    مقدمه
    ارگانیک اسیدوری ها دسته ی مهمی از بیماری های ارثی متابولیک هستند که در صورت عدم تشخیص و درمان زود هنگام، بسیاری از این اختلالات منجر به آسیب مغزی دایمی و یا مرگ می شوند. به همین دلیل، تشخیص دقیق و سریع این بیماری ها به منظور شروع درمان موثر از اهمیت بالایی برخوردار است. تشخیص این دسته اختلالات، مستلزم آنالیز ارگانیک اسیدهای ادرار توسط گاز کروماتوگرافی- طیف سنجی جرمی (GC/MS یا Gas chromatography/mass spectrometry) می باشد. از آن جا که عوامل ژنتیکی و محیطی و عادت های غذایی می تواند در غلظت و پروفایل ارگانیک اسیدها تغییر ایجاد کند، برای هر جمعیتی بایستی الگوی متناسب با همان جمعیت جمع آوری شود. چون چنین اطلاعاتی در مورد ایران وجود ندارد، پس لازم است تا فراهم گردد.
    روش ها
    140 نمونه ی ادرار راندوم از افراد سالم در 4 رده ی سنی نوزادان رسیده (< 30 روزه)، نوزادان نارس، کودکان با سن 1 ماه تا 2 سال و کودکان بالاتر از 2سال جمع آوری شد. علاوه بر آن، تعداد 10 نمونه از کودکان مشکوک به بیماری مادرزادی متابولیک جمع آوری گردید. پس از استخراج از ادرار و تهیه ی مشتق TMS (Trimethylsilyl) اسیدهای ارگانیک به روش GC/MS مورد آنالیز کمی و کیفی قرار گرفتند.
    یافته ها
    تعداد 61 ترکیب ارگانیک اسید در 140 نمونه ی ادرار طبیعی مطالعه شده، قابل اندازه گیری بود. این ترکیبات شامل اجزای طبیعی ادرار و همچنین نشانگرهای بیماری ها می باشند. غلظت این ترکیبات به صورت محدوده ی طبیعی 5/97-5/2 درصد گزارش شد. همچنین از میان 10 نمونه ی مشکوک به بیماری متابولیک، 2 بیمار مبتلا به متیل مالونیک اسیدمی، 1 بیمار مبتلا به گلوتاریک اسیدمی نوع 2 و 1 بیمار پروپیونیک اسیدمیا شناسایی شدند.
    نتیجه گیری
    آزمایش آنالیز ارگانیک اسیدهای ادرار تا به امروز در ایران انجام نگرفته است و نمونه ها برای آنالیز به خارج از کشور فرستاده می شدند. همچنین اطلاعات مربوط به غلظت و پروفایل اسیدهای ارگانیک ادرار برای جمعیت سالم کشور موجود نبود و این مقاله برای اولین بار این نتایج را گزارش نمود. با برپایی این روش در کشور و آنالیز ادرار بیماران مشکوک به اختلالات متابولیک، می توان با تشخیص سریع این بیماری ها و درمان به موقع از عوارض جبران ناپذیر آن ها جلوگیری کرد. این مطالعه همچنین نشانگر این بود که در درصد بالایی از بیمارانی که از لحاظ بالینی دارای علایم بیماری ارثی متابولیک هستند، این بیماری ها در حقیقت وجود دارد و به نظر می رسد این بیماری ها دارای شیوع بالایی هستند. همچنین به نظر می رسد که در ایران متیل مالونیک اسیدمی، شایع ترین بیماری در بین بیماران دارای علایم کلاسیک ارگانیک اسیدمی باشد.

  • بررسی 5 ساله ی شاخص های برنامه ی ریشه کنی فلج شل حاد در استان اصفهان و مقایسه ی آن با شاخص های استاندارد جهانی

    مقدمه
    شرط لازم جهت رسیدن به هدف ریشه کنی بیماری پولیومیلیت، ارزیابی و پایش روش ریشه کنی پولیومیلیت با اجرای نظام مراقبت (Surveillance) از این بیماری می باشد. این مطالعه با هدف تعیین وضعیت شاخص های ریشه کنی فلج شل حاد در اصفهان و مقایسه ی آن با شاخص های استاندارد جهانی طرح ریزی شد.
    روش ها
    در این مطالعه که یک بررسی به روش توصیفی مقطعی بدون جهت است، کلیه ی موارد فلج شل حاد گزارش شده در استان اصفهان از سال 86-1382، که به مرکز بهداشت شهرستان و استان گزارش شده بودند، به روش سرشماری مورد مطالعه قرار گرفتند. اطلاعات پس از مرور پرونده های بیماران استخراج و طبقه بندی شد و توسط نرم افزار آماری SPSS مورد تجزیه و تحلیل قرار گرفت.
    یافته ها
    تعداد 115 مورد بیمار با فلج شل حاد گزارش شد که از این میان، 41 درصد زن و 59 درصد مرد بودند. میانگین سنی مراجعین 83/3 ± 71/5 سال، با دامنه ی سنی 15-1 سال بود. میانگین بروز فلج شل حاد 74/1 محاسبه شد که کمتر از میزان مورد انتظار (2) است. از بین شاخص های کشوری، کشف موارد (80 درصد < در عرض 7 روز)، شاخص بررسی و معاینه ی موارد (80 درصد در عرض 48 ساعت)، شاخص به هنگام بودن نمونه گیری (80 درصد < طی 14 روز)، شاخص ارسال به هنگام نمونه ها به آزمایشگاه (80 درصد < طی 3 روز)، شاخص پیگیری روز 60 (90 درصد <)، شاخص طبقه بندی موارد (100 درصد طی 70 روز)، دو شاخص بررسی موارد و پیگیری روز 60 کمتر از سطح مورد انتظار با استاندارد جهانی بود.
    نتیجه گیری
    طبق مطالعه ی حاضر، نظام مراقبت در اصفهان جهت تشخیص موارد دچار فلج شل حاد دقیق عمل می کند، اما همچنان جهت ریشه کنی این بیماری نیاز به ارزیابی بیشتر و بهبود کیفیت نظام مراقبت وجود دارد. بنابراین، ادامه ی واکسیناسیون گسترده علیه ویروس فلج اطفال و پیگیری بیماران مبتلا به این بیماری توصیه می شود.

    مقدمه
    ویروس آنفلوانزای پرندگان تحت تیپ 2N9H در طول دو دهه ی گذشته در آسیا به صورت پانزوتیک در آمده است. این ویروس، علاوه بر ایجاد بیماری در طیور پرورشی، چندین مورد ابتلای انسانی در آسیا را سبب شده است. این مطالعه به منظور بررسی ارتباط فیلوژنی ژن غیر ساختمانی (NS یا Non structural) و خصوصیات رشدشان بر روی سلول های 549A در جدایه هایی در استان تهران انجام شد.
    روش ها
    از میان ویروس های ایزوله شده در سال های 91-1388 ویروس آنفلوانزای 2N9H، دو ویروس که در درخت فیلوژنتیک جایگاه های متفاوتی با هم داشتند، انتخاب شدند و ژن NS طی واکنش RT-PCR (Reverse transcription polymerase chain reaction) تکثیر شد. قطعه ی مورد نظر هر جدایه، کلون گردید و پلاسمیدهای حاصل توالی یابی شدند. بروز تغییرات احتمالی در توالی ژن NS جدایه های مورد نظر با مقایسه با جدایه های ثبت شده در پایگاه داده GenBank ارزیابی شد. بر اساس نتایج، رشد این دو جدایه در سلول های 549A بررسی شد.
    یافته ها
    پردازش اولیه ی داده های فیلوژنی نشان می دهد که ژن NS ویروس های ایران با همسانی 4/96 درصد به زیر دودمان 439Y و آلل A تعلق دارند. این ویروس ها بر اساس زمان جداسازی در دو زیر گروه قرار می گیرند: زیر گروه یک شامل ویروس های جدا شده از طیور پرورشی طی سال های 2004-1998 و زیر گروه دوم ویروس های جدا شده طی سال های 11-2006 بودند. ارزش s3GC و مقایسه ی میزان جایگزینی Ka/Ks در نواحی رمز دهنده ی ژن بررسی شد. ارزش s3GC بین 392/0-371/0 متغیر بود و میزان Ka/Ks، برای زیر گروه اول 38/0 و برای زیر گروه دوم 42/0 محاسبه شد.
    نتیجه گیری
    پردازش توالی اسید آمینه ی NS نشان می دهد که این دو ایزوله، دارای توالی KSLR در محل دمین PDZ خود هستند و چندین تغییر همزمان در طول توالی اسید آمینه رخ داده است. عیار دو ویروس به روش ارزیابی پلاک برآورد شد که تفاوت معنی داری با هم نداشتند. این داده ها نشان می دهد که دو ویروس 2N9H جدید ایران دچار تغییرات بسیار اندکی شده اند.

    مقدمه
    دیابت شایع ترین بیماری غدد درون ریز و یکی از شایع ترین بیماری های غیر واگیر دنیا است که سالانه افراد زیادی را درگیر می کند. جمعیت فراوانی از افراد، از ابتلا به دیابت از نفروپاتی و رتینوپاتی دیابتی رنج می برند. استرس های اکسیداتیو در بروز این دو عارضه نقش به سزایی دارند. آنزیم سوپراکسید دیسموتاز (SOD یا Superoxide dismutase) نقش عمده ای در سم زدایی گونه های فعال اکسیژن دارند. بنابراین، یک نقش محافظتی علیه نفروپاتی و رتینوپاتی دیابتی دارد. مطالعات مختلفی تاثیر پلی مورفیسم های مختلف این ژن را در بروز آسیب های بافتی افراد دیابتی آشکار کرده اند.
    روش ها
    در مطالعه ی حاضر، دو پلی مورفیسم تک نوکلئوتیدی در 145 نفر از افراد بیمار (گروه مورد) و 145 نفر از افراد سالم (گروه شاهد)، بررسی شد. نمونه های DNA بعد از جمع آوری نمونه ی خون افراد مورد و شاهد استخراج شد و پلی مورفیسم نمونه ها با به کارگیری روش PCR-RFLP (Polymerase chain reaction-Restriction fragment length polymorphism) مورد بررسی قرار گرفت.
    یافته ها
    با بررسی پلی مورفیسم 2070424rs در افراد مورد و شاهد مبتلا به نفروپاتی، توزیع فراوانی ژنوتیپی معنی داری مشاهده شد (005/0 = P و 55/7 = 2χ). در حالی که در افراد مبتلا به رتینوپاتی دیابتی، توزیع این پلی مورفیسم ارتباط معنی داری نشان نداد. از آن جا که در پلی مورفیسم 2234694rs، تمام افراد بیمار مورد مطالعه دارای ژنوتیپ AA بودند، بنابراین هیچ گونه آزمون آماری برای بررسی ارتباط بین این پلی مورفیسم و نفروپاتی دیابتی و رتینوپاتی دیابتی قابل انجام نبود.
    نتیجه گیری
    توصیه می شود افرادی که در مراحل اولیه ی دیابت شناسایی می شوند، آزمایش بررسی مولکولی برای بررسی این پلی مورفیسم انجام شود تا افراد مستعد به ابتلا به نفروپاتی دیابتی مشخص شوند و اقدامات درمانی مناسب برای آن ها صورت گیرد.

    مقدمه
    سیلیمارین یک کمپلکس فلاونولیگنان مشتق از گیاه خار مریم (Milk thistle) با نام علمی SilybumMarianum است که به خاطر اثرات حفاظت کبدی و ضد التهابی اش معروف است. در سال های اخیر، توجه محققان به اثرات تعدیل ایمنی (Immunomodulatory) این گیاه معطوف شده است و در این خصوص، مطالعاتی انجام گردیده است. در مطالعه ی حاضر، اثر داروی سیلی مارین بر بیان رسپتور کموکاینی 3CXCR (3Chemokine (C-X-C motif) receptor) لنفوسیت 1Th (1T helper) در مقایسه با دی متیل سولفوکساید (DMSO یا Dimethyl sulfoxide) مورد بررسی قرار گرفت.
    روش ها
    ابتدا از افراد سالم خون گیری انجام شد (8 = n). سپس با استفاده از فایکول سلول های تک هسته ای، خون محیطی (PBMC یا Peripheral blood mononucleated cell) جداسازی گردید و در محیط RPMI (Roswell Park memorial institute medium) و در مجاورت سیلی مارین و DMSO با غلظت مناسب (Mμ 100) قرار گرفت. درصد سلول های T+4CD بیان کننده ی رسپتور کموکاینی در بین جمعیت های لنفوسیت های T توسط رنگ های FITC (Fluoresseinisothiocyanate) و PE (Phycoerythrin) که روی آنتی بادی های منوکلونال ضد انسانی کونژوگه شد، با استفاده از فلوسایتومتری نسبت به شاهد منفی (DMSO) تعیین گردید
    یافته ها
    سیلی مارین در غلظت مورد استفاده (Mμ 100) در مقایسه با DMSO به طور قابل ملاحظه ای باعث افزایش بیان رسپتور 3CXCR گردید (05/0 > P).
    نتیجه گیری
    ارزیابی سیلی مارین بر بیان رسپتورهای کموکاینی، به عنوان یک عامل تعدیل کننده ی ایمنی (Modifier) نشان داد که در شرایطی که سرکوب ایمنی (Immunosuppression) مورد نیاز است، می تواند داروی ارزشمندی باشد.

    Background
    Organic acidurias are a heterogenous group of inherited metabolic disorders characterized by the accumulation and urinary excretion of organic acids. Delay in the detection and treatment of many of these disorders may lead to permanent brain damage or death. Accurate and early diagnosis is therefore paramount. The diagnosis of these disorders is achieved by the analysis of organic acids in the urine using gas chromatography/mass spectrometry (GC/MS). Since genetic and environmental factors and feeding habits can affect the concentration and profile of organic acids, for each population, data should be collected appropriate to that population. Such data did not exist for the Iranian population and thus, needed to be collected.
    Methods
    140 random urine samples were collected from healthy children in 4 age groups: term neonates (< 30 days), premature neonates, infants (1 month-2 years), and children (> 2 years). In addition, urine samples were collected from 10 children with classical symptoms of inherited metabolic disorders. After extraction and preparation of trimethylsilyl derivatives, organic acids were analyzed using GC/MS.
    Findings
    61 organic acids in 140 healthy urine samples were measured and the results were reported as the 2.5th-97.5th percentiles. These compounds were normal components of urine and some marker of diseases. In addition, among 10 samples collected from patients suspected of a metabolic disease, 2 patients with methyl malonic academia, 1 patient with propionic acidaemia and 1 patient with glutaric academia type 2 were identified.
    Conclusion
    Urinary organic acid analysis is not currently performed in Iran and samples are sent abroad for investigation. In addition, no reference ranges for urinary organic acids in healthy Iranian children is available. This article, for the first time, reported the urinary organic acid analysis using GC/MS and reference ranges for the Iranian population. Urinary organic acids can now be analysed and results become available within the same day of patient admission in order to initialize early and targeted treatment. This study shows that metabolic disorders do exist in high proportion of patient with clinical signs and it appears that these diseases are prevalent. It also seems that methyl malonic acidemia is the most common disorder in patient with classic symptoms of organic acidemia in Iran.

  • Evaluation of the Surveillance System for Acute Flaccid Paralysis (AFP) in Iran during the Years 2003-2007 and Its Comparison with Worldwide Standards

    Background
    This study aimed to evaluate the surveillance system for acute flaccid paralysis (AFP) in Iran during the years 2003-2007 and compare it with worldwide standards.
    Methods
    In this cross-sectional study, we analyzed all cases of acute flaccid paralysis reported to health centers in Isfahan province, Iran, from 2003 to 2007. Required information was gathered after reviewing the medical records of patients, and was analyzed using SPSS software.
    Findings
    A total of 115 patients with acute flaccid paralysis were reported, out of which 41% were women and 59% were men. Mean age of subjects was 5.71 ± 3.83, ranging from one to fifteen years. The average incidence of acute flaccid paralysis was 1.74%, which was lower than expected (2). Among the national indicators including case notification (80% within 7 days <), case investigation (80% within 48 hours <), specimen collection (80% within14 days <), specimen transport and testing (80% within 3 days <), 60 days of follow-up (90% <), and classification of cases (100% within 70 days), only the two indicators of case investigation and 60 days of follow-up were less than the expected level compared with global standards.
    Conclusion
    Based on the present study, the surveillance of acute flaccid paralysis cases is working properly, but further evaluation and improving the quality of health care systems is required for the eradication of this disease. Thus, continuing the massive vaccination against polio virus and following up of patients with this disease is recommended.

    Background
    Avian influenza virus of H9N2 subtype has become panzootic during the last two decades in Asia. In addition to causing disease in poultry, this virus has been identified as the reason for several human diseases in Asia. The present study aimed to investigate the phylogenetic relationship between NS gene of two viruses (SS2\2008 and SS8\2011) and other Iranian isolated viruses, and we characterized the growth ability of two viruses in human A549 cell.
    Methods
    Among the H9N2 viruses isolated from 2008 to 2011, two isolated were placed in different positions in the phylogenetic tree. The whole NS gene fragments of two virus isolates were amplified using reverse transcription-polymerase chain reaction (RT-PCR) method; the fragment of each isolate was cloned and then sequenced. Possible changes in the NS gene sequence of isolates were compared with subtypes registered in Gene Bank Database and their growth abilities in human A549 cells were evaluated. GC3s value and replacement rate of Ka/Ks value in NS gene locus were compared.
    Findings
    The initial phylogenetic process revealed that the NS gene of Iranian isolates had a homology of 96.4% belonging to clade Y439 and Allele A. These viruses, based on the time of isolation, were divided into two subgroups of viruses isolated from poultry during the years 1998-2004 and viruses isolated during a time curse of the years 2006-2011. GC3s value varied between 0.371-0.392 and the Ka/Ks value NS gene locus was 0.38 and 0.42 in subgroups 1 and 2, respectively.
    Conclusion
    Processing of NS amino acid sequence not only exhibited that these two isolates contained KSLR sequence in their PDZ domain and but also it revealed that multiple simultaneous changes had been occurred. The viruses titers of two isolates, based on plaque assay, were not statistically significant. These data demonstrate that two Iranian H9N2 isolates are affected by some genetic drifts.

    Background
    Diabetes is the most prevalent non-infectious disorder of endocrine glands with an increasing rate of incidence. Several people with this type of disease suffer from diabetic retinopathy and nephropathy. Oxidative stress is involved in the path physiology of diabetic nephropathy and retinopathy. The superoxide dismutase (SOD) enzymes play a major role in detoxification of reactive oxygen species and have a protective effect against diabetic nephropathy and retinopathy. Several studies claimed that the polymorphisms of this gene have a key-role in tissue damage results from diabetes.
    Methods
    In the present study, two SNPs in SOD1 gene were analyzed in 145 patients with type 2 diabetes mellitus in Ahvaz, Iran. Blood samples of the patients and 145 individuals without diabetes as controls were collected. DNA was extracted and the polymerase chain reaction-restriction fragment length polymorphism method (PCR-RFLP) was performed.
    Findings
    For the rs2070424 polymorphism, there were significant differences between the patients who suffer from nephropathy and the control group (P = 0.005; χ2 = 7.55); but no significant difference in the rate of this gene polymorphism between the patients with retinopathy and the control group was seen. The analysis of rs2234694 polymorphism showed that all individuals with and without diabetes had AA genotype. Therefore, it was impossible to perform any statistical test to determine the relationship of this polymorphism and diabetic nephropathy or retinopathy.
    Conclusion
    Understanding the role of polymorphisms of SOD1 gene to modify the course of retinopathy could be an important target for future pharmacological interventions.

    Background
    Silymarin, apolyphenolic flavnoid derived from milk thistle (Silybum marianum), is known to have anti-inflammatory, hepatoprotective, and anticarcinogenic effects. The aim of this study was to investigate the effect of silymarin on the chemokine receptor of (T helper) Th1 cells.
    Methods
    Peripheral blood mononuclear cells (PBMCs) from 8 healthy individuals were activated with Conconavalin 'A' and treated with silymarin (100 µM) or dimethyl sulfoxide (DMSO), as negative control, in a standard condition (RT: 37 and CO2: 5%). Cells were incubated (72 hours) and then examined for the cytometric evaluation of chemokine receptor CXCR3 expression on Th1 cells. Peripheral blood lymphocyte subpopulations were identified and evaluated by two color flow cytometric analysis. A nonparametric paired samples Wilcoxon test was applied to compare the grouped data. Results were expressed as the mean ± standard deviation (SD). P-values < 0·05 were considered to indicate significant differences.
    Findings
    PBMCs treated with silymarin increased the expression of CXCR3 on Th1 cells. These results were significantly (P = 0.017) in all samples.
    Conclusion
    This study provided evidences of effectiveness for silymarine on the expression of CXCR3. Therefore, in future, silymarine could be used instead of other drugs such as immunosuppressive with fewer side effects in autoimmune diseases.
