پرفشاری خون از دلایل اصلی مرگ و میر در جهان است. هدف از انجام مطالعهی حاضر، بررسی سطح سرمی ویتامین D و ارتباط آن با شاخصهای ابتلا به سندرم متابولیک و مراقبت و پایش 24 ساعتهی فشارخون بود.
روشها:
مطالعهی حاضر، مطالعهی مقطعی بر روی 171 (79 مرد) بیمار پرفشاری خون اولیه با میانگین سنی 88/10 ± 70/50 سال که در طی 6 ماه قبل از مطالعه از مکمل ویتامین D استفاده نمیکردند، بود. افراد به دو گروه با کمبود (≤ 30 نانوگرم/میلیلیتر) و سطح طبیعی ویتامین D (> 30 نانوگرم/میلیلیتر) تقسیم و از نظر شاخصهای ابتلا به سندرم متابولیک و متغیرهای مراقبت و پایش 24 ساعتهی فشار خون مقایسه شدند. پرفشاری خون بر اساس معیارهای Eighth Joint National Committee (JNC8) تعریف و با استفاده از مراقبت و پایش 24 ساعتهی فشار خون اندازهگیری شد. متغیرهایمراقبت و پایش 24 ساعتهی فشار خون به صورت فشار خون 24 ساعته (میانگین در طول 24 ساعت)، روزانه (ساعت 23-6) و شبانه (ساعت 23-6) تعریف گردید. سطح سرمی ویتامین D به روش الکتروکمی لومینسانس اندازهگیری شد.
یافتهها:
میانگین سرمی ویتامین D، 70/4 ± 43/30 نانوگرم/میلیلیتر بود و 5/65 درصد کمبود ویتامین D داشتند. سطح سرمی ویتامین D، با فشار خون سیستول اندازهگیری شده در بدو مراجعه به مرکز (001/0 = P)، 24 ساعته، روزانه و شبانه رابطهی معکوس داشت (001/0 > P). کمبود ویتامین D ارتباط معنیداری با تریگلیسیرید (001/0 > P) و کلسترول (016/0 = P) بالاتر داشت.
نتیجهگیری:
کمبود ویتامین D با فشار خون سیستول بالاتر، فشارخون دیاستول شبانه و پروفایل لیپیدی ارتباط دارد

انجام جراحی زودهنگام به منظور برداشتن بافتهای مرده همراه با بستن زودهنگام زخمهای سوختگی، اکنون به عنوان یکی از مهمترین عوامل دخیل در زنده ماندن بیماران با صدمات سوختگی بزرگ شناخته شده است. پژوهش حاضر با هدف مقایسهی کارایی و ایمنی پوشش موقت زخم سوختگی با اجیکت در مقایسه با آلوگرافت انجام شد.
روشها:
در این مطالعهی کارآزمایی بالینی، بیماران بستری در ردههای سنی 18 تا 45 سال با سوختگی 30-20 درصد و درجهی 2 عمیق و 3، در دو گروه شاهد (پانسمان با اجیکت) و آزمون (پانسمان با کاداور) قرار گرفتند. روی زخمها با اجیکت یا کاداور پوشانده شد و بیماران در طی درمان از نظر بروز علایم عفونت مورد بررسی قرار گرفتند و میزان درد آنها نیز اندازهگیری گردید. همچنین، کنترل نیاز به دبریدمان مجدد به عهدهی جراح بود که در چکلیست ثبت میشد.
یافتهها:
در مجموع، 50 بیمار مبتلا به سوختگی با معیارهای ورود وارد تحقیق شدند. دبریدمان در گروه کاداور به طور معنیداری کمتر از گروه اجیکت بود (003/0 = P). میانگین نمرهی درد در روزهای اول تا ششم و همچنین، روز نهم عمل در گروه کاداور به طور معنیداری کمتر از گروه اجیکت گزارش شد )050/0 < P). مقدار مورفین تزریق شده طی 24 ساعت درگروه کاداور به طور معنیداری کمتر از گروه اجیکت بود (050/0 < P).
نتیجهگیری:
با توجه به نتایج به دست آمده، میتوان گفت که استفاده از کاداور در مقایسه با اجیکت، نیاز به دبریدمان و مصرف مورفین را کاهش میدهد.

داشتن فرزند، قویترین خواسته در تمام جهان است و عدم تحقق آن، میتواند با استرس، کاهش ارتباط بین فردی و اجتماعی، عدم رضایت جنسی و در نهایت گسستگی زندگی زناشویی همراه باشد. مطالعهی حاضر، با هدف بررسی میزان رضایت جنسی زوجین نابارور با علت ناباروری زنانه و عوامل مرتبط با آن، انجام گرفت.
روشها:
این مطالعهی توصیفی مقطعی، بر روی 103 زوج نابارور (206 نفر) مراجعه کننده به مرکز ناباروری تهران انجام شد. ابزار گردآوری دادهها، پرسشنامهی اطلاعات دموگرافیک و پرسشنامهی رضایت جنسی Larson بود.
یافتهها:
میانگین نمرهی کلی پرسشنامهی رضایت جنسی زنان نابارور 00/75 (رضایت جنسی کم) و همسرانشان 92/79 (رضایت جنسی متوسط) بود. زنان نابارور به طور معنیداری نسبت به همسرانشان از رضایت جنسی کمتری برخوردار بودند (001/0 > P). رضایت جنسی زنان با میزان تحصیلات آنان، سن اولین مراجعه به مرکز ناباروری و سابقهی ناباروری در خانواده و رضایت جنسی همسران زنان نابارور با طول مدت ناباروری و سن ازدواج زنان، ارتباط آماری معنیداری داشت (001/0 > P).
نتیجهگیری:
مطالعهی حاضر نشان داد، زنان نابارور از رضایت جنسی کم و همسران آنان از رضایت جنسی متوسط برخوردار بودند و رضایت جنسی زنان نابارور، به طور معنیداری از همسرانشان کمتر بود؛ این موضوع، توجه ویژه به رضایت جنسی زوجین نابارور را میطلبد.

Hypertension is one of the leading causes of mortality in the world. The aim of this study was to evaluate the relationship of plasma 25-hydroxyvitamin D levels with markers of metabolic syndrome and ambulatory blood pressure monitoring.
MethodsThis was a cross-sectional study on 171 adults (79 men) with primary hypertension and mean age of 50.70 ± 10.88 years, who did not take vitamin D supplement for the past 6 months. Subjects were divided into two groups as deficient (30 ng/ml ≥) and normal levels of vitamin D (30 ng/ml <) and compared for metabolic syndrome indices and 24-hour blood pressure monitoring parameters. Hypertension was defined based on Eighth Joint National Committee (JNC8) scale, and measured by an ambulatory blood pressure monitoring device. Ambulatory blood pressure monitoring parameters were defined as 24-hour blood pressure (mean during 24 hours), daytime (6-23 hours), and nighttime (23-6 hours) measurements.
FindingThe mean vitamin D level was 30.43 ± 4.70 ng/ml, and 65.5% of patients had vitamin D deficiency. Serum vitamin D levels had an inverse and significant relationship with office systolic blood pressure (P = 0.001), and 24-hour, daytime, and nighttime systolic blood pressures (P < 0.001). Vitamin D deficiency was significantly associated with higher levels of triglycerides (P < 0.001) and cholesterol (P = 0.016).
ConclusionVitamin D deficiency was associated with higher systolic blood pressure, nighttime diastolic blood pressure, and impaired serum lipid profiles.

Early surgery to remove dead tissue with early closure of burn wounds has now been identified as one of the most important factors contributing to the survival of patients with major burn injuries. This study, the efficiency and safety of temporary cover wound dressing with agicoat was compared with allograft (cadaver) in burned patients.
MethodsIn this randomized clinical trial study, hospitalized patients in the age group of 18-45 years with burning of 20-30 percent and grade 2 deep and 3, were divided into two groups of control and test. Treatment was carried out in the operating room after debridement, covered with wounds or cadaver wounds, and patients were also treated for signs of infection such as warmth, discoloration, redness of the fluid, and fever. Patients were evaluated in two groups, and the surgeon checked the need for re-debridement, and recorded it in the checklist.
FindingsA total of 50 burn patients met the inclusion criteria. Debridement in cadaver group was significantly lower than that of Agicoat group (P = 0.003). Mean pain score during the first to sixth days of surgery, and also on the ninth day of surgery was significantly lower in cadaver group (P < 0.050). The mean daily morphine injected over 24 hours was significantly lower in the cadaver group as well (P < 0.050).
ConclusionAccording to the findings of this study and other similar studies, it can be concluded that the use of cadaver reduces the need for debridement and morphine consumption in comparison with Agicoat.

Having a child is the strongest desire in the world, and failure in it can be associated with stress, reduced interpersonal and social relationships, sexual dissatisfaction, and ultimately the breakdown of married life. The aim of this study was to evaluate sexual satisfaction and related factors in infertile couples with female infertility.
MethodsThis descriptive cross-sectional study was performed on 103 infertile couples (206 patients) referred to the infertility center of Tehran, Iran. The Data were collected by two questionnaires including demographic questionnaires and Larsons Sexual Satisfaction Questionnaire.
FindingsThe total score of Sexual Satisfaction Questionnaire was 75.00 (low sexual satisfaction) for infertile women, and 79.92 (moderate sexual satisfaction) for their husbands. Infertile women had significantly less sexual satisfaction than their husbands (P < 0.001). Women's sexual satisfaction had a statistically significant relationship with their education level, age of referral to infertility center, and infertility history (P < 0.001); and sexual satisfaction of infertile women's husbands had a statistically significant relationship with duration of infertility and age of marriage of women (P < 0.001).
ConclusionThe present study showed that infertile women had low sexual satisfaction, and their husbands had moderate sexual satisfaction. The sexual satisfaction of infertile women was significantly lower than their husbands, which requires special attention.
