عدم استفاده از داروهای ضد درد در بیماران کاندید ماستکتومی و عوارض داروهای ضد درد در این بیماران و همچنین افزایش شدت درد در بیماران ماستکتومی شده ای که قبلا تحت رادیوتراپی بوده اند موجب شد مطالعه حاضر را با هدف بررسی تاثیر ماساژ بازتابی کف پا بر نیاز به اوپیویید و درد ناشی از جراحی ماستکتومی در مبتلایان به سرطان پستان رادیوتراپی شده به انجام برسانیم.
روش بررسیاین مطالعه از نوع کارآزمایی بالینی تصادفی شده است که با مشارکت 54 بیمار (برآورد بر اساس مطالعه پایلوت) که به کمک جدول اعداد تصادفی به دو گروه تقسیم شده بودند، در طی ده ماهه منتهی به شهریور 1398 در بیمارستان امام رضا (تبریز) انجام شد. گروه مداخله از چهار ساعت پس از ترخیص از ریکاوری به مدت دو روز (هر شش ساعت) ماساژ کف پارا دریافت می کردند (هر پا ده دقیقه) اطلاعات بیماران در فرم اطلاعات دموگرافک ثبت شد و شدت درد نیز با کمک ابزار دیداری درد سنجیده شد. آزمون های کای دو و تست دقیق فیشر و تی مستقل در نرم افزار SPSS21 تحلیل شدند. P<0.05 معنی دار در نظر گرفته شد.
یافته هادو گروه کنترل (45/9±29/51) و مداخله (96/8±03/52) از نظر شدت درد در قبل (293/0=P) از مداخله بدون اختلاف آماری معناداری بودند؛ حال آنکه پس از اتمام مداخله (003/0=P) اختلاف آماری معناداری در متغیر ذکر شده بین دو گروه کنترل (95/8±88/49) و مداخله (75/6±12/20) مشاهده شد و از طرفی دیگر در گروه مداخله بعد از دریافت مداخله، اختلاف آماری معناداری نسبت به قبل از مداخله مشاهده شد (P<0.05). میزان پتدین مصرفی بین دو گروه مداخله (49/4±99/29) و کنترل (50/4±18/29) قبل از مداخله بدون اختلاف آماری معنادار بود (782/0=P)، ولی بعد از مداخله اختلاف آماری معنادار بین دو گروه کنترل (01/5±12/32) و مداخله (56/4±41/10) مشاهده شد (001/0=P). از طرفی دیگر مقایسه تغییرات شدت درد و میزان مصرف پتدین قبل و بعد از مداخله بین دو گروه نشان دهنده تغییرات آماری معناداری بود (001/0=P).
نتیجه گیریماساژ بازتابی کف پا توانست موجب کاهش شدت درد و نیاز به اوپیویید در زنان کاندید ماستکتومی با سابقه دریافت رادیوتراپی گردد.

ریزش مو (آلوپسی) یکی از عوارض جانبی رایج داروهای شیمی درمانی است که بار روانی زیادی بر بیماران تحمیل می کند. سرد کردن پوست سر با استفاده از کلاه سرد یک روش متداول برای پیشگیری از آلوپسی است، اما نتایج در این زمینه قطعی نیست. هدف مطالعه حاضر بررسی تاثیر استفاده ازکلاه سرد بر آلوپسی در بیماران مبتلا به سرطان پستان بود.
روش بررسیدر این مطالعه تجربی (دی 1397 تا فروردین 1398)، 100 بیمار مبتلا به سرطان پستان بصورت در دسترس انتخاب شده و به صورت تصادفی و به شکل مساوی به دو گروه مداخله (استفاده از کلاه سرد در حین شیمی درمانی) و کنترل (شیمی درمانی بدون کلاه سرد) تقسیم شدند. برای گردآوری داده از دو پرسشنامه اطلاعات دموگرافیک و پرسشنامه عوارض شیمی درمانی محقق ساخته استفاده شد.
یافته هاگروهی که از کلاه سرد استفاده کرده بود در مقایسه با گروه کنترل نمره ریزش موی کمتری داشت (49/0± 25/2 در مقابل 34/0 ± 49/2؛ 005/0 = p).
نتیجه گیریاستفاده از کلاه سرد در کاهش ریزش موی ناشی از شیمی درمانی موثر است

چالش های ناشی از بیماری سرطان پستان، می تواند روابط صمیمانه زوجین را هدف قرار داده و ضرورت مداخلات روانشناختی را در طول دوره بیماری و بعد از اتمام درمان نیز در اولویت قرار دهد. این مطالعه با هدف تعیین اثربخشی برنامه آموزش مبتنی بر درمان تعهد و پذیرش بر کیفیت زندگی و الگوهای ارتباطی زنان مبتلا به سرطان پستان و همسران آنها انجام شد.
روش بررسیمطالعه از نوع کارآزمایی بالینی کنترل شدهی تصادفی با طرح پیش آزمون- پس آزمون و با گروه کنترل بود. افراد حایز شرایط، به شکل تصادفی ساده در دو گروه آزمایش و کنترل قرار گرفتند. هر گروه شامل 10 نفر (5 زوج) بودند. هر دو گروه، پرسشنامه های کیفیت زندگی بیماران مبتلا به سرطان پستان و کیفیت زندگی سازمان بهداشت جهانی و نیز پرسشنامه جدایی- فاصله گاتمن را تکمیل کردند. سپس، گروه آزمایش در جلسات آموزشی درمان تعهد و پذیرش به مدت 8 جلسه دو ساعته شرکت کردند. پس از اتمام دوره آموزش و سه ماه بعد از آن از هر دو گروه، آزمون گرفته شد. داده ها با استفاده از آزمون های تی مستقل و کوواریانس مورد تحلیل قرار گرفتند.
یافته هانتایج نشان داد میانگین نمرات الگوی ارتباطی (50/4 ± 40/10) و کیفیت زندگی (17/11 ± 46/54) در گروه آزمایش پس از مداخله، نسبت به میانگین الگوی ارتباطی (40/3 ± 30/15) و کیفیت زندگی (34/11 ± 34/45) در گروه کنترل، تفاوت معناداری داشت (001/0>p).
نتیجه گیریبراساس یافته های به دست آمده از پژوهش حاضر، آموزش درمان تعهد و پذیرش می تواند باعث بهبود کیفیت زندگی و الگوهای ارتباطی بیماران مبتلا به سرطان و همسران آنها شود و در کاهش تنش ها و چالش های ارتباطی زوجین موثر باشد.

با توجه به شیوع بالای همبودی مشکلات روان شناختی با دردهای مزمن و سرطان نیاز به طراحی و اجرای مداخلات مناسب در این زمینه ضروری است؛ بنابراین این مطالعه با هدف بررسی اثربخشی درمان شناختی - رفتاری یکپارچه شده با ذهن آگاهی بر دردهای سرطانی مبتلایان به سرطان پستان صورت گرفت.
روش بررسیدر این مطالعه نیمه آزمایشی با پیش- پس آزمون و کنترل تعداد 24 بیمار مبتلا به سرطان پستان به روش هدفمند انتخاب و به روش تصادفی در دو گروه مداخله و کنترل جایگزین شدند (هر گروه 12 نفر). شرکت کنندگان با پرسشنامه های درد سرطانی کوتاه، خودکارآمدی درد و فاجعه سازی درد قبل، بلافاصله بعد و یک ماه بعد از مداخله ارزیابی شدند. شرکت کنندگان در گروه مداخله به مدت 8 هفته مداخله درمان شناختی- رفتاری یکپارچه شده با ذهن آگاهی را دریافت کردند. افراد گروه کنترل در این بازه زمانی از طرف تیم تحقیق مداخله ای دریافت نکرد. جهت تحلیل یافته از روش های آماری توصیفی (درصد، فراوانی، میانگین و انحراف معیار)، خی دو، تی تست و تحلیل کوواریانس چند متغیره استفاده شد.
یافته هایافته های حاصل از آزمون کوواریانس حاکی از این بود که با کنترل اثر پیش آزمون در مرحله پس آزمون و پیگیری در هر سه متغیر درد ادراک شده، فاجعه سازی درد و خودکارآمدی درد تفاوت معناداری بین گروه کنترل و مداخله وجود دارد (001/0>P). در مرحله پس آزمون و پیگیری کاهش در نمرات درد و فاجعه سازی درد و افزایش در نمرات خودکارآمدی درد مشاهده شد. ضریب اتا برای مرحله پس آزمون در متغیرهای درد، فاجعه سازی درد و خودکارآمدی درد معادل 85/0، 78/0 و 70/0 تغییرات در گروه مداخله نسبت به کنترل نشان داد. در مرحله پیگیری ضریب اتا برای متغیرهای درد ادراک شده، فاجعه سازی درد و خودکارآمدی درد به ترتیب 87/0 ، 75/0 و79/0 به دست آمد.
نتیجه گیریدرمان شناختی- رفتاری یکپارچه شده با ذهن آگاهی در کاهش درد ادراک، خودکارآمدی درد و فاجعه سازی درد در بیماران مبتلا به سرطان پستان موثر است؛ بنابراین با توجه به صرفه اقتصادی و دسترسی آسان تر این روش درمانی به عنوان یک درمان تکمیلی برای بیماران سرطان پستان پیشنهاد می شود.

تمرین منظم از افزایش حجم تومور پیشگیری کرده، با راه اندازی مسیرهای سیگنالی در بیان ژن کاردیومیوپاتی را در افراد مبتلا به تومور پستان کاهش میدهد. هدف از پژوهش حاضر اثر 10 هفته تمرین هوازی تداومی بر نسبت حجم تومورر، پاسخ IGF-1، ژن های AKT و mTORC1 میوکارد موش مبتلا به سرطان پستان بود.
روش بررسیپژوهش حاضر از نوع تجربی است. بدین منظور 12 سر موش ماده نژاد بالبسی به صورت تصادفی به دو گروه 6تایی، تمرین هوازی تدوامی (E) و کنترل (C) تقسیم شدند. القای تومور به وسیله تزریق MC4-L2 انجام شد. تمرین هوازی تداومی شامل 75 دقیقه دویدن با سرعت ثابت 60 تا 65 درصد Vo2peak، 5 روز در هفته به مدت 10 هفته انجام شد. حجم تومور به صورت هفتگی توسط کولیس اندازه گیری و ثبت شد. 24 ساعت بعد از آخرین جلسه تمرین موش ها قربانی و بطن چپ آنها خارج شد. پروتئین IGF-1 با روش وسترن بلات و بیان ژن های AKT و TORC1توسط qReal-time سنجش شدند. جهت تحلیل آماری از آزمون t مستقل در سطح آلفای 05/0 استفاده شد.
یافته هانتایج نشان داد بیان ژنی IGF-1 در گروه تمرین هوازی تداومی نسبت به گروه کنترل (08/0=P) تفاوت معناداری نداشت. در حالی که بیان ژن های AKT (016/0=P) و m TORC1 (022/0=P) در گروه تمرین نسبت به کنترل افزایش معنی داری داشتند. حجم تومور در گروه تمرین نسبت به کنترل کاهش معناداری داشت (03/0=P).
نتیجه گیریبر اساس یافته های به دست آمده می توان چنین نتیجه گیری کرد احتمالا تمرین هوازی تداومی حجم تومور را کاهش می دهد و با افزایش بیان ژن های AKT و mTORC1 عملکرد قلب را بهبود می بخشد.

در افراد مبتلا به سرطان پستان، مقادیر سرمی عامل مغذی عصبی مشتق از مغز (Brain-Derived Neurotrophic Factor) پایین و فاکتور نکروز تومور آلفا (Tumor necrosis factor alpha) TNF-α بالا گزارش شده است. بنابراین، مطالعه حاضر با هدف بررسی تاثیر هشت هفته تمرینات دایره ای با شدت بالا بر سطوح سرمی BDNF و TNF-α زنان نجات یافته از سرطان پستان انجام شد.
روش بررسیاین کارآزمایی بالینی تصادفی شده در مرکز انکولوژی بیمارستان شهید رحیمی خرم آباد در سال 1397 انجام شد. 17 زن نجات یافته از سرطان پستان از بین 55 نفر بیمار که مراحل شیمی درمانی و پرتودرمانی را پشت سر گذرانده بودند، گزینش و به صورت تصادفی به دو گروه تمرین (9 نفر) و کنترل (8 نفر) تقسیم شدند. گروه تمرین به مدت هشت هفته، سه جلسه در هفته تمرینات را انجام دادند. برای تحلیل داده ها از آزمون های t همبسته و کوواریانس استفاده شد.
یافته هانتایج آزمون t همبسته نشان داد که پس از هشت هفته سطوح سرمی BDNF و TNFα در هر دو گروه به طور معناداری افزایش یافت (05/0˂p) اگر چه بین دو گروه تفاوت معنادار نشد، اما توان هوازی تنها در گروه تمرین افزایش معنادار یافت (0005/0=p). به علاوه، نتایج آزمون کوواریانس تنها تفاوت معنادار در توان هوازی را بین دو گروه نشان داد (05/0˂p).
نتیجه گیریهشت هفته تمرین HICT در زنان نجات یافته از سرطان پستان بدون تاثیر بر سطوح سرمی BDNF و TNFα، می تواند توان هوازی آنها را افزایش دهد.

Because of not applying analgesics in mastectomy candidates and the complications of analgesic therapy in these patients on the one hand and increased pain intensity in patients who undergo mastectomy following radiotherapy on the other hand, the present study aimed to evaluate foot reflexology massage on pain and the need for opioids in breast cancer patients undergoing mastectomy after radiotherapy.
MethodsThis randomized clinical trial was performed at Imam Reza Hospital, Tabriz, over the 10 months leading up to September 2019. Fifty-four patients (pilot-based estimation) were divided into two groups using a random-numbers table. The intervention group received foot massage every 6 hours (10 minutes per foot) for two days starting 4 hours after discharge. Patients’ demographic information was recorded and pain intensity was measured with a visual analog scale. Chi-square tests, Fisher’s exact test, and an independent t test were used for data analysis on SPSS 21. P < 0.05 was considered significant.
ResultsThe control (51.29 ± 9.45) and the intervention (52.03 ± 8.96) groups were not different in pain intensity at baseline. However, after the intervention, a statistically significant difference was observed in pain intensity between the intervention and control groups (20.12 ± 6.75 vs 49.88 ± 8.95, respectively, P = 0.003). Also, while there was no difference in pethidine consumption between the intervention (29.99 ± 4.49) and control (29.18 ± 4.50) groups before the intervention, the intervention group experienced a significant reduction in the medicine usage compared with the controls (10.41 ± 4.56 vs 32.12 ± 5.01, P = 0.001).
ConclusionFoot reflexology massage reduced the severity of pain and the need for opioids in women who are candidates for mastectomy following radiation therapy.

Hair loss (alopecia) is a common side effect of chemotherapy drugs, imposing further psychological stress on the patient. Scalp cooling is a common method used to prevent chemotherapy-induced alopecia; however, the results are not conclusive. The present study aimed to investigate the effectiveness of using a cold cap during chemotherapy in preventing alopecia in women with breast cancer.
MethodsIn this experimental study (from December 2018 to March 2019), 100 women with breast cancer were recruited via convenience sampling and randomized into two equal groups of intervention (chemotherapy while wearing a cold cap) and control (chemotherapy without using a cold cap). Data were collected using a questionnaire including items regarding demographic characteristics and chemotherapy side effects.
ResultsThe mean alopecia score (as rated by the patient on a scale from 1 to 4) was significantly lower in the intervention group compared with the control group (2.25 ± 0.49 vs 2.49 ± 0.34, p = 0.005).
Conclusion. Using a cold cap while receiving chemotherapy for breast cancer is effective in preventing hair loss.Keywords: Scalp cooling, alopecia, breast cancer, chemotherapy -

Cancer can negatively affect couples'’ intimate relationships, creating a need for psychological interventions. The purpose of this study was to determine the effectiveness of a group intervention based on acceptance and commitment therapy on quality of life and communication patterns in patients with breast cancer and their spouses.
MethodsThe study was a randomized controlled trial. The participating couples were randomly assigned to either the experimental or control groups. Each group consisted of 10 individuals (5 couples). Both groups completed quality of life questionnaires of cancer patients. the world health organization Quality of Life questionnaire and the Gottman distance and isolation Scale. Then, the experimental group participated in Acceptance and Commitment therapy–based intervention for eight sessions of two hours. Quality of life and communication patterns were evaluated immediately and 3 months after the intervention, Data were analyzed using independent t- tests and analysis of covariance.
ResultsThe mean scores for communication pattern and quality of life for the experimental group (10.40 ± 4.50 and 54.46 ± 11.17, respectively) were significantly different compared with the control group (15/.30 ± 3/.40 and 45/.34 ± 11/.34, respectively) (both P < 0.001).
ConclusionBased on the findings of the present study, acceptance and commitment therapy can improve quality of life and communication patterns of patients with cancer and their spouses and reduce couples'’ stress and communication challenges.

Given the high comorbidity of mental conditions and chronic pain in cancer, there is a need for development and administration of interventions for this population. This study aimed to investigate the effectiveness of mindfulness-integrated cognitive-behavioral therapy (MiCBT) in breast cancer pain management.
MethodsIn this clinical trial, 24 patients with breast cancer from Omid Hospital in Mashhad were selected via a purposive sampling method and randomly assigned to two groups of intervention (N = 12) and control (N = 12). The participants were assessed using the Short Cancer Pain Scale, Pain Self-Efficacy Questionnaire, and Pain Catastrophizing Scale before, immediately after, and one month after the intervention. The participants in the intervention group received MiCBT for 8 weeks. The control group did not receive any intervention from the research team during the research period. Descriptive statistics (percentage, frequency, mean and standard deviation), chi-square tests, t-tests, and multivariate analysis of covariance were used for data analysis.
ResultsAfter controlling for the pretest scores, there were significant differences between control and intervention groups in perceived pain, pain catastrophizing, and pain self-efficacy immediately and one month after the intervention (P > 0.001). The perceived pain, pain catastrophizing decreased and pain self-efficacy increased immediately and one month after the intervention (P > 0.001). Eta square showed 0.85,0.78 and 0.70 change in perceived pain, pain catastrophizing, and pain self-efficacy immediately and one month. In the follow-up phase, Eta squre were 0.87,0.75 and 0.79 for perceived pain, pain catastrophizing, and pain self-efficacy respectively.
ConclusionMiCBT is effective in improving pain, pain self-efficacy, and pain catastrophe in patients with breast cancer. Therefore, considering the cost-effectiveness and availability, this program is recommended as a complementary treatment for breast cancer patients.

Regular exercise training inhibits tumor growth and prevents cardiomyopathy in breast cancer patients by activating signaling pathways leading to changes in gene expression. The purpose of the present study was to investigate the effects of 10 weeks of continuous aerobic training on tumor volume and expression of IGF-1, Akt, mTORC1 genes in the myocardium of mice with breast cancer.
MethodsTwelve female BALB/c mice were divided into 2 groups of 6: Continuous Aerobic Exercise and Control. The tumor was induced by the injection of MC4-L2 cells. The aerobic exercise included 75 minutes of steady running at 60% to 65% of VO2 peak, 5 days a week for 10 weeks. Tumor volume was measured by a caliper weekly. Twenty-four hours after the last training session, the mice were killed and their left ventricles were harvested. IGF-1 protein levels were measured with western blotting analysis, and real-time PCR was used to determine the expression of Akt and mTORC1 genes. The comparison of the groups was made using t tests at a significance level of 5%.
ResultsIGF-1 did not show significant changes in the Aerobic Exercise group compared with the Control group (P = 0.08). The expression of Akt was significantly higher in the Aerobic Exercise group than in the controls (P = 0.016). The Aerobic Exercise group had also a significantly higher mTORC1 gene expression compared with the Control group (P = 0.022). Tumor volume also showed a significant decrease in the Aerobic Exercise group compared with the Control group (P = 0.03).
ConclusionIt can be concluded that 10 weeks of aerobic exercise training may reduce tumor volume and, possibly, improve heart function in breast cancer patients by increasing the expression of genes.

In patients with breast cancer, low serum brain-derived neurotrophic factor (BDNF) levels and high serum tumor necrosis factor (TNF)-α level have been reported. Therefore, the present study aimed to investigate the effect of eight weeks of high-intensity circuit training on serum levels of BDNF and TNFα and aerobic power in breast cancer survivors.
MethodsThis randomized controlled trial was conducted at Shahid Rahimi Hospital Oncology Center in Khorramabad in 2018. Seventeen breast cancer survivors were selected from a group of 55 women who had undergone chemotherapy and radiation therapy and randomly divided into an exercise (n = 9) or a control (n = 8) group. The exercise group performed high-intensity circuit training (HICT) three sessions per week for eight weeks. Data were analyzed using a paired-samples t test and an analysis of covariance.
ResultsThe results of the t test showed that after eight weeks, serum levels of BDNF and TNF-α were significantly increased in both groups (p ˂ 0.05), although the difference between the groups was not significant. Aerobic power significantly increased only in the exercise group (p = 0.0005). In addition, the results of the analysis of covariance showed only a significant difference in aerobic power between the two groups (p ˂ 0.05).
ConclusionEight weeks of HICT training in breast cancer survivors can increase aerobic capacity without affecting serum levels of BDNF and TNFα.
