صفحه 441مقدمهعوارض دیابت حاملگی از جنبه های مختلف، بر سلامت مادر و جنین موثر است. برخورداری از دانش و مهارت های مراقبت از خود، به ویژه در زمینه ی تغذیه و ورزش، نقش مهمی در کنترل بیماری دارد. پژوهش حاضر با هدف تعیین تاثیر آموزش تغذیه و ورزش، بر سطح دانش و شاخص های کنترل متابولیک بیماران مبتلا به دیابت حاملگی مراجعه کننده به بیمارستان های دانشگاه علوم پزشکی شیراز طراحی و اجرا گردید.مواد و روش هادر این پژوهش مداخله ای، 61 بیمار دیابت حاملگی کلاسA1 به روش نمونه گیری آسان انتخاب، و به شیوه ی تصادفی در دو گروه آزمایش و شاهد قرار گرفتند. قبل از مداخله، شاخص های کنترل متابولیک به روش آزمایشگاهی و سطح دانش، به وسیله ی آزمون کتبی 34 پرسش چند گزینه ای خود ایفا که روایی آن مورد تایید، و پایایی آن (KR 20 = 0/81) بود در دو گروه اندازه گیری شد، سپس مداخله ی مدون آموزشی به مدت حدود 6 ساعت طی یک هفته در گروه آزمایش اجرا گردید. پیامدهای مورد نظر، بلافاصله و دو هفته پس از پایان مداخله در دو گروه اندازه گیری، و با استفاده از نرم افزارSPSS نسخه ی 5/11 و آزمون های آماری تی مستقل، بنفرونی و مجذور خی پردازش و تحلیل شدند.یافته هاپژوهش حاضر نشان داد پس از مداخله ی آموزشی، در گروه آزمایش، میانگین های سطح دانش، قند خون یک و دو ساعت پس از غذا خوردن به طور معنی داری تغییر نمود (001/0>P).نتیجه گیرییافته های پژوهش حاضر اثربخشی مداخله ی آموزشی بر ارتقای سطح دانش و شاخص های کنترل متابولیک حمایت، و بر تدارک و اجرای آموزش برنامه ریزی شده ی تعاملی تاکید نمودند.

صفحه 449مقدمهبهبود الگوی لیپیدی و فشار خون در پیشگیری از عوارض بعدی دیابت دارای اهمیت است. با توجه به تمایل مردم برای مصرف داروهای گیاهی، پژوهش حاضر با هدف تعیین اثر عصارهی هیدروالکلی گزنه بر الگوی لیپیدی و فشار خون در بیماران مبتلا به دیابت نوع 2 انجام شد.مواد و روش هاپژوهش حاضر به صورت کار آزمایی بالینی تصادفی یک سو کور، بر 50 بیمار مبتلا به دیابت نوع 2 که به صورت تصادفی در دو گروه مداخله (25 نفر) و شاهد (25 نفر) تقسیم شدند، صورت گرفت. گروه مداخله، روزانه 100 میلی گرم به ازای کیلوگرم وزن بدن، عصارهی هیدروالکلی گزنه و گروه شاهد، روزانه 100 میلی گرم به ازای کیلوگرم وزن بدن، دارونما را به مدت 8 هفته مصرف نمودند. الگوی لیپیدی، فشار خون سیستولی (SBP) و دیاستولی (DBP) در ابتدا و پایان مداخله اندازه گیری شد.یافته هاتری گلیسرید (TG)، SBP و شاخص های آتروژنیک پلاسما (نسبت LDL-C/HDL-C و TC/HDL-C) در پایان هفتهی هشتم در گروه مداخله نسبت به گروه شاهد، کاهش معنی داری را نشان دادند، در تمام موارد (05/0P<). لیپوپروتئین با چگالی بالا (HDL-C) افزایش معنی داری را در گروه مداخله نشان داد (05/0P<). کلسترول تام (TC)، لیپوپروتئین با چگالی پائین (LDL-C)، در پایان هفته ی هشتم در گروه مداخله نسبت به گروه شاهد تغییر معنی داری را نشان ندادند (05/0P>).نتیجه گیریعصارهی هیدروالکلی گزنه، اثر مثبتی را در وضعیت لیپیدی و فشار خون در مدت 8 هفته مداخله در بیماران مبتلا به دیابت نوع 2 نشان داد، بنابراین عصارهی هیدروالکلی گزنه را می توان به عنوان درمان کمکی در بیماران مبتلا به دیابت نوع 2 توصیه نمود.

دیابت کنترل نشده ی طول کشیده، می تواند روی حرکت و عملکرد دستگاه گوارش تاثیر بگذارد و تمام دستگاه گوارش از مری تا ناحیه آنورکتال را درگیر کند. هدف از پژوهش حاضر، تعیین فراوانی علایم گوارشی در بیماران دیابتی نوع 2 و همراهی آن با سن، جنس، میزان کنترل قند خون و طول مدت ابتلا به دیابت است.
مواد و روش هاپژوهش حاضر به روش مقطعی روی 350 بیمار دیابتی نوع 2 مراجعه کننده به کلینیک مرکز تحقیقات دیابت اهواز با روش نمونه گیری تصادفی ساده در سال 1389 انجام شد. داده ها توسط پرسش نامه جمع آوری و توسط آزمون های تی مستقل و مجذور خی در نرم افزار SPSS، نسخه ی 18 تحلیل شدند.
یافته هامیانگین سن بیماران 11±56 سال بود. فراوانی علایم گوارشی معادل 67% محاسبه گردید. شایع ترین علایم گوارشی به ترتیب یبوست (43%)، نفخ شکم (35%) و رگورژیتاسیون (31%) بود. فراوانی کلی علایم گوارشی با طول مدت ابتلا به دیابت همراهی داشت (001/0>P). افزایش سطح گلوکز ناشتا در پلاسما، فراوانی علایم گوارشی را افزایش داد (003/0=P). فراوانی علایم گوارشی در زنان نسبت به مردان بالاتر بود (001/0P<). با افزایش سن بیماران، فراوانی علایم گوارشی بیشتر گردید (003/0=P). فراوانی علایم گوارشی با سطح هموگلوبین A1C و BMI همراهی نداشت.
نتیجه گیریفراوانی علایم گوارشی در بیماران دیابتی نوع 2، 67% است و شایع ترین علایم گوارشی به ترتیب یبوست، نفخ شکم و رگورژیتاسیون می باشند. فراوانی علایم گوارشی با میزان کنترل حاد قند خون (FBS)، طول مدت ابتلا به دیابت، جنس زن و سن بیمار همراهی دارد ولی با سطح هموگلوبین A1C و BMI همراهی ندارد.

صفحه 467مقدمهخودمدیریتی دیابت جنبه ی مهم درمان دیابت است و عوامل روان شناختی، اجتماعی و ساختاری، اثرات مهمی بر خودمدیریتی دیابت در بیماران مبتلا به دیابت نوع 2 دارند. هدف پژوهش حاضر، بررسی تعیین کننده های روان شناختی، اجتماعی و ساختاری خودمدیریتی دیابت بود.مواد و روش هادر یک زمینه یابی مقطعی، تعداد 500 نفر (245 مرد و 255 زن) بیمار سرپایی مبتلا به دیابت نوع 2 به شیوه ی نمونه گیری در دسترس انتخاب شدند. داده ها با استفاده از پرسش نامه ی داده های جمعیت شناختی و مقیاس های خودمراقبتی دیابت، خودکارآمدی دیابت، باور به اثربخشی درمان، باور به قطعیت بیماری، انگیزش درمان، ارتباط درمانی مراقبان- بیمار، دانش مرتبط با دیابت، رضایت بیمار از خدمات درمانی، دسترسی به خدمات درمانی، حمایت اجتماعی از جانب اعضای خانواده، تصمیم گیری مشارکتی جمع آوری و به کمک همبستگی و تحلیل رگرسیون چندگانه به روش گام به گام مورد تجزیه و تحلیل قرار گرفتند.یافته هاخودمدیریتی دیابت دارای میانگین (انحراف استاندارد)، 77/45 (81/21) بود. خودکارآمدی، باور به اثربخشی درمان، حمایت اجتماعی، 7% HbA1c> و درمان با انسولین، قادر بودند تغییرات مربوط به خودمدیریتی دیابت را پیش بینی کنند (001/0P<). در کل تمام متغیرها 91/0% خودمدیریتی دیابت را تبیین نمودند (91/0R2=). سهم خودکارآمدی در پیش بینی خودمدیریتی دیابت بالاترین میزان (338/0β= و 001/0P<) بود.نتیجه گیریخودکارآمدی، باور به اثربخشی درمان، حمایت اجتماعی، شدت دیابت و نوع درمان در انجام رفتارهای خودمدیریتی مهم هستند و می توانند میزان بالایی از تغییرپذیری خودمدیریتی دیابت را تبیین نمایند، بنابراین توجه به این متغیرها به منظور بهبود رفتارهای خودمراقبتی امری مهم می باشد.

صفحه 479مقدمهسندرم متابولیک با افزایش عوامل خطرساز بیماری های عروق کرونر مانند افزایش فشار خون، قند خون، دیس لیپیدمی و چاقی مشخص می شود. پژوهش حاضر، با هدف تعیین برخی عوامل خطرساز سندرم متابولیک و درصد چربی بدن، و ارتباط آن با بیشینه ی اکسیژن مصرفی در مبتلایان به بیماری های عروق کرونر صورت گرفت.مواد و روش هاپژوهش حاضر از نوع توصیفی همبستگی بود و 90 بیمار عروق کرونری 40 تا 80 سال به روش مبتنی بر هدف در دسترس، در این پژوهش شرکت داشتند. بیشینه ی اکسیژن مصرفی از فرمول غیرتمرینی اکسیژن مصرفی محاسبه گردید. آزمایش خون ناشتا برای تعیین غلظت تری گلیسیرید، کلسترول HDL و قند خون انجام شد. درصد چربی بدن با اندازه گیری ضخامت چربی زیر پوستی چهار نقطه از بدن محاسبه و نیز فشار خون بیماران در شرایط مناسب و نشسته اندازه گیری گردید.یافته هامیانگین عوامل سندرم متابولیک و درصد چربی بدن در بیماران بالا و این میزان در زنان بیش از مردان بود. شایع ترین آن ها کاهش کلسترول HDL و افزایش فشار خون بود. هم چنین، یافته ها نشان دهنده ی وجود ارتباط معکوس و معنی داری بین بیشینه ی اکسیژن مصرفی با درصد چربی بدن، قند خون ناشتا و فشار خون سیستولی بود.نتیجه گیرییافته های به دست آمده از این پژوهش، حاکی از افزایش عوامل خطرساز سندرم متابولیک و درصد چربی بدن در افراد مبتلا به بیماری های عروق کرونر بود. هم چنین یافته های این پژوهش نشان داد با افزایش درصد چربی بدن، فشارخون سیستولی و قند خون ناشتا که نتیجه ی سبک زندگی ناسالم است، بیشینه ی اکسیژن مصرفی کاهش می یابد.

صفحه 486مقدمهفعالیت ورزشی در جلوگیری از پیشرفت گرفتگی شریان ها موثر است، و کلسترول LDL- اکسید شده (ox-LDL-c)، عمل آتروژنیکی مونوسیت ها را تسهیل می نماید. اثر آموزش اصلاح فعالیت های بدنی در زندگی (LAM)، براساس دستورالعمل مراکز کنترل بیماری (CDC) و کالج آمریکایی طب ورزش (ACSM)، بر ox-LDL-c سرم در افراد چاق به خوبی مشخص نیست. پژوهش حاضر به بررسی اثر 12 هفته LAM بر ox-LDL-c سرم مردان چاق میان سال پرداخته است.مواد و روش ها16 مرد میان سال سالم چاق (میانگین و انحراف معیار سن 01/6±06/42 سال) به دو گروه شاهد (8=تعداد) و LAM (8=تعداد) تقسیم شدند. افراد گروه LAM براساس دستورالعمل CDC و ACSM، 4 روز در هفته و روزانه 2 مایل را در مدت طی 30 دقیقه روی نوارگردان، برای 12 هفته پیاده روی نمودند.یافته هایافته ها نشان داد وزن بدن، نمایه ی توده ی بدن، توده ی چربی، درصد چربی، نسبت دور کمر به لگن و مقدار کلسترول LDL- پس از 12 هفته تمرین LAM نسبت به گروه شاهد کاهش معنی داری داشت (05/0>P). هم چنین، بیشینه ی اکسیژن مصرفی پس از 12 هفته تمرین LAM نسبت به گروه شاهد افزایش معنی داری را نشان داد (05/0>P). از سوی دیگر، پس از 12 هفته تمرین LAM اختلاف معنی داری در سطح کلسترول ox-LDL-، کلسترول تام (TC)، تری گلیسرید (TG) و کلسترول HDL بین دو گروه مشاهده نشد.نتیجه گیریهر چند 12 هفته تمرین LAM موجب بهبود ترکیب بدن، آمادگی قلبی تنفسی و کاهش سطح کلسترولLDL- در مردان چاق میان سال گردید، اما این نوع روش تمرینی بر مقدار ox-LDL-c سرم مردان چاق میان سال تاثیری نداشت.

صفحه 493مقدمهسطح فعالیت بدنی به عنوان یک عامل تاثیرگذار مستقل در بروز بیماری های غیرواگیر نقش دارد. هدف پژوهش حاضر، ارزیابی میزان فعالیت بدنی افراد در اوقات فراغت و عوامل مرتبط با آن در جمعیت بزرگسال تهرانی طی سال های 83- 1381 بود.مواد و روش هاپژوهش حاضر، روی 7285 نفر (1/43 % مرد) افراد ≥20 سال شرکت کننده در فاز دوم مطالعه ی قند و لیپید تهران صورت گرفت. برای برآورد سطح فعالیت بدنی در اوقات فراغت از MAQ استفاده شد. به هر یک از فعالیت ها بر اساس شدت نسبی آن ها، بر حسب MET وزن و نیز تعداد دفعات و مدت زمان انجام هر کدام از آن فعالیت ها ثبت و سپس افراد به دو گروه با تحرک کافی و کم تحرک تقسیم بندی شدند.یافته هاشیوع کم تحرکی در جمعیت مورد پژوهش 8/69% بود. 2/30% (1/33-2/27= ضریب اطمینان 95%) از مردان و 3/30% (8/32-7/27= ضریب اطمینان 95%) از زنان در گروه فعال قرار گرفتند. کاهش معنی دار فعالیت بدنی در اوقات فراغت در مردان با افزایش سن، نمایه ی توده ی بدن، مصرف سیگار و ساعات کاری، و افزایش معنی دار آن با افزایش سطح تحصیلات و سطح فعالیت بدنی در اوقات فراغت مشاهده گردید. در هر دو جنس بین سطح فعالیت بدنی در اوقات فراغت و گروه های شغلی ارتباط معنی داری دیده شد (05/0P<).نتیجه گیریاین بررسی، شیوع بالای کم تحرکی را در جامعه ی تهرانی نشان داد که مستلزم اجرای سیاست های بهداشتی برای تشویق فعالیت بدنی در جامعه ی تهرانی است.

صفحه 504مقدمهاختلال در میزان لیپیدها و آپولیپوپروتئین ها از عوامل خطرساز بیماری قلبی عروقی محسوب می شود و خوشه ی ژنی APOAI-CIII-AVIنقش مهمی در تنظیم سوخت و ساز و میزان لیپیدها دارد. هدف پژوهش حاضر، بررسی ارتباط 5 پلی مورفیسم در ناحیه ی ژنی Apo11q با میزان لیپیدها در سرم می باشد.مواد و روش هامطالعه ی مقطعی حاضر روی 823 نفر (340 مرد و 483 زن) از جمعیت مورد مطالعه ی قند و لیپید تهران (TLGS)انجام گرفت که از میان 10759 نفر 19 تا 70 ساله، به روش تصادفی انتخاب شدند. متغیرهای تن سنجی و میزان غلظت TG، Chol، HDL، Apo Apo AIV، Apo B، Apo CIII و AI برای افراد مورد پژوهش اندازه گیری شد. قطعات ژنی به روش PCR، تکثیر و پلی مورفیسم ها با کمک آنزیم های محدودکننده تعیین شدند.یافته هافراوانی اللی پلی مورفیسم ها در جمعیت زنان و مردان، تفاوت معنی داری نداشت. توزیع ژنوتیپ ها برای تمام پلی مورفیسم ها به جز Apo AI (+83C>T) از قانون هاردی واینبرگ تبعیت نمود. یافته های این پژوهش، بیانگر ارتباط بین پلی مورفیسم های ناحیه ی ژنی مورد نظر با میزان TG، HDL-C، HDL2، Apo AI و Apo B می باشد. در نتیجه ی تجزیه و تحلیل هاپلوتیپ ها، علاوه بر متغیرهای یاد شده، میزان کلسترولLDL- و کلسترول تام نیز با پلی مورفیسم ها، ارتباط نشان دادند.نتیجه گیرییافته های پژوهش حاضر نشان دهنده ی ارتباط تغییرات ژنتیکی در خوشه ی ژنی Apo AI-CIII-AIV با میزان لیپیدها در سرم و تنظیم سوخت و ساز آن ها در جمعیت تهرانی بود.

صفحه 514مقدمهبیش از 50 سال است که زیست دسترسی پایین املاح در نان ایرانی به دلیل وجود اسید فیتیک بالا مشخص شده است. هنوز هم بیش از یک سوم انرژی غذایی در کل کشور از نان تامین می شود. بنابراین بهبود زیست دسترسی املاح نان می تواند نقش بسزایی در کاهش شیوع فقر بسیاری از املاح داشته باشد. هدف از این پژوهش، بررسی تاثیر مکمل یاری با آنزیم فیتاز بر وضعیت روی در خون، کبد، استخوان و آهن در خون و کبد، و در نهایت کلسیم استخوان در موش های صحرایی نر ویستار تغذیه شده با رژیم دارای نان پرفیتات سنگک بود.مواد و روش ها30 موش صحرایی تازه از شیر گرفته شده در پژوهشی 6 هفته ای به دو گروه فیتاز (آسپرژیلوس نایگر) و کنترل تقسیم شدند. طراحی غذای مورد استفاده بر پایه ی الگوی رژیم غذایی ایرانی ها و 2/34% انرژی از نان سنگک تامین شد.یافته هاغذای خورده شده، افزایش وزن، وزن کبد و استخوان فمور بین دو گروه تفاوت معنی داری را در انتهای پژوهش نشان نداد. روی خون در گروه فیتاز بالاتر بود (03/0P=). به این ترتیب اثرات مفید مکمل یاری فیتاز بر عنصر روی خون مستقل از وضعیت رشد می باشد. متغیرهای دیگر تفاوت معنی داری را بین دو گروه نشان ندادند.نتیجه گیرییافته های این پژوهش نشان می دهد اضافه کردن آنزیم فیتاز به رژیم دارای نان پرفیتات ایرانی می تواند وضعیت عنصر روی خون را در موش های صحرایی بهبود بخشد.

ازآنجا که اختلال عملکرد نورواندوکرین به دنبال ضایعات ضربه ای مغز، علایم بالینی چندانی ایجاد نمی کند، به طور کلی تشخیص داده نشده و به سایر مشکلات جسمی و روحی به دنبال ضربه ی مغزی نسبت داده می شود. بر این اساس، بیشتر بیماران مورد درمان قرار نمی گیرند. این مورد ممکن است بر روند بهبودی آن ها تاثیرگذار باشد. پژوهش حاضر، به منظور ارزیابی وضعیت ترشحی هورمون های هیپوفیز قدامی در بیماران دچار ضایعات ضربه ای مغز صورت گرفت.
مواد و روش هاتمام بیماران دچار ضربه ی مغزی که در بخش تروما بستری شدند و GCS آن ها کمتر یا مساوی 12 بود، وارد پژوهش شدند. 154 بیمار انتخاب شدند، ولی در نهایت 70 نفر از آن ها تا پایان پژوهش در طرح مشارکت نمودند. 6 ماه بعد از ضربه به مغز، بررسی تمام آزمودنی های عملکرد هیپوفیز قدامی با اندازه گیری غلظت پرولاکتین، تستسترون،FSH، LH Cortisol، FT4، TSH،و IGF-1 در سرم انجام، و در صورت نیاز آزمون تحریکی کوزینتروپین و گلوکاگون به ترتیب برای اثبات کمبود کورتیزول و هورمون رشد صورت گرفت.
یافته هادر بررسی حاضر، 41 بیمار (6/58%) دچار اختلال ترشحی در کمینه یک هورمون هیپوفیز قدامی بودند. در 4 بیمار (7/5%) درگیری در 2 رده ی هورمونی وجود داشت. اشکال کمبودهای هورمونی، به صورت کمبود گنادوتروپین ها و کورتیکوتروپین هر یک در 9 بیمار (9/12%) و سوماتوتروپین در 3 بیمار (3/4%) مشاهده گردید. هیپر پرولاکتینمی در 23 مورد (5/31%) دیده شد. در این پژوهش هیچ موردی از کمبود تیروتروپین مشاهده نگردید.
نتیجه گیریپژوهش حاضر نشان داد که اختلالات هورمونی پس از ضایعات ضربه ای مغز شایع می باشد و بایستی مورد توجه کافی از طرف پزشکان قرار گیرد. کمبود بعضی از این هورمون ها مانند کورتیکوتروپین می تواند تهدید کننده ی سلامت بیمار باشد.

صفحه 530مقدمهنقش مورفین در تغییرات گلوکز خون شناخته شده است. این پژوهش با هدف بررسی سازوکار های هورمونی تاثیر مورفین بر گلوکز خون موش های دیابتی و غیردیابتی انجام گرفت.مواد و روش هاموش ها به دو گروه دیابتی و غیردیابتی، و سپس هر گروه به زیرگروه های 1-کنترل (سالین + سالین)، 2- سالین+ مورفین، و 3- نالوکسان+مورفین تقسیم شدند. نمونه های خون به منظور تعیین گلوکز خون و هم چنین آنالیزهای هورمونی(انسولین، کورتیزول و اپی نفرین) دریافت گردید.یافته هادر موش های غیردیابتی و دیابتی میزان گلوکز خون در گروه سالین+ مورفین، در مقایسه با گروه کنترل به ترتیب تا 3 و 1 ساعت کاهش معنی داری داشت، که این کاهش در گروه نالوکسان+ مورفین جبران گردید. پس از 3 ساعت، میزان انسولین در موش های غیردیابتی کاهش معنی داری را نسبت به گروه کنترل نشان داد که این کاهش در گروه نالوکسان+ مورفین جبران گردید. در حالی که انسولین در موش های دیابتی تغییر معنی داری نداشت. کورتیزول در موش های غیر دیابتی و دیابتی پس از 3 ساعت تغییر معنی داری را نسبت به گروه کنترل نشان نداد. اپی نفرین پس از 3 ساعت در موش های دیابتی و غیردیابتی کاهش معنی داری را نسبت به گروه کنترل نشان داد، که این کاهش در گروه نالوکسان + مورفین جبران گردید.نتیجه گیریمورفین زیرجلدی موجب هیپوگلیسمی در موش های دیابتی و غیر دیابتی می شود. هیپوگلیسمی به دست آمده و کاهش انسولین در موش های غیردیابتی، و بدون تغییر ماندن آن در گروه دیابتی به احتمال زیاد از راه غیروابسته به انسولین صورت می گیرد.

Page 441IntroductionComplications of gestational diabetes mellitus (GDM) have drastic effects on the health of the mother and fetus from different aspects. Sufficient knowledge and self care skills, especially regarding nutrition and exercise, play a major role in the management of GDM. This study aimed at determining the impact of education on nutrition and exercise awareness and metabolic control indicators (fasting and post prandial blood sugars) among a group of gestational diabetes mellitus (GDM) patients, referred to hospitals of Shiraz University of Medical Sciences.Materials And MethodsIn this interventional study, 61 class A1 gestational diabetes mellitus (GDM) patients were selected through easy sampling and randomly assigned to two groups of experimental and controls. Before the intervention, we measured metabolic control indicators and levels of knowledge in both groups, using laboratory tests and a specifically designed and validated written test (containing 34 multiple choice questions respectively, with a reliability of KR20: 0.81). Educational interventions were then implemented for about 6 hours weekly for the experimental group, and the expected outcomes were measured in both groups immediately following and again two weeks after the end of the intervention. Finally, the data collected were analyzed by SPSS software, version 11.5, using independent sample t-test, Chi-square and Bonferroni.ResultsThe results of data analysis revealed that after educational intervention the means for knowledge and post-prandial blood sugars (1 and 2hpp) were significantly different in the experimental group (p<0/001).ConclusionResults show that the effectiveness of the educational intervention in the improvement of the level of knowledge and metabolic control indicators, emphasizing hence the need for appropriate interactive education among such groups.

Page 449IntroductionImprovements in lipid profiles and blood pressure are essential for preventing complications of diabetes. Considering the strong inclination people have to use herbal medicines, we aimed to investigate the effects of hydro alcoholic nettle extract on lipid profiles and blood pressure in type 2 diabetes patients.Materials And MethodsA single-blind, randomized clinical trial was conducted on 50 type 2 diabetes patients, randomly divided into two groups the intervention (n=25) and control (n=25). Subjects in the intervention group consumed 100 mg/kg of hydro alcoholic nettle extract daily, while those in the control group consumed 100 mg/kg of placebo daily for 8 weeks. Lipid profiles, systolic) and diastolic blood pressure were measured at baseline and at the end of 8th week of intervention.ResultsThe intervention group showed a significant reduction in levels of triglycerides (TG), SBP and plasma atherogenic indexes (LDL-C / HDL-C and TC / HDL-C) (P<0.05), and a significant increase in level of High Density Lipoproteins (HDL-C) (P<0.05), at the end of 8th week of intervention, compared to control group. No significant changes however were observed in Total Cholesterol (TC), Low density lipoproteins (LDL-C), and DBP at the end of 8th week of intervention.ConclusionHydro alcoholic extract of nettle had positive effects on the lipid profiles and blood pressure status of type 2 diabetes patients, indicating its consumption can be recommended as auxiliary therapy in patients for type 2 diabetes.

Long-standing uncontrolled type 2 DM may affect the motility and function of the gastrointestinal (GI) system, throughout the GI tract, from the esophagus to the anorectal region. This study was designed to evaluate the prevalence of GI symptoms in type 2 DM patients and its association to age, sex, glycemic control and duration of diabetes.
Materials And MethodsThis was a cross-sectional study conducted on 350 adult type 2 DM patients, selected with sequential sampling from the Ahvaz Diabetes Research Center Clinic in 2010. Data were collected by a questionnaires and analyzed using independent t-test and Chi-score with SPSS version18.
ResultsPatient mean age was 56 ± 11 years, and the prevalence of GI symptoms was 67%. The most prevalent GI symptoms were constipation (43%), bloating (35%) and regurgitation (31%). Prevalence of GI symptoms was related to duration of diabetes (P<0.001). Higher levels of fasting plasma glucose increased the prevalence of GI symptoms (P=0.003), which were significantly higher in females than in males (P<0.001), and also in older patients (P=0.003). There was no association between prevalence of GI symptoms and levels of HbA1C or BMI.
ConclusionPrevalence of GI symptoms in type 2 DM was 67%, the most prevalent being constipation, bloating and regurgitation. There was significant association between prevalence of GI symptoms and acute glycemic control (FBS), duration of type 2 diabetes, female gender and age.

Page 467IntroductionSelf-management of diabetes, an important aspect of diabetes treatment, is strongly affected by psychological and sociostructural factors. The aim of this research was to investigate the psychological, social and structural determinants of diabetes self-management in patients with type 2 diabetes.Materials And MethodsIn cross-sectional design study, 500 out patients (245 men and 255 women) with type II diabetes were selected by convenience sampling. Data collected by demographic questionnaires on the Scales of diabetes self-management, diabetes self- efficacy, belief to treatment effectiveness, belief to illness certainty, treatment motivation, patient- provider therapeutic communication, diabetic knowledge, satisfaction for therapeutic services, access to therapeutic services, familial social support, and on collaborative decision-making, were analyzed by correlation and stepwise multiple regressions.ResultsDiabetes self-management mean (standard deviation) was 45.77 (21.81). Self-efficacy, belief of treatment effectiveness, social support, >HbA1c%7 and Insulin therapy significantly predicted diabetes self-management variance (P<0.001). The variables explained 91% of diabetes self-management (R2 = 0.91). Self-efficacy has highest weight in prediction of diabetes self-management (β = 0.338, P<0.001).ConclusionSelf-efficacy, belief of treatment effectiveness, social support, mild HbA1c and treatment type are important factors for diabetes self-management and these variables explained the high rate of the diabetes self-management variance, emphasizing the importance of improvement of self-management behaviors in these patients.

Page 479IntroductionMetabolic syndrome is a physiologic syndrome, characterized by increasing coronary heart disease risk factors like hypertension, diabetes, dyslipidemias and obesity. The aim of this study was to determine some metabolic syndrome factors and percent age of body fat and its relationship with maximal oxygen consumption among patients with Coronary Artery Disease hospitalized in the Coronary Care Unit.Materials And MethodsThis was descriptive correlative study that 90 coronary patients 40-80 years old participated by available sampling. We used Non-Exercise Test formulas for maximal oxygen consumption Predicting. Fasting blood sample used for TG, FBS, HDL-C. Percent body fat was estimated by measuring 4 skin fold thicknesses and blood pressure was measured.ResultsThe means of the factors of Metabolic syndrome and percent body fat were higher among females, compared to males. Decrease in HDL and increased blood pressure were the most prevalent among risk factors. There was a significant negative relationship between maximal oxygen consumption and body fat, FBS, SBP.ConclusionAccording to our findings, the prevalence of risk factors of metabolic syndrome and percentage of body fat were high. With increase of body fat, FBS and SBP, all results of unhealthy lifestyles, maximal oxygen consumption is decreased.

Page 486IntroductionExercise significantly prevents the progression of atherosclerosis and oxidized LDL-C (ox-LDL-C), facilitates monocyte-related atherogenesis. The effects of Lifestyle Activity Modification (LAM) training based on guidelines of the Centers for Disease Control (CDC) and the American College of Sports Medicine (ACSM) on serum oxidized LDL-C (ox-LDL-C) in obese people are not well known. We investigated the effect of LAM on serum ox-LDL-C in obese middle aged men.Materials And MethodsSixteen healthy obese middle aged men (aged 42.06±6.01 years, mean ± SD) were randomly assigned to the LAM (n=8) and the control (n=8) groups. Subjects in the LAM group walked 2 miles daily in 30 minutes 4 days weekly for 12 weeks on treadmills, according to the CDC and ACSM guidelines.ResultsResults showed that body weight, BMI, body fat mass, body fat percent, waist to hip ratio (WHR) and LDL-C decreased significantly after 12 weeks in the LAM group compared to the controls (P<0.05), and their maximum oxygen uptake was increased significantly compared to the controls after 12 weeks (P<0.05). On the other hand, after 12 weeks of LAM training, no significant differences were observed in serum ox-LDL-C, total cholesterol (TC), triglycerides (TG) and HDL-C between the groups.ConclusionAlthough twelve weeks of LAM improved body composition and cardio-respiratory fitness and decreased LDL-C levels in obese middle aged men, serum ox-LDL-C were not affected by this protocol in this group.

Page 493IntroductionPhysical activities represent an independent risk factor for a number of chronic diseases. The aim of this study was to assess physical inactivity in a sample of Tehranian adults.Materials And MethodsWe selected 7285 TLGS participants (≥ 20 years), who had completed physical activity questionnaires between 2002-2004. Leisure time physical activity level was assessed using the modifiable Activity Questionnaire (MAQ), with each activity being weighted by its relative intensity, referred to as a Metabolic Equivalent (MET). Subjects were asked to report the activities they had participated in during past 12 months and to estimate the frequency and duration for each leisure time activity indentified and were categorized into the active and inactive groups.ResultsThe overall prevalence of inactivity was 69.8% and only 30.2% (95%CI=27.2-33.1%) of males and 30.3% (95%CI=27.7-32.8%) of females were considered active. In males, the prevalence of inactivity increased with age and the highest rate of physical inactivity was observed in the 40-49 year group. There was a statistically significant decrease in leisure time physical activity level (LTPA) with increasing body mass index, and smoking and hours of work. A positive association was observed between LTPA and high educational levels in males. Moreover, a statistically significant difference was found between the physical activity and occupations between men and women (p<0.05).ConclusionsFindings of the present study showed that a high prevalence of physical inactivity among Tehranian adults, raising concerns about public health, and necessitating health policies encouraging the population to be more active.

Page 504IntroductionChanges in lipids and apo lipoproteins levels are considered a risk factor for cardiovascular disease. The APOAI-CIII-AIV gene cluster plays an important role in regulating of the metabolism and level of lipids. The aim of this study was to elucidate the associations of five single nucleotide polymorphisms in the Apo11q cluster gene with lipid levels.Materials And MethodsA cross-sectional study of 823 subjects (340 males and 483 females) from the Tehran Lipid and Glucose Study (TLGS) was performed. Anthropometrical and serum concentrations of TG, Chol, HDL, Apo AI, Apo AIV, Apo B, Apo CIII were measured. The segments of the APOAI-CIII-AIV gene cluster were amplified by PCR and the polymorphisms were revealed by RFLP using restriction enzymes.ResultsAllele frequencies of each SNP did not differ significantly between males and females. Genotypes and alleles distributions were in Hardy-Weinberg equilibrium, except for Apo AI (+83C>T). Results demonstrated a significant association between TG, HDL-C, HDL2, Apo AI and Apo B levels and the presence of some alleles in the polymorphisms studied. After haplotype analysis not only did the association between these variables and SNPs remain, but the levels of total cholesterol and LDL-C were also added.ConclusionThe results of the present study showed that in addition to environmental factors, genetic variations are also important in the regulation of the metabolism and level of lipids such as TG and HDL-C.

Page 514IntroductionRestricted mineral bioavailability of minerals in Iranian breads due to high levels of phytic acid has been reported for more than 50 years. Bread intakes still provide over one-third of the food energy through out the country. Hence improving bread mineral bioavailability can play an important role in decreasing the prevalence of many mineral deficiencies. The aim of this study was to assess the effect of phytase supplementation on blood, liver and bone zinc, blood and liver iron and femur bone calcium in growing Wistar male rats, fed a diet containing high phytate Iranian bread (Sangak).Materials And MethodsThirty weaning Wistar rats were assigned to the phytase (Aspergillus niger) or the control group for 6 weeks. The diet was designed based on Iranian food patterns and 34.2 % of energy was supplied from Sangak bread with high amount of phytic acid.ResultsFeed intakes, weight gain, liver and femur bone weight did not differ between the groups. Blood zinc was higher in the phytase group (p=0.03), indicating the positive effect of phytase supplementation on blood zinc levels, independent of the growth process. Other variables did not show any differences between groups.ConclusionWe conclude that the addition of high phytate Iranian bread phytase to the diet can improve blood zinc status in growing rats.

Neuroendocrine dysfunction following traumatic brain injury (TBI) is frequently missed due to the absence of major symptoms and very often, no appropriate management is prescribed, thus delaying the patient recovery. This study is aimed to determine the frequency and pattern of anterior pituitary dysfunction following TBI.
Materials And MethodsFrom June to December 2009, overaperiod of seven months, a total of 156 cases were admitted to a trauma center following TBI. Of the seventy patients, (61 males and 9 females mean age 30 years) included in the study, 39 patients had moderate (GCS; 9 – 12) and 31 cases had severe (GCS < 8) TBI. Patients were tested 6 months after injury for possible secretory abnormalities of anterior pituitary hormones. Anterior pituitary secretary function was assessed by measurement of serum levels of FT4, TSH, basal GH, IGF-1, 8 am Cortisol, FSH, LH, total testosterone and prolactin. Dynamic tests of ACTH and glucagon stimulation were used to evaluate the pituitary-adrenal axis and GH secretory status.
ResultsForty-one patients (58.6%) were found to be suffereding from at least one hormonal secretory abnormality, with patterns and frequencies of: Gonadotropins (LH, FSH), 12.9%, corticotropin (ACTH), 12.9%, somatotropin (GH), 4.3%, and prolactin (PRL), 1.4%. There was no case with thyrotropin deficiency. Hyperprolactinemia was found to be present in 23 cases (31.5%).
ConclusionThe results of this study showed that anterior pituitary hormone deficiencies occur frequently, 6 months following traumatic brain injury. The two most commonly involved axes were the pituitary-gonadal and the pituitary-adrenal. Hypocortisolism may be particularly harmful for the patient's health.

Page 530IntroductionThe role of morphine in blood glucose changes has been documented. The aim of the present study was to investigate the hormonal mechanisms involved in changes caused by morphine on blood glucose in diabetic and non-diabetic mice.Materials And MethodsAnimals were divided into two, the diabetic and non-diabetic, groups. Each group was further divided into subgroups: 1. Saline+saline 2. Naloxone+morphine and 3. Control (saline+saline). Blood samples were used for determining the blood glucose as well as for hormonal (insulin, cortisol and epinephrine) analyses.ResultsIn non-diabetic and diabetic mice, blood glucose was significantly decreased in the saline+morphine group, compared to controls at 3h and 1h respectively, a decrease which compensated in the naloxone+morphine group, compared to saline+morphine group. After 3h, the level of insulin in non-diabetic mice was significantly decreased compared to the controls and was compensated in the naloxone+morphine group. At this time, the level of insulin showed no changes in diabetic animals. The level of cortisol remained constant both in diabetic and non-diabetic animals at 3h. The level of epinephrine displayed a significant decrease in diabetic and non-diabetic mice at 3h when compared to the controls and was compensated in the naloxone+morphine compared to the saline+morphine group.ConclusionThe administration of subcutaneous morphine caused hypoglycemia in diabetic and non-diabetic mice. The hypoglycemia and the decrease of insulin level in non-diabetic mice as well as the unchanged level of this hormone in diabetic animals may suggest an insulin-independent hypoglycemia induced by morphine.
