مقدمه:
اختلال تنفسی خواب، از شایع ترین اختلالات خواب می باشد. هدف از این مطالعه، بررسی ارتباط عملکرد ریوی و آپنه ی انسدادی خواب در کارگران شاغل در صنعت آلومینیوم سازی بود.
روش هااین مطالعه ی توصیفی- تحلیلی در یکی از صنایع آلومینیوم سازی جنوب کشور ایران در سال 1399 انجام شد. ظرفیت ریوی کارگران بر اساس دستورالعمل انجمن متخصصان ریه ی آمریکا ارزیابی گردید. پرسش نامه های برلین جهت ارزیابی آپنه ی انسدادی خواب استفاده شد. داده ها با استفاده از آزمون های آماری T، ضریب همبستگی Pearson و مدل رگرسیون لجستیک چندگانه آنالیز شد.
یافته هادر این مطالعه، 152 نفر از کارگران کارخانه ی آلومینیوم سازی با میانگین سن 6/34 ± 33/63 سال و شاخص توده ی بدنی 3/07 ± 25/06 شرکت کردند. 34/9 درصد کارگران در معرض ریسک بالای آپنه ی انسدادی خواب قرار داشتند. سن (1/37-1/08 = CI 95% ,23/1 = OR)، شاخص توده ی بدنی (2/08-1/18 = CI 95%، 1/57 = OR) و FEV1/FVC (0/98-0/59 = CI 95%، 0/76 = OR) ارتباط معنی داری با افزایش خطر اختلال تنفسی در خواب داشتند (0/001 > P).
نتیجه گیریدر این پژوهش سن، شاخص توده ی بدنی و نسبت FEV1/FVC از پیش بینی کننده های اصلی آپنه ی انسدادی خواب کارگران بودند که می توان از آن ها برای تست های غربالگری و آزمایشات بالینی استفاده نمود.

مقدمه :
عامل اصلی مرگ و میر بیماران مبتلا به دیابت، کاردیومیوپاتی است. ورزش منظم، سبب بهبود مقاومت به انسولین و کاهش مرگ بیماران مبتلا به دیابت می گردد. هدف این پژوهش، مقایسه ی تاثیر شش هفته تمرین هوازی و مقاومتی بر بیان miR195 (خاص میوکارد است) در رت های نر مبتلا به کاردیومیوپاتی مبتلا به دیابت بود.
روش هارت های نر با میانگین وزن 20 ± 200 گرم و سن 8 هفته، پس از القای دیابت (mg/kg STZ 50) به طور تصادفی به چهار گروه 6تایی (شم، شاهد، تمرین مقاومتی و هوازی) تقسیم شدند. یک گروه سالم (6 = n) نیز برای مقایسه با گروه مبتلا به دیابت در نظر گرفته شد. پروتکل های ورزشی به مدت 6 هفته انجام گردید. با استفاده از PCR-Real Time سطح بیان miR195 مورد ارزیابی قرار گرفت. از آزمون های آماری One Way Anova و T همبسته برای تحلیل داده ها استفاده شد.
یافته هاهر دو پروتکل هوازی و مقاومتی منجر به کاهش معنی دار گلوکز خون ناشتا و شاخص مقاومت به انسولین (HOMA-IR) شدند. Vvo2peak گروه هوازی و 1RM گروه مقاومتی افزایش معنی داری پیدا کردند. سطح بیان miR195 گروه شاهد مبتلا به دیابت و شم مبتلا به دیابت نسبت به گروه سالم، تفاوت بیشتری داشت، اما در گروه های تمرینی کمتر بود.
نتیجه گیریتمرین مقاومتی در قیاس با تمرین هوازی، اثر بهتری بر بیان miR195 و شاخص های دیابت داشت. بنابراین توصیه می شود بیماران مبتلا به کاردیومیوپاتی به سمت تمرینات مقاومتی گرایش پیدا کنند.

گزارش مورد:
مقدمه نوروفیبروماتوز، یکی از سندرم های فاکوماتوز است که باعث درگیری ارگان های مختلفی در بدن می شود و به دو نوع تقسیم می گردد. نوروفیبروماتوز نوع 1 می تواند به اشکال مختلف، باعث ایجاد درگیری چشمی شود که می توان به درگیری پلک، ملتحمه، عنبیه یا عصب بینایی اشاره کرد. درگیری عروق شبکیه به صورت ایسکمی شبکیه در این بیماری نادر است و معمولا در دهه های دوم و سوم عمر بروز می کند و با درمان با لیزر یا جراحی کنترل می شود.
گزارش مورد:
در این گزارش، ما به معرفی دختر بچه 3 ساله ی مبتلا به نوروفیبروماتوز نوع 1 می پردازیم که دچار ایسکمی شدید شبکیه شد. این کودک دچار درگیری یک طرفه شبکیه به صورت رتینوپاتی ایسکمیک همراه با خون ریزی شبکیه و زجاجیه در چشم چپ شد و با وجود دو بار جراحی ویترکتومی و لیزر شبکیه چشم چپ، درگیری شبکیه پیشرفت کرد و در نهایت دید چشم چپ بیمار از بین رفت.
نتیجه گیریاین گزارش برای اولین بار درگیری شبکیه در سن پایین بیمار مبتلا به نوروفیبروماتوز نوع 1 و ماهیت مقاوم آن را نشان داد.

Respiratory sleep disorder is one of the most common sleep disorders. The aim of this study was to investigate the factors affecting sleep apnea in workers working in one of the aluminum industries.
MethodsThis descriptive-analytical study was conducted in one of the aluminum industries in the south of Iran in 2021. The lung capacity of the workers was assessed according to the guidelines of the American Association of Lung Specialists. Berlin questionnaires were used to assess obstructive sleep apnea. Data were analyzed using T-test, Pearson correlation coefficient, and multiple logistic regression.
FindingsIn this study, 152 workers in the aluminum industry with an average age of 33.63 ± 6.34 years and a body mass index of 25.06 ± 3.07 participated. 34.9% of workers were at high risk for obstructive sleep apnea. Age (OR = 1.23, 95% CI = 1.08-1.37), body mass index (OR = 1.57, 95% CI = 1.18-2.08), and FEV1 / FVC (OR = 0.76, 95% CI = 0.59-0.98) were significantly associated with an increased risk of sleep apnea (P < 0.001)
ConclusionWorkers, in the aluminum industry, have a higher risk of developing obstructive sleep apnea. Age, body mass index, and FEV1 / FVC are the main predictors of sleep apnea. These variables should be considered in the screening tests and clinical trials.

The main cause of death in diabetic patients is cardiomyopathy. Regular exercise improves insulin resistance and reduces the mortality of diabetic patients. The aim of this study was to compare the effect of six weeks of aerobic and resistance training on the expression of miR195 (specific to the myocardium) in male rats with diabetic cardiomyopathy.
MethodsMale rats with an average weight of 200 ± 20 grams and age of 8 weeks were randomly divided into four groups of 6 (sham, control, resistance and aerobic training) after the induction of diabetes (50 mg/kg STZ). A healthy group (n = 6) was also considered for comparison with the diabetic group. Exercise protocols were performed for 6 weeks. The expression level of miR195 was evaluated using PCR-Real Time. One Way Anova and T-correlated statistical tests were used for data analysis.
FindingsBoth aerobic and resistance protocols led to a significant decrease in fasting blood glucose and insulin resistance index (HOMA-IR). Vvo2peak of the aerobic group and 1RM of the resistance group increased significantly. The expression of miR195 in the diagnostic and solar control groups was more different than the healthy group, but it was less in the exercise groups.
ConclusionCompared to aerobic exercise, resistance training had a better effect on the expression of miR195 and diabetes indicators. Therefore, it is recommended that patients with cardiomyopathy should focus on resistance training.

Neurofibromatosis is one of the phacomatosis syndromes that affects different organs in the body and is divided into two types. Neurofibromatosis type 1 can cause eye involvement in various forms, including eyelid, conjunctiva, iris or optic nerve involvement. Involvement of retinal vessels in the form of retinal ischemia is rare in this disease and usually occurs in the second or third decades of life and is controlled by laser treatment or surgery.
Case Report:
In this report, we introduce a 3-year-old girl with neurofibromatosis type 1 who suffered severe retinal ischemia. This child suffered unilateral retinal involvement in the form of ischemic retinopathy with retinal and vitreous hemorrhage in her left eye, and despite two vitrectomy and retinal laser surgery in the left eye, the retinal involvement progressed and eventually the vision in the left eye was lost.
ConclusionThis report shows for the first-time retinal involvement at a young age in a patient with neurofibromatosis type 1 and its resistant nature.
