مقدمه :
Multiple sclerosis (MS) یک بیماری حاد و خودایمنی است که سبب التهاب و تخریب میلین در سیستم عصبی مرکزی میشود. در این مطالعه، از سکانس دیفیوژن با مقادیر مختلف b-value استفاده شد تا مقدار بهینهی آن مشخص گردد. این مطالعه با هدف بررسی پلاکهای فعال MS با استفاده از روش Diffusion-weighted imaging (DWI) و مقایسهی دقت تشخیصی آن با روش افزایش کنتراست انجام شد.
روشها:
این مطالعهی مقطعی بر روی 90 بیمار مبتلا به MS -بر اساس معیارهای Mc Donald- که به مرکز تصویربرداری شفای اصفهان ارجاع شده بودند، انجام شد. محل پلاکها و تعداد آنها توسط پزشک رادیولوژیست در چک لیست مربوط ثبت گردید. تصویربرداری با استفاده از سیستم Siemens مدل Avanto، با قدرت 5/1 تسلا و کویل سر استاندارد انجام شد.
یافتهها:
تصاویر وزن دیفیوژن (Diffusion weighted) با b-valueهای پایین، رابطهی مستقیم و معنیداری با روش Contrast enhancement (CE) در تعیین پلاکهای فعال دارند (005/0 = P). تصاویر با b-value برابر 500، بیشترین حساسیت (75 درصد) و ویژگی (7/87 درصد) را در تشخیص پلاکهای فعال داشتند.
نتیجهگیری:
نتایج نشان داد که روش DWI با دستگاه 5/1 تسلا توانایی متمایز نشان دادن پلاکهای فعال از غیر فعال را دارد؛ چرا که تصاویر DWI با b-valueهای پایین، رابطهی مستقیم و معنیداری با روش CE در تعیین پلاکهای فعال داشتند.

درماتیت آتوپیک بیماری التهاب مزمن پوست میباشد و شیوع آن به صورت پیوسته رو به افزایش است. هدف از انجام این مطالعه، بررسی شیوع بیماری درماتیت آتوپیک و ارتباط آن با بیماریهای آلرژی تنفسی در بزرگسالان ساکن بوشهر بود.
روشها:
در این مطالعهی مقطعی مبتنی بر جمعیت با نمونهگیری تصادفی- خوشهای چند مرحلهای طبقهبندی شده، مجموع 5420 نفر با سن 65-15 سال که حداقل یک سال ساکن بوشهر بودند، وارد مطالعه شدند. در نهایت، 5201 نفر پرسشنامهی استاندارد شبکهی جهانی آسم و آلرژی اروپا (Global Allergy and Asthma European Network یا GA2LEN) را تکمیل کردند. شیوع کلی درماتیت آتوپیک و شیوع آن در بیماریهای حساسیتی دیگر و همچنین، ارتباط آن با بیماریهای آلرژی بر اساس روشهای آماری محاسبه شد.
یافتهها:
شیوع کلی درماتیت آتوپیک در بوشهر 1/29 درصد بود. میزان ابتلا به درماتیت آتوپیک در مردان 9/25 درصد و در زنان 0/32 درصد بود؛ به طوری که ابتلا به درماتیت آتوپیک با جنسیت رابطهی معنیداری داشت. بیشترین میزان شیوع درماتیت آتوپیک در گروههای سنی 34-25 و 44-35 سال بود و بین گروههای سنی و درماتیت آتوپیک رابطهی معنیداری وجود داشت. همچنین، شیوع درماتیت آتوپیک در بیماران مبتلا به بیماریهای آسم، رینوسینوزیت مزمن و آلرژی بینی به ترتیب برابر 5/50 درصد، 4/48 درصد و 1/47 درصد بود. درماتیت آتوپیک با آلرژی بینی ارتباط معنیداری داشت.
نتیجهگیری:
شیوع بیماری درماتیت آتوپیک در بوشهر بالا بود. همچنین، شیوع درماتیت آتوپیک با بروز هر کدام از بیماریهای آلرژیک نظیر آسم، رینوسینوزیت مزمن و آلرژی بینی ارتباط معنیداری داشت.

Enteroaggregative Escherichia coli (EAEC)، به تازگی به عنوان علت اسهال آبکی مزمن به صورت اپیدمیک و اندمیک در تمام دنیا به خصوص در کشورهای در حال توسعه شناخته شده است. هدف از انجام این مطالعه، تعیین فراوانی این سویه و ژنهای مهم ویرولانس آن در نمونههای اسهال بود.
روشها:
این مطالعه بر روی 80 ایزولهی Escherichia coli جدا شده از کودکان مبتلا به اسهال مراجعه کننده به بیمارستان امام حسین (ع) شهر اصفهان صورت گرفت. برای تایید باکتری، از روشهای بیوشیمیایی و برای تشخیص سویهی EAEC، از آزمایش Multiplex polymerase chain reaction (Multiplex PCR) ژنهای aggR/aap/aatA استفاده شد.
یافتهها:
از 80 ایزولهی جدا شده از بیماران دچار اسهال، 21 سویه (6/26 درصد) به عنوان EAEC شناسایی شد. طبق نتایج Multiplex PCR، ژن aggR در 21 (100 درصد)، aap در 7 (3/33 درصد) و aatA در 3 (3/14درصد) سویه شناسایی شد. همچنین، 7 سویه (3/33درصد) حامل دو ژن aggR و aap، 3 سویه (3/14درصد) ژن aatA و aggR و 2 سویه (5/9 درصد) ژن aatA و aap همزمان بودند. در هیچ یک از سویهها، سه ژن aggR، aatA و aap به طور همزمان ردیابی نشدند.
نتیجهگیری:
مطالعهی حاضر حاکی از فراوانی به نسبت بالای سویهی EAEC تیپیک در کودکان زیر 5 سال با ایجاد علایم اسهال و بیماری بود. بر اساس نتایج به دست آمده، فراوانی بالای ژنهای بیماریزای مهم به ویژه aggR در ایزولهها، نشان دهندهی اهمیت شناسایی این سویه در بیماران با علایم اسهال حاد و مزمن بود؛ به خصوص که در کشور ما، به طور معمول جداسازی و تشخیص این عامل در آزمایشگاه بالینی انجام نمیگیرد.

Multiple sclerosis (MS) is an acute and autoimmune disease that causes inflammation and destruction of myelin in the central nervous system. In this study, the diffusion sequence with different values of b-value was used to determine its optimal value for detection of active plaques.
MethodsThis cross-sectional study was performed on 90 patients with MS basede on Mc Donald's criteria, referred to the Shafa Imaging Center, Isfahan, Iran. The locations of the plaques and their number were recorded by the radiologist in the relevant checklist. Imaging was performed using the Siemens Avanto system, 1.5 Tesla, with of a standard head coil.
FindingsDiffusion weighted images (DWI) with low b-values had a direct and significant relationship with contrast enhancement (CE) method in determining active plaques (P = 0.005). Images with a b-value of 500 had the highest sensitivity (75%) and sensitivity (87.3%) in detecting active plaques.
ConclusionThe results showed that the DWI with the 1.5 Tesla had the ability to distinguish active and inactive plates, because DWI images with low b-values had a direct and significant relationship with CE method in determining active plaques.

Atopic dermatitis (AD) is a chronic inflammatory disease of the skin and its prevalence is continuously on the rise. This study aimed to evaluate the prevalence of AD in adults living in Southwestern part of Iran and its association with respiratory allergic disorders.
MethodsIn this population-based cross-sectional stratified, using multistage cluster random sampling method, a total of 5420 individuals aged 15-65 years were selected from Bushehr City, Iran, from which, 5201 completed the Global Allergy and Asthma Network of Excellence (GA2LEN) questionnaire (Response rate = 96.1%). The total prevalence of AD and its prevalence in allergic diseases was calculated, and the association of AD and these diseases was evaluated using multiple logistic regression model.
FindingsThe overall prevalence of AD was 29% (32% women vs 25.9% men; P < 0.001). The highest prevalence was seen among participants in the age group of 25-34 and 35-44 years (P < 0.001). The prevalence of AD among the patients with asthma, chronic rhinosinusitis (CRS), and allergic rhinitis (AR) was 50.5%, 48.4%, and 47.1%, respectively, that it was significantly related only with AR (P < 0.001).
ConclusionThe prevalence of allergic diseases in Bushehr City was high. The comorbidities of the other allergic diseases such as asthma, chronic rhinosinusitis (CRS), and allergic rhinitis (AR) leads to a higher prevalence of AD.

Enteroaggregative Escherichia coli (EAEC) has recently been identified as the cause of epidemic and endemic chronic watery diarrhea worldwide, especially in developing countries. The aim of this study was to determine the frequency of this strain and its important virulence genes in diarrhea samples.
MethodsThis study was performed on 80 Escherichia coli isolates from children with diarrhea referred to Imam Hossein hospital, Isfahan, Iran. The bacteria were confirmed using biochemical tests and multiplex polymerase chain reaction (multiplex PCR) was performed to detect aggR, aap, aatA genes in EAEC strains.
FindingsThe frequency of EAEC was 21 (26.6%). According to multiplex PCR results, the aggR, aap, and aatA genes were detected in 21 (100%), 7 (33%), and 3 (14.3%) of strains, respectively. Moreover, 7 strains (33.3%) harbored two aggR and aap genes, 3 (14.3%) carried the aatA and aggR genes, and aatA and aap genes were presented in 2 strains (9.5%). None of the strains had 3 genes simultaneously.
ConclusionThis study provides the first report of a relatively high frequency of typical EAEC strains in children under 5 years of age with symptoms of diarrheal disease in Isfahan. Moreover, we found that the EAEC strains not only exhibited high frequency, but also showed a high frequency of carrying virulence genes especially aggR that indicated the importance of identifying this strain in patients showing the symptoms of acute and chronic diarrhea, especially in our country, as the diagnosis of this strain is not done routinely.
