یکی از مشکلات اصلی سیستم درمان، مرگ و میر بالا و طول مدت بستری بیماران در Intensive care unit (ICU) میباشد که هزینههای زیادی دارد. پژوهش حاضر با هدف تعیین تاثیر تعدیل Fraction of inspired oxygen (FIO2) و فشار مثبت انتهای بازدمی (Positive end-expiratory pressure یا PEEP) بر اساس میزان فشار اکسیژن خون شریانی بر پیامدهای مراقبتی بیماران بستری در ICU انجام شد.
روش هااین مطالعه از نوع کارآزمایی بالینی تصادفی و جامعهی آماری آن، متشکل از بیماران بستری در بخش ICU بیمارستان امام خمینی (ره) اهواز در نیمهی دوم سال 1399 بود. 40 بیمار به روش در دسترس انتخاب و به صورت تصادفی در دو گروه مداخله و شاهد تقسیمبندی شدند. جهت جمعآوری دادهها، از پرسشنامهی اطلاعات دموگرافیک و سیستم نمرهدهی Sequential Organ Failure Assessment (SOFA) استفاده گردید. در گروه مداخله، تعدیل FIO2 و PEEP بر اساس میزان فشار اکسیژن خون شریانی بیماران طی 7 روز انجام شد.
یافته هااز نظر متغیرهای دموگرافیک، تفاوت معنیداری بین دو گروه مشاهده نشد. میانگین نمرات SOFA قبل از مداخله در دو گروه تفاوت معنیداری نداشت، اما پس از مداخله، شدت بیماری، طول مدت اتصال به ونتیلاتور و بستری در ICU در گروه مداخله کمتر بود.
نتیجه گیریتعدیل FIO2 و PEEP بر اساس میزان فشار اکسیژن خون شریانی، بر کاهش نمرهی SOFA، طول مدت اتصال به ونتیلاتور و ماندگاری بیمار در ICU موثر میباشد. این روش میتواند به عنوان یک مداخلهی پرستاری و با هماهنگی تیم درمان به منظور کاهش پیامدهای بستری در ICU مورد استفاده قرار گیرد.

کوتاهی قد ایدیوپاتیک، از جمله علل شایع مراجعه به درمانگاههای غدد کودکان میباشد. با وجود سطح طبیعی هورمون رشد، سطح فاکتور رشد شبه انسولین 1 (Insulin-like growth factor 1 یا IGF-1) در برخی از این افراد پایین است که علت آن به درستی مشخص نیست. پژوهش حاضر با هدف تعیین تاثیر مکمل ویتامین D بر سطح IGF-1 در کودکان 5 تا 10 سالهی مبتلا به کوتاهی قد ایدیوپاتیک انجام شد.
روش هادر این مطالعهی کارآزمایی بالینی، 30 کودک 5 تا 10 ساله با تشخیص کوتاهی قد ایدیوپاتیک، به مدت 8 هفته تحت درمان با 50 هزار واحد مکمل ویتامین D3 قرار گرفتند و سطح سرمی ویتامین D و IGF-1 آنان قبل و پس از مداخله اندازهگیری و مقایسه گردید.
یافته هامیانگین سطح سرمی ویتامین D کودکان قبل و پس از مداخله به ترتیب 80/6 ± 03/17 و 20/9 ± 40/44 نانوگرم در میلیلیتر (001/0 > P) و میانگین سطح سرمی IGF-1 قبل و پس از مداخله به ترتیب 90/17 ± 50/81 و 40/37 ± 40/124 نانوگرم در میلیلیتر (001/0 > P) گزارش گردید. با تجویز مکمل ویتامین D، سطح IGF-1 به طور معنیداری افزایش یافت.
نتیجه گیریبا برطرف شدن کمبود ویتامین D، سطح IGF-1 افزایش مییابد. به نظر میرسد در کودکان با کوتاهی قد ایدیوپاتیک، برطرف کردن کمبود ویتامین D، میتواند در افزایش سطح IGF-1 موثر باشد.

اثرات ضد التهابی ورزش در بیماریهای مزمن، با کاهش سایتوکاینهای پیشالتهابی و افزایش سایتوکاینهای ضد التهابی میانجیگری میشود. هدف از انجام پژوهش حاضر، بررسی تاثیر 4 هفته فعالیت هوازی داوطلبانه همراه با اینترفرون بتا، بر سطوح سرمی عامل نکروز دهندهی تومور آلفا (Tumor necrosis factor alfa یا TNF-α) و اینترلوکین -10 (Interleukin 10 یا IL-10) در موشهای مدل مالتیپل اسکلروزیس (Multiple sclerosis یا MS) بود.
روش هادر این مطالعهی تجربی، 80 سر موش ماده به 8 گروه 10تایی تقسیم شدند. جهت القای Experimental autoimmune encephalomyelitis (EAE)، مخلوط Myelin oligodendrocyte glycoprotein (35-55) (MOG) به صورت زیرجلدی و سم سیاه سرفه به صورت داخل صفاقی به موشها تزریق شد. پروتکل دارویی اینترفرون بتا به میزان 150 واحد بینالملل در گرم و پروتکل 4 هفتهای تمرین هوازی داوطلبانه با چرخ دوار روی گروههای درمان اجرا گردید. در پایان، نمونههای خونی از قلب استخراج شد و دادهها با استفاده از آزمون One-way ANOVA مورد تجزیه و تحلیل قرار گرفت.
یافته هاسطوح TNF-α در گروه مبتلا به MS دریافتکنندهی اینترفرون در مقایسه با گروه شاهد مبتلا به MS دریافتکنندهی اینترفرون، کاهش معنیدار (001/0 = P) و میزان 10-IL در مقایسهی همین گروهها افزایش معنیداری (010/0 = P) را نشان داد، اما فعالیت هوازی در کنار تزریق اینترفرون، به طور موثرتری منجر به افزایش 10-IL و کاهش TNF-α سرمی در گروه مبتلا به MS دریافتکنندهی اینترفرون و دونده در مقایسه با گروه شاهد مبتلا به MS دونده و دریافتکنندهی اینترفرون (نسبت به ورزش یا دارودرمانی به تنهایی) گردید (001/0 = P).
نتیجه گیریفعالیت هوازی در کنار دارودرمانی شاید بتواند از طریق کنترل عوامل التهابی، منجر به بهبود بالینی و کیفیت زندگی بیماران مبتلا به MS شود.

One of the problems of the treatment system is the high mortality and length stay of patients in the intensive care unit (ICU), which has high costs. The aim of this study was to determine the effect of adjustment of positive end-expiratory pressure (PEEP) and fraction of inspired oxygen (FiO2) based on PaO2 on the care and treatment outcomes of ICU patients.
MethodThis was a randomized clinical trial study in which the statistical population consisted of all patients admitted to the ICU of Imam Khomeini hospital in Ahvaz, Iran, in the second half of 2020. 40 samples were selected based on available sampling method, and randomly divided into two groups of intervention and control. To collect the data, a demographic information questionnaire as well as Sequential Organ Failure Assessment (SOFA) scoring system were used. In the intervention group, adjustment of PEEP and FiO2 was performed based on PaO2 of patients in 7 days.
FindingsThere was no difference between the two groups in terms of demographic variables. The mean scores of disease severity before intervention in the two groups were not statistically different; but after the intervention, disease severity score, duration of ventilator connection, and length of ICU stay were significantly lower in the intervention group.
ConclusionThe adjustment of PEEP and FiO2 based on PaO2 is effective in reducing the disease severity score, duration of ventilator connection, and ICU stay. It should be used as a nursing intervention in coordination with treatment team to reduce the consequences of hospitalization in ICU.

Idiopathic short stature is a common cause for referral to children's endocrinology clinics. Despite the normal level of growth hormone, the level of insulin-like growth factor 1 (IGF-1) is low in some of these individuals, which is not well-defined. The aim of this study was to determine the effect of vitamin D on the level of IGF-1 in children aged 5 to 10 years with idiopathic stunting.
MethodsIn a clinical trial study, 30 children aged 5-10 years with the diagnosis of idiopathic short stature were selected, and treated with 50000 vitamin D3 supplements every week for 8 weeks. Serum levels of vitamin D and IGF-1 were measured before and after intervention and compared.
FindingsBefore and after intervention, mean serum levels of vitamin D were 17.03 ± 6.8 and 44.4 ± 9.2 ng/ml (P < 0.001), and mean serum level of IGF-1 were 17.9 ± 81.5 and 124.4 ± 37.4 ng/ml (P < 0.001), respectively. The level of IGF-1 significantly increased by vitamin D supplementation.
ConclusionThe present study showed that improvement of vitamin D deficiency led to increased level of IGF-1. It seems that in children with idiopathic short stature, the elimination of vitamin D deficiency can be effective in increased level of IGF-1.

The anti-inflammatory effects of exercise in chronic diseases are mediated by a decrease in pro-inflammatory cytokines and an increase in anti-inflammatory cytokines. The Aim of this study was to evaluate the effect of 4 weeks of voluntary aerobic activity along with interferon beta on the serum levels of tumor necrosis factor alfa (TNF-α) and interleukin 10 (IL-10) in mice model of multiple sclerosis.
MethodsIn this experimental study, a total number of 80 female mice were divided into eight groups. To induce of Experimental Autoimmune Encephalomyelitis (EAE), mixture of myelin oligodendrocyte glycoprotein (MOG) (35-55) was injected subcutaneously, and pertussis toxin was injected intraperitoneally. The interferon beta drug protocol in amount of 150 IU/g as well as the 4-week protocol of voluntary aerobic exercise with running wheel were administered to the treatment groups. Finally, blood samples were taken from the heart and data analysis was done using one-way ANOVA test.
FindingsTNF-α levels in the MS+INF-β group significantly reduced compared to the MS + solvent group (P = 0.0010), and a significant increase was observed in the amount of IL-10 in comparison with the same groups (P = 0.01). But aerobic activity along with interferon beta-1 treatment, more effectively led to an increase in IL-10 of serum and a decrease in serum TNF-α in MS + INF-β + running wheel group compared to MS + solvent + running wheel group (compared to exercise or medication alone) (P = 0.001).
ConclusionAerobic activity along with medication may possibly lead to clinical improvement and quality of life in patients with MS by controlling inflammatory factors.
