How to Start DMEK Step by Step
Descemet membrane endothelial keratoplasty (DMEK) is lamellar corneal transplantation that involves the removal of endothelium and Descemet membrane of the recipient and subsequent transplantation of endothelium and Descemet membrane from a donor. To date, DMEK revealed promising outcomes, including the fast visual recovery, the better final visual acuity, and the lower risk of allograft rejection. In the present review article, firstly, an introduction to endothelial keratoplasty is explained with the focus on similarities and differences between DMEK and Descemet membrane automated endothelial keratoplasty (DSAEK). Then, the method of performing DMEK preferred in authors’ experience is reviewed in a stepwise fashion. The steps include preparation of the donor tissue, corneal incisions, removal of the recipient’s tissue, insertion of the graft, and conclusion of the surgery. Finally, the postoperative care of the patients is briefly mentioned.
