تبادلات گازی بهتر، زمینهساز نتیجهی مطلوبتری با فشارهای پایینتر اکسیژناسیون است. وضعیت قرارگیری بیمار در بیماران تحت تهویهی مکانیکی، یکی از عوامل موثر بر راندمان تبادل گازهای تنفسی میباشد. هدف از انجام پژوهش حاضر، مقایسهی سطح فشار گازهای خون شریانی و درصد اشباع اکسیژن در وضعیت خوابیده به پهلو با وضعیت خوابیده به پشت در بیماران مبتلا به پنومونی ناشی از تهویهی مکانیکی بود.
روشهادر این مطالعهی کارآزمایی بالینی، 50 بیمار سکتهی مغزی ایسکمیک مبتلا به پنومونی ناشی از تهویهی مکانیکی بستری در بخش مراقبتهای ویژه بیمارستان کاشانی شهرکرد در سال 1398، به روش نمونهگیری در دسترس انتخاب شدند. سپس درصد اشباع اکسیژن بیماران در وضعیت خوابیده به پشت و دو ساعت خوابیده به پهلو با استفاده از دستگاه پالس اکسیمتر و سطح فشار گازهای اکسیژن و دیاکسیدکربن خون شریانی نیز به کمک نمونهی خون شریانی ارزیابی و در چکلیست ثبت گردید.
یافتههادر هر دو وضعیت خوابیده به پهلو و خوابیده به پشت، میانگین فشار اکسیژن شریانی و درصد اشباع اکسیژن هموگلوبین تفاوت معنیداری نداشت (050/0 < P)؛ در حالی که میزان فشار دیاکسیدکربن شریانی در وضعیت خوابیده به پهلو، به طور معنیداری کمتر از وضعیت خوابیده به پشت بود (008/0 = P).
نتیجهگیریتغییر وضعیت خوابیده به پهلو، تاثیر مثبتی در بهبود گازهای خون شریانی و درصد اشباع اکسیژن بیماران سکتهی مغزی ایسکمیک مبتلا به پنومونی ناشی از تهویهی مکانیکی نداشت، اما منجر به کاهش معنیدار فشار دیاکسیدکربن شد.

کلبسیلا پنومونیه، پاتوژن فرصتطلب و یکی از عوامل شایع عفونتهای بیمارستانی است که امروزه برخی از سویههای آن در برابر آنتیبیوتیکها مقاوم شده و درمان عفونت را دشوار کردهاند. هدف از انجام پژوهش حاضر، استفاده از تکنیک واکنش زنجیرهای پلیمراز چندگانه (Multiplex polymerase chain reaction یا Multiplex PCR) به منظور ردیابی ژنهای مقاومت به تتراسایکلین در ایزولههای بالینی کلبسیلا پنومونیه بود.
روشهاپس از جمعآوری نمونههای بالینی از بیمارستانهای رضوی و قایم مشهد در سال 1399، جداسازی و شناسایی کلبسیلا پنومونیه با استفاده از روشهای بیوشیمیایی و مولکولی (پرایمر اختصاصی Kp16S rRNA) انجام شد. الگوی مقاومت آنتیبیوتیکی برای 30 ایزولهی شناسایی شده به روش دیسک دیفیوژن تعیین گردید. پس از طراحی پرایمر و Blast، ردیابی ژنهای tet با روش PCR بهینهسازی شد. در نهایت، تکنیک Multiplex PCR با استفاده از سه جفت پرایمر و شاهد مثبت بهینهسازی شد.
یافتههابیشترین و کمترین مقاومت آنتیبیوتیکی ایزولهها به ترتیب شامل آمپیسیلین (0/90 درصد) و سفتریاکسون (7/26 درصد) بود و 3/53 درصد ایزولههای کلبسیلا پنومونیه از نظر فنوتیپی به تتراسایکلین مقاوم بودند. Multiplex PCR با دمای اتصال 55 درجهی سانتیگراد بهینهسازی شد. همزمان با ردیابی ژن Kp16S rRNA، ژنهای tetA و tetB به ترتیب در 5/87 و 7/68 درصد از ایزولههای مقاوم به تتراسایکلین ردیابی شدند. 2/56 درصد ایزولهها به طور همزمان هر دو ژن tetA و tetB را حمل میکردند و 5/12 درصد ایزولهها حامل یکی از ژنهای tetA و یا tetB بودند.
نتیجهگیریتکنیک Multiplex PCR طراحی شده در مطالعهی حاضر، روشی سریع، دقیق و حساس به منظور شناسایی کلبسیلا پنومونیه مقاوم به آنتیبیوتیک میباشد که میتواند برای نمونههای بالینی و یا در تحقیقات اپیدمیولوژیک استفاده شود.

گال یکی از بیماریهای واگیردار مهم جلدی است که عامل آن هیره (Mite) از گونهی Sarcoptes scabiei var Hominis میباشد. بیماری گال پوستهدار نوع نادری از گال است که با تکثیر سریع و وسیع هیره در پوست فرد مبتلا، اغلب در بیماران دارای نقص ایمنی با بروز واکنش آلرژیک شدید همراه میباشد. در پژوهش حاضر، یک مرد 85 سالهی ساکن روستا که از دو ماه قبل دچار خارش شدید شده بود، مورد معاینات بالینی قرار گرفت.
گزارش موردبیمار 85 سالهی مذکر دارای علایم پوستی شدید، برای تشخیص بیماری انگلی پوستی به گروه انگلشناسی دانشکدهی پزشکی دانشگاه علوم پزشکی اصفهان ارجاع گردید و به روش Scraping test، از ضایعات پوست نمونه تهیه و زیر میکروسکوپ مشاهده گردید. در آزمایش میکروسکوپی، با دیدن هیره و تخمهای آن، بیماری گال تشخیص داده شد و با توجه به علایم بالینی، تشخیص نوع گال پوستهدار قطعی شد.
نتیجهگیریگال پوستهدار در افراد دارای نقص سیستم ایمنی ایجاد میشود، اما با توجه به مورد گزارش شده، به نظر میرسد امکان بروز آن در افراد با ایمنی سالم نیز وجود دارد.

In patients under mechanical ventilation, the patient's position is one of the factors affecting the efficiency of respiratory gas exchange. The aim of this study was to compare the level of arterial blood gas pressure and the percentage of arterial oxygen saturation in the supine position with the lateral position in patients with ventilator-associated pneumonia.
MethodsIn a clinical trial study with a sample size of 50 patients with ischemic stroke under mechanical ventilation admitted to the intensive care unit of Kashani hospital affiliated to Shahrekord University of Medical Sciences, Shahrekord, Iran, in 2019 were selected using convenience sampling method. Oxygen saturation percentage was measured by a pulse oximeter in patients lying on their backs and then, on their sides for two hours; arterial blood pressure and carbon dioxide levels were measured with arterial blood sample by laboratory device, and recorded in the relevant checklist as well.
FindingsIn both supine and lateral positions, the mean arterial oxygen pressure and the percentage of hemoglobin oxygen saturation were not significantly different (P > 0.050); while the arterial carbon dioxide pressure in the lateral position was significantly lower than the supine position (P = 0.008).
ConclusionThe lateral position has no significant effect on the improvement of arterial blood gases and oxygen saturation in patients with pneumonia due to mechanical ventilation; but has significantly reduce the pressure of carbon dioxide.

Klebsiella pneumoniae (K. pneumonia) is an opportunistic pathogen and one of the most common causes of nosocomial infections, some of which are now resistant to antibiotics and make infection difficult to treat. The main purpose of this study was to use multiplex polymerase chain reaction (PCR) to detect tetracycline-resistance genes in clinical isolates of K. pneumonia.
MethodsAfter collecting clinical samples from hospitals in Mashhad City, Iran, in 2021, K. pneumoniae was isolated and identified by biochemical and molecular methods (Kp16S rRNA specific primer). Antibioticresistance pattern was determined for 30 isolates by disk diffusion method. After designing primer and its blast, detection of tet genes was optimized by PCR. Finally, multiplex PCR was set up using three pairs of primers and positive control.
FindingsThe highest and lowest antibiotic resistance of the isolates was for ampicillin (90%) and ceftriaxone (26.66%), respectively, and 53.33% of Klebsiella isolates were phenotypically resistant to tetracycline. Multiplex PCR was set up by annealing temperature of 55°C. Simultaneously with detection of Kp16S rRNA gene, tetA and tetB genes were detected in 87.5% and 68.75% of tetracycline-resistant Klebsiella isolates, respectively. 56.25% of the isolates carried both tetA and tetB genes, simultaneously, and 12.5% of the isolates transferred one of the tetA or tetB genes.
ConclusionThe results show that the multiplex PCR technique designed in this study is a fast, accurate, and sensitive method for identifying antibiotic-resistant K. pneumoniae that can be used for clinical samples or in epidemiological studies.

Scabies is one of the most important infectious skin diseases caused by the arthropod mite Sarcoptes scabiei var Homonis. Crusted scabies is a rare form of scabies that multiplies rapidly and extensively in the skin of an infected person, usually in immunocompromised patients, and is accompanied by a severe allergic reaction. Here, we report an 85-year-old man, a resident of the village, who had been severely itchy for two months.
Case ReportAn 85-year-old man with severe skin symptoms was referred to the Parasitology Department of Isfahan University of Medical Sciences, Isfahan, Iran, for diagnosis of skin parasitic disease. A sample of skin lesions was prepared by scraping method and observed under microscope. Under microscopic examination, by observing the mite and its eggs, scabies was diagnosed and the diagnosis of crusted scabies was confirmed according to the clinical signs.
ConclusionThis form of the disease occurs in people with defective immune systems, but according to the case reported here, it seems that it can also occur in people with healthy immunity.
