ناک اوت ژنی سلولهای T پرایمری با واسطهی Clustered regularly interspaced short palindromic repeats/ CRISPR-associated protein 9 (CRISPR/Cas9) محدودیتهای زیادی برای کاربردهای بالینی دارد. انتقال مستقیم پروتئین نوترکیب Cas9 و Guide RNA (gRNA) سنتتیک به عنوان یک کمپلکس ریبونوکلیوپروتئینی از پیش مونتاژ شده (Ribonucleoprotein یا RNP) به یک روش قدرتمند برای ویرایش ژنی بسیار کارآمد در سلولهای T پرایمری تبدیل شده است. در این مطالعه، از سیستم انتقال مبتنی بر Cas9 RNP برای ناکاوت هدفمند ژنهای T Cell receptor alpha constant (TRAC) و β2 microglobulin (B2M) در سلولهای T پرایمری انسانی استفاده گردید.
روشها:
بدین منظور gRNAهای اختصاصی برای هدف قرار دادن اگزونهای ابتدایی ژنهای TRAC و B2M طراحی شدند. پروتئین Cas9 و gRNAهای سنتتیک مربوطه به طور جداگانه ترکیب شدند و به سلولهای T پرایمری انسانی جدا شده از سلولهای تک هستهای خون محیطی (Peripheral blood mononuclear cell یا PBMC) الکتروپوریت شدند. کارایی ویرایش ژنی با روش تجزیه و تحلیل Tracking of Indels by Decomposition (TIDE) و فلوسایتومتری اندازهگیری شد.
یافتهها:
سه روز پس از الکتروپوریشن سلولهای T پرایمری با استفاده از کمپلکسهای RNP هدفگیری کنندهی TRAC و B2M، آنالیز TIDE کارایی ناکاوت 60-13 درصد را برای gRNAهای هدفگیری کنندهی TRAC و 53-21 درصد را برای gRNAهای هدفگیری کنندهی B2M مشخص کرد. آنالیز فلوسایتومتری نیز به ترتیب میزان 76 و 27 درصد سرکوب کامل بیان ژن را برای کارآمدترین gRNAهای هدفگیری کنندهی TRAC (TRAC-gRNA3) و (B2M-gRNA2) B2M تایید کرد.
نتیجهگیری:
نتایج این مطالعه، نشان میدهد که سیستم Cas9 RNP میتواند به طور کارآمدی به سلولهای T پرایمری منتقل و منجر به ناکاوت هدفمند ژن گردد. شیوهنامهی شرح داده شده در این مطالعه، راهکار ساده و بسیار کارآمدی به حداکثر رساندن کارایی ویرایش در سلولهای T پرایمری فراهم میسازد و روند ویرایش ژن برای ایمنوتراپیهای نسل بعدی را تسهیل میکند.

بیماری COVID-19، بزرگترین و شدیدترین بیماری همهگیر بعد از آنفلوانزای سال 1918، محسوب میشود. ظهور این بیماری، چالشهای بیسابقهای را برای سلامت عمومی ایجاد کرده است. تظاهرات بالینی در بیماران مبتلا بسیار متنوع است و میتواند شامل بیماری بدون علامت تا بیماری شدید همراه با مرگ باشد. هدف اصلی پژوهش حاضر، مقایسهی پیامدهای بیماران مبتلا به COVID-19، با علایم اولیه ریوی، گوارشی و عصبی در بیماران بستری شده در بیمارستانهای شهر قزوین بود.
روشها:
مطالعهی حاضر، یک مطالعهی گذشتهنگر با تعداد 300 نمونه بود که بعد از اخذ مجوز اخلاق و اخذ رضایت از بیماران اجرا شد. نمونهها به روش هدفمند انتخاب شدند. معیار ورود شامل سن بالای 18 سال، تایید ابتلا به COVID-19 به واسطهی داشتن اسکن ریه که درگیری آن توسط متخصص داخلی یا عفونی تایید شده باشد و آزمایش Polymerase chain reaction (PCR) بیمار نیز مثبت باشد. معیار خروج نیز شامل بارداری بیمار بود.
یافتهها:
نتایج این مطالعه نشان داد گروه بیمارانی که با علایم اولیهی ریوی مراجعه کرده بودند، با دو گروه دیگر از نظر میزان مرگ، بستری در بخش ویژه، لولهگذاری و بستری در بیمارستان، اختلاف معنیداری داشت (050/0 > P).
نتیجهگیری:
بیمارانی که با علایم اولیهی ریوی مراجعه میکنند، در اولویت بستری در بخشها هستند.

Clustered regularly interspaced short palindromic repeats/ CRISPR-associated protein 9 (CRISPR/Cas9)-mediated gene knockout of primary T cell has several limitations for clinical applications. Direct delivery of recombinant Cas9 protein and synthetic gRNA, as a pre-assembled ribonucleoprotein (RNP) complex, has become a potent approach to introduce highly efficient gene editing in primary T cells. In this study, we employed Cas9 RNP-based delivery system for targeted T Cell receptor alpha constant (TRAC) and β2 microglobulin (B2M) genes knockout in human primary T cells.
MethodsSpecific gRNAs were designed to target the first exons of TRAC and B2M genes. Cas9 protein and respective synthetic gRNAs were then mixed separately, and electroporated into human primary T cells isolated from peripheral blood mononuclear cells (PBMCs). The gene editing efficiency was quantified using tracking of indels by decomposition (TIDE) analysis and flow cytometry.
FindingsThree days after electroporation of primary T cells with the TRAC and B2M targeting RNP complexes, TIDE analysis revealed the knockout efficiency of 13-60 percent for the TRAC-targeting gRNAs and 21-53 percent for B2M-targeting gRNAs. Flow cytometry analysis confirmed ~76% and ~27% complete loss of expression for the most efficient gRNAs targeting TRAC (TRAC-gRNA3) and B2M (B2M-gRNA2), respectively.
ConclusionOur results demonstrate that Cas9 RNP system can be efficiently delivered into primary T cells and result in targeted gene knockout. The protocol described here enables a streamlined and highly efficient solution for maximizing editing efficiency in primary T cells, and simplifies the gene editing process for next-generation immunotherapies

Coronavirus disease 2019 (COVID-19) is the largest and most severe epidemic since the 1918 flu, and its emergence has created unprecedented challenges to public health. Clinical manifestations in patients are very diverse, and can range from asymptomatic disease to severe illness with death. The main purpose of this study was to compare the outcomes of patients with Covid-19 with early pulmonary, gastrointestinal, and neurological symptoms in patients admitted to hospitals in Qazvin City, Iran.
MethodsThis was a retrospective study with 300 samples, performed after obtaining ethics license and obtaining patient consent. The samples were selected using purposive method. Confirmation of COVID-19 was done due to having a lung scan confirmed by an internal medicine or infectious disease specialist, as well as having a positive polymerase chain reaction (PCR) test. Exclusion criteria also included pregnancy.
FindingsThe group of patients presented with early pulmonary symptoms were significantly different from the other two groups in terms of mortality, intensive care unit, intubation and hospitalization (P < 0.050).
ConclusionPatients presenting with early pulmonary symptoms should be given priority in hospitalization.
