Background
The novel coronavirus (COVID-19) disease is a global pandemic of different severity ranging from a mild respiratory disease to severe septic shock. Previous studies have established the association of acute stroke and COVID-19. Herein, we aimed to describe the characteristics of patients with stroke during COVID-19 pandemic.
Methodin this cross sectional study, all patients with acute stroke who referred to Bouali hospital were enrolled from March to June 2020. The patients were categorized into two groups with and without COVID-19. In the following, the demographic and clinical characteristics of stroke patients in both groups were evaluated. Eventually SPSS software version 22 was used to analyze the data.
Results61 patients with acute stroke were identified in which 22 patients were positive for COVID-19. Except for the in-hospital mortality (P=0.07) and type of hemorrhagic transformation (P=0.02), we did not find a significant difference in the demographic and clinical characteristics of patients in both groups. The majority of patients with COVID-19 were severely symptomatic. However, the severity of chest CT involvement was statistically correlated with the mean mRS (P=0.05).
ConclusionIn this study, we revealed acute stroke affected COVID-19 patients with traditional stroke risk factors at an age typically seen in non-COVID populations which mainly constitute cryptogenic acute ischemic stroke. We also noted a higher in-hospital mortality rate in patients with COVID-19 associated stroke which reflects a worse outcome of COVID-19. These results highlight the possible hypercoagulopathy state associated with COVID-19 which predisposes patients to develop stroke.

مقدمه
یکی از شایعترین آریتمیهای قلبی بعد از جراحی بایپس عروق کرونر، فیبریلاسیون دهلیزی است و بروز آن با افزایش طول بستری، میزان بروز بیماری و مرگ و میر پس از عمل همراه است. به تازگی، نقش استرسهای اکسیداتیو در پاتوفیزیولوژی این آریتمی مطرح شده است. مطالعهی حاضر، جهت بررسی نقش تجویز ویتامین C در پیشگیری از بروز این آریتمی طراحی شده است.
روشها:
در این مطالعهی کارآزمایی بالینی، 140 بیمار که در بیمارستان بوعلی در سالهای 97-1396 تحت جراحی بایپس عروق کرونر قرار گرفتهاند، به صورت تصادفی و دو سو کور به دو گروه مساوی مورد و شاهد تقسیم شدند. گروه مورد، از 24 ساعت قبل تا 48 ساعت بعد از عمل، روزانه 1 گرم ویتامین C و گروه شاهد در این مدت دارونما دریافت کردند. در نهایت، دادهها در دو گروه مورد بررسی و واکاوی با نرمافزار SPSS قرار گرفت و بروز فیبریلاسیون دهلیزی بعد از عمل در هر دو گروه با هم مقایسه شد.
یافتهها:
از بین 140 بیمار مورد مطالعه، از 70 نفر گروه مورد 4 نفر (70/5 درصد) و از 70 نفر گروه شاهد 11 نفر (71/15 درصد) دچار فیبریلاسیون دهلیزی شدند که بر اساس واکاوی آماری، تفاوت دو گروه معنیدار بود (028/0 = P).
نتیجهگیری:
مطالعهی حاضر نشان داد که تجویز ویتامین C، میتواند به طور موثری سبب کاهش بروز فیبریلاسیون دهلیزی بعد از جراحی بایپس عروق کرونری شود.

Recently, the role of oxidative stress in the pathophysiology of this arrhythmia has been raised. Current research was done to assess the role of prescribing vitamin C in preventing this arrhythmia.
MethodsIn this clinical trial study, 140 patients who underwent coronary artery bypass graft surgery in Booali hospital, Qazvin, Iran, in the years 2016 to 2017 were randomly divided into two equal case and control groups. The case group received 1 g of vitamin C per day from 24 hours prior to 48 hours after surgery, and the control group received placebo at same time. Finally, data were analyzed using SPSS software, and incidence of postoperative atrial fibrillation was compared between the two groups.
FindingsOf 140 studied patients, 4 cases (5.7%) of the case group and 11 cases (15.71%) of control group had atrial fibrillation, and the difference between the two groups was significant (P = 0.028).
ConclusionOur study showed that administration of oral vitamin C can effectively reduce the incidence of atrial fibrillation after coronary artery bypass graft surgery.
