    مقدمه

    صافی کف پا یکی از شایع ترین اختلالات اسکلتی است که علاوه بر عملکرد پا، بر تنظیم راستای مفاصل مچ، زانو و هیپ نیز تاثیر می گذارد. بر اساس مطالعات موجود، الگوی راه رفتن افراد مبتلا به صافی کف پا با افراد سالم متفاوت است، اما تفاوت های موجود در تعادل و تاثیر کفی طبی بررسی نشده است. بنابراین هدف از این مطالعه، بررسی تعادل افراد مبتلا به صافی کف پا، با و بدون استفاده از کفی طبی بود.

    مواد و روش ها

    در مطالعه کارآزمایی بالینی حاضر، تعداد 40 نفر از دانشجویان دختر دانشگاه علوم پزشکی اصفهان در دو گروه سالم و مبتلا به صافی کف پا شرکت داشتند. برای اندازه گیری مرکز فشار از یک صفحه نیروی Kistler استفاده شد. تعادل با محاسبه نوسان مرکز فشار، طول مسیر و سرعت در راستای داخلی- خارجی و قدامی- خلفی به دست آمد. تفاوت بین دو گروه با و بدون کفی طبی چرمی توسط آزمون Two sample t و Paired t ارزیابی شد (05/0 = α).

    یافته ها

    تفاوت قابل توجهی در تعادل بین افراد سالم و افراد مبتلا به صافی کف پا بر اساس سرعت مرکز فشار در صفحات قدامی- خلفی و داخلی- خارجی و سرعت کل مشاهده شد. مقدار میانگین سرعت کل مرکز فشار طی ایستادن با و بدون کفی طبی، 160 ± 42/1645 و 683 ± 2752 میلی متر بر دقیقه بود (05/0 > P).

    نتیجه گیری

    افراد مبتلا به صافی کف پا به دلیل از دست دادن تنطیم راستای ساختار پا و کاهش درک حرکتی لیگامان ها و تاندون های پا در مقایسه با افراد سالم، تعادل کمتری طی ایستادن دارند. به نظر می رسد که کفی طبی سبب بهبود تنظیم راستای ساختار پا و کاهش نیروهای وارد بر لیگامان ها و در نتیجه بهبود تعادل می شود.

    مقدمه
    بر خلاف اثرات تسهیلی بیان شده پس از اعمال ویبراسیون کل بدن (Whole body vibration یا WBV)، نتایج برخی مطالعات افزایش مهار پاسخ های عضلانی را نشان می دهد. هدف مطالعه حاضر، بررسی پاسخ کوتاه مدت منحنی فراخوانی رفلکس H عضله سولئوس پس از اعمال یک جلسه WBV بود.
    مواد و روش ها
    26 زن سالم غیر ورزشکار (سن 85/2 ± 11/26 سال) داوطلبانه در مطالعه شرکت کردند. افراد در دو جلسه آزمون و شم با حفظ وضعیت نیمه اسکوات بر روی صفحه ویبراسیون قرار گرفتند. در جلسه آزمون ویبراسیون با فرکانس 30 هرتز و دامنه 3 میلی متر به صورت 3 دوره 1 دقیقه ای اعمال شد. قبل، بلافاصله، 5 و 10 دقیقه بعد از اعمال ویبراسیون منحنی فراخوانی رفلکس H از عضله سولئوس ثبت شد.
    یافته ها
    در جلسه شم و آزمون شدت تحریک آستانه منحنی و شدت لازم برای ثبت دامنه حداکثر بلافاصله بعد از WBV افزایش معنی دار نشان داد (05/0 > P). دامنه رفلکس H ابتدایی در هر دو جلسه و دامنه Hmax در جلسه شم کاهش معنی داری نشان داد (05/0 > P). آنالیز پارامترهای مربوط به آستانه و قله منحنی فراخوانی رفلکس H بین دو جلسه آزمون و شم تفاوت معنی داری نشان نداد (05/0 < P).
    نتیجه گیری
    با توجه به جابه جایی منحنی فراخوانی به سمت راست و پاسخ مهاری در دو جلسه شم و آزمون، به نظر می رسد حفظ وضعیت نیمه اسکوات و مکانیسم های کنترلی فعال شده مانند انقباض عضلات اکستانسوری در مهار منحنی فراخوانی رفلکس H موثر باشد و اعمال ویبراسیون نیز هیچ اثر تسهیلی بر این پاسخ مهاری نداشته است.

  • ارزیابی اثربخشی گروه درمانی آموزشی- روانی بر ارتقای سلامت عمومی خانواده های بیماران مبتلا به اختلالات دو قطبی: یک مطالعه پایلوت

    مقدمه
    مراقبین خانوادگی بیماران روان در واقع اساس سیستم مراقبت های بهداشتی- درمانی می باشند و مراقبت از بیمار مبتلا به اختلال روان پزشکی مسوولیتی است که فشار و استرس زیادی برای اعضای خانواده به همراه دارد. پژوهش حاضر با هدف، بررسی اثربخشی گروه درمانی آموزشی- روانی بر ارتقای سلامت عمومی خانواده بیماران مبتلا به اختلالات دو قطبی انجام شد.
    مواد و روش ها
    این پژوهش یک طرح شبه آزمایشی با گروه شاهد و انتخاب تصادفی بود. در این پژوهش، 6 نفر به عنوان گروه آزمایش و 9 نفر به عنوان گروه شاهد شرکت داشتند. کلیه شرکت کنندگان که اعضای خانواده بیماران مبتلا به اختلالات دو قطبی را تشکیل می دادند، پرسش نامه سلامت عمومی (General Health Questionnaire یا 28-GHQ) را تکمیل نمودند. گروه آزمایش به مدت 12 جلسه در جلسات گروه درمانی آموزشی- روانی شرکت داشتند، ولی گروه شاهد در این مدت هیچ گونه مداخله ای دریافت نکردند. همچنین به منظور تحلیل داده ها از آزمون ANCOVA استفاده شد.
    یافته ها
    افزایش معنی داری در میانگین نمره سلامت عمومی گروه آزمایش مشاهده گردید (001/0 > P).
    نتیجه گیری
    گروه درمانی آموزشی- روانی می تواند سطح سلامت عمومی خانواده های بیماران مبتلا به اختلال دو قطبی را بهبود بخشد

    مقدمه
    پتانسیل عضلانی برانگیخته دهلیزی، آزمون قابل اطمینان و جدیدی است که با فراهم آوردن اطلاعات تشخیصی در مورد عملکرد ساکول و یا عصب دهلیزی تحتانی، مجموعه آزمون های تعادلی حاضر را تکمیل نموده و سلامت رفلکس ساکولوکولیک را ارزیابی می نماید. دامنه پتانسیل عضلانی برانگیخته دهلیزی متناسب با سطح فعالیت الکترومیوگرافی زمینه می باشد که این امر لزوم حفظ ثبات فشار عضله در حین ثبت را نشان می دهد. تغییر در وضعیت چشم باعث تغییر در فعالیت عضله جناغی- چنبری- پستانی و بنابراین تغییر در دامنه پتانسیل عضلانی برانگیخته دهلیزی می گردد. از این رو، پژوهش حاضر با هدف، بررسی امکان تاثیر باز و بسته بودن چشم بر پارامترهای پتانسیل عضلانی برانگیخته دهلیزی صورت گرفت.
    مواد و روش ها
    در مطالعه مقطعی- مقایسه ای حاضر، پتانسیل عضلانی برانگیخته دهلیزی با ارایه محرک Tone-Burst 500 هرتز با شدت dBnHL 95 در 40 فرد هنجار در محدوده سنی 30-18 سال ثبت شد.
    یافته ها
    میانگین دامنه و زمان نهفتگی امواج 13P و 23N در هر دو گوش، اختلاف آماری معنی داری را نشان نداد (05/0 < P).
    نتیجه گیری
    باز و بسته بودن چشم ها تاثیری بر پتانسیل عضلانی برانگیخته دهلیزی نداشت، بنابراین در صورت لزوم و جهت حذف آرتیفکت (Artifact) چشمی می توان آزمون پتانسیل عضلانی برانگیخته دهلیزی را با چشم بسته ثبت نمود.

    مقدمه
    محدودیت درمانی و درمان فشرده دو دستی، روش های درمانی نویدبخشی در بهبود کارکرد اندام فوقانی کودکان با فلج مغزی نیمه بدن می باشند. محدودیت درمانی شامل: محدودیت اندام سالم و انجام تمرینات فشرده با اندام مبتلا می باشد و درمان فشرده دو دستی، مکملی برای درمان های دیگر اندام فوقانی می باشد و هدف آن بهبود هماهنگی دو دست با استفاده از تمرینات ساختار یافته است که درون فعالیت های کارکردی و بازی های دو دستی گنجانده شده است. هدف از این مطالعه، بررسی تاثیر تلفیق محدودیت درمانی با درمان فشرده دو دستی بر کارکرد اندام فوقانی کودکان با فلج مغزی نیمه بدن بود.
    مواد و روش ها
    در کارآزمایی بالینی حاضر، 25 کودک 10-5 ساله به روش تصادفی ساده به دو گروه درمان و شاهد تقسیم شدند. گروه شاهد خدمات رایج در کاردرمانی را دریافت نمودند و گروه درمان علاوه بر خدمات رایج در کاردرمانی، تلفیق محدودیت درمانی و درمان فشرده دو دستی را به صورت دریافت نمودند که ابتدا اندام فوقانی سالم با محدود کننده (اسلینگ) به مدت 3 ساعت بسته شد و کودکان تکالیف داده شده را تنها با اندام مبتلا انجام دادند، سپس اسلینگ را درآورده و کودکان تکالیف داده شده را به مدت 3 ساعت با هر دو دست تمرین نمودند. این روند به مدت 10 روز در طی دو هفته متوالی ادامه یافت. کارکرد اندام فوقانی و تون عضلانی کودکان با استفاده از آزمون کارکرد حرکتی جبسن- تیلور (Jebsen-Taylor test of hand function) و مقیاس اصلاح شده آشورث (Modified Ashwort scale) پیش و پس از مداخله درمانی مورد سنجش قرار گرفت.
    یافته ها
    کارکرد حرکتی کودکانی که تحت درمان تلفیق محدودیت درمانی با درمان فشرده دو دستی قرار گرفتندف نسبت به گروه شاهد تفاوت معنی داری داشت (05/0 > P)، اما میزان اسپاستیسیته عضلانی بعد از انجام مداخله درمانی تفاوت معنی داری نداشت (05/0 < P).
    نتیجه گیری
    تلفیق محدودیت درمانی با درمان فشرده دو دستی موجب بهبود کارکرد حرکتی کودکان با فلج مغزی نیمه بدن می شود

    مقدمه
    توان بخشی فرایندی مستمر با طیفی از تخصص های مختلف است. یکی از جنبه های مهم این فرایند، کار تیمی است. تیم توان بخشی شامل متخصصین طب فیزیکی، فیزیوتراپیست، کاردرمانگر، گفتار درمانگر و کارشناس اعضای مصنوعی است، بنابراین نگرش این اعضا نسبت به نحوه انجام کار تیمی تاثیر بسزایی در کیفیت فرایند توان بخشی خواهد داشت. هدف این مطالعه، بررسی نگرش متخصصین یاد شده نسبت به کار تیمی بود.
    مواد و روش ها
    مطالعه حاضر از نوع توصیفی- مقطعی بود. روش نمونه گیری به شیوه سرشماری و جامعه آماری شامل دستیاران و متخصصین طب فیزیکی، کارشناسان کاردرمانی، گفتار درمانی، فیزیوتراپی و اعضای مصنوعی شاغل در مراکز درمانی خصوصی و دولتی شهر شیراز بود. ابزار جمع آوری اطلاعات پرسش نامه محقق ساخته نگرش تیمی بود. ضریب Cronbach¢s alpha برای بررسی پایایی پرسش نامه، تحلیل عاملی برای بررسی ویژگی های ساختاری پرسش نامه و آزمون t برای بررسی وضعیت نگرش مورد استفاده قرار گرفت.
    یافته ها
    ضریب Cronbach¢s alpha (87/0) موید پایایی خوب پرسش نامه بود. بر اساس یافته های مطالعه تمامی گروه های شغلی از نگرش مثبتی نسبت به کار تیمی برخوردار بودند (05/0 > P). با توجه به برآورد درصد پاسخ داده شده به سوالات پرسش نامه، عوامل موثر و مخاطره آمیز در روند کار تیمی از دیدگاه افراد استخراج گردید.
    نتیجه گیری
    رویکرد بیمارمحور، آگاهی کافی نسبت به دانش و مهارت سایر اعضا و رعایت ارزش های اخلاقی و حرفه ای برخی عوامل موثر در بهبود کار تیمی و اختلافات شخصی اعضا، ارتباطات درون گروهی ضعیف، پنداشت نامناسب از درخواست کمک و عدم آگاهی کافی نسبت به روند انجام کار گروهی از عوامل مخاطره آمیز می باشد.

    مقدمه

    اثر وضعیت غیر طبیعی پا بر عملکرد حرکتی اندام تحتانی به ویژه در بین کودکان و نوجوانان کمتر مورد توجه قرار گرفته است. هدف مطالعه حاضر، مقایسه الگوی فعالیت عضلات منتخب اندام تحتانی و ویژگی های فشار کف پایی پسران نوجوان دچار صافی کف پا منعطف با گروه نرمال هنگام راه رفتن بود.

    مواد و روش ها

    تعداد 12 نفر نوجوان با ساختار صافی کف پا و 12 نفر با ساختار پای نرمال با استفاده از روش شاخص قوس طولی میانی پا در این مطالعه شرکت کردند. فعالیت الکترومایوگرافی عضلات تیبیالیس انتریور، پرونئوس لانگوس، سولئوس، بخش خارجی و داخلی عضله گاستروکنیمیوس به همراه ویژگی های فشار کف پایی آزمودنی ها هنگام راه رفتن با پای برهنه در یک مسیر 10 متری با سرعت خودانتخابی جمع آوری شد. داده ها با روش آماری Independent t تجزیه و تحلیل گردید.

    یافته ها

    فعالیت الکتریکی قسمت داخلی و خارجی عضله دو قلو طی مرحله انتقال وزن در افراد با پای نرمال نسبت به گروه با صافی کف پا به طور معنی داری بیشتر بود (001/0 = P)؛ در حالی که قسمت داخلی گاستروکنیمیوس در مرحله انتهای استانس فعالیت کمتری (048/0 = P) داشت. حداکثر فشار وارد شده در نواحی انگشت شست، انگشتان دوم تا پنجم، متاتارسال دوم و سوم و قسمت داخلی پاشنه در افراد دچار صافی کف پا بیشتر بود. به علاوه میزان تغییرات مرکز فشار پا در مراحل انتقال وزن، مید استانس و کل مرحله استانس، بین دو گروه تفاوت مشاهده شد (به ترتیب: 011/0 = P، 015/0 = P و 050/0 = P).

    نتیجه گیری

    وضعیت غیر طبیعی پا می تواند بر تغییر فعالیت عضلات اندام تحتانی و ویژگی های توزیع فشار کف پایی هنگام راه رفتن موثر باشد. نتایج این تحقیق بر لزوم بررسی دقیق و جامع وضعیت غیر طبیعی پا بر عملکرد اندام تحتانی هنگام راه رفتن برای طراحی برنامه درمانی مناسب تاکید داشت.

    مقدمه
    تعداد افرادی که از وسایل کمکی برای ایستادن و راه رفتن استفاده می کنند در حال افزایش است. انواع مختلفی از واکر و کراچ برای بهبود عملکرد افراد طراحی شده است، ولی تجویز این وسایل بر طبق ملاک هایی است که به لحاظ کلینیکی آزمایش نشده است. به علاوه تفاوت بین کراچ و واکر واضح نیست، این که کدام نوع از واکر مناسب است و همچنین تفاوت بین طراحی های جدید و قدیمی واکر و کراچ موضوعی است که به آن پرداخته نشده است. بنابراین هدف از مقاله مروری حاضر پاسخ به سوالات ذکر شده بود.
    مواد و روش ها
    یک جستجوی الکترونیک در پایگاه های اطلاعاتی PubMed، ISI web of science و Embase از سال 1960 تا 2012 انجام گردید. عنوان و خلاصه مقالات بر طبق معیارهای انتخاب ارزیابی شدند. کیفیت مقالات از طریق ابزار Black and Down مورد ارزیابی قرار گرفت.
    یافته ها
    حدود 24 مقاله برای تحلیل نهایی انتخاب شدند. کیفیت اکثر مطالعات پایین بود و بیشتر تحقیقات روی نمونه های طبیعی انجام شده بود. به علاوه، فقط بعضی پارامترها مثل نیروی اعمالی به پا در حین راه رفتن و مصرف انرژی ارزیابی گردید است. شواهد در دسترس برای تشخیص عملکرد واکر و کراچ کافی نیست.
    نتیجه گیری
    بر طبق مطالعات تحقیقی انجام شده، تفاوت بین انواع مختلف واکر و کراچ و مزایای واکر نسبت به کراچ واضح نیست. تاثیر طول کراچ و واکر بر کارایی نمونه ها بحث برانگیز است. به علاوه تعداد نمونه های اندک مطالعات، نتیجه گیری محکم را تحت تاثیر قرار داده است. بنابراین انجام مطالعات بزرگ تر برای تست و ارزیابی انواع مختلف کراچ و واکر روی افراد معلول توصیه می شود.

    مقدمه
    اسکولیوز به عنوان یک بدشکلی رایج ستون فقرات محسوب می شود. بسیاری از مطالعات پیشین، آنالیز حرکات افراد مبتلا به اسکولیوز را بر پایه اندازه گیری های کینماتیک بررسی کردند، اما از آن جایی که اسکولیوز یک بدشکلی ساختاری است که تقارن ستون فقرات را به هم می زند و در نتیجه باعث تغییر مکان مرکز جرم (center of mass یا COM) و توزیع نامتقارن وزن در اندام تحتانی می شود. بررسی و ارزیابی اطلاعات کینتیکی باعث فهم بهتر علل اسکولیوز می شود. هدف این تحقیق، بررسی شاخص های کینتیکی در گیت افراد مبتلا به اسکولیوز و تفاوت آن با افراد سالم با توجه به مطالعات گذشته است.
    مواد و روش ها
    در روند این تحقیق از سایت های Google scholar، PubMed، Elsevier، Ovid و از کلمات کلیدی Scoliosis، Kinetic، نیروی عکس العمل زمین (Ground reaction force)، مرکز فشار (Center of pressure)، گشتاور (Moment) و نیروهای بین مهره ای (Intervertebral force) و در این بین مقالات از سال 1984 تا 2012 انتخاب و استفاده گردید. در این تحقیق کیفیت مقالات نیز بر حسب جدول Black and down tools بررسی و امتیازگذاری شدند.
    یافته ها
    با استفاده از روش جستجوی ذکر شده، 13 مقاله به دست آمد که با توجه به معیارهای ورود و خروج، از 11 مقاله استفاده شد. در بین بررسی های انجام شده بر روی شاخص های تقارن، محققان یک عدم تقارن را مشاهده کردند، اما این عدم تقارن الگوی خاصی را متناسب با انحنا دنبال نمی کرد. در بررسی که بر جابجایی مرکز فشار انجام شد به این نتیجه رسیدند که جابجایی بیشتر در جهت داخل- خارج است. مطالعه دیگری ارزیابی اطلاعات کینتیکی را حین گام برداشتن به پهلو پیشنهاد کرد؛ چرا که در این حالت تفاوت در عدم تقارن شاخص ها بین دو گروه شاهد و اسکولیوزی بیشتر بود.
    نتیجه گیری
    در آخر با بررسی مطالعات گذشته، یک عدم تقارن در شاخص ها دیده شد، اما هنوز رابطه بین این شاخص ها و میزان انحنا در این افراد یافت نشده است.

    مقدمه
    اسکولیوز با علت ناشناخته یک ناهنجاری سه بعدی در ستون فقرات است که بیشتر در دختران نوجوان روی می دهد. اگر که اسکولیوز بدون درمان رها شود، می تواند پیشرفت کند و منجر به ایجاد درد کمر، نقص در ظرفیت حیاتی و اختلالات روانی- اجتماعی گردد. این چنین به نظر می رسد که درمان های ورزش بنیان می توانند در درمان اسکولیوز موثر واقع شوند، بنابراین هدف اولیه در مطالعه مروری حاضر، ارزیابی تاثیرگذاری ورزش های خاص اسکولیوز در درمان اسکولیوز با علت ناشناخته (Idiopathic) بود.
    مواد و روش ها
    جستجوی انجام شده در پایگاه های اطلاعاتی بین المللی Ebsco (Medline از طریق PubMed)، ISI Web of Knowledge صورت گرفت. در این جستجو از اصطلاحاتی مانند: اسکولیوز همراه با مرور، اسکولیوز همراه با تمرین ورزشی (Exercise)، اسکولیوز همراه با ورزش (Sport) و اسکولیوز همراه با توان بخشی استفاده گردید. معیارهای ورود به مطالعه حاضر شامل موارد زیر می باشد: 1) بیماران فقط با فرایند استفاده از ورزش های فیزیکی درمان شده باشند. 2) طراحی مطالعه در قالب بازبینی سیستماتیک باشد.
    یافته ها
    سرانجام پنج مقاله بر طبق معیارهای ورود بررسی گردید. از پنج مقاله باقی مانده سه مورد به دلیل کمبود کارآزمایی تصادفی شاهددار با کیفیت بالا در این زمینه، استفاده از ورزش را در درمان اسکولیوز با علت ناشناخته مورد حمایت قرار نداد، ولی دو بازبینی سیستماتیک در سال های 2008 و 2011 موثر بودن ورزش را در کاهش نرخ پیشرفت و بهبود زاویه انحراف به اثبات رسانید.
    نتیجه گیری
    نتایج نویدبخش حاصل از مطالعات ذکر شده یک بنیان را برای این فرضیه که «ورزش فیزیکی در درمان اسکولیوز با علت ناشناخته تاثیرگذار است» فراهم می آورند. فرضیه ای که هر چند باید به وسیله یک کارآزمایی تصادفی شاهددار با طراحی مناسب، مورد بازبینی قرار گیرد.

    مقدمه
    اسکولیوز (Scoliosis)، یک ناهنجاری سه بعدی ستون فقرات و قفسه سینه است که منجر به تغییر ارتباط بین اجزای سیستم اسکلتی بدن، آناتومی ستون فقرات و تقارن تنه شده و باعث تغییر الگوی راه رفتن بیماران می شود. اسکولیوز موجب تغییر وضعیت مرکز ثقل بدن و نحوه توزیع وزن بر اندام تحتانی می گردد. بنابراین می توان استنباط کرد که بیماران مبتلا به اسکولیوز تعادل کم تری نسبت به همسالان سالم خود دارند
    مواد و روش ها
    جستجوی مقالات مرتبط با اسکولیوز و تاثیر این ناهنجاری بر پارامترهای کینتیکی و کینماتیکی و نیز تاثیر بریس بر پارامترهای ذکر شده با استفاده از سایت های جستجوگر PubMed، Google scholar و ISI web of knowledge انجام گرفت. مقالات مرتبط با این حوزه بین سال های 1998 تا 2012 منتشر شده بودند.
    یافته ها
    با استفاده از کلمات کلیدی مدنظر 54 مقاله به دست آمد. 26 مقاله بر اساس عنوان و چکیده، مطابق با موضوع تحقیق بود و 6 مقاله دیگر با بررسی رفرنس مقالات انتخاب شده در مطالعه شرکت داده شدند. مقالات محدودی تاثیر بریس را بر متغیرهای کینتیکی و کینماتیکی مورد بررسی قرار بودند.
    نتیجه گیری
    با بررسی مطالعات موجود نمی توان به نظر واحدی در مورد تاثیر اسکولیوز بر پارامترهای کینتیکی و کینماتیکی و نیز تاثیر بریس بر این پارامترها رسید.

    مقدمه
    اسکولیوز یکی از رایج ترین مشکلات ارتوپدی در کودکان و بزرگسالان است که علت آن مشخص نیست. اسکولیوز یک بدشکلی در سه صفحه است که در صفحه فرونتال و عرضی قابل ملاحظه می باشد. این بیماری با تغییراتی که در آناتومی ستون فقرات و لگن ایجاد می کند باعث تغییر در پارامترهای گیت می شود. هدف از انجام این مطالعه، بررسی تغییرات پارامترهای کینماتیکی در اندام تحتانی، ستون فقرات و اندام فوقانی افراد اسکولیوزی در مقایسه با افراد نرمال است که در اثر اسکولیوز ایجاد شده است. با مشخص شدن الگوی دقیق این تغییرات و تاثیر شدت اسکولیوز بر روی این پارامترها می توان از این داده ها برای بررسی پیشرفت و یا بهبود اسکولیوز استفاده کرد.
    مواد و روش ها
    با استفاده از کلید واژه های اسکولیوز (Scoliosis)، گیت (Gait)، کینماتیک (Kinematic) در سایت های Google scholar، PubMed، Elsevier، Ovid و Springer مقالات مرتبط مشخص شد و با توجه به معیارهای ورود و خروج مقالات بررسی شدند. با استفاده از روش Black and tool down مقالات امتیازدهی شدند.
    یافته ها
    با توجه به روش جستجو، 25 مقاله پیدا شد و با در نظر گرفتن معیارهای ورود و خروج 9 مقاله مورد بررسی قرار گرفت. مقالات از سال 1998 تا 2012 هستند و به صورت طرح های پژوهشی به مقایسه متغیرها بین افراد سالم و مبتلا اسکولیوز پرداخته اند. متغیرهای کینماتیکی اندام تحتانی کاهش دامنه حرکتی را نشان دادند که البته الگوی حرکتی تغییر زیادی نداشته است. بیشترین عدم تقارن در داده ها در تنه مشاهده شد و مشخص گردید فقط در مواردی که فرد یک انحنای بزرگ ناحیه پشتی دارد، چرخش لگن در جهت انحنا است و در موارد دیگر انحنا، تغییرات لگن قابل پیش بینی نیست. لگن تغییراتی نیز در عکس های رادیولوژی نشان می دهد که با توجه به ارتباط آناتومیکی بین لگن و ستون فقرات کمری انحنای ناحیه توراکولومبار و لومبار بیشترین تاثیر را در لگن دارد.
    نتیجه گیری
    بیشتر مقالات به بررسی پارامترها در حرکت با سرعت های مختلف پرداخته اند. برای دستیابی به یک نتیجه جامع باید یک استاندارد از نظر نوع انحنا و شرایط گرفتن تست وجود داشته باشد، به خصوص در موارد اندام فوقانی که مطالعات کمی نیز صورت گرفته است.

    مقدمه
    اسکولیوز (Scoliosis)، یکی از ناهنجاری های سیستم اسکلتی- عضلانی است که بر عملکرد فرد طی راه رفتن و ایستادن اثرگذار است. اطلاعات کمی در مورد تاثیر ارتز بر پارامترهای تعادل و راه رفتن بیماران اسکولیوزی وجود دارد، بنابراین هدف از مطالعه حاضر، بررسی پارامترهای ذکر شده بود.
    مواد و روش ها
    در مطالعه حاضر یک دختر 8 ساله مبتلا به اسکولیوز شرکت داده شد. این بیمار دارای انحنای اسکولیوزی با شدت 29 درجه و راس انحنا در سطح 8T قرار گرفته بود. پارامترهای راه رفتن و تعادل بیمار با و بدون استفاده از ارتز با استفاده از سیستم آنالیز گیت مجهز به 7 دوربین و صفحه فشار (Kistler force plate) بررسی شد. ضمن آن که طول تاندون عضلات ارکتور اسپاین و عضلات مایل شکمی داخلی و خارجی هنگام راه رفتن توسط نرم افزار Open- SIMM مورد ارزیابی قرار گرفت.
    یافته ها
    به نظر می رسد ارتز باعث بهبود عملکرد بیماران حین راه رفتن و ایستادن می شود، ضمن آن که باعث کشش عضلات پارااسپینال در سمت تقعر انحنای اسکولیوزی می شود.
    نتیجه گیری
    ارتز باعث تنظیم راستای ستون فقرات و بهبود عملکرد بیمار در راه رفتن و ایستادن می شود. از آن جا که ارتز باعث افزایش طول عضلات پارااسپینال می شود، بنابراین می توان استنباط کرد که ارتز می تواند باعث اصلاح انحنای اسکولیوزی شود.

    Introduction

    Flat foot is one of the most common disorders which not only influences the performance of the foot, but also affects the alignments of the ankle, knee and hip joints. The performance of flat arched differs from that of normal subjects during walking. However, there is not enough information in the literature regarding the difference between standing stability. Moreover, the influence of use of foot insole on standing stability has not been yet evaluated. This study was aimed to investigate the stability of subjects with flat foot with and without insole.

    Materials And Methods

    Forty female students in Isfahan University of Medical sciences participated in this study. Then they classified into two equal groups of twenty normal and flat arched individual. Center of pressure (COP) was measured using Kistler force plate. The stability was evaluated by calculating the COP excursions, path length, and velocity in the mediolateral and antroposterior directions. The differences between stability of normal and flat foot subjects with and without insole were evaluated by use of sample and paired t-test, respectively (α = 0.05).

    Results

    There was a significant difference between the stability of flat arched and normal subjects based on COP velocity and total velocity (P < 0.05). However, there was not a direct correlation between the severity of flat foot and instability during quiet standing (P > 0.05). The mean values ± SD of COP total velocity during standing with and without insole were 1645.42 ± 160 and 2752±683 mm/min, respectively (P < 0.001).

    Conclusion

    The flat foot is more unstable compared with normal subjects during quiet standing due to miss-alignment of foot structure and a decrease in proprioception awareness of the foot ligaments and tendon. Insole can help to align foot structure and also reduce the exceed forces applied on the ligament and finally improves the standing stability.

    Introduction
    Despite the facilitatory effects after applied whole body vibration (WBV), results of some studies have shown the increase of muscle response depression. The purpose of this study was to investigate the acute response of H-reflex recruitment curve in soleus muscle after one session of WBV.
    Materials And Methods
    Twenty six non-athletes volunteer healthy women (mean age ± SD; 26.11 ± 2.85 years) were participated in this study. All subjects received two different sham and test sessions. They stood on the vibration plate in semi squat position. In test session, WBV was applied with frequency of 30Hz and peak to peak amplitude of 3mm during 3 minutes. Soleus H-reflex recruitment curve was recorded before, immediately after, 5 and 10 minutes after WBV.
    Results
    The intensity of H-reflex recruitment curve threshold and Hmax were significantly increased immediately after WBV in both sessions (P < 0.05). Peak to peak amplitude of initial H-reflex were prominently decreased in both sessions whereas, peak to peak amplitude of Hmax was decreased only in sham session (P < 0.05). The analysis of threshold and peak to peak amplitude, the parameters of H-reflex recruitment curve, between two sessions showed no significant difference (P > 0.05).
    Conclusion
    Considering the right shift of H-reflex recruitment curve and depression response in two sham and test sessions, it seems that maintaining of semi squat position and activated control mechanisms, such as extensor muscles contraction, affects this inhibitory response and WBV had no facilitatory effect on this depression response.

  • Evaluation of the efficacy of psycho-educational group therapy in general health promotion on family of patients with bipolar disorders: A pilot study

    Introduction
    Family caregivers of patient with psychiatric disorder are actually the base of health care systems. Caring patients with psychiatric disorder is a stressful responsibility for family members. The purpose of present study was to evaluate the efficacy of group psycho -educational interventions in general health promotion on family of patients with bipolar disorders.
    Materials And Methods
    This research was a semi-experimental study with control group and random allocation. In this study, 6 subjects in experimental group and 9 individuals in control group were recruited. All participants were family members of patients with mood disorders. They completed general health questionnaire. Experimental group attended twelve sessions of group psycho -educational interventions, but control group received no intervention. Analysis of covariance was used for statistical analyses.
    Results
    Our findings showed that patients significantly improved in general health (P < 0.001).
    Conclusion
    These results indicated that group psycho-educational interventions can promote general health in family of patients with bipolar disorders.

    Introduction
    Vestibular evoked myogenic potential is a novel and reliable test which provides diagnostic information about the saccule function and/or inferior vestibular nerve; it also completes the present vestibular tests, and evaluates the sacculocollic reflex. Vestibular evoked myogenic potential amplitude is proportional to the level of background EMG that it shows the necessitate of maintaining constant tonicity of muscles during test recording. Change in eye position results in change in sternocleidomastoid muscle activity and consequently change in vestibular evoked myogenic potential amplitude. Therefore, this study aimed to assess if eye open and closed has effect on vestibular evoked myogenic potential parameters.
    Materials And Methods
    In this cross-sectional study, vestibular evoked myogenic potential was recorded for forty normal subjects, aged 18 to 30 years old, using 500 Hz-tone bursts (95 dBnHL).
    Results
    Considering the results of both ears, there was no significant difference between mean amplitude, p13 and n23 latencies of the two situations (P > 0.05).
    Conclusion
    Eye open and closed has no significant effect on vestibular evoked myogenic potential. Therefore as necessary and for artifact rejection one can be recorded vestibular evoked myogenic potential with close eyes.

    Introduction
    Constraint- Induced Movement Therapy (CIMT) and bimanual approach (BIM training) are a promising treatment for upper limb function improvement in children with spastic hemiplegia. CIMT involved restraint of non-involved upper extremity and intensive practice with involved upper extremity. BIM training is complementary to other treatments of upper extremity and improves bimanual coordination using structured task practice embedded in bimanual play and functional activities. This study was aimed to evaluate the influence of combination of CIMT with BIM training on fine motor skills of children with hemiplegic cerebral palsy.
    Materials And Methods
    In this study, twenty five children with hemiplegic cerebral palsy aged 5-10 years participated and randomly assigned into two intervention and control groups. Children in Control group received current occupational therapy intervention. Subjects in intervention group received a combination of CIMT and BIM training in addition to current occupational therapy. Each session was started with restraint on non-involved upper extremity and practicing with involved upper extremity for three hours. This was followed with BIM training for another three hours. This process lasted for 10 out of 12 consecutive days. Upper limb function and muscle tone were assessed by Jebsen-Taylor test of hand function and modified Ashwort scale, respectively.
    Results
    Upper limb function in intervention group significantly improved (P < 0.05) compared with control group, however the level of spasticity was not significantly different between two groups (P > 0.05).
    Conclusion
    Our findings suggest that combination of constraint induced movement therapy and bimanual intensive therapy improved upper limb Function in the hemiplegic cerebral palsy children.

    Introduction
    Rehabilitation is an ongoing process with a range of different specialties. Team working is one of the important aspects of this process. Rehabilitation team includes physiatrist, physiotherapists, occupational therapists, speech therapists and orthotic and prosthetic experts. So, the attitudes of these members towards team working will influence the quality of rehabilitation process. The purpose of this study was attitude survey of mentioned experts towards team working process.
    Materials And Methods
    This is a descriptive and cross sectional study. The sampling method was enumeration and statistical population included physiatrist and residents, professionals of occupational therapy, speech therapy, physical therapy, orthotics and prosthetics whom working in public and private treatment centers in the city of Shiraz. Information gathering tool was a researcher-designed attitude survey questionnaire. Cronbach's alpha coefficient is applied to check the questionnaire reliability. Factor analysis is used for investigating the structural characteristics of questionnaire, and T-test is used to check the attitude status.
    Results
    Cronbach's alpha coefficient of 0/87 confirms the good reliability of the questionnaire. Based on findings, all occupational groups have a positive attitude towards team working (P < 0.05). Effective and risky factors within teamwork process were extracted from the members’ perspective with regard to estimate the percentage of answers to the items of questionnaire.
    Conclusion
    Client-centered approach, sufficient knowledge to other members’ skills, and compliance with ethical and professional values are some of effective factors in teamwork improvement. On the other hand, personal disputes between members, poor communication within the group members, inappropriate deemed of requesting help and lack of sufficient knowledge to the team working process are risky factors.

    Introduction

    The effect of abnormal foot structure on dynamic function in lower extremity, particularly among children and adolescents has yet received little attention. The purpose of this study was to determine the electromyographic (EMG) activation patterns of selected lower limb muscles and plantar pressure characteristics during gait in teenage boys with normal and flat foot deformity.

    Materials And Methods

    Twelve subjects with flat foot and twelve with normal foot defined by arch index method participated in this study. Surface electromyographic activity of selected lower limb muscles for tibialis anterior, peroneus longus, soleus, medial and lateral gastrocnemius were recorded. Plantar pressure characteristics were measured at their self-selected walking speed along a 10 m walkway. Independent t-test was used for statistical analyses.

    Results

    EMG of medial and lateral gastrocnemius muscles were significantly greater in individuals with normal foot compared with flat foot group during loading response phase (P = 0.001) While medial gastrocnemius showed a decreased activation in terminal stance phase (P = 0.048). Peak pressure under foot regions including hallux and other toes, head of 2nd and 3th metatarsals and medial part of heel was greater in pes planus group. Center of pressure displacement was different between two groups during loading response, mid stance, and stance phase (P = 0.011, P = 0.015, and P = 0.050, respectively).

    Conclusion

    Abnormal foot posture could change the lower limb muscle activity and plantar pressure parameters during gait. The results of this study suggest that further studies can focus on the effects of foot abnormality on dynamic function in lower extremity to understand the underlying mechanisms and also set the suitable treatment manner.

    Introduction
    The number of subjects using ambulatory assistive devices during standing and walking is increasing. Various kinds of walkers and crutches have been designed to improve the performance of subjects. However, they have been prescribed based on some criteria which have not been tested clinically. Moreover, it is not clear what the difference between crutch and walker is which type of crutch is suitable; and also there is any difference between the new and traditional designs of walker and crutch. Therefore, the aim of this review is to answer to the above mentioned questions.
    Materials And Methods
    An electrical search was performed in PubMed, ISI web of science, Embase, and Google scholar from 1960 to 2012.The abstract and title of papers were evaluated based on selected criteria. The quality of the papers was evaluated based on Black and Down tool.
    Results
    Nearly twenty four papers have been selected for find analysis. The quality of most of studies was poor. Many researches were done on normal subjects. Moreover, only some parameters like force applied on leg during walking and energy consumption were evaluated. The available evidence is not enough to distinguish between the performance of the crutch and walker.
    Conclusion
    Based on the available research studies, the difference between various types of walkers, crutch and advantage of walker upon crutch is not cleared. The influence of crutch and walker length on the performance of subjects is controversial. Moreover, the sample size was not large enough to have a strong conclusion. Therefore, it is recommended to do a good research study with an acceptable sample size to test the various kinds of crutch and walker on the handicapped subjects.

    Introduction
    Rotatory chair test is used to evaluate the peripheral vestibular system and central compensation. It is also applied to follow up disease and evaluate specific management in some disorders. Analysis parameters are phase, gain, and asymmetry. This study was aimed to analyze previous researches based on rotatory chair testing protocol and its clinical application.
    Materials And Methods
    The present study had a review design. To find and collect related articles some key words like rotatory chair, central compensation, and nystagmus were used. All searches performed on Google Scholar, Science Direct, EBSCO, Pubmed and Thieme.
    Results
    Fifty eight original papers and 4 review articles were initially found. Finally, twenty of them were selected based on identified inclusion and exclusion criteria.
    Conclusion
    Due to some advantages such as the use of physiological stimulus, the ability to evaluate central compensation with asymmetry, and the ease of performance, rotatory chair can today be easily utilized in evaluating vestibular protocol.

    Introduction
    Scoliosis is a common deformity of spinal column. Most previous researches have concentrated mainly on kinematic parameters. However, this spinal abnormality is also a structural deformity affects on the normally symmetrical vertebral column, alters the centre of mass position, and weight distribution on the lower limbs. Therefore, kinetic evaluation can lead to a better understanding of the etiology of scoliosis. The aim of this study was to evaluate kinetic parameters in scoliotic subjects and compare with normal participants considering previous studies.
    Materials And Methods
    In this research, some key words like scoliosis, kinetic, ground reaction force, center of pressure, moment, and intervertebral force were used to find suitable articles. These researches were found on data bases such as Google Scholar, PubMed, Elsevier, Ovid, and Springer. Papers were selected based on identified inclusion and exclusion criteria. Then articles were scored by downs and black tool.
    Results
    Among thirteen articles, eleven were selected from 1998 to 2012. For symmetric index, researchers indicated an asymmetry, but these asymmetries did not follow any special pattern with curves. Also, researchers showed that maximum displacement in center of pressure occurred on mediolateral direction. Another article investigated kinetic parameters evaluation during lateral steps, because in this gait pattern asymmetries were greater between two normal and patient groups.
    Conclusion
    According to previous studies, there was an asymmetry in parameters. But, there is not any relationship between asymmetry and curves pattern in scoliotic subjects.

    Introduction
    The idiopathic scoliosis is a spinal deformity develops in three-dimensions mostly occurring in female adolescents. Untreated scoliosis can progress to where it results back pain, impaired vital capacity, and psychosocial disturbances. It seems that exercise-based therapies can be useful in the treatment of scoliosis; therefore the aim of this review is to clarify the effectiveness of physical exercises in the treatment of idiopathic scoliosis.
    Materials And Methods
    We performed a search through different international databases (Ebsco, Medline (via PubMed), and ISI Web of Knowledge) with the following keywords: scoliosis combined with review, scoliosis combined with exercise, scoliosis combined with sport and scoliosis combined with rehabilitation. The inclusion criteria were both; patients treated solely by the physical exercises and systematic review of the study design.
    Results
    Finally, Five articles fulfilled the inclusion criteria. Three of them had revealed poor quality evidence supporting the use of exercise in the treatment of idiopathic scoliosis. Two systematic reviews in 2008 and 2011confirmed the efficacy of exercises in reducing the progression rate and/or improving the Cobb angles.
    Conclusion
    The encouraging results provided by the discussed studies can give birth to the hypothesis that physical exercises are effective in treatment of adolescent idiopathic scoliosis, a hypothesis that must be verified by a well-designed randomized controlled trial.

    Introduction
    Scoliosis, is a three dimensional deformity of the spine and ribcage that can lead to changes in the relationship between body segments, spinal anatomy, trunk symmetry and cause pathological gait pattern. Scoliosis also is a structural deformity, so it can alter center of mass (COM) position and weight distribution on lower limbs. It seems that individuals with scoliosis deformity have the poorer stability comparison with normal subjects.
    Materials And Methods
    We conducted a Medline search via PubMed, Google scholar and ISI web of knowledge to identify studies on scoliosis field and the effect of this deformity on kinetic and kinematic parameters as well as the influence of bracing on mentioned parameters. The publication dates of the full-length articles were between 1998 and 2012. Some key words such as kinetic, kinematic, gait, and walking were used in combination with scoliosis to search in database.
    Results
    Multiple Medline searches led to the find out fifty four papers that twenty six of which were relevant based on title and abstract. 6 more articles were retrieved according to their references. Finally, thirty two papers were considered in this literature review. A few studies performed the influence of brace on kinetic and kinematic parameters in scoliotic subject.
    Conclusion
    There was no sufficient robust evidence to judge about the influence of scoliosis deformity on kinetic and kinematic parameters.

    Introduction
    Scoliosis is one the most common orthopedic problems in children and adults which is still idiopathic. Scoliosis is a three planar deformity and results in changes in anatomy of spine and pelvic that alters gait parameters. The purpose of this study was to evaluate changes in kinematic parameters in lower limb, upper limb, and trunk in scoliosis patients compared with normal subjects. Determining the exact pattern of these changes and the effect of it's severity on this parameters can be used to check the progression or improvement of scoliosis.
    Materials And Methods
    To find and collect suitable articles some key words like scoliosis, gait, and kinematics were used in data bases such as Google Scholar, Pubmed, Elsevier, Ovid and Springer. Papers were selected based on identified inclusion and exclusion criteria. Then articles were scored by Downs and black tool.
    Results
    Among of twenty five articles, 9 articles were selected on basis of inclusion and exclusion criteria from 1998 to 2012. Step length was the only temporospatial parameter that showed a significant reduction. Kinematic parameters in Lower limb identified reduced range of motion, but motion pattern did not show any significant difference. The most differences in data were observed in trunk and pelvic rotation. It was cleared only in person who has a large curvature of the thoracic spine and the same direction of pelvic rotation. Pelvic changes were not predicted in other cases. Pelvic also indicated some changes in x-ray and according to anatomical relation between pelvic and lumbar, thoracolumbar and lumbar curves had the most effect on the pelvic changes.
    Conclusion
    The majority of these articles investigated walking parameters in different speeds. In order to achieve the comprehensive outcomes, the standard model should exist with regarding a curve type and test condition, especially in the upper limb that has been paid low attention, so far.

    Introduction
    Scoliosis is one of the musculoskeletal disorders which influence the performance of subjects during standing and walking. There is no enough information regarding the influence of orthosis on gait parameters and stability functions in scoliotic subjects, therefore the aim of this study is to investigate the effects of orthosis on the mentioned parameters. Case Report: An 8 years old scoliotic girl was recruited in this study. She walked and stood with and without Milwaukee orthosis. Joint movements and force applied on the leg were measured using motion analysis system and Kistler faceplate. The length of erector spine, external oblique and internal oblique abdominalis tendons was evaluated during walking with and without orthosis by use of Open SIMM software
    Results
    The orthosis seems to improve the performance of subjects during standing and walking. Moreover it stretches the contracted muscles on the concave side of vertebral column.
    Conclusion
    The orthosis can align the vertebral column and improve subject abilities to stand and walk. As the length of vertebral column muscles increases following the use of orthosis, it can be concluded that it influences on the curve correction.
