بیماری آلزایمر یک بیماری مزمن تخریبی مغز است که با انباشت پروتئین غیرطبیعی، اختالل عملکرد سیناپسی و اختالل شناختی شناخته میشود. آگونیستهای گیرنده فعالکننده پرولیفراسیون پروکسیزوم آلفا نقش مهمی در تنظیم حساسیت به انسولین دارند و ممکن است به عنوان اهداف بالقوه درمانی برای بیماری آلزایمر استفاده شوند. پیوگلیتازون به عنوان آگونیست این گیرنده ها باعث کاهش آمیلوییدهای بتا و پروتئینهای تایو و مهار التهاب نورونی میشود. هدف در این مطالعه اثرات پیوگلیتازون بر یادگیری و حافظه موشهای آلزایمری مدل استرپتوزوتوسین مورد بررسی قرار گرفت.
مواد و روش هادر این مطالعه تجربی، 42 سر موش صحرایی نر به دو گروه سالم شامل شاهد، DMSO و پیوگلیتازون و آلزایمری شامل حامل، استرپتوزوتوسین و استرپتوزوتوسین به همراه DMSO) 2/0 میلیلیتر) و استرپتوزوتوسین به همراه پیوگلیتازون (10 میلیگرم بر کیلوگرم، 21 روز) تقسیم شدند. برای ایجاد آلزایمر، استرپتوزوتوسین (3 میلیگرم برکیلوگرم، 5 میکرولیتر در محل تزریق) به داخل بطنهای طرفی مغز تزریق شد. تمام موشها در ماز آبی موریس آموزش داده شدند.
یافته هاپیوگلیتازون، یادگیری و حافظه فضایی موشهای سالم را مختل کرد. نتایج نشان داد که تزریق داخل بطنی استرپتوزوتوسین باعث افزایش معنی دار تاخیر زمانی و فاصله شنا کردن برای پیدا کردن سکو در گروه آلزایمری در مقایسه با گروه شاهد میشود (05/0>P). از اثرات فراموشی استرپتوزوتوسین با تزریق پیوگلیتازون جلوگیری شد، به طوری که تاخیر زمانی و مسافت طیشده برای پیدا کردن سکو در گروه استرپتوزوتوسین+پیوگلیتازون (10 میلیگرم بر کیلوگرم) به طور معنی داری کمتر از گروه استرپتوزوتوسین به همراه DMSO بود (05/0>P)برعکس، درصد زمان سپری شده و مسافت طیشده در ربع هدف در تست پروب در گروه استرپتوزوتوسین به همراه پیوگلیتازون به طور معنی داری بیشتر از گروه استرپتوزوتوسین بود.
نتیجه گیرییافته ها نشان داد که پیوگلیتازون، یادگیری و حافظه موشهای نرمال را مختل میکند؛ در حالی که یادگیری و حافظه در موشهای آلزایمری مدل استرپتوزوتوسین را بهبود میبخشد. نتایج نشان میدهد که درمان با پیوگلیتازون برای درمان اختالالت شناختی در بیماران آلزایمری مفید است.

اثرات مفید فعالیت ورزشی و عصاره گیاه استویا بر بهبود شاخصهای مرتبط با دیابت گزارش شده است. با این حال مزایای قلبی فعالیت ورزشی و عصاره گیاه استویا در دیابت نوع 1 مشخص نیست. هدف هدف از این تحقیق، تاثیر هشت هفته تمرین استقامتی و مکمل استویا بر بیان ژن ANP و MHC-b بافت قلب در موشهای صحرایی دیابتی نوع 1 بود.
مواد و روش هادر این مطالعه تجربی، 25 سر موش صحرایی با میانگین وزن 250-300 گرم به پنج گروه کنترل سالم، کنترل دیابتی نوع 1 ،دیابتی نوع 1+استویا، دیابتی نوع 1 +تمرین استقامتی و دیابتی نوع 1 +تمرین استقامتی+استویا تقسیم شدند. موشهای صحرایی با استفاده از استرپتوزوتوسین به صورت تک دز 55 میلیگرم به ازای هر کیلوگرم وزن بدن و به صورت درونصفاقی دیابتی نوع 1 شدند. تمرین استقامتی، پنج روز در هفته با سرعت 20 تا 30 متر بر دقیقه و شیب صفر درجه به مدت هشت هفته اجرا شد. دز مصرفی استویا 250 میلیگرم به ازای هر کیلوگرم وزن بدن به صورت گاواژ بود. کشتار موشهای صحرایی 48 ساعت بعد از آخرین جلسه تمرین انجام شد. بافت قلب برای آنالیز شاخصها مورد استفاده قرار گرفت. بیان ژنهای ANP و MHC-b بافت قلب به روش PCR Time Real اندازه گیری شد. جهت تجزیه و تحلیل داده ها از آزمون آنالیز واریانس یکطرفه و آزمون تعقیبی بونفرونی استفاده شد.
یافته هانتایج نشان داد که بیان ژن ANP و MHC-b بافت قلب موشهای صحرایی کنترل دیابتی نسبت به گروه کنترل سالم به طور معنیداری بیشتر بود (001/0=P). همچنین بیان ژن ANP و MHC-b بافت قلب موشهای صحرایی در گروه دیابتی نوع 1+تمرین استقامتی و دیابتی نوع 1+تمرین استقامتی+استویا نسبت به گروه دیابتی کنترل به طور معنیداری کمتر بود (001/0=P).
نتیجه گیریبا توجه به نتایج، مداخله تمرین استقامتی و مکمل استویا میتواند مزایای قلبی برای موشهای دیابتی نوع 1 به همراه داشته باشد.

تشنج یک اختلال عملکرد مغزی ناشی از تخلیه الکتریکی غیرطبیعی نورونهاست که به صورت فعالیت الکتریکی پاتولوژیک در مغز بروز میکند. استرس اکسیداتیو در آسیب ناشی از تشنج در نورونهای مغز نقش دارد. هدف در این مطالعه اثرات ضدتشنج و آنتی اکسیدانت پیوگلیتازون، یک داروی مورد استفاده در دیابت نوع 2 ،در برابر تشنج ناشی از پنتیلن تترازول در موش سوری مورد بررسی قرار گرفت.
مواد و روش هادر این مطالعه تجربی 28 موش سوری نر بالغ با وزن بین 20 تا 30 گرم مورد مطالعه قرار گرفتند. موشها به چهار گروه کنترل، پیوگلیتازون، پنتیلن تترازول و درمان تقسیم شدند. پیوگلیتازون (80 میلیگرم بر کیلوگرم) به صورت خوراکی تجویز شد و چهار ساعت پس از آن نرمال سالین یا پنتیلن تترازول (85 میلیگرم بر کیلوگرم) به صورت داخلصفاقی تزریق شد. در گروه های کنترل و پنتیلن تترازول به جای پیوگلیتازون از کربوکسی متیل سلولز استفاده شد. در بازه زمانی یک ساعت بعد از تزریق پنتیلن تترازول شدت تشنج با استفاده از معیار راسین مورد سنجش قرار گرفت. سپس موشها کشته شدند و میزان مالون دیآلدهید و فعالیت آنزیم های کاتالاز و سوپراکسید دیسموتاز در نمونه های هیپوکامپ اندازه گیری شد.
یافته هاپیوگلیتازون باعث افزایش مدتزمان لازم برای شروع مراحل یک تا چهار تشنج و پیشگیری از مرحله پنج تشنج بر اساس معیار راسین شد. این دارو باعث کاهش میزان لیپید پراکسیداسیون ناشی از تشنج ایجادشده توسط پنتیلن تترازول و همچنین افزایش فعالیت آنزیمهای کاتالاز و سوپراکسید دیسموتاز در هیپوکامپ شد. در پیشگیری از تشنج پایدار ناشی از پنتیلن تترازول و نتیجه گیری نتایج این مطالعه نشان داد اثرات آنتی اکسیدانت پیوگلیتازون احتماال محافظت از نورونها در برابر آسیبهای ناشی از تشنج نقش دارد.

نقص یا سطوح ناکافی ویتامین D در بیماران کبدی بسیار شایع است و ارتباط نزدیکی با دردهای اسکلتی عضلانی از جمله کمردرد دارد. هدف پژوهش حاضر با هدف بررسی فعالیت ترانس آمینازهای سرمی در زنان مبتلا به کمردرد مزمن متعاقب یک دوره تمرینات ثبات دهنده مرکزی و مصرف ویتامین D انجام شد.
مواد و روش هااین مطالعه کارآزمایی بالینی روی زنان مبتال به کمردرد مزمن در سال 1398 انجام شد. 48 بیمار به روش نمونه گیری دردسترس انتخاب و به طور تصادفی به چهار گروه کنترل، تمرین، ویتامین D و ترکیبی (تمرین+ویتامین D) تقسیم شدند. گروه های تمرین و ترکیبی، هشت هفته تمرینات پایدارکننده کمر را با سطوح مختلف طبق برنامه ورزشی انجام دادند. گروه های ویتامینD و ترکیبی، 50 هزار واحد ویتامین D در هفته (به مدت هشت هفته)دریافت کردند. فعالیت آنزیمهای آلانین آمینوترانسفراز (ALT) ،آسپارتات آمینوترانسفراز (AST) و آلکالین فسفات (ALP) قبل و پس از مداخله ها تعیین شد. برای آنالیز داده ها از آزمونهای تی زوجی و آنالیز واریانس یکطرفه استفاده شد.
یافته هاهشت هفته تمرینات منتخب، مصرف ویتامین D و مداخله ترکیبی با کاهش آنزیمهای AST، ALT و ALP همراه بود. به عالوه مداخله ترکیبی به کاهش بیشتری در فعالیت AST و ALP در مقایسه با دو مداخله دیگر منجر شد و کاهش فعالیت ALT گروه ترکیبی در مقایسه با گروه تمرین بیشتر بود (05/0<P).
نتیجه گیریبه نظر میرسد تمرینات ورزشی، مصرف ویتامین D و مداخله ترکیبی ممکن است از طریق کاهش فعالیت آنزیمهای کبدی منجر به بهبود عملکرد کبدی در بیماران مبتال به کمردرد مزمن دارای سطوح پایین ویتامین D شوند و مداخله ترکیبی با اثربخشی بیشتری بر عملکرد کبدی همراه باشد.

اختلالات چرخه قاعدگی در دختران دانش آموز در سالهای آغازین چرخه قاعدگی، بر سلامت فیزیکی، عاطفی و کارکرد اجتماعی آنان تاثیر بسزایی دارد. هدف مطالعه حاضر با هدف بررسی شیوع اختلالات قاعدگی در بین دانش آموزان دبیرستانی شهر قزوین و ارتباط آن با کیفیت زندگی صورت گرفت.
مواد و روش هادر این مطالعه مقطعی هزار دانش آموز دختر دبیرستانی شهر قزوین در سال تحصیلی 1396-1397 به طور تصادفی وارد مطالعه شدند. افراد از نظر اختلالات سیکل قاعدگی و تاثیر آن بر کیفیت زندگی ارزیابی شدند. جمع آوری اطالعات از طریق چک لیست و پرسشنامه کیفیت زندگی صورت گرفت. اطلاعات با استفاده از آزمون تی مستقل آنالیز شدند.
یافته هامیانگین سنی افراد شرکت کننده 93/0±9/15 سال بود. شایعترین اختلال (60 درصد)، سندرم پیش از قاعدگی (PMS) بود. بین کیفیت زندگی دانش آموزان با ابتلا به PMS ،منوراژی، متروراژی، دیسمنوره، لکه بینی، هیپرمنوره (001/0> P) پلی منوره (005/0=P) هیپومنوره (002/0=P) و آمنوره (025/0=P) و دخترانی که به اختلالات قاعدگی مبتلا نبودند، ارتباط آماری معنی دار وجود داشت، به طوری که نمرات کیفیت زندگی دختران مبتلا به این اختلالات پایین تر از دخترانی بود که به اختلالات قاعدگی مبتال نبودند.
نتیجه گیرییافته های مطالعه حاضر نشان داد اختلالات قاعدگی در بین این دانش آموزان شیوع بالایی دارد و این اختلالات تاثیر قابل توجهی بر کیفیت زندگی آنها دارد. از آنجا که این تاثیر، پتانسیل داشتن پیامدهای بلندمدت تری را دارد، انجام تلاشهای بیشتر برای درک و رفع این مسایل ضروری به نظر میرسد.

عفونتهای بیمارستانی حدود یک سوم مرگومیرها را در بیمارستان به خود اختصاص داده است. هدف در این مطالعه سعی شدبا بررسی عوامل موثردر بروز عفونتهای بیمارستانی اطلاعاتی جامعتر به دست آورده وموجبات ارتقای سطوح پیشگیری ودرمانرا دربیمارستانهای منطقهای فراهم آوریم.
مواد و روش هاپژوهش حاضر از نوع مقطعی توصیفی بوده و با بررسی پرونده 86 بیمار که در سال 1396 درICU بیمارستان بوعلی بستری بودند انجام شد. اطلاعات جمعیت شناختی وب الینی (از قبیل نتایج کشت خون، ادرارو غیره) جمع آوری شدو نتایج مورد تجزیه و تحلیل قرار گرفتند.
یافته هامیزان بروز عفونت بیمارستانی در کل 3/24 درصدبرآورد شد که 4/38 درصد آسینتوباکتر، 6/18 درصد پسودومونا آیروژینوزا و مابقی از انواع دیگر میکروارگانیسمها گزارش شد. بیشترین مقاومت آنتیبیوتیکی نسبت به سفپیم)64 درصد(و سیپروفلوکساسین (60 درصد) بوده است.
نتیجه گیریمیزان بروز عفونت بیمارستانی در این بیمارستان اندکی بیشتر از مقادیرمطالعات مشابه دردیگر کشورها بوده است. بروز عفونتهای بیمارستانی با سویههای متفاوت و نیز الگوی متفاوت مقاومت آنتیبیوتیکی لزوم انجام مطالعات جامعتر و اقدامات پیشگیرانه در بروز عفونتهای بیمارستانی را خاطر نشان میکند.

دی اکسی نیوالنول از عمومی ترین مایکوتوکسینهایی است که در دانه های غلات یافت میشود. هدف این مطالعه با هدف تعیین تفاوت میزان سم قارچی دی اکسی نیوالنول در آرد خام و نان در نانواییهای شهرستان محمودآباد انجام شد.
مواد و روش هادر مطالعه تحلیلی مقطعی حاضر، 44 نمونه تصادفی آرد گندم و نان (لواش و بربری) جمع آوری و میزان سم به وسیله االایزا اندازه گیری شد. داده ها با روش آنووا و آزمون تی مورد ارزیابی و بررسی قرار گرفتند (05/0≥P).
یافته هامقدار کل دی اکسی نیوالنول در آرد و نان به ترتیب 04/0±03/0 و 21/0±12/0 میکروگرم بر کیلوگرم بود. مقدار سم موجود در آرد لواش و بربری به ترتیب 02/0±01/0 و 01/0±01/0 میکروگرم بر کیلوگرم بود. همچنین مقدار سم در نان لواش و بربری به ترتیب 03/0±04/0 و 27/0±16/0 میکروگرم بر کیلوگرم بود. مقایسه بین سطح متوسط دی اکسی نیوالنول در نمونه های آرد و نان از نظر آماری معنی دار نبود (05/0≥P).
نتیجه گیریوجود سم در نمونه های مورد مطالعه تایید شد؛ اما آلودگی نمونه های آرد و نان به سم دی اکسینیوالنول کمتر از حد بیشینه استاندارد بود.

طبق آخرین دستورالعملها، در سکته قلبی با صعود قطعه ST ،بهترین روش بازگشایی عروق مسدودشده کرونر، آنژیوپالستی در زمانی کمتر از 90 دقیقه از ورود بیمار به بیمارستان است. تاخیر در برقراری مجدد جریان خون، از مسایل کلیدی در مدیریت درمان سکته حاد قلبی با صعود قطعه ST) STEMI) است. در این مطالعه مروری که در سال 1398 انجام شده است، از مقالات نمایه شده در پایگاه های اطلاعاتی پروکوییست، اس.آی.دی، ساینتس دایرکت، گوگلاسکالر و اسکوپوس با کلمات کلیدی اورژانس پیش بیمارستانی و بیمارستانی، آنژیوپلاستی اولیه، مدیریت زمان، ایسکمی و سکته قلبی حاد استفاده شد. مجموعه مقالات بررسیشده شامل 59 مقاله (از ده کشور مختلف) بود که تحقیقات کیفی و کمی مربوط به بیست سال اخیر (از سال 1999 تاکنون) را به زبانهای فارسی و انگلیسی دربر داشت. مواردی که به جای مقاله مروری، مفهوم مرور مقاله یا نقد مقاله را دربر داشت از مطالعه حذف شد. درضمن مقالاتی که روش کار آنها مشخص نبود و متن کامل آنها در دسترس نبود از مرور حذف شدند. ری پرفیوژن سریعتر باعث کاهش اندازه ناحیه نکروز قلبی و درنتیجه کاهش عوارض و مرگومیر بیماران میشود. در صورت گرفتن نوار قلب بیمار در صحنه توسط تیم اورژانس پیش بیمارستانی بر بالین بیمار و تشخیص STEMI توسط سیستم اورژانس پیش بیمارستانی و فعال سازی کد STEMI در بخش اورژانس، امور تشخیص، تصمیم گیری و انتقال بیمار قلبی جهت دریافت بهترین درمان با هماهنگی و سرعت بیشتری صورت خواهد گرفت.

Alzheimer’s Disease (AD) is a chronic neurodegenerative disease characterized by abnormal protein accumulation, synaptic dysfunction, and cognitive impairment. Peroxisome Proliferator-Activated Receptor-γ (PPARγ) play a crucial role in regulating insulin sensitivity and may serve as potential therapeutic targets for AD. Pioglitazone (PIOG), as a PPARγ agonist, reduces β-amyloid and tau proteins, and inhibits neuroinflammation.
ObjectiveThis study aims to evaluate the effects of PIOG chronic administration on learning and memory in rat model of Streptozotocin (STZ)-induced AD
MethodsForty-two male Wistar rats were divided into two groups: A. Normal rats divided into three subgroups of Control, Dimethyl Sulfoxide (DMSO), and PIOG; and B. AD rats divided into four subgroups of Vehicle, STZ, STZ+DMSO and STZ+PIOG. The last two AD subgroups received 0.2 mL DMSO and PIOG (10 mg/kg per day) for 21 days. For induction of AD, STZ (3 mg/kg, 10 μl per injection site) were administered into lateral ventricles. All rates were trained under the Morris water maze task.
FindingsPIOG impaired the spatial learning and memory in normal rats. Intracerebroventricular injection of STZ significantly increased escape latency and swimming time to find the hidden platform compared to the control group (P<0.05). The amnesic effect of STZ was prevented by PIOG administration such that the escape latency and swimming time to find the hidden platform in the STZ+PIOG group were significantly lower than in the STZ+DMSO group (P<0.05). Conversely, the percentage of time spent and distance swimming in the target quadrant in the probe test in the STZ+ PIOG group rats were significantly higher than those in the STZ + DMSO group.
ConclusionPIOG administration impaired spatial learning and memory in normal rats, but improved learning and memory in rats with STZ-induced AD. It can be useful for treatment of cognitive impairment in AD patients.

The effects of exercise and stevia extract on diabetes-related indicators have been already reported, but their cardiac benefits on Type 1 Diabetes (T1D) are unclear.
ObjectiveThis study aims to evaluate the effect of eight weeks of endurance training and stevia supplementation on gene expression levels of Atrial Natriuretic Peptide (ANP) and β-Myosin Heavy Chain (β-MHC) in the heart tissue of T1D rats.
MethodsIn this experimental study, 25 rats with the average weight of 250-300 g were divided into five groups; healthy control, diabetic control, diabetic+supplementation, diabetic+training, and diabetic+training+supplementation. T1D was induced by intraperitoneal injection of streptozotocin (55 mg/kg/ body weight). Endurance training was performed 5 days a week at a speed of 20-30 meters per minute on a surface with a zero slope for 8 weeks. Stevia was gavaged in a dose of 250 mg/kg/body weight. Rats were slaughtered 48 hours after the last training session. Cardiac tissue was used to measure the parameters. The gene expression of ANP and b-MHC in cardiac tissue was measured by real-time Polymerase Chain Reaction (PCR) method. Data were analyzed by using one-way ANOVA and Bonferroni post hoc test.
FindingsThe gene expression levels of ANP and β-MHC were significantly higher in the diabetic control group compared to the healthy control group (P=0.001), and significantly lower in the diabetic+training and diabetic+training+supplementation groups compared to the diabetic control group (P=0.001).
ConclusionEndurance training and stevia supplementation can have beneficial effects on the heart of T1D rats.

Epilepsy is a neurologic dysfunction caused by abnormal electrical activity in the brain. Oxidative stress is involved in the seizure-induced brain damage.
ObjectiveThis study aimed to evaluate the anticonvulsant and antioxidant effects of pioglitazone (a peroxisome proliferator-activated receptor gamma agonist used for treatment of type 2 diabetes) on Pentylenetetrazole (PTZ)-induced seizure in rats.
MethodsIn this experimental study, 28 rats weighing 20-30 g were divided into four groups of control, pioglitazone, PTZ, and treatment. For treatment, PTZ (85 mg/kg) or normal saline was injected intraperitoneally and 4 hours later, pioglitazone (80 mg/kg) was administrated orally. Carboxymethylcellulose was administered orally in the control and PTZ groups, instead of pioglitazone. One hour after PTZ injection, seizure severity was assessed using Racine scale. Then, the rats were decapitated and the Malondialdehyde (MDA) level and the activity of Catalase (CAT) and Superoxide Dismutase (SOD) in their hippocampus samples were measured by standard methods.
FindingsPioglitazone administration significantly increased the latency to the onset of seizure stages 1-4 and prevented the stage 5. It significantly reduced the lipid peroxidation caused by PTZ-induced seizure and increased the activity of CAT and SOD enzymes in the hippocampus of rats.
ConclusionAntioxidant effects of pioglitazone may play a role in preventing stable PTZ-induced seizures and protecting neurons from seizure-caused damage.

Deficiency or insufficient level of vitamin D is very common in patients with liver disease, and is closely related to musculoskeletal pains, including low back pain.
ObjectiveThe current study aims to investigate of the activity of liver enzymes in women with chronic low back pain following core stability exercises and vitamin D intake.
MethodsThis clinical trial was conducted on 48 women (Mean±SD of age=36.59±5.50 years) in 2019 selected by using a convenience sampling method. They were randomly divided into four groups of control, vitamin D, exercise and combined (exercise +vitamin D). The last two exercise groups performed 8 weeks of core stability exercises with different intensities. The vitamin D and combined groups received 50,000 IU/week of vitamin D for 8 weeks. Alanine aminotransferase (ALT), Aspartate aminotransferase (AST) and Alkaline phosphatase (ALP) activities were measured before and after the interventions. Data were analyzed by using paired t-test and one-way ANOVA.
FindingsEight weeks of core stability exercises, vitamin D intake, and the combined exercise and vitamin D supplementation caused a significant reduction in the activity of AST, ALT, and ALP enzymes. The combined intervention induced more reduction in the AST and ALP activities compared to other two groups received exercise and supplementation alone, and the decrease in ALT activity following the combined intervention was higher compared to the exercise group.
ConclusionIt seems that the core stability exercises, vitamin D intake, and combination of exercise and vitamin D supplementation can improve liver function in women with chronic low back pain who have vitamin D deficiency by reducing the activity of liver enzymes. The combined intervention has greater effectiveness in improving liver function.

Menstrual disorders in female students in the early years after menarche affect their physical, emotional and social functions.
ObjectiveThis study aimed to investigate the prevalence of menstrual disorders among female high school students and its relationship with their Quality of Life.
MethodsThis cross-sectional study was conducted on 1000 high school girls in Qazvin, Iran during 2017-2018. Their menstrual disorders and quality of life were evaluated by a demographic checklist and KIDSCREEN questionnaire, respectively. Data were analyzed using independent t-test.
FindingsThe mean age of high school girls was 15.9±0.93 years. The most common disorder was Premenstrual Syndrome (PMS) with a prevalence of 60%. Their quality of life had a significant relationship with PMS, menorrhagia, metorrhagia, dysmenorrhea, spotting, hypermenorrhea (P<0.001), polymenorrhea (P=0.005), hypomnorrhea (P=0.002) and amenorrhea (P=0.025). The quality of life of girls with these disorders were poorer compared to the girls with no menstrual disorder.
ConclusionMenstrual disorders are highly prevalent among female students, and have a considerable impact on their quality of life. Since this impact has the potential to have longer-term consequences, more efforts are needed to address and treat menstrual disorders in female students.

Nosocomial infections account for about one third of deaths in hospitals.
ObjectiveIn this study, we aim to determine the factors affecting the incidence of nosocomial infections in the Intensive Care Units (ICUs) to provide more information for helping improve the prevention and treatment of these infections in the hospitals located in Iran.
MethodsThis is a comparative cross-sectional study conducted in 2017 on 86 patients admitted to the general ICU of
Boo Ali Sina Hospital in Qazvin, Iran. We extracted the patients’ demographic and clinical information (e.g. blood and urine culture results) from their medical records.FindingsThe prevalence of nosocomial infections were reported 24.3; 38.4% of infections was related to Acinetobacter and 18.6% to Pseudomonas aeruginosa and other microorganisms. The highest antibiotic resistance in patients was to Cefepime (64%) and Ciprofloxacin (60%).
ConclusionThe prevalence of nosocomial infections in the general ICU of the study hospital is relatively high with different strains and varied patterns of antibiotic resistance indicating the need for more comprehensive studies and preventive measures in the country.

Deoxynivalenol (DON) is one of the most common mycotoxins found in cereal products.
ObjectiveThe purpose of this study is to compare the DON contamination level in raw wheat flour and bread.
MethodsIn this analytical study with cross-sectional design, a total of 44 wheat flour and bread samples (Lavash flour and bread, and Barbari flour and bread) were collected. The DON level was measured using ELISA method. Collected data were analyzed in SPSS software by using ANOVA and t-test considering a significance level of P≥0.05.
FindingsThe Mean±SD total DON level in the flour and bread samples was 0.03±0.04 and 0.12±0.21 µg/kg. The mean DON level in the Lavash and Barbari flour samples was 0.01±0.02 and 0.01±0.01 µg/kg, and in the Lavash and Barbari bread samples as 0.04±0.03 and 0.16±0.27 µg/kg, respectively. There was no statistically significant difference in the DON levels between flour and bread samples (P≥0.05).
ConclusionThe presence of DON in the studied samples was observed, but its contamination level was lower than the permissible limit.

According to the latest guidelines, in ST-segment elevation myocardial infarction (STEMI) cases, the best intervention to restore blood flow in the occluded coronary arteries is angioplasty at a time less than 90 minutes. Delay in timely implementation of reperfusion is one of the key problems in the management of STEMI. In this review study, the aim is to investigate the role of pre-hospital and hospital emergency staff in time management of STEMI. For this purpose, the articles published 1999-2019 with available full texts in ProQuest, SID, Science Direct, Google Scholar and Scopus databases were searched using the keywords: Pre-hospital Emergency, Hospital Emergency, Primary Angioplasty, Time Management, Ischemia, and Acute Myocardial Infarction. A total of 59 articles in Persian and English (from 10 different countries) covering both qualitative and quantitative studies were initially yielded. Those which were not a review paper and their full texts were unavailable were excluded from the review. Faster reperfusion reduces the cardiac necrosis area, resulting in reduced morbidity and mortality. Early ECG recording by the pre-hospital emergency team followed by the activation of the code STEMI by the emergency team can increase the speed of diagnosis, decision making, and transfer of the STEMI patient to receive the best treatment.
