تریاژ، فرایند ارزیابی و طبقه بندی بیماران به منظور بهبود مدیریت و ارائه ی خدمات است. یک سیستم تریاژ خوب باید قادر به شناسایی دقیق بیمارانی باشد که نیاز به مراقبت فوری پزشکی دارند. شاخص شدت اضطرار (Emergency severity index) ESI، در حال حاضر پرکاربردترین روش برای تریاژ و یک سیستم تریاژ معتبر است. هدف از این مطالعه، بررسی توانایی پرستاران بخش اورژانس در تریاژ صحیح بیماران با استفاده از ESI بود.
روش هامطالعه ی مقطعی حاضر، بر روی 272 بیمار مراجعه کننده به اورژانس بیمارستان شهید باهنر کرمان در تیرماه 1401 می باشد. تریاژ بیماران که توسط پرستاران اورژانس انجام شده بود با تریاژ تعیین شده توسط پزشکان مقایسه شد. ضریب کاپا بین دو گروه (پرستاران و پزشکان) برای تعیین توافق، مورد ارزیابی قرار گرفت.
یافته هاتریاژ برای 72/1 درصد از 272 بیمار به درستی شناسایی شد، در حالی که 13/6 درصد تریاژ سبک و 14/3 درصد تریاژ سنگین شده بودند. اولویت تنها برای 56/6 درصد بیماران دقیق بود و به 7/1 درصد از بیماران، اولویت کمتری اختصاص داده شده بود. سطح توافق بین دو گروه، که با ضریب کاپا اندازه گیری شد برابر با 5/60 درصد بود.
نتیجه گیریدر این مطالعه توافق تریاژ به دست آمده در حد متوسط بود. با این حال فرایند تریاژ بیشتر از اینکه خطرناک باشد، ناکارآمد بود. برنامه های آموزشی طولانی مدت برای افزایش دقت تریاژ توصیه می شود.

این مطالعه با هدف تعیین تاثیر مصرف دو مکمل سلنیوم و روی بر میزان بروز فیبریلاسیون دهلیزی بعد از عمل جراحی پیوند عروق کرونر انجام شد.
روش هادر این مطالعه ی کارآزمایی بالینی، 54 بیمار به روش تصادفی در دو گروه 27 نفره تقسیم شدند. گروه مداخله، روزانه یک کپسول سل پلاس که حاوی 200 میکرو گرم سلنیوم و 15 میلی گرم روی از یک هفته قبل عمل دریافت کردند. گروه شاهد مداخله ای دریافت نکردند و تحت مراقبت های روتین (دارو درمانی و مانیتورینگ قلبی) بودند. 24 ساعت قبل از عمل، سطح سرمی سلنیوم و روی بیماران بررسی شد. کلیه ی بیماران بعد از عمل جراحی از نظر معیارهای بالینی شامل: فیبریلاسیون دهلیزی، کسر جهشی قلب، خونریزی، مدت بستری در بخش مراقبت های ویژه و بیمارستان بررسی و مقایسه شدند.
یافته هااختلاف میانگین کسر جهشی قلب در گروه مداخله 0/77 ± 2/41 درصد (افزایش یافته) و در گروه شاهد 0/86 ± 2/78 درصد (کاهش یافته) بوده و اختلاف بین دو گروه معنی دار بود (001/0 > P). در طی مدت مطالعه، 4 نفر (14/8 درصد) از گروه شاهد دچار فیبریلاسیون دهلیزی شدند و موردی از AF در گروه مداخله دیده نشده و اختلاف دو گروه معنی دار بود (0/015 = P). میانگین حجم خونریزی (0/011 = P) و میانگین مدت بستری در بیمارستان بین دو گروه تفاوت معنی دار داشت.
نتیجه گیریاحتمالا تجویز مکمل سلنیوم و روی قبل از عمل بای پس عروق کرونر می تواند به پیش آگهی مطلوب تر و کاهش بروز فیبریلاسیون دهلیزی و کاهش مدت در بستری بیمارستان وکاهش میزان خونریزی بعد از عمل شود.

مطالعات محدودی در زمینه ی تاثیر مسکن ها بر درد ناشی از کوله سیستکتومی لاپاروسکوپیک و کنترل درد پس از عمل بیماران با داروهایی که حداقل عوارض ممکن را داشته باشند انجام شده و همین موضوع باعث طراحی مطالعه ی کنونی به منظور مقایسه ی اثر پره امپتیو آپوتل و کتورولاک برای اولین بار در بیماران تحت عمل کوله سیستکتومی لاپاروسکوپیک بوده است.
روش هااین کارآزمایی بالینی سه سوکور، بر روی 90 بیمار که تحت بیهوشی عمومی جهت کوله سیستکتومی لاپاروسکوپیک قرار گرفتند، انجام شد. گروه A کتورولاک وریدی، گروهB ، آپوتل و گروه C به عنوان شاهد، نرمال سالین را پیش از بیهوشی عمومی و 30 دقیقه قبل از برش جراحی دریافت کردند.
یافته هادر مجموع و پس از گذشت 24 ساعت، نیاز به مخدر پس از عمل جراحی کوله سیستکتومی در بیماران تحت بیهوشی همراه با کتورولاک (2/10 ± 0/93) کمتر از آپوتل (2/72 ± 0/97) و گروه شاهد (2/85 ± 1/02) بود. این یافته ها برای شدت درد و رضایت بیماران نیز مورد تایید قرار گرفت. رضایت بیماران در گروه کتورولاک و سپس آپوتل در مقایسه با گروه شاهد از سطح بالاتری برخوردار بود.
نتیجه گیریبه نظر می رسد نیاز به مخدر در گروه کتورولاک کمتر از آپوتل و در آپوتل کمتر از شاهد بود. همچنین با لحاظ کردن فاکتور زمان و نیمه ی عمر موثر هر یک از داروها، پیشنهاد می شود که هر دو داروی آپوتل و کتورولاک می توانند در کاهش نیاز به مخدر در بیماران تحت کله سیستکتومی موثر واقع شوند.

Triage is the process of assessing and categorizing patients to improve management and service delivery. A reliable triage system should be capable of accurately identifying patients who require immediate medical attention. The emergency severity index (ESI) is currently the most widely used method for triage, and it is a validated triage system. This study aimed to assess the ability of emergency department nurses to triage patients utilizing ESI.
MethodsA cross-sectional study was conducted on 272 patients who were referred to the emergency department (ED) of Shahid Bahonar Hospital of Kerman in July 2022. The triage of the patients was performed by emergency nurses, and the results were compared with those determined by physicians. Cohen's kappa coefficient was evaluated between the two groups (nurses and physicians) to determine the inter-rater agreement.
FindingsThe triage group was correctly identified for 72.1% (196) of the 272 patients, while 13.6% (37) were under-triaged and 14.3% (39) were over-triaged. The priority was accurate for only 56.6% of patients, and 7.1% of patients received a lower priority. The agreement level between the two groups was 60.5% (95% CI: 0.532-0.677) measured by the kappa coefficient.
ConclusionIn this study, the agreement that was found in terms of patients’ triage was average. Although the triage was more inefficient than risky, long-term training programs are highly recommended for enhancing the accuracy of the triage.

This study was conducted to determine the effect of zinc and selenium supplements on the incidence of atrial fibrillation after coronary artery bypass surgery.
MethodsIn this clinical trial, 54 patients were randomly divided into two groups of 27. The intervention group received a daily capsule containing 200 micrograms of zinc and 15 mg of selenium a week before the operation, and the control group received a placebo. 24 hours before the operation, the zinc and selenium levels were checked. All patients after surgery were examined and compared in terms of clinical parameters including atrial fibrillation, cardiac ejection fraction, bleeding, and length of hospitalization in the intensive care unit and hospital.
FindingsThe mean difference of cardiac ejection fraction in the intervention group was -2.41 ± 0.77% (increased) and in the control group was 2.78 ± 0.86% (decreased), and the difference between the two groups was significant (P < 0.001). During the study, 4 (14.8%) of the control group had ventricular fibrillation, and there was no case of AF in the intervention group, and the difference between the two groups was significant (P = 0.015). The mean volume of bleeding (P = 0.011) and the mean length of hospital stay were significantly different between the two groups.
ConclusionPrescribing zinc and selenium supplements before coronary artery bypass surgery can lead to a more favorable prognosis and reduce the incidence of ventricular fibrillation, reduce the length of hospital stay, and reduce the amount of bleeding after surgery.

Laparoscopic cholecystectomy has been accepted as a safe and gold-standard procedure in the world and has many advantages. However, one of the common complications after laparoscopic cholecystectomy is chronic and sometimes very severe pain. Considering the patients' need to control postoperative pain with drugs that have the least possible side effects, as well as limited studies on the effect of painkillers on pain caused by laparoscopic cholecystectomy, the purpose of this study is to compare the preemptive effect of two drugs. Apotel and ketorolac were used for the first time in patients undergoing laparoscopic cholecystectomy.
MethodsThis study was conducted as a three-blind randomized clinical trial on 90 patients aged 18 to 68 years, with ASA class one and two, who underwent general anesthesia for cholecystectomy.
FindingsThe findings of this study show generally, the need for narcotics after cholecystectomy surgery in patients under anesthesia with ketorolac was less than in Apotel and the control group. These findings were also confirmed in patients' pain intensity and patients' satisfaction, so patients' satisfaction in Ketorolac and then Apotel group was higher compared to the control group, which is in line with previous studies in this field.
ConclusionTherefore, it seems that the overall need for narcotics was lower in the ketorolac group than in Apotel and lower in Apotel than in control.
