صفحه 1مقدمهبا توجه به اهمیت فیزیولوژیک تنفس از طریق بینی بررسی های زیادی در ارتباط با عوامل درگیر در تبدیل تنفس بینی به بینی دهانی در حین ورزش انجام گرفته است. مطالعه ی حاضر به منظور شناخت نقش مقاومت قدامی بینی به عنوان یکی از عوامل موثر بر این زمان تبدیل صورت گرفت.روش هاتعداد 12 نفر داوطلب سالم و جوان غیر سیگاری و غیر ورزشکار با شاخص توده ی بدنی طبیعی بعد از معاینات پزشکی انتخاب شدند و پس از انجام رینومانومتری قدامی در حالت استراحت، وارد پروتوکل های ورزشی شامل اندازه گیری تهویه، بار کاری و زمان تبدیل اجباری تنفس بینی دهانی (OSP) در حین ورزش شدند. پروتوکل ورزشی به صورت افزایش پلکانی 25 وات در دقیقه در بار کاری تا زمان وقوع تبدیل بود.یافته هااین مطالعه ارتباط معنی داری را بین مقاومت قدامی بینی در حالت استراحت با OSP (Oronasal switching point)، بار کاری و نسبت اختلاف تهویه به OSP نشان داد (05/0 > P).نتیجه گیرینتایج نشان داد که مقاومت قدامی بینی در حال استراحت را می توان به عنوان عامل موثری در وقوع تبدیل تنفس بینی به دهانی بینی در نظر گرفت و کاستن مقاومت مسیر بینی می تواند تحمل تنفس از راه بینی را برای مدت طولانی تری در جریان ورزش افزایش دهد.

صفحه 2مقدمهآندومتریوزیس یک بیماری شایع در زنان می باشد که توسط رشد بافت آندومتر در خارج از حفره ی رحم ایجاد می شود و علت آن همچنان نامعلوم است. کدون 72 اگزون شماره ی 4 ژن 53P دارای پلی مورفیسم شایعی است که زمینه ی ابتلا یه بیماری های متعددی از جمله آندومتریوزیس را فراهم می نماید.روش هادر این تحقیق، بررسی پلی مورفیسم کدون 72 ژن 53TP در 90 بیمار مبتلا به آندومتریوزیس و 90 فرد سالم به عنوان گروه شاهد در شهر اصفهان انجام شد. ژنوتیپ های مختلف کدون 72 ژن 53TP با استفاده از واکنش زنجیره ای پلیمراز (Polymerase chain reaction یا PCR) مشخص شد.یافته هافراوانی ژنوتیپ Arg/Arg (Arginine/Arginine) و Pro/Pro (Proline/Proline) به ترتیب در نمونه های آندومتریوزیس 9/28 و 6/15 درصد و در نمونه های سالم 2/42 و 3/3 درصد بود. فراوانی افراد هتروزیگوت Arg/Pro در گروه آندومتریوزیس 6/55 درصد و در گروه سالم 4/54 درصد بود. در مقایسه بین فراوانی ژنوتیپ Pro/Pro با دو ژنوتیپ دیگر در دو گروه تفاوت معنی دار آماری بین گروه شاهد و آندومتریوزیس دیده شد.نتیجه گیریتحقیق حاضر نشان داد که ژنوتیپ Pro/Pro کدون 72 ژن 53TP یک عامل ژنتیکی مستعد کننده برای ابتلا به آندومتریوزیس در شهر اصفهان می باشد.

تاکنون مطالعات متعددی در زمینه ی بررسی پیامد درمان شکستگی های دو قوزکی انجام گرفته است که نتایج آن ها متفاوت بوده اند. مطالعه ی حاضر به بررسی پیامد این شکستگی ها در بیماران جراحی شده در اصفهان پرداخت.
روش هادر این مطالعه 43 بیمار که از مهر ماه 1388 به دنبال شکستگی دو قوزکی در مراکز آموزشی درمانی شهر اصفهان تحت عمل جراحی قرار گرفتند، بررسی شدند. برای جمع آوری اطلاعات از پرسش نامه ی پیامدهای پا و مچ پا (FAOS یا Foot and ankle outcome score) استفاده شد. بیماران 6 ماه بعد از عمل جراحی نیز با این پرسش نامه ارزیابی شدند. میانگین نمره ی FAOS با توجه به سن، جنس، ابتلا به دیابت، آناتومیک بودن جا اندازی و سیگاری بودن فرد بررسی گردید.
یافته هامیانگین نمره ی FAOS در مردان و زنان تفاوت معنی داری داشت (001/0 > P). میانگین این نمره در بیمارانی که Reduction آناتومیک داشتند نیز با افرادی که فاقد آن بودند، تفاوت معنی داری داشت (001/0 > P). میانگین این نمره در افراد سیگاری با غیر سیگاری متفاوت نبود (39/0 = P). در افراد مبتلا به دیابت و غیر دیابت نیز نمره ی FAOS تفاوت معنی داری داشت (002/0 > P).
نتیجه گیریعواملی نظیر جنس، Reduction نامناسب و ابتلا به دیابت در وضعیت بهبودی و کیفیت زندگی بیماران تاثیرگذار می باشند.

صفحه 4مقدمهیکی از معضلات کنونی جامعه، عدم اطلاع کافی مردم از رفتارهای پرخطر می باشد. در این مطالعه سعی اصلی بر این بود تا با تعیین الگو و شیوع رفتارهای پرخطر در افراد مبتلا به ایدز، بتوان در کنار حمایت و کمک رسانی به برنامه ریزان فن، گام موثری در جهت حل گوشه ای از این معضل خانمان سوز برداشت.روش هامطالعه ی حاضر یک مطالعه ی مقطعی توصیفی- تحلیلی بود که به صورت سرشماری بر روی کلیه ی 1461 بیمار مبتلا به ایدز مراجعه کننده به مرکز مشاوره ی بیماری های دانشگاه علوم پزشکی شیراز بین سال های 88-1383 با در اختیار داشتن اطلاعات مربوط به رفتارهای پرخطر آن ها، جهت تعیین شیوع این عوامل خطر به عنوان رفتارهای پرخطر در کنار توزیع اطلاعات دموگرافیک و فردی، انجام پذیرفت. همچنین ارتباط عوامل خطر با ابتلای به بیماری ایدز با استفاده از آزمون های آماری 2χ و t-test یک نمونه ای در سطح معنی داری 05/0 به دست آمد.یافته هادر این مطالعه 1260 نفر (3/86 درصد) بیمار مرد و 201 نفر (7/13 درصد) بیمار زن بررسی شدند. میانگین سنی بیماران 67/7 ± 5/41 سال بود. از این تعداد 9/78 درصد سابقه ی زندان، 9/75 درصد سابقه ی اعتیاد تزریقی، 1/45 درصد سابقه ی تماس جنسی، 5/32 درصد سابقه ی خالکوبی خارج از زندان، 24 درصد سابقه ی خالکوبی داخل زندان، 2/30 درصد سابقه ی استفاده از تیغ مشترک در داخل زندان و 6/4 درصد سابقه ی استفاده از تیغ مشترک در خارج از زندان را داشتند. همچنین 58/63 درصد مبتلا به هپاتیت C، 04/7 درصد مبتلا به هپاتیت B، 57/6 درصد سابقه ی ابتلا به عفونت های آمیزشی و 43/1 درصد مبتلا به بیماری سل بودند.نتیجه گیریبا توجه به تمام این توضیحات می توان چنین نتیجه گرفت که آموزش روابط جنسی و خطرات اعتیاد تزریقی و مخاطرات حضور در زندان و همچنین آموزش نکات مهم بهداشتی در زمینه ی رفتارهای پرخطر به افراد در معرض خطر که به طور عمده جوانان هستند، از طرف نهادهای ذیربط با سیاست گذاری های صحیح و همکاری های بین بخشی، ضروری است.

صفحه 5مقدمهمایکوباکتریوم بوویس به عنوان عامل سل مزمن گاوی به صورت موردی انسان را نیز درگیر می کند. یک موتاسیون منحصر به فرد در نقطه ی 169(G←C) ژن pncA در مایکوباکتریوم بوویس آن را از مایکوباکتریوم توبرکلوزیس متمایز می کند. استفاده از روش PCR-RFLP برای اولین بار در این تحقیق در جدا سازی این دو ارگانیسم مورد استفاده قرار گرفت.روش هااین مطالعه بر روی 98 بیمار مبتلا به سل با بررسی میکروسکوپی، همراه با کشت و آنتی بیوگرام انجام شد. در قدم بعد، با استفاده از روش های Allele-specific PCR و PCR-RFLP تغییر نوکلئوتید 169 ژن pncA مورد بررسی قرار گرفت و همچنین نتایج حاصل با Spoligotyping مقایسه شد.یافته هااز میان نمونه های مورد بررسی، با روش آنتی بیوگرام 28 نمونه (57/28 درصد) مقاوم، 54 نمونه (1/55 درصد) حساس به پیرازینامید و 16 نمونه (23/16 درصد) فاقد رشد بودند. از 28 نمونه ی مقاوم، 3 نمونه (1/3 درصد) با روش های کلاسیک مایکوباکتریوم بوویس تشخیص داده شدند. با روش های مولکولی از 98 نمونه، 95 نمونه (93/96 درصد) مایکوباکتریوم توبرکلوزیس و 3 نمونه (06/3 درصد) م.بوویس تشخیص داده شدند. مقایسه ی روش های Allele-specific PCR و PCR-RFLP با حساسیت و ویژگی 100 درصد با نتایج Spoligotyping انطباق داشتند.نتیجه گیرینتایج این تحقیق نشان داد که فراوانی مایکوباکتریوم بوویس در جمعیت ایرانی با فراوانی 1/3 درصد، بالاتر از میانگین موارد بررسی شده ی پیشین است (5/0 تا 1 درصد). همچنین نشان داد که روش Allele-specific PCR به علت ارزان تر وسریع تر بودن روش انتخابی است و روش PCR-RFLP به منظور پیش گیری از عدم تطبیق های احتمالی در Allele-specific PCR می تواند جایگزین مناسبی باشد.

Page 1BackgroundSince nasal respiration is physiologically important, many studies have been conducted to find factors necessitating a change from nasal breathing into oronasal breathing during exercise. The present study tried to understand the role of anterior nasal resistance in this switching.MethodsTwelve young, healthy, non-athletic and non-smoker male volunteers with normal body mass index (BMI) were selected after medical examinations. Following anterior rhinomanometry at rest, the subjects participated in an exercise protocol during which ventilation, workload and oronasal switching point (OSP) were measured. The protocol involved a 25 watt increase in workload per minute until the OSP was reached.FindingsThere were significant correlations between anterior nasal resistance and OSP, workload, and ventilation difference to OSP ratio (P < 0.05).ConclusionAnterior nasal resistance can be considered as an effective factor on OSP occurrence. In addition, by reducing nasal resistance, one can tolerate longer periods of nasal respiration during exercise.

Page 2BackgroundEndometriosis is a common disease among women which is a result of endometrial tissue growth outside the uterus. However, its cause is still unclear. This study surveyed TP53 codon 72 gene polymorphism among 90 patients suffering from endometriosis and 90 healthy subjects, as the control group, in Isfahan.MethodsDifferent genotypes of TP53 codon 72 gene were determined by polymerase chain reaction (PCR).FindingsThe frequency of Arg/Arg (Arginine/Arginine) genotype in subjects with endometriosis and healthy individuals were 28.9% and 42.2%, respectively. In addition, the frequency of Pro/Pro (Proline/Proline) genotype in patients with endometriosis and healthy participants were 15.6% and 3.3%, respectively. The frequency of heterozygotes Arg/Pro was 55.6% in endometriosis patients and 54.45% in healthy subjects. Comparing the frequency of Pro/Pro genotype with the two other genotypes in both groups revealed a statistically significant difference between the control and endometriosis groups [P < 0.05; CI = 95%; OR = 5.34 (range: 1047-19.29)].ConclusionThe present research showed that Pro/Pro genotype of TP53 codon 72 gene is a predisposing genetic factor for endometriosis occurrence in Isfahan.

Various studies in the field of bimalleolar ankle fractures resulted in different findings. However, no reported study is available in Iran.
MethodsThe study population included 43 patients with bimalleolar ankle fractures who referred to hospitals associated with Isfahan University of Medical Sciences and underwent surgery during 2009. Foot and Ankle Outcome Score (FAOS) was used to collect data 6 months after the surgery. Mean scores were evaluated considering different factors such as age, sex, having diabetes, smoking and anatomic reduction.
FindingsA significant difference was found in FAOS scores between men and women (P = 0.001) and between those with and without anatomic reduction (P < 0.001). However, the scores were not significantly different between smokers and non-smokers (P = 0.39). In addition, diabetic and non-diabetic patients were significantly different in terms of FAOS scores (P = 0.002).
ConclusionFactors such as sex, inappropriate reduction and having diabetes can affect the healing process and quality of life among patients with bimalleolar ankle fractures.

The Survey of Risk Factors in HIV Positive Patients Covered by Shiraz University of Medical SciencesPage 4BackgroundUnawareness of people about high risk behaviors lead in major problems in the present Iranian society. In this study, we tried to determine the prevalence of high risk behaviors among HIV positive patients. We also aimed to provide a pattern for these behaviors to help both policy makers and patients.MethodsThis descriptive-analytic cross-sectional study used a census method to evaluate 1461 HIV patients that referred to the Disease Counseling Center at Shiraz University of Medical Sciences during 2003-2008. The study analyzed the information about high risk behaviors among these patients and extracted the risk factors, as well as their demographic and personal characteristics. In addition, the correlations between HIV and risk factors were determined by chi-square test and single-sample t-test at a significance level of 5%.FindingsThis Study involved 1260 (86.3%) men and 201 (13.7%) women with a mean age of 41.5 ± 7.67 years. Among these patients, 78.9% had a history of imprisonment, 75.9% experienced drug injection, 45.1% had a history of sexual relationships, 32.5% tattooed outside the prison while and 24% tattooed inside the prison. Moreover, 30.2% and 4.6% had used a common razor inside and outside the prison, respectively. Hepatitis C and B and tuberculosis were found in 63.58%, 7.04% and 1.43% of the subjects, respectively. Finally, 6.57% had a history of sexually transmitted infections.ConclusionBased on our findings, government and policy makers need to educate the youth on sexual relations, risks of drug injection and imprisonment, and also main health issues regarding high risk behaviors.

Page 5BackgroundBovine tuberculosis is a chronic bacterial disease of cattle that occasionally affects human. A unique mutation at position 169 (C→G) in Mycobacterium bovis differentiates it from Mycobacterium tuberculosis. For the first time, PCR-RFLP method was used in this study to separate the two organisms.MethodsIn this study, 98 tuberculosis patients were assessed using microscopic evaluations, culture and antibiogram. Then, nucleotide changes in position 169 were investigated using Allele-specific PCR and PCR-RFLP methods. Finally, the results were compared with spoligotyping results.FindingsAntibiogram revealed 28 subjects (28.57%) to be resistant and 54 (55.1%) to be sensitive to pyrazinamide while and 16 cases (16.23%) showed no growth. Of 28 resistant patients, 3 (3.1%) were diagnosed with M. Bovis using the classical methods. Using molecular methods, 95 (96.93%) out of 98 subjects were diagnosed with M. tuberculosis and 3 (3.06%) with M. bovis. Allele-specific PCR and PCR-RFLP matched with spoligotyping results.ConclusionOur result showed the incidence of M. bovis infection to be higher in Iranian population (3.1%) than other studied cases (0.5-1%). In addition, we showed that Allele-specific PCR is the method of choice because it is faster and cheaper than PCR-RFLP. However, PCR-RFLP could be a proper alternative to prevent probable mismatches in Allele-specific PCR.
