  • مقدمه

    شواهد علمی نشان می دهد که عدم تعادل بین سایتوکین های التهابی و ضد التهابی نقش مهمی در ایجاد بیماری پره اکلامپسی ایفا می کند.

    هدف

    در مطالعه حاضر ارتباط سطح سرمی اینترلوکین-23 (IL-23) مادر، یک سایتوکین پیش التهابی، با خطر بروز بیماری پره اکلامپسی و شدت آن بررسی شد.

    مواد و روش ها

    یک مطالعه مورد-شاهدی شامل 145 زن با 75 مورد بیمار پره اکلامپسی، 35 زن باردار سالم و 35 فرد کنترل غیر باردار سالم از زاهدان، جنوب شرق ایران انجام شد. سطوح مادری IL-23 در گردش خون با استفاده از روش الایزا تعیین شد.

    نتایج

    سطح سرمی IL-23 مادر در گروه پره اکلامپسی و دو زیر گروه آن، پره اکلامپسی شدید و خفیف افزایش یافت، به طوری که این افزایش در گروه های پره اکلامپسی و پره اکلامپسی شدید معنی‌دار، اما در پره اکلامپسی خفیف در مقایسه با گروه کنترل معنی‌دار نبود (به ترتیب 001/0 > p، 001/0 >p و 08/0 =p). علاوه بر این، سطح سرمی IL-23 مادر در پره اکلامپسی با شروع زودرس از نظر آماری معنی‌دار بود، اما در گروه پره اکلامپسی دیررس در مقایسه با گروه کنترل باردار سالم معنی‌دار نبود (به ترتیب 001/0p < و 46/0 = p).

    نتیجه گیری

    نتایج مطالعه ما ارتباط مثبتی بین سطح IL-23 و بیماری پره اکلامپسی به ویژه در پره اکلامپسی نوع شدید و زودرس نشان داد که نشان می دهد IL-23 ممکن است در پاتوژنز این سندرم سیستمیک نقش داشته باشد.

    Background

    Scientific evidence support that imbalance between inflammatory and anti-inflammatory cytokines play a critical role in preeclampsia (PE).

    Objective

    To investigate the relationship between the maternal serum level of interleukin (IL)-23, a pro-inflammatory cytokine, PE and its severity risk was investigated.

    Materials and Methods

    The case-control study included a total of 145 women counting 75 PE cases, 35 healthy pregnant and 35 healthy non-pregnant controls from Zahedan, southeast of Iran. The maternal levels of IL-23 in circulation were determined via enzyme-linked immunosorbent assay.

    Results

    The maternal serum levels of IL-23 were increased in PE and its 2 subgroups severe PE and mild PE, so that these increases were significant in PE and severe PE, but not in mild PE compared with the controls (p < 0.001 and p < 0.001, p = 0.08, respectively). Besides, the maternal IL-23 serum level was statically significant in the early onset PE, but not in the late onset-PE group compared to healthy pregnant controls (p < 0.001, p = 0.46 respectively).

    Conclusion

    The results of our study showed a positive association between IL-23 level and PE, especially in severe type and early onset PE, which suggests that IL-23 may be involved in the pathogenesis of this systemic syndrome.

    Purpose

    To explore the ccRCC clinical and immune characteristics correlated with IL-23 expression level and build pre-operative prediction models based on contrast CT scans.

    Materials and Methods

    The study included the cancer genome atlas kidney renal clear cell carcinoma cases to build a bioinformatics cohort. The cases with qualified contrast CT images were selected as radiographic and ra-diomics cohort. The IL-23 expression level groups were defined by median-based thresholding. The clinical char-acteristics were compared between groups. The impacts of IL-23 on immune microenvironment composition were measured via the CIBERSORT. Two radiologists evaluated the pre-operative contrast CT images. The radiomics features were automatically extracted. IL-23 group-specific radiographic and radiomics features were collected and used for prediction model establishment via Orange Data Mining Toolbox. P < 0.05 was set as statistically significant.

    Results

    For total, 530 ccRCC cases were included. The IL-23 group was significantly associated with survival, histologic grade, AJCC tumor stage, AJCC cancer stage, and plasma calcium level. Except for Treg and other T cells, IL-23 showed correlation with NK cell, mast cell, monocyte infiltration. Axial length was the only significant radiographic measurement between IL-23 groups. The radiomics features established an IL-23 group prediction model with the highest 10-fold cross-verification AUC of 0.842.

    Conclusion

    The clear cell renal cell carcinoma IL-23 expression level had prognosis and immune microenviron-ment correlation and could be predicted by pre-operative radiomics features.

    Background

    The possible risk factor of Toxoplasma infection and its possible correlation with Interleukin-12 (IL-12) and Interleukin-23 (IL-23) in breast cancer patients was investigated.

    Methods

    Overall, 190 female patients referred to the Oncology Teaching Hospital in the Medical City Hospital, Baghdad, Iraq were enrolled from 2017-2018. All serum samples were tested for T. gondii immunoglobulins (IgG and IgM (antibodies and IL-12, IL-23 levels

    Results

    In patients with breast cancer, the results revealed a high positivity percentage for anti- Toxoplasma IgG. In breast cancer patients infected with T. gondii, the IL-12 level was lower than the controls while the mean level of IL-23 was higher than the controls. According to the cancer grade in breast cancer patients infected with T. gondii, the higher mean titer of IgG and IL-23 was in grade 3 in contrast the highest mean titer of IL-12 was in grade 1. Concerning the tumor stages in breast cancer patients infected with T. gondii, the higher mean titer of IgG, IL-12 and IL-23 was in stage (III). According to the tumor size in breast cancer patients infected with T. gondii, the highest mean titer of IgG, IL-12 and IL-23 was in size >3cm.

    Conclusion

    The levels of IL-23 could be a candidate as a non-invasive primitive marker for earlier prediction of breast cancer stage.

    Introduction

    The use of a biologic agent, ustekinumab, for psoriasis has lead to an observation of tumor growth retardation in a patient with unresectable pancreatic cancer. High circulating levels of interleukin-23 have been found in certain cancers.

    Case Presentation

    This case report of an 86 year old gentleman with psoriasis and pancreatic cancer highlights a pre-liminary role of interleukin 23 blockade in altering the tumor microenvironment of certain cancers to influence their growth and metastases.

    Conclusions

    29 months since his diagnosis this patients remains well with no metastases, no secondary symptoms from an obstructive malignancy, and has demonstrated no significant tumor progression despite no curative therapy. Ustekinumab has been the only immunotherapy he has received since his diagnosis and his existing medications remained unchanged. Preliminary evidence for interleukin 23 inhibition and tumor retardation is present however larger studies investigating in vivo levels of interleukin 23 in cancer patients and subsequent blockade is necessary to substantiate a possible new target in pancreatic cancer.

    Background
    Several cytokines contribute to cutaneous leishmaniasis outcomes. IL-23 is a key cytokine in inflammation and a central cytokine in controlling Th17. IL-8, also known as a neutrophil chemotactic factor in the lesion site of cutaneous leishmaniasis, indicates disease promotion.
    Objectives
    This study aimed to evaluate the relationship of Leishmania major with IL-8 and IL-23 expression.
    Methods
    This case-control study was performed on 25 healthy individuals and 25 cutaneous leishmaniasis patients who lived in Southwestern Iran, in 2017. Peripheral blood mononuclear cells (PBMCs) were first isolated. Total RNA was extracted and reverse-transcribed into cDNA. The expression levels of IL-8 and IL-23 were measured by the real-time PCR. The data were statistically analyzed using SPSS version 16.
    Results
    Our findings indicated that the expression of IL-8 was significantly higher in the leishmaniasis group than in the control group with the median (IQR) of 0.39 (0.92) and 0.03 (0.11), respectively (P = 0.02). The expression of IL-23 was higher in the leishmaniasis group than in the control group with the median (IQR) of 0.13 (0.43) and 0.11 (0.61), respectively (P = 0.48). The increased IL-8 expression accompanied the location sites in face and hand and the increased number of skin lesions.
    Conclusions
    The expression of IL-8 in patients with cutaneous leishmaniasis is an index of increased activity of local neutrophils that can contribute to leishmaniasis survival or inflammation increase. However, IL-23 appears to be less important in the inflammatory reaction than IL-8.

    Cytokines have prominent roles in activating of different T cells and shifting the immune response, in this study the role of three cytokines (IL-21, IL-23 and IL-27) is investigated in the liver transplant rejection. Three EDTA-treated blood samples were collected from each liver transplanted patient in 1st, 4th and 7th day of post-transplantation. The expression level of the mentioned cytokines was determined using real-time PCR for all samples. Also, the serum levels of cytokines were determined using ELISA tests. In acute rejection (AR) group (51 patients), mRNA expression pattern of IL-21and IL-23 showed a steady increase, but this pattern was converse for IL-27. Our results in non-acute rejection (non-AR) group (54 patients) showed an elevation in day 4 and then a decrease in day 7 for IL-21 and IL-23 genes. This pattern was converse again for IL-27 gene. In comparison between the two groups, in all 3 sampling times the mean of mRNA expression level of IL-21 and IL-23, showed an increase in AR group which this increase was significant for IL-21 in the 3rd (p=0.007) and for IL-23 in 2nd (p=0.048) and 3rd (p=0.049) sampling time, but the pattern of mRNA expression for IL-27 was contrary to the results of IL-21 and IL-23. Furthermore, ELISA technique also, showed the serum level changes the same as cytokines. In this study IL-21 and IL-23 showed pro-inflammatory properties in the liver transplant rejected patients. Also, IL-27 having different expression pattern, showed anti-inflammatory behavior which needs more considerations in future.

    Serous otitis media with effusion (OME) is a middle ear inflammatory response to allergens and microbes which stimulate leukocytes to produce different inflammatory mediators after obstruction of Eustachian tube. Here, we investigated the levels of these mediators, IL-17 and IL-23, in serum and middle ear fluids of children with OME. 75 patients with otitis media and 75 age and sex-matched healthy controls were enrolled in this study. IL-17 and IL-23 levels in serous secretion of the patients and their serum levels were measured in both groups by ELISA. Serum IL-17 levels were significantly higher in the patients than controls (p=0.001). There was no significant difference between serum IL-23 levels in patients and controls. Patients’ serous levels of both cytokines of IL-17 and IL-23 were higher than those in serum according to different parameters of sex, age, and duration of the disease. This study shows an elevated presence of IL-17 and IL-23, as pro inflammatory cytokines, in OME. These finding may represent the contribution of such cytokines in the pathogenesis of OME. Blocking such molecules may yield new non-surgical therapeutics.

    Background
    Asthma is the chronic inflammation of airways characterized by eosinophilic infiltration, mucus overproduction, airway hyper-responsiveness and airway remodeling. These changes are induced mostly by cytokines which are produced by T helper (Th) 2 cells. Recently, the role of interleukin-23 (IL-23) in the pathogenesis of adult allergic asthma has been studied.
    Objective
    To explore IL-23 serum levels and its expression in persistent asthma compared with healthy children younger than five years old.
    Method
    Blood samples of 40 children with mild and severe persistent asthma were compared to 34 healthy children regarding IL-23 serum levels and gene expression using enzyme-linked immunosorbentassay (ELISA) and real time quantitative polymerase chain reaction (PCR).
    Results
    The IL-23 gene expression level was significantly different in the 25 children with mild persistent asthma and the 15 children with severe persistent asthma compared to the control group (p=0.001). There was no significant difference in IL-23 gene expression level between the two groups of patients with mild and severe persistent asthma. A significant difference was seen in IL23 serum levels between the 25 children with persistent asthma and control group (p=0.002).
    Conclusion
    For pre-school children with history and physical exam in favor of asthma which cannot be tested by spirometry, IL-23 serum levels may be an auxiliary biomarker for the diagnosis of asthma.

    زمینه و هدف
    برهمکنش گلیکوپروتئین Bسیتومگالوویروس با پذیرنده های شبه تول بر سلول های دندریتیک منجر به پیام دهی اولیه و پاسخ ایمنی ذاتی می شود. هدف از این تحقیق، مطالعه اثرات گلیکوپروتئین B سیتومگالوویروس بر بلوغ و عملکرد سلول های دندریتیک گروه تیمار با این گلیکوپروتئین ویروسی در مقایسه با گروه کنترل بود.
    مواد و روش ها
    نمونه خون محیطی از 5 فرد داوطلب سالم جمع آوری شد. پس از تولید سلول های دندریتیک مشتق شده از منوسیت ها در روز پنجم کشت، نیمی از سلول های دندریتیک نابالغ با گلیکوپروتئین B سیتومگالوویروس تیمار شدند و مابقی سلول ها بدون تیمار به سلول های دندریتیک بالغ القاء و به عنوان کنترل در نظر گرفته شدند و بلوغ و عملکرد سلول های دندریتیک در این دو گروه با هم مقایسه گردید.
    نتایج
    در گروه تیمار با گلیکوپروتئین B میزان بیان ژن پذیرنده شبه تول 4 در سلول های دندریتیک به طور معنی داری افزایش یافت (p < 0.05). در حالی که اختلاف معنی داری در درصد بیان شاخص های CD83، CD86، CD1a و HLA-DR و غلظت سایتوکاین IL-23 مترشحه از سلول های دندریتیک در دو گروه تیمار و کنترل مشاهده نگردید.
    نتیجه گیری
    افزایش بیان ژن پذیرنده شبه تول 4 در سلول های دندریتیک تیمار با گلیکوپروتئین B نشان داد که تنها تماس سلولی جهت روشن ساختن پاسخ سلولی ضد ویروسی و فعالیت پذیرنده شبه تول ضروری است. بنابراین مطالعات بیشتر در مورد شاخص های سلولی موثر فوق در ساخت راه کارهای درمانی علیه سیتومگالوویروس می تواند نقش مهمی داشته باشد.

    Background and Objectives
    Interaction of cytomegalovirus glycoprotein B with toll-like receptors of dendritic cells leads to early signaling and innate immune responses. The aim of this study is to evaluate the effects of cytomegalovirus glycoprotein B on the maturation and function of monocyte-derived dendritic cells in treated groups in comparison with control groups.
    Materials and Methods
    Blood samples were taken from 5 healthy volunteers. Following the generation of monocyte-derived dendritic cells on the fifth day of cell culture, half of the immature dendritic cells were treated with cytomegalovirus glycoprotein B, and the rest of them were induced to mature dendritic untreated cells and were used as the control group. The maturation and function of dendritic cells were evaluated in these two groups.
    Results
    The gene expression level of toll-like receptor-4 significantly increased in the group treated with glycoprotein B (p < 0.05), whereas there were no significant differences in the expression rates of CD83, CD86, CD1a, and HLA-DR and the secretion of IL-23 from monocyte-derived dendritic cells between the treated groups and the controls.
    Conclusion
    The increase in the gene expression of toll-like receptor-4 in monocyte-derived dendritic cells treated with cytomegalovirus glycoprotein B showed that cell contact is required to elicit cellular antiviral response and toll-like receptor activation. Thus, it is critical to recognize the viral and cellular determinants of the immune system in order to develop new therapeutic strategies against cytomegalovirus.

    Methods
    In this case-control study, we assessed IL-23 and IL-27 levels in serum of 37 healthy individuals and 64 patients with chronic HCV using Enzyme-linked immunosorbent assay (ELISA). The relationship of cytokines level with liver enzymes (ALT, AST, and ALP), HCV genotype and viral load were analyzed. The differences of these cytokine levels in the groups of treatment and no treatment was compared. HCV genotypes were classified by HCV-specific primers methods. HCV RNA loads were determined by fluorescence quantitative PCR..
    Results
    Serum level of IL-23 was higher in HCV infected patients compared to control group (P = 0.005). However, no significant difference was seen in IL-27 serum level between patients compared to the control group (P = 0.65). There was no significant difference in IL-23 and IL-27 level between genotype 1 HCV-infected- and 3a HCV-infected- patients. Positive moderate correlation between IL-23 and IL-27 with viral load was found in type 3a and 1 HCV-infected patient. Positive relative correlation was seen between ALT and IL-23 in 1a HCV-infected patients, which was higher than 3a HCV-infected patients; but there were no significant difference between serums liver enzymes with IL-23 and IL-27 in respect to genotype 3a and 1a HCV-infected patients.
    Conclusions
    These findings may reflect a vigorous pro-inflammatory reaction orchestrated by the host immune system against chronic HCV. Also, a better understanding of the involvement mechanism considering the correlation between other genotypes with inflammatory cytokines in various stages of disease can be obtained..
