صفحه 1مقدمهمقاومت عفونت های باکتریایی به آنتی بیوتیک ها به علت مصرف بیش از حد آنتی بیوتیک ها و مکانیسم های متعدد مقاومت به آن ها توسط باکتری ها رو به افزایش است. باکتری های مقاوم به چند دارو (MDR یا Multi drug resistance) در بخش های مختلف بیمارستان از جمله ICU (Intensive care unit)، باعث بروز عفونت های مهم بیمارستانی شده اند که درمان آن ها با آنتی بیوتیک ها را با مشکل مواجه ساخته است. استفاده از روش های جایگزین مانند استفاده از باکتریوفاژها، یکی از راه های حل این معضل است. باکتریوفاژها یکی از عوامل مهم ضد باکتریایی می باشند که به طور اختصاصی به باکتری های میزبان خود حمله می کنند و باعث انهدام آن ها می شوند. هدف از این مطالعه، جداسازی فاژهای اختصاصی سودوموناس آئروژینوزا و بررسی تاثیر آن ها بر روی سویه های MDR سودوموناس آئروژینوزا بود.روش هادر یک مطالعه ی مقطعی در سال 1392، جدایه های سودوموناس آئروژینوزا از نمونه های کلینیکی مران بستری بخش ICU روش بیوشیمیایی یت شدند. ای و آآن روش بی ا بر طبق CLSI (Clinical and Laboratory Standards Institute) و به عنوان اندیکاتور میزبان جهت جستجوی فاژ از منابع آب استفاده گردید.یافته ها42 سویه ی سودوموناس آئروژینوزا از بخش ICU به دست آمد. الگوی مقاومت آنتی بیوتیکی نشان داد که 88 درصد ایزوله ها به آمیکاسین، 90 درصد به سفی پیم، 85 درصد به سفتازیدیم، 90 درصد به جنتامایسین، 75 درصد به ایمی پنم و مروپنم و همچنین 95 درصد جدایه ها به سیپروفلوکسازین مقاوم بودند. در آزمایش های جداسازی فاژ، تنها از نمونه ی آب فاضلاب بیمارستان باکتریوفاژ لیتیک جداسازی شد و باکتریوفاژ جداسازی شده بر روی سودوموناس آئروژینوزاهای غیر MDR و 3 سویه ی باکتری گرم منفی دیگر تاثیری نداشت.نتیجه گیریبا توجه به مقاومت سودوموناس آئروژینوزا به آنتی بیوبیک های رایج استفاده از روش های درمانی جدید مانند فاژدرمانی افق جدیدی را در درمان عفونت های باکتری که به تمامی آنتی بیوتیک ها مقاومند باز می کند.

صفحه 2مقدمهاستفاده ی گسترده از داروهای گیاهی به دلیل اثرات حفاظتی آن ها بر ارگان های مختلف بدن در مطالعات بسیاری نشان داده شده است. سیلیمارین یک کمپلکس فلاونولیگنان مشتق از گیاه خار مریم (Milk thistle) با نام علمی Silybum marianum است. از این ترکیب، به علت داشتن اثرات ضد التهابی و حفاظتی در بیماری های کبدی (Hepatoprotective) در کلینیک استفاده می شود. در سال های اخیر، اثرات تعدیل ایمنی (Immunomodulatory) این گیاه از سوی محققین مورد توجه قرار گرفته و در این خصوص مطالعاتی صورت پذیرفته است. هدف از این مطالعه، بررسی اثر سیلیمارین و DMSO (Dimethyl sulfoxide) بر تکثیر لنفوسیت های T انسانی در شرایط کشت سلولی بود.روش هاجداسازی سلول های تک هسته ای خون محیطی (PBMCs یا Peripheral blood mononuclear cells) از افراد سالم داوطلب و سپس تحریک آن با کانکاناوالین A (Con A یا Concanavalin A) انجام گرفت و میزان تکثیر سلول ها پس از نشان دار کردن سلول ها با ماده ی فلورسنت CFSE (Carboxyfluorescein succinimidyl ester) بررسی و ارزیابی آن با فلوسیتومتری نسبت به شاهد مورد سنجش قرار گرفت.یافته هاسیلیمارین در غلظت های μM 100 و μM 200 نسبت به DMSO به طور قابل ملاحظه ای تکثیر سلول های تک هسته ای خون محیطی را مهار می کند.نتیجه گیریبا توجه به اثر مهاری سیلیمارین بر تکثیر سلول های T، شاید بتوان از آن به عنوان جایگزین داروهای شیمیایی مهار کننده ی ایمنی و یک داروی حیات بخش در شرایط نیاز به سرکوب ایمنی، استفاده نمود.

صفحه 3مقدمهبا افزایش تعداد باکتری هایی که توالی یابی می شوند، نیاز به بهره برداری از این داده ها نیز بیشتر می شود. استفاده از این اطلاعات ژنومی از طریق به کارگیری همزمان علوم بیوانفورماتیک و باکتری شناسی امکان پذیر می باشد. مطالعه ی بیوانفورماتیکی اندازه ی ژنوم باکتری های بیماری زا، تنوع قابل توجهی را نشان می دهد. فرضیات زیادی در مورد علت این تنوع و همچنین ارتباط اندازه ی ژنوم و فرایندهای اساسی باکتری مطرح است. هدف از انجام این مطالعه بررسی ارتباط اندازه ی ژنوم با تعدادی از خصوصیات ژنتیکی و رفتاری باکتری های بیماری زا بود.روش هااطلاعات توالی ژنوم 100 گونه باکتری بیماری زا از منابع گوناگون جمع آوری شد. ارتباط اندازه ی ژنوم با چند خصوصیت باکتری های بیماری زا مورد ارزیابی قرار گرفت. برای آنالیز داده ها، از نرم افزاری SPSS نسخه ی 19 استفاده شد.یافته هااندازه ی ژنوم در باکتری های بیماری زا، ارتباط معنی داری با تعدادی از خصوصیات ژنومی مانند درصد C + G، تعداد کلی ژن ها، پروتئین ها، سودوژن ها (Pseudogenes)، ژن های کسب شده و برخی از خصوصیات رفتاری و اکولوژیکی باکتری مانند تحرک، محدوده ی میزبان و تنفس نشان می دهد. هیچ گونه ارتباط معنی داری بین اندازه ی ژنوم، تراکم ژن در طول ژنوم و همچنین درصد DNA غیر کد کننده وجود ندارد.نتیجه گیریدر باکتری های بیماری زا، DNA انعطاف پذیری بالایی دارد و در طی تکامل، دچار کاهش اندازه می شود. در طی این روند، باکتری ها تعدادی از ژن های خود را از دست می دهند و تعداد کمتری نیز ژن کسب می کنند. این عوامل با کاهش تعداد پروتئین ها، کاهش درصد C + G و محدود ماندن زیستگاه باکتری همراه است.

صفحه 4متفورمین به عنوان یک دارو از دسته ی بی گوانیدها حدود 50 سال پیش وارد بازار دارویی شد و هم اکنون به عنوان یک داروی خط اول در درمان دیابت نوع 2، به خصوص در افراد با وزن بالا مطرح است. در سال های اخیر، برای این دارو کاربردهای جدیدی نیز مطرح شده است. از جمله این که به این دارو، به عنوان محافظ کلیه توجه زیادی می شود. مطالعات اخیر، مشخص کرده اند که متفورمین خواص آنتی اکسیدانی خوبی دارد. کاهش آپوپتوز ناشی از اکسیداتیو استرس در سلول های اندوتلیال و مهار اختلالات عروقی نیز با مصرف متفورمین گزارش شده است.

Page 1BackgroundBacterial drug resistance due to overuse of antibiotics and several mechanisms of resistance by bacteria is increasing. Multi-drug resistant bacteria (MDR) in different parts of the hospital including the ICU, cause nosocomial infections and resistance to antibiotics made them difficult to treat. Alternative methods of treatment are one way to solve this problem. Bacteriophages are antibacterial agents that specifically attack their host. In this study, Pseudomonas specific lytic phages isolated from the clinical environment and its effect on MDR P. aeruginosa was investigated.MethodsIn a cross sectional study in 2013, isolates of Pseudomonas aeruginosa from clinical specimens of patients admitted to the ICU were identified by biochemical methods then antibiotic resistance patterns was determined by standard disk diffusion method according to CLSI and used as indicator hosts to screen phages from water samples.Findings42 strains of Pseudomonas aeruginosa have been isolated from the ICU.The antibiotic resistant patterns of bacterial isolates were as follows: 88%to amikacin, 90% to cefepime, 85% to ceftazidime, 90% to gentamicin, 75%to imipenem and meropenem and 95% to ciprofloxacin. Lytic bacteriophage was isolated only from hospital wastewater. The isolated bacteriophage had no effect on non-MDR P. aeruginosa and other Gram-negative bacteria.ConclusionBacterial resistance to antibiotics is increasing. New alternative methods such as phage therapy would open new insights in treatment of multidrug resisitant bacterial infections.Keywords: Pseudomonas aeruginosa, Bacteriophage, Phage therapy, MDR

Page 2BackgroundThe widespread use of herbal medicines because of their protective effects on various organs has been shown in many studies. Silymarin is a flavonolignan complex isolated from the milk thistle (Silybum marianum L. Gaertn), has been clinically used for its anti-inflammatory and hepatoprotective effects. Recently, immunomodulating effects of this plant have considered by the researchers. This study aimed to investigated the effect of silymarin and DMSO on the human T lymphocyte proliferation under the cell culture conditions.MethodsPeripheral blood mononuclear cells (PBMCs) from the healthy individuals were activated with Con A (10 μg/ml) and treated with silymarin at different concentrations (0.001, 0.01, 0.1, 1, 10, 100 and 200 μM). Then, cells were incubated for five days (at 37◦ C with 5% CO2) for proliferation assay using the CFSE dye and for viability analysis using flow cytometry. We analyzed the data using the Graph pad prism (version 6).FindingsThis study showed that silymarin has the ability to inhibit the T cell proliferation in vitro. Moreover, 100 μM and 200 μM of silymarin (P < 0.001) had more inhibitory effect on T cells.ConclusionWe conclude that Silymarin exerts immunosuppression effects, and it could be used in therapeutic situations with fewer side effects compared with the other immunosuppressive drugs.Keywords: Silymarin, Carboxyfluorescein, succinimidyl ester, T cells, Peripheral blood mononuclear cells, Dimethylsulfoxide

Page 3BackgroundIncreasing number of bacteria which their genome sequenced leads to wide spread utilization of this data. Use of genomic information is possible through the simultaneous application of bioinformatics and bacteriology. Bioinformatics study of pathogenic bacteria indicates significant variation in genome size. There are several hypotheses about the reason of this variety as well as the relationship between fundamental processes of bacteria and genome size. The aim of this study was to investigate the relationship between genetic characteristics and behavior of pathogenic bacteria and genome size.MethodsThe genome sequence data of 100 species of pathogenic bacteria were collected from different sources. The association of genome size with some characteristics of pathogenic bacteria was evaluated. Data were analyzed using SPSS statistical software, version 19.0.FindingsA significant relation was observed between the genome sizes of pathogenic bacteria with regard to some genomic features including G+C % content, the total number of genes, proteins, pseudogenes, acquired genes and some behavioral and ecological characteristics of bacteria, such as mobility, host range and breathing. Gene distribution and percent of non-protein-coding DNA have no correlation with genome size.ConclusionThere is a high DNA flexibility in pathogenic bacteria, and the size of the DNA is decreased during the evolution. This may lead to loss of some genes and less often acquire genes. These factors cause a reduction in the number of proteins and G + C percentage of bacteria which may account for their habitat limitation.Keywords: Genome size, Bioinformatics, Base sequence

Page 4Metformin as a biguanid drug entered to the market 50 years ago and now is generally recommended as the first-line treatment of type 2 diabetes, especially in overweight patients, however in recent years new indications for its use have found. Recently, much attention has been directed toward the possible renal protective efficiency of metformin. Recent studies have proven that metformin, possesses antioxidant efficacy as well. Page 5Pulmonary arterial hypertension (PHA) is a frequent complication of congenital heart disease (CHD), most commonly in systemic-to-pulmonary shunt lesions. In patients with an uncorrected left-to right shunt, PAH will end up to Eisenmenger syndrome,that is contraindication of surgery. What about patients with Moderate to severe PAH who do not have criteria of Eisenmenger syndrome. Immediate cardiac repair is preferred or medical treatment? Heart catheterization and evaluation of pulmonary artery reactivity test or lung biopsy might be helpful to find presence of Eisenmenger syndrome. Although these guidelines are not always helpful or possible. Look at the following article will show how important this subject is. 192 patient with CHD and PAH were evaluated in 4 groups, Eisenmenger syndrome (90 patients), left- to- right shunt with PAH (48 patients), PAH with small defect (10 patients) and PAH after defect correction (44 patients). One, 5, 10, and 20 year survival was estimated for each group. Findings showed that patients with history of surgery in presence of PAH have worst outcome. And patients with Eisenmenger syndrome and those with PAH- associated with left- to- right shunt have best prognosis, one of the reasons of this result is presence of a cardiac defect which allows a pulmonary- to- systemic shunt which can maintain the cardiac output in the end- stage of disease. Finally I present a young girl with history of CHD with PAH. She had been operated several years ago. When I admitted her, she was in a bad condition, with Functional class IV. Echo study showed severe PAH. Treatment of PAH with specific target PAH drugs started immediately, but these treatment were too late and patient expired in the hospital. Finally, I would like to pose the question”Wouldn’t it be better if the patient had not undergone the surgery in the first place”.Keywords: Congenital heart disease, Pulmonary hypertension, Treatment
