مقدمه :
تصویربرداری تشدید مغناطیسی قلبی (Cardiovascular Magnetic Resonance) با استفاده از نقشه های T2 و نقشه های ضریب انتشار ظاهری (Apparent diffusion coefficient) ADC، روش های جدید برای نمایش ایسکیمی و ادم میوکارد محسوب می شوند. در این مطالعه حساسیت، ویژگی و کاربرد تشخیصی نقشه های ADC در تصویربرداری دیفیوژنی نسبت به نقشه های T2 در تشخیص ایسکیمی قلبی مورد بررسی و مقایسه قرار گرفت.
روش هااین یک مطالعه ی توصیفی- تحلیلی است که در بیمارستان پیوند اعضای شیراز از بهمن ماه 1399تا اسفندماه 1400 بر روی 58 بیمار مبتلا به ایسکیمی قلبی با میانگین پنج روز پس از اینفارکت، تحت تصویربرداری ام آر آی 3 تسلا قرار گرفتند. برای این منظور، تصاویر نقشه های T2 و نقشه های ADC از میوکارد اخذ و مقایسه شدند. تصاویر سینمایی (Cine) برای بررسی کارکرد بطن چپ مورد استفاده قرار گرفت و بر اساس تصاویر افزایش تاخیری گادلنیوم (Late Gadolinium Enhancement) ناحیه ی اینفارکت شده ی میوکارد تعریف شد.
یافته هادر بررسی های آماری که با قطعه بندی بطن چپ انجام گرفت، حساسیت و ویژگی نقشه های T2 به ترتیب 84 و 95 درصد و حساسیت و ویژگی نقشه های ADC به ترتیب 88 و 97 درصد گزارش شد.
نتیجه گیریبا توجه به ارزیابی های انجام شده، نقشه های ADC حساسیت و ویژگی بالاتری نسبت به نقشه های T2 که به طور روتین در کلینیک استفاده می شوند نشان دادند، لذا استفاده از آن ها به منظور بررسی ایسکیمی در بیماران قلبی پیشنهاد می گردد.

مقدمه:
دردهای عضلانی- اسکلتی، از شایع ترین دردها در مراجعین به مراکز درمانی است که می تواند دارای ابعاد روان شناختی باشد. هدف پژوهش حاضر، تدوین مدل ساختاری در تبیین نقش واسطه ای فاجعه سازی درد، افسردگی و اضطراب بین انعطاف پذیری روان شناختی با خودکارآمدی درد بیماران مبتلا به دردهای عضلانی- اسکلتی بود.
روش هادر یک مطالعه ی مقطعی، 200 بیمار مبتلا به دردهای مزمن عضلانی- اسکلتی به روش نمونه گیری در دسترس از میان مراجعه کنندگان به مراکز درمانی وابسته به دانشگاه علوم پزشکی اصفهان در 1400-1399 انتخاب شدند. شرکت کنندگان واجد شرایط ورود به مطالعه، پرسش نامه های پذیرش درد مزمن (Chronic Pain Acceptance Questionnaire)، گسلش شناختی (Cognitive Fusion Questionnaire)، خود مشاهده گر (Self-Experience Questionnaire)، زندگی ارزش محور (Value-based Life Questionnaire)، اقدام متعهدانه (Committed Action Questionnaire)، خودکارآمدی درد (Pain Self-Efficacy Questionnaire) و مقیاس های بهشیاری (Mindful Attention Awareness Scale)، فاجعه سازی درد (Pain Catastrophizing Scale) و اضطراب و افسردگی بیمارستانی (Hospital Anxiety and Depression Scale) را تکمیل کردند. داده ها با روش آماری معادلات ساختاری مورد تجزیه و تحلیل قرار گرفت.
یافته هانتایج پژوهش نشان داد، ضریب میزان تاثیر مستقیم فاجعه سازی درد، اضطراب، افسردگی و انعطاف پذیری روان شناختی بر خودکارآمدی درد به ترتیب 0/357، 0/255، 0/127 و 0/849 است و در سطح 99 درصد اطمینان این تاثیر معنی دار است (0/01 > P). انعطاف پذیری روان شناختی به صورت غیرمستقیم با میانجیگری فاجعه سازی درد، اضطراب، افسردگی بر خودکارآمدی درد (502-/0) تاثیر می گذارد (0/01 > P). رابطه ی کل بین انعطاف پذیری روان شناختی و خودکارآمدی درد (0/346) می باشد. برای آزمایش مناسب بودن تناسب مدل ها، از شاخص برازش مقتصد هنجار شده (NFI)، شاخص تاکر-لوییس (TLI)، شاخص برازش تطبیقی (CFI) و شاخص برازش افزایشی (IFI) استفاده شد که در محدوده ی قابل قبولی بودند.
نتیجه گیریمتغیر انعطاف پذیری روان شناختی با میانجی گری فاجعه سازی درد، افسردگی و اضطراب، پیش بینی کننده ی خودکارآمدی درد می باشد و به نظر می رسد انعطاف پذیری روان شناختی با تعدیل اثر فاجعه سازی درد، افسردگی، اضطراب، خودکارآمدی درد را افزایش می دهد. در نهایت بهبود این عوامل روان شناختی ممکن است بر روند مدیریت بهتر بیماری تاثیر بگذارد.

مقاله مروریبر اساس تعریف سازمان بهداشت جهانی، ناباروری به ناتوانی زوجین در رسیدن به بارداری پس از حداقل یک سال رابطه ی جنسی محافظت نشده اطلاق می گردد. ناباروری در مردان حدود نیمی از علل مختلف دخیل در ناباروری زوجین را تشکیل می دهد. هدف این مقاله، بررسی سلولی و مولکولی تمایز اسپرم با تمرکز بر ساختار اسکلت سلولی، میکروتوبول ها، رشته های اکتین، پروتیین های حرکتی و غیرحرکتی، بررسی ژن های شناخته شده مرتبط با تشکیل و عملکرد آن ها و پروتیین های دخیل در نقل و انتقال درون سلولی و در نهایت بررسی نقش اساسی آن ها در حفظ مورفولوژی و تحرک اسپرم و متعاقب آن تولید مثل و ناباروری مردان می باشد. اهمیت میکروتوبول ها در فرایندهای مهم تولید، تغییر شکل، بلوغ و توانایی باروری اسپرم تا جایی است که اخیرا واژه ی «میکروتوبولوم» استفاده شده است که نمایانگر میکروتوبول و تمام اجزای مرتبط با میکروتوبول در سلول اسپرم است. با توجه به نیاز فعلی و آینده ی محققین و پزشکان بالینی در حیطه ی ناباروری به این حوزه ی تحقیقاتی بنیادی نوظهور، ابتدا فرایند سلولی تکامل و تمایز اسپرم، سپس محور سیتواسکلتی آکروفراموزوم-آکروپلاکسوم-مانشت مورد بحث قرار گرفته که در مکانیسم های تشکیل و توسعه ی آکروزوم و شکل دهی هسته ی اسپرم و تشکیل تاژک نقش دارد. سپس نقش عملکرد غیر طبیعی اجزاء مختلف اسکلت سلولی اسپرم در ایجاد بیماری های مهم دخیل در ناباروری مردان مورد تجزیه و تحلیل قرار گرفت تا زوایای مبهم این فرایند پیچیده روشن گردد.

Cardiovascular Magnetic Resonance Imaging by using new quantitative mapping techniques such as T2 and ADC mapping, is a new approach to detect ischemia and myocardial edema. In this study, the sensitivity, specificity and diagnostic utility of ADC maps in diffusion imaging compared to T2 maps in the diagnosis of cardiac ischemia were investigated and compared.
MethodsThis is a cross- sectional study that was conducted at Shiraz Peyvand Hospital using 3-Tesla MRI from February 2020 to March 2022 on 60 patients with cardiac ischemia an average of five days after the infarct. For this, images of T2 and ADC maps of the myocardium were obtained and compared. Cine images were used to check the function of the left ventricle, and the infarcted area of the myocardium was defined based on the Late Gadolinium Enhancement images.
FindingsIn the analysis that was done with the segmentation of the left ventricle, the sensitivity and specificity of T2 maps were reported as 84% and 95%, and the sensitivity and specificity of ADC maps were reported as 88% and 97%, respectively.
ConclusionBased on the evaluations, ADC maps showed higher sensitivity and specificity than T2 maps that are commonly used in the clinic, so their use is recommended to investigate ischemia in cardiac patients.

Musculoskeletal pain is one of the most common pains in patients referred to medical centers which may have a psychological basis. The aim of this study was to develop a structural model to explain the mediating role of catastrophizing pain, depression and anxiety between psychological flexibility and pain self-efficacy in patients with musculoskeletal pain.
MethodsIn a cross-sectional study, 200 patients with chronic musculoskeletal pain were selected by convenience sampling method from those referred to medical centers affiliated to Isfahan University of Medical Sciences in 2020-2021. Eligible participants filled out: Chronic Pain Acceptance Questionnaire (CPAQ), Cognitive Fusion Questionnaire (CFQ), Self-Experience Questionnaire (SEQ), Value-based Life Questionnaire (VLQ), Committed Action Questionnaire (CAQ-8), Pain Self-Efficacy Questionnaire (PSEQ) and the Mindful Attention Awareness Scale (MAAS), Pain Catastrophizing Scale (PCS), and Hospital Anxiety and Depression Scale (HADS). Then the data were analyzed using SPSS-24 and AMOS software and statistical methods of regression analysis and structural equations.
FindingsThe results showed that the direct effect coefficient of pain catastrophizing, anxiety, depression and psychological flexibility on pain self-efficacy is (0.357), (0.255), (0.127) and (0.849) respectively. The total relationship between psychological flexibility and pain self-efficacy is (0.346). To test the suitability of the models, NFI, TLI, CFI and IFI indices were used, which were within acceptable limits.
ConclusionThe psychological flexibility variable predicts pain self-efficacy mediated by catastrophic pain, depression and anxiety (P < 0.01, r = 0.85) and it appears that predicting pain catastrophe, depression, anxiety and psychological resilience can increase pain self-efficacy. Ultimately, the improvement of these psychological factors may affect the process of better disease management.

According to the World Health Organization, infertility refers to a couple's inability to conceive following at least one year of regular unprotected sexual intercourse. Male infertility accounts for about half of the various causes of couples' infertility. This review aimed to investigate the cellular and molecular differentiation of spermatozoa, focusing on the structure of the cytoskeleton, microtubules, actin filaments, motor, and non-motor proteins, to study the known genes associated with their formation and function, as well as the proteins involved in intracellular cargo transport, and ultimately investigate their essential role in maintaining sperm morphology and motility and subsequent male reproduction and infertility. The importance of microtubules in the critical processes of sperm production, transformation, maturation, and fertility of spermatozoa is such that the term "microtubule" has been recently used to denote the microtubule and all microtubule-related components in the sperm cell. The cellular process of sperm evolution and differentiation was discussed first, followed by a description of the cytoskeletal system of the acroframosome-acroplaxome-manchette axis, which is involved in acrosome formation and development, sperm head and flagellum shaping mechanisms, in response to the current and future demands of infertility researchers and clinicians in this emerging field of research. The significance of the aberrant function of different components of the sperm cytoskeleton in creating major disorders associated with male infertility was next inspected to clarify the ambiguous aspects of this complex process.
