این مطالعه، به منظور مقایسهی اثرات همودینامیک دکسمدتومیدین و میدازولام در بیماران جراحی اعصاب که همراه با تعبیهی نگهدارندهی سر حین جراحی بودند، انجام پذیرفت.
روشها:
در این مطالعهی کارآزمایی بالینی تصادفی شده، بیماران کاندیدای جراحی اعصاب به دو گروه دریافت کنندهی دکسمدتومیدین و یا دریافت کنندهی میدازولام تقسیم شدند و پس از القای بیهوشی، یک گروه دکسمدتومیدین و گروه دیگر میدازولام دریافت نمود. 10 دقیقه پس از القای بیهوشی، به جراح اجازهی قرار دادن نگهدارندهی سر داده شد. متغیرهای همودینامیک پیش از عمل و حین عمل ثبت شدند.
یافتههابررسی 68 بیمار در این مطالعه نشان داد که بین دو گروه دریافت کنندهی دارو از لحاظ فشار خون سیستول (001/0 > P)، دیاستول (002/0 = P)، متوسط فشار خون (045/0 = P) و تعداد ضربان قلب (003/0 = P) اختلاف معنیداری وجود داشت. فشار خون سیستول، دیاستول و متوسط فشار خون در گروه میدازولام، از گروه دکسمدتومیدین به صورت معنیداری در زمانهای 1، 2 و 3 دقیقه بعد از گذاشتن نگهدارندهی سر بیشتر بود. تعداد ضربان قلب نیز از قبل از گذاشتن نگهدارندهی سر تا 3 دقیقه بعد از گذاشتن آن در گروه میدازولام از گروه دکسمدتومیدین به صورت معنیداری بیشتر بود (050/0 > P).
نتیجهگیریدر مقایسه با میدازولام، دکسمدتومیدین در ایجاد پایداری متغیرهای همودینامیک طی جراحی کرانیوتومی با نیاز به استفاده از نگهدارندهی سر اثر چشمگیرتری را نشان داده و نیاز به داروهای کنترل همودینامیک به دنبال دکسمدتومیدین کمتر بوده است.

این مطالعه، با هدف تعیین ارتباط بین سازههای مدل ارتقای سلامت Pender با به کارگیری رویکردهای تغذیهای جهت کنترل فشار خون بالا در بیماران مبتلا به پرفشاری خون در مناطق روستایی شهرستان اصفهان انجام گرفت.
روشها:
در یک مطالعهی مقطعی، 233 بیمار مبتلا به فشار خون بالا از سطح مراکز بهداشتی و درمانی روستایی شهرستان اصفهان انتخاب شدند و دریافتهای غذایی آنان با پرسشنامهی ثبت 3 روزهی غذایی و عوامل شناختی و تعهد به رویکرد تغذیهای آنان با مدل ارتقای سلامت Pender بررسی شد و ارتباط بین دریافتهای غذایی و رویکرد تغذیهای آنان بررسی گردید.
یافتهها:
بین سدیم دریافتی و آگاهی ارتباط معکوس و معنیداری به میزان 316/0 وجود داشت (001/0 > P). همچنین، بین نمرهی فواید درک شده و میزان سدیم دریافت ارتباط معکوس و معنیداری به میزان 211/0 مشاهده شد (002/0 = P)، اما بین سایر معیارها (خودکارآمدی درک شده، احساس مرتبط با رفتار، تاثیر گذارندههای وضعیتی و حمایت اجتماعی) با میزان سدیم دریافتی ارتباط معنیداری دیده نشد.
نتیجهگیری:
برابر یافتههای این مطالعه، بین مدل ارتقای سلامت Pender و رویکردهای تغذیهای جهت کنترل فشار خون ارتباط معنیداری وجود داشت. از این رو، توصیه میشود مولفههای ارتقای سلامت Pender در واحدهای بهداشتی که با بیماران مبتلا به پرفشاری خون در ارتباط هستند، به کار گرفته شود.

مسمومیت کبدی، از عوارض جدی داروهای ضد سل میباشد که تا کنون روش ایدهآل و واحدی برای پیشگیری از آن ارایه نشده است. این مطالعه، با هدف مقایسهی تاثیر قرص Livercare (ترکیب گیاهان خارمریم، قاصدک، زرشک، زردچوبه و کنگر فرنگی) در پیشگیری از سمیت کبدی ناشی از داروهای ضد سل انجام شد.
روشها:
در یک مطالعهی کارآزمایی بالینی تصادفی و کنترل شده، 44 بیمار مبتلا به سل ریوی در دو گروه 22 نفره توزیع شدند. در گروه اول، گروه مداخله قرص Livercare ساخت شرکت HealthAid، دو بار در روز همزمان با شروع داروهای ضد سل به مدت 2 هفته تجویز شد و در گروه شاهد، تنها درمان استاندارد ضد سل به همراه دارونما تجویز شد و بروز مسمومیت کبدی در دو گروه تعیین و مقایسه شد.
یافتهها:
بررسی سطح آنزیمهای کبدی در طی دو هفته از شروع درمان ضد سل نشان داد که 7 نفر (9/15 درصد) از این بیماران دچار مسمومیت کبدی شدند که 6 نفر (3/27 درصد) از گروه شاهد و 1 نفر (5/4 درصد) از گروه مداخله بودند و بروز مسمومیت کبدی در گروه مداخله به طور معنیداری کمتر بود (039/0 = P).
نتیجهگیری:
به نظر میرسد، مصرف همزمان داروی گیاهی Livercare بتواند سمیت کبدی ناشی از داروهای ضد سل را به طور قابل ملاحظهای کاهش دهد.

This study aimed to compare the hemodynamic effects of dexmedetomidine and midazolam in neurosurgery candidates who underwent skull-pin head-holder insertion.
MethodsIn this randomized clinical trial study, 68 patients candidate for neurosurgery were divided into two groups receiving dexmedetomidine or midazolam. After induction of anesthesia, the patients received dexmedetomidine or midazolam based on the group they allocated to. Ten minutes after induction of anesthesia, the surgeon was allowed to insert the skull-pin head-holder. Hemodynamic variables were recorded before and during the surgery.
FindingsAmong 68 studied patients, there was significant difference between the groups regarding systolic blood pressure (P < 0.001), diastolic blood pressure (P = 0.002), mean blood pressure (P = 0.450), and heart rate (P = 0.003). Systolic, diastolic, and mean blood pressure in the midazolam group was significantly higher at first, second, and third minute after the insertion of the skull-pin head-holder in comparison with the dexmedetomidine group. Moreover, the heart rate was significantly higher in the midazolam group before inserting the skull-pin head-holder up to 3 minutes after the insertion in comparison with the dexmedetomidine group (P < 0.050).
ConclusionCompared with midazolam, dexmedetomidine showed a more significant effect on stabilizing hemodynamic parameters during craniotomies that required skull-pin head-holder insertion. Besides, the need for hemodynamic control drugs was lower after the administration of dexmedetomidine

This study aimed to investigate the relationship between Pender's Health Promotion Model constructs and applying nutritional supplements in patients with hypertension in rural areas of Isfahan Township, Iran.
MethodsIn a cross-sectional study, 233 patients with high blood pressure were selected from rural health centers in Isfahan Township. Patient’s dietary intake was assessed by a 3-day dietary questionnaire, and cognitive factors and a commitment to nutritional approach with Pender Health Promotion Model. The relationship between dietary intake and nutritional approach was studied as well.
FindingsThere was an inverse and significant relationship of 0.316 between sodium intake and knowledge (P < 0.001). Moreover, there was a significant negative correlation of 0.121 between the benefit score and the sodium intake (P = 0.002), but no relationship between other criteria (perceived self-efficacy, behavioral sensation, status affecting, and social support) and sodium intake.
ConclusionThe findings of this study showed that there was a significant relationship between Pender Health Promotion Model and nutritional approaches for controlling blood pressure. Therefore, it is recommended to use Pender Health Promotion Components in health units associated with hypertension.

Hepatotoxicity is a serious complication of anti-tuberculosis drugs, and so far, no ideal preventive measure has been suggested for it.. The aim of this study was determining the effects of Livercare tablet [combination of milk thistle, dandelion, barberry, turmeric (curcumin), and artichoke] in the prevention of hepatotoxicity induced by anti-tuberculosis drugs.
MethodsIn a randomized controlled clinical trial study, 44 patients with pulmonary tuberculosis were divided into two groups of 22 patients. In the first group (intervention group), Livercare tablets made by HealthAid Company were prescribed twice a day for 2 weeks at the same time with the start of anti-tuberculosis drugs. In the second group (control group), only standard anti-tuberculosis treatment was prescribed. The incidence of hepatotoxicity was determined and compared between the two groups.
FindingsExamination of liver enzyme levels during two weeks from the start of anti-tuberculosis treatment showed that 7 patients (15.9%) had hepatotoxicity, of which 6 (27.3%) were in the control group and 1 (4.5%) in the intervention group. The incidence of hepatotoxicity was significantly lower in the intervention group (P = 0.039).
ConclusionConcomitant use of the herbal drug Livecare seems to reduce the hepatotoxicity of anti-tuberculosis drugs significantly.
