    Background
    Adherence to regular physical activity (PA) remains a challenge for hypertensive (HTN) patients. The present study was aimed at assessing the effectiveness of theory-based training intervention on PA and blood pressure in HTN patients.
    Methods
    The randomized control trial conducted at the healthy heart house in Shiraz, Iran, from January 2015 to January 2016 on 78 HTN patients (39 experimental group (EG) and 39 control group (CG). The exercise program for the EG included 30 minutes of fast walking at 40% to 60% of maximum heart rate reserve 5 days a week for 3 months. The study outcomes included PA stages of change, exercise self-efficacy (ESE), decisional balance, PA and 24-hour ambulatory blood pressure (ABP) and were measured at the onset and at three months post-intervention.
    Results
    Between baseline and three months post-intervention, a greater increase in PA was observed in the EG than in the CG, giving a mean difference of 872.50 metabolic equivalent (MET), p = 0.004. The mean change from baseline to three months post-intervention in 24-hour systolic and diastolic ABP were significant between the EG and CG (mean difference -9.48 mmHg, P = 0.007 and -4.51 mmHg, P = 0.010 respectively). In the EG, 35 subjects (89.7%) and in the CG, 8 subjects (20.5%) entered the stage of action. The mean change from baseline to three month post intervention in ESE and decisional balance was significant between the EG than the CG (mean difference = 20.20, P = 0.001 and 0.50, P = 0.024 respectively).
    Conclusions
    Among HTN patients, a three-month Theory-based training intervention used was effective in increasing PA and reducing 24 hour ABP.

    Background
    Although physical activity is necessary, but not performed by most hypertensive individuals. One of the most common theoretical frameworks used for understanding health behaviour change is the trans-theoretical model.
    Objectives
    This study was conducted to evaluate the effect of a trans-theoretical model based intervention on physical activity in hypertensive patients.
    Methods
    We conducted a randomized clinical trial among 60 hypertensive patients (31 experimental and 29 control). Participants in experimental group received a fast walking intervention based on a trans-theoretical model. Physical activity stages of change, processes of change, frequency, duration and intensity of walking were measured at the onset, 3 and 6 months post-intervention. The data was analyzed using SPSS 22 software.

    Background
    Hypertension (HTN) is considered to be the most important modifiable risk factor for cardiovascular morbidity and mortality. Although lifestyle modifications such as increase in physical activity (PA) are effective for lowering blood pressure (BP), it can often be difficult for hypertensive (HTN) individuals to modify their lifestyle and maintain such modification. So this study aimed at assessing the effectiveness of trans-theoretical model (TTM)-based intervention on PA in HTN patients.
    Materials And Methods
    This randomized controlled trial was conducted on 78 hypertensive patients (39 in the experimental group (EG) and 39 in the control group (CG)) from January 2015 to September 2015 in the city of Shiraz in southern Iran. The EG received TTM-based intervention about PA for four weeks. The study outcomes included PA stages of change, processes of change, exercise self-efficacy (ESE), decisional balance and PA, which were measured at the onset and at one week post-intervention. The data were analyzed using SPSS 16 software.
    Results
    At one week post-intervention, PA increased significantly in the EG (1218.35±917.95) compared to the CG (810.28±785.21) (p=0.04). A significantly larger percentage of the EG (80.6%, n= 25) progressed to the stage of action compared with the CG (16.1%, n=5).     The present study showed that the TTM-based intervention effectively increased PA. So the use of TTM-based interventions can be effective in increasing the commitment to PA in HTN patients.
