کیست هیداتیک، جزء بیماریهای زیونوز است و در حال حاضر، یک معضل بهداشتی در بسیاری از مناطق جهان از جمله در ایران به شمار میرود. داشتن اطلاعات کافی اپیدمیولوژیک جهت مراقبت، نظارت و کنترل موثر بیماری لازم است. این مطالعه، با هدف ارزیابی وضعیت اپیدمیولوژی، کلینیکی و پاراکلینیکی بیماری هیداتیدوز در مرکز آموزشی- درمانی الزهرای (س) اصفهان در بازهی زمانی 8 ساله انجام شد.
روشها: در یک مطالعهی توصیفی گذشتهنگر، اطلاعات دموگرافیک (سن، جنس، شغل و غیره)، علایم بالینی و روشهای تشخیصی (تصویربرداری، سونوگرافی و سرولوژی) از پروندهی پزشکی بیماران بستری شده در بیمارستان طی سالهای 95-1387 اخذ شد.یافتهها:
در کل، از 698 بیمار، 352 نفر (4/50 درصد) مرد و 346 نفر (6/49 درصد) زن بودند که 647 بیمار به کمک یافتههای بالینی، پیش بالینی و پاتولوژی تشخیص هیداتیک داده شده بود. محدودهی سنی بیماران 87-3 سال و بیشترین شیوع بیماری در گروه سنی 40-20 (39 درصد) بود. بیشترین عضو درگیر (8/74 درصد) کبد و بیشترین موارد کیستهای کبدی (4/72 درصد) در لوب راست بود.
نتیجهگیری:
نتایج نشان داد که مواردی از کیستهای هیداتیک قبل از جراحی با روشهای تصویربرداری و سرولوژی تشخیص داده نشده بودند یا برعکس، مواردی از تشخیص کیست هیداتیک که بعد از عمل جراحی مشخص گردید که هیداتیک نبوده است. این مساله، نمایانگر این است که جهت تشخیص کیست هیداتیک، نیاز به روشهای تشخیصی مطمینتر و دقیقتر میباشد

پرتودرمانی هدفمند، به عنوان یکی از روشهای رایج برای تسکین درد در بیماران مبتلا به متاستاز استخوانی محسوب میشود. در این نوع روش درمانی نیاز است که دز جذبی اندام بحرانی مغز استخوان به عنوان یک عامل محدود کنندهی درمان در نظر گرفته شود. تاکنون پرتوداروهای بتازا برای این نوع درمان مورد توجه بودند که یکی از متداولترین آنها Samarium-153-Ethylene diamine tetramethylene phosphonate (153Sm-EDTMP) است، اما به تازگی پرتوداروی آلفازای Radium-223-dichloride (223Ra-Cl2) مورد تایید سازمان غذا و داروی آمریکا (Food and Drug Administration یا FDA) قرار گرفته است. هدف از انجام مطالعهی حاضر، مقایسهی دز جذبی مغز استخوان پرتوداروهای 153Sm-EDTMP و 223Ra-Cl2 بود.
روشها:
در این مطالعه، از یک فانتوم استخوان مهره برای شبیهسازی استفاده شد. پرتوداروهای در نظر گرفته شده در فانتوم توزیع شدند و شبیهسازی با استفاده از کد مونتکارلوی GATE صورت پذیرفت. بعد از برآورد دز جذبی به ازای واپاشی در بخشهای مختلف فانتوم، دز جذبی پرتوداروها در مغز استخوان یک بیمار 70 کیلوگرمی با استفاده از روش Medical internal radiation dosimetery (MIRD) محاسبه شد.
یافتهها:
برای 153Sm-EDTMP دز جذبی مغز استخوان به ازای اکتیویتهی تجویزی 2590 مگابکرل برابر با 60/9 گری به دست آمد. برای 223Ra-Cl2 دز جذبی مغز استخوان به ازای اکتیویتهی تزریقی 85/3 مگابکرل، 83/0 گری محاسبه شد.
نتیجهگیری:
با توجه به کمتر بودن دز جذبی مغز استخوان توسط پرتوداروی آلفازای 223Ra-Cl2 بنابراین استفاده از این پرتودارو به عنوان یک جایگزین برای پرتوداروی بتازای 153Sm-EDTMP برای درمان تسکینی بیماران مبتلا به متاستاز استخوانی پیشنهاد میشود.

تشخیص به موقع بیماری COVID-19 برای بهبود پیامد بیماری و قطع زنجیرهی انتقال، ضروری است. آگاهی از راهکارهای تشخیصی Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)، چالشهای آنها، جمعآوری با ارزشترین نمونهی تشخیصی در زمان مناسب از محل آناتومی مناسب و تفسیر یافتههای آنها اهمیت زیادی دارد. این مطالعهی مروری، با هدف بررسی چالشهای فعلی و تفسیر واکنش زنجیرهای پلیمراز رونویسی معکوس (Reverse transcription polymerase chain reaction یا RT-PCR) به عنوان روش مرجع، آزمایش Loop-mediated isothermal amplification (LAMP)، تشخیص آنتیبادی و آنتیژن، ویژگیهای تصویربرداری ریه، تغییرات غیر طبیعی در یافتههای آزمایشگاهی مبتلایان به COVID-19 و تغییر نتایج با گذشت زمان انجام شد. نمونههای مایع لاواژ برونکوآلویولار (93 درصد) و خلط (72 درصد)، بالاترین میزان مثبت را در تشخیص مولکولی COVID-19 نشان میدهند. از طرف دیگر، سنجشهای مکرر RT-PCR توصیه میشود؛ چرا که با گذشت زمان، احتمال حضور SARS-CoV-2 در نازوفارنکس افزایش مییابد. ترکیب شواهد بالینی با نتایج توموگرافی کامپیوتری قفسهی سینه (Computed tomography scan یا CT scan) و RT-PCR، میتواند خطاهای تشخیصی را به حداقل برساند. تصور میشود که سطح بالای اینترلوکین 6 (IL-6) و D-dimer با بروز فرم شدید COVID-19 در بزرگسالان ارتباط نزدیکی دارد و تشخیص ترکیبی آنها، میتواند به عنوان عوامل اولیهی پیشبینی شدت بیماری باشد. همچنین، افزایش پروتئینهای مرحلهی حاد با پیامد نامطلوب بیماری COVID-19 همراه است. تشخیص سرولوژی، همچنین ابزار مهمی برای درک میزان COVID-19 در جامعه و شناسایی افراد مصون است. سطح آنتیبادیها از هفتهی دوم بیماری شروع به افزایش میکند.

Hydatidosis is a zoonosis disease, and an important public health problem in many areas of the world including Iran. Sufficient epidemiological information is required for taking care, monitoring, and effective control of the disease. In the present study, we evaluated epidemiological, clinical, and paraclinical characteristics of hydatidosis in the Alzahra hospital in Isfahan University of Medical Sciences, Isfahan, Iran, over a 9-year period.
MethodsIn a descriptive cross-sectional study, demographic information (age, sex, job, etc.), clinical signs, and diagnosis methods (imaging, sonography, and serology) were obtained from the medical record of hospitalized patients during the years 2008-2016.
FindingsOverall, from 698 patients, 352 men (50.4%) and 346 women (49.6%), 647 patients were diagnosed as having hydatid cyst by clinical and paraclinical findings, and pathologic documentation. Patients' age ranged from 3 to 87 years, and the peak prevalence of the disease was between 20 and 40 years (39% of cases). The involvement of liver was the most frequent (64.8%) site, and most cases of liver cysts (54.2%) were in the right lobe.
ConclusionThe results showed that some cases of hydatid cysts were not diagnosed before surgery by imaging and serology method; on the contrary, cases of diagnosis of hydatid that were determined to be non-hydatid cyst after surgery. This suggests that hydatid cyst diagnosis requires more reliable and accurate diagnostic methods.

Targeted radiopharmaceutical therapy is one of the most common methods for relieving pain in patients with bone metastasis. In this type of treatment, the bone marrow absorbed dose, as a critical organ, needs to be considered as a limiting factor. Until now, beta-emitter radiopharmaceuticals have been considered for this treatment modality, which one of the most commons is samarium-153-ethylene diamine tetramethylene phosphonate (153Sm-EDTMP), but recently the radium-223-dichloride (223Ra-Cl2) alpha emitter radiopharmaceutical has been approved by the US Food and Drug Administration (FDA). The aim of this study was to compare the bone marrow absorbed dose of 153Sm-EDTMP and 223Ra-Cl2 radiopharmaceuticals.
MethodsIn this study, a lumbar vertebral bone phantom was used for simulation. The radiopharmaceuticals were distributed in the phantom, and the simulation was performed using the GATE Monte Carlo toolkit. After estimating the absorbed dose per decay in different parts of the phantom, the absorbed dose of radiopharmaceuticals in the bone marrow of a patient with 70 kg weight, was calculated using the Medical Internal Radiation Dosimetry (MIRD) method.
FindingsFor 153Sm-EDTMP, the bone marrow absorbed dose was equal to 9.60 Gy for the administered activity of 2590 MBq. For 223Ra-Cl2, the bone marrow absorbed dose was calculated as 0.83 Gy for administered activity of 3.85 MBq.
ConclusionDue to the lower bone marrow absorbed dose by 223Ra-Cl2 alpha emitter radiopharmaceutical, the use of this radiopharmaceutical as an alternative to 153Sm-EDTMP beta emitter radiopharmaceutical is recommended for palliative therapy of patients with bone metastasis.

Detection of coronavirus disease 2019 (COVID-19) in early stage is indispensible for outcome improvement and interruption of transmission chain. Clear understanding of the nature of the diagnostic tests for severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) and their challenges, collecting the most diagnostically valuable specimen at the right time from the right anatomic site, and interpretation of their findings is important. This review scrutinizes current challenges and interpretation of reverse transcriptase polymerase chain reaction (RT-PCR), as the reference method, loop-mediated isothermal amplification (LAMP), antibody and antigen detection, typical lung imaging characteristics and prominent abnormal changes in laboratory findings of patients with proven COVID-19, and describes how the results may vary over time. Bronchoalveolar lavage fluid and sputum specimens demonstrate the highest positive rates (93% and 72 %, respectively) in molecular diagnosis of COVID-19. Alternatively, repeated RT-PCR assays can be performed; as over time, it is an increase in the likelihood of the SARS-CoV-2 being present in the nasopharynx. Combining clinical evidence with results of chest computed tomography (CT) and RT-PCR can minimize the risk of diagnostic errors. Elevated levels of interleukin 6 (IL‐6) and D-dimer are thought to be closely associated with the occurrence of severe COVID‐19 in adults, and their combined detection can serve as early factors predicting the severity of COVID‐19. Moreover, elevated acute phase proteins are associated with a poor outcome in COVID-19. Serological diagnosis also is an important tool to understand the extent of COVID-19 in the community, and to identify individuals, who are immune. Antibodies begin to increase from the second week of symptom onset.
