جای خالی مطالعه ای که به صورت متاآنالیز در رابطه با مقایسه درد پس از جراحی به دنبال بیهوشی عمومی با سووفلوران و پروپوفول انجام شده باشد و دارای کیفیت بالا باشد و بتواند نتایج مطالعات کارآزمایی با کیفیت بالا که در طی چند سال اخیر انجام شده اند را در یک مطالعه ارایه دهد، احساس می شود. لذا مطالعه حاضر با هدف مقایسه اثرات سووفلوران با پروپوفول در بیهوشی عمومی کودکان بر درد پس از جراحی به صورت مرور سیستماتیک و متاآنالیز انجام شد.
روش کاریک جستجوی سیستماتیک (کلیدواژه ها: پروپوفول یا دیپریوان یا پروپوفولوم (برای داروی پروپوفول) و سووفلوران یا سوو (برای داروی سووفلوران) ، کودک، درد و بیهوشی)در پایگاه های داده ای پاب مد، Embase، کتابخانه کاکرین، CNKI، اسکاپوس، وب آو ساینز و مگ ایران و SID بدون محدودیت زمانی انجام شد. هدف نهایی مقایسه پیامد درد پس از جراحی بود. تجزیه و تحلیل های آماری با نرم افزار RevMan 5.3 و Cochrane Collaboration’s انجام شد.
نتایجافزایش قابل توجهی در بروز درد پس از جراحی برای گروه سووفلوران در مقایسه با گروه پروپوفول وجود دارد.(72/1=OR – (64/2-11/1) 95%CI- 45/2=Z Test – 01/0=P)
نتیجه گیریدر بیهوشی عمومی کودکان استفاده از پروپوفول می تواند با درد پس از جراحی کمتری همراه باشد.

چند کارآزمایی بالینی اخیر اثر تجویز پیشگیرانه دگزامتازون پس از اکستوباسیون بر گلودرد را پس از جراحی بررسی کردهاند که تابحال نتایج آنان به صورت یک مطالعه منسجم ارایه نشده است. بنابراین، ما یک مرور سیستماتیک و متاآنالیز با بررسی اثرات تجویز پیشگیرانه دگزامتازون قبل از جراحی بر شیوع گلودرد در طی 24 ساعت اول پس از بیهوشی را انجام دادیم.
روش کاراین مطالعه از نوع مروری سیستماتیک و متاآنالیز بود؛ بر این اساس پایگاه های اسکاپوس، وب آو ساینز، پابمد، EMBASE ، ثبت مرکزی کارآزماییهای کنترلشده کاکرین، مگ ایران و SID را برای جستجوی مطالعات واجد شرایط بر اساس کلیدواژه های پیشگیری، جراحی، پس از جراحی، پس از بیهوشی، گلودرد و دگزامتازون جستجو شدند.
نتایجبررسی مقالات به کمک نمودار جنگلی برای بروز گلودرد پس از جراحی در 24 ساعت پس از جراحی جراحی یا لوله گذاری، کاهش بروز گلودرد را در بیماران تحت درمان با دگزامتازون وریدی در مقایسه با گروه کنترل (نرمال سالین) نشان داد (62/0=RR- 75/0-51/0:95%CI- 001/0=P- 7=df).(شکل 1)
نتیجه گیریمطالعه ما نشان میدهد که تجویز دگزامتازون داخل وریدی 15 دقیقه قبل از اکستوباسیون، گلودرد پس از جراحی را به طور موثرتری کاهش میدهد.

مطالعه حاضر با هدف شناسایی تاثیر سن و جنس بر مرگ بیماران کووید-19 انجام شد.
روش بررسیدر این مطالعه توصیفی 780 بیمار مبتلا به کووید 19 مراجعه کننده به بیمارستان بررسی و اطلاعات دموگرافیک شامل سن ، جنس و مرگ و میر احتمالی در هر دو جنس در دو گروه مجزا در پنج رده سنی (>15، 15 تا 34 ، 35 تا 54 ، 55 تا 74 و < 75 سال) از سیستم بیمارستانی ثبت گردید.
نتایجاز میان گروهای سنی بیشترین میزان مرگ و میر مربوط به گروه های سنی بالای 75 سال بود (38.7%) و بعد از آن گروه سنی 55-74 سال (23.1%) بیشترین میزان فوتی را شامل بود. هر چند میزان مرگ و میر در مردان بیشتر از زنان مشاهده شد ولی این اختلاف از نظر آماری معنی دار نبود (22.4% Vs . 19.0%; P-value=0.255.
نتیجه گیریعفونت با COVID-19 با مرگ و میر قابل توجهی به طور عمده در بیماران مسن و جنس مرد همراه است .این یافته ها می تواند به شناسایی بیماران مبتلا به کووید-19 که در معرض خطر بیشتر پیش آگهی ضعیف هستند کمک کند و پایش این عوامل می تواند به هشدارهای اولیه برای مداخلات مناسب کمک کند.

عمل جراحی سپتورینوپلاستی یکی از شناخته شده ترین جراحی های زیبایی در بین جراحی های گوش، حلق و بینی است. درد بعد از عمل هنوز هم یکی از مهمترین عوامل موثر بر رضایت بیمار است. این علایم می تواند برای بیمار ناراحت کننده باشد. هدف از انجام این مطالعه مقایسه تجویز حین عمل کتورولاک و مورفین در کنترل درد پس از عمل در بیماران تحت عمل جراحی سپتورینوپلاستی می باشد.
روش کارمطالعه حاضر از نوع کارآزمایی بالینی تصادفی شده دو سویه کور می باشد که طی یک دوره شش ماهه از اسفند 1400 تا مرداد 1401 در 180 نفر از بیماران مراجعه کننده به اتاق عمل بیمارستان مطهری شهرستان جهرم که تحت عمل جراحی سپتو رینوپلاستی قرار گرفتند، انجام شد. بیماران به صورت تصادفی، به سه گروه مورفین 6 میلی گرم و کتورولاک با دوزهای 30 و 60 میلی گرمی تقسیم بندی شدند. ابزار جمع آوری اطلاعات شامل؛ سن، جنسیت و میزان درد بعد از عمل بود. شدت درد با مقیاس سنجش درد (VAS) اندازه گیری شد. تجزیه و تحلیل اطلاعات با استفاده از spss نسخه 21 و آمارهای توصیفی (میانگین و انحراف معیار) و آزمون آماری استنباطی (فیشر) صورت گرفت.
نتایجاز نظر جنسیتی، 67.22 درصد شرکت کنندگان مرد و 32.77 زن بودند. توزیع تعداد مردان و زنان در گروه های مطالعه با هم تفاوت معنی داری نداشت (P=0.80). اختلاف معنی داری بین گروه های مطالعه از نظر سن وجود نداشت (P=0.14). از نظر درد حس شده، آزمون آماری فیشر نشان داد که افراد بیشتری در گروه دریافت کننده کتورولاک 30 میلی گرم نسبت به دو گروه دیگر درد را حس کرده بودند که از نظر آماری این اختلاف معنی دار بود (P=0.003).
نتیجه گیریبه دلیل اهمیت کاهش درد بیماران بعد از انجام عمل جراحی و به حداقل رساندن عوارض عمل جراحی، می توان به دلیل عوارض کمتر کتورولاک نسبت به مورفین، از داروی کتورولاک با دوز 60 میلی گرم حین عمل جراحی سپتو رینوپلاستی، جهت کنترل درد بعد از عمل استفاده نمود.

اندوسکوپی دستگاه گوارش یک روش تشخیصی و درمانی مناسب برای بیماری های دستگاه گوارش بوده و انتخاب یک روش آرام بخشی مناسب برای انجام این پروسیجر حایز اهمیت می باشد. آرام بخشی با پروپوفول-کتامین یکی از روش های رایج مورد استفاده است. این مطالعه جهت بررسی لیدوکایین وریدی در اندوسکوپی تحت آرام بخشی طراحی گردید.
مواد و روش هامطالعه ی طراحی شده از نوع کارآزمایی بالینی دو سوی کور می باشد که پس از تصویب طرح پیشنهادی و اخذ تاییدیه ی کمیته اخلاق پزشکی دانشگاه علوم پزشکی جندی شاپور اهواز ، بر روی 138 بیمار کاندید اندوسکوپی سرپایی انجام شد . بیماران به دو گروه تقسیم شدند، در گروه اول 0٫5 میلی گرم بر کیلوگرم پروپوفول و 0٫5 میلی گرم بر کیلوگرم کتامین و در گروه دوم 0٫5 میلی گرم بر کیلوگرم پروپوفول، 0٫5 میلی گرم بر کیلوگرم کتامین و 1٫5 میلی گرم بر کیلوگرم لیدوکایین تزریق شد. میزان آرام بخشی، تغییرات همودینامیک و تهوع و استفراغ در بیماران ثبت و مقایسه گردید.
نتایجدو گروه از نظر سن و جنسیت و وزن همگن بودند. بین مدت زمان اندوسکوپی در دو گروه تفاوت معناداری وجود نداشت. میان مدت زمان ریکاوری بین دو گروه تفاوت معنی داری مشاهده نشد. میزان پروپوفول اضافی در گروه دریافت کننده ی لیدوکایین به طور معنی داری کمتر بود. بین تعداد ضربان قلب و فشار خون متوسط شریانی در دو گروه ارتباط معنی داری مشاهده نشد. میزان آرام بخشی بین دو گروه تفاوتی نداشت و میزان تهوع و استفراغ در گروه دریافت کننده ی لیدوکایین به طور معنی داری کمتر بود.
نتیجه گیریافزودن لیدوکایین وریدی به ترکیب پروپوفول -کتامین باعث بهبود پیامدهای بالینی و کاهش عوارض جانبی می شود.

با توجه به شیوع بالای سزارین واهمیت کنترل درد پس از ان این مطالعه به بررسی تاثیر شیاف دیکلوفناک سدیم، پاراستامول وریدی و ترکیب ان ها بر درد حاد بعد از عمل جراحی سزارین پرداخته است.
روش کار99 بیمار کاندید عمل جراحی سزارین به صورت تصادفی به سه گروه تقسیم شدند،جهت بی دردی پس از عمل در گروه A از شیاف دیکلوفناک سدیم + پمپ PCA حاوی دارونما،در گروه B از پمپ PCA حاوی پاراستامول وریدی + شیاف دارونما و در گروه C از پمپ PCA حاوی پاراستامول وریدی + شیاف دیکلوفناک سدیم استفاده شد.شدت درد پس از عمل با استفاده از مقیاس VAS (visual analogue scale) در ساعات (1و4و12و24) ارزیابی شد و مقدار اپیویید مصرفی جهت بی دردی پس از عمل ثبت گردید.
نتایجاز نظر شدت درد، تفاوت معناداری در ساعات 1، 4، 12 و 24 پس از عمل بین گروه های A ، B و C مشاهده شد (05/0 <p). درد در گروه C در تمام ساعات به طور قابل توجهی کم تر از دو گروه دیگر بود (p<0.0001). در مقایسه ی گروه A و B در ساعات 1 و 4 پس از عمل درد در گروه A کم تر بود و در ساعات 12 و 24 شدت درد در گروه B کمتر از گروه A بود (05/0 <p). کمترین میزان مورفین مصرفی در دوره ی پس از عمل به ترتیب در گروه C ، B و A مشاهده شد (05/0 <p).
نتیجه گیریترکیب شیاف دیکلوفناک و پمپ پاراستامول درد پس از عمل و نیاز به مخدررا به طور موثری کاهش می دهد، به علاوه شیاف دیکلوفناک در ساعات (1و4)بی دردی بیشتری نسبت به پمپ پاراستامول داشته است اما میزان کلی مصرف مخدر وریدی در گروه پمپ پاراستامول کمتر بوده است .

There is a need for a meta-analysis of a study comparing post-surgery pain following general anesthesia with sevoflurane and propofol, which is of high quality and can compare the results of high-quality trial studies that have been conducted in the last few years. Therefore, the present study aimed to compare the effects of sevoflurane and propofol in children's general anesthesia on pain after surgery in the form of a systematic review and meta-analysis.
Materials and methodsA systematic search (keywords: propofol or diprivan or propofol (for propofol) and sevoflurane or sevo (for sevoflurane), child, pain and anesthesia) in databases PubMed, Embase, Cochrane Library, CNKI, Scopus, Web of Science, MagIran and SID without time limit. The final aim was to compare the outcome of pain after surgery. Statistical analyzes were performed with RevMan 5.3 and Cochrane Collaboration's software.
ResultsThere is a significant increase in the incidence of pain after surgery for the sevoflurane group compared to the propofol group (OR=1.72 - 95%CI (1.2-11.64) - Z Test =2.45 - P=0.01)
ConclusionIn pediatric general anesthesia, the use of propofol can be associated with less pain after surgery.

Several recent clinical trials have investigated the effect of prophylactic administration of dexamethasone after extubation on sore throat after surgery, and their results have not yet been presented in a coherent study. Therefore, we performed a systematic review and meta-analysis examining the effects of preoperative dexamethasone prophylactic administration on the prevalence of sore throat during the first 24 hours after anesthesia.
Materials abd MethodsThis study was a systematic review and meta-analysis; Based on this, Scopus, Web of Science, PubMed, EMBASE, Cochrane Central Register of Controlled Trials, Mag Iran and SID were used to search for eligible studies based on the keywords prevention, surgery, post-surgery, post-anesthesia, sore throat and dexamethasone.
ResultsA review of articles using a forest diagram for the incidence of sore throat after surgery in 24 hours after surgery or intubation showed a decrease in the incidence of sore throat in patients treated with intravenous dexamethasone compared to the control group (normal saline) (62/ 0=RR- 0.51-0.75: 95% CI- 0.001. P= df=7). (Figure 1)
ConclusionOur study shows that administration of intravenous dexamethasone 15 minutes before extubation reduces postoperative sore throat more effectively.

The present study was conducted with the aim of identifying the effect of age and gender on the death of COVID-19 patients.
Materials and methodsIn this descriptive study, the files of 780 patients with COVID-19 referred to the hospital were examined, and demographic information including age, sex and possible mortality in both sexes in two separate groups in five age groups (>15, 15 up to 34, 35 to 54, 55 to 74 and < 75 years) were recorded from the hospital system.
ResultsAmong the age groups, the highest mortality rate was related to the age groups over 75 years old (38.7%) and then the age group 55-74 years old (23.1%). Although the mortality rate was higher in men than in women. But this difference was not statistically significant (22.4% vs. 19.0%; P-value=0.255.
ConclusionInfection with COVID-19 is associated with significant mortality, mainly in elderly and male patients. These findings can help to identify patients with COVID-19 who are at higher risk, and on the other hand, monitor these factors. It can help with early warnings for appropriate interventions.

Septorhinoplasty surgery is one of the most well-known cosmetic surgeries among ear, nose and throat surgeries. Postoperative pain is still one of the most important factors affecting patient satisfaction. These symptoms can be uncomfortable for the patient. The purpose of this study is to compare the intraoperative administration of ketorolac and morphine in controlling postoperative pain in septorhinoplasty patients.
Materials and MethodsThe present study is a double-blind randomized clinical trial that was conducted during a six-month period from March 2022 to August 2022 in 180 patients referred to the operating room of Motahari Hospital in Jahrom city who underwent septorhinoplasty surgery. Patients were randomly divided into three groups of morphine 6 mg and ketorolac with doses of 30 and 60 mg. Information collection tools include; Age, gender and pain level after surgery. Pain intensity was measured by pain measurement scale (VAS). Data analysis was done using spss version 21 and descriptive statistics (mean and standard deviation) and inferential statistical test (Fisher).
ResultsIn terms of gender, 67.22% of the participants were male and 32.77% were female. The distribution of the number of men and women in the study groups was not significantly different (P=0.80). There was no significant difference between the study groups in terms of age (P=0.14). In terms of felt pain, Fisher's statistical test showed that more people in the ketorolac 30 mg group had felt pain than the other two groups, which was statistically significant (P=0.003).
ConclusionDue to the importance of reducing the pain of patients after surgery and minimizing the side effects of surgery, it is possible to use Ketorolac at a dose of 60 mg during septorhinoplasty surgery to control pain due to the lower side effects of Ketorolac compared to morphine Used after surgery.

Gastrointestinal endoscopy is a suitable diagnostic and therapeutic method for gastrointestinal diseases and choosing a suitable sedation method for this procedure is important. Sedation with propofol-ketamine is one of the most commonly used methods. This study was designed to evaluate intravenous lidocaine in endoscopy under sedation.
Material and methodsThe designed study is a double-blind clinical trial that was conducted on 138 patients candidates for outpatient endoscopy after approving the proposed plan and obtaining approval from the Medical Ethics Committee of Ahvaz Jundishapur University of Medical Sciences. Patients were divided into two groups. In the first group, 0.5 mg/kg of propofol and 0.5 mg/kg of ketamine were, and in the second group, the same drugs and 1.5 mg/kg of lidocaine were injected. The level of sedation, nausea, vomiting and hemodynamic changes in the patients were recorded and compared.
ResultsThe two groups were matched for age, gender, and weight. There was no significant difference in the duration of the endoscopy. . There was no significant relationship between the heart rate and mean arterial pressure in the two groups(p>0.05). There was no significant difference in recovery time between the two groups. The amount of additional propofol was significantly lower in the group receiving lidocaine The level of sedation did not differ between the two groups, and nausea and vomiting were significantly lower in the group receiving lidocaine.
ConclusionAdding intravenous lidocaine to a propofol-ketamine combination improves clinical outcomes and reduces side effects.

Considering the high prevalence of cesarean section and the importance of pain control afterwards, this study investigated the effect of diclofenac sodium suppository, intravenous paracetamol and their combination on acute pain after cesarean surgery.
Materials and methods99 patients candidates for cesarean surgery were randomly divided into three groups. For postoperative analgesia, diclofenac sodium suppository + PCA pump containing placebo was used in group A, PCA pump containing intravenous paracetamol + placebo suppository was used in group B, and PCA pump containing intravenous paracetamol + diclofenac sodium suppository was used in group C. Postoperative pain intensity was assessed using the VAS scale at 1, 4, 12, and 24 hours, and the amount of opioid used for postoperative analgesia was recorded.
ResultsIn terms of pain intensity, a significant difference was observed between groups A, B and C at 1, 4, 12 and 24 hours after the operation (p<0.05). Pain in group C was significantly less than the other two groups at all hours (p<0.0001). Comparing groups A and B, the pain in group A was less in the 1st and 4th hours after the operation, and in the 12th and 24th hours, the pain intensity in group B was less than that in group A (p<0.05). The lowest amount of morphine consumed in the postoperative period was observed in groups C, B and A respectively (p<0.05).
Conclusionthe combination of diclofenac suppository and paracetamol pump effectively reduces postoperative pain and the need for narcotics, in addition, diclofenac suppository has more pain relief than the paracetamol pump in hours 1 and 4, but the overall amount of intravenous narcotic consumption was lower in the paracetamol pump group. Is.
