has been to study the “Relationship between psychological problems and quality of work life of intensive care unit nurses”.MethodsThis cross-sectional study which is descriptive –correlation was accomplished on 143 nurses working in intensive care unit of Tehran hospitals in 2010. Data with using of three questionnaires including demographic characteristics, questionnaire of quality of work life based on Walton’s model and the standard questionnaire of DASS21 was gathered. The information was analyzed using SPSS16 software and Pearson correlation test and chi-square test.ResultsThere has been no correlation between quality of work life and stress (p=0.07) and depression (p=0.05), but there has been a weak inverse correlation with anxiety (p=0.002, r=-0/27).ConclusionIt is essential to consider nurses’ anxiety especially by directors due to anxiety impact on reduction of nurses’ quality of work life.

Page 141AimsHospitalization, especially in intensive care unit, because of its specific conditions and characteristics can be stressful. The purpose of this study was to determine the stressors associated with hospitalization in the cardiac care unit patients.MethodsThis cross-sectional study was conducted by convenience sampling method on 150 patients who were hospitalized in the cardiac care unit of Ramsar’s Imam Sajjad Hospital, in 2009. Data collection tools included personal details and the Valiser questionnaire. To analyze the gathered data, a parametric test was carried out and SPSS18 software was utilized.Results. The highest physical stressful factor was insomnia which was a result of being connected to a monitor; the main psychological stressor was the difficulty in saying prayers and requesting catheter and bedpan; and finally the major environmental stressor was the voice of others’ talking. A significant relationship was observed between the psychological stressors scores and gender, employment status and educational level of patients (p<0.05). There was also a meaningful relationship between the physical stressors scores and employment status of patients and the number of hospitalized people in the past year (p<0.05).ConclusionThe findings can help nurses to identify stressors and toimprove the quality of nursing care.

Page 149Aimes: In renal replacement therapies, the goal is not only prolonging life and maintaining health, but also maintaining and improving the quality of life. This study seeks to "evaluate the quality of life of kidney transplant patients."MethodsThis cross-sectional investigation, which was performed in 2009 in Tehran, studied 220 patients with kidney transplant and was conducted by the method of convenience sampling. Data collection tools include a demographic data collection form and a questionnaire which measured quality of life (SF-36). Data were analyzed log SPSS14 statistical software and descriptive and inferential statistical tests.ResultsThe mean quality of life in these patients was 60.6 ± 18.7, the highest and lowest scores respectively on the pain scale were 66.27 ± 28.84, and working limitations due to physical disabilities were 46.67 ± 36.91. A significant relation was found between the overall quality of life score and gender (p=0.001). However, no significant relation was observed between the overall quality of life score and other demographic variables (p>0.05).ConclusionThe patients’ average quality of life, in this study, was a bit more than medium. Regarding transplant expenses and psychological and financial pressures on patients and their families, it seems vital to recognize the difficulties and impediments that the patients confront in their lives.

Page 153AimsRestless Legs Syndrome is a movement disorder characterized by a compelling urge to move the legs during rest or inactivity. Patients often experience muscle twitching leading to sleep disturbance. Restless legs syndrome is more prevalent among patients in critical care units than ordinary people. Hence, the present study has been implemented to "investigate the correlation between restless legs syndrome and sleep disturbance."MethodsIn this descriptive-analytic study in 2010, 30 patients admitted to critical care units of University of Medical Sciences affiliated hospitals of Urmia city were selected using available-sampling method. Data-collecting instruments were information-included questionnaires on demographic characteristics, screening as well as the intensity of restless legs syndrome, and sleep quality. Data were analyzed by SPSS16 statistical software and descriptive and inferential statistical tests.Results36.3% of patients in critical care units showed signs of restless legs syndrome. In groups with and without sleep disturbance, respectively 52.7% and 20% of participants were suffering from restless legs syndrome. The syndrome incidence was 4.45-fold higher in patients with than those without sleep disorder. Findings revealed that developing the mentioned syndrome is far more probable in people with compared to those without sleep disturbance.ConclusionGiven the high prevalence of restless legs syndrome and sleep disorder in critical care units patients, using the screening methods seems to be necessary for disorder identification and lifestyle alteration to reduce the associated risk factors among these patients.
