برخی مطالعات نشان دادهاند اندازهگیری برخی متغیرهای آزمایشگاهی مانند D-Dimer و گازهای خون شریانی میتواند در تشخیص آمبولی ریوی کمک کننده باشد. این مطالعه، با هدف تعیین معیارهای ارزش تشخیصی آزمایشهای D-Dimer و Alveolar-arterial gradient (A-a gradient) در تشخیص و بررسی وضعیت یک ماههی بیماران مبتلا به تنگی نفس انجام گرفت.
روشهادر یک مطالعهی توصیفی- تحلیلی، 196 بیمار با علایم تنگی نفس که به اورژانس بیمارستانهای الزهرا (س) و آیتاله کاشانی اصفهان ارجاع شده بودند، مورد مطالعه قرار گرفتند و سطح D-Dimer، سطح گازهای خون شریانی و معیار A-a gradient در آنان اندازهگیری شد. کلیهی بیماران تحت CT scan ریه قرار گرفتند و وجود آمبولی ریوی در آنان بررسی گردید. معیارهای ارزش تشخیصی دو آزمایش D-Dimer و A-a gradient شامل حساسیت، ویژگی، ارزش اخباری مثبت و منفی در مقایسه با CT scan تعیین گردید.
یافتههاحساسیت، ویژگی، ارزش اخباری مثبت و منفی برای آزمایش D-Dimer به ترتیب 41/70، 79/94، 20/93 و 80/75 درصد و برای آزمایش A-a gradient به ترتیب 7/92، 24/17، 30/55 و 20/68 درصد به دست آمد. سطح زیر منحنی Receiver operating characteristic (ROC) برای D-Dimer 85/0 (001/0 > P) و برای A-a gradient، 5/0 (820/0 = P) به دست آمد.
نتیجهگیریآزمایش D-Dimer، دارای معیارهای ارزش تشخیصی بالایی برای تشخیص آمبولی ریوی در بیماران با علایم تنگی نفس میباشد؛ در حالی که متغیرهای مربوط به گازهای خون شریانی مانند A-a gradient دارای قدرت تشخیصی بالایی برای تشخیص عارضهی پیشگفته نیستند.

در این مطالعه، اثرات سایتوتوکسیک عصارههای مختلف گیاه Allium giganteum بر روی ردههای سلولی MCF-7 و HeLa به منظور دستیابی به ترکیبات جدید واجد اثرات ضد سرطان بررسی گردید.
روشهاگلهای گیاه با روش عصارهگیری 4 مرحلهای با حلالهایی با قطبیت متفاوت، عصارهگیری، و سپس، سمیت سلولی عصارهها پس از تیمار 48 ساعتهی سلولها با غلظتهای 10، 20 و 50 میکروگرم/میلیلیتر از هر کدام از عصارهها، به روش MTT بررسی شد. همچنین، مکانیسم مرگ سلولی القا شده روی سلولهای MCF-7 به روش فلوسایتومتری، با رنگآمیزی Annexin/PI تعیین شد.
یافتههاآزمون MTT عصارههای دیکلرومتانی، کلروفرم- متانولی (9:1) و بوتانولی در غلظت 50 میکروگرم/میلیلیتر روی سلولهای MCF-7 و در غلظتهای 20 و 50 میکروگرم/میلیلیتر بر روی سلولهای HeLa سمیت سلولی معنیداری نسبت به گروه شاهد نشان داد (050/0 > P). عصارهی آبی، هیچ گونه اثر سمیت معنیداری نسبت به گروه شاهد نشان نداد. نتایج واکاوی فلوسایتومتری، القای آپوپتوز توسط هر سه عصارهی موثر فوق را نشان داد.
نتیجهگیریعصارههای دیکلرومتانی، کلروفرم- متانولی (9:1) و بوتانولی این گیاه، واجد اثرات سایتوتوکسیک روی سلولهای سرطانی مورد بررسی بودند و اثرات القای آپوپتوز قابل توجهی (تا 26 درصد) روی سلولهای MCF-7 القا کردند. با توجه به نتایج مطلوب حاصل از این مطالعه، استخراج و شناسایی ترکیبات موجود در این عصارهها به ویژه استرویید ساپونینهای موجود در آنها، حایز اهمیت است.

تاموکسیفن، یکی از داروهای موثر برای درمان سرطان پستان میباشد که باعث کاهش عود سرطان پستان میگردد، اما در عین حال، گاهی شانس سرطان رحم در مصرف کنندگان این دارو افزایش مییابد. در مطالعهی حاضر، یک مورد نادر سرطان رحم به فاصلهی زمانی کوتاهی پس از درمان سرطان پستان، معرفی میگردد.
گزارش موردییک زن 50 سال با سابقهی 6 حاملگی و 5 زایمان زنده با سابقهی یک مرتبه سقط و شرح سرطان پستان درمان شده، با شکایت خونریزی رحمی غیر طبیعی از حدود یک سال قبل ارجاع گردید. سرطان پستان ایشان دو سال قبل تحت جراحی و به دنبال آن تاموکسیفن درمانی قرار گرفت که به دنبال بروز این علامت، به لتروزول تغییر یافته بود. اگر چه بررسی بیشتری در آن زمان انجام نگرفت، تشخیص فعلی لیومیوسارکوم میکسویید رحم بود.
نتیجهگیریتاموکسیفن، یکی از داروهای موثر در درمان سرطان پستان میباشد که میتواند باعث افزایش خطر بروز سرطان رحم شود. مصرف کنندگان این دارو، به خصوص در صورت بروز علایمی نظیر خونریزی غیر طبیعی واژینال، لازم است از نظر ابتلا به سرطان رحم بررسی شوند.

Some of studies have shown that measurement of some laboratory parameters such as D-dimer and arterial blood gases can be helpful in the diagnosis of pulmonary embolism. This study aimed to determine the diagnostic values of D-dimer and alveolar-arterial gradient (A–a gradient) for detection of pulmonary embolism in patients with shortness of breath.
MethodsIn a descriptive-analytical study, 196 patients with symptoms of shortness of breath referred to the emergency departments of Alzahra and Ayatollah Kashani hospitals in Isfahan City, Iran, were studied, and their D-dimer and arterial blood gas levels and A-a gradient were measured. All patients underwent CT scan of the lungs, and the presence of pulmonary embolism was evaluated. The diagnostic value criteria of the two tests including sensitivity, specificity, and positive and negative predictive values were evaluated in comparison with CT scan.
FindingsSensitivity, specificity, and positive and negative predictive value were obtained 70.41, 94.79, 93.20, and 75.80 percent for the D-dimer test, respectively, and 92.7, 17.24, 55.3, and 68.2 percent for the A–a gradient test, respectively. The area under the rock curve of receiver operating characteristic (ROC) was 0.85 (P < 0.001) for the D-dimer and 0.5 (P = 0.820) for the A-a gradient.
ConclusionThe D-dimer test has high diagnostic value criteria for diagnosis of pulmonary embolism in patients with symptoms of shortness of breath, while parameters related to arterial blood gases such as A-a gradient do not have high diagnostic values to diagnose this complication.

Here, the cytotoxicity of various Allium giganteum extracts was evaluated on MCF-7 and HeLa cell lines, to introduce novel anti-cancer agents from natural resources,
MethodsFlowers of the plant were subjected to extraction in a four steps method, and the extracts were evaluated for their cytotoxicity on MCF-7 and HeLa cell lines. MTT assay was performed 48 hours. Following treatment of the cells with 10, 20, and 50 µg/ml of different extracts. The mechanism of the induced cell death was assessed by flow cytometry following Annexin/PI staining of the treated cells.
FindingsThe MTT results showed significant toxicity of dichloromethane, chloroform-methanol (9:1), and butanolic extracts at 50 µg/ml on MCF-7, and at 20 and 50 µg/ml on HeLa cells compared to control (P < 0.05). No significant cytotoxicity and apoptosis induction was observed following treatment with aquatic extract. The flow cytometric analysis results indicated apoptosis induction by all the three extracts on MCF-7 cells.
ConclusionThe dichloromethane, chloroform-methanol (9:1), and butanolic extracts of Allium giganteum showed cytotoxic effects on the cancer cells, and up to 26% apoptosis of MCF-7 cells. Due to the desirable results of this study, extraction and identification of constituents of the extracts, especially steroidal saponin constituents, could be valuable.

A Rare Case of Uterine Leiomyosarcoma Following the Use of Tamoxifen in a Patient with Breast CancerPages 584-587Background
There is a controversial effect of Tamoxifen in reducing breast cancer recurrences, while it may increase the chance of uterine cancer occurrence. In this study, we report a rare case uterine neoplasm shortly after breast cancer treatment.
Case ReportA 50-year-old woman, gravida 6 live 5 and abortion 1, with history of treated breast cancer referred with the complaint of prolonged abnormal uterine bleeding from one year before. Her breast cancer was treated by surgery and Tamoxifen therapy, which was exchanged to letrozole after her new complaint. Although, no further evaluation had been performed earlier, recently uterine myxoid leiomyosarcoma was defined.
ConclusionTamoxifen is one of the most effective drugs in the treatment of breast cancer, but it can increase the risk of uterine cancer. In our case, after taking tamoxifen, she developed uterine leiomyosarcoma. As a result, users of this drug, especially in cases with symptoms such as abnormal bleeding, should be carefully screened for uterine cancer.
