Impact of Health evolution Plan on common surgical procedures tariffs (Global): Longitudinal study 2011-2017

A case payment is used in Global payment system. The third phase of the Health evolution Plan emphasizes on the reality of tariffs and their increase, the purpose of this study is to investigate the impact of health evolution on global health services tariffs. This study was a descriptive and longitudinal study which was conducted to evaluate the Iranian common surgeries (Global) surgeries from 2011-2017. Data source was costs table of Global Surgery, which is provided annually by the Ministry of Health and Medical Education to the relevant units. California Tariff Book (2013 Edition) and Relative Value of Health Services and Healthcare (1395) were used. Then, the weighted average of the relative value of the anesthetic and surgical services was calculated by the group and the percentage of changes was compared. The basic data was categorized and the weighted average and percentage of changes were calculated using excel software. Data were analyzed by SPSS software version 18 and paired t-test. After eplimentation of health evolution plan, the average unit of surgery growth 124% in first year and 8.6 % in second year. While the average unit of anesthesia in the first year after the development plan growth 19.8%, and it decreased by 2.9% in the second year. Also, the rate of increase in the average tariff on the basis of the hospital grade has also increased over the course of six years, and in the year 2013 has a jump of almost 80 percent. The California book and the book of New Relative Value were 25,976 K and 22,307 Anesthesia, respectively, in all 13 specialist groups. (14.1% decreased). The use of exact methods of determine the tariff based on community realities and the use of case payment  and based on prospective payments methods can play a significant role in controlling health costs.
