صفحه 193زمینه و هدفهدف از انجام این پژوهش بررسی عوامل اجتماعی و روان شناختی موثر بر میزان مصرف مواد آرایشی در بین دانشجویان دختر بوده است.
روش اجرا: 250 نفر از دانشجویان دختر دانشگاه های علوم پزشکی اصفهان و دانشگاه اصفهان با روش نمونه گیری تصادفی ساده انتخاب شدند و به پرسش نامه های پژوهش پاسخ دادند. در این پژوهش از پرسش نامه شخصیتی NEO جهت سنجش ویژگی های شخصیتی آزمودنی ها استفاده شد. عوامل اجتماعی که در این پژوهش به عنوان متغیر مستقل مورد بررسی قرار گرفتند عبارت بودند از مدیریت بدن، مصرف رسانه، دینداری، پایگاه اجتماعی اقتصادی و پذیرش اجتماعی.یافته هارابطه ی متغیر های مدیریت بدن، مصرف رسانه، دینداری، پایگاه اجتماعی اقتصادی و پذیرش اجتماعی با میزان مصرف مواد آرایشی معنادار بود. هم چنین نتایج حاصل از تحلیل رگرسیون نشان داد که متغیرهای پیش بین 41% از واریانس میزان مصرف مواد آرایشی را تبیین می کنند و متغیر مدیریت بدن بهترین پیش بین بوده است.نتیجه گیریعامل اجتماعی مهم تر از ویژگی های شخصیتی در مصرف مواد آرایشی توسط دانشجویان دختر می باشند.

صفحه 203زمینه و هدفویتیلیگو یکی از بیماری های مزمن پوستی است که با لکه های بدون رنگ ناشی از ازبین رفتن ملانوسیت های پوست مشخص می شود. ویتیلیگو اختلال فیزیکی زیادی به همراه ندارد، اما مشکلات زیبایی فراوانی را به وجود می آورد که با اختلالات روحی و روانی بسیاری برای بیماران همراه است. هدف مطالعه ی کنونی بررسی افسردگی و اختلال کیفیت زندگی در بیماران ویتیلیگو مراجعه کننده به بیمارستان رازی در سال های 1388و 1389 بود.
روش اجرا: از دی ماه 1388 تا دی ماه 1389، از 100 بیمار مبتلا به ویتیلیگو که به روش تصادفی ساده از درمانگاه سرپایی بیمارستان رازی انتخاب شده بودند، درخواست شد که به پرسش نامه های روا و پایای Beck Depression Inventory)، SF-36 و DLQI (Dermatology Life Quality Index پاسخ دهند.یافته هامیانگین سنی بیماران 5/11±9/28 سال و میانگین زمان ابتلا به بیماری 5/5±7/5 سال بود. میانگین نمره ی Beck، SF-36، DLQI همه ی بیماران 8/11±9/15، 4/19±8/63 و 9/6±4/8 بود. درحالی که ارتباط معنی داری بین نمره های DLQI و SF-36 و طول مدت بیماری وجود داشت (به ترتیب 3/0r= و 001/0>P و 263/0=r و 01/0=P)، نمره های هیچ یک از پرسش نامه ها با سن ارتباطی نداشت.نتیجه گیریتوصیه می شود ارزیابی کیفیت زندگی و افسردگی در بیماران مبتلا به ویتیلیگو جهت بررسی مشکلات ناشی از بیماری بر زندگی آن ها انجام گیرد.

صفحه 210زمینه و هدف(consumption of the epidermis (COE با نازک شدن اپی درم و از دست دادن رت ریج های مجاور با دسته های سلول های بدخیم ملانوسیتی مشخص می شود. هدف از این مطالعه، بررسی اهمیت COE به عنوان یک معیار تشخیصی برای ملانوم بدخیم و ارزیابی ارتباط آن با یافته های clinicopathological بوده است.
روش اجرا: در این پژوهش سن، جنسیت، محل ضایعه و پارامترهای آسیب شناسی مانند نوع تومور، ضخامت Breslow، وجود زخم، سطح Clark و COE در 90 مورد ملانوم مورد بررسی قرار گرفت.یافته هابرخلاف دیگر مطالعات، COE در بیماران با acral lentiginous melanoma و در نواحی آکرال شایع تر بود.نتیجه گیریگرچه COE می تواند به عنوان یک معیار آسیب شناختی در تشخیص ملانوم بدخیم استفاده شود، اما همبستگی COE با وجود زخم و سایر عوامل پیش آگهی در ملانوم وجود ندارد.

صفحه 215زمینه و هدفواکنش های دارویی ناخواسته یک پیامد اجتناب ناپذیر از درمان های دارویی می باشند که باعث مرگ و ناتوانی بیماران می گردد. واکنش های دارویی پوستی تقریبا در3-2% از بیماران بستری در بیمارستان دیده شده ولی تنها 2% آن ها شدید و کشنده می باشند. در این مطالعه بر آن شدیم تا به بررسی دو ساله ی بیماران بستری و سرپایی مبتلا به واکنش های دارویی پوستی ازنظر نوع تظاهرات بالینی، سن، جنس و نوع داروی مصرفی بپردازیم.
روش اجرا: این مطالعه، یک مطالعه ای مقطعی بود که بر روی 308 بیمار با تشخیص واکنش های دارویی ناخواسته ی پوستی مراجعه کننده به بیمارستان فرشچیان همدان طی دو سال انجام شد. در ابتدا از کلیه ی بیماران مشکوک، شرح حال کامل بالینی و دارویی گرفته شد و سپس توسط متخصص پوست مجرب معاینه شده و ضایعات منطبق با اشکال بالینی قطعی واکنش های دارویی وارد مطالعه شدند. اطلاعات گردآوری شده وارد نرم افزار SPSS و داده ها با استفاده از آمار توصیفی و آزمون t توصیف و تحلیل شدند.یافته هادر بین 308 بیمار واردشده در مطالعه 114 بیمار مرد (37%) و 194 بیمار خانم (63%) بودند، میانگین سنی مجموع شرکت کنندگان (میانگین±انحراف معیار) 8/16±3/35 سال بود. 163 بیمار (9/52%) مبتلا به کهیر حاد دارویی، 57 بیمار (5/18%) با بثورات ثابت دارویی و 46 بیمار (9/14%) مبتلا به بثورات اگزانتماتیک بودند. آنتی بیوتیک ها (7/42%) و سپس داروهای ضدالتهاب غیراستروئیدی (5/16%) بیشترین داروی مصرفی در بیماران بوده است.نتیجه گیریاین مطالعه نشان داد واکنش دارویی بیشتر در خانم ها رخ داده و آنتی بیوتیک ها به ویژه بتالاکتام ها و داروهای ضدالتهاب غیراستروئیدی شایع ترین داروهای ایجادکننده ی واکنش های دارویی ناخواسته پوستی می باشند. هم چنین کهیر دارویی شایع ترین نوع بالینی واکنش دارویی بوده است. این یافته مغایر با سایر مقالات بوده است.

صفحه 221زمینه و هدفآلوپسی آندروژنتیک یکی از اختلالات شایع پوستی می باشد که تغییرات هورمونی و عوامل ژنتیک در بروز آن موثر می باشد. شایع ترین شکل ریزش موی پیشرونده منتشر و قرینه می باشد که هر دو جنس را مبتلا می کند. این مطالعه با هدف بررسی شیوع آلوپسی آندروژنتیک در دختران 18-14 ساله ی شهر شیراز انجام گردیده است.
روش اجرا: این مطالعه ی مقطعی در سال 1388 در مناطق مختلف آموزش و پرورش شهر شیراز روی 3190 دانش آموز 18-14 ساله انجام شد. انتخاب دانش آموزان به با استفاده از روش نمونه گیری آسان صورت گرفت. در این پژوهش آلوپسی، موارد متوسط و شدید کاهش تراکم موهای فرق سر و خط پیشانی در نظر گرفته شد. جهت ارتباط آلوپسی با سندرم تخمدان پلی کیستیک، اندازه گیری تستوسترون خون و سونوگرافی انجام شد. پس از جمع آوری داده های مطالعه با استفاده از نرم افزار SPSS و با به کاربردن آزمون های مربع کای، دقیق فیشر و t تجزیه شدند.یافته ها135 نفر (2/4 درصد) از کل افراد مورد مطالعه براساس طبقه بندی Ludwig آلوپسی متوسط تا شدید داشتند. ارتباط آماری معنی داری بین سطح تستوسترون و تستوسترون آزاد خون و الگوی ریزش آلوپسی به دست نیامد (05/0نتیجه گیریاین مطالعه ارتباط آماری معنی داری بین مبتلایان به آلوپسی با هورمون های آندروژنتیک و سندرم تخمدان پلی کیستیک نشان نداد. انجام مطالعه در گروه سنی بالاتر توصیه می شود.

صفحه 229در جامعه ی بشری تغییرات بدفرم و ناهنجاری های پوستی ناشی از سوختگی و جراحات حاصل از جراحی و حوادث و امثال آن ها در فرد بازتاب های روانی و رفتاری ناخوشایندی ایجاد می کنند. ماتریکس خارج سلولی ([extra cellular matrix [ECM) بزرگ ترین جزء تشکیل دهنده ی پوست طبیعی است که ژل مانند است و به وسیله ی سلول های پوست تولید می شود. ساخته شدن ECM یک عامل کلیدی برای پرشدن ضایعات پوستی از قبیل سوختگی، سالک، آبله مرغان و محل جراحات و جوش است. اجزاء ماتریکس خارج سلولی عبارت اند از: پلی ساکارید های مختلف، آب و پروتئین های کلاژنی. قدرت انبساط و استحکام پوست طبیعی با در نظر گرفتن وزن آن نزدیک به قدرت آهن است در حالی که دارای الاستیسیتی و قابلیت تراکم پذیری بالایی می باشد. این ویژگی ها به دلیل اثر توام دو دسته ی اصلی از مولکول های تشکیل دهنده ی ECM می باشد که به وسیله ی فیبروبلاست و سلول های اپی درم ترشح می شوند: 1) پروتئین های ساختاری فیبری شکل مانند: کلاژن ها، الاستین، فیبرونکتین و لامینین که به ECM استحکام و حالت ارتجاعی می بخشند. 2) پروتئوگلیکان ها مثل درماتان سولفات و هیالورونیک اسید، که معمولا از چندین رشته گلیکوزآمینوگلیکان که از یک هسته ی پروتئینی منشعب می شوند. پروتئوگلیکان ها مولکول های بزرگ و جاذب آب هستند که حالت ضربه گیری ECM را برای حفظ سلول ها ایجاد می کنند. در کل اطلاع از ساختار و مراحل ترمیم پوست ما را در طراحی آزمایشات و انجام تحقیقاتی دقیق تر در این راستا کمک می کند.

صفحه 245درماتولوژی تمام جنبه های بیماری هایی را که روی پوست و ضمایم آن شامل مو، ناخن، غدد عرق و غشاهای مخاطی دهانی و تناسلی خارجی رخ می دهند را دربرمی گیرد. در این رشته تخصصی، قریب 3000 اختلال و بیماری وجود دارد که از نظر سبب شناختی طیف گسترده ای را دربرمی گیرند و برخی از آن ها از شیوع و میزان بروز بالایی در جوامع برخوردارند. علی رغم شیوع بالای این گروه از بیماری ها، که با توجه به درگیر کردن سطح بدن با ایجاد تنش و اضطراب، اختلال کیفیت زندگی و نهایتا بار بیماری، در سطح فرد و متعاقبا خانواده و اجتماع همراه هستند، غالبا اهمیت این بیماری ها از منظر بهداشت عمومی و سلامت جامعه نادیده گرفته می شود، بنابراین بر آن شدیم تا بیان مثال هایی از وضعیت بیماری های مربوط به حیطه ی درماتولوژی در کشورها و جمعیت های مختلف و تاثیر آن ها بر سلامت جامعه، از منظر بار بیماری و اقتصاد سلامت، به دلایل بی توجهی به این رشته ی مهم که به عواقب نامطلوب متعددی در حیطه ی بهداشت عمومی منجر می شود، بپردازیم. هم چنین، راهبردهایی نیز برای اصلاح وضع موجود پیشنهاد می شود.

Page 193Background And AimThe purpose of this study was to investigate the psychological and sociological factors influencing cosmetics usage by female students.MethodsThe statistical population comprised 250 female students of Isfahan University of Medical Sciences and Isfahan University were selected using a simple random sampling method and were asked to respond to the research questionnaires. NEO Personality Inventory was used to assess personality characteristics. Sociological factors that were assessed were comprised of body management, media consumption, religiousness, socio-economic status and social acceptance.ResultsThere was a significant association between body management, media consumption, religiousness, socio-economic status and social acceptance with cosmetics use. Regression analysis showed that predictive variables were able to predict 41% of attitude toward cosmetics use and body management was the best predictor of it.ConclusionSociological, but not psychological, factors influence cosmetics use by female university students.

Page 203Background And AimVitiligo is a chronic skin disease which is characterized by depigmented patches due to loss of melanocytes. On contrary of low physical impairment, vitiligo causes a lot of cosmetic problems which can lead to psychological problems. The aim of this study was to evaluate the quality of life and depression in Iranian patients with vitiligo, who were referred to Razi Hospital in 2009 and 2010.MethodsFrom January 2009 until January 2010 one hundred vitiligo patients were randomly selected from outpatient clinic of Razi Hospital and asked to answer to the following valid and reliable instruments Beck Depression Inventory (BDI), SF-36 and Dermatology Life Quality Index (DLQI).ResultsThe mean age ± standard deviation of the participants was 28.9±11.5 years and mean scores of BDI, SF-36 and DLQI of all patients were 15.9±11.8, 63.8±19.4 and 8.4±6.9 respectively. There was a significant relationship between DLQI scores and the duration of the disease (r=0.3, P<0.001) while there was no significant relationship between DLQI scores and age (r=0.1, P=0.2).ConclusionEvaluation of quality of life and depression in patients with vitiligo and consequences of the disease on patient's lives should be considered.

Page 210Background And AimConsumption of the epidermis (COE) is defined as thinning of the epidermis with attenuation of the basal and suprabasal layers and loss of rete ridges in areas of direct contact with malignant melanocytes. The aim of this study was to investigate the importance of COE as an additional diagnostic criterion for malignant melanoma and to evaluate its relationship to clinicopathological findings.Methodes: The age, gender, localization of the lesion and the histopathological parameters such as tumor type, Breslow thickness, ulceration, and Clark’s level were recoeded in 90 malignant melanoma cases.ResultsIn contrast to other studies, we found that COE was more common in tumors with an acral localization and in the acral lentiginous melanoma.ConclusionAlthough COE can be used as a pathological criterion in the diagnosis of malignant melanoma, but no correlation no of COE with ulceration and other prognostic factors were found.

Page 215Background And AimAdverse drug reactions are inevitable outcomes of drug therapy, which may cause mortality and morbidity for those whom are treated in this way. Cutanous adverse drug reactions are abserved in 2% to 3% of inpatients but only 2% of them are considered serious and may cause death. The aim of this study was to determine the cause and clinical forms of adverse drug reactions referred to department of dermatology.MethodsThis cross-sectional study that was done on patients with diagnosis of cutanous adverse drug reactions in 2007 and 2009. All patients suspected with diagnosis of adverse drug reactions were examined by an experienced dermatologist for determination of different kinds of clinical manifestations.ResultsDuring the study period, 308 patients were recruited. Cutanous adverse drug reactions were found to be more frequent in females (63%) than in males (37%). Betalactam antibiotics were found to be the most frequent cause of adverse cutaneous drug reactions (42.7%), followed by non-steroidal anti-inflammatory drugs (16.5%). Acute urticaria was the most frequent observed drug reaction (59.2%) followed by fixed drug eruption (18.5%) and maculopapular rashes (14.9%).ConclusionIn this study adverse cutaneous drug reactions was mainly induced by betalactam antibiotics and non-steroidal anti-inflammatory drugs. The most common forms of cutaneous adverse drug reactions were found to be: acute urticaria, fixed drug eruption and maculopapular rashes.

Page 221Background And AimAndrogenic alopecia is one of the most common dermatological disorders. Hormonal and genetic factors determine the incidence of alopecia. Androgenic alopecia is the most common form of hair loss. It is progressive and symmetric in both sexes. The aim of this study was to determine the prevalence of androgenic alopecia in 14- to 18-year-old female high school students in Shiraz.MethodsThis cross-sectional study was conducted on 3190 14- to 18-year-old female high school students in Shiraz. A questionnaire was designed to collect personal information and record clinical features of androgenic alopecia. Moderate and severe hair losses were defined as alopcia. For evaluation of alopecia associated with polycystic ovary syndrome, total and free testosterone blood levels were measured and abdominal ultrasound was performed. The collected data were described and analyzed through performing chi-square, Fisher's exact and and t tests using SPSS.Results135 (4.2%) of participants had moderate to severe alopecia according to Ludwig's classification. No statistically significant correlation was observed in the mean of eather total or free testosterone levels between patients with androgenic alopecia, and those without (P>0.05). There was no statistical significant association between alpocia and polycystic ovary existence (P=0.2).ConclusionThis study did not show a statistically significant relationship between patients with alopecia and androgenic hormones and polycystic ovary syndrome.

Page 229Skin injury caused by burns, surgery and other traumas may result in unpleasant psychological experiences and be reflected in behaviors. Extracellular matrix (ECM) is the largest component of natural skin which is gel-like and is produced by skin cells. ECM synthesis is a key factor for filling up skin wounds such as burns, leishmaniasis, chicken pox, acne, etc. ECM is composed of a variety of polysaccharides, water, and collagen proteins. Considering its weight, natural skin strength and its expandability are like steel, while it has high elasticity and compaction capacities. These characteristics are due to dual effects of main ECM molecules, which are secreted by fibroblasts and epidermal cells: 1) structural fiber proteins like: elastin, fibronectin and laminin which give strength and flexibility to ECM, and 2) proteoglycans such as dermatan sulfate and hyaluronic acid which are consisted of few glycosaminoglycan chains that branch out from a linear protein core. Proteoglycans are large and hydrated molecules which are resistant to external forces and protect underneath cells. In general, understanding the skin structure and wound healing phases can help us to design useful experiments and to conduct proper researches in this area.

Page 245Dermatology is a branch of medicine which is related to the diagnosis, treatment and prevention of the diseases and disorders that affect the skin and its appendages including hair, nail, sweat glands and mucosal surfaces of mouth and external genitalia. There are about 3000 clinical entities in the field of dermatology, which are very diverse in terms of their etiologies. Although some of these diseases are really prevalent and some others of them have high incidence rates and acknowledging that because of their visibility in many cases they are associated with stress, anxiety and impairment of patient's quality of lives, their resultant burden of disease at individual, familial and social levels are usually ignored. Herein, we provide some examples of the adverse public health and health economics effects of different dermatological diseases and disorders in different countries and populations. We also provide some reasons for this negligence and suggest some strategies to tackle this problem.
