صفحه 1مقدمهبی ثباتی وضعیتی، یکی از پیامدهای مهم بیماری پارکینسون است که موجب کاهش تحرک و کیفیت زندگی مبتلایان به این عارضه می شود. انجام تمرین و فعالیت بدنی، جزیی از برنامه ی درمانی برای کاهش عوارض پارکینسون محسوب می شود. در سال های اخیر، انجام تمرین در آب برای افراد دارای سطح تحرک پایین توصیه می شود. هدف این مطالعه، مقایسه ی تاثیر یک برنامه ی تمرین درمانی در دو محیط آب و خشکی بود.روش ها20 مرد مبتلا به پارکینسون ایدیوپاتیک بر اساس شدت بیماری و طبق تشخیص پزشک متخصص، در دو گروه 10 نفری تمرین در آب و تمرین در خشکی تقسیم شدند. هر دو گروه به مدت 8 هفته و هر هفته 3 جلسه، برنامه ی تمرین درمانی را زیر نظر یک مربی انجام دادند. ثبات قامتی و کیفیت زندگی شرکت کنندگان به ترتیب توسط صفحه ی نیرو و پرسش نامه در دو مرحله ی قبل و بعد از مداخله سنجیده شد. با استفاده از نرم افزار SPSS و آزمون های آماری Repeated-measures ANCOVA، Shapiro-Wilk و t مستقل داده های ثبت شده تجزیه و تحلیل و مقایسه گردید.یافته هاهر دو دسته از متغیرهای ثبات وضعیت بدن و کیفیت زندگی افراد، در پس آزمون بهبود معنی داری نسبت به پیش آزمون نشان دادند. همچنین، میانگین بهبود متغیرها در گروه تمرین در آب به صورت معنی داری بیش از گروه خشکی بود.نتیجه گیریتمرین درمانی در محیط آب، روش مفیدتری نسبت به خشکی، جهت افزایش تعادل و کیفیت زندگی مبتلایان به پارکینسون است.

صفحه 2مقدمهزردی یکی از شایع ترین بیماری های دوران نوزادی است. اگر زردی در نوزادان رسیده بیش از 14 روز و در نوزادان نارس بیشتر از 21 روز طول کشیده باشد، زردی طول کشیده نامیده می شود. عفونت ادراری در دوره ی نوزادی و کودکی یک بیماری بالقوه خطرناک محسوب می شود و تکرار آن در این دوره ها، می تواند موجب مشکلات زیاد و دایمی شود که باعث ایجاد عوارض جبران ناپذیری می گردد. از این رو، مطالعه ی حاضر با هدف شناسایی عوامل موثر بر عفونت ادراری در نوزادان مبتلا به زردی طول کشیده، بستری در بخش مراقبت های ویژه ی نوزادان (Neonatal intensive-care unit یا NICU) در شهرستان کرمانشاه در سال های 91-1389 انجام گردید.روش هادر این مطالعه ی توصیفی- تحلیلی، 1844 نوزاد مورد بررسی قرار گرفتند. 91 نوزاد که به دلیل زردی طول کشیده در طی 3 سال در بخش NICU بستری بودند، وارد مطالعه شدند. نمونه گیری به روش در دسترس با استفاده از داده های موجود در پرونده ی بیماران انجام گردید. داده ها با استفاده از فرم اطلاعاتی محقق ساخته که شامل اطلاعات دموگرافیک، نوع زایمان مادر، نوع تغذیه ی بیمار، وزن هنگام تولد بیمار، سن حاملگی، وجود یا عدم وجود عفونت ادراری و نسبت والدین بود، جمع آوری گردید و با استفاده از نرم افزار SPSS و آمار توصیفی و استنباطی تحلیل شد.یافته هامیزان شیوع عفونت ادراری در نوزادان مبتلا به زردی طول کشیده، 3/ 59 درصد بود. جنسیت، سن حاملگی، نوع زایمان و نسبت والدین، با ابتلا به عفونت ادراری ارتباطی نداشتند و از لحاظ آماری معنی دار نبودند. بین نوع تغذیه و عفونت ادراری (05/ 0 > P) رابطه ی معنی دار دیده شد.نتیجه گیرینوع تغذیه ی نوزادان در بروز عفونت ادراری تاثیر دارد و توصیه می شود، تغذیه با شیر مادر در اولویت باشد. همچنین، توصیه می شود در نوزادانی که زردی آن ها بیش از 2 هفته طول می کشد، یا علت مشخصی برای زردی آن ها یافت نمی شود، آزمایش ادرار انجام گردد.

صفحه 3مقدمهباکتری Escherichia coli، توانایی ایجاد عفونت مجاری ادراری را دارد و تحت عنوان سویه های ایجاد کننده ی عفونت ادراری (UPEC یا Uropathogenic Escherichia coli) نامیده می شود. Escherichia coli عامل عفونت ادراری انواع مختلفی از Adhesin مانند Adhesinهای پیلی (PaP یا Pyelonephritis associated pili) را بیان می کند که واسطه ی اتصال به سطح سلول های اپی تلیال مجاری ادراری می باشند. پیلی یا فیمبریه ی P، کلونیزاسیون باکتری را تسهیل می کند، از حذف باکتری توسط جریان فیلتراسیون ادراری جلوگیری می نماید و قدرت تکثیر و تهاجم به بافت کلیه را افزایش می دهد. Adhesin نوع papG در نوک پیلی P قرار دارد و دارای سه کلاس متفاوت است. این مطالعه، با هدف شناسایی و تعیین فراوانی ژن های کد کننده ی Adhesin نوع papG در Escherichia coli ایجاد کننده ی عفونت ادراری انجام شد.روش هادر این تحقیق، 55 نمونه ی ادراری افراد مبتلا به عفونت دستگاه ادراری، از آزمایشگاه های بالینی شهر تهران جمع آوری شد. پس از جداسازی باکتری و استخراج DNA، حضور ژن های papG I، papG II و papG III با استفاده از روش Multiplex PCR (Multiplex polymerase chain reaction) مورد بررسی قرار گرفت.یافته ها18 نمونه (7/ 32 درصد) واجد ژن papG بودند. از بین 18 نمونه ی واجد ژن papG، 17 نمونه (9/ 30 درصد) دارای ژن papG II و 1 نمونه (8/ 1 درصد) واجد ژن papG III بود. در هیچ یک از نمونه ها، ژن papG I شناسایی نشد.نتیجه گیریژن papG II شایع ترین ژن کد کننده ی Adhesin نوع papG فیمبریه ی P در Escherichia coli جدا شده از عفونت دستگاه ادراری در شهر تهران است. این مساله، می تواند اطلاعات با ارزشی را در آسیب شناسی عفونت دستگاه ادراری و راهکارهای درمانی پیش رو، فراهم آورد.

صفحه 4مقدمهلرز، حرکات و انقباضات عضلانی غیر ارادی است که بیمار قادر به کنترل آن ها نمی باشد. لرز پس از بیهوشی عمومی، عارضه ی شایعی است که به علت استفاده از داروهای بیهوشی و هیپوترمی در زمان بیهوشی عمومی ایجاد می شود و می تواند باعث افزایش مصرف اکسیژن، افزایش فشار خون سیستمیک، افزایش ضربان قلب و آریتمی های قلبی در بیماران گردد. یکی از راهکارهای کاهش لرز پس از بیهوشی، استفاده از داروهایی است که لرز کمتری پس از بیهوشی ایجاد می کنند. در هنگام بیهوشی، از داروهای متنوعی استفاده می شود که هر کدام عوارض خاص خود را دارند. هدف از انجام این مطالعه، مقایسه ی اثر داروهای ایزوفلوران و پروپوفول بر لرز پس از بیهوشی در بیماران تحت بیهوشی عمومی جهت اعمال جراحی الکتیو عمومی و ارتوپدی بود.روش هادر این مطالعه ی کارآزمایی بالینی دو سو کور، 100 بیمار 65-18 ساله که طی سال های 92-1391 در بیمارستان امام خمینی (ره) ایلام، تحت عمل جراحی الکتیو ارتوپدی و عمومی قرار گرفته بودند، به صورت تصادفی در دو گروه 50 نفره تقسیم بندی شدند. وقوع و شدت لرز بعد از عمل در دو گروه مقایسه شد و با امتیاز بین 3-0 درجه بندی گردید. داده ها با استفاده از نرم افزار SPSS و آزمون های Paired t و 2χ مورد تجزیه و تحلیل قرار گرفت.یافته ها47 درصد بیماران، زن و 53 درصد، مرد بودند. بین دو متغیر جنسیت و لرز پس از بیهوشی عمومی (067/ 0 = P) و همچنین، بین دو متغیر سن و لرز پس از بیهوشی عمومی (774/ 0 = P)، ارتباط معنی داری وجود نداشت. بین بیماران، در رابطه با لرز پس از بیهوشی به تفکیک بیهوشی با ایزوفلوران و پروپوفل تفاوت معنی داری مشاهده نشد (211/ 0 = P).نتیجه گیریدر بیماران بزرگ سال تحت عمل جراحی الکتیو ارتوپدی و عمومی، جهت کاهش لرز بعد از عمل، استفاده از ایزوفلوران فرقی با پروپوفول ندارد.

صفحه 5از دهه ی 1970، جریان یکپارچه شدن ذهن آگاهی با روان درمانی شروع شد. درمان های مبتنی بر ذهن آگاهی از جمله ذهن آگاهی مبتنی بر کاهش استرس، کاربرد وسیعی در درمان بیماران مبتلا به شرایط طبی مزمن دارند. این درمان ها تاکید می کنند که آگاهی نسبت به لحظه حال و پذیرش آن، از جنبه های مهم درمانی است. این اختلالات، از مشکلات بزرگ بهداشت عمومی هستند که کمتر درمان موثری برای آن ها روی کار آمده است. نشانگان جسمی عملکردی و جسمی سازی است که با اصطلاحی کلی تر، تحت عنوان نشانگان ناراحتی بدنی یکپارچه شده اند. در این اختلالات، پایه های جسمی مشخصی برای تبیین نشانه های بیماران یافت نمی شود. یکی از درمان های جدید در این حوزه، درمان های ذهن آگاهانه است که با آموزش آگاهی بدنی، ذهنی و ارتباط بین این دو، منجر به افزایش ماده ی خاکستری مغز در شکنج اینسولا، هیپوکامپ و کورتکس پیش پیشانی، افزایش فعالیت نواحی بالابرنده ی خلق در کورتکس پیشانی چپ، تقویت سیستم ایمنی و بهبود عملکرد روان شناختی مرتبط با پذیرش خود می گردد و علایم اختلال را کاهش و بهبود می دهد.

Page 1BackgroundPostural instability is a major complication of Parkinson’s disease (PD) leads to the reduced mobility and poor quality of life in patients. Physical activity and exercise is a part of treatment plan to reduce the symptoms of the disease. Water-based exercise is recommended for patients with a lower level of mobility. This study aimed to compare the effects of water- and land-based exercises on patients with Parkinson’s disease.Methods20 patients who were diagnosed to have idiopathic Parkinson’s disease by a neurologist were equally divided into two 10-people groups based on their severity of complications. Both groups completed an 8-weeks (3 sessions per week) exercise program. Postural stability and quality of life (QoL) parameters were recorded using a force-plate and questionnaire in pre- and post-test sessions. Recorded outcomes were analyzed and compared using analysis of covariance statistical test.FindingsPostural stability and quality of life parameters improved at the post-test session compared to pre-test in both groups. The improvement of these outcomes was significantly greater in the water-based group.ConclusionWater-based exercises can be more beneficial to improve the balance ability and quality of life in patients with Parkinson’s disease.Keywords: Parkinson's disease, Exercise therapy, Postural stability, Quality of life

Page 2BackgroundJaundice is the most common disease in neonates; prolonged jaundice lasts for more than 14 days in neonates and more than 21 days in premature neonates. Urinary tract infection (UTI) is a potentially serious disease during infancy and childhood and with replication can cause many problems and irreversible effects. This study aimed to identify the factors affecting urinary tract infection in hospitalized neonates in neonatal intensive-care unit (NICU) with prolonged jaundice.MethodsIn this cross-sectional study, from 1844 studied infants during 3 years of 2010-2013, 91 infants admitted to the NICUs of hospitals in Kermanshah city, Iran, due to prolonged jaundice, were enrolled. Via convince sampling, data were collected using a checklist including the demographic data, type of the delivery, type of neonate feeding, birth weight, gestational age, presence or absence of urinary tract infection and parent's kinship. Data were analyzed using descriptive and inferential statistics via SPSS software.FindingsThe prevalence of urinary tract infection in neonates with prolonged jaundice was 59.3 percent. Gender, gestational age, type of delivery, and parent's kinship were not related to urinary tract infection. Feeding type and urinary tract infection were significantly related (P < 0.05).ConclusionFeeding type has an impact on urinary tract infection and breastfeeding is a priority. In addition, urine culture in neonates with prolonged jaundice, or those with unknown cause, may be advantageous.Keywords: Neonatal, Jaundice, Urinary tract infection (UTI)

Page 3BackgroundEscherichia coli strains with ability to cause urinary tract infection (UTI) are called uropathogenic strains. Uropathogenic Escherichia coli expresses a variety of cell adhesions, such as adhesion PaP (pyelonephritis associated pili), that causes the urinary tract infection via binding to the surface of epithelial cells. P pili facilitates the colonization of bacteria, prevents the elimination of bacteria via flow of urine filtration and increases kidney tissue proliferation and invasion. PapG adheres at the tip of P pili, and has three different classes. This study aimed to identify and determine the frequency of encoding papG genes in uropathogenic Escherichia coli.MethodsIn this study, 55 samples of patients with urinary tract infection werecollected from clinical laboratories in Tehran city, Iran. After the extraction of bacteria DNA, the papG I, papG II and papG III genes were investigated using multiplex polymerase chain reaction (Multiplex PCR) method.Findings18 samples (32.7%) were papG gene positive. Of those 18 samples, 17 (30.9%) were papG II, and 1 (1.8%) was papG III gene positive. PapG I gene was not detected in any of the samples.ConclusionThe results showed that papG II gene is the most common gene encoding adhesion papG of the P pili in Escherichia coli isolated from urinary tract infections in Tehran. Page 4BackgroundChills are muscle contractions and involuntary movements that the patient is not able to control them. Chill (shivering) is a frequent complication after the general anesthesia, which is due to the use of anesthesia drugs and hypothermia; it can increase oxygen intake, systemic blood pressure, and heart rate and can cause cardiac arrhythmias in patients. One of the mechanisms for decreasing postoperative shivering, is the use of the drugs that create less shivering. A variety of anesthetic drugs are used during anesthesia that each one has its own special side effects (complications). The aim of this study was to compare the effects of isuflorane and propofol on postoperative shivering in patients underwent general anesthesia for elective general or orthopedic surgeries.MethodsIn a double-blinded clinical trial study, 100 patients at the age of 18-65 years, who underwent elective orthopedic and general surgery in Imam Khomeini Hospital in city of Ilam, Iran, were randomly divided in two groups of 50 during the years 2012-2013. The occurrence and severity of postoperative shivering was compared in the two groups and via 0-3 classification scoring. The findings were analyzed via SPSS16 software using paired-t and chi-square tests.Findings47% of the patients were women and 53% were men. Between the gender and postoperative shivering (P = 0.067) as well as between the age and postoperative shivering (P = 0.774), no significant relationship was found. There was no significant difference between the two groups of isoflurane and propofol in postoperative shivering (P = 0.211).ConclusionIn adult patients undergoing elective general or orthopedic surgery, to reduce the postoperative shivering, there was no difference in using isoflurane or propofol.Keywords: Isoflurane, Propofol, Postoperative shivering

Page 5Mindfulness and psychotherapy integration movement has begun since the 1970s. Mindfulness-based therapies such as mindfulness-based stress reduction (MBSR), are widely used in the treatment of patients with chronic medical conditions. These therapies focus on the present moment awareness and acceptance as an important aspect of treatment. One of the major public health problems is functional somatic syndromes and somatization. There are a few effective treatments for them. These two problems have been united with the term as bodily distress syndrome (BDS); the clear somatic basis has not been found. Of the new therapies in the treatment of bodily distress syndrome are mindfulness-based therapies. These therapies, via mind and body awareness training and connection between mind and body, increase brain gray matter in the insula, hippocampus and prefrontal cortex. They increase activity of the left frontal areas elevating mood and boost the immune system, too. These therapies improve mental performance related to its acceptance and decrease the symptoms of bodily distress syndrome.Keywords: Bodily distress syndrome, Mindfulness, Mindfulness, based stress reduction
