    با توجه به اینکه حجم عظیمی از سرمایه های ملی برخی کشورها صرف ارتقای پژوهش به عنوان یکی از زیرساخت های توسعه ای می گردد. شناخت و ارزیابی جریان پژوهشی هر کشور برای سیاستگذاران و نیز محققان هر کشور امری ضروری است. این بررسی می تواند با روش علم سنجی مقالات پژوهشی منتشر شده در نشریات علمی کشورها صورت گیرد. در این اندازه گیری کمی می توان، به تعیین فراوانی، نوع، روند، تجزیه و تحلیل پژوهش ها و زمینه های علاقمندی و فعالیت پژوهشگران پرداخت. از سوی دیگر توسعه اخلاق زیستی در دهه های اخیر به واسطه مواجهه جامعه بشری با چالش های نوین، امری غیر قابل انکار است. با پیشرفت های علمی، سوالات اخلاقی و نیازهایی که به فراخور تعمیق خودمختاری انسان، روز به روز افزون تر می گردند به پاسخ های کاربردی نیاز دارند. لذا لزوم بازنگری در تمامی سطوح فعالیت در حوزه اخلاق پزشکی جهت ارائه کارآمدترین پاسخ ها به بیشترین پرسش ها امری بدیهی می نماید. تحقیقات بسیاری در حوزه های مختلف اخلاق زیستی در ایران در حال اجراست و ما شاهد تولید و نشر مقالات متنوعی در موضوع اخلاق زیستی و پزشکی در کشور هستیم. شاید مهم ترین هدف در ارزیابی پژوهش ها و تعیین شاخص های دقیق کمی و کیفی در این حوزه، بررسی هماهنگی مطالعات با نیازها و حرکت در راستای برنامه های توسعه ای کشور باشد. فقدان بانک اطلاعاتی منسجم برای ثبت و طبقه بندی مقالات، پایش و ارزیابی و در نتیجه سیاست گذاری ها را با مشکل مواجه می نماید. لذا در چنین شرایطی سوالی که در محافل علمی از سوی علاقمندان و فعالین اخلاق زیستی مطرح می شود این است که آیا روند امور پژوهشی و در نتیجه تولید و انتشار مقالات منجر به تولید یا ترویج علم شده است یا خیر؟ چقدر این تولیدات در راستای نیازها بوده است؟ خلاها و نقاط مغفول مرتبط به چه موضوعاتیست؟ و آیا موضوعاتی هست که علیرغم اولویت پایین، مورد بیش توجهی قرار گرفته باشد؟ پاسخ به این پرسش ها ما را در تدوین برنامه کلان پژوهشی اخلاق پزشکی، برای رفع نقاط ضعف و تقویت نقاط قوت این عرصه یاری می نماید.

    حساسیت اخلاقی توجه به ارزش های اخلاقی و لحاظ کردن آن در تصمیم ها و در نظرگرفتن نقش و وظیفه آن به خصوص در مواجهه با شرایط ویژه است. با توجه به نیاز به شناسایی روش های آموزشی موثر در افزایش حساسیت اخلاقی در پژوهش حاضر تاثیر آموزش کارگاهی و پیگیری آموزش بر حساسیت اخلاقی پرستاران مورد سنجش قرار گرفت.
    در این پژوهش مداخله ای از نوع سری زمانی 25 پرستار در کارگاه دو روزه با موضوعات اخلاقی شرکت کردند و سپس به مدت دو ماه آموزش به صورت پیگیری از طریق پیام های اخلاقی دریافت نمودند. از پرسشنامه حساسیت اخلاقی «هان» برای جمع آوری اطلاعات استفاده شد. داده ها به وسیله آزمون من ویتنی یو و فریدمن تجزیه و تحلیل شدند. تمام ابعاد حساسیت اخلاقی پس از انجام آموزش به وسیله کارگاه و پیگیری افزایش یافتند (P<0.05). کارگاه تاثیر قابل ملاحظه ای بر حساسیت اخلاقی پرستاران دارد که با پیگیری این آموزش می توان اثرات آنرا حفظ نمود.

    رضایتمندی بیماران یکی از مشخصه های اثربخشی بیمارستانی محسوب می شود یکی از مهم ترین مسائل در بحث رضایتمندی بیماران رعایت منشور حقوق بیمار است. حمایت و اجرای حقوق بیمار توسط پرستاران زمانی محقق می شود که آنان آگاهی کافی و لازم را در این زمینه داشته باشند. این مطالعه با هدف «بررسی نیاز آموزشی پرستاران زن در خصوص حقوق بیمار» انجام گرفت.
    در این مطالعه توصیفی مقطعی 119 پرستار زن با روش تصادفی انتخاب و مورد بررسی قرار گرفتند. جمع آوری داده ها با پرسشنامه دو قسمتی انجام شد. میزان سطح آگاهی در سه طبقه خوب، متوسط و ضعیف رتبه بندی گردید. پس از تهیه پرسشنامه و تعیین اعتبار محتوا و پایایی بین واحدهای مورد پژوهش توزیع و سپس اطلاعات با نرم افزار SPSS ویرایش 15 و آمار توصیفی و استنباطی تجزیه و تحلیل شد.
    سطح آگاهی 8/60 درصد از پرستاران زن در مورد حقوق بیمار خوب بود که با سن (004/0=P) و سابقه کار (013/0=P) ارتباط آماری داشته است. بیشترین(1/83٪) میزان آگاهی پرستاران در حیطه «حق اعلام رضایت شخصی از خاتمه درمان و مراجعه به سایر مراکز» و کمترین (3/40٪) آن نیز در حیطه «حق دریافت اطلاعات ضروری در مورد گروه درمانی، میزان تعرفه و پوشش بیمه ای» بود.
    در پرستاران با سطح آگاهی پایین و متوسط نیاز است روش هایی برای افزایش میزان آگاهی صورت گیرد. با توجه به آگاهی روزافزون بیماران از حقوق خود و برای جلوگیری از چالش های احتمالی در روابط بیمار و پرستار نیاز است پرستاران زن با منشور حقوق بیمار آشنایی کاملی پیدا کنند.

    مفهوم سلامت باروری به عنوان جزء اصلی سلامت جامعه توسط همه کشورهای منطقه مدیترانه شرقی از جمله ایران مورد پذیرش قرار گرفته است. مباحث حقوقی و اخلاقی سلامت باروری در ایران از جنبه های متعددی حائز اهمیت می باشد. طراحی برنامه ها و روش های جدید مراقبتی و درمانی در حیطه سلامت باروری انتخاب های جدیدی را برای زنان واقع در سنین باروری اعم از روش های جدید غربالگری و تشخیص و درمان را به ارمغان آورده است و از طرفی دیگر این پیشرفت فناوری مباحث اخلاقی متعددی را برای ارائه کنندگان خدمات سلامت باروری، زنان و جامعه برای تطابق با آن مطرح نموده است. یک مطالعه مروری با هدف بررسی سلامت باروری از منظر اخلاق و حقوق انجام گردید که طی آن با جستجو در پایگاه های اطلاعاتی SID، IRAN DOC، PUBMED، CINAHL و MEDSCAPEاز سال 1990 تا 2012 با استفاده از کلید واژگان Reproductive Health، Ethics، Law تعداد 40 مقاله مرور شدند. به نظر می رسد در ایران علی رغم وجود پیشینه ارزشمند تاریخی و اسلامی در زمینه حقوق و اخلاق پزشکی به تلاش بیشتری در این خصوص نیاز هست. توجه بیشتر به این مباحث در ارائه خدمات سلامت باروری می تواند سبب ارتقای کیفیت خدمات شده و علاوه بر تامین رضایتمندی مددجو و رعایت حقوق مصوب آنان به ارائه کنندگان خدمات نیز کمک کند تا در یک چهارچوب اخلاقی و قانونی نظام مند خدمات خود را ارائه نمایند.

    (Xenotransplantation)[i] کلمه ای یونانی است که به انتقال ارگان، بافت و سلول از گونه ای به گونه دیگر اطلاق می شود (حیوان به انسان). برخی از دلایل توجیه انتقال عضو از حیوان به انسان شامل محدود بودن اعضای انسانی قابل پیوند، انتظار طولانی مدت بیماران برای دریافت عضو انسانی و ایجاد آسیب های برگشت ناپذیر یا مرگ به دلیل تاخیر یا عدم دریافت عضو و نامحدود بودن اعضای حیوانات، می باشد. از دلایل عدم پذیرش انتقال عضو از حیوان نیز می توان به مواردی مثل خطر انتقال بیماری ناشناخته از حیوان به انسان و عدم امکان کنترل بیمار برای پیش گیری از انتقال این بیماری ها به سایر افراد، عدم پذیرش بیمار توسط اجتماع، احتمال مشکل روحی در بیماری که عضو حیوانی در بدن دارد و در نتیجه کاهش کیفیت زندگی وی و اعتقادات دینی مربوط به مساله طهارت و نجاست بعضی از حیوانات، اشاره کرد. بنابراین غفلت و بی توجهی نسبت به عواقب اخلاقی، حقوقی و اجتماعی ناشی از این نوع پیوندها ممکن است لطمه های غیرقابل جبرانی بر جای گذارد.

    اتخاذ بسیاری از تصمیمات در اخلاق پزشکی به ویژه در ملاحظات اخلاقی آغاز و پایان حیات به تعریف ما از انسان و جایگاه اخلاقی او بستگی دارد. برای شخص انگاشتن فردی از گونه انسانی ملاک های گوناگونی مطرح شده است از جمله: زمان لقاح، لانه گزینی در رحم، تکامل سیستم حسی و عصبی، حرکت جنین در رحم مادر، تولد و آگاهی. زمان تبدیل موجود انسانی به شخص در دیدگاه های مختلف متفاوت است. در باور ادیان ابراهیمی انسان علاوه بر ابعاد جسمانی دارای جنبه های روحانی نیز بوده و با استناد به همین امر می توان شان اخلاقی او را تعریف نمود. در این دیدگاه شخص انگاری موجود انسانی هم زمان با روح دارشدن او معنا می یابد، اما روح و نحوه ولوج آن به بدن پدیده ای غیر مادی است. این سخن به معنای نفی تاثیر و ارتباط این پدیده یا تقارن آن با برخی آثار مادی و جسمانی در انسان نیست. پس، این احتمال وجود دارد که به جهت ارتباط تنگاتنگ روح با بدن، بتوان بروز تحولاتی در جسم را مشخص نمود که از طریق آن ها بتوان زمان ولوج روح را حدس زد یا چنین ادعا کرد که برای روح دارشدن جنین رسیدن به حد خاصی از تکامل جسمانی لازم است. در این راستا امکان طرح این پرسش فراهم می آید که گرچه رویکرد فقه اسلامی به مساله ولوج روح و تعیین زمان حدود 120 روزگی جنین برای وقوع چنین امری، رویکردی عملگرایانه و با توجه به اهمیت حیات انسان در دین اسلام است، اما آیا می توان پس از آنکه جنین از لحاظ فیزیکی کامل شد او را برای پذیرش روح آماده دانست و از آن زمان او را شخص انسانی به حساب آورد و به تبع آن کلیه آثار و تبعات اخلاقی مترتب بر شخص را بر او وارد دانست؟

    حقوق یک بیمار عبارت است از وظایفی که تیم پزشکی در قبال او دارند و به مجموع امتیاز و توانایی ها یا مجوزهای خاص گفته می شود که قانون به بیمار اعطا کرده است. در پژوهش حاضر، میزان آگاهی بیماران و رضایت آن ها را نسبت به رعایت منشور حقوق بیمار در چند بیمارستان خصوصی و دولتی شهر تهران مورد بررسی قرار دادیم. جامعه آماری پژوهش شامل 600 نفر از بیماران مراجعه کننده به بیمارستان های منتخب شهر تهران و ابزار گردآوری داده ها پرسشنامه بود. بین «میزان آگاهی» بیماران مراکز درمانی دولتی و خصوصی از مفاد منشور تفاوت معنی داری وجود نداشت. ولی در «اجرای مفاد» از نظر دریافت مطلوب خدمات سلامت و احترام به حریم خصوصی بیمار و رعایت اصل رازداری در بیمارستان های خصوصی و دولتی تفاوت آماری معنی داری وجود داشت. تا زمانی که بیماران از حقوق و تعهدات متقابل خود مطلع نباشند، خواستار رعایت آن نیز نخواهند بود یا این که اعتراض خود را به نحو نادرستی ابراز می کنند. در مصاحبه با بیماران، اکثرا وجود گروه های مختلف آموزشی مثل دانشجو، اینترنت و... را علتی برای عدم دریافت خدمات سلامتی مطلوب و رعایت رازداری می دانند. برای رفع این مشکل، لازم است تیم درمانی به بیمار معرفی شوند.

    دیسترس اخلاقی پدیده ای شایع در حرفه پرستاری است که می تواند زندگی کاری پرستاران را تحت تاثیر قرار داده و تاثیرات متفاوتی را بر کارکنان، بیماران و سازمان های بهداشتی داشته باشد. مطالعه توصیفی حاضر جهت بررسی دیسترس اخلاقی در 220 نفر از پرستاران بیمارستان های آموزشی شهر بیرجند با استفاده از پرسشنامه دیسترس اخلاقی کورلی صورت گرفته است. این پرسشنامه 21 سوالی پس از ترجمه مورد روایی و پایایی قرار گرفته و روایی آن به صورت 88 درصد و پایایی آن (آلفای کرونباخ) 93 درصد محاسبه شد. نتایج به دست آمده حاکی از آن است که میانگین شدت دیسترس اخلاقی در واحدهای مورد پژوهش 25/2 (ازکل5-0) و میانگین تکرار آن 11/2 (از کل5-0) بوده است. بین دیسترس اخلاقی و ویژگی های دموگرافیک افراد همانند نوع بخش محل خدمت رابطه معنی داری وجود داشته است (05/0P<)، ولی این ارتباط با نوع استخدام و جنس پرستاران معنی دار نبوده است (05/0P>). با توجه به بالاتربودن سطح شدت دیسترس اخلاقی در پرستاران توجه هرچه بیشتر در جهت آشنایی آن ها با این پدیده و علل پیدایش آن بایستی مورد توجه قرار گیرد.

    برای بسیاری از زنان تصمیم گیری در مورد انتخاب روش زایمان کار ساده ای نیست و این تصمیم گیری تحت تاثیر عوامل بسیاری واقع می شود. مطالعه حاضر یک مطالعه مقطعی با هدف بررسی منابع دریافت اطلاعات به منظور انتخاب روش زایمان در زنان باردار مراجعه کننده به بیمارستان های شهر تهران انجام شد. افراد مورد مطالعه 1000 خانم باردار مراجعه کننده به بیمارستان های شهرتهران به منظور انجام زایمان بودند. جمع آوری اطلاعات از طریق مصاحبه و پرسشنامه انجام گرفت. نتایج مطالعه بیانگر آن بود که زنان باردار در1/4درصد اطلاعات خود را در ارتباط با روش زایمان از طریق رادیو و تلویزیون، 7/14 درصد از کتاب، 3/42 درصد از اطرافیان و 3/13 درصد از پرسنل بهداشتی به دست آورده بودند. همچنین بین شغل (001/0p=)، میزان تحصیلات (001/0p=)، سن مادر (001/0p=)، نوع بیمارستان (001/0p=)، ترس از زایمان طبیعی (032/0p=)روش زایمان (001/0p=) با منبع کسب اطلاعات ارتباط معنی دار آماری وجود دارد. با توجه به وجود ارتباط بین عوامل متفاوت با منابع اطلاعاتی به نظر می رسد ارائه اطلاعات صحیح، از طریق پرسنل بهداشتی می تواند نقش مهمی را در ارتقاء دانش و نگرش مادران در زمینه مذکور داشته باشد.

    در بررسی طرح های پژوهشی رسیده به معاونت پژوهشی دانشگاه علوم پزشکی اصفهان دیده شد که پژوهشگران، علی رغم تمایل به تهیه یک فرم رضایت آگاهانه متناسب، به دلیل عدم وجود یک دستورالعمل و در دسترس نبودن یک فرم نمونه جهت الگوبرداری، بعضا فرم های بسیار مختصر و ناکامل به پروپوزال ها ضمیمه می نمودند. این کار باعث برگشت پروپوزال و اتلاف وقت می شد. لازم بود یک ساختار مناسب برای تهیه فرم رضایت آگاهانه در اختیار پژوهشگران باشد.
    برای رفع این مشکل، این تحقیق به روش کتابخانه ای و با استفاده از منابع چاپی و الکترونیک، به جستجوی اجزای یک فرم رضایت آگاهانه پرداخته شد. همچنین به منظور تهیه یک فرم نمونه، تارنمای[i] همه دانشگاه های علوم پزشکی کشور بررسی شد.
    با استفاده از منابع یاد شده و مرور متون مرتبط به ویژه کدهای 26 گانه حفاظت از آزمودنی در پژوهش های علوم پزشکی جمهوری اسلامی ایران کلیه اجزاء قابل ذکر در فرم های رضایت آگاهانه استخراج شدند. این اجزا در دو دسته با عناوین «مواردی که لازم است در فرم قید شود» و «مواردی که بهتر است در فرم قید شود»، مشخص گردید. همچنین دو نمونه فرم رضایت آگاهانه نمونه ها هم که از وب سایت دیگر دانشگاه ها به دست آمده بود به عنوان نمونه در ذیل آن دستورالعمل قرار داده شد تا پژوهشگران محترم با توجه به نوع پژوهش خود و با عنایت به فرم های نمونه، نسبت به تهیه نمودن فرم متناسب و ضمیمه کردن آن به پروپوزال، اقدام نمایند.
    رعایت نکات و آیین نامه (کدهای) مصوب اخلاقی در همه انواع پژوهش های علوم پزشکی به ویژه پژوهش های علوم پزشکی دارای آزمودنی انسانی لازم و ضروری است. بهترین نماد رعایت این اصول اخلاقی گرفتن «رضایت آگاهانه» خوب و جامع از آزمودنی یا ولی قانونی اوست. از میان کدهای مذکور 9 کد - به صورت مستقیم یا غیر مستقیم- به موضوع مهم رضایت آگاهانه مربوط می شود.
    قاعدتا هر تحقیق در حیطه علوم پزشکی دارای طراحی[ii] منحصر به فرد است و بنابراین به فرم رضایت آگاهانه منحصر به فرد نیاز دارد که باید طراحی و مورد استفاده قرار گیرد. با استفاده از دسته بندی انجام شده در این تحقیق محقین خواهند توانست کار تدوین فرم رضایت آگاهانه منحصر به فرد هر تحقیق را خود متقبل شوند تا با اشراف و تسلطی که بر موضوع و محتوای تحقیق دارند بهترین و مناسب ترین فرم رضایت آگاهانه تحقیق را تدوین نمایند.

    مقدمه
    یکی از اساسی ترین حقوق بیماران، دادن رضایت به قبول یک روش معالجه است، همه مراکز مراقبتی ملزم به اخذ رضایت نامه درمانی از بیمار یا قیم وی می باشند.رضایت آگاهانه عبارت است از موافقت آزادانه و ابطال پذیر فرد واجد صلاحیت مبنی بر مشارکت در تصمیم گیری درمانی به دنبال آگاهی از ماهیت،هدف و پیامدهای آن و براساس مطالعات، رضایت آگاهانه و سهیم کردن بیماران در تصمیم گیری های مربوط به خود می تواند به بهبودی آنها سرعت بخشد. پژوهش حاضر با هدف تعیین فرایند میزان آگاهانه بودن رضایت کسب شده ازبیماران تحت عمل جراحی در دانشگاه علوم پزشکی بیرجند می باشد.
    مواد و روش ها
    در این پژوهش توصیفی- مقطعی 248 بیمار (رضایت دهنده) بخش های جراحی بیمارستان های دانشگاه علوم پزشکی بیرجند در سال 1389 به صورت تصادفی ساده وارد مطالعه شدند، داده ها از طریق مصاحبه با بیماران بعد از عمل جراحی با استفاده از پرسش نامه خود ساخته جمع آوری و با آمار توصیفی و استنباطی،تحلیل شد.
    یافته ها
    در 9/44% موارد رضایت از خود بیماران کسب شده بود که در 88% موارد از نوع کتبی بود و زمان اخذ رضایت 6/57% موارد، موقع پذیرش بود.میزان اطلاعات اکثر بیماران هنگام تصمیم گیری بالینی و کسب رضایت ضعیف،اما میزان درک وفهم بیماران و مشارکت در تصمیم گیری های بالینی و میزان تعامل پزشک با بیماران در حد متوسط بود. میزان درک و فهم بیماران از فرم رضایت نامه برحسب بیمارستان و شکل و زمان اخذ رضایت نامه معنی دار بود.همچنین میزان مشارکت بیماران در تصمیم گیری های بالینی بر حسب بیمارستان تفاوت معنی داری داشت.
    نتیجه گیری
    با توجه به نتایج، لازم است راهکارهایی درجهت افزایش میزان اطلاعات بیماران برای درک بهتر فرم رضایت نامه و مشارکت در تصمیم گیری های بالینی اتخاذ نمود

    Ethical Sensitivity is to consider the ethical values and considering them in decisions with attending to its professional role and duty, especially in critical situation. In this study, due to the need to recognize of effective educational Methods in increasing the ethical sensitivity, the effect of a workshop with ethical content and fallow-up with ethical messages was assessed on ethical sensitivity of nurses.In this interventional study of confidential type, 25 nurses participated in a two days’ workshop with ethical content. After the workshop they took two month fallow-up education. The Han Questionnaire in Moral Sensitivity was used for gathering the information. The data was analyzed by Mann-Whitney U and Freedman Tests.All domains of ethical sensitivity was increased significantly after fallow-up education (P<0.05).
    Discussion
    Workshop has a significant influence on ethical sensitivity. But the effects could be remaining by fallow-up.

    Patient satisfaction is one of the characteristics of the hospital and compliance with the patients’ Charter of Rights is one of the most controversial issues in patients’ satisfaction. Support and the implementation of patients’ rights will be met by nurses when they do not have sufficient and necessary knowledge in this area. This study was conducted in "Evaluation of educational needs of female nurse's regarding patients’ rights in hospitals in Tehran in 2010”.This cross-sectional study was conducted in 2010. In this study 119 female nurses were randomly selected. Two-part questionnaire was used for data collection. The awareness was ranked in three categories (good, moderate and week).The validity and reliability of questionnaire was determined and then was distributed between subjects. The data were analyzed by SPSS version 15 using descriptive and inferential statistics.The research study showed that knowledge of 60.8% of female nurses from patient rights was good which had a statistical connection with age range (P=0/004) and work experience (P=0/013). Most (83/1%) of the nurses’ level of knowledge was in “right to declare personal satisfaction of ending the treatment and referring to other centers” and the lowest (40/3%) level was in "the right to receive necessary information about the treatment groups, level of expenses and insurance coverage ".It is necessary to conducts methods to increase the level of knowledge in nurses with low and intermediate knowledge. According to the increasing knowledge of patients from their rights, it is necessary for female nurses to be completely aware of Patients’ Rights Charter to avoid possible challenges in the relationship of patients and nurses.

    All EMRO countries including Iran have adopted the concept of reproductive health as the main element of the society health. Ethical and legal topics of reproductive health in Iran are important from various aspects. Designing new care and treatment programs and methods in reproductive health area has brought new choices for women in reproductive age whether new methods of screening or diagnosis and treatments and from other hand this technology improvement has brought various ethical debates for reproductive health providers, women and society to respond it. In this regard, a review with the purpose of study of reproductive health from ethic and law point of view was established which in this review by researching in SID, IRAN DOC, PUBMED, CINAHL websites from1990 to 2012 and by using keywords from reproductive health, ethics, law key words, 40 assays were retrieved and reviewed.in general it seems that despite the valuable historic and Islamic grounds in law and medical ethics in Iran, more efforts is needed. Paying more attention to legal and ethical debates in offering reproductive health services In Iran can improve the quality of the services and along with providing the satisfaction of patients and their approved rights, it will be helpful for service providers to offer their services in a systematic ethical and legal framework.

    Xenotransplantation is a Greek phrase and is referred to the process in which an organ or living cell is transplanted from an animal into the human body.The reasons for transmission of an organ from an animal to a human include: Limitation of transplantable human organs, Long term waiting list to receive organ transplants, irreversible damages or death due to postponement of receiving transplant organs or not receiving it and illimitable animal organs. The reasons for rejection of animal-human transmission include: The risk of possible transmission of unknown diseases from animals to human and benign lack of controlling the patients to avoid transferring the disease to other people, rejection of patients by the community, possibility of psychological problems in patient who has animal organs in his/her body and as result decreasing patients’ quality of life and religious beliefs of purity and impurity of some animals.Therefore, inadvertency and inattention to ethical, legal and social consequences as a result of xenotransplantation may leave irrecoverable damages.

    Most of the decisions in Medical Ethic, especially in ethical considerations in beginning and end of life depend on our definition of embryo and his/her ethical status. There are various factors to recognize someone as he human species such as impregnation time, implantation in the uterus, evolution of nervous system and senses, the embryos movement in the uterus of the mother, birth and conscious. The time of transformation of a human being to a person is different in various points of views, in the beliefs of Abrahamic Religions; besides physical features humans had spiritual features and with citing on this matter his/her ethical status can be defined. In this point of view personalizing a human being will be defined with his/her ensoulment, but the spirit and the procedure of ensoulment is a spiritual matter. This does not mean to deny the effect and the relationship of this phenomenon or synchrony of it by some physical effects. So there is a chance that because of this close relationship of spirit and the body, one can be able to identify physical changes in the body, which by them one can be able to guess the time of ensoulment or be able to claim that for embryo to have a spirit, he needs a certain extent of physical evolution. In this regard it is possible to ask that although the Islamic juridical point of view about the ensoulment and to determine 120 day of pregnancy as the time of ensoulment is a practical view but can we consider the embryo ready for ensoulment after being improved physically and to consider him as a person and to take all of the ethical consequences in to effect?!

    individual rights are based on person needs and rights of a patient including the duties that medical team have towards him and the total score, abilities or special permits that the law has granted to the patients. In the present study, the patient's knowledge and consent of Patients’ Right Charter was studied in several private and public hospitals in Tehran.This study was done cross-sectional. The samples were selected from 600 patients referred to the hospitals in Tehran. The two-part questionnaire was used for data collection and the spss 17 software was applied for data analysis.There was no significant difference between the "knowledge" of patients in public and private medical centers. But there was a significant statistical difference in "implement of contents" of receiving proper health care, respecting patient privacy and confidentiality in public and private hospitals.Patients should be aware of their rights to acquire. If the patients are not aware of their mutual rights and obligations, they will not demand respecting them or they would request them in an inappropriate way. In interviews with patients, the existence of different study groups such as students, Internet,. .. was expresses as the reason of not receiving a proper health care, respecting patient privacy and confidentiality. To resolve this problem, it is necessary to introduce the care team to the patients.

    Moral distress is a common phenomenon in the nursing which can affect nurse's work life and a make a different impact on employees, patients and healthcare organizations. The present study is a cross-sectional study which has investigated the moral distress in 220 nurses in Birjand educational hospitals by Corley moral distress scale. This questionnaire has 21 question and has been translated and calculated, the validity was determined 88% and the reliably (Cronbach Alfa) 93%.The results indicate that the average intensity of moral distress in subjects is 25/2 (from 0-5) and the average of repeat is 11/2 (from 0-5) respectively. There is significant relationship between moral distress and demographic characteristics of the type of service (P <0/05). But this relationship would not make any sense in the type of employment and the sex of nurses (P>0/50). Due to the high intensity level of moral distress in nurses, paying more attention to familiarize them with the emergence of the phenomenon and its causes must be considered.

    It is not simple for women to decide the type of delivery and this decision-making would be affected by various factors. The present study is a sectional study seeking to review the data resources regarding the choice of delivery type in pregnant women who visit Tehran’s hospitals. The subjects of the study were 1000 pregnant women attending to Tehran’s hospitals for delivery. The Data were gathered by review and questionnaire. The result of the study showed that 4.1% of pregnant women received their information about delivery type from Radio and Television, 14.7% from family and 13.3% from health participants. There was also a significant statistical connection between job (p=0.001), education level (p=0.001), mothers age (p=0.001), type of the hospital (p=0.001), fear of natural delivery (p=0.032) and the resource of information. According to the connection between various factors and information resources, it seems that giving appropriate information by health participants can play a significant role in promoting the knowledge and view of mothers in this regard.

    Introduction
    Through investigating all research proposals reached at the research deputy of Isfahan University of Medical Sciences, it was observed that researchers tend to attach very short and incomplete forms to their proposals despite the common tendency towards preparing appropriate informed consent form which is mainly due to lack of access to a sample form for modeling as well lack of instructional manual. This leads to return of proposals and a sheer waste of time. Hence, it was necessary to accommodate a suitable structure for preparing informed consent form for researchers.
    Method
    in order to solve this problem, this research was carried out in a library-based mode and by using printed and electronic sources for searching the compartments of an informed consent form. In addition, in order to come up with a sample form, all websites of medical sciences universities were explored.
    Findings
    All mention-worthy resources in an informed consent form were extracted by using all aforementioned sources and going over relevant texts, esp. the 26-part codes for protecting subjects in researches done in Islamic Republic of Iran. These parts were identified within two groups titled, “the cases needed to be mentioned in the form” and “the cases which are better to be mentioned in the form”. Furthermore, two sample informed consent forms were placed under the instructions which had been retrieved from the websites of other universities so that reverent researchers would opt them for preparing appropriate form and append them to their proposals with regard to the type of their research and the sample forms.Discussion and
    Conclusion
    Attending to the ratified ethical codes and points in all researches done within medical sciences particularly those that involve human subjects are quite necessary. The best symbol for following such ethical guidelines is getting “informed consent” from subjects or their legal guardians. Among these codes, 9 refer directly or indirectly to the important issue of informed consent.bassically, any research done within medical sciences field has its own unique design and therefore, it requires a unique informed consent form which must be designed and used. By using the conducted classification in this research, researchers would be able to take care of preparing the informed consent form unique to each research on their own so that they could compile the best and most suitable research informed consent form by relying on their knowledge and prowess over their research content and topic.

    Background
    one of the fundamental rights of patients is giving consent to a particular treatment method, and all health care centers are required to obtain consent from the patients or their guardians. Informed consent is defined as free and voidable agreement of a qualified person regarding participation in treatment; by understanding the nature, purpose and consequences of the consent. Studies show that informed consent and shared decision-making can be related to the speed of the patients’ recovery. This study seeks to study the level of awareness in consents obtained from patients under surgery at the Medical University of Birjand.
    Methods
    In this descriptive-cross-sectional study, 248 patients (content provider) in Surgery wards of Birjand University of Medical Sciences enrolled randomly in this study in 2011. Data were collected through interviews with patients after surgery using a self-designed questionnaire and were analyzed by descriptive and inferential statistics.
    Results
    The 44.9% of consents were received of patients themselves, out of which 88% were in written form, and the time to get 57/6% of them was at the time of patient's admission. The extent to which patients were informed at the time of clinical decision-making and giving informed consent was poor, but the patient's understanding and participation in clinical decision-making and physician's interaction with patients were moderate. Patient's understanding of the consent form was significant according to the hospital and the time of receiving the consent form. Also, the patient's participation in clinical decision-making had significant differences according to the hospital.
    Conclusion
    According to the results, strategies must be devised to increase patient's information in order for their better understanding of the consent form and their participation in clinical decisions.
