خانوادهی بیماران مبتلا به اختلالات روانی و جسمانی مزمن، اقدامات حمایتی متعددی را برای بیمار انجام میدهند. مراقبان این گروه از بیماران، فشار مراقبتی زیادی را متحمل میشوند. شناسایی میزان این فشارها میتواند نقش تعیینکنندهای در ارتقای سلامت روانی آنها داشته باشد. پژوهش حاضر با هدف مقایسهی فشار مراقبتی در مراقبان بیماران جسمانی و روانی مزمن انجام شد.
روشها:
این مطالعه، مقطعی از نوع توصیفی بود که در آن 300 نفر از مراقبان بیماران مبتلا به سرطان و اختلالات روانی مراجعهکننده به بخشهای بستری و سرپایی بیمارستانهای سیدالشهدا (ع)، نور و فارابی در تابستان سال 1398 به روش در دسترس انتخاب شدند. ابزار جمعآوری دادهها شامل پرسشنامهی فشار مراقبتی Novak و Guest و پرسشنامهی محقق ساختهی ویژگیهای جمعیتشناختی بود. دادهها با استفاده از آزمون Independent t مورد تجزیه و تحلیل گرفت.
یافته ها:
میانگین فشار مراقبتی در مراقبان بیماران مبتلا به سرطان، 92/17 ± 80/55 و میانگین فشار مراقبتی در مراقبان بیماران مبتلا به اختلالات روان، 02/20 ± 60/62 بود. تفاوت معنیداری بین نمرهی کلی فشار مراقبتی و زیرمقیاسهای آن در مراقبان بیماران مبتلا به سرطان و اختلالات روان وجود داشت که با توجه به میانگین نمرات، این مقادیر در مراقبان بیماران روان بیشتر از مراقبان بیماران مبتلا به سرطان بود، اما بین مولفهی فشار مراقبتی وابسته به زمان تفاوت معنیداری مشاهده نشد.
نتیجه گیریمراقبت از بیماران مبتلا به بیماریهای مزمن، فشار مراقبتی زیادی را در مراقبان ایجاد میکند که میانگین فشار مراقبتی در مراقبان بیماران مبتلا به اختلالات روان بیشتر از مراقبان بیماران مبتلا به سرطان میباشد.

استفاده از روش تصویربرداری توموگرافی کامپیوتری (Computed tomography scan یا CT Scan)، احتمال ایجاد عارضه در مردمک چشم، غدهی تیرویید و پستان را افزایش میدهد. یکی از روشهای حفاظت پرتویی بیماران از تابشهای غیر لازم و ناخواسته در آزمونهای CT Scan، استفاده از محافظهای غیر سربی میباشد. لازم است قبل از طراحی و استفاده از این محافظها، کارایی آنها توسط شبیهسازی بررسی گردد.
روش ها:
بیسموت، تنگستن و قلع با لبههای جذب در محدودهی انرژی تشخیصی، برای مطالعهی حاضر انتخاب شدند. هر یک از فلزات به همراه سیلیکون، به عنوان مواد سازنده در شبیهسازی Monte Carlo استفاده گردید. جهت تعیین ضریب تضعیف جرمی نمونهها، باریکه به صورت تک انرژی شبیهسازی شد. بازهی انرژی با توجه به آزمونهای تصویربرداری متداول در آزمایشگاههای تشخیصی، از 20 تا 140 کیلوالکترون ولت با پلههای 20 کیلوالکترون ولتی انتخاب گردید. چشمهی فوتونی مسطح به شعاع 5 میلیمتر در مبدا هندسه قرار گرفت. اطلاعات به دست آمده از شبیهسازی مشخص نمود که کدام محافظ در چه انرژیهایی ضریب تضعیف بیشتری دارد؟
یافته ها:
محافظ حاوی 10 درصد بیسموت و 90 درصد سیلیکون، تضعیف بیشتری را در انرژیهای بالای 100 کیلوالکترون ولت نشان داد. محافظ تنگستن و قلع نیز در انرژیهای میانی و پایین تضعیف بیشتری داشتند.
نتیجه گیری:
با بهرهگیری از اثر پنجرهی تضعیف و انتخاب درصد و نوع فلز متناسب با انرژی مورد استفاده در رادیولوژی تشخیصی، میتوان از پرتوگیری ارگانهای سطحی حساس به پرتوی بیماران محافظت کرد.

هدف از انجام پژوهش حاضر، بررسی تاثیر شش هفته تمرین هوازی بر بیان ژنهای استرس اکسیداتیو شبکهی آندوپلاسمی (Endoplasmic reticulum یا ER) شامل عامل شروعکنندهی ترجمهی یوکاریوتی 2 (Eukaryotic Initiation Factor-2α یا eIF2α) و پروتیین متصل شونده به جعبهی X (X-Box Binding Protein-1 یا XBP-1) در بافت قلب موشهای صحرایی نر دیابتی شده با استرپتوزوتوسین بود.
روش ها:
20 سر موش صحرایی نر با میانگین وزنی 3/11± 0/204 گرم، به طور تصادفی در چهار گروه شامل شاهد سالم، تمرین سالم، شاهد مبتلا به دیابت و تمرین مبتلا به دیابت قرار گرفتند. جهت القای دیابت، از تزریق دورن صفاقی استرپتوزوتوسین (Streptozotocin یا STZ) (50 میلیگرم بر کیلوگرم) استفاده گردید. پس از تایید القای دیابت از طریق اندازهگیری سطح گلوکز خون ناشتا با استفاده از گلوکومتر، برنامهی تمرین هوازی با شدت متوسط به مدت 30 دقیقه و سرعت 18 متر بر دقیقه، 5 روز در هفته طی 6 هفته اجرا شد. بیان ژنهای XBP-1 و eIF2α با استفاده از روش Real-time polymerase chain reaction (RT-PCR) اندازهگیری گردید. دادهها با استفاده از آزمون One-way ANOVA و آزمون تعقیبی Tukey مورد تجزیه و تحلیل قرار گرفت.
یافته ها:
میزان بیان ژنهای XBP-1 و eIF2α در بافت قلب موشهای مبتلا به دیابت، به طور معنیداری بالاتر از گروه شاهد سالم بود. پس از اتمام پروتکل تمرین هوازی، میزان بیان ژنهای فوق در بافت قلب موشهای گروههای تمرین مبتلا به دیابت، تمرین سالم و شاهد سالم به طور معنیداری نسبت به گروه شاهد مبتلا به دیابت پایینتر بود.
نتیجه گیری:
تمرین هوازی با کاهش شاخصهای استرس اکسیداتیو ER در قلب موشهای صحرایی نر مبتلا به دیابت، میتواند به عنوان مداخلهی غیر دارویی در درمان بیماران مبتلا به دیابت مفید باشد.

Families of patients with chronic mental and physical disorders provide patients with a number of supportive measures. Caregivers of this group of patients endure a lot of care stress Identifying the extent of these stresses can play a decisive role in promoting their mental health. This study was conducted to compare the care stress in caregivers of patients with chronic physical and mental diseases.
MethodsThis was a descriptive cross-sectional study in which 300 patients with cancer or mental disorders who referred to the inpatient and outpatient wards of Seyed Al-Shohada, Noor, and Farabi hospitals Isfahan City, Iran, in the summer of 2019, were selected by convenient methods. Data collection tools included Novak and Gast Stress Pressure Questionnaire and a researcher-made questionnaire of demographic characteristics. Data analysis was performed using independent t test.
FindingsThe mean care stress among caregivers of patients with cancer was 55.8 ± 17.92 as well as 62.60 ± 20.02 among the caregivers of patients with mental disorders. There was a significant difference between the overall score of caring stress and its subscales among the caregivers of patients with cancer or mental disorders. According to the mean scores, these values were higher among the caregivers of patients with psychiatric disorders than that of patients with cancer. However, there was no significant difference between the timedependent care stress components.
ConclusionBased on the findings of this study, patients with chronic diseases place a great deal of care stress on caregivers. In addition, the mean care stress in caregivers of patients with mental disorders are higher than in patients with cancer.

Using computed tomography (CT) scan may cause complications in the eye lens, thyroid gland, and breasts. One of the radiation protection methods to protect patients from unnecessary and unintended exposures is utilizing non-lead shields in CT-scan tests. Before designing and using these shields, it is necessary to study their performance by a simulation.
MethodsBismuth, tungsten, and tin with absorption edges in diagnostic radiology energy ranges were selected for this research. Each of the metals along with the silicon was used as building blocks in the Monte Carlo simulation. To determine the mass attenuation coefficient of the samples, the beam was simulated as single energy. The energy range was selected according to the common imaging tests in diagnostic laboratories from 20 to 140 keV with 20 keV steps. The flat photon source with a radius of 5 mm was placed at the origin of the geometry. Information obtained from the simulation could specify that the shield had the most attenuation coefficient at which energies.
FindingsThe shield containing 10% bismuth and 90% silicon showed more attenuation in the energies above 100 keV. Tungsten and tin shields also showed greater attenuation at medium and low energies.
ConclusionBy using the effect of attenuation window and selecting proper percentage and metal for the energy applied in diagnostic radiology, patients can be protected from radiation exposure to radiosensitive superficial organs.

The aim of this study was to investigate the effect of six weeks of aerobic training on expression of endoplasmic reticulum stress genes [X-box binding protein-1 (XBP1) and eukaryotic initiation factor 2α (eIF2α)] in the heart tissue of male rats with streptozotocin (STZ)-induced diabetes.
MethodsTwenty male rats with the mean weight of 204.0 ± 11.3 grams were randomly divided into four groups including of healthy control, healthy exercise, diabetic control, and diabetic training. They became diabetic by intraperitoneal injection of STZ (50 mg/kg). After confirmation of induction of diabetes by measuring fasting blood glucose level with glucometer, aerobic training program was performed with an average intensity for 30 minutes at a speed of 18 meters per minute, 5 days a week for 6 weeks. The expression of endoplasmic reticulum stress genes (XBP1 and eIF2α) was measured using real-time polymerase chain reaction (RT-PCR) technique. One-way analysis of variance and Tukey's post hoc test were used for statistical analysis.
FindingsThe expression of endoplasmic reticulum stress genes (XBP1 and eIF2α) in heart tissue in diabetic rats was significantly higher than the healthy control group. After 6 weeks of aerobic exercise protocol, the expression of these genes in heart tissue was significantly lower in the diabetic group of exercise, healthy exercise, and healthy control than the diabetic control group.
ConclusionThe results of the present study showed that by reducing endoplasmic reticulum stress factors in the heart of male rats with diabetes, aerobic training can be useful as a preventive and non-pharmacological factor in the treatment of patients with diabetes.
