عقرب گزیدگی، یکی از موضوعات ناخوشایند مردم می باشد. در شهرستان شوش امکان زیست مناسبی برای عقرب ها وجود دارد. پژوهش حاضر در خصوص پایش میزان عقرب گزیدگی در شهرستان شوش به منظور ارایه ی مستندات برای تصمیم گیرندگان نظام سلامت انجام شد.
روش هادر یک مطالعه ی مقطعی، تمامی پرونده های موارد عقرب گزیدگی که از ابتدای فروردین ماه تا پایان شهریورماه سال 1398 جهت درمان به مراکز درمانی شهرستان شوش مراجعه کرده بودند، مورد بررسی قرار گرفتند. داده های حاصل از میزان عقرب گزیدگی برحسب جنسیت، گروه های سنی، ماه های سال، نوع منطقه (شهری یا روستایی) و عضو در معرض نیش عقرب طبقه بندی و با استفاده از آزمون آماری Wilcoxon، تجزیه و تحلیل شدند.
یافته هامیزان عقرب گزیدگی در میان زنان (50/3 درصد) بیشتر از مردان (49/7 درصد) بود. بیشترین و کمترین میزان عقرب گزیدگی به ترتیب در گروه های سنی زیر 15 سال (30/4 درصد) و بالای 65 سال (1/5 درصد) و ماه های شهریور (23/6 درصد) و فروردین (6/9 درصد) مشاهده شد. میزان عقرب گزیدگی در مناطق روستایی (79/7 درصد) بیشتر از مناطق شهری (20/3 درصد) بود. دست ها (43/2 درصد) نسبت به پاها (32/2 درصد) و سر و تنه (24/6 درصد) بیشتر در معرض نیش عقرب قرار گرفته بودند. آزمون Wilcoxon اختلاف معنی داری بین میزان عقرب گزیدگی برحسب جنسیت، گروه های سنی، نوع منطقه ی شهری یا روستایی و عضو در معرض نیش نشان نداد. اما اختلاف معنی داری بین میزان عقرب گزیدگی برحسب ماه های سال نشان داد.
نتیجه گیریروند میزان عقرب گزیدگی به تدریج از فروردین ماه تا شهریورماه افزایش یافته و در شهریورماه به حداکثر رسید. همچنین روند میزان عقرب گزیدگی از حداکثر اوج خود در گروه سنی زیر 15 سال، به تدریج با افزایش سن به حداقل خود در گروه سنی بالای 65 سال رسید.

مسمومیت با سوپروارفارین ها، فسفید روی و فسفید آلومینیوم، شایع ترین مسمومیت های با جونده کش ها می باشند. هدف از انجام این مطالعه، بررسی فراوانی یافته های ماکروسکوپی و میکروسکوپی حاصل از کالبدگشایی در متوفیان ناشی از مسمومیت با سموم جونده کش بود.
روش هااین مطالعه ی مقطعی در بیمارستان خورشید و مرکز پزشکی قانونی اصفهان در سال 1400 انجام گرفت. داده های مربوط به یافته های پاتولوژی و کالبدگشایی متوفیان ناشی از سموم جونده کش مذکور در طی سال های 1397 تا 1399 از سازمان پزشکی قانونی اصفهان و بررسی گردید.
یافته ها165 بیمار بدلیل مسمومیت با جونده کش های سوپر وارفارینی و فسفید روی و 105 بیمار به علت مسمومیت با فسفید آلومینیوم بستری شده بودند. 38 بیمار (14 درصد از کل بیماران) فوت شدند. مرگ بر اثر سوپر وارفارین ها گزارش نگردید. یک نفر بر اثر فسفید روی فوت کرده بود که کالبد شکافی نشده بود. 37 نفر (35/2 درصد از بیماران فسفید آلومینیوم) به دنبال مسمومیت با فسفیدآلومینیم فوت شدند که از میان آن ها، 33 نفر کالبدگشایی و 5 نفر بررسی پاتولوژی شده بودند. در یافته های پاتولوژی متوفیان ناشی از فسفید آلومینیوم، دژنراسیون کبدی (1 نفر)، ادم ریوی (2 نفر) و خونریزی ریوی (2 نفر) مشاهده و در یافته های کالبدگشایی ادم ریوی (69/6 درصد) و پلورال افیوژن (21/2 درصد) شایع ترین یافته ها بودند. یافته های اتوپسی کبد با میزان بی کربنات خون و پاتولوژی کبد با زمان مصرف سم تا مراجعه، ارتباط معنی داری داشتند.
نتیجه گیریبا توجه به فراوانی ادم ریوی و پلورال افیوژن در متوفیان، توجه به مایع درمانی با کنترل فشار ورید مرکزی در مسمومیت توصیه می گردد.

آدنوم های نفروژنیک در کودکان مشابه بزرگسالان، ضایعات خوش خیم نادری هستند که اغلب در بیماران با سابقه ی جراحی قبلی یا تحریک مزمن مجاری ادراری رخ می دهند. این ضایعات اغلب با هماچوری ظاهر می شوند و یا به صورت ضایعات پولیپوید یا پاپیلاری در سیستوسکوپی تظاهر پیدا می کنند و ممکن است با ضایعات بدخیم تشخیص افتراقی داشته باشند. اکثر بیماران شرح حال اگمنتاسیون مثانه، عفونت ادراری مکرر و سنگ های ادراری را دارند. تاکنون موردی از نفروژنیک آدنوما به دنبال پرولاپس محل وزیکوستومی در اطفال گزارش نشده است.
گزارش مورد:
یک مورد نفروژنیک آدنوما را در مثانه ی پسر 2 ساله و با سابقه ی وزیکوستومی و پرولاپس وزیکوستومی و جراحی مکرر روی مثانه گزارش می کنیم. وزیکوستومی به علت ریفلاکس وزیکویورترال دو طرفه و دریچه ی مجرای خلفی انجام شده بود. پس از انجام دو نوبت جراحی اصلاح محل وزیکوستومی، وزیکوستومی در سن 18 ماهگی به دنبال رزکشن والو مجرای خلفی بسته می شود و در سونوگرافی پیگیری در سن 2 سالگی، توده ی 2 سانتی متری در دیواره ی خلفی مثانه دیده شد و در گزارش پاتولوژی، توده ی رزکت شده نفروژنیک آدنوما گزارش گردید.
نتیجه گیریپرولاپس وزیکوستومی و تحریک مداوم مخاط مثانه به علت آن می تواند زمینه ساز بروز نفروژنیک آدنوما در مثانه کودک باشد.

Scorpionism is one of the unpleasant topics for people. The Shush County has a suitable living environment for scorpions. This research was conducted to monitor scorpionism in Shush to provide documents to health system decision-makers.
MethodsIn a cross-sectional study, all cases of scorpion stings were investigated who were referred to medical centers in Shush County in 2019. The data obtained from the amount of scorpion stings were classified according to gender, age groups, months of the year, type of region (urban or rural) and member exposed to scorpion stings and were analyzed using the Wilcoxon statistical test.
FindingsThe scorpion sting rates among women (50.3%) were higher than among men (49.7%). The highest and lowest scorpion sting rates were observed in the age groups under 15 years (30.4%) and over 65 years (1.5%), and in September (23.6%) and April (6.9%), respectively. The scorpioism rates were higher in rural areas (79.7%) compared to urban areas (20.3%). Hands (43.2%) were more likely to be stung by scorpions than legs (32.2%) and trunks (24.6%). The Wilcoxon signed-rank test did not show significant differences between scorpionism rates related to gender, age groups, urban or rural type, and organs exposed to scorpion stings. But it showed a significant difference between the scorpionism rates among months.
ConclusionThe scorpionism trend gradually increased from April to September to reach its highest peak in September. Moreover, by increasing the age, scorpionism tends from its peak in the age group under 15 years, gradually achieving its minimum in the age group over 65.

Poisoning with super-warfarin, zinc phosphide (ZnP) and aluminum phosphide (ALP) are among the most common poisonings with rodenticides. The purpose of this study was to investigate the frequency of macroscopic and microscopic findings obtained from autopsy in the deceased due to poisoning with these rodenticide poisons.
MethodsThis cross-sectional study was conducted in 2020 in Isfahan Forensic Medical Center and referral poisoning emergency center in Khorshid Hospital, Isfahan, Iran. The autopsy and pathological findings in the deceased cases due to phosphides and super warfarin poisoning from 21 March 2018 to 20 Feb 2021 were evaluated.
FindingsDuring the study period, 165 patients were hospitalized due to super warfarin and ZnP; and 105 patients with ALP poisoning. Death due to super warfarin was not reported. One patient died due to ZnP poisoning who did not undergo autopsy. 37(35.2%) people died from ALP of which 33 cases underwent postmortem examination. 24 (72.7%) of the deceased were men. And the average age was 29.39 years. In the autopsy findings, pulmonary edema (69.6%) and pleural effusion (21.2%) were the most common findings in ALP poisoning. The pathological findings were liver degeneration, pulmonary edema, and pulmonary hemorrhage. There was a significant relationship between serum bicarbonate on admission and liver autopsy finding. Also time from ingestion to hospital admission was correlated with liver pathology finding.
ConclusionConsidering the prevalence of pulmonary edema and pleural effusion in deceased patients with ALP, guiding fluid therapy by controlling central venous pressure should be considered.

Nephrogenic adenomas in children, similar to adults, are rare benign lesions that often occur in patients with a history of previous surgery or chronic irritation of the urinary tract. These lesions often appear with hematuria and appear as polypoid or papillary lesions in cystoscopy and may have a differential diagnosis with malignant lesions. Most of the patients have a history of bladder augmentation, recurrent urinary tract infections, and urinary stones. So far, no case of nephrogenic adenoma has been reported in children following vesicostomy site prolapse.
Case Report:
We report a case of nephrogenic adenoma in the bladder of a 2-year-old boy with a history of vesicostomy and vesicostomy prolapse and repeated surgery on the bladder. Vesicostomy was performed due to bilateral vesicoureteral reflux and posterior vesicoureteral valve. After performing two surgeries to correct the vesicostomy site, the vesicostomy was closed at the age of 18 months following resection of the posterior urethral valve, and in the follow-up ultrasound at the age of 2 years, a 2 cm mass was seen in the posterior wall of the bladder, and in the pathology report of the resected mass Nephrogenic adenoma is reported.
ConclusionProlapse of the vesicostomy and continuous irritation of the bladder mucosa can cause nephrogenic adenoma in the child's bladder.
