هدف پژوهش حاضر، بررسی تمرینات هوازی همراه با مصرف گالیک اسید و کامپفرول بر نوروژنر ناشی از عوارض جانبی مصرف داروی پاکلیتاکسل در موش های مبتلا به سرطان پستان بود.
روش ها36 سر موش ماده BALB/c به صورت تصادفی به 6 گروه تقسیم شدند که شامل گروه شاهد منفی، گروه سرطان پستان (القا با تزریق رده ی سلولی 4T1)، سرطانی- شیمی درمانی شده (10 میلی گرم بر کیلوگرم وزن بدن موش، تزریق پاکلیتاکسل)، سرطانی شیمی درمانی شده+مکمل (گالیک اسید و کامپفرول)، سرطانی شیمی درمانی شده+تمرین هوازی و سرطانی شیمی درمانی شده+مکمل+تمرین هوازی به مدت هشت هفته انجام شد. بیان ژن ها به روش Real Time-PCR سنجیده شد. ژن ها به کمک نرم افزارهای بیوانفورماتیک انتخاب شدند.
یافته هامیزان بیان JAG1 در گروه شیمی درمانی کاهش یافته است. بیان JAG1 در اثر مکمل های گالیک اسید و کامپفرول و تمرینات هوازی بطور معنی داری نسبت به موش های القا شده به سرطان سینه و شیمی درمانی شده کاهش یافته است. مصرف همزمان مکمل های گالیک اسید و کامپفرول همراه با انجام تمرینات هوازی، سبب کاهش معنی دار بیان ژن JAG1 نسبت به سایر گروه ها شده است. میزان بیان ژن های BDNF و NGF در گروه سرطانی شیمی درمانی شده+مکمل، سرطانی شیمی درمانی شده+تمرین هوازی افزایش یافته است. بیان BDNF و NGF در گروه سرطانی شیمی درمانی شده+مکمل+تمرین هوازی بطور معنی دار نسبت به سایر گروه ها افزایش یافت.
نتیجه گیریتمرینات هوازی+مکمل، سبب کاهش عوارض جانبی پاکلیتاکسل و افزایش میزان نوروژنز گردید.

بیماران پسوریازیس تحت درمان با داروهای ایمنوساپرسیو و ایمنومدولاتور، در معرض خطر بیشتر ابتلا به عفونت ها هستند. همچنین عفونت ها می توانند باعث تحریک پسوریازیس شوند. هدف این مطالعه، بررسی میزان ابتلا به عفونت کووید-19 با شدت علایم مختلف در بیماران پسوریازیس تحت درمان داروهای ایمنوساپرسیو و ایمنومدولاتور می باشد.
روش هامطالعه ی حاضر به صورت مقطعی، در تابستان 1400 در شهر اصفهان انجام شد. جامعه ی مورد مطالعه، بیماران پسوریازیس دارای پرونده ی پزشکی الکترونیکی در مرکز درمانی صدیقه طاهره و بیمارستان الزهرا(س) بودند. طی برقراری تماس تلفنی با بیماران، مدت بیماری پسوریازیس، داروی مصرفی، دوز دارو مصرفی، سابقه ی ابتلا به عفونت کووید-19، شدت بیماری (بستری شدن در بیمارستان، فوت) و عود یا تشدید پسوریازیس بعد از ابتلا به عفونت کووید -19، مورد بررسی قرار گرفت.
یافته ها38 درصد بیماران پسوریازیس، تحت درمان داروهای ایمنوساپرسیو و ایمنومدولاتور به کووید-19 مبتلا شده که 75 درصد سرپایی و 23/5 درصد در بیمارستان بستری شده بودند. 39 درصد بیماران مبتلا به پسوریازیس، تحت درمان با (Methotrexate) MTX، 40 درصد افراد تحت درمان باAdalimumab و 48 درصد مصرف کنندگانInfliximab ، سابقه ی ابتلا به کووید-19 را متذکر شدند. در 22 درصد بیماران، عود بیماری پسوریازیس متعاقب کووید-19، مشاهده شد. بیشترین سابقه ی ابتلا به کووید-19، به ترتیب در بیماران پسوریازیس تحت درمان با Infliximab، Adalimumab و MTX گزارش شد.
نتیجه گیریداروهای ایمونوساپرسیو و ایمونومدولاتور با سرکوب سیستم ایمنی و کاهش عملکرد آن می توانند احتمال ابتلا به کووید-19 و شدت آن را در بیماران پسوریازیس افزایش دهند.

حضور آگلوتینین های سرد پلاکتی، پدیده ای نادر است که باعث ترومبوسایتوپنی کاذب غیر وابسته به K-EDTA در دمای آزمایشگاه می گردد. در صورتی که اسمیر خون محیطی بررسی نشود این پدیده می تواند باعث تشخیص نادرست ترومبوسایتوپنی گردد. در این بررسی، شمارش پلاکت مرد 52 ساله ی تحت درمان با همودیالیز در آزمایشگاه از نظر وجود آگلوتینین های سرد بررسی شد.
روش هانمونه ی خون بیمار بر روی ضد انعقاد K3-EDTA و سدیم سیترات جمع آوری و تعداد پلاکت در زمان ها و دماهای مختلف با دستگاه سل کانتر شمارش شد. همزمان با شمارش پلاکت توسط دستگاه، اسمیر خون محیطی، بررسی گردید. هیچ نوع علایم خونریزی در بیمار مشاهده نشد.
یافته هاشمارش پلاکت ها هنگام نمونه گیری L/109×164 بود، اما پس از نگهداری در دمای آزمایشگاه شمارش پلاکت ها به شدت کاهش یافت. همچنین در انتهای اسمیر خون محیطی تعداد زیادی تجمعات پلاکتی (Platelet clump) مشاهده گردید.
نتیجه گیریآگلوتیناسیون سرد پلاکتی، پدیده ای نادر است که با القای ترومبوسایتوپنی کاذب، می تواند خطای آزمایشگاهی را به دنبال داشته باشد. این وضعیت در ترومبوسیتوپنی های شدید بدون علایم بالینی باید در نظر گرفته شود. از آن جایی که آنتی بادی های سرد عملکرد پلاکتی را در In-vivo تحت تاثیر قرار نمی دهند، این افراد هیچگونه علایم بالینی را نشان نمی دهند، اما این وضعیت در شرایطی همچون جراحی هایی که در شرایط هیپوترمی انجام می شوند، باید مورد توجه قرار گیرد.

The research aims to determine the effect of aerobic exercise, Gallic acid, and Kaempferol on neurogenesis caused by the side effects of the paclitaxel drug in mice with breast cancer.
Methods36 female BALB/c mice were randomly divided in to six groups, including the negative control group, breast cancer group (induced with 4T1 cell line injection), cancer-chemotherapy group (10 mg/kg injected paclitaxel), cancer mice treated with chemotherapy+supplement (Gallic acid and Kaempferol), cancer mice treated with chemotherapy+aerobic exercise, cancer mice treated with chemotherapy+supplement+aerobic exercise for eight weeks. The gene expressions were measured by the Real Time-PCR method. Genes were selected using bioinformatics software.
FindingsThe expression level of JAG1 was decreased in the chemotherapy group. Moreover, the expression level of JAG1 was significantly reduced by Gallic acid, Kaempferol supplements, and aerobic exercise compared with the breast cancer and cancer-chemotherapy groups. In addition, Gallic acid and Kaempferol supplements, along with aerobic exercise, significantly declined JAG1 gene expression compared with other groups. The expression levels of the BDNF and NGF genes were increased in the cancer group treated with chemotherapy+supplement and chemotherapy+aerobic exercise group. The expression genes of BDNF and NGF in the cancer group treated with chemotherapy+supplements+aerobic exercise significantly increased compared to other groups.
ConclusionAerobic exercises+supplements declined the side effects of paclitaxel and improved the neurogenesis.

Psoriasis patients treated with immunosuppressive and immunomodulatory drugs are at higher risk of infections, also infections can trigger psoriasis. The aim of this study is to investigate the rate of COVID-19 infection with different severity of symptoms in psoriasis patients treated with immunosuppressive and immunomodulatory drugs.
MethodsThis cross-sectional study was conducted in the summer of 2021 in the Isfahan city. The studied population consisted of psoriasis patients with electronic medical records in Siddiqa Tahereh Medical Center and Al-Zahra Hospital. During phone calls with patients, duration of psoriasis disease, drug used, dose of drug used, history of COVID-19 infection, disease severity (hospitalization, death) and recurrence or exacerbation of psoriasis after COVID-19 infection, were investigated.
Findings38% of psoriasis patients treated with immunosuppressive and immunomodulatory drugs were infected with COVID-19, 75% were outpatients and 23.5% were hospitalized. In psoriatic patients, 39% of whom treated with Methotrexate (MTX), 40% of whom treated with Adalimumab, and 48% of whom treated with Infliximab mentioned a history of COVID-19 disease. In 22% of patients, the recurrence of psoriasis following COVID-19 was observed. The most history of infection with COVID-19 was reported in psoriasis patients treated with Infliximab, Adalimumab, and MTX, respectively.
ConclusionBy suppressing the immune system and reducing its function, immunosuppressive and immunomodulatory drugs can directly increase the chance of contracting COVID-19 and its severity.

The presence of platelet cold agglutinins is a rare phenomenon that causes K-EDTA independent pseudo thrombocytopenia at laboratory temperature. If the peripheral blood smear is not examined, this phenomenon can cause a misdiagnosis of thrombocytopenia.
MethodsThe present study examined the platelet count of a 52-year-old man undergoing hemodialysis in the laboratory for the presence of cold agglutinins. The patient's blood sample was collected using K3-EDTA and sodium citrate anticoagulant. The number of platelets was counted at different times and temperatures with a cell counter machine. Peripheral blood smear was examined at the same time as the platelet count by the device. No sign of bleeding was observed in the patient.
FindingsAt the time of sampling, platelet count was 164x109/L, but it decreased drastically after storage at laboratory temperature. Moreover, at the end of the peripheral blood smear, a large number of platelet clumps were observed.
ConclusionPlatelet cold agglutination is a rare phenomenon that can lead to laboratory error by inducing pseudo thrombocytopenia. This issue has to be taken into consideration in cases of severe thrombocytopenia without clinical symptoms. Since cold antibodies do not affect in-vivo platelet function, such cases do not show any clinical symptoms; however, it has to be considered in situations such as surgeries performed in hypothermic conditions.
