میزان شیوع آسم آتوپیک و غیر آتوپیک و برخی عوامل مرتبط با آن بین مراجعه کنندگان به کلینیک آسم و آلرژیصفحات 944-950مقدمه
شیوع آسم و بیماریهای آلرژی در جهان طی دهههای اخیر افزایش یافته است و شناخت آلرژنها در مناطق مختلف ضروری میباشد. پژوهش حاضر با هدف بررسی میزان فراوانی آلرژنها در بیماران مبتلا به آسم آتوپیک و غیر آتوپیک در کلینیک آسم و آلرژی بیمارستان دزیانی گرگان در سال 1399 انجام شد.
روش هااین مطالعه از نوع مقطعی- توصیفی بود که بر روی 96 کودک 1 تا 16 سالهی مبتلا به آسم انجام گردید. تشخیص بیماری آسم بر اساس علایم بالینی بیمار، داروهای نجاتی مورد استفاده، محدودیت فعالیت ایجاد شده و آزمایشهای عملکرد ریوی صورت گرفت. بر همین اساس، تشخیص قطعی آسم به صورت افزایش Forced expiratory volume-1 (FEV1) به میزان 15 درصد پس از استنشاق 2 پاف سالبوتامول بود. کودکان بر اساس آزمایشهای انجام شده، به دو گروه اتوپیک و غیر اتوپیک تقسیمبندی شدند. نمونهها در صورت بالا بودن میزان Immunoglobulin E (IgE) و مثبت شدن تست Prick، در گروه آتوپیک و در غیر این صورت در گروه غیر آتوپیک قرار گرفتند. مقادیر سرمی ایوزینوفیل و IgE به تفکیک نوع آسم و شدت آسم، تجزیه و تحلیل گردید.
یافته هااز بین کودکان مورد بررسی، 17/54 درصد دارای فنوتیپ آتوپیک و 83/45 درصد دارای فنوتیپ غیرآتوپیک بودند. در مجموع، 78/71 درصد از کودکان مبتلا به آسم، حداقل حساسیت به یکی از مواد آلرژن مورد بررسی را داشتند. همچنین، ارتباط معنیداری بین شدت آسم و آتوپیک بودن آن، میزان IgE و ایوزینوفیل مشاهده شد.
نتیجه گیریآسم در کودکان مبتلا به آسم آتوپیک نسبت به غیر آتوپیک شدیدتر و میانگین IgE و ایوزینوفیل در آسم آتوپیک بالاتر از غیرآتوپیک میباشد. همچنین، میزان IgE و ایوزینوفیل، ارتباط مستقیمی با شدت آسم دارد.

بستری مجدد، یکی از سنجههای کیفیت مراقبت و عملکرد بیمارستان است. هدف از انجام پژوهش حاضر، بررسی میزان بستری مجدد و علل و هزینههای آن در بخشهای بستری یک بیمارستان عمومی دانشگاهی بود.
روشهادادههای یکسالهی یک مرکز آموزشی 320 تختخوابی به منظور مشخص شدن موارد بستری مجدد، بخش بستری، تشخیص نهایی و هزینهها استخراج گردید. با بررسی تخصصی، موارد بستری مجدد در یکی از گروههای 20گانه و یکی از علل 10گانهی بیماری طبقهبندی شد. دادهها با استفاده از آمارههای توصیفی، آزمون 2c و Logistic regression در نرمافزار SPSS مورد تجزیه و تحلیل قرار گرفت.
یافتههااز مجموع 17542 مورد بستری، 1214 مورد (92/6 درصد) بستری مجدد شامل 27/8 درصد کل هزینههای بستری، رخ داده بود. بین علل بستری مجدد، بیارتباط و عوارض بیماری به ترتیب بیشترین علتها و بیشترین درصد بستری مجدد نیز در بخش زنان و زایمان بود. نتایج Logistic regression چندگانه نشان داد که شانس بستری مجدد در گروههای سنی 15 تا 65 سال و کمتر از 15 سال، به ترتیب 3/32 درصد [58/0-78/0 :Confidence interval (CI)، 67/0 = Odds ratio (OR)] و 1/58 درصد (32/0-51/0 :CI، 41/0 = OR) نسبت به گروه سنی بالای 65 سال، کمتر بود (050/0 > P). شانس بستری مجدد در بیماران بیمهی تامین اجتماعی، تصادفی، بیمهی سلامت و متفرقه نسبت به بیماران دارای بیمهی نیروهای مسلح به ترتیب 0/35 درصد (50/0-84/0 :CI، 65/0 = OR)، 8/37 درصد (43/0-89/0 :CI، 62/0 = OR)، 1/23 درصد (59/0-98/0 :CI، 76/0 = OR) و 9/68 درصد (11/0-89/0 :CI، 32/0 = OR) کمتر بود (050/0 > P).
بحثنتایج مطالعه حاکی از اهمیت پایش میزان، روند و هزینههای بستری مجدد به عنوان یکی از نشانگرهای کلیدی کیفیت عملکرد بیمارستانی است.

کاشت حلزون شنوایی (Cochlear implant یا CI) اغلب از طریق ماستوییدکتومی و تمپانوتومی خلفی (Posterior tympanotomy یا PT) برای دسترسی به حلزون انجام میشود. این روش جراحی میتواند جراح را در معرض عوارض احتمالی یا قرار دادن اشتباه الکترود قرار دهد. تکنیکهای جایگزین برای کاشت حلزون با روشهای اندومیاتال یا سوپرامیاتال بدون PT شرح داده شدهاند. در این گزارش مورد، بیمار 4 سالهای که کاندید کاشت حلزون شد ولی به دلیل مشکل بودن دسترسی به دریچهی گرد، از روش ترکیبی برای رسیدن به حلزون استفاده گردید، معرفی میشود.
گزارش مورددر مورد این بیمار خاص، پنوماتیزاسیون ماستویید محدودیت نداشت و برای دسترسی به دریچهی گرد، از همان ابتدا با رویکرد PT جراحی شروع شد، اما پس از باز کردن ماستویید و Facial recess، به دلیل این که مجرای نیمدایرهی جانبی تکامل پیدا نکرده بود، دریچهی بیضی و استخوان رکابی و دریچهی گرد به سمت عقب جابهجا شده بود و دریچهی گرد از طریق Facial recess قابل رویت نبود. بنابراین، با اضافه کردن رویکرد Trans-canal، دسترسی به دریچهی گرد امکانپذیر و پروتز با موفقیت جایگذاری گردید. عارضهای رخ نداد و شش ماه پس از جراحی، بیمار عملکرد شنوایی و گفتاری قابل قبولی داشت.
نتیجه گیریآشنایی و داشتن تجربه در مورد رویکردهای مختلف کاشت حلزون، از ملزومات کار یک جراح کاشت حلزون است تا در موارد عدم دسترسی به دریچهی گرد یا پیدا کردن محل مناسب کوکلیوستومی، بتواند از روشهای جایگزین استفاده کند.

The prevalence of asthma and allergic diseases in the world has increased in recent decades, and it is necessary to identify allergens in different regions. The aim of this study was to evaluate the frequency of allergens in patients with atopic and non-atopic asthma in Dazyani Asthma and Allergy Clinic, Gorgan City, Iran in 2020.
MethodsThis cross-sectional descriptive study was performed on 96 children with asthma aged 1 to 16 years. Diagnosis of asthma was based on the patient's clinical signs, life-saving medications used, activity limitation established, and pulmonary function tests. Accordingly, the definitive diagnosis of asthma was a 15% increase in forced expiratory volume-1 (FEV1) after inhalation of 2 puffs of salbutamol. Children were divided into two groups of atopic and non-atopic based on experiments. In this study, if immunoglobulin E (IgE) levels were high and Prick test was positive, they were in the atopic group, otherwise they were in the non-atopic group. Serum levels of eosinophils and immunoglobulin E were analyzed by type of asthma and severity of asthma.
FindingAmong the studied children, 54.17% had atopic and 45.83% non-atopic phenotype. A total of 71.78% of children with asthma were at least allergic to one of the allergens tested. There were also significant relationships between the severity of asthma and being atopic, immunoglobulin E, and eosinophil count.
ConclusionAsthma is more severe in children with atopic asthma than non-atopic asthma. The mean of immunoglobulin E and eosinophils in atopic asthma is higher than non-atopic. Immunoglobulin E and eosinophil levels are also directly related to the severity of asthma.

Readmission is a hospital quality and outcome measure. The aim of this study was to investigate the rate of readmission and its causes and costs in inpatient wards of a university hospital in Iran.
MethodsOne-year data from a 320-bed training center were extracted to determine readmission, admission, final diagnosis, and costs. According to a specialized study, readmission cases were classified into one of 20 groups, and one of the 10 causes of the disease. Data analysis was performed using chi-square and logistic regression tests via SPSS software.
FindingsOut of a total of 17,542 hospitalizations, 1214 (6.92%) were readmitted, accounting for 8.27% of the total hospitalization costs. Among the classes of causes of readmission, unrelated complications of disease and recurrence of the disease were the most common causes of readmission, respectively. Among the inpatient wards, the highest percentage of readmission was related to the gynecology ward. The results of multiple logistic regression showed that the chances of readmission in the age groups between 15 to 65 years and under 15 years, was respectively 32.3% [confidence interval (CI): 0.78-0.58; odds ratio (OR) = 0.67] and 58.1% (CI: 0.51-0.32; OR = 0.41) lower than the age group over 65 years (P < 0.050). The chance of readmission in patients covered by social security, accidental, health and miscellaneous insurance was respectively 35.0% (CI: 0.50-0.84; OR = 0.65), 37.8% (CI: 0.43-0.89; OR = 0.62), 23.1% (CI: 0.98-0.59; OR = 0.76) and 68.9% (CI: 0.89-0.11; OR = 0.32) lower than patients with armed insurance coverage (P < 0.050).
ConclusionThe results of the study indicate the importance of monitoring the rate, process, and costs of readmission as one of the key indicators of hospital performance quality.

Cochlear implantation (CI) is usually performed through mastoidectomy and posterior tympanotomy (PT) to access the cochlea. This surgical procedure can expose the surgeon to possible complications or misplacement of the electrode. Alternative techniques for cochlear implantation with endomeatal or suprameatal approach without posterior tympanotomy have been described. Here, we report a 4-year-old patient, a candidate for cochlear implantation, for whom due to the difficulty of accessing the round window, we used the combined approach to reach the cochlea.
Case ReportIn this particular patient, there was no restriction on mastoid pneumatization; so, we began surgery with a posterior tympanotomy approach to access the round window. But after opening the mastoid and facial recess, because the lateral semicircular canal was not developed well, the oval window and the stapes were displaced posteriorly. Moreover, because of posterior displacement, the round window was not visible through the facial recess. Therefore, by adding the trans-canal approach, we could access the round window, and the prosthesis was successfully inserted. No complication occurred, and six months after surgery, the patient had acceptable auditory and speech performances.
ConclusionFamiliarity and having experience in different approaches to the cochlea is a prerequisite for a cochlear implant surgeon to be able to use alternative methods in cases of difficulty in accessing the round window or finding a proper site for cochleostomy.
