مدیریت بحران به عنوان یکی از حوزه های مدیریت پیامد می تواند تاب آوری در برابر بحران ها را افزایش داده و باعث کاهش سطح آسیب پذیری سازمانی در هنگام بروز بحران شود. این مطالعه با هدف ارزیابی مولفه های مدیریت بحران در بیمارستان های خصوصی و دولتی طراحی و انجام شده است.
روشمطالعه حاضر در سال 1399 در 10 بیمارستان در شهر تهران انجام شده است. حجم نمونه مورد مطالعه 200 نفر تخمین زده شد. مولفه های مدیریت بحران در این پرسشنامه شامل: تعهد مدیریت، فرهنگ خطاپذیری، یادگیری، آگاهی، آمادگی، انعطاف پذیری و شفافیت بود. تجزیه تحلیل داده ها با استفاده از آزمون های آماری t مستقل و آزمون مجذور کای یا آزمون دقیق فیشر انجام شده است.
یافته هانتایج مطالعه نشان داد دو گروه بیمارستان های دولتی و خصوصی از نظر متغیرهای فردی دارای اختلاف معنی دار نیستند (0/05<p). شاخص مدیریت بحران در دو گروه بیمارستان های دولتی و خصوصی به ترتیب 4/11 و 3/22 ارزیابی شد. نتایج ارزیابی مقایسه مولفه های مدیریت بحران در دو گروه مورد مطالعه نشان داد همه این مولفه ها در گروه بیمارستان های دولتی بیشتر از گروه بیمارستان های خصوصی برآورد شده و اختلاف آن ها از نظر آماری نیز معنی دار می باشد (0/05>p).
نتیجه گیرینتایج مطالعه بیانگر این بود که هرچند میزان تاب آوری بیمارستان های خصوصی بالاتر از حد متوسط ارزیابی شده است، اما ارزیابی مقایسه ای مولفه های مدیریت بحران نشان داد سطح تاب آوری در برابر بحران در بیمارستان های دولتی بیشتر از این گروه می باشد؛ بنابراین، بایستی با یک طراحی مناسب و با استفاده از یک برنامه جامع مدیریت بحران مولفه های مدیریت بحران در این بیمارستان ها را ارتقاء داد.

عوامل انسانی از موضوعات محوری و مهم در محیط های کاری مختلف ازجمله صنایع می باشد. فرایندهای عملیاتی شرکت گاز ازجمله فرایندهایی است که درروند اجرایی آن حوادث متعددی رخ داده است. حوادث از مهم ترین علل هزینه های مستقیم و غیرمستقیم وارده بر صنعت می باشد، لذا نیاز است عوامل انسانی موثر بر بروز خطا که بیشترین نقش را در وقوع حوادث دارند، شناسایی و تعیین گردند تا با شناخت آن ها راهکارهای کنترلی و پیشگیرانه ای اتخاذ و از وقوع مجدد حوادث، جلوگیری نمود. بر این اساس بررسی عوامل انسانی موثر بر بروز خطا از اهمیت بالایی در صنعت برخوردار است. لذا هدف از این مطالعه تحلیل عوامل انسانی موثر بر بروز خطا در فرایندهای عملیاتی شرکت گاز می باشد.
روش بررسیمطالعه حاضر از نوع توصیفی - مقطعی می باشد که در یک شرکت گاز انجام شد. آمار و مستندات از حوادث دوره 10 ساله شرکت گاز از امور HSE این مرکز دریافت شد و از طریق روش های آنالیز عوامل انسانی و طبقه بندی سیستم برای صنعت نفت و گاز (HFACS-OGI) و تحلیل علل ریشه ای (RCA) موردبررسی و تحلیل قرار گرفت.
یافته هانتایج حاصل از کاربرد روش HFACS-OGI نشان داد که در سطح یک، بخش خطا، زیر گروه "خطای ادراکی" و در سطح دوم زیر گروه "مدیریت منابع انسانی"، در سطح سوم "نظارت ناکافی"، در سطح 4 بخش " فرهنگ ایمنی" و در سطح 5 چارچوب قانونی بیشترین درصد فراوانی را داشته است. از طریق روش RCA بیشترین فراوانی برای نقص در مقتضیات کاری و کمترین فراوانی مربوط به عدم صلاحیت مورد نیاز می باشد.
نتیجه گیریعوامل انسانی موثر بر بروز خطا شامل عوامل شناختی و رفتاری کارکنان، نقص در سیستم مدیریت ایمنی فرایندهای عملیاتی، عدم نظارت کافی و صحیح، عدم/نقص در آموزش و عدم تجربه کافی و موثر کارکنان، عدم رعایت اصول ایمنی عمومی، عدم /نقص در اجرای اقدامات کنترلی درج شده در مدیریت ریسک های مصوب سازمان، عدم/نقص در اجرای بهینه مدیریت منابع انسانی می باشد که از طریق بهبود شاخص های عملکردی، رویه های اجرایی و برنامه ریزی مناسب و به کارگیری دوره های نظارتی و سرپرستی صحیح می توان عوامل انسانی موثر بر روز خطا را کاهش داد. در نهایت نیز مشخص شد که به کارگیری روش های HFACS-OGI و RCA به دلیل انطباق و هم خوانی که در ارایه نتایج داشتند می توانند در محیط های کاری مشابه به کار برده شوند.

هم زمان با پیشرفت علم و تکنولوژی در کشورهای جهان سوم و در حال توسعه عوامل زیان آور محیط کار بیشتری زندگی کارکنان را به خطر می اندازد. یکی از این عوامل زیان آور، عدم تطبیق کار با انسان است که باعث اختلالات اسکلتی عضلانی می شود. بخش مهمی از غرامت های ناشی از کار در کشورهای در حال توسعه مربوط به اختلالات اسکلتی عضلانی است. به همین دلیل مطالعه ای با هدف بررسی تاثیر مداخلات ارگونومیک بر کاهش شیوع اختلالات اسکلتی عضلانی و سرانجام استفاده از نتایج به دست آمده برای پیشگیری از اختلالات اسکلتی عضلانی و بهبود محیط کار در میان کارکنان مجتمع ذوب و احیای روی قشم در سال 1399 انجام گرفت.
روش بررسیدر این مطالعه کارکنان به صورت تصادفی ساده با استفاده از پرسشنامه نوردیک، به صورت شاهد موردی به روشQEC و نقشه بدن مورد ارزیابی قرار گرفتند. سپس مداخلات ارگونومیک برای گروه مورد انجام شد. بعد از یک ماه و چهار ماه ارزیابی مجدد به روش های QEC و نقشه بدن انجام شد.
نتیجه گیریبنابراین با انجام مداخلات صحیح ارگونومیک شامل آموزش اصول ارگونومیک، به کارگیری تجهیزات اتوماتیک و غیره اختلالات اسکلتی عضلانی نواحی کمر، شانه، گردن و زانو ایجاد شد و اختلالات اسکلتی عضلانی به 58/13 درصد کاهش یافت. همچنین پس از اجرای مداخلات ارگونومی سطح ریسک این اختلالات به مقدار قابل توجهی کاهش یافت.

ارزیابی حادثه توقف تولید و پارامترهای موثر بر آن در صنعت پالایشگاه گاز به روش ترکیبی Fishbone و SCATصفحات 334-349مقدمه
آنالیز علت های رخداد حوادث به منظور پیشگیری از رخداد مجدد آن ها امری ضروری می باشد. مطالعه حاضر با هدف ترکیب دو روش fishbone و SCAT جهت ریشه یابی حوادث، پرداختن به جزییات، جهت ردیابی کردن ریشه های حوادث در عمق لایه های سازمانی می باشد.
روش بررسیدر این مطالعه داده های حادثه به روش ترکیبی استخوان ماهی- اسکات تجزیه وتحلیل شدند. ابتدا علل واسطه و علل ریشه ای به دست آمده از روش استخوان ماهی به همراه اقدامات کنترلی در جدول علت و معلول اسکات جایگذاری گردید. نتایج حاصل از پرسشنامه گردآوری شدند و علل وقوع حوادث از طریق آزمون کای دو بررسی گردید. تجزیه وتحلیل داده ها به کمک نرم افزار SPSS 24 انجام شد.
یافته هانتایج تحقیق نشان داد که مهم ترین علت مستقیم بروز حادثه، متغیر مرتبط با محیط داخلی و خارجی پالایشگاه و مهم ترین علت ریشه ای حادثه، تغییرات اقتصادی پیش بینی نشده مانند تغییر نرخ ارز، تغییر نرخ مالیات و عدم پیاده سازی برنامه های آموزشی مستمر برای کارکنان می باشد. در میان اقدامات کنترلی پیشنهادی ارتقاء اثربخشی آموزش کارکنان مهم ترین می باشد. همچنین بین دلایل سطحی و ریشه ای ارتباط معناداری وجود داشت.
نتیجه گیریبا توجه به آزمون آماری مشخص گردید که بین علل ریشه ای و علل واسط حادثه ارتباط سیستماتیک و معنی داری وجود دارد. با استفاده از ترکیب دو روش در بسیاری از موارد نیاز سازمان ها را برای ریشه یابی حوادث برآورده می کند و در حال حاضر یکی از مهم ترین روش ها می باشد؛ از این رو انجام مطالعات مشابه از قبیل ترکیب روش SCAT با سایر روش ها در آنالیز دلایل حوادث پالایشگاهی پیشنهاد می گردد.

افزایش کیفیت زندگی کاری زنان شاغل به عنوان نیروهای موثر در تحقق اهداف سازمان ها اهمیت خاصی دارد زیرا آن ها هم زمان عهده دار مسوولیت های همسری، مادری و کارمندی هستند. پژوهش حاضر با هدف مطالعه وضعیت سبک های هویت و هوش چندگانه و رابطه آن با کیفیت زندگی کاری زنان شاغل در دانشگاه یزد در سال 1397 انجام شد.
روش بررسیاین پژوهش از لحاظ روش گردآوری داده ها، توصیفی- پیمایشی و از نظر هدف، بنیادی است. جامعه آماری پژوهش، شامل کلیه کارمندان زن دانشگاه یزد (159 نفر) بود که از بین آن ها بر مبنای جدول کرجسی و مورگان 120 نفر با روش نمونه گیری تصادفی ساده به عنوان نمونه پژوهش انتخاب شدند. داده ها با استفاده از پرسشنامه های سبک هویت برزونسکی، هوش چندگانه آرمسترانگ و کیفیت زندگی کاری والتون گردآوری و با استفاده از آزمون t تک نمونه ای، آزمون کای اسکویر، تحلیل واریانس با اندازه گیری مکرر و آزمون بنفرونی و ضریب همبستگی پیرسون تحلیل شد.
یافته هاسبک هویت هنجاری سبک هویت غالب و هوش درون فردی بر انواع دیگر هوش زنان شاغل دانشگاه یزد غلبه داشت، کیفیت زندگی کاری زنان در حد متوسط بود. بین سبک های هویت با کیفیت زندگی کاری، رابطه ای وجود نداشت. از بین هوش های چندگانه، فقط هوش منطقی- ریاضی با کیفیت زندگی کاری، رابطه مثبت و معناداری داشت.
نتیجه گیریمدیران سازمان ها، افراد دارای هویت اطلاعاتی و هنجاری را در اولویت استخدام قرار داده و فرصت هایی را برای استفاده از توانش های هوشی متفاوت کارکنان برای بهبود کیفیت کاری کارکنان و در نهایت افزایش بهره وری فراهم کنند.

حوادث صنعتی یکی از مشکلات عمده جوامع امروزی است. مطالعات انجام شده در زمینه حوادث نشان می دهد که مهم ترین و اصلی ترین علت در بروز آن ها عامل انسانی است. به همین منظور هدف از مطالعه حاضر شناسایی و ارزیابی خطای انسانی در عملیات راه اندازی کوره با استفاده از تکنیک تحلیل رویداد انسانی در یک صنعت پتروشیمی می باشد.
روش هامطالعه ی حاضر، از نوع کیفی و به صورت مقطعی می باشد که با مشاهده مستقیم فعالیت ها، مصاحبه آنالیز و بررسی مستندات و دستورالعمل ها، وظایف و زیر وظایف با روش تجزیه وتحلیل سلسله مراتبی Hierarchical task analysis (HTA) آنالیز و نتایج در قالب چارت های H.T.A ارایه گردید. در مرحله بعد، براساس دستورالعمل A Technique for Human Error Analysis (ATHEANA) برگه کار مربوطه تکمیل گردید.
نتایجنتایج نشان داد که شکستگی کویل یا اتصالات ابزار دقیق، فشارگیری نامناسب مسیرها برای چرخش کوره به سمت اتمسفر یا فرایند و نشتی از لاین های گاز، فلنج ها و ولوها به ترتیب با احتمال نقص 0/201، 0/14 و 0/131 دارای بیشترین احتمال خطا می باشد. مقدار احتمال خطای انسانی در عملیات راه اندازی کوره مقدار 0/635 محاسبه گشت.
نتیجه گیریبه کارگیری اقدامات اصلاحی از قبیل انتخاب افراد متخصص، برگزاری دوره های آموزشی برای افراد، پایش روتین تجهیزات اندازه گیری دما، فشار و... به منظور کارکرد صحیح دستگاه و پایش چشمی تمامی قطعات کوره از جمله ولوها از مواردی هستند که می توان با فراهم کردن آن ها احتمال خطای انسانی در عملیات راه اندازی کوره ها را کاهش داد.

در صنعت پتروشیمی عوامل زیان آور مختلفی وجود دارند که عدم توجه و مدیریت صحیح آن ها، پیامدهای نامطلوبی را برای سازمان و کارکنان به دنبال خواهد داشت. این مطالعه با هدف شناسایی و ارزیابی مخاطرات بهداشتی صنعت پتروشیمی با استفاده از روش آنالیز مخاطرات شغلی بومی سازی شده انجام شد.
روش بررسیدر این مطالعه مقطعی- توصیفی که در یکی از پتروشیمی های ایران در سال 1399 انجام شد به منظور ارزیابی ریسک بهداشتی ناشی از عوامل زیان آور از روش آنالیز مخاطرات شغلی (Job Hazard Analysis: JHA) بومی سازی شد استفاده گردید. ابتدا پس از تعیین شغل مورد نظر و تقسیم آن به وظایف شغلی، تعداد وظایف مشخص گردید. سپس به منظور محاسبه عدد اولویت ریسک (Risk Priority Number: RPN) از حاصل ضرب 3 فاکتور احتمال وقوع، شدت، تناوب مواجهه استفاده شد. کلیه محاسبات توسط Excell انجام گرفت.
یافته هانتایج نشان داد که از 353 ریسک محاسبه شده 48/93 درصد از کل ریسک ها در سطح ریسک قابل قبول، 82/4 درصد از ریسک ها در سطح اهمیت کم و 7/1 درصد در سطح ریسک های با اهمیت متوسط قرار دارند. همچنین در بین مخاطرات مورد بررسی عوامل زیان آور ارگونومی با 22 درصد بیشترین فراوانی و عوامل زیان آور بیولوژیکی با 1 درصد کمترین فراوانی مخاطرات بهداشتی را به خود اختصاص دادند.
نتیجه گیریبا استفاده از روش JHA بومی سازی شده می توان مخاطرات بهداشتی محیط کار را ارزیابی و متناسب با سطح ریسک طبقه بندی کرد. با توجه به اهمیت مخاطرات بهداشتی لازم است علاوه بر شناسایی و ارزیابی آن ها، اقدامات مدیریتی مناسبی به منظور کاهش و کنترل سطوح ریسک تعیین و اجرا نمود تا از حوادث و بیماری های شغلی مرتبط و هزینه های سنگین پیشگیری شود.

امروزه مشخص شده است که صدا علاوه بر افت شنوایی می تواند بر روی عملکردهای طبیعی فیزیولوژی بدن تاثیرگذار باشد. مطالعه حاضر با هدف تعیین تاثیر صدا بر کاهش شنوایی و فشارخون در کارگران یکی از صنایع فولاد جنوب غرب کشور ایران انجام شد.
روش بررسیاین مطالعه بر روی 54 نفر از کارگران صنعت فولاد انجام شد. برای تعیین تراز مواجهه صوتی و فشارخون به ترتیب از دزیمتری با استفاده از دستگاه صداسنج TES 1358 و دستگاه فشارسنج Beurer مدل BC16 استفاده گردید. جهت آنالیز آماری از نرم افزار آماری SPSS24 در سطح معنی داری 0/05 استفاده شد.
یافته هابین میانگین افت شنوایی و تراز فشار صوت در دو گروه هدف و شاهد ارتباط معناداری وجود داشت (0/05˂P). بر خلاف ارتباط معنی دار افت شنوایی با سن (0/05˂P)، بین متغیر سابقه کار با افت شنوایی ارتباط معنی داری به دست نیامد (0/05>P). بین سن و سابقه کار با کاهش آستانه شنوایی در برخی فرکانس مورد بررسی رابطه ی معنی داری وجود داشت (0/05˂P) همچنین بین میانگین فشارخون گروه هدف و شاهد اختلاف معنی داری مشاهده گردید (0/05˂P).
نتیجه گیرینتایج این مطالعه نشان داد مواجهه با صدا موجب افزایش فشارخون در افراد گردید و با افزایش سن اثرات مواجهه با صدا تشدید می یابد؛ راهکار های کنترلی نظیر کاهش تراز فشار صوت، برنامه های آموزشی و غربالگری منظم فشارخون کارگران جهت کاهش اثرات صدا توصیه می گردد.

Crisis management, as one of the areas of consequence management, can increase crisis resilience and reduce the level of organizational vulnerability in times of crises. Therefore, this study was designed and conducted to assess the components of crisis management in private and public hospitals.
MethodThe present study was conducted in 10 hospitals, in 2020. The sample size was estimated to be 200. The components of crisis management in this questionnaire included: management commitment, taking the blame, learning, awareness, preparedness, flexibility and transparency. Data analysis was performed using Independent samples t-test and Chi-square/Fisher exact test.
ResultsThe results showed that the two groups of public and private hospitals did not have significant differences in terms of individual variables (p<0.05). The crisis management index in the two groups of public and private hospitals was estimated to be 4.11 and 3.22, respectively. The results of comparative assessment of the crisis management components in the two groups revealed that all these components in public hospitals are estimated to be higher than that of the private hospitals, and their differences are also statistically significant (p<0.05).
ConclusionThe findings of the study indicated that although the resilience of private hospitals was higher than average, the comparative assessment of the crisis management components revealed that the level of resilience in times of crises in public hospitals was higher than the private ones. Therefore, the components of crisis management in these hospitals should be improved with a proper design and a comprehensive crisis management program.

Human factors are central and vital issues in different working environments, including industries. The operational processes of a gas company are among the processes in which several accidents have occurred. Accidents are of the most important causes of direct and indirect costs to the industry, so it is necessary to identify the human factors which have the greatest effect on the occurrence of errors regarding accidents, recognize them, and adopt control and preventive strategies to prevent them from happening again. Accordingly, the study of human factors affecting error occurrence is of great importance in industry. Therefore, this study aimed to analyze the human factors affecting the occurrence of errors in the operational processes of the gas companies.
MethodThis was a descriptive cross-sectional study conducted in a gas company. Statistics and documents of the accidents from a 10-year period in the gas company were received from the HSE (Health and Safety Executive) of this center, and examined and analyzed through the methods of human factor analysis and system classification for the oil and gas industry (HFACS-OGI) and root cause analysis (RCA).
ResultsThe results of applying HFACS-OGI method showed that in level one, error section, subgroup of "perceptual error" ; in level two, subgroup of "human resource management"; in level three, "inadequate supervision"; in level 4, "safety culture" section ;and at level 5, the "legal framework" had the highest percentage. Through the RCA method, the highest frequency was for defects related to work requirements and the lowest frequency was related to disqualification.
ConclusionHuman factors affecting the occurrence of error include cognitive and behavioral characteristics of employees, defects in the operational processes of safety management system, inadequate and proper supervision, lack of/deficiency in training and the employees' practical experience, non-compliance with general safety principles, lack of/deficiency in implementation of control measures regarding the management of the risks approved by the organization , the lack of/defect in the optimal implementation of human resource management, which can be reduced by improving the performance indicators, executive procedures, proper planning, providing monitoring courses ,and proper supervision .Finally, it was found that HFACS-OGI and RCA methods can be used in similar working environments due to their consistency in presenting the results.

With the advancement of science and technology in third world and developing countries, more and more harmful factors in the working environment endanger the employees’ lives. One of these detrimental factors is the incompatibility of work with people’s characteristics, which causes musculoskeletal disorders. In developing countries this happens at the cost of musculoskeletal disorders. Therefore, this study aimed to investigate the effect of ergonomic interventions on reducing the prevalence of musculoskeletal disorders, and finally, use the results obtained to prevent musculoskeletal disorders and improve the working environment among the staff of Qeshm Zinc Smelting company, in 1399.
MethodsIn this study, staff were randomly evaluated as a case control by the Nordic questionnaire, using QEC method and body map. Ergonomic interventions were then performed on the case group. After one month and four months, re-evaluation was performed by QEC and body map methods.
ResultsFindings indicated that 84.21% of the participants had musculoskeletal disorders in the past year before the study. Ergonomic interventions for musculoskeletal disorders in the lower back, shoulders, neck, knees and hands were significantly reduced.ftAer ergonomic interventions, the risk factor of these disorders was significantly reduced.
ConclusionTherefore, by doing ergonomic interventions, including teaching ergonomic principles, using automatic equipment, etc., musculoskeletal disorders in the back, shoulders, neck and knees were created and musculoskeletal disorders were reduced to 13.58%.

Analyzing the causes of accidents in order to control and prevent their recurrence is of utmost importance. The aim of this study was to combine the two methods of Fishbone and SCAT to analyze the root cause of the accidents, address them in details, and track them in the depth of organizational structures.
MethodsThis study analyzed the data about accidents using a Fishbone and SCAT Combined Method. Initially, the interface and root causes obtained by the Fishbone Method, as well as control measures were entered into SCAT cause and effect table. The results of the questionnaires were collected, and the causes of accidents were investigated using the Chi-Square test. Data analysis was done using SPSS software version 24.
ResultsThe findings indicated that the most important direct cause of the accidents was the the variable related to indoor and outdoor environment of the refinery, and the most important root cause of the accident was “unforeseen economic changes such as changing exchange rate, tax rate”, and “lack of a continuous planning for the staff's training”. Increasing the efficiency of training the staff is the most important control measure suggested. There was also a significant relationship between surface and root causes.
ConclusionsAccording to the statistical test, a systematic connection between root causes and intermediate causes of the accident were found. The combined method presented in this study can meet the needs of different organizations regarding the causes of accidents in many cases, and is currently one of the most important methods. Therefore, it is suggested to combine the SCAT with other methods to analyze the causes of accidents in refinery plants.

One of the most important goals of organizations is to increase the quality of working life of employees and the most important part of employees of organizations are working women who, in addition to personal life responsibility, must have a relatively high quality of work life to be able to meet the goals satisfy the formation of organizations. The present study aims to investigate the situation of identity styles and multiple intelligence and their relationship with quality of working life of female employees at Yazd university in 2019.
MethodThis study is descriptive- survey in terms of data collection and pure in terms of purpose. The statistical population of this study consisted of 159 female employees of Yazd university which of 120 employees were selected based on the Krejcie and Morgan table by using simple random sampling method. The scales used to collect data were: Berzonskychr('39')s identity styles scale, Armstrongchr('39')s multiple intelligence scale, and Waltonchr('39')s work life quality questionnaire. The data were analyzed using pearson correlation coefficient, chi- square test, one- sample t- test, analysis of variance with repeated measures and bonferroni test.
ResultsThe results showed that intrapersonal intelligence and normative identity style were more prevalent among female employees of Yazd university than other types of intelligence and identity style. Quality of working life was average and commitment among female employees was above average, as well as, no relationship was found between identity styles with quality of work life. Also among multiple intelligences, only rational- mathematical intelligence had a positive and significant relationship with quality of working life.
ConclusionIt can be concluded that managers of organizations should strive to employ people with informational and normative identity styles, as well as, provide opportunities to utilize multiple intelligence of employees to increase the quality of working life in order to strengthen the productivity.

Industrial accidents are among the major problems of today's societies. Studies on accidents indicated that the most important cause of their occurrence was the human factor. Therefore, the purpose of this study was to identify and evaluate human error in the operation of furnace using the human event analysis technique in a petrochemical industry.
MethodsThis was a qualitative and cross-sectional study which was analyzed by directly observing the activities, interviewing the analysis, and reviewing the documents and instructions, tasks and sub-tasks by Hierarchical task analysis (HTA) method, and the results were provided in HTA charts. In the next step, according to Technique for Human Error Analysis (ATHEANA) instructions, the relevant worksheet was completed.
ResultsThe results indicated that coil breakage or precision instrument connections, improper bypassing of pressure in the paths to rotate the furnace to the atmosphere, or process and leakage of gas lines, flanges and valves whose probability of defect was 0.20, 0.14 and 0.13, respectively, had the highest probability of error. The probability of human error in the operation of the furnace was calculated to be 0.63.
ConclusionApplying corrective measures such as: selecting skilled people, holding training courses, routine monitoring of temperature, pressure measuring equipment, etc. for proper operation of the apparatus and visual monitoring of all furnace parts, including valves, are some of the things with which the likelihood of human error in the operation of the furnaces was reduced.

In the petrochemical industry, there are various harmful factors that lack of attention and proper management will have adverse consequences for the organization and employees. The aim of this study was to Identifying and assessment the health hazards of the petrochemical industry using the localized JHA method.
Materials and MethodsIn this cross-sectional descriptive study that conducted in one of Iran's petrochemicals, in order to assess the health risk caused by harmful factors was used from localized JHA method. First, after determining the desired job and dividing it into job duties, the number of tasks was specified. Then, in order to calculate the Risk Priority Number (RPN), from multiplication of 3 factors of probability of occurrence, severity and frequency of exposure was used. All calculations were performed by Excell.
ResultsThe results showed that from 353 calculated risks, 93.48% of the total risks are at the acceptable risk level, 4.82% of the risks are at the low importance level and 1.7% is at risk level with medium importance. Also, at between the studied hazards, ergonomic harmful factors with 22%, had the highest frequency and biological harmful factors with 1% had the lowest frequency of hazards.
ConclusionRegarding to importance of health hazards, in addition to identifying and evaluating them, it is necessary to determine and implement appropriate management measures to reduce and control of risk levels to prevent from accidents and related occupational diseases and the heavy costs.

Today, it is well-established that, in addition to hearing loss, noise affects the natural physiological functions of the body. The aim of this study was to determine the effect of noise on hearing loss and blood pressure in workers of a steel industry in southwest of Iran.
MethodsThis study was performed on 54 steel workers. Dosimetry was performed using TES 1358 sound meter and Beurer BC16 blood pressure monitor, respectively. Statistical analysis was done using SPSS 24 software at the significant level of 0.05.
ResultsThere was a significant relationship between hearing loss and sound pressure level in each of the case and control groups (P< 0.05). Despite the significant relationship between hearing loss and age, (P˂0.05), there was no significant relationship between the work experience variable and hearing loss (P> 0.05). Age and work experience had a significant relationship with hearing loss at some frequencies under the study (P˂0.05). There was also a significant difference between the mean of blood pressure in the case and control group (P˂0.05).
ConclusionThe results of this study indicated that exposure to noise increased blood pressure in individuals and the effects of exposure intensified with age. Therefore, control measures such as reducing sound pressure level, training programs and regular screening of the workers' blood pressure are recommended to reduce the effects of noise.
