    Background and aims. The aim of the present study was to systematically review fluoride concentration of drinking waters and prevalence of fluorosis in Iran through systematically evaluating results of studies conducted in this regard. Materials and methods. In this systematic review study, the required data was collected using keywords including drinking water fluoride, fluoride concentration, Fluorosis, dent*, Iran*, and their Persian equivalents through PubMed, ScienceDirect, IranMedex, SID, MEDLIB, and Magiran databases. Out of 617 articles, 29 articles were finally considered after excluding the remaining articles which were not related to the study objectives. Following precise studying and extraction, the relevant data were summarized in extraction tables and analyzed manually. Excel 2007 software was used to draw diagrams. Results. 4434 samples of surface, ground, and tap water resources collected within 236 months during all seasons in 17 provinces of Iran were used in 29 articles determining fluoride concentrations of drinking water. Average fluoride concentration was estimated to be 0.43 ± 0.17 ppm with zero and 3.06 as minimal and maximal values. The least concentration was seen in tap water. Fluoride concentration of only three provinces was in accordance with the global standard. According to estimations, prevalence of fluorosis was 61% with only 1% as severe fluorosis. Conclusion. Despite lower than standard concentrations of fluoride in drinking water, a relatively high level of fluorosis was seen in Iran.

    Background and aims. The aim of present study was to investigate pulp-dentin complex reactions following direct pulp capping (DPC) with calcium hydroxide [Ca(OH)2] and simvastatin as pulp-capping materials in the primary human molars. Materials and methods. 120 primary molar teeth which had to be extracted for orthodontic reasons were randomly allocated into four groups. Group Ι as a control, underwent DPC with calcium hydroxide. The dental pulp in group ІІ, ІІІ and ІV were directly capped with simvastatin-based materials at concentrations of 1, 5 and 10 µM, respectively. All of the teeth were restored with stainless steel crown. After a mean period of 7.41 months teeth were extracted and processed for histological examination and categorized in terms of hard tissue formation and pulp inflammation. Results. Teeth in group I had statistically favorable outcomes in hard tissue formation and pulp inflammation than did the groups ІІ, ІІІ and ІV (P < 0.001). Considering three different concentrations of simvastatin, the result showed a dose dependent trend. Teeth in group ІV showed significantly lower rates of hard tissue formation and higher rates of pulp inflammation and necrosis compared to those of groups ІІ (P < 0.05). Conclusion. The findings of this study depicted that healing with no inflammation and hard tissue formation following statin treatment occurs with a lower rate than that with the calcium hydroxide.

    Background and aims. Biocompatibility of root-end filling materials is a matter of debate. The aim of this study was to compare the biocompatibility of a variety of commercial ProRoot WMTA cements and a resin-based cement (Geristore®) with different pH values of setting reaction and different aluminum contents, implanted into the subcutaneous connective tissue of rats at various time intervals. Materials and methods. Fifty Sprague-Dawley rats were used in this study. Polyethylene tubes were filled with Angelus WMTA, ProRoot WMTA, Bioaggregate, and Geristore. Empty control tubes were implanted into subcutaneous tissues and harvested at 7-, 14-, 28- and 60-day intervals. Tissue sections of 5 μm were stained with hematoxylin and eosin and observed under a light microscope. Inflammatory reactions were categorized as 0, none (without inflammatory cells); 1, mild (inflammatory cells ≤25); 2, moderate (25–125 inflammatory cells); and 3, severe (>125 inflammatory cells). Statistical analysis was performed with Kruskal-Wallis and Mann Whitney U tests. Results. ProRoot WMTA and Angelus elicited significantly less inflammation than other materials (P< 0.05). After 7 days, however, all the materials induced significantly more inflammation than the controls (P< 0.05). Angelus-MTA group exhibited no significant differences from the Bioaggregate group (P=0.15); however, ProRoot WMTA elicited significantly less inflammation than Bioaggregate (P=0.02). Geristore induced significantly more inflammation than other groups (P< 0.05). Conclusion. Geristore induced an inflammatory response higher than ProRoot WMTA; therefore, it is not recommended for clinical use.

    Background and aims. Plasma rich in growth factors (PRGF) is a concentrated suspension of growth factors, which is used to promote periodontal tissue regeneration. The aim of this randomized, controlled, clinical trial was to evaluate of the treatment of grade II mandibular molar furcation involvement using autogenous bone graft with and without PRGF. Materials and methods. In this double-blind clinical trial, thirty mandibular molars with grade II furcation involvement in 30 patients were selected. The test group received bone graft combined with PRGF, while the control group was treated with bone graft only. Clinical parameters included clinical probing depth (CPD), vertical clinical attachment level (V-CAL), horizontal clinical attachment level (H-CAL), location of gingival margin (LGM), surgically exposed horizontal probing depth of bony defect (E-HPD), vertical depth of bone crest (V-DBC), vertical depth of the base of bony defect (V-DBD), and length of the intrabony defect (LID). After six months, a re-entry surgery was performed. Data were analyzed by SPSS 14, using Kolmogorov, Mann-Whitney U, and paired t-test. Results. After 6 months, both treatment methods led to significant improvement in V-CAL and H-CAL and significant decreases in CPD, E-HPD, V-DBD and LID; there was no significant difference in LGM and V-DBC in any of the treated groups compared to the baseline values. Also, none of the parameters showed significant differences between the study groups. Conclusion. Although autogenous bone grafts, with or without PRGF, were successful in treating grade II furcation involvement, no differences between the study groups were observed

    Background and aims. The correct relationship of the total mesiodistal width of the mandibular teeth to that of the maxillary teeth (Bolton’s ratios) is among the main factors considered in obtaining an optimal occlusion. The present study was conducted to determining the Bolton’s ratios before and after treatment in skeletal class I patients. Materials and methods. In this descriptive analytical study, 200 study casts of 100 patients (including 73 females and 27 males) were selected from 1,700 patient files with fixed, non-extraction orthodontic treatment protocol. The greatest mesiodistal width of teeth was measured using a digital caliper. The total Bolton index (TBI) and anterior Bolton index (ABI) were calculated for subjects and the obtained results were compared with the values reported by Bolton using one-sample t-test. The alterations in these indexes before and after treatment were compared with paired t-test. Results. Post-treatment ABI and TBI in patients were 77.35% ± 4.12% and 91.2% ± 1.66%, respectively. No significant difference was detected in ABI and TBI of patients after treatment when compared with the results of Bolton’s study (P > 0.05). However, a significant correlation was observed in values of ABI (P < 0.001) and TBI (P < 0.01) before and after treatment. Conclusion. These findings suggest that patients with mild hypodontia have narrower teeth than normal subjects especially in posterior segments, which may have clinical implications during the orthodontic treatment process.

    Background and aims. Hypodontia is a common developmental abnormality of dentition. This study aimed to determine tooth width in patients affected with mild hypodontia and compare the results with a control group without tooth agenesis. Materials and methods. The orthodontic records of 25 patients with congenital missing of one or two teeth (hypodontia group), and 25 subjects with full dentition (control group) were selected. The greatest mesiodistal width of each tooth was measured on the study models by a digital caliper. Tooth width measurements were compared between the groups using a student t-test at p < 0.05 of significance. Results. Patients with hypodontia showed narrower teeth than the control subjects. The differences in tooth size between the two groups were statistically significant for the first and second premolars and first molar in the maxillary right and for the second premolar in the maxillary left quadrants (p < 0.05). In the lower arch, the first and second premolars and also first molar in both sides of hypodontia patients demonstrated significant reduction in tooth size compared to the control group (p < 0.05). Conclusion. These findings suggest that patients with mild hypodontia have narrower teeth than normal subjects especially in posterior segments, which may have clinical implications during the orthodontic treatment process.

    Background and aims. Although microwave irradiation has been used for disinfection of dental stone casts, there are concerns regarding mechanical damage to casts during the process. The aim of this study was to evaluate the effect of microwave irradiation on the compressive strength (CS) and diametral tensile strength (DTS) of stone casts. Materials and methods. In this in vitro study, 80 cylindrical type III and IV stone models (20 × 40 mm) were prepared and divided into 8 groups of 10. The DTS and CS of the specimens were measured by a mechanical testing machine at a crosshead speed of 0.5 cm/min after 7 times of frequent wetting, irradiating at an energy level of 600 W for 3 minutes and cooling. Data were analyzed by Student’s t-test. Results. Microwave irradiation significantly increased DTS of type III and IV to 5.23 ± 0.64 and 8.17 ± 0.94, respectively (P < 0.01). Conclusion. According to the results, microwave disinfection increases DTS of type III and IV stone casts without any effects on their CS.

    A case of a compound odontoma which caused delayed eruption of right maxillary central incisor in a ten year old girl is presented with clinical and radiographic findings. The patient presented with complaint of a hard painless swelling in the right anterior region of the maxilla and absence of right maxillary central incisor. After clinical examination, periapical and occlusal radiographs of the mentioned region were taken. Impacted maxillary right central incisor was seen malformed shape on the intraoral radiographs. After taking a cone beam computed tomography (CBCT) a compound odontoma associated with the labial aspect of impacted maxillary right central incisor was diagnosed and then removed by simple local excision under local anesthesia. The removal of the odontoma was followed by forced eruption of the impacted central incisor. After three months the tooth returned to its original position.

    Odontogenic tumors are derived from epithelial, ectomesenchymal and/or mesenchymal elements that are or have been a part of the tooth-forming apparatus. Of all the odontogenic tumors, calcifying epithelial odontogenic tumor accounts for 1% of the cases. Approximately 200 cases have been reported to date. There is no sex predilection, with a 2:1 predilection for the mandible, mostly in the premolar/molar region. It is often locally invasive. Most often, it is associated with an impacted tooth, is asymptomatic and requires biopsy for diagnosis. Presented here is a rare case of an intraosseous calcifying epithelial odontogenic tumor surrounding a supernumerary tooth. Furthermore, the occurrence of this tumor in the anterior maxilla (an uncommon site) in a pediatric patient makes it rarer. Although the present case was asymptomatic, root resorption and displacement of adjacent teeth necessitated its surgical removal. The lesion was surgically enucleated and histopathological examination confirmed calcifying epithelial odontogenic tumor, showing abundant calcifications in the form of Liesegang rings.
