تجارب معنوی افراد میتواند نقش برجستهای در سلامت آنها داشته باشد. هدف از انجام پژوهش حاضر، بررسی ارتباط تجارب معنوی با امید به زندگی و اضطراب مرگ در بیماران مبتلا به سوختگی بود.
روشها:
در این مطالعهی توصیفی- تحلیلی- مقطعی، بیماران مبتلا به سوختگی با سن بیشتر از 16 سال و کمتر از 80 سال مراجعهکننده به بیمارستان ولایت شهر رشت که دچار سوختگی بیش از 20 درصد از کل بدن و یا سوختگی بیش از 10 درصد در نواحی سر و صورت شده بودند، به روش نمونهگیری در دسترس انتخاب شدند. ابزار جمعآوری دادهها، چهار پرسشنامه شامل مشخصات دموگرافیک، پرسشنامهی امید به زندگی Snyder، مقیاس اضطراب مرگ (Death Anxiety Scale یا DAS) Templer و مقیاس تجارب معنوی روزانه (Daily Spiritual Experience Scale یا DSES) بود. دادهها به صورت مصاحبهی حضوری محقق با بیماران دچار سوختگی جمعآوری شد.
یافتهها:
بین تجارب معنوی و امید به زندگی، ارتباط معنیدار و مثبتی مشاهده شد؛ به نحوی که بیماران با تجارب معنوی قویتر، امید به زندگی بالاتری را نشان دادند. همچنین، بین وسعت سوختگی و اضطراب مرگ، رابطهی معکوس معنیداری وجود داشت. با کنترل متغیرهای دموگرافیک، ارتباط بین تجارب معنوی با اضطراب مرگ و امید به زندگی معنیدار بود.
نتیجهگیری:
نتایج به دست آمده ارتباط مثبت و معنیدار بین امید به زندگی و تجارب معنوی روزانهی بیماران را نشان داد. به نظر میرسد برای تقویت تاثیرات مثبت امید به زندگی در برنامههای مراقبت از بیماران مبتلا به سوختگی، مواردی همچون حمایت، تسهیل و توجه به تمایلات و نیازهای معنوی بیماران باید مورد توجه قرار گیرد

با توجه به اهمیت کمپلکس غضروفی فیبری سهگوش (Triangular fibrocartilage complex یا TFCC) در کیفیت زندگی بیماران، پژوهش حاضر با هدف بررسی نتایج درمان عمل جراحی باز و مقایسهی آن با درمان حمایتی در ضایعهی تروماتیک TFCC انجام شد.
روشها:
در این مطالعهی کارآزمایی بالینی، 32 بیمار مبتلا به ضایعهی تروماتیک TFCC درجهی دو انتخاب و به طور تصادفی در دو گروه مداخله و شاهد تقسیم شدند. گروه مداخله تحت عمل جراحی باز و گروه شاهد تحت درمان حمایتی [دریافت داروهای ضد التهاب غیر استروییدی (Non-steroidal anti-inflammatory drugs یا NSAID)، فیزیوتراپی و حرکات اصلاحی] قرار گرفتند. بیماران به مدت سه ماه پیگیری شدند و میزان بهبودی و کیفیت زندگی آنان با استفاده از Mayo Score قبل و سه ماه پس از عمل جراحی مورد بررسی و مقایسه قرار گرفت.
یافتهها:
میانگین نمرهی کیفیت زندگی قبل از درمان در دو گروه تحت عمل جراحی و درمان دارویی به ترتیب 8/7 ± 0/30 و 7/10 ± 4/29 بود و اختلاف معنیداری بین دو گروه مشاهده نشد (560/0 = P). میانگین Mayo Score پس از درمان در دو گروه به ترتیب 4/6 ± 2/68 و 3/10 ± 2/42 گزارش گردید و بیماران تحت عمل جراحی سه ماه بعد از عمل، از کیفیت زندگی بالاتری برخوردار بودند (001/0 > P).
نتیجهگیری:
عمل جراحی باز در بیماران دچار ضایعهی تروماتیک TFCC، با بهبودی مطلوبتری نسبت به درمان دارویی همراه است

این مطالعه با هدف بررسی تاثیر تمرین هوازی بر شاخصهای بیوشیمیایی ویژهی سلولهای قلبی در زنان مبتلا به دیابت نوع 2 بود.
روشها:
این مطالعه به صورت کارآزمایی بالینی روی 30 زن مبتلا به دیابت انجام گرفت. آزمودنیها به طور تصادفی به گروه مورد و شاهد تقسیم شدند. گروه مورد 8 هفته، 3 جلسه در هفته [75-55 درصد بیشینهی ضربان قلب و Rating perceived exertion (RPE) 13-12] در تمرینات هوازی فزاینده (دویدن) شرکت کردند. قبل و بعد از اجرای شیوهنامه، خونگیری انجام شد. روش اندازهگیری متغیرها، Enzyme-linked immunosorbent assay (ELISA) و آنزیماتیک بود.
یافتههاتروپونین I، 6 ساعت بعد از آخرین جلسهی تمرین در گروه مورد نسبت به 6 ساعت بعد از آخرین جلسهی تمرین در گروه شاهد کاهش معنیداری داشت (001/0 = P). تروپونین I، 6 ساعت بعد از اولین جلسهی تمرین در گروه مورد نسبت به 6 ساعت بعد از اولین جلسهی تمرین در گروه شاهد تفاوت معنیداری نداشت (983/0 = P). مقادیر Creatine kinase-MB (CK-MB) در دو گروه به طور معنیداری سیر افزایشی داشت. این سیر افزایشی، در گروه مورد بیشتر از گروه شاهد بود (001/0 = P).
نتیجهگیری:
دستورالعمل تمرینی با وجود افزایش CK-MB که ممکن است ناشی از ماهیت تمرین و آسیب عضله در حین تمرین باشد، تروپونین I به طور معنیداری کاهش یافت. این امر، خود میتواند موید خاصیت حفاظتی این شیوهنامهی تمرینی از بافت قلب در برابر آسیب در مبتلایان به دیابت باشد.

Past studies have shown that people's spiritual experiences can play a prominent role in their health. The aim of this study was to investigate the relationship between spiritual experiences with life expectancy and death anxiety in burned patients.
MethodsIn a cross-sectional study, burned patients aged 16-80 years referred to Velayat hospital in Rasht, Iran, who suffered burns more than 20% of whole body surface or/and burns more than 10% of head and face surface were included using available sampling method. Data collection tools were 4 questionnaires including demographic characteristics, Snyder's Hope Life Questionnaire, Templer Death Anxiety Scale (DAS) and Daily Spiritual Experiences Scale (DSES). Data were collected through face-to-face interviews with burn patients.
FindingsThere was a statistically significant positive relationship between spiritual experiences and life expectancy; so that patients with stronger spiritual experiences had higher life expectancy. There was also a significant inverse relationship between burn extent and death anxiety. By controlling demographic variables, a statistically significant relationship was observed between spiritual experiences with death anxiety and life expectancy.
ConclusionThe results of this study showed a positive and significant relationship between life expectancy and patients' daily spiritual experiences. It seems that in order to enhance the positive effects of life expectancy in care programs for burned patients, things such as support, facilitation, and attention to the spiritual desires and needs of patients should be considered.

Notice to importance of traumatic triangular fibrocartilage complex (TFCC), this study aimed to compare the effect of open surgery and supportive medical treatment on the improvement of trauma and quality of life in patients with TFCC.
MethodsIn a clinical trial study, 32 patients with triangular fibrocartilage trauma were selected and randomly divided into two groups of intervention and control. The intervention group underwent open surgery and the control group were receiving supportive treatments as non-steroidal anti-inflammatory drugs (NSAIDS), physiotherapy, and corrective action. All patients were followed for 3 months after intervention, and quality of life and improvement was evaluated using Mayo score, and compared between the two groups.
FindingsThe mean Mayo score before intervention was 30.0 ± 7.8 and 29.4 ± 10.7 in open surgery and supportive treatment groups, respectively, and no statistically difference was seen between the two groups (P = 0.560). The mean Mayo score after intervention was 68.2 ± 6.4 and 42.2±10.3 in surgery and supportive treatment groups, respectively, and the difference between groups was statistically significant (P < 0.001).
ConclusionOur study showed that open surgery in patients with TFCC is better than the conservative treatment

The aim of this study was to investigate the effect of aerobic exercise on heart cells specific biochemical indicators in women with type 2 diabetes mellitus.
MethodsThis randomized clinical trial study was performed on 30 women with type 2 diabetes mellitus randomly divided into experimental and control groups. The experimental group participated in an increasing aerobic exercise (running) for eight weeks, three sessions per week [55% to 75% of maximum heart rate and rating perceived exertion (RPE) of 12 to 13]. Blood samples were taken before and after the protocol. The enzyme-linked immunosorbent assay (ELISA) and enzymatic method were used to measure the variables.
FindingsTroponin I decreased significantly 6 hours after the last training session in the experimental group compared to 6 hours after the last training session in the control group (P = 0.001). There was no significant difference in troponin I, 6 hours after the first training session in the experimental group compared to 6 hours after the first training session in the control group (P = 0.983). Creatine kinase-MB (CK-MB) values significantly increased in both groups; this increase was higher in the experimental group than the control group (P = 0.001).
ConclusionThe results showed that the training instructions significantly decreased troponin I despite the increase in CPK-MB, which may be due to the nature of training and muscle damage during training. This can confirm the protective nature of this exercise protocol for heart tissue against the injury in women with type 2 diabetes mellitus.
