صفحه 1مقدمههورمون گنادوتروپین (GnRH یا Gonadotropin release hormone) اولین هورمون کلیدی تولید مثل است. امروزه از آنالوگ های GnRH به طور گسترده ای در لقاح آزمایشگاهی و درمان سرطان های وابسته به هورمون های جنسی ناشی از مواد شیمی درمانی استفاده می شود. به همین منظور در تحقیق حاضر تاثیر دکاپپتیل که یک آنالوگ GnRH است، بر نورون های ناحیه ی هیپوکامپ موش سوری نر بررسی شد.روش هادر مطالعه ی حاضر از 24 سر موش سوری نر با وزن متوسط 5 ± 30 گرم پس از سازش با شرایط آزمایشگاهی استفاده شد. موش ها به چهار گروه تقسیم شدند. تیمار اول سیکلوفسفامید با دوز 65 میلی گرم بر کیلوگرم وزن بدن و تیمار دوم دکاپپتیل با دوز 05/0 میلی گرم بر کیلوگرم وزن بدن دریافت کردند. درموش های تیمار سوم ابتدا دکاپپتیل و پس از 10 روز سیکلوفسفامید با همان دوزها به صورت داخل صفاقی تزریق شد. در گروه شاهد فقط سرم فیزیولوژی استریل به روش مشابه تزریق گردید. یک ماه پس از تزریق، همه ی موش ها به روش آسان کشی، کشته شدند و سپس مغز و ناحیه ی هیپوکمپ آن ها جدا گردید و پس از تثبیت در فرمالین، از آن ها مقاطع بافتی تهیه شد. پس از رنگ آمیزی با هماتوکسیلن و ائوزین و پاس با میکروسکوپ نوری ساختمان بافتی هیپوکامپ مورد مطالعه قرار گرفت.یافته هانورون های ناحیه های DG (Dentate Gyrus)، CA1 و CA3 در ناحیه ی هیپوکمپ موش ها دچار تغییرات بافتی شد. در موش های شیمی درمانی شده بیش از 60 درصد نورون های ناحیه های DG، CA3 و CA1 دچار نکروز شده بودند، در حالی که در موش های تیمار شده با دکاپپتیل نکروز نورون ها کمتر مشاهده شد.نتیجه گیریبر اساس نتایج مطالعه ی حاضر می توان نتیجه گرفت که دکاپپتیل ضد آسیب نورونی ناحیه ی هیپوکمپ در حیوانات شیمی درمانی شده اثر محافظتی دارد.

صفحه 2مقدمهپنومونی اکتسابی در جامعه می تواند از بهبودی کامل تا مرگ باشد. جهت تعیین شدت پنومونی و پیش آگهی، سیستم های نمره دهی متعددی ابداع شده است. شاخص شدت پنومونی (Pneumonia severity index یا PSI) یک روش استاندارد قابل قبول، با معیارهای پیش گویی کننده ی دقیق، موثر و مطمئن جهت تصمیم گیری چگونگی نوع و محل درمان می باشد. هدف این مطالعه، بررسی شدت پنومونی در بیماران بستری شده در بخش عفونی بزرگ سالان بود.روش هادر این مطالعه ی 15 ماهه (از دی ماه 1388 لغایت اسفند 1389) بیماران با تشخیص پنومونی اکتسابی که در بخش بیماری های عفونی بیمارستان الزهرای (س) اصفهان بستری بودند، با استفاده از سیستم نمره دهی PSI مورد بررسی قرار گرفتند و در کلاس های پنج گانه دسته بندی شدند.یافته هامیانگین نمره ی شدت پنومونی در بیماران مذکور 3/33 ± 4/91 با دامنه ی 166-19 بود که بر اساس آن، 9 بیمار (1/19 درصد) در کلاس I، 7 نفر (9/14 درصد) در کلاس II، 7 نفر (9/14 درصد) در کلاس III، 17 نفر (2/36 درصد) در کلاس IV و 7 نفر (9/14 درصد) در کلاس V قرار گرفتند.نتیجه گیریحدود نیمی از بیماران مورد مطالعه ی ما که در بخش بیماری های عفونی بستری بودند، با در نظر گرفتن این سیستم نمره دهی می توانستند درمان سر پایی شوند. از طرف دیگر، حدود 20 درصد از بیماران این مطالعه که در کلاس V قرار داشتند، بر اساس این سیستم نمره دهی اندیکاسیون بستری در بخش مراقبت های ویژه را داشتند. به نظر می رسد باید در هنگام پذیرش بیماران در بخش اورژانس دقت بیشتری به سیستم نمره دهی PSI و تعیین شدت بیماری در جهت انتخاب نوع و محل درمان شود. از سوی دیگر با استفاده از این سیستم نمره دهی می توانیم تا حدودی پیش آگهی مرگ احتمالی بیماران را حدس بزنیم.

آفلاتوکسین ها ترکیبات به شدت سمی هستند که موجب سرکوب سیستم ایمنی، سرطان زایی، موتاژنی و ناقص الخلقه زایی می شوند. این مطالعه با هدف بررسی میزان آلودگی شیر مادر به آفلاتوکسین M1 در شهر اصفهان صورت گرفت. از آفلاتوکسین M1 در شیر مادر به عنوان بیومارکری برای شناسایی تماس مادر با آفلاتوکسین B1 در جیره ی غذایی روزانه استفاده شد.
روش هانمونه های مورد مطالعه به صورت تصادفی از بین80 مادر شیرده مراجعه کننده به مراکز بهداشتی درمانی شهر اصفهان، که کودکان آن ها تنها شیر مادرخواران 4-2 ماهه بودند، انتخاب گردیدند. اطلاعات مربوط به مشخصات فردی و آنتروپومتریک و 24 ساعت یادامد غذایی از مادران شیرده به دست آمد. نمونه ها با روش ELISAی مستقیم رقابتی مورد آنالیز قرار گرفتند.
یافته هاشیوع آلودگی آفلاتوکسین M1 در مادران اصفهانی 25/1 درصد بود. آفلاتوکسین M1 در شیر یکی از مادران به میزان 88/6 نانوگرم در لیتر بود. آزمون آماری 2χ نشان داد مصرف سوسیس در مادر با شیر آلوده نسبت به مادران با شیر سالم به طور معنی داری بیشتر بود (001/0 > P).
نتیجه گیریبا توجه به اهمیت تغذیه ی مادر در دوران شیردهی و همچنین تاثیر آفلاتوکسین ها به عنوان یکی از عوامل خطرزا برای رشد شیرخواران، توجه به مصرف رژیم غذایی مغذی، سالم و عاری از آلودگی با آفلاتوکسین ها توسط مادر شیرده ضروری به نظر می رسد.

صفحه 4مقدمهسندرم تخمدان پلی کیستیک (Polycystic ovary syndrome یا PCO) شایع ترین اختلال اندوکرین در زنان سنین باروری می باشد و شیوع آن حدود 10 درصد گزارش شده است. علت این بیماری به طور کامل شناخته نشده است، ولی سطح سرمی برخی از هورمون ها غیر طبیعی می باشد. یکی از هورمون هایی که دخالت آن در ایجاد تخمدان پلی کیستیک هنوز مورد بحث می باشد، هورمون پرولاکتین است. این مطالعه با هدف تعیین سطح سرمی پرولاکتین در بیماران مبتلا به PCO انجام شد.روش هااین مطالعه یک مطالعه ی توصیفی تحلیلی بود که در سال 1390 در شهرستان اصفهان به انجام رسید. جامعه ی آماری مورد مطالعه شامل بیماران مبتلا به PCO مراجعه کننده به کلینیک غدد و زنان این شهرستان بود. سطح سرمی پرولاکتین در افراد مورد مطالعه اندازه گیری شد. اطلاعات جمع آوری شده به وسیله ی نرم افزار SPSS تجزیه و تحلیل گردید.یافته هامیانگین سطح سرمی پرولاکتین در بیماران تحت مطالعه 53/11 ± 56/18 نانوگرم بر میلی لیتر با دامنه ی81/69-56/18 بود. انجام آزمون One sample t بر روی داده های مذکور نشان داد که سطح پرولاکتین در بیماران مورد مطالعه با حد طبیعی آن (25 نانوگرم در میلی لیتر) اختلاف معنی داری داشت (001/0 > P).نتیجه گیریبا توجه به موارد ذکر شده در بالا، دست یابی به یک نتیجه گیری قطعی که بتواند ارتباط بین PCO و سطح پرولاکتین سرم را به اثبات رساند و یا این هورمون را به عنوان بیومارکری در تشخیص PCO معرفی نماید، هنوز راه طولانی در پیش دارد و نیاز به مطالعات متعدد دیگر در این زمینه می باشد.

بهینه سازی تولید اینترفرون بتا در سلول های CHO (Chinese hamster ovary) به عنوان یک میزبان بیانی که قادر به تولید پروتئین دارویی با کیفیت بالا می باشد، جهت کاهش قیمت و سرعت تولید این پروتئین نوترکیب دارویی از اهمیت زیادی برخوردار است. هدف این مطالعه، بررسی تاثیر پایداری mRNA به واسطه ی ایجاد یک تغییر در اولویت کدونی در ناحیه ی ناپایدارکننده ی انتهای پروتئین اینترفرون بتا (CRID یا Coding region instability determinant) و بررسی اثر این موتاسیون ها بر روی میزان بیان گذرای سلول های CHO به عنوان فاکتوری در راستای بهبود بیان ژن بود.
روش هابا استفاده از جدول اولویت کدونی (هامستر چینی) Cricetulus griseus 4 موتاسیون مورد نیاز برای ایجاد یک اولویت کدونی در توالی CRID ژن اینترفرون بتای انسانی طراحی شد. با روش SOEing PCR (Polymerase chain reaction-splicing by overlapping extension) در ناحیه ی CRID از ژن اینترفرون بتا موتاسیون های مورد نظر ایجاد شد و به وسیله ی کیت ®Express in درون سلول های CHO ترانسفکت گردید. میانگین میزان اینترفرون بتای تولید شده توسط سلول های CHO ترانسفکت شده با ژن موتانت اینترفرون بتا با استفاده از کیت ELISA مخصوص این ماده با میانگین میزان اینترفرون بتای تولید شده توسط سلول های CHO ترانسفکت شده با ژن طبیعی مقایسه گردید.
یافته هاموتاسیون های انجام شده به وسیله ی بررسی ژل و توالی یابی قطعات حاصل تایید گردید. موتاسیون های حاصل جهت ایجاد یک اولویت کدونی در توالی CRID پس از تراسفکت کردن ژن موتانت در سلول های CHO سبب افزایش معنی دار 9/2 برابری (05/0 > P) در میزان اینترفرون بتای تولید شده توسط سلول های CHO ترانسفکت شده با ژن موتانت نسبت به میزان اینترفرون بتای تولید شده توسط سلول های CHO ترانسفکت شده با ژن طبیعی گردید.
نتیجه گیریموتاسیون های مدنظر با موفقیت انجام گردید و نشان داده شد که انجام این جهش ها در توالی ناپایدار کننده ی انتهایی CRID می تواند به صورت معنی داری باعث افزایش پایداری و در نتیجه افزایش غلظت اینترفرون تولید شده در محیط کشت شود. این موتاسیون ها جدید بودند و تاثیر قابل توجه آن ها نیز به اثبات رسید. در مجموع، نتایج این تحقیق جالب توجه و کاربردی به نظر می رسد.

Page 1BackgroundGonadotropin hormone (GnRH) is the first reproductive key hormone. Today, GnRH analogues are widely used in in-vitro fertilization and treatment of sex hormone-dependent cancers induced by the materials used in chemotherapy. Therefore, the aim of the present study was to evaluate the effects of Decapeptyl (an analogue of GnRH) on neurons of hippocampus area in male mice.MethodsIn this study, 24 male mice with a mean weight of 30 g were used. After the mice were adapted to the experimental conditions, they were divided into four groups. The first group was injected with intraperitoneal (ip) cyclophosphamide (65 mg/kg of body weight). The second group was injected with Decapeptyl (0.05 mg/kg of body weight). The third group was first treated with Decapeptyl for 10 days and then injected with ip cyclophosphamide with the same dose. In the control group, only sterile saline was injected using the same method. After a month, the mice were killed. The hippocampus area was separated from the brain and after being fixed in formalin 10%, tissue sections were prepared and stained with eosin, hematoxylin, and periodic acid-Schiff (PAS) for microscopic studies.FindingsThe results showed that DG, CA1, and CA3 areas of hippocampal neurons had some tissue changes. In mice that had received chemotherapy, more than 60% of the neurons in DG, CA1, and CA3 areas were necrotic, but there were a few neuronal necrotic in mice treated with Decapeptyl.ConclusionThe results of this study suggested that Decapeptyl has a supporting effect on hippocampal neuronal (hippocampus area neurons) damage in animals that received chemotherapeutic agents.Keywords: Mice, Cyclophosphamide, Decapeptyl, Chemotherapy, Hippocampal neurons

Page 2BackgroundThe outcome of community-acquired pneumonia (CAP) ranges from full recovery to severe complications and death. Several scoring systems have been developed to determine the severity and prognosis of CAP. Among these, pneumonia severity index (PSI) has become the standard reference because of the prognostic accuracy, effectiveness, and safety as a decision making aid.MethodsWe conducted a descriptive, prospective study on all patients with CAP who were admitted to the infectious diseases ward of Alzahra Hospital (Isfahan, Iran) during 15 months. We assessed our patients based on PSI scoring system and classified them into 5 risk classes.FindingsThe mean PSI score in this study was 91.4 ± 33.3 (range: 19-166). According to the calculated scores for each patient, 9 patients (19.1%) belonged to class I, 7 (14.9%) to class II, 7 (14.9%) to class III, 17 (36.2%) to class IV, and 7 (14.9%) to class V.ConclusionAbout half of patients admitted to the infectious diseases ward were fine enough to be managed as outpatients. On the other hand, 20% of the patients in class V needed intensive care. It seems that physicians in emergency departments should pay more attention to PSI scoring system to determine the kind and site of care. Aflatoxins are highly toxic, immunosuppressive, mutagenic, teratogenic, and carcinogenic compounds. Aflatoxin M1 (AFM1) is a hydroxylated metabolite of aflatoxin B1 (AFB1) formed in liver and excreted into breast milk. It is considered to impose certain hygienic risks to infant health. The aim of this study was to evaluate the presence of aflatoxin in breast milk using AFM1 in milk as a biomarker for exposure to AFB1. We also tried to determine the level of AFM1 in lactating mothers in Isfahan, Iran.
MethodsThe study was carried out on 80 lactating women who were randomly selected from 2 urban health centers. Milk samples and information on food intake were collected from the participants using the structured food-frequency questionnaire. Breast milk samples were tested to determine AFM1 levels by a competitive enzyme-linked immunosorbent assay (ELISA) technique.
FindingsOur findings showed that the incidence of contamination with AFM1 in our participants was 1.25%. AFM1 was found only in one sample at a concentration of 6.8 ng/L. Chi-square analysis revealed the presence of AFM1 to be significantly associated with consumption of sausages (P < 0.001). AFM1 level was found to be lower than the maximum tolerable limit (25 ng/l) accepted by the European Communities and Codex Alimentarius.
ConclusionThe benefits of breast milk have been well documented. Therefore, early childhood exposure to the environmental toxicants like aflatoxins may be a critical determinant of later health effects. The findings of this study pointed out that AFM1 was present in only one milk sample. The findings confirmed the need for developing strategies to reduce exposure to aflatoxin in foods and also to routinely carry out biological monitoring of aflatoxins as a food quality control measure.

Page 4BackgroundPolycystic ovary syndrome (PCOS), with a prevalence of 10%, is the most common endocrine disorder in women of reproductive age. Although the main cause of PCOS has not been identified yet, abnormal serum levels of some hormones such as increased luteinizing hormone (LH) to follicle-stimulating hormone (FSH) ratio, and free and total testosterone are suggested. Prolactin is one of the PCOS interventional hormones. The aim of this study was to measure prolactin serum levels in patients with PCOS.MethodsThis descriptive analytical study was performed on PCOS patients who referred to Obstetrics and Gynecology Clinic (Isfahan, Iran) during 2011. Prolactin serum levels were measured in these patients and the data was analyzed by SPSS.FindingsThe mean prolactin serum level was 18.56 ± 11.53 ng/l (range: 18.56-69.81 ng/l). It was significantly different from the normal level. One sample t-test showed a statistically significant difference (P < 0.01) between prolactin levels of the patients and the normal value (25 ng/ ml).ConclusionDespite our results, the relation between PCOS and serum prolactin level would still need to be further investigated.Keywords: Polycystic ovary syndrome, Prolactin level

Improvement of mRNA stability and therefore, production of interferon beta (IFNb) in Chinese hamster ovary (CHO) cell, as a recombinant protein expression system, is very important. The aim of this study was to investigate this effect by creating a codon bias change in instability sequence located in C-terminal of interferon beta protein (Coding region instability determinant or CRID).
MethodsMutations were designed in 4 points, according to codon bias table of Cricetulus griseus, using polymerase chain reaction/splicing by overlapping extension (SOEing PCR) method inside the CRID of interferon beta gene. The concentration of interferon beta in the medium of Chinese hamster ovary cell was determined using ELISA test kit.
FindingsThe mutations were verified by gel-electrophoresis and sequencing the resultant DNA. The mutations in resulted in a significant (2.9 times) increase in interferon-b concentration (P < 0.05).
ConclusionThe mutations in CRID could significantly increase the stability of mRNA and the resultant Interferon b concentration in the medium. These mutations were novel and their effectiveness was proved.
